El estrés subsiguiente después de un traumatismo, representa un trastorno disfuncional – tanto interno como externo – que se manifiesta en alteraciones en el reconocimiento cognitivo y en los comportamientos, llevando además asociados síntomas somáticos, afloramiento de problemas inconscientes y ansiedades. Pues una de las características del estrés post-traumático, es precisamente la pérdida de equilibrio entre el mundo interno y externo de quien lo sufre. Por eso, ante la complejidad de las respuestas post-traumáticas, éstas se pueden catalogar en gran medida dentro de las perturbaciones psicopatológicas.
El estrés psicológico surge por una situación estresante “real”, externa, tangible y la reacción ante esta difícil experiencia, evoca un conjunto bastante universal y coherente de síntomas y respuestas que provocan reacciones primitivas relacionadas con temores inconscientes ante las amenazas a la vida, que hacen aflorar incipientemente fantasías e impulsos abrumadores. Los resultados son los pensamientos disfuncionales que conducen a las respuestas y a los comportamientos desadaptativos.
Objetivos: el taller está diseñado para proporcionar a estudiantes y profesionales de la Psicología – que trabajan o desean trabajar en esta área del trauma y el cambio de comportamientos -, la habilidad para utilizar eficaz y rápidamente intervenciones breves, que puedan poner en practicar incluso en casa. En el taller se estudiará la forma inicial de evaluar, tanto al trauma como a la clientela. Se trabajará la historia del trauma y se profundizará en sus consecuencias y en cómo diseñar las intervenciones breves para hacerles frente. También se centrará en averiguar qué cambios quieren los y las clientes a través de relatos y visualizaciones, utilizando una adaptación de las terapias EMDR y la Solución Enfocada tanto a crear el cambio deseado, como a mantenerlo.
En consecuencia el taller es de particular interés para quienes trabajan con personas que han sufrido cualquier tipo de trauma, o quienes perciben acontecimientos de la vida, experiencias, etc. que les afectan de forma negativa en su día a día. También es útil para las personas que sufren de TOC, sobre todo trastornos del pensamiento.
Los aspectos más útiles de las dos técnicas que se han adaptado junto con otras innovaciones de la autora, guardan relación con la creación de un ambiente seguro, no-traumático, que actúa rápidamente y que además, se puede realizar en casa sin peligro de consecuencias negativas.
Objetivos específicos:
1. Ser capaz de describir e identificar las manifestaciones del trauma.
2. Aprender y describir dos técnicas terapéuticas breves en el tratamiento del trauma
3. Definir una técnica breve terapéutica que puede ser utilizada para fomentar el cambio
4. Destacar el rol del o de la terapeuta durante el tratamiento de los traumatismos
5. Ser capaz de describir, diseñar y establecer metas de la terapia y promover cambios mediante el uso de técnicas de terapia breve.
Métodos: la Desensibilización y Reprocesamiento por Movimientos Oculares (EMDR), es un método complejo e integrador de la psicoterapia individual, mediante el que se guía al cliente utilizando un procedimiento para acceder a sus experiencias y resolver sus problemas conductuales y emocionales. El EMDR utiliza elementos de múltiples orientaciones psicoterapéuticas tanto psicodinámicas, como cognitivo- conductuales, enfoques centrados en el cliente, gestalt y bioenergéticos.
La premisa subyacente de EMDR es que las experiencias de pánico y ansiedad se procesan de forma diferente por el cerebro que las experiencias habituales. La teoría subyacente es que durante el estrés, la memoria grava en una parte del cerebro responsable de las emociones de modulación (la amígdala) y se cierra temporalmente otra parte del cerebro (el hipocampo), responsable de procesamiento de la memoria normal. La experiencia traumática queda atrapada en el exterior y potencialmente no forma parte del procesamiento normal del cerebro, y el EMDR permite a la persona acceder a la experiencia y transformarla en memoria declarativa en el hipocampo. Con el método EMDR, el hipocampo se puede abrir a las emociones evocadas por la experiencia para que el/la cliente pueda soportarlas mientras se realiza el tratamiento. La distracción y la atención a la estimulación bilateral, desempeñan un importante papel que ayuda al cliente a experimentar las emociones como tolerables. Aunque cómo la distracción bilateral en concreto, facilita el procesamiento de las experiencias dolorosas, sea algo que todavía no se termina de entender.
Por otra parte, las Intervenciones Breves de Terapias enfocadas al Cliente se centran en las excepciones del problema, pensando que a continuación se desarrollará un cambio natural en el comportamiento. Es una especie de visión orientada no en las formas tradicionales, sino hacia el futuro, sin profundizar demasiado en la “patología” sino más bien centrándose en lo que el sistema puede hacer para adaptarse a ella, puesto que ambos pueden decidir si esa “patología” es un problema o no lo es.
Las Intervenciones de Terapia Breve enfocadas al Cliente se utilizan para resolver una variedad de problemas de comportamientos y actitudes, mediante el uso de los propios recursos de los y las clientes y las observaciones de las estrategias que utilizan para alcanzar los resultados deseados, en sus situaciones vitales habituales. Se trata de una buena técnica para establecer y mantener un contexto de cambio en el que los pequeños, pero útiles cambios, se anticipan y se buscan.
En definitiva, la combinación de ambas técnicas con algunas variaciones desarrolladas por la Dra. Barreda-Hanson, han demostrado ser una herramienta poderosa para mejorar la respuesta al tratamiento en un período de tiempo más corto, teniendo también la ventaja de permitir practicar los ejercicios en casa.
Aplicaciones: la aplicación habitual del EMDR ha sido el tratamiento de trastornos emocionales relacionados con eventos muy perturbadores o traumáticos. Pero también se usa para trabajar síntomas preocupantes como la ansiedad, la depresión, la culpa y la ira. E igualmente, se puede utilizar para mejorar recursos emocionales tales como la confianza y la autoestima.
Procedimientos:
- El taller se impartirá en español y el alumnado recibirá amplios folletos complementarios.
- Se realizará en una única jornada, en sesión de mañana para teoría y de tarde para prácticas, trabajando cada modelo por separado.
- Se espera que quienes asistan lleven una cuestión-problema sobre la que trabajar utilizando las diversas técnicas, pues aunque se utilizarán múltiples ej. de casos reales, se alentará a quienes participen a traer sus propias experiencias e ideas para debatir y trabajar sobre ellas.
The subsequent stress after trauma represents a dysfunctional disorder - internal and external - that is manifested in alterations in cognitive recognition and behavior, besides being associated somatic symptoms outcrop unconscious problems and anxieties. As one of the characteristics of post-traumatic stress, is the loss of balance between internal and external world of the sufferer. Therefore, given the complexity of post-traumatic responses, they can be categorized largely into psychopathological disturbances.
Psychological stress arises from a stressful situation "real" external, tangible and reaction to this difficult experience, quite evokes a universal and consistent set of symptoms that cause reactions and responses primitive unconscious fears related to threats to life, which bring out fantasies and impulses incipiently overwhelming. The results are the thoughts that lead to dysfunctional responses and maladaptive behaviors.
Objectives: The workshop is designed to provide students and psychology professionals - who work or want to work in this area of trauma and behavior change - the ability to quickly and efficiently use brief interventions, which may put in practice even in house. The workshop will explore how to evaluate initial both trauma as to clients. It will work history of trauma and its consequences will deepen and how design brief interventions to address them. It will also focus on finding out what changes customers want and through stories and views, using an adaptation of EMDR therapy and Solution Focused both to create the desired change, and to keep it.
Thus the workshop is of particular interest to those working with people who have suffered any kind of trauma, or who perceive life events, experiences, etc.. that negatively affect them in their day to day. Also useful for persons suffering from OCD, especially disorders of thought.
The most useful of the two techniques that have adapted along with other innovations of the author, are related to the creation of a safe, non-traumatic, acting quickly and also can be done at home without fear of consequences negative.

Methods: Desensitization and Reprocessing Eye Movement (EMDR), is a complex and inclusive method of individual psychotherapy, which is guided by the client using a procedure to access their experiences and address their behavioral and emotional problems. The EMDR uses multiple elements of both psychodynamic psychotherapeutic approaches as cognitive-behavioral, client-centered approaches, gestalt and bioenergy.
The underlying premise of EMDR is that experiences panic and anxiety are processed differently by the brain than normal experiences. The underlying theory is that during stress, gravel memory part of the brain responsible for emotions modulation (amygdala) and temporarily closes another part of the brain (hippocampus), responsible for normal memory processing. The trapped traumatic experience abroad and potentially not part of the normal brain processing, and EMDR allows people access to the experience and transform it into declarative memory in the hippocampus. With EMDR, the hippocampus can be opened to the emotions evoked by the experience that he / the client is able to bear while performing the treatment. Distraction and attention to bilateral stimulation, play an important role to help the client to experience emotions as tolerable. Although bilateral distraction how specifically facilitates the processing of painful experiences, is something that is not yet fully understood.
Moreover, brief interventions focused Customer Therapies focus on the exceptions of the problem, thinking that then will develop a natural change in behavior. It is a kind of non-oriented view on traditional forms, but to the future, without going too deeply into the "pathology" but rather focus on what the system can do to adapt to it, since both can decide whether this "pathology "is a problem or not.
Brief therapy interventions focused Customer are used to solve a variety of problem behaviors and attitudes, using their own resources and comments from customers and the strategies used to achieve the desired results in their situations normal life. This is a good technique to establish and maintain a context of change in that small but useful changes, anticipate and seek.
In short, the combination of both techniques with some variations developed by Dr. Barreda Hanson, have proved a powerful tool for improving the response to therapy in a shorter period of time, having also the advantage of allowing in practice exercises house.
Applications: the routine application of EMDR has been the treatment of emotional disorders associated with very disturbing or traumatic events. But also used to work worrying symptoms such as anxiety, depression, guilt and anger. And also, can be used to enhance emotional resources such as confidence and self-esteem.

