Shapiro suggested that eye movement procedure could accelerate information processing and further reduced the client’s anxiety and disturbance. However, psychotherapy evaluation regarding crucial therapeutic parameters awaits elucidation. This dismantling study was to investigate the therapeutic effects of eye movement and positive cognition on phobias. Specifically, via a 2×2 between subject design, with “eye movement/non eye movement” and “treatment process” being two independent variables, a total of 40 female university students with fear of cockroaches were screened and recruited from introductory psychology class to explore the treatment effect of those two components. The results showed that, according to macro therapeutic indices, the effect of therapy was significant, and was not significantly different among groups. As to micro treatment process, while the participants’ SUDs decreased linearly, and so did the negative cognition VOCs, the positive cognition VOCs increased linearly only for the eye movement condition. In addition, while for macro index, the physical index, heart rate, was significantly higher at pre-test than at baseline and returned to baseline at post-test, the process measures indicated that heart rate increased during the first treatment stage, returned during rest period, and increased again during the second treatment stage. According to the micro process, the results also suggested that when presented with positive cognition participants’ VOCs of positive cognitions increased only for the eye movement condition. As to the therapeutic effects, although exposure by itself might be effective, eye movement could further promote participants’ VOCs of positive cognitions at the second treatment stage, probably by facilitating information processing. Nonetheless, regarding macro therapeutic index, there were equal therapeutic effects across groups within one-session therapy, and the superiority of positive cognition installation remained obscure, which implied that to become obvious more sessions might be called for. The implications of the present results and further research possibilities are postulated.

This article is a preliminary report on the remarkable results some of my clients and I have been achieving using EMDR to target prenatal trauma, with a focus on the discovery of an experience of the self prior to any trauma occurring and the enormous healing power that derives from revisiting and reactivating this extraordinarily positive pretraumatic experience. It is with some hesitation that I am reporting my experience with prenatal trauma processing, as I do not wish to be seen as on the fringe or even over the edge by my colleagues. However, I realize that I was able to overcome my prejudices -through examination of the facts- about the nature of prenatal experience, when memory begins, and how it can be accessed. I have found there is a body of scientific investigation and knowledge on prenatal experience and trauma, and that indeed we do experience and are influenced by our environment in the womb. We can learn from such experience, and therefore, can be traumatized prior to birth. The prenatal self can feel and record this experience. I refer the reader to www.birthpsychology.com/resources/ index.html for a list of publications on this matter. The various kinds of pre- and perinatal trauma and the deep healing that results when processed with EMDR will be the object of other articles by myself and Dr. Heather Pearson, who is also investigating this same field. What I intend to focus on here is the discovery of a pre-traumatic experience at the embryonic stage, which I have found to be a remarkably powerful internal resource for healing, already developed and installed, simply requiring reactivation. When I saw the powerful healing results of reactivation of this pre-traumatic experience in a number of relatively “stuck” clients, I felt ethically bound to report this immediately to other clinicians using EMDR so that others may benefit.

Abstract:
EMDR is a treatment developed by Francine Shapiro (2001) to reprocess traumatic experiences that are “locked” in the nervous system and give rise to current symptomatology. It involves an eight stage protocol in which clients select a target memory and, with the clinician, assess its cognitive, somatic, and emotional components as well as associated level of distress. The memory is then reprocessed through bilateral stimulation (most commonly eye movements) of the brain until the level of distress is reduced. Shapiro developed the “information-processing model” to explain EMDR’s “…treatment effects in terms of the association of memory networks” (Shapiro, 2002, p. 29). The reprocessing allows the client to “digest” a stuck traumatic memory by connecting it with more adaptive memory networks in the brain. EMDR originated as a treatment for PTSD, but EMDR protocols now exist for a variety of issues, such as phobias and grief. Originally developed with adults, its use has also been extended to children, but with modifications in technique.

Dagli ultimi dati statistici risulta che la Depressione Post Partum viene sviluppata da circa il 13% delle puerpere nel corso del secondo – terzo mese successivi al parto e la durata dei sintomi varia da qualche settimana ad un anno, con rischi di recidiva. A differenza del Baby Blues (colpisce il 70% delle puerpere, insorge in III – VI giornata e si risolve spontaneamente nell’arco di un paio di settimane), imputabile essenzialmente alle fisiologiche modificazioni ormonali (calo degli estrogeni e progesterone) dell’im-mediato post partum, lo sviluppo della DPP sembrerebbe invece essere determinata da fattori di natura strettamente psicologica: l’esperienza del parto, il riemergere di problematiche irrisolte nelle relazioni con le figure di attaccamento, il cambiamento di ruolo della donna sia nell’ambito sociale che all’interno della coppia, il timore di non essere in grado di attendere adeguatamente alle nuove responsabilità (sia sul piano delle proprie capacità, che del nuovo carico di fatica fisica), ecc. Le donne che hanno vissuto il parto come esperienza traumatica risultano essere maggiormente destabilizzate da tale evento e quindi maggiormente esposte allo sviluppo della DPP. Il parto richiede alla donna il reclutamento di molteplici risorse personali. Nel corso del “travaglio” la donna deve riuscire a contenere il dolore, mentre nella fase dell’“espulsione” le viene richiesto inoltre di “spingere”, ossia di contrastare una reazione antalgica automatica (che chiuderebbe il canale da parto) per andare invece “incontro al dolore”. Se consideriamo che le “Prestazioni di Picco” si caratterizzano per essere “al di fuori” del proprio ambito di sicurezza, di agio e di conforto, nel tentativo di oltrepassare il limite estremo delle proprie capacità, l’esperienza del parto può essere inscritta a pieno titolo nelle “Prestazioni di Picco”. Nel presente lavoro vengono descritte le modalità e i tempi di applicazione del RDI nel corso della Preparazione al Parto, finalizzata al rafforzamento delle diverse risorse personali di cui ogni gestante ha bisogno per poter vivere il parto come esperienza egosintonica. In tal senso l’RDI, tramite EMDR, è da considerarsi un vero e proprio Intervento di Prevenzione Primaria, in grado di insegnare alle donne qualcosa di positivo riguardo sé stesse, contrastando efficacemente l’insorgere della DPP. Verranno esposti, inoltre, i risultati dell’applicazione di tale tecnica su 48 gestanti, raccolti nella fase del Post Partum.

The latest statistics show that postpartum depression is developed by about 13% of mothers during the second to third months after delivery and the duration of symptoms varies from few weeks to a year, with risks of recurrence. Unlike the Baby Blues (affects 70% of mothers, occurs in III - VI day and resolves spontaneously within a couple of weeks), largely because of the physiological hormonal changes (decline in estrogen and progesterone) of IM-mediated post-partum, the development of the DPP seems to be determined by factors strictly psychological: the experience of childbirth, the resurgence of unresolved issues in relations with attachment figures, the changing role of women both in social the couple, the fear of not being able to wait adequately to new responsibility (both in terms of its ability, that the new burden of physical labor, etc.). Women who have experienced childbirth as a traumatic experience become more undermined by this event and, therefore, at greater risk of developing the DPP. The birth of the woman requires the recruitment of many personal resources. During the "Labor" the woman must be able to contain the pain, while in phase the 'expulsion' is the also required to "push", i.e. a reaction to counter analgesic automatic (which close the channel by birth) to go instead "to meet the pain." If we consider that "Peak Performance" are characterized by being "outside" the extent of its security, ease and comfort, in an attempt to go beyond the bounds of their abilities, experience delivery can be fully inscribed in the "peak performance". The present paper describes the methods and timing of application of RDI during the preparation for childbirth, which aims to reinforce the various personal resources which each pregnant woman needs to live the experience of childbirth as ego syntonic. In this sense, the RDI, through EMDR is considered true primary prevention interventions that can teach women something positive about themselves to effectively counter the rise DPP. Will be exposed, in addition, the results of applying this technique on 48 pregnant women, collected at the stage of post-partum.

Abstract: EMDR heeft zich bewezen als een effectieve behandelvorm voor patiënten met een posttraumatische stressstoornis. Min of meer bij toeval werd ontdekt dat sommige patiënten die ook last hadden van chronische pijn, minder pijn hadden na EMDR. Daarom gaan steeds meer EMDR therapeuten ook pijnpatiënten behandelen. Het gebruik van EMDR bij chronische pijn bevindt zich echter in een experimenteel stadium en er zijn nog weinig wetenschappelijke publicaties. Deze folder is vooral gebaseerd op praktijkervaringen en op wetenschappelijke inzichten over pijn, de hersenen en EMDR.

EMDR has proven to be an effective form of treatment for patients with posttraumatic stress disorder. More or less by chance it was discovered that some patients who also suffer from chronic pain, had less pain after EMDR. Why more and more EMDR therapists also treat pain patients. The use of EMDR in chronic pain is however, in an experimental stage and there are few scientific publications. This leaflet is mainly based on practical experience and scientific knowledge about pain, the brains and EMDR.

