Posttraumatiskt stressyndrom är en komplex och mångdimensionell störning.
Lyckligtvis finns det effektiva behandlingsmetoder. 1989 utvecklade
Francine Shapiro metoden, sedermera namngiven, Eye Movement Desensitization
and Reprocessing (EMDR), för behandling av traumatiska minnen.
Det hävdas att mellan 84 – 100% som behandlats med EMDR inte längre
uppfyller diagnoskriterierna för PTSD. Uppsatsens huvudsyfte är att göra en
grundläggande granskning av EMDR och metodtillämpningen i Sverige.
Datainsamling skedde genom litteraturgranskning, enkätundersökning samt
intervjuer med EMDR-utbildade kliniker. Resultatet visar att EMDR är effektivare
än ingen behandling alls eller icke-validerade behandlingsmetoder,
såsom aktivt lyssnande. Det är däremot inte styrkt att EMDR är effektivare
än KBT. Vidare antyder komponentstudier att ögonrörelsestimulering, eller
alternativ bilateral stimulering, inte är nödvändig för att uppnå behandlingsframgångar.
Samtliga intervjuade utövare upplever EMDR som ett värdefullt
och verksamt verktyg i behandlingsarbetet. Diskussion förs kring resultaten
från effektstudierna och nödvändigheten av fortsatt forskning.
Post-traumatic stress disorder is a complex, multidimensional disorder.
Fortunately there are effective treatments. 1989 Developed
Francine Shapiro method, later named, eye movement desensitization
and Reprocessing (EMDR), for the treatment of traumatic memories.
It is alleged that between 84 - 100% treated with EMDR are no longer
meet diagnostic criteria for PTSD. The main purpose is to make a
fundamental review of the EMDR method and application in Sweden.
Using data obtained from literature review, survey and
interviews with EMDR-trained clinicians. The results show that EMDR is more effective
than no treatment or non-validated therapies,
such as active listening. It is however not shown that EMDR is more effective
than CBT. Further studies suggest component of eye movement stimulation, or
alternative bilateral stimulation, it is not necessary to achieve treatment success.
All interviewees experienced EMDR practitioners as a valuable
and effective tool in the treatment process. Talk out of the result,
from efficacy trials and the need for further research.

Abstract:
Objective: To explore the possibility of using eye movement desensitisation and reprocessing (EMDR) to treat women who have experienced post-traumatic stress after childbirth. Design: The pilot study consisted of a "before and after" treatment design combined with follow-up measurements 1-3 years after EMDR treatment. Quantitative data from questionnaires (Traumatic Event Scale [TES]) were collected. In addition, qualitative data from individual interviews with the participants were collected as well as data from the psychotherapist's treatment notes of the EMDR treatment sessions. Setting: The north of Sweden. Participants: 4 women with PTSD after childbirth (1 pregnant and 3 non-pregnant). Findings: All participants reported reduction of post-traumatic stress after treatment. After 1-3 years, the beneficial effects of EMDR treatment remained for 3 of the 4 women. Symptoms of intrusive thoughts and avoidance seemed most sensitive for treatment. Implications for Practice: EMDR might be a useful tool in the treatment of non-pregnant women severely traumatised by childbirth; however, further research is required. [Author Abstract]

Vid vårt novembermöte presenterade leg. psykolog Gunvor Ingemansson sin erfarenhet av behandling med EMDR-Eye Movement Desensitization and Reprocessing vid enstaka och komplexa traumatillstånd.
Under 90-talet arbetade Gunvor Ingemansson inom psykiatrin i Malmö med Rosengård som upptagningsområde. Där mötte hon många människor, som hade svåra och traumatiserande krigsupplevelser. Hon använde sig av krisbearbetande samtal och symboldramaterapi. Dessa arbetssätt hade hon lång erfarenhet av. Hon märkte emellertid att hon inte riktigt kom åt traumat i botten.
Pröva nytt
Detta ledde till att hon måste pröva något annat. 1994 hade metoden EMDR kommit till Sverige och introducerats av Roger Salomon. Han i sin tur hade en förstahandserfarenhet av metoden, eftersom han utbildats och samarbetar med Francine Shapiro, som skapat metoden. Hennes grundläggande bok är ”Eye Movement Desensitization and Reprocessing”, utgiven på Guilford förlag. Shapiro har senare benämnt metoden ”Adaptive Information Processing Model”. I vardagligt språk i Sverige benämns metoden ofta som ögonrörelseterapi.
Shapiro utvecklade ett sätt att arbeta med traumatiska erfarenheter till följd av egna upplevelser. Hon drabbades nämligen av en svårbotad cancer, som läkaren inte kunde ge besked om hur den skulle utvecklas. Hon lade märke till att när hon under sina promenader rörde ögonen på ett visst sätt hände någonting i hennes kropp. Hon undersökte detta vidare och successivt växte den behandlingsmetod fram som i dag spridit sig till många länder för att behandla bl a traumatillstånd.
Det finns på sina håll ett visst ifrågasättande av metoden, men de evidensbaserade studier som gjorts visar på effekt av behandlingen.
Nya behandlingsmetoder.
At our November meeting presented the leg. Psychologist Gunvor Ingemansson his experience with EMDR-Eye Movement desensitization and Reprocessing of single and complex trauma condition.
During the 90's worked Gunvor Ingemansson in psychiatry in Malmö with Rosengard that catchment area. There she met many people who had difficult and traumatic war experiences. She used the emergency call processing and symbolic drama therapy. These practices, she had extensive experience. She noticed that she does not really come to the trauma of the bottom.

