Abstract:
Recent research in the areas of animal conditioning, the neural systems underlying emotion and memory,
and the effect of fear on these systems is reviewed. This evidence points to an important distinction between
hippocampally-dependent and non-hippocampally-dependent forms of memory that are differentially affected
by extreme stress. The cognitive science perspective is related to a recent model of posttraumatic stress
disorder, dual representation theory, that also posits separate memory systems underlying vivid reexperiencing
versus ordinary autobiographical memories of trauma. This view is compared with other accounts in
the literature of traumatic memory processes in PTSD, and the contrasting implications for therapy are

Abstract:
Animal models conceptualize anxiety as a response to potential danger while fear is a response to present danger. The way humans experience anxiety involves our capacity for higher thinking while the human experience of fear appears to be much the same as the animal model. This article examines these differences at both a phenomenological and neurological level and highlights implications for the treatment of conditioned fear in PTSD. The stimuli for human fear are sensory-perceptual, while the stimuli for most forms of anxiety are conceptual-linguistic. Individuals in a state of fear/terror undergo a radical shift from top-down to bottom-up processing in which access to conceptual-linguistic thought processes is severely restricted and the frontal regions of the cortex are no longer able to override impulses from brain stem and midbrain regions. Conditioned fear involves actual neurological changes in the limbic system. To overcome a traumatic memory, the individual must (1) gain some level of access to the bottom-up state in order to habituate or extinguish the conditioned fear response, and (2) also achieve access to the top-down state in order to process the fear. ]Sage Journals]

In the December issue of The Psychologist, Willem van der Does was a comparison between the nineteenth-century miraculous treatment method of "animal magnetism" and the contemporary eye movement desensitization and reprocessing (EMDR). To give the following contribution Jongh and Ten Broeke indicated that they are completely unable to find Van der Does' case.

Abstract:
Objective: Advocates of new therapies frequently make bold claims regarding therapeutic effectiveness, particularly in response to disorders which have been traditionally treatment-refractory. This paper reviews a collection of new therapies collectively self-termed "The Power Therapies", outlining their proposed procedures and the evidence for and against their use. These therapies are then put to the test for pseudoscientific practice. Method: Therapies were included which self-describe themselves as "Power Therapies". Published work searches were conducted on each therapy using Medline and PsychInfo databases for randomized controlled trials assessing their efficacy, except for the case of Eye Movement Desensitization and Reprocessing (EMDR). Eye Movement Desensitization and Reprocessing has more randomized controlled studies conducted on its efficacy than any other treatment for trauma and thus, previous meta-analyses were evaluated. Results and conclusions: It is concluded that these new therapies have offered no new scientifically valid theories of action, show only non-specific efficacy, show no evidence that they offer substantive improvements to extant psychiatric care, yet display many characteristics consistent with pseudoscience. Abstract:
The development of eye movement desensitization and reprocessing (EMDR) has been hotly debated, with rhetoric often being posited as evidence either for or against the technique. This paper aims to provide a brief overview of the procedure, a critical review of the studies completed to date, a meta-analytic review of the available data, and a chronology of the evolution of EMDR over the past 10 years. Treatment-outcome studies were of such disparate quality-even studies meeting similar broad criteria-that combining their results in a meta-analysis was of very questionable value. Overall, an appraisal of the published research supported the following conclusions: (1) There is overwhelming evidence that eye movements are neither a necessary nor a useful addition to the procedure; (2) there is strong and consistent evidence that EMDR is better than no treatment, yet only as good as any other treatment that utilizes some aspect of exposure therapy; and (3) there is strong evidence that a full-exposure-based intervention package is superior to EMDR. There is also some evidence that "reprocessing" is likewise superfluous to EMDR and that the effects of EMDR dissipate over time. It is also concluded that the current debate cannot be entirely settled through scientific investigation due to the rapid and constant reshaping of what constitutes EMDR, the similarity to extant alternative methods, and the lack of a falsifiable theory underpinning the procedure. Abstract:
As the empirically supported treatment (EST) effort has expanded, there are efforts to make the
study of ESTs a more integral part of training programs. In its present form, the EST list provides
a poor model of how to evaluate treatment and scientific issues related to our field. This article
offers several suggestions regarding how to establish a more relevant scientific agenda for the
committee’swork if the study of ESTs is to usefully influence training programs. Recommendations
are made to encourage programs and the CSP to study mechanisms of change, important
contextual variables for therapy delivery, the distinction between statistical significance and
clinical meaningfulness, dissemination, cost-effectiveness, and iatrogenic effects. It is argued
that any program that created a curriculum educating students to thoughtfully address these
issues when evaluating therapies would be producing sound clinical scientists regardless of the
quality of the EST list itself.