When I initially heard of EMDR, I was totally uninterested. I was happy with my repertoire of skills and simply could not be bothered. However, the reports of colleagues who had the training were so exceptional I thought it was time to have a look. I still was not prepared to waste my precious hours on a training, and my way around this was to experience EMDR myself. I had genuine difficulties isolating a problem since I was at a particularly fulfilling stage of my life; however, I finally settled on a minor irritation that I was having with one of my clients. That session, plus two others, moved me very rapidly to a decision to move into private practice, to implementing that decision, and to currently experiencing a life of ease, gentle pacing, and tranquility that I had no previous vision of being possible. In fact, there has been a total life style transformation. I now work only three days a week, have time to follow the joyous explorations of my toddler for hours, am writing a novel, and am experiencing considerable relaxation of the Puritan work ethic. The starting point had been a life style I had previously perceived as fulfilling and perfectly for me.

Performance anxiety is ubiquitous in our present culture and is considered one of the most prevalent forms of anxiety. The famous actor and comedian Jerry Seinfeld once joked, “ At a funeral, most people would rather be in the casket than giving the eulogy!” While nearly eighty percent of people experience some form of anxiety when they are the center of attention, individuals who experience performance anxiety are severely distressed and debilitated by their anxiety (Plaut, 1990). In most cases, performance anxiety threatens to restrain an individual’s profession, goals, education, relationships or daily life activities. While the phenomenon of acute anxiety is commonly labeled as stage fright in the world of the performing arts, in psychological literature it is rarely specified or considered a diagnosable mental health disorder. It is often clustered with specific phobias or social phobia. Indeed, performance anxiety is not an experience solely limited to actors, musicians, singers and dancers. It affects athletes, politicians, writers, students, professionals, leaders, and individuals in all walks of life. Abstract:
Edwin werd op 13-jarige leeftijd opgenomen in de kinderkliniek van het Dr. Leo Kannerhuis (LKH) in verband met zijn stoornis in het autistisch spectrum. Na enkele jaren in de kinderkliniek wordt hij doorgeplaatst naar de jongerenkliniek waar hij, inmiddels 16 jaar oud, behandeling krijgt in een groep van zes adolescenten. Edwin is een jongen met een forse autistische stoornis, wat zich met name uit in een zeer vertraagde informatieverwerking, moeite met sociale contacten en gebrekkig sociaal inzicht. Daarnaast is er bij Edwin sprake van preoccupaties in het denken (steeds dezelfde herhalende gedachten). Edwin raakt snel overprikkeld wanneer hij te veel informatie krijgt of te veel sociale interacties moet verwerken. Hij raakt dan in de war en probeert weer grip te krijgen op zijn verwarring door verklaringen te zoeken. Door Edwins beperkte inzicht in de omgeving zijn deze verklaringen vaak niet conform de werkelijkheid en veroorzaken ze bij hem nog meer verwarring. Edwin was 13 years of age included in the pediatric clinic of the Dr.. Leo Kanner (LKH) associated with their disorder in the autistic spectrum. Abstract:
The clinical adherence to and combination of methods with EMDR to treat PTSD has not been explored. This is of some concern as 27 000 clinicians are trained in EMDR globally. This exploratory study gained information on Australian trained EMDR clinicians' adherence to EMDR, and the methods they combined with EMDR to treat PTSD. Survey responses from 126 clinicians were analysed using predominantly descriptive statistics. The findings of this study showed that the majority of respondents perceived that they closely adhered to the standard EMDR treatment. However, respondents reported adhering more closely to the phases than the steps of EMDR. Statistically significant findings showed that more experienced EMDR level 1 clinicians were more likely to add steps and phases to EMDR, and to change the standard sequence of steps. Clinicians with greater years of experience were also more likely to change the standard sequence of steps. In addition, more experienced EMDR level 2 clinicians, were less likely to educate their clients about PTSD. Almost all respondents combined EMDR with other methods to treat PTSD. Combined methods included CBT, hypnosis / relaxation, system / solution focused methods and exposure. This research has contributed to theory and practice by uncovering that clinicians appear to use EMDR differently to researchers, by changing EMDR to meet client needs, combining EMDR with other methods and Holly Byron 16/9/99 vi therapies, and introducing EMDR in the mid stage of treatment. These findings are vital to reduce the scientist-practitioner divide by accurately testing EMDR's efficacy and enabling future controlled trials to reflect the clinical use of EMDR.

The concepts, interventions, and techniques presented in this workshop are culled from theory and techniques of working with the range of the dissociative disorders). They have proved to be an effective addition to the preparation stage of the EMDR protocol. In other words, they can be used with clients who dissociate under certain conditions but do not have a dissociative disorder Most of the traumatized clients seen for EMDR treatment have a range of dissociative symptoms as well as symptoms of PTSD. This combination of PTSD and a dissociative disorder is often labeled DDNOS. However, people with a more complex variety of PTSD usually have experienced very early and enduring severe physical or sexual abuse (generally perpetrated by a family member), atrocities, war, or severe environmental disruption such as earthquakes. They are more accurately diagnosed with disorders of extreme stress (DESNOS). For these clients, the dissociated neural networks, or dissociative fragmentation, cause serious problems in adult life. The adaptive information processing system is on hold for these dissociated fragments or parts. They are easily triggered by internal or external cues to which they can have extreme reactions, (flashbacks, amnesia, losing time and place, and so forth.) Our goal as therapists is to use EMDR to help clients (and their internal dissociated neural networks or parts) find stability and resources to function adaptively in their present life, and then desensitize and reprocess the dissociated trauma memories and the PTSD symptoms. We aim to help our clients manage their symptoms. It is not our goal to eliminate dissociation, which has been a major survival strategy, but to help the client utilize it with conscious control. It is important to note that attachment issues are an aspect of development that are especially impacted by trauma. The attachment styles of the family pre trauma may have already affected the client in negative ways, impacting the client's resources and responses to trauma. One way to look at this set of problems is to utilize two approaches in the preparation phase of EMDR. These approaches combine the treatment of dissociative symptoms with ego state work and are an essential aspect of treating these clients with EMDR. This work may extend the preparation phase considerably, but will add safety and structure to the trauma processing experiences for these clients.

This dismantling study investigated the therapeutic effects of eye movement and positive cognition components on phobias. Forty female Ss with cockroach phobias received a single therapy session. The therapy conditions constituted a 2 (eye movement/non eye movement) × 2 (treatment procedure: positive cognition installed/negative cognition prolonged) between subject design. The results revealed that all groups showed significant therapeutic effects according to macro therapeutic indices and with regard to some micro indices such as SUDs, HRs and VOCs for negative cognition. However, VOCs for positive cognition were significantly increased only for the eye movement group. The findings suggested that although exposure itself might be effective in treating phobias, eye movement could further promote participants’ VOCs for positive cognitions at the second treatment stage, probably by facilitating information processing.

A number of single case reports have made impressive claims for the efficacy of "eye movement desensitization" (EMD) in the treatment of traumatic memories. Many of these case reports claim that EMD reduces the unpleasant feelings associated with traumatic images. However, at present, there are no published controlled studies that provide evidence for these claims. The present experiment investigated whether EMD inhibits emotional responding during retrieval of aversive information. Normal Ss (N = 40) were exposed to an aversive slide. During a next stage, half of the Ss underwent EMD while they rehearsed the slide information, whereas the other half underwent a control procedure (i.e., finger tapping) while rehearsing slide information. Before and after EMD or control intervention, heart rate and self-report data were obtained while Ss retrieved and visualized the aversive slide. The nature of this study is both observational and clinical. On the one hand it’s aimed to see how the criteria of PTSD (DSM-­‐IV, 2000) are represented in traumatized cancer patients in between two different illness stages -­‐ medical treatments and medical follow-­‐up stages, -­‐ and to see witch coping strategies are used by these patients with PTSD in each stage. On the other hand it’s aimed to know: a) whether CBT integrated with EMDR produces different effects on PTSD in between the two illness stages and their specific coping strategies; b) whether CBT with EMDR and CBT without EMDR produce different effects on PTSD and on the coping strategies within the only medical follow-­‐up stage. All 31 subjects were recruited randomly, 10 for treatment stage, 11 for follow-­‐up stage, both receiving CBT and EMDR, and 10 for a follow-­‐up stage treaty with CBT and no EMDR. CAPS and COPE were used to assess PTSD and coping strategies respectively. Results show similar changes in between the two illness stages after CBT with EMDR, and different ones within the follow-­‐up stage depending on whether CBT with EMDR or without EMDR was used. This may suggest considering PTSD such a disorder that removes specific stage demands/threats of cancer illness, just described in literature, as well as the more functional coping for each of them. You may instead claim that only CBT with EMDR allows cancer patients to overcome the PTSD and consequently retrieve selective specificity in coping strategies for each stage.