EMDR as a psychotherapeutic approach has been shown to be effective. However, there is a paucity of studies examining the efficacy of EMDR for clients diagnosed with mild mental retardation. The DSM-IV defines mild mental retardation as below average intelligence (IQ between 70 and 55) along with some deficiencies in adaptive functioning skills demonstrated before age 18. The purpose of our clinical project was to explore the applicability of EMDR for our adult clients (n = 12; mean age 22.5) diagnosed with mild mental retardation by adapting the standard protocol. Studies showed that individuals with mental retardation learn and retain information more effectively when materials are presented to them in a visual, concrete, and interactive manner while utilizing the principles of positive reinforcements.
The prevalence of mental health problems and the wide range of clinical symptoms among these individuals have been reported in several studies. Additionally, these adults are misunderstood as being overly limited in their ability to reap any therapeutic benefit from counseling interventions. Prout and Strohmer (1998), for instance, argued that adults with mental retardation do benefit from counseling interventions. However, they stressed the need for more sophisticated or modified use of psychotherapeutic interventions. Psychotherapeutic techniques and models should be modified, if feasible, in regards to language and cognitive levels commensurate with the clients’ background.
Based on our clinical observations, the following are some examples of proposed guidelines to assist the EMDR clinicians in thinking more creatively when adapting the standard protocol.
1) Considerable amount of preparation at the onset of EMDR is necessary, as it plays a pivotal role in ensuring a successful outcome.
2) Visual depiction of the SUDs and VOCs on a scale of 0 through 5, or 0 through 10, depending on the client’s cognitive abilities is beneficial. Use of “faces” to depict concretely various levels of distress should be made.
3) The concepts of PC and NC may be too abstract for some in this population. We assist clients by operationally defining those concepts with the list of commonly used PCs and NCs in simpler language.
4) Coping resources are sometimes limited for these adults. Clients will benefit from having multiple reinforcements of self-soothing skills thorough the installation of the “safe place” and “resources”.
5) Positive reinforcements (e.g., frequent verbal reminders) should be used regularly in sessions throughout treatment. However, be mindful of clients’ desire to please the clinician.
6) Role-playing should be used when feasible throughout treatment, e.g., during the installation of future templates, as it heightens more sensory, affective, and behavioral modes of learning rather than verbal modality alone.
Based on the treatment outcomes reported by our clients, EMDR is an effective treatment option, as evidenced by sustained reduction in their level of distress to traumatic memories. Findings from this clinical project have practice and research implications. First, the standard protocol should be adapted for use with adults with mild mental retardation to achieve optimal gain. Second, empirical research is needed to provide further evidence for the efficacy of EMDR for adults with mild mental retardation.

Clients with complex PTSD and dissociation present many challenges. The neurosciences have helped us to begin to understand and deal with them through a recent clarification of mirror neurons and associated neural structures in both the clinician and client. Concepts from the Boston Change Process Study Group and ego state therapy provide methods of analyzing and intervening in the “microprocesses” that occur in treatment. Hoppenwasser’s concept of “dissociative attunement” challenges thinking about the “multiple self states” both clinician and client operate from. Her ideas push us to rethink current conceptualizations of relatedness. Participants will learn how to deal with ruptures in positive empathy that may result in the history taking, assessment, and desensitization phases. In the preparation phase, participants learn to use the therapeutic relationship as an additional resource for containment. Concepts of dyadic regulation of affect, now moments and moments of meeting will be taught to deal with ruptures to the therapeutic relationship throughout treatment. Dealing productively with countertransference ruptures poses additional challenges. Participants will learn a strategy called the “relational interweave”. Its function is to restore EMDR processing when an interpersonal “event” has temporarily derailed the work. A practicum using Dworkin’s Clinician Self Awareness Questionnaire will be held in the afternoon part of the workshop to enhance learning this strategy.

Although several efficacious treatments for PTSD exist, these treatments are currently underutilized in clinical practice. To address this issue, research must better identify barriers to dissemination of these treatments. This study investigated patient preferences for PTSD treatment given a wide range of treatment options in an analog sample. 160 individuals, with varying degrees of trauma history, were asked to imagine themselves undergoing a trauma, developing PTSD, and seeking treatment. Participants evaluated 7 different treatment descriptions, which depicted treatment options that they might encounter in a clinical setting. Participants rated their most and least preferred treatments along with their personal reactions to and the perceived credibility of each treatment. Participants also completed a critical thinking skills questionnaire. Participants predominantly chose exposure or another variant of cognitive-behavioral therapy as their most preferred therapy, and those who chose exclusively empirically supported treatments evidenced higher critical thinking skills. The present study contributes to a growing literature indicating that patients may be more interested in these therapies than indicated by utilization rates. The problem of underutilization of empirically supported treatments for PTSD in clinical practice may be due to therapist factors. [Author Abstract]

1. Introduzione.
Il pensiero controfattuale è un processo che esprime la capacità di riflettere e
modificare gli scenari di situazioni da cui sarebbero potuti conseguire esiti differenti
dalla realtà. Nella quotidianità è comune che un sentimento di rammarico – il regret -
venga provato a seguito di una discrepanza tra i risultati attesi e la realtà.
La ricerca e l’esperienza condivisa dicono che, rilevare le differenze tra ciò che
avremmo voluto e ciò che abbiamo ottenuto, ci aiuta a pianificare azioni più efficaci nel
futuro. Tuttavia, in condizioni di elevato stress, come quello provato dopo un evento
indesiderato e traumatico, possono fare irruzione nella coscienza pensieri nella forma
“Se solo non fossi stato così…”, oppure “Se solo non avessi fatto” o “Se avessi fatto
qualcosa per evitare tutto ciò”; nel tentativo di annullare (undoing) idealmente le
conseguenze dell’accaduto. Questi pensieri controfattuali possono sortire l’effetto di
amplificare emozioni e sentimenti come il biasimo, la rabbia, la vergogna e il senso di
colpa. L’attitudine alla generazione automatica, indiscriminata e non finalizzata
all’azione, di domande e affermazioni ricorsive, può condensarsi in un particolare stile
di pensiero – la ruminazione – che sottende sensazioni di disagio diffuse e invalidanti,
come gli stati depressivi, ansiosi ed ossessivi.
La prima parte di questo lavoro è occupato da una rassegna sulle ricerche che,
inizialmente, pongono il pensiero controfattuale nel quadro delle più generali abilità di
problem-solving; successivamente, la prospettiva funzionale, riesce a farne risaltare il
versante adattivo, rilevante ai fini della regolazione degli stati affettivi e, più in
generale, del mantenimento della salute mentale.
Nella seconda parte viene discusso il ruolo del pensiero controfattuale, in relazione al
suo versante disfunzionale , con particolare riguardo agli esiti post-traumatici.
Nella terza parte vengono analizzati i modelli di elaborazione delle informazioni che
riconoscono un ruolo al pensiero controfattuale nella regolazione dell’umore, citando
alcuni paradigmi psicoterapeutici, tra cui l’Eye Movement Desensitization and
Reprocessing (EMDR), ideato da Francine Shapiro.
Per verificare l’applicazione del pensiero controfattuale nel trattamento degli esiti
post-traumatici sono stati riportati due esempi: uno tratto dalla letteratura sull’EMDR e
un caso di disturbo ansioso-depressivo, in trattamento con psicoterapia a orientamento
cognitivo.
Application of counterfactual thinking in a post-traumatic results. Introduction. The counterfactual thinking is a process that expresses the ability to reflect and change scenarios of situations that could have been large gains different from reality. In everyday life it is common that a feeling of regret - the regret - should be tried as a result of a discrepancy between expected results and reality. The research says that shared experience, point out the differences between what we wanted and what we have achieved, helps us to plan more effective actions in the future. However, under conditions of high stress, such as that experienced after a traumatic event and unwanted, they can break into the conscious thoughts in the form "If only I had not been so ..." or "If only I had not done" or "If I had done something to avoid anything "in an attempt to cancel (undoing) the ideal of what the consequences. These counterfactual thoughts may have the effect of amplifying emotions and feelings such as blame, anger, shame and guilt. The ability to generate automatic, indiscriminate and not action-oriented questions and statements recursive, it can condense into a particular style of thinking - rumination - that underlies feelings of discomfort common and disabling, such as depression, anxiety and obsessive . The first part of this work is occupied by a review of the research that initially pose the counterfactual thinking in the context of more general skills of problem-solving, since then the functional perspective, can bring out the side adaptive, relevant to the regulation of affective states and, more generally, of maintaining mental health. The second part discussed the role of counterfactual thinking, in relation to its dysfunctional side, especially with regard to post-traumatic results. In the third part analyzes the patterns of information processing that recognize a role in regulating mood counterfactual thinking, citing some psychotherapeutic paradigms, including eye movement desensitization and reprocessing (EMDR), developed by Francine Shapiro. To test the application of counterfactual thinking in the treatment of post-traumatic results were two examples: one taken from the literature on EMDR and a case of anxiety-depressive disorder, treatment with cognitive-oriented psychotherapy.