Cet article a pour objectif de passer en revue la littérature scientifique afin d'évaluer l'état des connaissances actuelles quant à l'efficacité et la spécificité de la technique et de pouvoir éventuellement envisager d'autres champs de recherche. Le nombre et la qualité scientifique des études sont encore insuffisants. Leurs conclusions contradictoires ne permettent pas de valider l'EMDR comme une technique efficace et spécifique dans le traitement du PTSD ou d'autres pathologies. Néanmoins, certains résultats positifs, la rapidité d'action et le faible coût de la technique justifient de plus amples recherches.
The "Eye Movement and Desensitizer Reprocessing (EMDR) is a controversial therapeutic intervention used primarily in the treatment of post traumatic stress syndrome (PTSD). This article aims to review the scientific literature to assess the state of current knowledge regarding the effectiveness and specificity of the technique and to possibly explore other fields of research. The number and quality of scientific studies are still insufficient. Their contradictory findings do not validate EMDR as an effective technical and specific in the treatment of PTSD or other disorders. Nevertheless, some positive results, speed of action and the low cost of technology warrant further investigation.

ainsi que les études qui ont été effectuées par des méthodes neuroscientifiques
pour tester ces processus. Plusieurs points restent néanmoins en suspens. Effectivement,
l’EMDR est d’efficacité démontrée dans le stress post-traumatique, et
les traumatismes plus légers, mais il n’y a pas de données dures pour le valider
dans d’autres indications. De plus, son processus, comme il est souligné dans le
chapitre, demeure controversé. Il ne faudrait pas oublier qu’un EMDR sans
mouvements oculaires aboutit à de résultats identiques à ceux d’un EMDR avec
mouvements oculaires, dans la méta-analyse de Davidson et Parker (2001), qui
inclut 13 études comparant ces deux conditions. Cinq méta-analyses ne retrouvent
pas de différence d’efficacité entre TCC et EMDR. Ces faits expérimentaux
ont amené aussi bien l’Association psychiatrique américaine (2004) que le rapport
INSERM (2004) à classer l’EMDR dans les TCC, dont il représente une variante
technique, sans véritable discontinuité. Par ailleurs, l’analogie souvent
faite entre EMDR et la phase REM (phase paradoxale du sommeil correspondant

d’exposition utilisent des distracteurs (relaxation, images mentales de sécurité,

L’hypnose, qui a montré son efficacité dans le stress post-traumatique lors
d’une seule étude contrôlée (Brom, 1989), elle aussi, se sert de mouvements
oculaires, pour capter l’attention. Ainsi faisait son ancêtre, Franz Anton Mesmer,
comme le montre le rapport établi en 1784 par la commission royale sur le
magnétisme animal (Darnton, 1995). Quoi qu’il en soit, l’EMDR propose un
protocole utile, robuste et aisé à enseigner et à appliquer.
France Haour and David Servan-Schreiber present with clarity desensitization
Eye Movement and reprocessing of information (EMDR)
and studies that have been conducted by neuroscientists methods
to test these processes. Several points are still open. Indeed,
EMDR is proven efficacy in post-traumatic stress, and
lighter injuries, but there is no hard data to validate
in other indications. In addition, its process, as outlined in the
chapter remains controversial. We should not forget that EMDR without
eye movements leads to results identical to those of EMDR with
eye movements in the meta-analysis of Davidson and Parker (2001), which
includes 13 studies comparing these two conditions. Five meta-analyzes found
no difference in efficacy between CBT and EMDR. These experimental facts
led both the American Psychiatric Association (2004) report that the
INSERM (2004) to classify EMDR in CBT, which is a variant
technique, no real discontinuity. Moreover, the analogy often
made between EMDR and REM sleep (REM sleep corresponding
dreams and rapid eye movement) is only a hypothesis and
not based so far on scientific data. Finally, all therapies
exposure using distractors (relaxation, mental imagery security
positive thoughts) to facilitate access to the emotions related to the trauma.
Hypnosis, which has shown its effectiveness in post-traumatic stress in
one controlled study (Brom, 1989), it also uses movements
eye for attention. So was his ancestor, Franz Anton Mesmer,
as shown in the report prepared in 1784 by the Royal Commission on
Animal Magnetism (Darnton, 1995). Anyway, EMDR offers a
useful protocol, robust and easy to teach and apply.

Abstract:
Despite impressive gains over the past three decades in the development and evaluation of empirically
supported psychotherapies, such treatments are not used widely by front-line practicing
clinicians. In an attempt to address this science-practice gap, efforts have turned recently to constructing
lists of empirically supported treatments (ESTs) and disseminating information about
these treatments to professionals and the public. This effort has been met with criticism, however,
by both practitioners, on one hand, and psychotherapy researchers on the other. The current
procedures for identifying ESTs are critically reviewed, and recommendations are offered to
improve the scientific viability of the process. It is argued that lists of ESTs are viewed most productively
as one step toward the development of best practice guidelines.