La naturaleza de este estudio es tanto observacional como clínica. Por un lado, pretende ver como se representan los criterios del TEPT (DSM-­‐IV, 2000) en pacientes con cáncer traumatizados entre dos fases diferentes de la enfermedad – fases de tratamientos médicos y seguimientos médicos -­‐ , y ver qué estrategias de afrontamiento son usadas por estos pacientes con TEPT en cada una de estas fases. Por otro lado, pretende saber: a) si la TCC integrada con EMDR produce efectos diferentes sobre el TEPT entre las dos fases de la enfermedad y sus estrategias de afrontamiento específicas; b) si la TCC con EMDR y la TCC sin EMDR producen efectos diferentes sobre el TEPT y sobre las estrategias de afrontamiento tan sólo dentro de la fase de seguimiento. Cada uno de los 31 sujetos fueron seleccionados aleatoriamente, 10 para la fase de tratamiento, 11 para la fase de seguimiento, ambos grupos recibiendo TCC y EMDR, y 10 para la fase de seguimiento con TCC y sin EMDR. Se utilizaron CAPS y COPE para evaluar el TEPT y las estrategias de afrontamiento respectivamente. Los resultados muestran cambios similares entre las dos fases de la enfermedad después de la TCC con EMDR, y cambios diferentes en la fase de seguimiento en función de si se utilizaba la TCC con o sin EMDR. Esto puede sugerir que se ha de considerar al TEPT como un trastorno que quita específicas demandas/amenazas de cada una de las fases de la enfermedad del cáncer, tal y como se describe en la literatura, así como el afrontamiento más funcional para cada una de ellas. En vez de eso, se puede afirmar que tan sólo la TCC con EMDR permite a los clientes superar el TEPT y consecuentemente recuperar especificidad selectiva en estrategias de afrontamiento para cada fase.

Após quase três anos de tentativas frustradas de engravidar naturalmente, Ana procurou uma clínica de reprodução humana. It is known in scientific circles connected to EMDR and perceived daily in the offices of psychotherapists who use this innovative technique that EMDR therapy has a range many times greater than originally planned. This presentation aims to report a case in point: the path of psychotherapy Ana (not her real name), thirty-six years old, since its initial order to mitigate their excessive autocobrança past the moment he realized that his negative belief more active could be one of the factors that hindered their fertility until her pregnancy before imagined as impossible. After nearly three years of failed attempts to conceive naturally, Ana sought a clinical human reproduction. In the seven years following Ana underwent various treatments, including two inseminations with positive outcome that resulted in miscarriages held a few weeks later, and a successful fertilization until the end of the pregnancy which was born his son, now five years old. To know the present status of EMDR in Asian countries: Method: Inquiries by e-mail to the representatives of Asian countries and related US or European people were sent. Replies were received from countries like Australia, Cambodia, China, Hong Kong, India, Indonesia, Japan, Korea, Sri Lanka, Taiwan, and Thailand. Questions were on the origin of EMDR, the first training, the number of trainings so far, the number of trained practitioners, credentials, academic organization, acceptance from government, media, and professional world, future possibilities, and difficulties expected. Results: Some countries like Australia, Japan and Korea have already reached the moderate stage, but still they have problems to be solved. In Australia, their first training was in 1993 and many therapists have received training, but, organization started very recently and network is not strong. In Japan, Japan EMDR Association has more than 800 members and started publishing their own academic journal in 2009. However sceptical statements about EMDR can be seen in some books on trauma. In Korea, they have health insurance system for EMDR but practitioners are few. The other countries are in the early stage to grow the EMDR community or support the EMDR therapists. Most of them began the history after a big natural disaster like Tsunami or earthquake. HAP from Europe and/or US supports their beginning. The first Asian conference could be a good opportunity to start mutual understanding and cooperation in Asia.

Grounded theory method was used to explore the experiences of patients suffering the effects of psychological trauma who had received eye movement desensitization and reprocessing approach (EMDR) as treatment. Saturation of the categories was achieved with the analysis of 15 interviews. The basic social psychological process that emerged is transforming suffering and the core category is changes in perception. The three subcategories, relinquishing, presencing and emerging, form the conceptual framework for the stages of transforming suffering. Para estos problemas las técnicas de "Flotar hacia atrás" y "Flotar hacia delante" desarrolladas por William Zangwill Ph. D., entrenador del Instituto EMDR, son métodos efectivos para ligar el pasado, presente y futuro en un ámbito terapéutico y proveen al terapeuta de instrumentos para abordar eficientemente ambos temas. LA TÉCNICA DE FLOTAR HACIA ATRÁS Abordar recuerdos tempranos asociados con el material perturbador es fundamental para EMDR. Shapiro dice que ayudar al paciente a encontrar un recuerdo temprano "debe ser una de las primeras opciones que debe considerar al terapeuta..." (Shapiro, 1995). La Técnica de Flotar hacia atrás es un camino eficiente y poderoso para llegar a esta meta, permitiendo al terapeuta asistir al paciente a llevar a cabo sus propias asociaciones con acontecimientos del pasado. El paciente comentó que no bajaba porque esta "peor escena" podría sucederle realmente y le había sucedido en el pasado. Paramos los movimientos oculares, charlamos un rato y elaboramos un plan para: a) una sesión con sus padres adoptivos para hablar sobre la permanencia de la adopción y b) una llamada en conferencia a su asesor legal para clarificar sus derechos y opciones. Volviendo al target después de esto, le fue posible reducir el SUD a 1 con unos pocos sets de movimientos oculares. Al utilizar la Técnica de Flotar hacia delante para reprocesar la peor escena, el paciente tiene una oportunidad para resolver la ansiedad anticipatoria. A aplicar las Técnicas de Flotar hacia Atrás y hacia Delante y ocuparse así del pasado, presente y futuro, el terapeuta de EMDR puede sanar mejor a su paciente. Es más, las Técnicas de Flotar hacia Atrás y hacia Delante están basadas en EMDR. Las dos incorporan las preguntas del protocolo standard y le dan al terapeuta y al paciente la oportunidad de manejarse más fluidamente con dicho protocolo.

EMDR standard protocol requires a focus of the origins of the disturbance, the triggers of this and create a pattern of behaviors appropriate to the future (Shapiro). Some patients, however, may have difficulty connecting the current problem with past events. Also, other patients may have difficulty creating positive patterns for the future, especially if you try new behaviors makes them anxious. For these problems the techniques of "float back" and "Float forward" developed by William Zangwill Ph.D., EMDR Institute trainer, are effective methods to link the past, present and future in a therapeutic area and provide the therapist tools to effectively address both issues. THE ART OF FLOATING BACK Addressing early memories associated with foreign material is essential to EMDR. Shapiro said that helping the patient to find early memory "must be one of the first options to consider when therapist ..." (Shapiro, 1995). Floating Technique back is a powerful and efficient way to reach this goal, allowing the therapist to assist the patient to carry out their own associations with past events. Its use is most appropriate when the clinician suspects that a disturbance that the patient is experiencing at present, is rooted in past experiences, especially when questions like "What is your earliest memory in relation to what you feel now? Not been successful in helping patients to connect with past events. Also when a patient has a recurrent theme or experience, the Backward Floating Technique is ideal for helping the patient to identify a target for reprocessing. Many patients come into contact with the current problems with relative ease. For example, a patient who complains that she feels abandoned when her husband goes on a business trip, you can probably recall their current problems with ease. Then the therapist can apply the technique Float Backwards to help the patient to remember a past event quickly and efficiently. To use the technique to back float, arm the protocol to the current problem, using the steps listed in the Training Manual Level I and Level II (Shapiro, 1994) including the image, negative cognition (NC) positive cognition (PC), validation of cognition (VoC), emotions, Subjective Unit of Disturbance (SUD) and bodily sensation. However, it still affects the processing (ie, eye movements or other stimulation). Instead, tell your patient: "Look at the picture ... and those words (repetition of the disturbing image of the patient and negative cognition), note that emotions come from and where you sit on the body. Now close eyes and let your mind float back to an earlier period in your life, look no further, just let your mind float to a time when you thought things like ... (repeat the emotions that said the patient) .. . (repeat parts of the body where the patient felt the sensation). When you are ready open your eyes and tell me the first thing that comes to mind. " Use this early experience as a target, completing all protocol items: image, CN, CP, VoC, emotions, SUD and location of bodily sensations and begin processing with eye movements or other bilateral stimulation. Once this material has been processed, return to the original target of the current material. Very often we generalize the work done on the earlier material and no longer have to render the current material. It is important to use general terms when the patient is given instructions Technique Float Backwards, ie a memory request early and not earlier. There are several reasons that support this. First, it is often the worst memory and not the first that works as the best target for reprocessing. In addition, using general terms is an aid for compulsive and perfectionistic patients who otherwise would be too concerned with avoiding failure and find exactly the first association. Finally, the flexibility that allows the use of general rather than specific terms increases the likelihood of success of the patient to connect with the past that is the goal of this technique. The essential feature of the technique is to use Float Backwards questions of protocol to connect the problems of the present with past events. Skip the questions and were developed by Shapiro is a powerful method to help patients to tune into all aspects of their experience of the problem. The foreign material becomes more vivid and present to the patient and possible recall similar experiences. It is assumed, arguendo, that having developed the protocol with all the questions about the current problem, the neural network encourages and facilitates partnerships almost effortlessly "float back" early associations. In addition, the patient-therapist relationship is enhanced because the therapist validates the patient's experience (current disruption) to start work from the point where the patient is. Partnerships are the patient, eliminating the issue of resistance to any idea or interpretation introduced by the therapist. The patient realizes experientially connecting the present with the past by using the technique Float Backwards, can avoid the avoidance and other defenses. THE ART OF FLOATING FORWARD While technology enables Float Backwards often patients to see and feel the connection between the current problem and past events, the forward float technique allows the patient to identify and reprocess anticipatory anxiety and develop positive patterns the future. It is a method that can be used at any time of the therapeutic process to troubleshoot crashes, reluctance and in some cases, resistance or topics of ancillary benefits or losses. It is especially useful for working with the patient's fear to do EMDR. To put this into practice, first ask the patient to imagine the worst that can happen if you "X" (eg. Try a new behavior, test a new skill, start a new experience.) What's the worst that can happen if you EMDR? That's the worst that can happen if you solve this problem? What's the worst that can happen if you put your head limits on the amount of work expected to do? The patient may need help to identify the worst scene. Some suggestions include fear of losing control of his emotions, fear of losing control of their bodily functions such as bowel and bladder control, fear of having a panic attack and can not manage their emotional life between sessions. Once the patient has identified the incident, ask for the worst part of that scene and use it as the target of EMDR, setting up the protocol with the standard questions, but with a slight modification: ask for the image that represents the worst of worst incident, eg. "When you see a picture of him / herself by ......, it's the worst that can happen?" Then follow with the rest of the standard questions, ie, CN, CP, VoC, emotions, SUD, and location of bodily sensation. Stimulate the processing of patients with eye movements or other bilateral stimulation. If the development of the patient's worst scene provokes a rational fear, you may have to take practical steps to address these concerns. For example, using the technique of floating forward with a boy of 13 who was in a temporary foster home, the worst scene evoked for him was: "I will return home if this adoption is not." During processing, the LDS was reduced from 8 to 3 fairly quickly but it does not down. The patient said he did not go down because the "worst scene" could really happen and had happened in the past. Eye movements stopped, we chatted a while and developed a plan for: a) a meeting with her adoptive parents to discuss the permanence of the adoption and b) a conference call to his legal adviser to clarify your rights and options. Returning to the target after that, it was possible to reduce the LDS-1 with a few sets of eye movements. Using Floating Technique forward to reprocess the worst scenario, the patient has an opportunity to resolve the anticipatory anxiety. During the installation of the positive cognition, the patient is creating positive patterns for future action. A woman whose brother was verbally abusive to her children and now intimidated, put together a "worst stage" with: "It will be equally unfair when I see him next time." The patient had done much EMDR reprocessing childhood incidents related to verbal abuse of his brother. However, without a positive reference experiential, still anxious every time I interacted with him. Asking him to "float forward" and using EMDR on one of the worst scenes, relieved her anxiety about a family party that was pending. Installing a CP of "I'm stronger now allowed him to create an image of herself driving her brother with humor and feeling safe. To apply the techniques to float back and forth and deal well past, present and future, the EMDR therapist can heal your patient better. Moreover, techniques to float back and forth are based on EMDR. Both incorporate the standard protocol questions and give the therapist and the patient the opportunity to be managed more smoothly with this protocol.

