Lo studio riguarda l’applicazione dell’EMDR ai Pazienti affetti da “Sindrome da deficit posturale” (Da Cuña). Il problema coinvolge il 10% della popolazione, di cui il 15% è sintomatico. È di difficile inquadramento, dato che i sintomi somatici che presenta coinvolgono tutti e tre i versanti delle regolazioni automatiche: dolori muscolo-scheletrici migranti, derivanti da disordini posturali; pseudovertigini, da deficit di localizzazione spaziale; disturbi cognitivi (fino alla dislessia), da alterata integrazione percettiva. Le modalità compensatorie a tali deficit possono investire la sfera psicologica (senso profondo di inadeguatezza, ansia, depressione...), sviluppare atteggiamenti strategici di evitamento (agorafobia da deficit di orientamento spaziale), spingere a interpretare reazioni fisiche come se fossero psicologiche (attacchi di panico, enuresi notturna…). Proprio il sottile intreccio tra aspetti somatici e psicologici rende complessa la diagnosi; può capitare infatti che un paziente con problemi di questa natura si rivolga a un terapista somatico (posturologo, osteopata…), che non coglie il problema. Abbiamo così messo a punto un sistema diagnostico che riconosca se l’aspetto psichico sia conseguenza di un vero disturbo somatico o se invece sostenga una serie di somatizzazioni tendenzialmente derivanti da traumi. Nell’uno o nell’altro caso, l’utilizzo dell’EMDR può essere risolutivo: evidentemente sul versante traumatico, ma pure per la desensibilizzazione delle convinzioni negative derivate dalle strategie psicologiche reattive al deficit somatico. Metodi. La valutazione diagnostica e le variazioni sintomatologiche sono state effettuate con DSMIV, SWAP-200, SCL90-R, TAS 20, VAS, anamnesi posturologica, kinesiologia, posturodinamica, riflessi primitivi e sensoriali, valutazione osteopatica, Maddox, stabilometria basale e con challenges). Risultati. Gli interventi su misura hanno ridotto la sintomatologia, come è rappresentato dalle rilevazioni effettuate e dai feedback dei pazienti. La conoscenza integrata, sia che il primo approccio sia psicologico o posturale, consente una diagnosi accurata e dunque una strategia terapeutica sinergica efficace che permette di sbloccare terapie cronicizzate.
The study concerns the application of EMDR in patients with "postural deficiency syndrome" (Da Cuna). The problem affects 10% of the population, of which 15% are symptomatic. It is
classification difficult, because the somatic symptoms presenting involving all three sides automatic adjustments: musculoskeletal pain migrants from postural disorders; pseudovertigini, spatial localization deficit, cognitive problems (up to dyslexia), from altered perceptual integration. We have thus developed a diagnostic system that recognize if the appearance is the result of a true mental disorder or whether it supports somatic a series of somatization tendency resulting from trauma. In either case, use EMDR can be resolutely on the side obviously traumatic, but also for desensitization of the negative beliefs derived from psychological strategies responsive to deficit somatic. Methods. The diagnostic evaluation and symptom changes are were made with DSMIV, SWAP-200, SCL90-R, TAS 20, VAS, history Posture, kinesiology, posturodinamica, primitive reflexes and sensory evaluation osteopathic, Maddox, stabilometry baseline and challenges). Results. Tailored interventions have reduced symptomatology, as represented by the surveys conducted and feedback from patients. The integrated knowledge, whether the first approach is psychological or postural, allows a diagnosis accurate and therefore a therapeutic strategy that allows you to unlock synergistic effective therapies chronic.

The scenarios, specific for ach patient, were composed of 4 active conditions: neutral image (N) traumatic memory (T), traumatic memory suppression (TS) and positive image (P) corresponding to 3 minutes where the patient had to imagine the corresponding sequence with eyes closed Measures of heat rate and subjective disturbance (SUD) were measured during the recoding. The MEG data were analysed in the source domain using a spatial filtering approach. Measurement of electrophysiological waves of various rhythms: delta: 1-4 Hetz (Hz), theta: 4.8-5.8 Hz, alpha: 8-12 Hz and beta: 12-25 Hz was obtained. Anatomical information was from MRI imaging.