resulted in a form of psychotherapy in which the major goal was to make psychotherapy more efficient and more effective. It evolved
out of research project on communication begun by anthropologist Gregory Bateson that soon involved the work of hypnotherapist
Milton H. Erickson and psychiatrist Don Jackson. John Weakland, Jay Haley, Paul Watzlawick and Richard Fisch began to publish
the ideas that resulted fiom the early research findings and in doing so developed a particular set of assumptions about the formation
and resolution of human problems that differed significantly from traditional treatment models of the time. Further refinements
through the clinical application of these methods resulted in a model of treatment that was a pioneer of the brief psychotherapy
movement. It is based on a non-normative and non-pathological way of viewing people with problems; it looks at people in the
context of their living situations; it resists the idea of client resistance, it places great emphasis on the use of language; and it seeks
to amplify client assets and resources and minimize client liabilities and shortcomings.
Brief therapists assume a willingness to be an active change agent for the benefit of their clients. They accept responsibility for
creating an atmosphere of respect, patience, and creativity in which clients can find alternative ways to think and behave. They
believe they have a set of tasks to perform that will hopefully result in the resolution or, as a minimum, the diminishment of the
problem situation for which the client originally sought help.
These tasks consist of a combination of ways of thinking and acting that are designed to increase the likelihood that the client will
experience relief from a painful problem. One of the main tasks for a brief therapist is to find ways to construe the problems
presented by the client so that a solution can be found. Brief therapists inquire into the interactional systemic aspects of a problem,
the context or environment in which the problem occurs, the people involved in the problematic situation, and the ways the client has
attempted to resolve the problem thus far.
Another very important task is to identify and gain access to the persons who are the most interested in and willing to work toward
changing the problem situation. The idea here is to spend the bulk of the therapeutic time and effort working with the person who is
most invested in the change process. Brief therapists find ways to appeal to this person's values and belief systems so that (s)he will
engage in activities and/or alter her/his behavior in ways that are likely to change the problem situation.
A third task on which brief therapists concentrate is the establishment of clear, concrete, and doable goals of treatment. They
collaborate with the client to determine what the client hopes to gain from treatment and when the client will know she is ready to
handle life on his/her own, this assumes an emphasis on the client's present and the possibilities for the client's future rather than
his/her past.
The fourth task brief therapists focus on is the development of ways of intervening in the way the presenting problem is being
handled in the present time. This is based on the central assumption that one of the main goals of psychotherapy is to induce
clients to change the way a problem is handled. Such intervening is the result of thoughtful and careful consideration of many factors
surrounding the problem situation and involves the use of a variety of skills.
A final task for the brief therapist is to find ways to remove him/herself from the client's life in such a way that the client has faith in
her/his own ability to function effectively without the therapist.
This treatment model offers clinicians an opportunity to work in positive, goal-directed ways that clients find helpful and therapists
find challenging and satisfying. It calls upon clinicians to develop keen observation skills, the ability to see things fiom a variety of
perspectives, and an appreciation for the vast resources clients bring with them to therapy. While it is a simple model of treatment, it
is by no means an easy one to master. It requires clinicians to step outside their usual frames of reference in the pursuit of creative
solutions to difficult human problems. It rewards them with a greater sense of accomplishment and increased client satisfaction.
In the ever-changing world of mental health, this is no small achievement.

Suzanne is 30 jaar als ze zich aanmeldt voor behandeling van al lang bestaande en zich steeds verder uitbreidende angst en onzekerheid in het contactmet mensen. Het is niet de eerste keer dat zij behandeling zoekt. Een aantal jaren eerder werd zij behandeld met een min of meer geprotocolleerde cognitieve gedragstherapie binnen een Riagg. De resultaten vielen echter tegen en hoewel zij een aantal zinvolle gedragsveranderingen doorvoerde en zich had getraind in een meer realistische manier van denken, staakte zij teleurgesteld de behandeling. Niet in het minst omdat de behandelaar geen mogelijkheden zag de behandeling aan te passen aan het specifieke van Suzannes klachten. Nu zoekt Suzanne opnieuw hulp. Ze heeft via-via gehoord dat EMDR – in combinatie met cognitieve gedragstherapie – vaak goede resultaten geeft bij (sociale) angst.
Suzanne is 30 years when they register for treatment of longstanding and ever expanding contactmet fear and insecurity in people. It is not the first time they seeking treatment. A few years earlier she was treated with a more or less within a manualized cognitive behavioral Mental Health. The results were, however, opposed and even though they had implemented a number of meaningful changes in behavior and had trained in a more realistic way of thinking, disappointed they stopped the treatment. Not least because the therapist saw no possibility of treatment to suit the specific complaints of Suzanne. Now looking for help again Suzanne. She has heard through-via EMDR - in combination with cognitive behavioral therapy - often with good results (social) anxiety.

De patiënt is een ernstig getraumatiseerde man, die na een periode van stabilisatie zijn traumatische ervaringen op papier tekende. De tekeningen zijn in het begin gebruikt bij de ordening en bij bepaling van de werkvolgorde van de EMDR- behandeling. Tijdens de behandeling kon hij zelf goed aangeven wat hem hielp en wat niet. Na een forse terugval was hij in staat om te analyseren waardoor dit kwam en wat er voor nodig was om hier weer uit te komen. Deelnemers krijgen mee wat de do’s en don’ts zijn vanuit patiënt perspectief. Het belang van het nadenken over de therapeutische houding wordt gestimuleerd. De mogelijke angst om blunders te maken is hierna verminderd.
"You need not type during the EMDR" Content Presentation: The treatment of complex PTSD goes with ups and downs. Satisfied with a success achieved can be varied with a period of despair. It is important that the treatment the therapist still keeps a helicopter view. Questions that the therapist himself, among other states are: where we are in the process, I as a therapist to energetically or I take too little risk. The patient may be encouraged where possible from a distance to his own treatment to look and learn to analyze: how do I get a relapse or how is it going with me? The responsibility and control are of course with the therapist. How does the patient subsequently returned to his treatment and the different phases in this? What has helped him in difficult times? What behavior of the therapist has really helped him and what was just annoying (see title)? To what extent humor was helpful? How does the patient back on the degree of involvement. In this presentation, using video footage and an interview with a patient look back on the therapy process.
The patient is a severely traumatized man, who after a period of stabilization are traumatic experiences on paper signed. The drawings are in the beginning when used in the arrangement, and determining the operating sequence of the EMDR-treatment. During treatment, he could well indicate what helped him and what not. After a sharp decline, he was able to analyze and so this was what it took to come here again. Participants will take what the do's and don'ts are from patient perspective. The importance of thinking about the therapeutic attitude is encouraged. The possible fear of making mistakes is reduced below.

questions. There are two research studies: a study

By comparing efficacious psychological therapies of different kinds, inferences about common effective treatment mechanisms can be made. We selected six therapies for review on the basis of the diversity of their theoretical rationales and evidence for superior efficacy: psychoanalytic psychotherapy, hypercapnic breathing training, hypocapnic breathing training, reprocessing with and without eye-movement desensitization, muscle relaxation, and cognitive behavior therapy. The likely common element of all these therapies is that they reduce the immediate expectancy of a panic attack, disrupting the vicious circle of fearing fear. Modifying expectation is usually regarded as a placebo mechanism in psychotherapy, but may be a specific treatment mechanism for panic. The fact that this is seldom the rationale communicated to the patient creates a moral dilemma: Is it ethical for therapists to mislead patients to help them? Pragmatic justification of a successful practice is a way out of this dilemma. Therapies should be evaluated that deal with expectations directly by promoting positive thinking or by fostering non-expectancy.