The term “post-traumatic stress disorder” (PTSD) is a relatively new diagnostic label, being formally recognized in 1980 in the Diagnostic Statistical Manual for Psychiatric Illness – Third Edition (DSM-III) of the American Psychiatric Association (APA, 1980). Complex Post-Traumatic Stress Disorder (CP) is a more recently discussed, and newly-classified, phenomenon, initially discussed in the early 1990s (Herman, 1992a). Thus, as research into effective treatments for CP is sparse, the treatment of CP is the topic of this study, in which a guideline-based treatment program developed by the researcher for the treatment of CP is implemented and evaluated. Ten individuals participated in this study, undertaking individualized, guideline-based treatment programs spanning a period of six months. In providing background information relevant to this study, an explanation is provided regarding the nature of CP, and the reasons for its consideration as a separate phenomenon to PTSD. The adequacy of the PTSD formulation in enabling effective assessment and treatment of CP is also explored, with endorsement of previous researchers’ conclusions that the CP construct is more useful than the PTSD construct for assessing and treating survivors of long-term and multiple forms of abuse. The PTSD classification is restrictive, and not necessarily appropriate for certain forms of trauma (such as prolonged trauma, or multiple forms of trauma), as such trauma experiences may lead to specific effects that lay outside those formerly associated with PTSD. Such effects include alterations in affect regulation, consciousness, self-perception, interpersonal relationships, and in systems of meaning. Following discussion regarding the PTSD/CP classification, an examination of treatment methods currently used in the treatment of PTSD, and a review of treatment outcome studies, takes place. The adequacy of primary treatment methods in treating CP symptoms is then examined, with the conclusion that a range of treatment methods could potentially be useful in the treatment of CP symptoms. Individuals with a diagnosis of CP may benefit from the adoption of an eclectic approach, drawing on different treatment options for different symptoms, and constantly evaluating client progress and re-evaluating interventions. This review of treatment approaches is followed by details of an initial study undertaken to obtain feedback from individuals who had suffered long-term/multiple trauma and who had received treatment. Participants in this initial study were asked open-ended questions regarding the treatment approach they had experienced, the most useful aspect of the treatment, the least useful aspect, and other strategies/treatment approaches that may have been useful – but which were not used. The feedback obtained from these individuals was used to inform the development of treatment guidelines for use in the main study, as were recommendations made by Chu (1998). The predominant focus of the treatment guidelines was “ego strengthening”, a term coined by Chu (1998) to describe the “initial (sometimes lengthy) period of developing fundamental skills in maintaining supportive relationships, developing self-care strategies, coping with symptomatology, improving functioning, and establishing a positive self identity” (p.75). Using a case study approach, data are then presented relating to each of the ten individuals involved in the treatment program: details of his/her trauma experience(s)and the impact of the trauma (as perceived by each individual); details of each individual’s treatment program (as planned, and as implemented); post-treatment evaluation of the positive and negative aspects of the treatment program (from the therapist’s perspective); and details of the symptoms reported by the individual post-treatment, via psychometric assessment and also during interview. Analysis and discussion of the data relating to the ten participants in the study are the focal point of this study. The evaluation of the effectiveness of each individual’s treatment has been based predominantly on qualitative data, obtained from an analysis of language (discourse analysis) used by participants to describe their symptoms pre- and post-treatment. Both blatant and subtle changes in the language used by participants to describe themselves, their behaviour, and their relationships pre- and post-treatment have provided an insight into the possible changes that occurred as a result of the treatment program. The language used by participants has been a rich source of data, one that has enabled the researcher to obtain information that could not be obtained using psychometric assessment methods. Most of the participants in this study portrayed notable changes in many of the CP symptoms, including being more stable and having improved capacity to explore their early abuse. Although no direct cause-effect relationship between the participants’ treatment program and the improvements described can be established from this study, the participants’ perception that the program assisted them with their symptoms, and reported many aspects of “ego strengthening”, is of major importance. Such self-perception of strength and empowerment is important if an individual is going to be able to deal with past trauma experiences. In fact, abreactive work may have a greater chance of succeeding if those who have experienced long-term or multiple trauma are feeling more empowered, and more stable, as were the participants in this study (post-intervention). In concluding this study, recommendations have been made in regard to the use of guideline-based treatment programs in the responsible treatment of CP. Strengths and limitations of this study have also been highlighted, and recommendations have been made regarding possibilities for future research related to CP treatment. On the whole, this study has supported strongly other research that highlights the importance of focusing on “ego strengthening” in assisting those who have suffered long-term/multiple trauma experiences. Thus, a guideline-based program focusing on assisting sufferers of long-term trauma with some, or all, of the symptoms of CP, is recommended as an important first stage of any treatment of individuals who have experienced long-term/multiple trauma, allowing them to develop the emotional and psychological strength required to deal with past traumatic events. Clinicians who are treating patients whose history depicts long-term or multiple trauma experiences (either from their childhood, or at some stage in their adult life) need, therefore, to be mindful of assessing individuals for symptoms of CP – so that they can treat these symptoms prior to engaging in any work associated directly with the past traumatic experiences. Forty-three individuals diagnosed with classic or common migraine were randomly assigned to either Integrated EMDR treatment or a medication treatment. lntegrated EMDR combines diaphragmatic breathing, cranial compression and EMDR for abortive migraine treatment. Various abortive medications were used in the comparison group including Demerol, DHE, oral triptans, Excedrin, Florinal, Percoset, Toradol and Vicodin. Participants were treated during mid to late stage acute migraine and assessed by an Independent evaluator at pretreatment, post treatment, 24 hours, 48 hours and 7 days for migraine pain level. Both medication and Integrated EMDR treatment groups demonstrated reduced migraine pain levels at post treatment, 24 hours, 48 hours and 7 days. Howevei lntegrated EMDR treatment showed significantly greater improvement compared to medication at post treatment. Also, lntegrated EMDR reduced or eliminated migraine pain level with greater rapdity than medications. This study introduces lntegrated EMDR as a new abortive behavioral treatment for acute migraine episodes.