Abstract:
The enormous popularity recently achieved by Eye Movement Desensitization and Reprocessing (EMDR) as a treatment for anxiety disorders appears to have greatly outstripped the evidence for its efficacy from controlled research studies. The disparity raises disturbing questions concerning EMDR's aggressive commercial promotion and its rapid acceptance among practitioners. In this article, we: (1) summarize the evidence concerning EMDR's efficacy; (2) describe the dissemination and promotion of EMDR; (3) delineate the features of pseudoscience and explicate their relevance to EMDR; (4) describe the pseudoscientific marketing practices used to promote EMDR; (5) analyze factors contributing to the acceptance of EMDR by professional psychologists; and (6) discuss practical considerations for professional psychologists regarding the adoption of EMDR into professional practice. We argue that EMDR provides an excellent vehicle for illustrating the differences between scientific and pseudoscientific therapeutic techniques. Such distinctions are of critical importance for clinical psychologists who intend to base their practice on the best available research.

EMDR, and the AIP,” the writer criticizes the
EMDRIA’s defi nition of eye movement desensitization
and reprocessing (EMDR). This defi nition affects
EMDR training requirements, EMDRIA continuing
education credits, the programs selected for the annual
conference, and the work of the Journal for EMDR
Practice and Research. The defi nition was put into place
in 2003 in order to set the professional standard for
the community. It has been revised once and, like all
policies, is subject to further revision based on new
information.

Background.
Flashbacks are the hallmark symptom of Posttraumatic Stress Disorder (PTSD). Although we have successful treatments for full-blown PTSD, early interventions are lacking. We propose the utility of developing a ‘cognitive vaccine’ to prevent PTSD flashback development following exposure to trauma. Our theory is based on two key findings: 1) Cognitive science suggests that the brain has selective resources with limited capacity; 2) The neurobiology of memory suggests a 6-hr window to disrupt memory consolidation. The rationale for a ‘cognitive vaccine’ approach is as follows: Trauma flashbacks are sensory-perceptual, visuospatial mental images. Visuospatial cognitive tasks selectively compete for resources required to generate mental images. Thus, a visuospatial computer game (e.g. “Tetris”) will interfere with flashbacks. Visuospatial tasks post-trauma, performed within the time window for memory consolidation, will reduce subsequent flashbacks. We predicted that playing “Tetris” half an hour after viewing trauma would reduce flashback frequency over 1-week.
Methodology/Principal Findings.
The Trauma Film paradigm was used as a well-established experimental analog for Post-traumatic Stress. All participants viewed a traumatic film consisting of scenes of real injury and death followed by a 30-min structured break. Participants were then randomly allocated to either a no-task or visuospatial (“Tetris”) condition which they undertook for 10-min. Flashbacks were monitored for 1-week. Results indicated that compared to the no-task condition, the “Tetris” condition produced a significant reduction in flashback frequency over 1-week. Convergent results were found on a clinical measure of PTSD symptomatology at 1-week. Recognition memory between groups did not differ significantly. Conclusions/Significance.
Playing “Tetris” after viewing traumatic material reduces unwanted, involuntary memory flashbacks to that traumatic film, leaving deliberate memory recall of the event intact. Pathological aspects of human memory in the aftermath of trauma may be malleable using non-invasive, cognitive interventions. This has implications for a novel avenue of preventative treatment development, much-needed as a crisis intervention for the aftermath of traumatic events.