A few years ago, my friend Ally, who had a somewhat unwarranted confidence in my knowledge of Things Psychological, asked if I knew anything about EMDR; she had suffered some severe childhood trauma, and was thinking of trying it. "EMDR?" I asked, "Is that a new street drug?" I guess that showed her what I knew. She explained that her therapist had suggested this relatively new technique, Eye Movement Desensitization and Reprocessing, that had been shown to ease symptoms of Post Traumatic Stress Disorder (PTSD) in soldiers and rape victims. From what she understood, the therapist would help her to focus on her devastating memories of childhood abuse while directing her eyes to twitch rapidly from side to side. This, according to her shrink, would help her to better "process" her memories. "Sounds like hypnotism," I expertly analyzed. "Who knows, maybe it will work." More recently, when I wrote here about the neurobiological advantages of emotionally "finding a safe place," several readers also wrote in to ask me about EMDR. Now I understand why people want an expert opinion (and I'm no expert, by the way): a heap of controversy surrounds this popular technique.

A recent meta-analysis of PTSD treatments (van Etten & Taylor,
1998) found EMDR to be as efficacious as behavioral and drug
treatments. There is considerable controversy, but little research, on
the underlying mechanisms of EMDR. A conditioning model by
Dyck (1993) suggests that eye movements (EM) effect a distraction
from trauma related thoughts, causing an extinction trial.
According to this model, the distraction of EM should cause
thoughts to be directed outward. A psychodynamic model by Allen
and Lewis (1996) suggests that EM facilitate the formation of new
associations to traumatic memories and allow the client to “remain
in the present while thinking of the past.” This model postulates
that EM cause thoughts to be directed inward. We report two
experiments in which thought processes were studied using a
stream of consciousness (SOC) technique (Singer, 1993). In both
studies, undergraduate participants wrote down a sad (or happy)
target event from their life. They then thought about the target
event and let their thoughts go where they may for 10 minutes. At
approximately 1-minute intervals they were asked to report their
thoughts. The baseline study (n = 42) looked at SOC with eyes
closed; the second study (n = 27) compared SOC with eyes open,
eyes closed, and with EM. Relative the to the eyes-open condition,
EM tended to keep the SOC internally focused. During the last 4
minutes of the SOC, eyes open participants were externally
focused (thoughts about the surroundings) about 50% of time;
EM participants were externally focused 25% of the time; and eyes
closed participants were externally focused 3% of the time, F(1,
11) = 6.08, p = .017. Eye movements produced a blend of external
(eyes open) and internal (eyes closed) thoughts, offering support
to the psychodynamic model.

The purpose of this replication study was either to support or refute the original Eye Movement Desensitization and Reprocessing research conducted by Shapiro. The present study was amended with two additional indices to assess anxiety and social functioning.14 subjects suffering long standing (one or more years) traumatic memory symptomatology, concerning rape, physical abuse, incest, and childhood sexual molestation, were randomly assigned to one of two treatment conditions. Traumatic memories were pivotal to presenting symptoms, which included panic attacks, self-blaming/guilt, intrusive thoughts, anxiety, nightmares, insomnia and avoidant thinking/behavior. All subjects were diagnosed with PTSD, by an independent licensed clinical psychologist. There were 13 females and 1 male. The male subject was in the Control Group. Age range was from 25 to 49 years with a Mean age of 38.64 years. Range for age of traumatic event was five to 19 years of age, with a Mean age of 10.14 years. Range for duration of the subjects' symptoms since traumatic event was 18 to 44 years with a Mean age of 28.5 years. Dependent variables were (1) anxiety level, (2) validity of a positive self-statement/assessment of the traumatic incident, (3) primary presenting symptom and (4) social adjustment. Measures utilized were the Subjective Units of Disturbance Scale (SUDS), the Validity of Cognition (VoC) self evaluation, primary presenting symptom self report, the Impact of Event Scale (IES), and the Social Adjustment-Self Report (SAS-SR). Initial measures demonstrated that all subjects were essentially the same prior to any treatment. Measures were obtained at the initial session and at 1- and 3-month follow-up sessions. Where applicable the analyses conducted paralleled those used in the original research. The results of the study indicated that a single session of EMDR successfully desensitized the subjects' traumatic memory, significantly mediated their cognitive assessment of the situation, as well as their social adjustment. Treatment effects were maintained over the period of the study for all subjects. These findings support the original conclusions in Shapiro's seminal study of the Eye Movement Desensitization and Reprocessing procedure. Psychotic phenomena are one of the most
challenging clinical presentations to treat, the most severe form being schizophrenia and
poor outcome schizoaffective disorder. Schizophrenia is a severe enduring mental illness
(SMI) and is characterised by core disturbances of thinking, perceptions and the emotions.
It is accepted as having a heavy burden, with significant effects on the patient, their
families and carers and it is one of the most costly illnesses worldwide. The weight of
burden falls with regard to legal problems, stigma and life expectancy: life expectancy in
this group of individuals is reduced by 10 years, mostly as a consequence of suicide
(Rossler, Salize et al. 2005). Despite more than 100 years of experience of the disease it is
still only a minority of individuals who make a full recovery.
This workshop aims to give further insight into the role of EMDR in the treatment of
schizophrenia - as defined by the genetic epidemiological work by Professor K Kendler
(Kendler, Spitzer et al. 1989; Kendler, McGuire et al. 1993; Kendler, Maguire et al. 1993).
It follows up a series of three patients with psychosis treated with EMDR one of whom met
strict criteria for schizophrenia. All 3 are now over 2 years post-EMDR. Of specific note the
man who presented with schizophrenia remains free of medication and symptoms.
The workshop will discuss the role of EMDR in the treatment of psychosis, including
schizophrenia and discuss case selection and protocol development in this area of practice.

Depending upon a client’s early life experiences some ego states can be malevolent, wanting
bad things for the client such as willing them to suffer in some way.
It is necessary for clients to remain safe during EMDR sessions and contained between
sessions. There is a need, therefore, to learn techniques to work with more difficult clients
so they too can benefit from EMDR. It is also helpful to know how to deal with blocked
processing due to the interference of an ego-state.
This workshop provides an overview of dissociation and a review of models to explain it.
There will be an explanation of Ego State Therapy (EST) and how it fits with EMDR. Clinical
case material is used to illustrate learning points. Through demonstration and practice
participants will learn how to access ego states in a controlled way and effect therapeutic
change. In the latter part of the day, participants will see a video of a live case where EST is
used effectively to moderate the malevolence displayed by two difficult ego states.

During this pre-conference, we will discuss the integration of EMDR and family therapy through the work done with domestic violence cases. In these particular situations, the therapist is faced with complex clinical issues that require one to think and intervene at different levels: the victim, the children's victim, the perpetrator, the family dynamics. This complex clinical situation is challenging us to develop a multilevel way of thinking and working; it is a good example of integration between personal and interrelation issues. Such an integration proves to be a necessary process when faced with the trauma consequences of domestic violence, which we all know impact each family member greatly. The information processing model has been described in many different ways and is potentially the underlying
process that describes the psychotherapy change process, regardless of orientation. In this poster I will briefly
compare Eye Movement Desensitisation and Reprocessing and Meta-Cognitive Therapy in light of this information
processing model and conclude that although they may use different language to describe their theories, a
unifying information processing model describes both paradigms. Human beings are meaning making machines
(Robbins, 2009). We give meaning to the world, the deeds we do and the interactions we have, as well as the
more fundamental senses we experience (Frankl, 2004). Information arises either from external stimulus or
internal process’s and this information goes through our filters where we delete, distort and generalise this
information based on the rules created in our information processing unit (Fig 1). This information processing
mechanism gives meaning to this filtered information and generates a response, this response interacts with this
information and the cycle then repeats in a self regulatory manner. This process is repeated for everything we do;
we are and continues until we die. Mental health issues arise when this self regulatory system fails to make a
useful functional meaning of the information. Eye Movement Desensitisation and Reprocessing (EMDR) was
developed by Francine Shapiro (1989) for the treatment of trauma memories and associated emotions, changing
the meaning the person gives to their experiences. Metacognitive Therapy (MCT) was developed by Wells and
Matthews (1984) as a way of defining the processes that underpin a range of psychological disorders.
One of the features of psychological disorders such as anxiety or depression is that thinking becomes difficult to
control and biased in particular ways that lead to a worsening and maintenance of emotional suffering. Many
clients report that they feel that they have ‘lost control’ over their thoughts and behaviours. Another important
feature is that the persons thinking and attention becomes fixed in patterns of brooding and dwelling on the self
and threatening information. Both EMDR and MCT describe an information management/processing system that
organises and carries out various tasks such as how to open a door, how to get dressed, how to be anxious, how to
respond to a trauma memory Both therapies appear to change this system / process to something that is much
more adaptive for the client, as if they change the master information management control programme that runs
this system, in the same way a conductor influences the orchestra and the music they produce. Both models
describe the same process, although they utilise different language to describe this change process.
Perhaps a change of focus for the development of pychotherapy towards more process change models rather than
content change interventions.