Strategic utilization of EMDR to move clients through the various stages of recovery will be discussed. 1) In the first stage of treatment, safety, stabilization, coping, and development of a strong therapeutic relationship are emphasized. Treatment focuses on decreasing (1) suicidal and parasuicidal behavior, (2) treatment - interfering behavior, and (3) quality-of-life-interfering behavior (Linehan, 1993). Efforts are made to assist the client in developing a repertoire of cognitive-behavioral coping skills; relevant skills address grounding, trigger awareness, basic self - care, mindfullness, distress tolerance, affect regulation, assertiveness, relaxation, self - monitoring, stress inoculation, and cognitive restructuring. At this stage, EMDR can be used to shift negative cognitions which interfere with commitment to treatment, skill development, and the restoration of hope. The following are examples of negative cognitions whlch interfere with first stage stabilization goals: - I will only get acknowledgment of my pain if I act out. - I don't deserve to feel better. - If I take care of myself, no one will know I hurt. - I'm pathetic, a failure. - I will die/go crazy fiom these feelings. - I can never do anything right. - I can't stand this feeling. The potential benefits of the use of a permissive style of clinical hypnosis as a therapeutic medium to enhance eye movement desensitization and reprocessing (EMDR) trauma treatment are explored. A comparative review of hypnosis and EMDR is provided, including putative psychophysiological mechanisms for both. A rationale for integrating clinical hypnosis with EMDR treatment is presented. It is suggested that hypnosis primarily enhances the accessibility of traumatic information while EMDR primarily enhances the reprocessing of traumatic information and that accessibility and reprocessing are reciprocal features. The relative and combined merits of hypnosis and EMDR for resource development are discussed. The author proposes that clinical hypnosis may be incorporated into EMDR without necessarily modifying the eight-stage EMDR protocol apart from modifications that are indicated for special conditions. In questi anni l’EMDR si è affermata come trattamento di evidenza nel più classico e più grave dei disturbi direttamente legati allo stress, cioè il PTSD (Bisson & Andrew, 2007) e sono stati evidenziati i suoi effetti sul SNC. Nonostante questo ed il suo progressivo impiego in una ampia varietà di situazioni sono ancora poche le riflessioni sull’EMDR dal punto di vista dell’integrazione mente-corpo, in particolare utilizzando i dati offerti dal campo di ricerca della PNEI. Nel presente lavoro ci soffermeremo su alcuni di questi aspetti per evidenziare come l’EMDR rappresenta una delle terapie più rispondenti alle nuove evidenze scientifiche ed alle esigenze di una scienza integrata della salute. Tre gli elementi salienti offerti dalla ricerca: 1. La vita ha plasmato nel percorso evolutivo strutture altamente integrate, descrivibili come sistemi complessi a rete in retroazione continua con il contesto. In particolare i sistemi nervoso, endocrino ed immunitario costituiscono nel network psicocorporeo un “super-sistema” di relazione e regolazione in grado di assicurare gli equilibri adattativi incorporando l’esperienza e gestendo la dinamica mantenimento-cambiamento (“regolazione allostatica”). Quindi l’organismo non è una macchina fatta di parti autonome tra loro assemblate, ma una realtà che parte dall’unità (zigote) e mantiene tale carattere unitario in tutte le sue articolazioni. 2. Lo stress non è qualcosa di per sé eccezionale e patologico, deve invece essere visto – all’opposto – come l’espressione di un insieme di processi (un sistema) che modulano la regolazione individuo-contesto a fini adattativi. In sostanza il sistema dello stress è sempre attivo e solo in situazioni di eccesso o di carenza (acute o croniche) produce effetti progressivamente dannosi per l’individuo. Le attività legate allo stress producono un “carico allostatico” (peso biologico, energetico) che – in relazione a fattori soggettivi ed esterni – può divenire “sovraccarico” innescando una catena di successive alterazioni nel funzionamento dell’organismo. Parallelamente vi è un “carico” ed un “sovraccarico psicologico” legati al primo da processi circolari. 3. Nel network corporeo e nel sistema dello stress, ovvero nei processi di regolazione generale interni e individuo-contesto, la mente svolge un ruolo cruciale di modulazione come dimensione nella quale i segnali (interni ed esterni) vengono trasformati in informazioni (assegnazione di significati) in base ai percorsi ed alle esigenze individuali di adattamento. La mente rappresenta così la più alta espressione dell’evoluzione della vita e delle sue strategie adattative e ciò spiega il suo ruolo rispetto al funzionamento complessivo dell’organismo. Le recenti acquisizioni delle neuroscienze hanno mostrato l’interdipendenza tra attività psichica, espressione genica e plasticità cerebrale che rende possibile il ruolo della mente. Il sovraccarico allostatico e psicologico legati allo stress dipendono in gran parte dall’attività mentale e costituiscono il principale fattore di rischio per la salute nelle società occidentali, antecedenti accertati delle patologie più diffuse. Se guardiamo a quanto detto sinora nell’ottica dello sviluppo individuale (fase intrauterina, processi di attaccamento, eventi significativi successivi) possiamo avere una idea di come mente, corpo e contesto interagiscono nella strutturazione dell’individuo ed il ruolo condizionante e de-strutturante delle situazioni e degli eventi che provocano una alterazione da stress. Le ricerche sul rapporto tra stress e sviluppo psicobiologico, tra modalità di attaccamento, strutturazione dei circuiti cerebrali, o tra queste e salute, confermano tali assunti (Lazzari, 2007). Venendo più da vicino all’EMDR, gli studi sulla psicobiologica del PTSD stanno mostrando che siamo di fronte sostanzialmente ad una manifestazione (particolarmente complessa ed accentuata) degli effetti dello stress sul network psicocorporeo (Iribarren et al., 2005). La ricerca sul PTSD (e gli altri disturbi da stress del DSM) deve incrociare necessariamente i filoni di studio sopra richiamati ed utilizzare la cornice concettuale che ne scaturisce. Gli effetti psicobiologici dello stress estremo e del trauma vanno inquadrati nell’ambito delle interazioni mente-corpo e della regolazione allostatica (carico vs sovraccarico allostatico). A nostro avviso, al di là degli specifici effetti sul SNC, l’EMDR si è mostrata efficace nel PTSD in relazione alla sua caratteristica generale di ricercare ed utilizzare come “target” non singoli aspetti di una esperienza, bensì l’esperienza nei suoi diversi aspetti: cognitivi, emotivi, fisiologici e relazionali. Il lavoro dell’EMR va a recuperare una integrazione perduta e lavora su questa. Si deve tenere presente infatti che, in via generale e preminente, lo stress compromette i livelli di integrazione, coerenza e flessibilità del sistema: lo stress cronico per processi progressivi di disregolazione, quello acuto per “rotture”, blocchi e sconnessioni. Pertanto l’elaborazione dell’informazione (e delle conseguenze) legata ad eventi e situazioni stressanti passa attraverso una re-integrazione di tutti gli aspetti psicobiologici correlati per giungere ad una ristrutturazione. Il livello di “penetrazione” (efficacia) di un approccio terapeutico è molto legato alla sua omogeneità con la realtà su cui interviene,cioè al fatto di parlare lo stesso linguaggio. E questo è un punto di forza notevole per l’EMDR. I dati disponibili sono coerenti con quanto enunciato: l’EMDR modifica i parametri fisiologici riducendo ed annullando l’attivazione da stress, spostando la bilancia dall’inibizione simpatica all’attivazione parasimpatica (Sack et al. 2007) e risulta associata l’abbassamento dell’arousal fisiologico, cioè con pattern di attività regolari e trofiche (Sack et al. 2008), mostrando similarità con quanto accade durante il sonno REM (Elofsson et al., 2008). L’EMDR incide altresì su disturbi fisici legati al sistema dello stress, come ad esempio hanno mostrato ricerche su patologie dermatologiche (Gupta, 2002).