Two experiments are reported that investigate the effects of saccadic bilateral eye movements on the retrieval of item, associative, and contextual information. Experiment 1 compared the effects of bilateral versus vertical versus no eye movements on tests of item recognition, followed by remember-know responses and associative recognition. Supporting previous research, bilateral eye movements enhanced item recognition by increasing the hit rate and decreasing the false alarm rate. Analysis of remember-know responses indicated that eye movement effects were accompanied by increases in remember responses. The test of associative recognition found that bilateral eye movements increased correct responses to intact pairs and decreased false alarms to rearranged pairs. Experiment 2 assessed the effects of eye movements on the recall of intrinsic (color) and extrinsic (spatial location) context. Bilateral eye movements increased correct recall for both types of context. The results are discussed within the framework of dual-process models of memory and the possible neural underpinnings of these effects are considered. [PsycINFO]

Abstract:
Objectives. Intrusive memories of extreme trauma can disrupt a stepwise approach
to imaginal exposure. Concurrent tasks that load the visuospatial sketchpad (VSSP)
of working memory reduce the vividness of recalled images. This study tested
whether relief of distress from competing VSSP tasks during imaginal exposure is at
the cost of impaired desensitization.
Design. This study examined repeated exposure to emotive memories using 18
unselected undergraduates and a within-subjects design with three exposure
conditions (Eye Movement, Visual Noise, Exposure Alone) in random, counterbalanced
order.
Method. At baseline, participants recalled positive and negative experiences, and
rated the vividness and emotiveness of each image. A different positive and negative
recollection was then used for each condition. Vividness and emotiveness were rated
after each of eight exposure trials. At a post-exposure session 1 week later,
participants rated each image without any concurrent task.
Results. Consistent with previous research, vividness and distress during imaging
were lower during Eye Movements than in Exposure Alone, with passive visual
interference giving intermediate results. A reduction in emotional responses from
Baseline to Post was of similar size for the three conditions.
Conclusion. Visuospatial tasks may offer a temporary response aid for imaginal
exposure without affecting desensitization.

It is hypothesized that eye movements used in EMDR practice influence both memory and emotional state, some laboratory studies provide evidence for such influence (Christman et al, 2003, Neurobiology; Barrowcliff et al., J. Forensic Psychiatry and Psychology, 2004). From a neurophysiological point of view, it is also interesting to examine to what extent some eye movement parameters may be modulated by memory and emotions. For instance, it is well established that memory driven saccades in simplified laboratory tests, present different characteristics (longer preparation time, lower accuracy, lower velocity) than visually guided movement. During EMDR the eye movements (mostly pursuit) are elicited externally by the therapist, yet at the same time the patient is in touch mentally with the traumatic image and related negative emotions. The physiology of such movements might be different and this could be explored with specific studies. Another important issue is to what extent movements of the eyes are helpful in coding and retrieving visual images. Accordingly to scanpath theory, eye movements are cognitively driven by sensory and motor representation; repetitive eye movements to the same area of an image are attributed to checking if the image corresponds to what the observer imagines (Noton & Stack, 1971; Brandt & Stark, 1997). Future laboratory research could bring together such theories and EMDR psychotherapy, e.g., by examining spontaneous eye movements related to negative image.
In this conference, we will present preliminary data from a laboratory study on eye movement patterns during the Roschach test. Eight healthy students conducted the experiment. Seated in front of a computer screen, subjects were equipped with a binocular video eye tracker (Chronos) sampling eye position images from each eye every 4 msec. Instantaneous eye position data were stored on a computer and analyzed afterwards with laboratory software. Each are of the Roschach test was presented for 30 sec., after which, the experimenter (a psychologist) recorded the response given by the subject.
Analysis of eye movement patterns were examined in relation to subjective verbal reports.
Results: The first question asked was to what extent the eye movement pattern reflected the imaged projected by the subject or by the visual properties of the Rorschach image itself. To gain some insight, we concentrated on the cards for which our subjected gave no standard responses, thus very different from one subject to the other (e.g., card IX). Eye movement exploration (the surface of the image explored, its patiaol composition e.g. the resprctive proportion of horizontal versus vertical movements) was correlated with the verbal report and the specific mental imagery (larger surface, multiple focusing points when several personages or objects were imaged). This provides evident for top-down influence; the eyes are exploring the projective interpretative image; repetitive movements back and forth to same points which help in turn consolidate this mental imagery.
Psychological analysis of eye movements (saccades and fixation) was also made. Saccades are found to be as fast as when looking reflexively to single visual targets; during fixations, however, between saccades, the eyes were more instable, particularly the visual uses were crossing at different depths from one fixation to another. Further ongoing analysis will correlate fixation duration and depth instability to spatial and emotional content of the subjective report.