In this chapter, EMDR is examined and considered from a different vantage point. For some time, I too have been engaged in developing an integrative approach to therapy. However, apropos the previous paragraph, my integrative efforts have a different flavor; they are rooted most deeply in the psychodynamic tradition, not the cognitive- behavioral.As a consequence of my psychodynamic vantage point, my view and use of EMDR differ somewhat from the "classic" approach to EMDR. This chapter is thus likely to reflect clinical and theoretical views that differ in important ways from those of many of this book's other authors. However, my discussion does not constitute a critique of EMDR. I began using EMDR and am writing this chapter because I am intrigued by EMDR -- by its clinical potential, the experiences it seems to generate, and the challenges to theory and research that it presents. My goal is to expand our ways of viewing and thinking about EMDR, and to consider the ways in which EMDR can be enriched by a psychoanalytic perspective and vice versa. For an integration or combination of EMDR and psychoanalysis to be viable, two conditions must be satisfied. The differences must not be so fundamental and unbridgeable that any effort to bring the two approaches together is inherently incoherent and contradictory. At the same time, the differences must not be so trivial or superficial that putting them together adds little or nothing. This presentation will present and demonstrate my clinical observations, that clients while talking or working on their issues will sometime lean their heads toward the right or the left shoulder. People with their heads to the left will tend to report thoughts, while those with their heads to the right will tend to report images and emotions. In therapy, utilizing EMDR when clients have leaned their heads to one side, I have tended to reinforce the side they put their head to, utilizing eye movements in a diagonal direction, corresponding to the direction the head was learning. That is. If the person learned their head to the left, I would move my hand from their upper left side to their lower right side, and opposite if their heads leaned to the right. The outcome of these diagonal eye movements was that it tended to evoke cognitions when the head leaned to the left or images when the head learned to the right. Client who come to therapy requesting help concerning fears, tend to lean their heads to the rights, corresponding with imagery and imagination of the right hemisphere and may lack the cognitive, logical thinking skills (left hemisphere) to counter their fears. With these clients, when the SUDS do not drop significantly, I will tend to do body-cognitive interweave. I request that the client lean their head on the left and work over and over with cirrsponding sets of diangonal eye movmenets. The left hemisphere of the brain is thus stimulated and logical thinking (PC) is enhanced. This helps to counter and balance out the negative images, and the fear decreased.
Through care histories and examples, I would like to accomplish the following objectives: 1) to make the participants more aware of body language and it its significance in therapy; 2) to examine the differences between the right and left hemispheres of the brain and their relationship to therapy with EMDR; and 3) to introduce and demonstrate a body-cognitive interweave in EMDR therapy with client s who request help with fears.

For a detailed presentation of a case of burnout, we will discover together how an integrated psychodynamic and EMDR enhances the understanding and treatment of the client and the therapist's work. Paterson, New Jersey, a
socioeconomically impoverished inner
city. The family was a grandmother and
four young grandchildren who had
recently been placed in her custody.
Their mother and little sister had been
brutally slaughtered with a knife by their
father while they were home. The four
surviving children were suffering from
nightmares about the incident, grief and
loss issues, and fears that their father
would get out of jail and come after them.
One of the children, a five-year-old girl,
had developed elective mutism. She had
narrowly escaped the same fate as her
mother and sister. The little girl had been
an eyewitness to the murders and as she
fled from her father, she slipped on her
mother’s blood and was nearly caught
by him. As I listened and learned at Level
I, I wished that I’d known EMDR when
I was working in Paterson.

Although eye movement desensitization and reprocessing (EMDR) has demonstrated efficacy in treating chronic posttraumatic stress disorder and old trauma memories, EMDR treatment of recent traumatic events has not received adequate attention from EMDR researchers or clinicians. This article presents current thinking and findings about early psychological intervention following recent traumatic events and examines the status of early EMDR intervention (EEI) concepts and research. It is contended that this area has not developed sufficient awareness and definition among EMDR clinicians. Francine Shapiro's theoretical adaptive information-processing model predicts that dysfunctionally stored trauma memories underlie many current psychological disorders. Consequently, the assumption that memories of a recent traumatic event and its sequelae are not fully consolidated offers a unique role for EEI not only in reducing acute distress but also in preventing the sensitization and accumulation of trauma memories. A call is made for a more comprehensive approach to the field of EEI to promote interest and awareness among EMDR practitioners and to generate research.

This presentation will outline the utilisation of EMDR with deaf clients exploring the
challenges it presents for clinicians whilst exploring the potential for EMDR. Through the use of case
examples this session will outline the ways in which EMDR needs to be adapted for this population
Many clinicians struggle with Cognitive Behavioural Therapy (CBT) with d/Deaf clients due to
some of the abstract ideas used and the heavy reliance on spoken language. Many people, both
hearing and deaf find it difficult to think about thinking, and develop levels of insight into their
emotions and behaviours simply by talking about them. Deaf people experience trauma and abuse
at a much higher rate than hearing people, but there are few established therapies to help them.
As Deaf people do not pick up all of the auditory stimuli it is likely that the trauma or difficult
experiences that bring them to therapy may have been stored visually or kinaesthetically rather than
linguistically. Trauma may also be caused by smaller events due to lack of information available to
them. EMDR is an evidence based therapy that attempts to resolve emotional reactions to traumatic
memories and their triggers. It does this through a combination of physical stimulation and the
recollection and discussion of memories. As such, it seems ideally suited for use with people who are
Deaf and have experienced trauma in their past. I describe how EMDR lends itself for successful use

EMDR is an innovative and rapidly expanding new, treatment technique. Therapists are provided with two weekends of intensive
training and encouraged to obtain supervision and be active in peer consultation groups and networks, and work to refine their skills.
It is uncertain what percentage follow through in this regard.
Estimates of the numbers of people who are victims of sexual trauma in our society are staggering (one in 3 girls, one in seven boys).
Research finds that approximately half the people who present for psychotherapy have some sexual trauma in their history. EMDR
has been hailed as an important new technique in facilitating the treatment of those who have been sexually traumatized. With
correct usage, EMDR can greatly facilitate the treatment. At the same time, sexual trauma is a complex and volatile issue and
awkward, poorly timed, or overly simplistic usage of EMDR could potentially lead to adverse consequences for the patient and
treatment. As with any therapeutic technique or paradigm it can only be as helpful as that of the overall treatment. In addition the
mere usage of EMDR will have an impact on the therapy, as well as the therapeutic container, and therapists need to be cognizant of
what that impact will be as well as to be sure that they know how to incorporate the patient reactions into the treatment in a positive
way and not allow them to undermine, or otherwise detrimentally effect both patient and therapist primary therapy goals.
Particularly in these times of increased litigation, malpractice suits, and professional grievances against therapists those
professionals utilizing EMDR would be wise to be aware of the particular risks inherent in the patient population in which they
work, as well as the inherent vulnerability of utilizing a newly developed technique. In treating sexual trauma many experts agree
that the crucible of the therapeutic container- is whether the healing will occur if the therapist sucessfully deals with the
challenges the patient will offer lip. Again how those utilizing EMDR negotiates those challenges may be he difference between a
successful course of therapy and a disaster.
Finally, working with sexual trauma is an emotionally, intellectually, and sometimes physically demanding undertaking. Many
therapists do not fully realize or acknowledge the toll that this type of work exacts and may be blind to the countertransferentia1
responses which arise and how they are communicated to the patient.
In this workshop we will first reveal, some of the current thinking on the primary treatment issues (and obstacles) in therapy with
victims of sexual trauma. We will then examine how and when is the most propitious time to use EMDR with this population and
what reactions patients are likely to have to this type of intervention. Specific ways that EMDR and its implementation may activate
certain issues in sexual trauma victims be elucidated as well as strategies for addressing those issues. Finally participants will
engage in a series of experiential exercises designed to heighten their awareness of their own personal reactions and feelings (i.e.,
countertransference) to working with the intensity of sexual trauma. Once again these potential reactions will be linked to more or
less effective usage of EMDR.