This led her to try something else. 1994 Approach EMDR come to Sweden and introduced by Roger Solomon. He in turn had a first hand experience of the method, since he trained and working with Francine Shapiro, who created the method. Her basic book is "Eye Movement desensitization and Reprocessing", published by Guilford publishers. Shapiro was later called the method "Adaptive Information Processing Model". In everyday language in Sweden method is referred to often as eye movement therapy.
Shapiro developed a way to work with traumatic experiences resulting from their own experiences. She suffered from a severe namely cured cancer, the doctor could not provide information on how it would evolve. She noticed that when she walks in their eyes moved in a certain way something happened to her body. She investigated this further and gradually grew the treatment up to now spread to many countries to treat conditions including trauma.
There are some places a particular challenge to the method, but the evidence-based studies show the efficacy of treatment.

Abstract:
This is a case presentation regarding the treatment of a severely traumatized woman
formerly treated for depression and PTSD following incarceration in prison,
“disappearance” of husband, and torture. After psychotherapy for several years, the
patient improved and started to work in a qualified job. After some years, however, the
patient returns because she has a feeling that the therapy was unfinished, and because of
remaining psychosomatic symptoms, difficulties breathing, obesity, overeating, and
recurrent urinary tract infections. The therapist then decided to try the resource installation
protocol. However, in an impulse, he asked her to concentrate on her bodily sensations.
Several video-recorded sequences illustrate how the patient, seemingly for the first time in
her life, discovered and re-experienced childhood trauma. It seems that the eye
movements during attempts at EMDR treatment made it possible to lift repression and
dissociation as well as to make processing possible, thus liberating the patient from a
heavy burden of mental and psychosomatic symptoms. At follow-up by the second author,
the patient is entirely asymptomatic, with low DES scores and is no longer obese.

Abstract:
Föreningen EMDR Sverige vill lämna följande kommentarer till
utkastet till riktlinjer avseende behandling av akut stressyndrom
och posttraumatiskt stressyndrom.
International Society for Traumatic Stress studies gör regelbundet sammanställningar
av evidensläget [1]. Det är viktigt att beakta att psykologiska
behandlingsformer i form av traumafokuserad KBT och EMDR är de viktigaste
och mest effektiva behandlingar. EMDR har varit kontroversiell men
är det inte längre, utan är en internationellt accepterad behandlingsmetod för
PTSD. Det är fortfarande inte allmänt accepterad att ögonrörelser har betydelse,
men nyligen har svensk och australisk forskning visat att ögonrörelser
under EMDR har tydliga fysiologiska effekter som är meningsfulla [2, 9],
dessutom har alla studier av fysiologi vid EMDR hittills samstämmande
visat dessa effekter, enligt en litteratursammanställning [3].
Under senaste åren har forskning om minnesfunktion och sakkadiska
ögonrörelser visat att minnessystem som till exempel episodminne (som ofta
är störd vid PTSD) påverkas på ett gynnsamt sätt av ögonrörelser [4-8]. Således
finns det i dag mycket som stödjer att ögonrörelser är meningsfulla
även om det kan vara svårt att förstå vid första anblicken. EMDR och exponeringsbehandling
är lika effektiva enligt metastudier, bland annat Cochrane
och i ISTSS aktuella genomgång av effektiva behandlingsmetoder för
PTSD.

EMDR Association of Sweden makes the following comments to
Draft Guidelines for the treatment of acute stress disorder
and post-traumatic stress disorder.
International Society for Traumatic Stress Studies makes regular summaries
of evidence-mode [1]. It is important to note that psychological
treatments in the form of trauma-focused CBT and EMDR is the most important
and most effective treatments. EMDR has been controversial but
it is no longer, but is an internationally accepted method of treatment for
PTSD. There is still no generally accepted that eye movements are important,
but lately, Swedish and Australian research has shown that eye movements
during EMDR has clear physiological effects that are meaningful [2, 9],
Moreover, all studies of physiology at the convergence of EMDR to date
shown these effects, according to a literature review [3].
In recent years, research on memory function and Sakka wash
eye movements showed that memory systems, such as episodic memory (which is often
is disturbed in PTSD) is affected in a favorable way of eye movements [4-8]. Thus
today there are a lot of support that eye movements are meaningful
although it may be difficult to understand at first glance. EMDR and exposure therapy
are as effective as meta-studies, including Cochrane
and in ISTSS current review of effective treatments for
PTSD.

Abstract:
This is a case study of a 17 year old adolescent who came in contact with Child and adolescent psychiatry at an inward basis, screening for dissociation gave very high dissociative symptom on Dis-Q-Sweden; 3.6 total scale, 3.88, 4.00, 2.64 and 3.5 on the subscales, she also had high scores on Trauma Symptom Checklist for Children. After screening a SCID- interview was done and she had symptoms of amnesia, derealizaition, depersonalization and identity confusion.
We will describe the psychotherapy with this adolescent girl, different stages of therapy individual work – with tf-cbt- EMDR and symboldrama-, family work with much work with not before worked with traumatic experiences. In the presentation we will connect to attachment theories of dissociation, dissociation in a generational perspective and theories of multimodal approach to dissociation.