In recent years, EMDR has established itself as the treatment of evidence in the most classical and most serious of disorders directly related to stress, that is, PTSD (Bisson & Andrew, 2007) and were highlighted its effects on the CNS. Despite this and its progressive use in a wide variety of situations are still few reflections from the perspective sull'EMDR mind-body integration, in particular using data provided by the search of PNEI. In this work we will focus on some of these issues to highlight how EMDR is one of the therapies are more responsive to new scientific evidence and the needs of an integrated science of health. Three main elements of research are: 1. Life has shaped the evolutionary highly integrated structures, we describe how systems complex feedback network continues with the context. In particular, the nervous, endocrine immune network and provide psycho in a "super-system" of relationship and adjustment able to ensure the balance incorporating adaptive expertise and managing the dynamic maintenance-change ( "regulation allostatica"). Then the body is a machine made of autonomous parts, assembled together, but a reality that leaves the unit (zygote) and maintains that unitary in all its joints. 2. Stress is not something in itself exceptional and pathological, but must be seen - in contrast -- as the expression of a set of processes (a system) that modulate the individual-regulation framework for adaptive. In essence, the system of stress is always on and only in situations of excess or deficiency (acute or chronic) effects progressively damaging to the individual. The Stress-related activity produces a "load allostatico" (weight biological energy) that -- relation to subjective factors and external - can become "overloaded" by triggering a chain of subsequent alterations in the functioning of the organism. In parallel there is a "load" and a "Overload psychological" processes related to the first round. 3. In the network system and body of stress, or in the process of setting general internal and individual-context, the mind plays a crucial role as a dimension of modulation in which signals (internal and external) are transformed into information (assigning meanings) depending on the paths and individual needs of adaptation. The mind is so the highest expression of the evolution of life and its adaptive strategies, which explains its role in relation to the overall functioning of the organism. The recent acquisitions of neuroscience have shown the interdependence of psychic activity, gene expression and plasticity brain that makes the role of the mind. Overload allostatico and psychological stress-related depend in large part by activism mental and constitute the main risk factor for health in Western societies, antecedents established disease spreading. If we look to the foregoing view of personal development (stage intrauterine attachment process, significant events later) we can have an idea of how mind, body and environment interact in the structuring the individual and the role conditioning and de-structuring of situations and events that cause an alteration by stress. Research on the relationship between stress and psychobiological development, including mode of attachment, structure of brain circuits, or between them and health, confirmation of these given (Lazzari, 2007). Coming closer all'EMDR, psychobiological studies of PTSD are showing that we are dealing essentially an event (especially complex and pronounced) the effects of stress on the network psycho (Iribarren et al., 2005). Research on PTSD (and other stress disorders DSM) must necessarily cross the strands study mentioned above and use the conceptual framework that arises. Psychobiological effects of extreme stress and trauma should be classified within mind-body interactions and regulation allostatica (load vs. overload allostatico). In our view, beyond the specific effects on the CNS, EMDR has been shown effective in PTSD in connection with his characteristic broad research and use as a "target" rather than individual aspects of an experience, but experience in its different aspects: cognitive, emotional, physiological and relational. Knowledge of sensory development in pregnancy can inform the treatment plan for mother and baby subsequently. The impact of domestic violence and traumatic birth will be explored. If untreated in the mother there can be long lasting effects in the mother, child and the parent child relationship. Clinical examples will explain how EMDR can be modified to treat unresolved traumatic events. In infancy and early childhood memories are stored in sensory form often with little language. We will illustrate on video a narrative approach combined with parent facilitated EMDR in a traumatised 30 month old infant whose parents have a history of drug use. The impact of traumatic stress on the developing brain will be discussed and illustrated by video of two EMDR sessions with 10 and 12 year old children. This will show how the normal EMDR protocol must be modified to take childhood factors into account.

In his book, Anxietv Disorders and Phobias, Aaron Beck, MD, wrote cogently about the so-called "evaluation anxieties." He employed the metaphor of the tightrope walker to describe the constant worry about a possible "fall from grace" experienced by the person troubled by concern about performing well in a variety of life situations. Beck divided these situations into three categories: social situations; school and work settings; and what he called "transactions with the outside world," meaning instances of shopping and traveling. The focus of this three-hour presentation is evaluation or performance anxiety (as it is more often termed) in the workplace and applications of EMDR to removing blocks to optimal functioning at work. The theoretical contribution of Beck and his colleagues will shape the presenters' information about why performance anxiety develops and who is likely to be vulnerable to it. The rationale for this extension of the EMDR model will be grounded in the theoretical framework of cognitive therapy. The two presenters bring their considerable experience with EMDR (five+ years) and expertise in peak performance consulting to participants in this session. From their background, they will derive the presentation's emphasis on EMDR applications that are immediately useful to the participants. To begin, the presenters will provide the aforementioned theoretical understanding of "evaluation anxiety" and its manifestation as performance anxiety in the workplace. Having established this basis of understanding, the presenters will move on to describe the most commonly observed psychological blocks that impede optimal performance in work settings: 1) external conflicts brought into work; 2) feeling like an 'impostor' in one's position at work, 3) perfectionism as a barrier to performance; 4) past failures that operate as anticipatory anxieties (for example, a client's worry that a past mistake or setback might recur in the future); 5) discrimination on the basis of gender, race, ethnicity, sexual orientation, education or age. Using actual transcripts and videotaped excerpts of their sessions, the presenters will demonstrate how their EMDR interventions may be applied. The presenters will show participants how to assess their own current and prospective clients for the psychological blocks that are interfering with work performance. Efficient ways to elicit negative and positive cognition for these work-related issues will be precisely described. The expected course of the EMDR processing will be illustrated using the presenters' cases which include a perfectionistic dentist, a high level executive after a layoff, a female manager desiring a promotion who is grappling with a chauvinistic boss, and performing artists struggling with stage fright and other barriers to their optimal performance. The presenters will then share with participants the ways in which they assist their clients in reaching and maintaining balance in their life- and work styles, in what the presenters call "Using EMDR to meet daily challenges with optimal response." Specific strategies for integrating EMDR into broader-based interventions will be described for assisting clients in: 1) increasing self-trust; 2) learning to capably manage crises; 3) increasing focus and attention at work; and 4) setting priorities and using time effectively. Lastly, Drs. Foster and Lend will demonstrate additional EMDR applications for assisting clients in attaining and maintaining what the presenters call "Optimal Well-Being." Citing case material, the presenters will show participants the means by which EMDR can be employed to speed recovery from illness and to decrease the rehabilitation time needed following an injury. Forty-three individuals diagnosed with classic or common migraine headache were randomly assigned to either phase 1 of integrated eye movement desensitization reprocessing (EMDR) treatment or a standard care medication treatment. Integrated EMDR combines diaphragmatic breathing, cranial compression, and EMDR for abortive migraine treatment. The comparison standard care medication group received various abortive medications, including Demerol, DHE, oral triptans, Excedrin, Fiorinal, Percocet, Toradol, and Vicodin. Participants were treated during mid- to late-stage acute migraine and assessed by an independent evaluator at pretreatment, posttreatment, 24 hours, 48 hours, and 7 days for migraine pain level. Both standard care medication and integrated EMDR treatment groups demonstrated reduced migraine pain levels immediately at posttreatment, 24 hours, 48 hours, and 7 days. Objectives: Eye movement desensitization and reprocessing(EMDR) is a novel, time-limited psychotherapy originally developed for treatment of psychological trauma. The effectiveness of this therapy has been validated only for posttraumatic stress disorder; however, EMDR is often applied to other psychiatric illnesses, including other anxiety disorders and depression. This pilot study tested the efficacy of EMDR added to the routine treatment for individuals with acute stage schizophrenia. Methods: This study was conducted in the acute psychiatric care unit of a university-affiliated training hospital. Inpatients diagnosed with schizophrenia were randomly assigned to either three sessions of EMDR, three sessions of progressive muscle relaxation(PMR) therapy, or only treatment as usual(TAU). All the participants received concurrent typical treatments(TAU), including psychotropic medication, individual supportive psychotherapy and group activities in the psychiatric ward. The Positive and Negative Syndrome Scale(PANSS), the Hamilton Depression Rating Scale and the Hamilton Anxiety Rating Scale were administered by a clinical psychologist who was blinded to the patients' group assignment. Results: Forty-five patients enrolled and forty patients(89%) completed the post-treatment evaluation. There were no between-group differences in the withdrawal rates of patients during the treatment or at the three-month follow-up session. All three groups improved significantly across each of the symptomatic domains including schizophrenia, anxiety, and depressive symptoms. However, a repeated measures ANOVA revealed no significant differences among the groups over time. Effect size for change in total PANSS scores was also similar across treatment conditions, but effect size for negative symptoms was large for EMDR(0.60 for EMDR, 0.39 for PMR and 0.21 for TAU only). Conclusion: These findings supported the use of EMDR in treating the acute stage of schizophrenia but the results failed to confirm the effectiveness of the treatment over the two control conditions in three sessions. Further studies with longer courses of treatment, more focused target dimensions of treatment, and a sample of outpatients are necessary.