De discussie over mindfulness die in het vorige nummer van Psychologie & Gezondheid is gevoerd, wordt in dit nummer voortgezet. Frank Vernooij reageert op de eerdere bijdragen vanuit zijn klinische ervaring metmindfulnessmeditatietraining (MTT). Hij relativeert de bijdrage die vanuit de wetenschap kan worden geleverd aan de klinische praktijk in het algemeen en aan het vaststellen van de waarde en effectiviteit van mindfulness in het bijzonder. Ook in het vorige nummer werd door Maya Schroevers en haar collega’s en door Ivan Nyklíček het effectonderzoek naar MTT al positiever ingeschat dan door Remco Havermans, maar Vernooij gaat nog een stapje verder. Hij lijkt de mogelijkheden van de wetenschap om uitspraken te doen over de klinische praktijk niet hoog in te schatten. Havermans vermoedt zelfs dat Vernooij hem beschuldigt van sciëntisme, de overtuiging dat wetenschap superieur is aan alle andere interpretaties van het leven. Havermans maakt glashelder waarom hij vindt dat wetenschap een cruciale rol vervult voor een op evidentie gebaseerde klinische praktijk. Tevens stelt hij nogmaals vast dat de evidentie voor MMT te wensen overlaat en dat ook het meest recente onderzoek, gepubliceerd in 2010 en 2011, dezelfde methodologische tekorten kent als eerder effectonderzoek naar MMT. De zoekterm ‘mindfulnesss’ leverde voor 2010 en 2011 weliswaar bijna 400 citaties op, maar hieronder vond hij slechts vijf relevante MMT-trials. Bij zijn beschrijving van deze trials stelt Havermans terloops vast dat hoge impactfactoren van tijdschriften niet garant staan voor kwalitatief het best mogelijke onderzoek. Dit laatste is mogelijk een troost voor Sandra Mulkens en andere Nederlandse onderzoekers die, geheel in de huidige academische traditie, zo hoog mogelijk proberen te scoren in Engelstalige wetenschappelijke tijdschriften, lees haar column in dit nummer.
Naast effectonderzoek is onderzoek in de traditie van de experimentele psychopathologie (Jansen, Van den Hout & Merckelbach, 2010) een beproefde manier om wetenschappelijke kennis te vergaren die van groot belang kan zijn voor de klinische praktijk. Het openingsartikel van dit nummer is hiervan een voorbeeld bij uitstek. Over de werkzame factoren van EMDR (eye movement desensitization and reprocessing) is veel gespeculeerd, maar nauwelijks iets bekend. Het onderzoek van Marcel van den Hout, Iris Engelhard en collega’s heeft hierover een aannemelijk theoretisch model opgeleverd. De theorie dat belasting van het werkgeheugen een cruciale rol speelt bij het vervagen van negatieve en positieve herinneringen is door hen in een serie experimenten overtuigend aangetoond. Een van die experimenten wordt in het openingsartikel beschreven. De publicaties hierover (zowel Engelstalig als in het Nederlands) en de klinische implicaties die door de onderzoekers zijn geformuleerd, hebben tot veel reacties geleid. Gelet op de geschiedenis van EMDR in Nederland, met uitgesproken pleitbezorgers en criticasters, wekt dat wellicht weinig verbazing. In nummer 1 van Dth (Directieve therapie) van dit jaar reagerenWillen van der Does en Hellen Hornsveld op het model van Van den Hout en Engelhard en de consequenties ervan voor de klinische praktijk. Een opmerkelijke overeenkomst met de discussie over mindfulness is de verschillende interpretatie van het wetenschappelijk onderzoek, in dit geval de evidentie voor EMDR in vergelijking met cognitieve gedragstherapie (CGT): Van der Does (2011) meent dat EMDR net iets minder effectief is dan CGT, terwijl Hornsveld (2011) de conclusie trekt dat CGT, in het bijzonder imaginaire exposure, het aflegt tegen EMDR. Ook al waarschuwen beiden tegen al te snelle gevolgtrekkingen van dit experimenteel onderzoek bij niet-patiënten voor de klinische praktijk, nieuwe wetenschappelijke informatie lijkt vooral geïnterpreteerd te worden vanuit reeds eerder bestaande opvattingen en oordelen over EMDR en CGT. De suggestie van Van den Hout en Engelhard (2011) dat EMDR een gewone CGT-techniek kan worden, zal hen door de snel groeiende Vereniging EMDR Nederland niet in dank worden afgenomen, al hoopt Van der Does (2011) op decimering van het ledental.
Al met al blijkt uit deze recente discussies het spanningsveld tussen wetenschap en klinische praktijk. De ontwikkelingen in de (klinische) psychologie, waarbij de kloof tussen wetenschappers en clinici alleen maar lijkt toe te nemen, onder andere omdat het door de toenemende specialisaties steeds moeilijker wordt voor psychologen om beide beroepsrollen te combineren, doen daar geen goed aan.
The discussion about mindfulness in the previous issue of Psychology & Health are hereby continued in this issue. Frank Vernooij responding to previous contributions from his clinical experience mindfulness and meditation training (MTT). He puts the contribution that science can be delivered to the clinical practice in general and to determine the value and effectiveness of mindfulness in particular. In the previous issue was Schroevers Maya and her colleagues and by Ivan Nyklicek impact study MTT been more positive assessments than by Remco Havermans, Vernooij but goes one step further. He seems the ability of science to make statements about the clinical practice to estimate high. Havermans even suspects that he Vernooij accused of scientism, the belief that science is superior to all other interpretations of life. Havermans makes clear why he thinks science is a crucial role for an evidence-based clinical practice. Also, he once again that the evidence of MMT is inadequate and that the latest research, published in 2010 and 2011, has the same methodological shortcomings as earlier research on effects MMT. The search term 'mindfulnesss "delivered in 2010 and 2011, while nearly 400 citations, but below, he found only five relevant trials MMT. In his description of these trials suggests that high Havermans casually impact factors of journals does not guarantee the best quality research. The latter may be a comfort to Sandra Mulkens and Dutch researchers, all in the current academic tradition, try to score as high as possible in English scientific journals, read her column in this issue.
Besides effects research, research in the tradition of experimental psychopathology (Jansen, Van den Hout & Merckelbach, 2010) a proven way to scientific knowledge is produced that may be important for clinical practice. The opening article of this issue is an example par excellence. On the effective factors of EMDR (Eye Movement Desensitization and Reprocessing) has been much speculation but very little known. The study by Marcel van den Hout, Iris Engelhard and colleagues has made a plausible theoretical model yielded. The theory that taxes working memory plays a crucial role in the blurring of negative and positive memories by them in a series of experiments convincingly demonstrated. One of those experiments in the opening article. The publications on this subject (both English and Dutch) and the clinical implications have been formulated by the investigators, have led to many responses. Given the history of EMDR in the Netherlands, with strong advocates and critics, suggests that perhaps little surprise. In a number of Dth (directive therapy) this year to respond to van der Does and Helen Horn Field on the model of van den Hout and Engelhard and its consequences for clinical practice. A remarkable agreement with the discussion of mindfulness is the different interpretations of scientific research, in this case the evidence for EMDR compared to cognitive behavioral therapy (CBT): Van der Does (2011) believes that EMDR is slightly less effective than CBT, while Horn Field (2011) concludes that CBT, in particular imaginal exposure, it looses against EMDR. Although both warn against too rapid conclusions from this experimental study in non-patients for clinical practice, new scientific information seems to be interpreted from pre-existing beliefs and judgments about EMDR and CBT. The suggestion of Van den Hout and Engelhard (2011) that EMDR is a simple CBT techniques may be, will bring them through the fast-growing Netherlands Association EMDR not be appreciated, though hopes Van der Does (2011) on the decimation of the membership.
All in all, of these recent discussions the tension between science and clinical practice. Developments in the (clinical) psychology, where the gap between scientists and clinicians only seems to be increasing, partly because it is the increasing specialization is becoming increasingly difficult for psychologists to both professional roles to combine, do not do well.