It has been claimed that the symptoms of post-traumatic stress disorder (PTSD) can be ameliorated by eye-movement desensitization-reprocessing therapy (EMD-R), a procedure that involves the individual making saccadic eye-movements while imagining the traumatic event. We hypothesized that these eye-movements reduce the vividness of distressing images by disrupting the function of the visuospatial sketchpad (VSSP) of working memory, and that by doing so they reduce the intensity of the emotion associated with the image. This hypothesis was tested by asking non-PTSD participants to form images of neutral and negative pictures under dual task conditions. Their images were less vivid with concurrent eye-movements and with a concurrent spatial tapping task that did not involve eye-movements. In the first three experiments, these secondary tasks did not consistently affect participants' emotional responses to the images. However, Expt 4 used personal recollections as stimuli for the imagery task, and demonstrated a significant reduction in emotional response under the same dual task conditions. These results suggest that, if EMD-R works, it does so by reducing the vividness and emotiveness of traumatic images via the VSSP of working memory. Recent work has demonstrated that horizontal saccadic eye movements enhance verbal episodic memory
retrieval, particularly in strongly right-handed individuals. The present experiments test three primary
assumptions derived from this research. First, horizontal eye movements should facilitate episodic memory
for both verbal and non-verbal information. Second, the benefits of horizontal eye movements should
only be seen when they immediately precede tasks that demand right and left-hemisphere processing
towards successful performance. Third, the benefits of horizontal eye movements should be most pronounced
in the strongly right-handed. Two experiments confirmed these hypotheses: horizontal eye
movements increased recognition sensitivity and decreased response times during a spatial memory test
relative to both vertical eye movements and fixation. These effects were only seen when horizontal eye
movements preceded episodic memory retrieval, and not when they preceded encoding (Experiment 1).
Further, when eye movements preceded retrieval, they were only beneficial with recognition tests
demanding a high degree of right and left-hemisphere activity (Experiment 2). In both experiments
the beneficial effects of horizontal eye movements were greatest for strongly right-handed individuals.
These results support recent work suggesting increased interhemispheric brain activity induced by bilateral
horizontal eye movements, and extend this literature to the encoding and retrieval of landmark
shape and location information.

Grant and Spivey (2003) proposed that eye movement trajectories can influence spatial reasoning by way
of an implicit eye-movement-to-cognition link. We tested this proposal and investigated the nature of this link
by continuously monitoring eye movements and asking participants to perform a problem-solving task under
free-viewing conditions while occasionally guiding their eye movements (via an unrelated tracking task), either
in a pattern related to the problem’s solution or in unrelated patterns. Although participants reported that they
were not aware of any relationship between the tracking task and the problem, those who moved their eyes in
a pattern related to the problem’s solution were the most successful problem solvers. Our results support the
existence of an implicit compatibility between spatial cognition and the eye movement patterns that people use
to examine a scene.

Spatial intelligence is emphasized in traditional EMDR protocols and instructional methods when focus is placed on imagery. Bodily-kinesthetic intelligence is honoured when body cues are addressed. However, individuals with primary musical or sonic intelligences are not optimally attended to with orthodox application of well-accepted EMDR strategies. Without making any changes to the stages of the Shapiro protocol, musical interventions can be implemented into EMDR treatment to amplify the holistic design of EMDR and to optimally serve those who struggle with spatial intelligence. In this workshop, participants will learn how to utilize music in resource development and EMDR preparation, to apply music as an appropriate “cognitive interweave” when processing is not flowing optimally, and to consider the impact that music can have on EMDR treatment. The presenter will implement a combination of lecture, participation activities, and case discussion to achieve learning objectives.