8 stationäre Patienten mit chronischen PTSD wurden mit einem Durchschnitt von 4 Sitzungen der Augenbewegung Desensibilisierung und Wiederaufbereitung (EMDR) behandelt, eine neue Behandlungsmethode. Konkordant mit anderen Fallberichten und Studien, 7 der Patienten berichteten eine deutliche Entlastung von 17 schmerzhaften Erinnerungen verarbeitet, das war in der signifikanten Abnahme der Suds (subjektive Einheiten des Unbehagens, einer Skala von 0-10 dargestellt) von durchschnittlich 6,5 bis 0,9 nach der Behandlung mit EMDR (P <0,001). Dies wurde durch einen Rückgang in anderen Symptome und eine Verbesserung der negativen Selbst-bezogene Denken einher. 1 Patient zeigte keine Besserung. In einem Drittel der Erinnerungen verarbeitet werden, einen starken Anstieg der Suds (Rückblende) aufgetreten war und aufbereitet werden. Die positiven therapeutischen Wirkungen waren stabil 3 und 6 Monate nach der Behandlung, in 2 Fällen, berichtet nach einem Jahr zeigten anhaltende positive Ergebnisse. 8 inpatients with chronic PTSD were treated with an average of 4 sessions of eye movement desensitization and reprocessing (EMDR), a new treatment method. Concordant with other case reports and studies, 7 of the patients reported a significant relief of 17 processed painful memories; this was shown in the significant decrease of SUDs (subjective units of discomfort, a 0-10 scale) from an average of 6.5 to 0.9 after treatment with EMDR (P < 0.001). This was paralleled by a decrease in other symptoms and an improvement in negative self-related thinking. 1 patient showed no improvement. In one third of the memories processed, a strong increase in SUDs (flashback) occurred and was reprocessed. The positive therapeutic effects were stable 3 and 6 months after treatment, In 2 cases, reports after a year showed persistent positive results. No negative results of the treatment were reported. It is suggested that EMDR could be a useful instrument in the treatment of patients with chronic PTSD. The procedure fitted well in a psychodynamically oriented inpatient setting. This combination seemed to help specifically in the treatment of trauma patients with impaired ego strength. [Author Summary]

Most sex offenders in treatment in the United States understand and adapt well to the predominantly cognitive/behavioral/relapse prevention (RP) aspects of their treatment. "No more victims" is the fundamental goal of sex offender treatment, and due to this focus on relapse prevention, most sex offenders do not adequately address their own emotional wounding from early trauma and victimization. This author believes these unresolved affective issues lead to the cognitive distortions and justifications that allow sex offenders to give themselves permission to offend in the first place. Eye Movement Desensitization and Reprocessing (EMDR) is a therapeutic intervention that assists in rapidly resolving troubling thoughts, feelings, and physical sensations. Incorporating EMDR into the current cognitive/behavioral/relapse prevention (RP) treatment model for sex offender treatment opens the possibility of reducing the reoffense rate in society. In this study, affective issues of adult male sex offenders (n = 27) were addressed, employing EMDR; this comprised the experimental treatment group. The experimental group received a pretest, three EMDR sessions, and a posttest over an average time of 3.8 months. The Multiphasic Sex Inventory (MSI) was the measure used for this research. The three scales on that test designed to measure for thinking errors were: the Cognitive Distortion and Immaturity (CDI Scale; the Justification (Ju) Scale; and the Treatment Attitudes (TA) Scale. Archived pretest/posttest scores of randomly selected and anonymous adult male sex offenders comprised the control group (n = 27). This group was tested on the MSI and MSI 2 before entering Module 4 and after finishing Module 5, representing 22.5 months of treatment pretest/posttest. All control and experimental group subjects took part in a mandated cognitive/behavioral/relapse prevention (RP) program. The independent two-sample t test was used to compare two means utilizing the rate of change between the experimental and control group. The results of the study indicate a statistically significant reduction in justifications for offender behavior in the experimental group on the Ju scale (p-value = 0.008). This research project evaluated the effectiveness of using Eye Movement Desensitization and Reprocessing (EMDR) techniques on children and adolescents (N = 20) between the ages of 7 to 18, who were experiencing anxiety due to traumatic memories. EMDR therapeutic techniques were administered in one 90-minute therapy session to subjects in an experimental group (N = 10) and after a 30-day waiting period, EMDR was administered to subjects in a delayed treatment group (N = 10) to help substantiate the effectiveness of using EMDR with this population. The results of the study indicated a highly significant reduction in anxiety related to traumatic memories in both an experimental group and a delayed treatment group, as measured by the Subjective Units of Distress Scale (SUDS), the Impact of Events (IES) scale, and in the delayed treatment group using the Children's Manifest Anxiety Scale (CMAS) after treatment. There was a lack of significance in the reduction of anxiety in the experimental group, as measured by the Children's Manifest Anxiety Scale (CMAS). The results showed a significant difference in cognition, changing from negative to positive thinking, as measured by the Validity of Cognition (VoC) scale. The findings demonstrate that EMDR may help to measure, treat, and enhance the lives of children and adolescents experiencing anxiety from traumatic memories. It is recommended that future research focus on using standardized psychological tests to support the efficacy of using EMDR with children and adolescents. Earlier studies have shown that eye movements during retrieval of disturbing images about past events reduce their vividness and emotionality, which may be due to both tasks competing for working memory resources. This study examined whether eye movements reduce vividness and emotionality of visual distressing images about feared future events: "flashforwards". A non-clinical sample was asked to select two images of feared future events, which were self-rated for vividness and emotionality. These images were retrieved while making eye movements or without a concurrent secondary task, and then vividness and emotionality were rated again. Relative to the no-dual task condition, eye movements while thinking of future-oriented images resulted in decreased ratings of image vividness and emotional intensity. Apparently, eye movements reduce vividness and emotionality of visual images about past and future feared events. This is in line with a working memory account of the beneficial effects of eye movements, which predicts that any task that taxes working memory during retrieval of disturbing mental images will be beneficial.

In the last few years several EMDR clinicians have introduced new approaches which have expanded on EMDR. Two examples are Ricky Greenwald's techniques for 'running a movie' and David Grand's performance enhancement techniques using visualization. GENTLE EMDR is a similar expansion of standard protocol. GENTLE EMDR follows basic EMDR protocol with additional emphasis on the emotions and body sensations connected to the target. Visualilzation is then tuilized to release these emotions and sensations one at a time until the target shows a SUDS of 0. At this point, positive cognitions are installed. The advantage of visualization is that it buffers the emotions with associated with the target, thus reducing abreactions, dissociations and resistance to further EMDR treatment. One advantage of Gentle EMDR is that a client does not need to face the target directly because emotions and body sensations are broken down into management segments. Thus, Gentle EMDR works well with clients who are too fragile to tolerate the standard protocol. One example of the use of visualization would be if the target is "being physically attacked." Clients are first asked what they feel when they think of the attack and where they feel it in their body. Next, clients are asked to visualize a river passing them, while they sit in a protective (safe) place. Finally, clients are asked how the water would appear if it was the identified emotion (such as a particular color) and to let the water flow past until the appearance of the emotional reaction dissipates. Once this is done, clients are asked to name the next emotion that eminates when thinking about the physical attack, allowing the water to 'clear' that emotion as well. When the emotions are approached in this way, the target typically becomes neutralized. The entire process is done using bilateral stimulation. Gentile EMDR is easily taught and has been well received by clinicians at Univerities and Clinical Practices in the United States.

Animal models conceptualize anxiety as a response to potential danger while fear is a response to present danger. The way humans experience anxiety involves our capacity for higher thinking while the human experience of fear appears to be much the same as the animal model. This article examines these differences at both a phenomenological and neurological level and highlights implications for the treatment of conditioned fear in PTSD. The stimuli for human fear are sensory-perceptual, while the stimuli for most forms of anxiety are conceptual-linguistic. Individuals in a state of fear/terror undergo a radical shift from top-down to bottom-up processing in which access to conceptual-linguistic thought processes is severely restricted and the frontal regions of the cortex are no longer able to override impulses from brain stem and midbrain regions. Conditioned fear involves actual neurological changes in the limbic system. The purpose of the current research was to examine the effectiveness of eye movement
desensitization and reprocessing (EMDR) for decreasing of deterministic thinking among
couples. Deterministic thinking is an absolute thought to ignore any probability in incidents
and their inferences. It is one of the most important cognitive distortions to sabotage martial
satisfaction of couples. Twenty couples were involved in this study who to come for
consultation centers from three areas of Tehran (Iran). They were randomly assigned to the
experimental and control groups. The experimental group was exposed to emdr in five weekly

questionnaire of deterministic thinking (younesi &mirafzal 2007) before and after the

This article examines the use of Eye Movement Desensitization and Reprocessing (EMDR) in helping a client address problems with persistent anxiety and a lack of self-esteem. During EMDR treatment, the client explored the dichotomous thinking that had plagued her since childhood, and correspondingly, the role of childhood physical and emotional abuse in her chronic feelings of inadequacy and anxiety. The client experienced significant improvement in her levels of anxiety and problems with self-esteem, both at the end of treatment and at 1-year follow up. Qualitative and quantitative data are utilized in this case study outlining the use of EMDR with a client diagnosed with dysthymic disorder.

Abstract:
EMDR represents an integrative model of psychotherapy at the theoretical level. During its 16-year history, it has created quite a controversy in academic psychology. Missing from these debates have been additional therapeutic elements that are necessary to propel productive thinking into ways of making greater use of the model. These elements—empathy, the intersubjective, and usage of the cognitive interweave in conjunction with transference and countertransference issues—are explored. This addition constitutes an assimilative approach to an ever-evolving model of resolving posttraumatic stress disorder.