to understand the cytoarchitecture of the frontal cortex, on the one

Abstract:
This dissertation was a study of a community corrections program which incorporated adjunct Energy Psychology Therapies for Substance Abuse Offenders (SAOs), who were transitioning back into community. Rates of relapse and recidivism were compared, upon success/failure to complete drug and alcohol treatment. When Energy Psychology therapies were added, assisting offenders in resolving underlying trauma issues, there were significance differences between groups. The Choices Program used brief therapies including: Eye Movement Desensitization and Reprocessing, Thought Field Therapy, Emotional Freedom Techniques, (EMDR, TFT, EFT) along with group and individual classes/sessions to relieve underlying trauma symptoms. The results indicated that people who chose to resolve underlying trauma achieved more successful treatment outcomes, thereby lowering rates of relapse and recidivism significantly.

EMDR is a powerful integrative psychotherapeutic intervention. However, in the case of disrupted attachment and significant dissociative symptoms EMDR can be destabilizing if used early on in treatment. That is, fragmentation of self and dissociative symptoms commonly interfere with information processing, thus barring the integration and resolution of the traumatic experience through EMDR. Dissociation interferes with clients sense of their own body, their ability to experience emotion, their capacity for emotional regulation and their sense of self. Addressing dissociative symptoms prior to proceeding with EMDR treatment is essential for positive treatment outcomes.
A neurobiological model is described that guides therapeutic interventions and integrates diverse approaches that include not only EMDR and relevant target selection, but also mindfulness, body therapy approaches, ego-state interventions, sensory integration, as well as neurobiologically based interventions. Such interventions can be used both in the preparation phase but can also form useful interweaves during EMDR information processing.
Using a neurobiologically informed approach, the case is made for the use of somatic and ego-state interventions when dissociation is a significant part of the clinical presentation. Specific focus is on different ego-state and body therapy interventions to increase awareness of the self and ones body. Body therapy and somatic interventions are distinguished from other psychotherapeutic interventions in that they are expressed in markedly slowed-down time, in order to give clients ample time to experience the felt sense of their bodies. Similarly ego-state work can be utilized to titrate information processing, as well as provide clients with internal resources that aid in enhanced information processing.
Attendees will gain knowledge about possible underlying neurobiological processes with regard to attachment, dissociation and adaptive information processing and how this relates to EMDR treatment. The workshop will teach specific interventions intended to stabilize clients, create safety, help the client stay connected or get reconnected and therefore minimize dissociative symptoms and their effect. Participants will learn how to effectively integrate different somatic and ego-state interventions in the treatment of attachment and trauma related syndromes and dissociative disorders, as well as how to enhance information processing during the EMDR treatment. The workshop also will discuss innovative use of opioid antagonists in the treatment of dissociative symptoms with a particular focus on EMDR.