Electroencephalography (EEG), due to its peculiar time and spatial resolution, was used for the first time to fully monitor neuronal activation during the whole eye movement desensitization and reprocessing (EMDR) session, including the autobiographical script. The present case report describes the dominant cortical activations (Z-score >1.5) during the first EMDR session and in the last session after the client processed the index trauma. During the first EMDR session, prefrontal limbic cortex was essentially activated during script listening and during lateral eye movements in the desensitization phase of EMDR. In the last EMDR session, the prevalent electrical activity was recorded in temporal, parietal, and occipital cortical regions, with a clear leftward lateralization. These findings suggest a cognitive processing of the traumatic event following successful EMDR therapy and support evidence of distinct neurobiological patterns of brain activations during lateral eye movements in the desensitization phase of EMDR.

Posttraumatic stress disorder is a challenging condition, as people become captured by their past experiences and have difficulty engaging with the present. At the core of this condition is the role of traumatic memories, which orientate the individual's awareness and reactivity to reminders of the instigating traumatic event. The role of traumatic events has not been fully understood and grappled with in the full range of psychopathological conditions. This has important implications for the application of EMDR as a treatment for disorders above and beyond posttraumatic stress disorder.
However, the problems with information processing in PTSD go above and beyond the fear circuitry and reactivity to traumatic memories. Individuals with PTSD also have major difficulties with their self-orientation, which is reflected in deficits in default networks, the idling systems of the brain. These changes are indicative of problems in self-registration and free-floating reflection. Dissociative symptoms may relate to these abnormalities of individuals resting states as they reflect a sense of disconnection and integration of internal states into consciousness.
Secondly, posttraumatic stress disorder is associated with major problems in dealing with neutral environmental information. This is reflected in the symptoms of difficulty with concentration and emotional numbing. The underlying neurobiology of the working memory abnormalities in posttraumatic stress disorder will be highlighted. These studies show that, in PTSD, relatively simple attentional tasks recruit neural networks normally reserved for more demanding and higher order tasks. When confronted with more demanding challenges, individuals with PTSD do not have any further capacity to allocate to processing complex environments.
Individuals with PTSD also demonstrate a problem with switching their attentional focus from an idling to active state. The data suggests that they continue to use visio-spatial networks more than language-based systems for dealing with verbal tasks. This observation is in keeping with a broad body of literature, which suggests that there are problems with the processing of verbal memory tasks in PTSD. EMDR, as a treatment, may have an advantage, as it is not so dependent on verbal representations of traumatic experiences as other treatment approaches.
Finally, an important development in the field is a better understanding of the patterns of abnormal cortical arousal that accompany the peripheral arousal abnormalities in PTSD. Quantitative EEG has given insights into the instability of the cortical neural networks. Neurotherapy represents a treatment that can further assist clinicians in the management of these patients. It is important to consider the underlying psychosomatic aspects of posttraumatic stress disorder and ensure that treatment addresses these components as well the traumatic memories. Treatment should be thought of as a staged process where the processing of traumatic memories is only one component of a disorder that impacts on a range of information processing domains.

Abstract:
This presentation compares two different descriptions: Paradigm and State of Consciousness, of the changes,
observable in a psychotherapy integrated with EMDR, to establish which of the two descriptions is the most likely
and consistent. Applying the method of the double description it will seek to identify contributions that may be

useful for the construction of a Unified Theory of Psychotherapy. At first, attention is focused upon the necessity
to construct a bridge-language between the different dialects of psychotherapies. Short specialized definitions,
such as Paradigm and State of Conscience, could be examples of the complex concepts, here denominated
synthetic �t� metaphors, transtheorical or Tran disciplinal, to be researched for constructing a common language
between the plurality of psychotherapies. Some implications of the methodological innovation carried on by
EMDR in psychology, psychotherapy, and in other disciplines are then analysed and proposed. In particular, the
easy integration of traditional psychotherapeutic methodologies with EMDR and the discovery of new target of
psychotherapy, have led to looking at all psychological activities in terms of a spatial metaphor. On the basis of
this a general map of the psychological territories of �sapiens sapiens� has been drafted. This general map is
divided in two main areas: A -the psychological territories of the individual, B -the psychological territories of the
species, This species map has some specific characteristics; since �sapiens sapiens� are social animals, their
relations have clear functions of social and environmental group interface too. These maps will be illustrated. In
its original meaning, the definition of Paradigm will be found within the maps and consequently analysed in its
variations. The amplified theory of State of Conscience will be presented and analysis, similar to the previous
one, will be carried out. A description of EMDR, that may be useful for implementing its understanding, will be
proposed at the end.