Background: Post-traumatic stress disorder (PTSD) is highly prevalent in children and adolescents who have experienced trauma and has high personal and health costs. Although a wide range of psychological therapies have been used in the treatment of PTSD there are no systematic reviews of these therapies in children and adolescents. Objectives: To examine the effectiveness of psychological therapies in treating children and adolescents who have been diagnosed with PTSD. Search methods: We searched the Cochrane Depression, Anxiety and Neurosis Review Group’s Specialised Register (CCDANCTR) to December 2011. The CCDANCTR includes relevant randomised controlled trials fromthe following bibliographic databases: CENTRAL (the Cochrane Central Register of Controlled Trials) (all years), EMBASE (1974 -), MEDLINE (1950 -) and PsycINFO (1967 -). We also checked reference lists of relevant studies and reviews. We applied no date or language restrictions. Selection criteria: All randomised controlled trials of psychological therapies compared to a control, pharmacological therapy or other treatments in children or adolescents exposed to a traumatic event or diagnosed with PTSD. Data collection and analysis: Two members of the review group independently extracted data. If differences were identified, they were resolved by consensus, or referral to the review team. We calculated the odds ratio (OR) for binary outcomes, the standardised mean difference (SMD) for continuous outcomes, and 95% confidence intervals (CI) for both, using a fixed-effect model. If heterogeneity was found we used a random-effects model. Main results Fourteen studies including 758 participants were included in this review. The types of trauma participants had been exposed to included sexual abuse, civil violence, natural disaster, domestic violence and motor vehicle accidents. Most participants were clients of a traumarelated support service. The psychological therapies used in these studies were cognitive behavioural therapy (CBT), exposure-based, psychodynamic, narrative, supportive counselling, and eye movement desensitisation and reprocessing (EMDR). Most compared a psychological therapy to a control group. No study compared psychological therapies to pharmacological therapies alone or as an adjunct to a psychological therapy. Across all psychological therapies, improvement was significantly better (three studies, n = 80, OR 4.21, 95% CI 1.12 to 15.85) and symptoms of PTSD (seven studies, n = 271, SMD -0.90, 95% CI -1.24 to -0.42), anxiety (three studies, n = 91, SMD -0.57, 95% CI -1.00 to -0.13) and depression (five studies, n = 156, SMD -0.74, 95% CI -1.11 to -0.36) were significantly lower within a month of completing psychological therapy compared to a control group. The psychological therapy for which there was the best evidence of effectiveness was CBT. Improvement was significantly better for up to a year following treatment (up to one month: two studies, n = 49, OR 8.64, 95% CI 2.01 to 37.14; up to one year: one study, n = 25, OR 8.00, 95% CI 1.21 to 52.69). PTSD symptom scores were also significantly lower for up to one year (up to one month: three studies, n = 98, SMD -1.34, 95% CI -1.79 to -0.89; up to one year: one study, n = 36, SMD -0.73, 95% CI -1.44 to -0.01), and depression scores were lower for up to a month (three studies, n = 98, SMD -0.80, 95% CI -1.47 to -0.13) in the CBT group compared to a control. No adverse effects were identified. No study was rated as a high risk for selection or detection bias but a minority were rated as a high risk for attrition, reporting and other bias. Most included studies were rated as an unclear risk for selection, detection and attrition bias. Authors’ conclusions: There is evidence for the effectiveness of psychological therapies, particularly CBT, for treating PTSD in children and adolescents for up to a month following treatment. At this stage, there is no clear evidence for the effectiveness of one psychological therapy compared to others. There is also not enough evidence to conclude that children and adolescents with particular types of trauma are more or less likely to respond to psychological therapies than others. The findings of this review are limited by the potential for methodological biases, and the small number and generally small size of identified studies. In addition, there was evidence of substantial heterogeneity in some analyses which could not be explained by subgroup or sensitivity analyses. More evidence is required for the effectiveness of all psychological therapiesmore than one month after treatment.Much more evidence is needed to demonstrate the relative effectiveness of different psychological therapies or the effectiveness of psychological therapies compared to other treatments. More details are required in future trials in regards to the types of trauma that preceded the diagnosis of PTSD and whether the traumas are single event or ongoing. Future studies should also aim to identify the most valid and reliable measures of PTSD symptoms and ensure that all scores, total and sub-scores, are consistently reported.

Sexual abuse survivor couples who choose to engage in marital therapy often present with problems around attachment, intimacy, infidelity, rage, a sense of entrapment, feelings of betrayal, low self-esteem, powerlessness, codependency, and a need to control or have power. Their individual histories become critical to understanding what type of interventions to implement as these individuals continue to respond to one another in an almost stylized and predictable manner. This chapter describes a treatment approach that combines Structural Family Therapy (SFT) and Eye Movement Desensitization and Reprocessing (EMDR) in marital therapy when one or both partners have a history of childhood sexual abuse. In this approach, the therapist begins with SFT and then shifts to EMDR treatment of the traumatized partner. This shift is to process the survivor's abuse experience so that he or she can come to an adaptive resolution. This sets the stage for the survivor to respond differently to the possible triggers in his or her life as well as in the relationship. Once the EMDR process is complete and the couple participates in joint debriefing of the EMDR intervention, they reengage in the SFT marital sessions while integrating insights and adaptations the trauma survivor has gained from the EMDR work. This approach involves the applications of the EMDR standard protocol. It also uses the core elements of SFT, such as joining, restructuring diffuse and rigid boundaries, relabeling, and enactments. Introduction: The prevalence of male sexual dysfunction in its various forms reaches the age of almost 90% in some stage of life. Among them, Erectile Dysfunction affects key aspects of the man and may have its origin in causes psychological, physical or mixed. Note the scientific investments to better understanding and treatment of these disorders, not always successful. The objective of this project is to compare cases diagnosed and referred by the urologist in treating erectile dysfunction, one group treated with EMDR method and handled exclusively by another doctor. Patients will undergo a structured interview and tests before and after treatment. Methods: A comparative study of two groups with a medical diagnosis of erectile dysfunction, undergoing medical treatment. The study group will be treated with EMDR and control, just by the doctor. Before and after the treatment of inventories to respond to Beck Anxiety, depression and hopelessness as well as a more specific questionnaire about their sexual dysfunction, International Index of Erectile Function (IIEF). Las estadísticas acerca del maltrato físico de los niños son alarmantes. Se estima que cientos de miles de niños han recibido abuso y maltrato a manos de sus padres o parientes. Los que sobreviven el abuso, viven marcados por el trauma emocional, que perdura mucho después de que las consecuencias físicas hayan desaparecido. A menudo el daño emocional severo a los niños maltratados no se refleja hasta la adolescencia, o aún más tarde, cuando muchos de estos niños maltratados se convierten en padres abusivos y comienzan a maltratar a sus propios hijos. El reconocer y dar tratamiento inmediato es importante para minimizar los efectos a largo plazo causados por el abuso o maltrato físico. En este sentido, en los últimos años venimos trabajando con menores que han sido víctimas de abuso, negligencia, violencia familiar o abandono. El abordaje terapéutico desde el EMDR se muestra como un paradigma muy eficaz a la hora de trabajar con niños y menores. El trabajo con la red de recuerdos y el reprocesamiento de recuerdos traumáticos ayuda a su recuperación. A través de los dibujos que los niños han ido haciendo en las sesiones se observa como el reprocesamiento en el niño es diferente al del adulto. Además la etapa del desarrollo del niño interfiere tanto en el modo de procesar información traumática como en la construcción de los recuerdos y su posterior reprocesamiento. Por lo que los protocolos de EMDR en el niño son diferentes a los del adulto para que sean igualmente eficaces. En esta comunicación se presenta el protocolo de trabajo para tratar a los menores víctimas de abuso, desarrollado desde la perspectiva de EMDR, exhibiendo cómo funciona el protocolo a través de la presentación de los datos que se han ido recogiendo durante la aplicación del programa de intervención.

The statistics on physical child abuse are alarming. It is estimated that hundreds of thousands of children are physically abused by their parents or relatives. Those who survive abuse, living marked by the emotional trauma remains long after the physical consequences are gone. Often the emotional damage severely abused children is not reflected until adolescence or even later, when many of these abused children become abusive parents and begin to abuse their own children. Early recognition and treatment is important for minimize long-term effects caused by abuse or physical abuse. In this regard, in recent years we have been working with children who have been victims of abuse, neglect, family violence or neglect. The therapeutic approach from the EMDR appears as a very effective paradigm for working with children and minors. EMDR is being successful in treatment of individuals with affect dysregulation from early traumatic experience included prenatal and birth traumatic experiences. There is a body of scientific investigation and knowledge on prenatal experience and trauma, and that indeed we do experience and are influenced by our environment in the womb. We can learn from such experience, and therefore, can be traumatized prior to birth. The prenatal self can feel and record this experience. Some researchers point that there is a pre-­‐traumatic experience at the embryonic stage that could let somatic memories in the brain. In some individuals, reactivation of this pre-­‐traumatic experience provokes some clinical disorders as specific phobia. We present two clinical cases of phobia treated with EMDR, one of them is a child with fireworks phobia and the other one is an adult with agoraphobia.

EMDR ha sido un tratamiento exitoso en individuos con desregulaciones desde una experiencia traumática temprana, incluyendo los traumas prenatales y del nacimiento. Existe un cuerpo de investigación científica y conocimiento acerca de la experiencia prenatal y el trauma, y de hecho, tenemos experiencias que están influenciadas por nuestro ambiente en el útero. Podemos aprender de ese tipo de experiencias y, por tanto, ser traumatizados antes del nacimiento. El yo prenatal puede sentir y grabar esta experiencia. Muchos investigadores señalan que existe una experiencia pre-­‐traumática en el estado embrionario que permite los recuerdos somáticos en el cerebro. En algunos individuos, la reactivación de esta experiencia pre-­‐traumática provoca algunas patologías clínicas y fobias específicas. Presentamos dos casos clínicos de fobia tratada con EMDR, uno de ellos es un niño con miedo a los petardos y el otro es un adulto que sufre de agorafobia.

The knowledge on the treatment of psychological trauma in Thailand is still in its rudimentary stage despite the fact that the country has been known for its highly trained medical personnel who are capable of providing sophisticated medical treatment such as plastic surgery, high quality dental care, and sex-change operation. And although the problem of violence has been well recognized, especially relating to child abuse and domestic violence, a more subtle form of violence such as bullying in school has been largely ignored. The author aims to present the summary from his survey of bullying incidence from over 3,000 students in all regions of Thailand. The problem appears to be wide-spread and resulted in fear and anxiety about going to school. During the past few years a number of bullying survivors have come forward to receive treatment for their traumatic memories that continue to trouble them. The presentation will outline treatment strategies for this client population and cite several case studies which may show how EMDR can be applied in a way that is unique to Thai culture.