Comments on the book by Francine Shapiro (see record 2001-05049-000). I was fortunate to observe Shapiro’s concern for the principles of science in the development of eye movement desensitization and reprocessing (EMDR). She insisted that EMDR be called experimental until after there was supportive peer-reviewed literature, limited training to mental health professionals, and strongly encouraged research. EMDRIA and its peer-reviewed journal have generally continued in this admirable direction. However, when an association is organized around a specific method of treatment rather than a problem area or a more general philosophy of approach, the question of orthodoxy must be addressed. At its best, orthodoxy ensures that practice is consistent with what has been demonstrated to be worthwhile, and all benefit from adherence. Also, there are aspects of the adaptive information processing (AIP), as it is described in the text, that are debatable and some that appear even self-contradictory. One example is Shapiro’s claim that the AIP is a “psychophysiological” model while also denying that enough is known about the details of psychophysiology to offer an explicit model. There is increasing evidence to support the biological basis of mental disorders. Subsequently, understanding the neurobiological context from which mental distress arises can help counselors appropriately apply cognitive behavioral therapy and other well-researched cognitive interventions. The purpose of this article is to describe the neurobiological context underlying the formation and treatment of posttraumatic stress disorders, a mental disorder frequently encountered by counselors, from a cognitive therapy framework.
Recent changes to the Council for Accreditation of Counseling and Related Educational Programs (2009) accreditation standards include the need for counselors-in-training to understand the neurobiological basis of behavior, which marks a new direction for the training of professional counselors who have historically reacted ambivalently toward medical models for understanding client concerns and treatments. Yet recent findings in neuroscience actually support the verbally based interventions that counselors typically use in treatment; therefore, there is much to be gained by counselors and counselor educators in understanding the basics of human neurobiology and how commonly used counseling interventions intervene on these biological systems. The National Institute of Mental Health (2010) stated in a recent strategic plan that “Important discoveries in areas such as genetics, neuroscience, and behavioral science largely account for the substantial gains in knowledge that have helped us to understand the complexities of mental illnesses and behavioral disorders over the past 15 years” (“Introduction,” para. 4).
Given the increasingly biological focus of mental health research, the practicing counselor is faced with the task of understanding and using the emerging mental health treatments and explaining to clients, to reimbursing agencies, and to the broader public how counseling fits within the medically dominated mental health culture. Some counselors have long reacted ambivalently toward the pathologically oriented diagnostic categories of the Diagnostic and Statistical Manual of Mental Disorders (4th ed.; DSM-IV;American Psychiatric Association, 1994) system and the medication-dominated world of psychiatry. For example, the contrasting viewpoints on this issue were published in the Journal of Counseling & Development between Allen and Mary Ivey (1998, 1999) and Scott Hinkle (1999). Ivey and Ivey (1998) argued for a developmental interpretation to the DSM-IV, opposing what they called the “pathological view” (p. 334) of the manual. According to Ivey and Ivey, disorders could be viewed through a positive development tradition to lie not within the individual but within the contextual systems in which a person lives. Subsequently, disorders are viewed as a “logical response to a developmental history” (Ivey & Ivey, 1999, p. 484). By contrast, Hinkle (1999) argued that because anxiety and depressive disorders “are the most common clinical symptoms associated with presentation to counseling” (p. 475), the counseling profession is weakened if counselors shy away from direct participation in the DSM nomenclature and treatment parlance. As Hinkle indicated, “mental disorders according to the medical model describe disease processes, not people” (p. 475). Regardless of the reader's philosophical perspective, practicing counselors know participation in medical and psychiatric systems is necessary at times. Also, recent discoveries in the field of neuroscience are providing evidence that interventions often used by counselors have direct physiological impact on client neurobiology (Kennedy et al., 2007; Linden, 2006). For example, Felmingham et al. (2007) demonstrated significant differences in brain activity before and after 8 weeks of exposure therapy, which correlated with a reduction in posttraumatic stress disorder (PTSD) symptom severity. Similarly, Paquette et al. (2003) found that cognitive behavioral therapy (CBT) alters the activation and metabolism of specific brain regions following successful treatment of spider phobia. These findings, along with others (for a detailed review, see Beauregard, 2007; Frewen, Dozois, & Lanius, 2008), are significant because they support the techniques, interventions, and approaches used by counselors and provide a mechanism by which counseling positively affects brain physiology. Within the emerging physiologically based treatment milieu, counselors should be prepared to articulate how cognitive counseling interventions make measurable changes to the client. Although cognitive-behavioral-based approaches are effective in the treatment of a number of psychiatric illnesses, adult PTSD is arguably one of the best understood mental disorders from a neurological perspective. It thus presents a valuable model for exploring not only the basic tenets of neurobiology but also the mechanisms behind its successful treatment. Furthermore, PTSD is a disorder that counselors will likely encounter in practice.
PTSD is a mental disorder characterized by a sudden onset of symptoms due to environmental exposure to a psychologically stressful event such as war, natural disaster, or sexual victimization. Thus, it provides a clear example of how, even in adulthood, neurological adaptation (in this case maladaptive changes) can functionally “rewire” the brain in a short period of time, resulting in a sustained array of clinical symptoms. The diagnostic criteria for PTSD are a history of exposure to a traumatic event meeting two criteria and symptoms from each of three symptom clusters: intrusive recollections, avoidant/numbing symptoms, and hyperarousal symptoms. A fifth criterion concerns duration of symptoms and a sixth assesses functioning (American Psychiatric Association, 2000).
The National Comorbidity Survey Replication, conducted between February 2001 and April 2003 (Kessler et al., 2005), determined that the estimated lifetime prevalence of PTSD among American adults is 6.8%, with women (9.7%) twice as likely as men (3.6%) to have the disorder at some point in their lives. These findings are very similar to those of the first National Comorbidity Survey conducted in the early 1990s (Kessler, Sonnega, Bromet, Hughes, & Nelson, 1995), which was composed of interviews of a representative national sample of 8,098 Americans ages 15 to 54 years. In this earlier sample, the estimated prevalence of lifetime PTSD was 7.8% in the general population. As in the more recent survey, women (10.4%) were more than twice as likely as men (5%) to have PTSD at some point in their lives (Kessler et al., 2005; Kessler et al., 1995).