This interview with Dr. Francine Shapiro, originator and developer of Eye Movement Desensitization and
Reprocessing (EMDR), provides an overview of the history and evolution of EMDR from its inception
to current findings and utilization, as well as future directions in research and clinical development.
Dr. Shapiro discusses the psychological traditions that informed the development of EMDR and the Adaptive
Information model, as well as the implications for current treatment. The rationale for the application
of EMDR to a wide range of disorders is discussed, as well as its integration with other therapeutic approaches.
Topics include research on the role of eye movements, the use of EMDR with combat veterans,

and Family Therapy Processes (Wiley), and Short-Term Therapy for Long-Term Change. She has written and co-authored more
than 60 articles and chapters and is an invited speaker at psychology conferences all over the world. Dr. Shapiro is a recipient
of the American Psychological Association Division 56 Award for Outstanding Contributions to Practice in Trauma
Psychology, the Distinguished Scientific Achievement in Psychology Award presented by the California Psychological Association
and the International Sigmund Freud Award for Psychotherapy presented by the City of Vienna in conjunction

Background and Objectives: Eye Movement Desensitisation and Reprocessing (EMDR) is now considered evidence based practice in the treatment of trauma symptoms. Yet in a previous meta-analysis, no significant effect was found for the eye movement component. However methodological issues with this study may have resulted in a type II error. The aim of this meta-analysis was to examine current published studies to test whether eye movements significantly affect the processing of distressing memories.
Method: A systematic review of the literature revealed two groups of studies. The first group comprised 15 clinical trials and compared the effects of EMDR therapy with eye movements to those of EMDR without the eye movements. The second group comprised 11 laboratory trials that investigated the effects of eye movements while thinking of a distressing memory versus the same procedure without the eye movements in a non-therapy context. The total number of participants was 849.
Results: The effect size for the additive effect of eye movements in EMDR treatment studies was moderate and significant (Cohen's d = 0.41). For the second group of laboratory studies the effect size was large and significant (d = 0.74). The strongest effect size difference was for vividness measures in the non-therapy studies (d = 0.91). The data indicated that treatment fidelity acted as a moderator variable on the effect of eye movements in the therapy studies.
Conclusions: Results were discussed in terms of current theories that suggest the processes involved in EMDR are different from other exposure based therapies.

In this report we will also summarize the findings of similar treatment modalities on four other sexual offender inmates who have been released from prison only two years ago compaired to the four years of follow up in the case study; each of these four subjects appears to be doing well in living a fully integrated life outside of prison.
It is ous view that the results of this study suggest that an integrated therapeutic approach, in a prison setting, can make a valuable contribution to the solution of some of the problems connected with sexual deviance.

The present research evaluated the psychometric properties of the credibility/expectancy questionnaire, a quick and easy-to-administer scale for measuring treatment expectancy and rationale credibility for use in clinical outcome studies. The results suggested that this questionnaire derives the two predicted factors (cognitively based credibility and relatively more affectively based expectancy) and that these factors are stable across different populations. Furthermore, the questionnaire demonstrated high internal consistency within each factor and good test-retest reliability. The expectancy factor predicted outcome on some measures, whereas the credibility factor was unrelated to outcome. The questionnaire is appended to the paper, yet the authors stress care when utilizing the scale. During the administration of the questionnaire, the participant sees two sections -- one related to thinking and one related to feeling. However, the researcher needs to be aware that the 2 factors derived are not grouped into those questions. Eye Movement Desensitization and Reprocessing therapy (EMDR) is a psychotherapy that uses
rapid eye movements to alleviate traumatic memories. This experiment examined two working
memory hypotheses proposed to explain how performing rapid eye movements can affect the
vividness, emotionality and completeness of traumatic memories. Participants (N=25) recalled
three traumatic memories and rated them on vividness, emotionality and completeness before
and after performing rapid eye movements. Participants also completed six working memory
tasks to see if a correlation existed between working memory and the effect of rapid eye
movements on memory rating variables. Findings illustrate that there was a significant decrease
pre-test to post-test in vividness. Additionally, the factor underlying the reading span operation
task and the Sternberg item order task significantly correlated with the effect of rapid eye
movements for all memory ratings. The results of the current study support the central executive
hypothesis explanation more than the visuospatial sketchpad storage hypothesis for EMDR.

Rapid Eye Movement Effects on Traumatic Memories: A Test of the Working Memory
Hypothesis
In 1987, Francis Shapiro discovered that performing horizontal eye saccades while
holding a traumatic event in mind helped her alleviate the negative symptoms she experienced
from that memory. She developed this intuition into a psychotherapy that is called Eye
Movement Desensitization and Reprocessing (EMDR). This therapy is now a widely-used
technique to treat victims of trauma, people suffering from post-traumatic stress disorder
(PTSD), and people suffering from phobias and other anxiety disorders (Muris & Meckleberger,
1999). Shapiro (2001) describes EMDR as an eight-phase treatment method that includes history
taking, client preparation, assessment, desensitization, installation, body scan, closure and reevaluation.
An important, and distinguishing, component of the EMDR procedure involves the
patient performing rapid bilateral eye movements while thinking about their traumatic memory
and communicating any negative cognition associated with that memory. The horizontal saccadic
eye movements generally involve watching the therapist’s quickly moving finger for 15-20
seconds/set (Shapiro, 2001). Eye saccade sets continue until the patient begins to report that
negative aspects of the memory are being alleviated, and that positive self-cognitions have
replaced the negative self-cognitions associated with the memory (Shapiro, 2001).

The question of how early to intervene with EMDR in the face of natural and manmade disasters has been an important part of the dialogue of those working in this field. Early EMDR intervention, before consolidation of the memory has taken place, may influence adaptive integration (e.g., process sticking points), promote positive coping (especially if this is not occurring spontaneously), and contribute to the development of resilience. Informed by the work of Francine Shapiro, Roger Solomon, and all of the friends and colleagues in the field who have contributed to the evolution of their thinking and practice and following clinical and empirical experience with early EMDR intervention (EEI) in the wake of the 2006 Lebanon war, the authors have observed that the existing EEI protocols appear to focus on certain aspects or parts of the traumatic episode along an approximate time line continuum following a trauma, in accordance with the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) (APA, 2000). They concluded that the unfinished processing of recent traumatic events may require a broader focus than existing EEI protocols provided. They propose a new protocol called the Recent- Traumatic Episode Protocol (R-TEP), which incorporates and extends the existing EEI protocols by providing a new comprehensive, integrative protocol. The R-TEP thus bridges the gaps left by previous protocols and facilitates a transition from the EMD and RE protocols to the Standard EMDR Protocol. The R-TEP takes the wisdom of the Standard EMDR Protocol (Shapiro, 1995, 2001), and applies it in adapted form for recent events to provide a comprehensive approach to Early EMDR Intervention. After describing the main issues in early EMDR intervention, the authors present the Recent-Traumatic Episode Protocol and the Episode Narrative and Initial Goodle Search Script. Si prosegue con
altri autori della corrente psicoanalitica, come Sandor Ferenczi, il qui il pensiero venne riscoperto recentemente (Bonomi e Borgogno). L’attualità del lavoro di Ferenczi, è riconoscibile nel suo sottolineare l’importanza del trauma per lo sviluppo della psicopatologia e l’importanza della relazione terapeutica con la rivalutazione critica della tecnica psicoanalitica seguita dai suoi originali contributi. La modalità “tecnica” risolutiva di Ferenczi nella forma della “neo-catarsi”, come viene nominata, si avvicina alle terapie attualmente accreditate per il trattamento dei vissuti traumatici, una delle quali è appunto l’EMDR. The author follows the trail from the philosophy of "associational" until the school of
"Dissociation" from Janet, Binet, Charcot. Focus is the theory of mind Freud, understood as the first theory, trauma theory, viewed from the standpoint of the "current" dissociation as well as subsequent removal with the second topic. Continue with other authors of the current psychoanalytic as Sandor Ferenczi, the thinking here was rediscovered recently (Bonomi and Burgundy). The actuality of the work of Ferenczi, is recognizable in its emphasis of the importance of trauma for the development of psychopathology and the importance of therapeutic relationship with the critical re-evaluation of psychoanalytic technique followed by its original contributions. Over the past two decades, professional psychology has witnessed a growing
movement towards the utilization of psychotherapies that have empirical support.
Despite this development, therapies that have not been empirically supported
continue to experience widespread use. Concurrently, a collection of novel
interventions, known as Power/Energy therapies (P/ET’s), has emerged. Although
these therapies are based on questionable theoretical foundations and enjoy little or no
empirical support, their popularity with clinicians appears to be strong and growing.
There is scant research examining individual differences with respect to the practice
habits of professional psychologists. The present study examined whether critical
thinking skills are a factor in psychologists’ choice of therapeutic interventions,
including their use of P/ET’s. As hypothesized, participants who reported using a
number of techniques from Power and Energy therapies scored significantly lower on
a measure of critical thinking skills. Also as hypothesized, individuals who reported
using a number of cognitive-behavioral techniques scored significantly higher on the
measure of critical thinking skills. Implications and suggestions for future research
are discussed.