Patients exhibiting chronic dissociative symptoms are among the most complicated patients to be treated in psychotherapy. Their treatment tends to be a multi-faceted approach of which EMDR is only one part, albeit, a very important component. The objective of this course is to assist the treating clinician in identifying the chronic dissociative patient, conceptualizing and implementing an effective treatment plan for the patient. This report summarizes the treatment of an attempted murder-robbery victim using EMDR. Although she responded rather poorly to the medications, the patient showed a rapid improvement with the introduction of six 90-minute EMDR sessions, as measured by the Impact of Event Scale - Revised (IES-R) and Clinician Administered PTSD Scale (CAPS). Of particular interest was that the intrusion and hyperarousal symptoms disappeared in the early stage of the treatment; in contrast, the avoidance symptoms needed several sessions for management. EMDR has been used and known to be effective in performance psychology area. Although there have been studies for performance enhancement with EMDR, specific studies regarding performance enhancement of actors are limited. In this presentation, a new study with EMDR for performance enhancement with professional actors will be proposed. 45 professional actors who were acting on stage and in front of the camera with a minimum 3 years of experience were the subjects of the current study. A survey was designed and used in order to evaluate the subjective performance level (SPL) before and after the end of the sessions. Due to the performance evaluation of the actor, two different protocols was used. The protocol for performance enhancement with EMDR was applied to 23 actors and the protocol for cracking performance blocks with EMDR was applied to 22 actors. The results of the survey suggest that both the protocols have affected the subjective performance level of the actors, and EMDR may be effective means of providing performance enhancement of professional actors. The case samples will be presented and the theoretical issues concerning the current study will be discussed.

Objective: The purpose of the study is to show the impact of the use of EMDR in survivors of suicide bomb blasts in North of Pakistan. Design and Settings: The study involves an ongoing compilation of clinical data and the study of therapeutic responses to various interventions including EMDR, at a tertiary mental health facility and Centre for Trauma Research and Psychosocial Interventions (CTRPI), Rawalpindi /Islamabad, Pakistan. This mental health facility is the catchment area of patients from Northern areas of Pakistan, currently the part of the country, worst affected by series of suicide bombings targeting military and civil population. Method: Families of the victims and those who survive suicide bombings without physical injuries are referred to CTRPI from peripheral areas / hospitals for assessment for psychosocial consequences of facing a man made disaster. Patients are interviewed at the point in time of referral and scoring is done on Impact of Event Scale (IES). Those who fulfill the criteria of Post traumatic Stress Disorder according to ICD-10 are registered for further studies and appropriate interventions. The individuals who fulfil the criteria for PTSD or any other psychiatric morbidity are then enrolled for regular psychiatric follow up. The patients are first offered the use of EMDR and all who give an informed consent are then assigned to a psychiatrist trained in EMDR (Level 2). Sessions of EMDR as per the protocol of 8 stages are carried out. Scoring on IES is recorded serially. According to the degree of improvement and severity of illness, sessions of EMDR are carried out using the bilateral stimulation during the hospital stay. Results: The three individuals who have completed EMDR treatment had survived the suicidal bombing attacks and fulfilled the entry criteria were administered 8 stage protocol EMDR. They all improved in their symptoms of intrusive images, hyper-arousal, autonomic instability and avoidance. Their sleep improved and nightmares diminished. Their social and interpersonal functioning improved. There was marked reduction of basal anxiety levels in all three. Scores on IES done after intervention (EMDR) improved from initial pre EMDR score of 41, 38 and 40 respectively to post EMDR scores of 18, 15 and 14 for the three subjects who completed EMDR protocol of 8 stages. On reporting to their respective units their occupational effectiveness has returned to previous levels of functioning. Conclusions: EMDR proves to be an effective non pharmacological intervention in terms of post traumatic stress disorder in special circumstances of acts of terrorism involving suicide bombing. The data presented is only preliminary and is based on a small number out of a larger sample.

EMDR can be used in the treatment of complex dissociative disorders. Both presenters have been treating dissociative disorders for decades and one is an approved EMDR trainer. This workshop will not include instruction on specific techniques: these can be learned from approved EMDR trainings which require six full days of workshop teaching, assigned readings, and 10 hours of supervision. Instead, the presenters will explain how EMDR is based on a trauma-dissociation model and is therefore highly suited to the treatment of complex dissociative disorders including DID. A brief description of EMDR will be provided, including its 8 phases, of which only one involves eye movements or other forms of bilateral stimulation. EMDR is consistent with three-stage models of trauma therapy: the eye movements are used in stage two, the active working phase of therapy. In EMDR this is called Phase 4. The work in trauma stage one (EMDR phases 1-3), in patients with DID or DDNOS, involves grounding, stabilization, system mapping, building co-consciousness, orientation of parts to the body and the present, and other elements from the dissociative disorders literature. The bilateral stimulation phase of EMDR should not be used until this phase one work is complete, or at least well underway. The presenters will then go on to provide case examples of how EMDR can be used in the psychotherapy of DID, DDNOS and the complex comorbidity that usually accompanies both.

Abstract: I am presenting the case of a 13-year-old boy who was referred because of uncontrolled rage evident in his relationship with peers in one incident he had a fellow pupil’s head under water until he was gasping for air.. He had shown little remorse towards the boy and described the incident as funny. The family had experienced a series of traumatic events. They were forced to evacuate their home when criminal gangs attempted to burn them out; his was to use the house for drug-related activity. Gang members had assaulted my patient and his mother in the street after going to the police. After meeting with the parents and brother and older sister, I had identified that on top of this recent trauma L had extreme hostility towards his mother who he described as “that woman.” My initial EMDR sessions were with L and his father. During processing of the trauma, my video will show bizarre movement including rolling his head back, hypotonic posture and behavior similar to an infant. This can be understood in relation to L’s early development. He was born at 33 weeks gestation and spends his first 6 weeks in a special care baby unit (CBU) in an incubator. My presentation will discuss the neurological consequences of early trauma and how a narrative approach using EMDR can help in recovery. This approach is based on the work of Dr. Joan Lovett. She is a California-based pediatrician who has developed a protocol for working with children who have experienced significant trauma following premature birth.
My second case is J, a 7-year-old boy who witnessed his father murder his mother approx 1 year ago. He was then held hostage by his father for 6 hours with the body of his mother lying in a pool of blood in the kitchen while the police negotiated with J’s father for his release. My first involvement with J was an in depth assessment of his emotional attachment and placement needs for the court. During this process, he developed frequent infantile rages and I advised the system of care around J on their management. At one stage, he had recurrent dreams about joining his mother in heaven and he was discovered attempting to strangle himself. I will discuss how I used the safe place protocol to alleviate his distress. I am now using EMDR directly with J. I use a variety of bilateral alternating stimuli. These include drumming, musical symbols, and a xylophone. I am able to get J to draw sequentially with each new drawing generated by J performing the “Butterfly hug.” I was inspired to use this technique after I attended a wonderful workshop facilitated by Michel Silvestre entitled “integrating family therapy and EMDR.” I hope to discuss in this presentation how EMDR can be combined with other therapeutic approached in a case of extreme trauma experienced by a 6-year-old boy who is now effectively orphaned as his father is in prison probably for the rest of his natural life. I will also allude to some of the techniques discussed by Dr. Atle Dyregrov at the 5th annual UK and Ireland conference in London in March last year. He presented in depth therapeutic work with a girl who suffered the trauma of her mother’s suicide. I will discuss some of the challenges presented by traumatic grief and how the EMDR protocol can be adapted for use in children.

When all you have is 90 minutes with the hysterical and/or traumatized client in times of crisis, efficient planning and conceptualization of EMDR casework is essential.
Using EMD(R) as a means of reducing immediate exacerbated stress is effectively enhanced with a genogram format that is brief, and specifically focuses on learned generational reactions to trauma. By tapping into the trans-generational information the processing in crises is facilitated and enhanced.

A career soldier, who had fought in both Gulf wars and was suffering from PTSD and alcoholism, experienced a severe panic attack whilst watching the opening scenes of the film “Saving Private Ryan”. After 8 sessions of EMDR he asked if he would be able to watch the film now – without a panic attack. We decided to watch it together and used the first five minutes of the film to shape a unique therapy session encompassing grief work, the completion of the trauma work we had been doing with EMDR and the use of EMDR to enable him to begin to address the imminent life stage issue of retirement from the military and how he was to make meaning for himself from his experience. This is a case study which demonstrates the immense potential and flexibility of EMDR if we are able to respond creatively to our clients. This session is an invitation to think about how we can use EMDR creatively, remaining true to its protocols at the same time as being able to integrate it into our previously existing skills and respond flexibly and appropriately to our clients needs and suggestions. It is an example of creative and collaborative working with the client.