The Journal ’s instructions to authors are located inside
the back cover of every issue. The relevant passage
stated, “Articles that recommend a clinical approach
that differs from EMDR’s standard protocol or its
foundational Adaptive Information Processing model
(Shapiro, 2001) should explain these differences.”The purpose of this instruction was to encourage
clarity rather than conformity. It is important for
Journal readers, some of whom have not been trained
in this method, to know whether the techniques described
are standard for eye movement desensitization
and reprocessing (EMDR) or variations on the
protocol. For example, the reader of an article describing
a technique that combines EMDR with aspects of
another psychotherapy approach such as hypnosis
could assume that hypnotic inductions were part of
standard EMDR unless it was stated clearly which elements
were adapted from that method. The request that authors also clarify points of divergence from
the adaptive information-processing (AIP) model was
similarly intended to generate clarity. It is consistent
with the recognition that a common platform for discussion
is needed, even if that platform is imperfect.
It is also consistent with Shapiro’s teaching of the AIP
model as a clinical heuristic that is subject to revision
in the face of new data or more compelling models.
Rather than thwarting discussion, the request that authors
explain points of disagreement was designed to
promote deeper consideration of the mechanisms and
models underlying EMDR.
In response to Dr. Lipke’s letter, the Journal ’s Editorial
Board engaged in a thorough review process to
examine the value of this instruction and invited responses
(see letters above) from Dr. Shapiro and the
EMDRIA’s Board of Directors to further illuminate
the process. The Editorial Board has a diverse membership,
and there were divergent opinions, including
some disagreements with the following response.

Talented entrepreneurs have been developing and marketing novel therapeutic methods, some touted as veritable miracle cures for diverse complaints. This phenomenon has caught the attention of scientist-practitioners in psychology, many of whom criticize these approaches as “pseudoscientific.” The purpose of this essay is to sketch a simpler, alternative approach to debunking dubious methods in clinical psychology. When therapeutic entrepreneurs make claims on behalf of their interventions, we should not waste our time trying to determine whether their interventions qualify as pseudoscientific. Rather, we should ask them: How do you know that your intervention works? What is your evidence?

Abstract:
Eye Movement Desensitization and Reprocessing (EMDR) is a treatment for trauma that has been approached primarily from cognitive-behavioral and neurobiological perspectives. This thesis explores the notion that much of EMDR's effect in the way of transformation and healing trauma is due to its ability to provoke imagination. The possibility of EMDR's connection with the imaginal faculty of the psyche is investigated with an emphasis on the qualities of EMDR that are grounded in depth psychology principles. Trauma and the importance of imagination in the healing process are discussed from the approach of the analytical and archetypal schools of psychology in addition to current bioscience perspectives. The research methodology used is alchemical hermeneutics, which facilitates the mining of unconscious material through transference dialogues. The thesis demonstrates that EMDR, when practiced within the context of depth psychology, is one avenue for activating significant, transformative imagery and accessing the unconscious to facilitate healing.