Shame is often a key component of post-traumatic stress and one that can inhibit processing because the person concerned feels no compassion for the self that was shamed. Shame may lead to despising or hating that self so that allowing the self to grow, to recovered, feels almost impossible. Shame is experienced cognitively, emotionally, and somatically: in “brain, heart, and body.”
Shame may lead to blocked processing that does not respond to cognitive interweaves or other approaches such as changes in speed, modality and direction of bilateral stimulation, or “TICES’ strategies, changing aspects of images, cognitions or emotional and sensory interventions. Typically, Subjective Units of Distress scale scored stick at 4.
This paper will draw on clinical work with people who have experienced rape, torture and organized violence and explore ways of unlocking the inhibiting factors of shame, enabling the victim of personal violence to have compassion for themselves, and forgiveness. With compassion, a person can allow themself to recover, and processing the memories of the traumatic incident or incidents can move to adaptive resolution.
The paper will present case material using the adaptive information processing model as a helpful way of enabling clinets to normalize their mental, emotional and somatic reactions, to structure what often seems like a chaotic inner world and to address issues including shame.
This paper will propose a protocol for EMDR psychotherapy with people who have been victims of rape, torture, and organized violence and will introduce a “protocol interweave” for working with people for whom shame is a factor that impedes effective processing.
The “protocol interweave” focuses on the ‘self who has been shamed” and adapts the desensitization phase to enable the individual to process material associated with their thoughts, feelings, and sensations with regards to the self of whom they are ashamed and whom they may despise. The paper will also examine recent thinking about shame, compassion and forgiveness and reflect on similarities across psychotherapeutic modalities such as gestalt and cognitive behaviour therapy and the way in which EMDR is an integrative model that accommodates these.

Theory - EMDR can be seen as working on one level of the human system - the neurobiological level. However
the human brain is formed and is constantly changed by it’s interactions with others. The neurobiology of
relationships is an expanding field and I will be arguing that EMDR and systemic theory have much to offer each
other at this juncture. I will be putting forward my ideas about the scope for creative thinking and practice at the
interface of these paradigms. By utilizing the client’s relationships and seeing them as a resource in the EMDR,
EMDR practitioners can facilitate change at that wider level. In order to make the presentation accessible to
those who are not particularly familiar with current systemic theory, I will briefly outline some of the main
systemic principles currently in use. Practice - I will illustrate my theoretical points with examples from my EMDR
practice where I have: 1) included more than one family member in the room at the same time; 2) where parallel
work has been done with the family and the individual; and 3) where I have incorporated significant systemic
thinking into my work with an individual. These examples will demonstrate skills in applying EMDR within a
broad systemic frame. They will illustrate how EMDR can enhance relationships, and conversely, how systemic
thinking can enhance the application of EMDR.

Since the introduction of EMDR training courses in Hong Kong a few years ago, the method has been gaining an
enormously growing interest among professionals. One of the indications to the growing interest in EMDR is the
high rate of participation in the training courses. While there is a volume of studies on the efficacy of EMDR
among different populations, therapists’ viewpoints on its use has not been studied extensively. The purpose of
the paper is to examine the views of therapists who have undergone EMDR training in Hong Kong and have used
the method in their practice. Data were collected through an open-ended questionnaire and follow-up in depth
interviews with a group of therapists. The questionnaire sought information about their initial experience of
using EMDR, the types of patients they used it with, the contribution of the method to the therapy and the
difficulties they encountered. Follow-up interviews aimed to shed more light on issues raised by the respondents
and to provide a platform for their further elaboration. Participants’ responses indicated the positive effects
EMDR use on clients and its facilitation of the therapy process. The respondents indicated difficulties in following
the exact EDMR protocol especially in relation to the terminology such as: “completely false”, “completely true”,
“emotions” and “sensation”. Additional issues raised around the use of the scales and of different bilateral
stimulations. On the basis of these finding special consideration should be given to the adaptation of the EMDR
protocol to the Chinese language and way of thinking.

For men and women alike, libido represents a primary aspect of sexual health. In fact, low libido is the most frequently reported sexual concern among women. Women of all ages, ethnic backgrounds, and education levels report concerns about low desire. Regardless of whether a woman is actually diagnosed with hypoactive sexual desire disorder (HSDD), if she seeks help for the subjective experience of a decreased libido, practitioners must embrace a theoretical framework for her care. In this chapter, we explore a holistic, integrative, biopsychosocial model for such treatment. We will outline first why we believe the traditional treatment model involving a sole practitioner providing the majority of patient care does not optimally serve patients with low libido. Second, we will review some of the benefits for both patients and clinicians of practicing within a biopsychosocial treatment paradigm. Third, we will describe our particular framework for treating low libido in women. This integrated biopsychosocial treatment model addresses four quadrants of a woman's experience; namely, her physical, emotional, intellectual, and spiritual health, as they relate to her libido. Finally, we find eye movement desensitization and reprocessing (EMDR) to be a helpful therapeutic tool in these circumstances. EMDR utilizes repetitive eye movements to assist patients in exploring and processing difficult emotional material. It can be utilized for women with low libido in a variety of ways, including the exploration and redefinition of negative thinking patterns. The consistent findings on the adverse psychological effects of war and terror on
children have led to a recognized need for suitable early intervention in the immediate,
midterm and post term, of mass crisis situation. This presentation will try to overview
the clinical and empirically supported interventions operating in the last two years in
Israel, with children and families living in a constant threat of bombardment and
shelling of populated areas on both sides. This presentation will focus on the war related
stress reactions of young Israeli children exposed to these experiences and
present some creative interventions done to foster resilience and coping, as well as the
treatment of PTSD reactions.
It was estimated that during this war, 4,000 Katyusha rockets and missiles hit the
northern parts of Israel. More then 1 million people were at the danger zone for a
potential direct hit. More then 500,000 Israelis relocated during the war to live in safer
areas out of the range of the rockets. The children in the war zones experienced
sounds of sirens' explosions and sound of artillery. They spent considerable time in
bomb shelters and were exposed to sights (directly or through the media of damaged
houses and casualties). This situation continues now in the southern part of the
country along the Gaza Strip were children on both sides live under the daily reality of
bombardment, casualties and death.
Many programs and individual treatments were and are still operating as the
professionals are trying to meet the needs of communities and individuals under this
chronic ongoing stress situation. A community project is operating in the southern part
of the country, where mothers and toddlers as well as the staff members of the
kindergartens, are being supported by dyadic trauma focused play therapy and
enrolled in stress managements programs. Findings on the interrelation between the
degree of the parent's PTSD on the child's aversive symptomology,and the follow up of
dyadic treatment data, will be presented.
Another program the "Huggy Puppy", exemplifies the implementation of a
developmental appropriate intervention with children in the midst of the crisis, and
the effects of building a sense of self efficacy and relatedness. This will be illustrated in
the study done by Prof. Avi Sadhe from Tel Aviv University on the effects of the "Huggy
Puppy".
The use of the modified EMDR Group Protocol for children during the war and the
efficacy of the EMDR individual treatment in the post war situation as compared to
CBT and SE FAR will be examined in the research done by Dr. Moshe Farchi of the Tel
Chai College.
These diverse interventions and treatments represent the wide range of theoretical
and practical thinking which are necessary in order to create a comprehensive holistic
model which will serve as a guideline for interventions with children in crisis.

This presentation describes an EMDR protocol for working with an adolescent in such a way as to enhance their self-empowerment
capabilities. As a preliminary, a medical and developmental history of the adolescent is taken from the parents, as well as obtaining
their view of present problems. Using this infomation and a subsequent history obtained from the adolescent provides the basis for
what may prove to be targets later.
I will talk about how, in early discussions with adolescents, when my aim is to gain rapport, I often go back over some of the
information their parents provided so as to obtain the teenager's views and feelings, idenfying the highs and lows in their view of
their life so far. I look for where they have felt successful and powerful, and where they have suffered trauma, loss and sadness with
reference to themselves as individuals and also in their family relationships. I seek to refine targets from both parental and
adolescent information. I also seek to understand what the teen would like in their life and begin to introduce the idea that they can
give direction to their life.
I believe it is important to provide the adolescent with new or clarified information about him or her. I use psychological tests to
assist in this process. I will cover briefly ways that I use the Behavior Assessment System for Children (BASC), as well as other
assessment instruments such as the Strong Interest Inventory, Rorschach and Millon Adolescent Personality Inventory (MAPI), to
give both the adolescent and me important information. Because the information stems from a comparison with the general
population, I can present it more objectively than if it were solely my assessment of them. The adolescent decides what fits them
and what doesn't. This process allows us jointly to identify problems and to look for ways to handle them. Emphasis is always
placed on the adolescent having the power - the information about themselves is power. What they choose to do with it breeds
power. They are in control. This process usually leads to a further explanation of EMDR and how they might choose to work with
me using EMDR.
In summary, I will cover :
Helping the adolescent look at choices as giving freedom Presenting EMDR as a way of having more choices; Looking at behavior problems as habits that can be changed if desired. Encouraging the use of imagination, imaging, finding a