Eye Movement Desensitization and Reprocessing (EMDR) is an integrative psychotherapy approach that has been consistently evaluated as efficacious in the treatment of PTSD. The information processing model that guides its clinical application posits that EMDR should be effective in treating other psychological disorders that have experiential contributors. Research is needed to assess such applications. This special issue features three case series in which EMDR was applied to the treatment of complex PTSD, phobias, and chronic pain, respectively. The authors discuss deficits in the research literature, provide preliminary data on EMDR treatment of these conditions, and offer descriptive guidelines for evaluation that are achievable by the practicing clinician. Two additional articles offer preliminary data on physiological and cognitive/affective concomitants of therapeutic change. It is argued that clinicians should play a greater role in the rigorous and extensive examination of psychological treatments in the context of the exigencies of clinical practice. [Author Abstract]

the adaptive information processing (AIP) model and
my text. However, I would like to begin by stating
that I applaud the free exchange of ideas regarding the

It is increasingly recognized that psychological trauma causes neurophyslological dysregulation in children, which then alters their developmental course. In this full-day seminar, we will present an organizational framework that clarifies how trauma-based diagnosis relates to traditional nosological systems, such as DSM-IV, and ICD-10, and how to treat these symptoms of dysregulation using EMDR. We will illustrate these concepts with videotapes of EMDR treatment sessions with children. These video seggments will cover a variety of applications with children
of different ages, including single traumas, multiple traumas, complex
disorders of childhood, attachment issues, dissociation, and group application of EMDR with war refugee children. Following the workshop,

In EMDR, we see results that inform us about neuroplastic abilities of the brain, that go beyond occurrences in conventional psychotherapy. For example, in EMDR, we sometimes see the emergence and resolution of stigmata; the elimination of phantom limb pain; resolution of trauma with very young children; the resolution of pre-verbal trauma in children and adults. Through case presentations, videos, photographs, and brain imaging, this offering will consider some neuroscientific implications, based on detailed analyses of several cases of adults and children. Until recently we had little knowledge how to help people integrate such disintegrated traumatic imprints. Traditionally, before the advent of contemporary methods of treatment outcome evaluation, many clinicians, from Pierre Janet to Milton Erikson and his followers, considered hypnosis to be the treatment of choice. Unfortunately the efficacy of hypnosis for the treatment of PTSD was never systematically studied. EMDR was the first of a group of new therapies that did not primarily rely on speaking about one’s traumatic experiences, but that claimed to be able to rapidly and effectively integrate traumatic memories by asking PTSD subjects to focus intensely on the emotions, sensations and meaning of the traumatic experience, while asking to follow the hand of a clinician who induces slow saccadic eye movements. EMDR had a number of advantages over hypnosis, including the fact that it could easily be put into a treatment protocol. This makes it relatively easy to conduct treatment outcome research. Since it was first articulated by Francine Shapiro, around 1988, it has received intense scientific scrutiny and has been found to be quite an effective treatment of PTSD (e.g. Chemtob et. al, 2000), even though the specific role of eye movements in its therapeutic action remains controversial.

Since its introduction, Eye Movement Desensitization and Reprocessing (EMDR) (Shapiro, 1989) has received the attention of many mental health professionals. There has been much critical debate on the subject of EMDR. Most of the clinical discussion has centered on the role of EMDR in the treatment of Posttraumatic Stress Disorder (PTSD).
While the EMDR procedure has been compared to Mesmerism (McNally, 1999), declared as pseudoscience (Herbert, Lilienfeld, Lohr, Montgomery, O’Donohue, Rosen, and Tolin, 2000), or regarded as a highly marketed placebo (Lilienfield, 1996), most studies support the efficacy of EMDR in treating PTSD (Ironson, Freund, Strauss, and Williams, 2002; Lee, Gavriel, Drummond, Richards, and Greenwald, 2002; Marcus, Marquis, and Sakai, 1997; Rothbaum, 1997; Van Etten and Taylor, 1998; Wilson, Becker, and Tinker, 1997). There has been some evidence for accompanying physiological changes in PTSD subjects treated with EMDR with patterns of cortex functioning, (Levin, Lazrove, and van der Kolk, 1999; Nicosia, 1994) event-related potential changes (Lamprecht, Kohnke, Sack, Matzke and Munte, 2004), as well as positive effects on the level of the stress hormone cortisol (Haber, Kellner and Yehuda, 2002).
