Numerous methods have attempted to repair distress remaining from very early trauma (especially adoption) or neglect, but most have been difficult, cumbersome, even traumatic. The presenter will describe and demonstrate a direct, simple, safe and incredibly effective EMDR
extension, which is based on our current scientific knowledge of early brain development. Participants will (1) learn how to be able to adapt the Basic EMDR protocol when treating preverbal experiences with infants, children, adolescents and adults; (2) what language to use when accessing and repairing preverbal experiences: (3) be able to use "imaginative

This workshop addresses the application of standard and modified EMDR treatment protocols to addictive and compulsive
behaviors including substance abuse/dependence, overeating, smoking, love addiction. Individuals with addictive and compulsive
behaviors frequently have suffered from childhood trauma and neglect resulting in developmental arrests, as well as a variety of
maladaptive behaviors which are trauma-related and serve to minimize pain. The successful implementation of EMDR to addictive
behaviors requires that EMDR be used as part of an overall treatment program carefully addressing the needs of individuals who
have been traumatized and are exhibiting addictive behaviors. A thorough diagnostic work up is needed aimed at assessing
comorbidity, dissociation, and a detailed trauma history covering childhood traumas and traumas suffered as adults including
traumas that occur as a consequence of addictive behaviors. Careful client preparation is essential to assist individuals in coping
adequately with the high levels of emotion experienced during EMDR Clients' readiness to stop compulsive/addictive behaviors
needs to be carefully evaluated.
A decision tree aimed at determining the appropriateness of EMDR to individuals diagnosed with addictive behaviors is presented
which assists clinicians in minimizing the premature use of EMDR. EMDR is a client centered method, and thus, careful pacing is
needed with this population to reprocess underlying traumatic issues. This frequently implies utilizing a modified EMDR treatment
protocol with only partial resolutions of underlying traumatic material. Guidelines will be discussed to assist clinicians in selecting
EMDR targets for optional results which relate to the stages of recovery. EMDR can be used at all stages of recovery to neutralize
the negative impact of memories contributing to problematic behaviors, such as urges to use, ambivalence about treatment, fear of
facing painfull feelings from the past. EMDR also has the power to install templates for future actions which assist individuals with
skill deficits in more rapidly acquiring necessary skills for a successful recovery. Examples of cognitive interweaves are presented
which take into consideration clients' readiness, as well as the need to accelerate the recovery process.
EMDR has a unique role in the recovery of traumatized individuals with addictive and compulsive behaviors since the accelerated
processing of negative experiences and the installation of positive adaptive cognitions assist clients in more rapidly overcoming
barriers throughout the recovery process. It also challenges rigid approaches to recovery which frequently stress that trauma work
should not be attempted before abstinence has been accomplished for a specified period of time. EMDR is especially valuable in
processing core issues which center around shame and manifest in cognitions, such as "I am defective," "There is something wrong
with me," "I am not good enough," "I am not quite right," "I don't belong," "I don't deserve to live." Case examples will be given as
to how such core issues can be targeted to accelerate the recovery process.
A.J. Popky has developed a specialized EMDR treatment protocol which targets levels of urges of addictive/compulsive behaviors
directly and installs a positive internal state of feeling empowered without relying on compulsive and addictive behaviors. Case
examples fiom clinical practice indicate that when levels of urges are targeted directly, underlying traumas frequently emerge
without increasing clients' usage. The symposium addresses the application of this protocol to a range of addictive and compulsive
behaviors.
The Wades' integrative psychotherapy combines ego-state therapy and EMDR in a psychosocial developmental context. Their
substance use disorders treatment program incorporates specialized applications of their integrative psychotherapy, which includes
both individual and group therapy and employs hypnosis as well as EMDR Their presentation focuses on applications of the
standard EMDR protocol in individual therapy, which is limited primarily to desensitization of dysphoric affect and reprocessing
negative cognitions associated with grief and trauma.
Their conceptual framework of substance use disorders proceeds from a goal of reducing the harm caused by substance use and a
primary distinction between functional and autonomous use (rather than the DSM conceptualizations of "dependence" or "abuse")
because this guides interventions. Initial treatment planning depends upon external constraints (e.g., lack of support for positive
change, hostile environment), internal limitations (e.g., severity of substance use and its effects, neurocognitive deficits, inadequate
"ego strength," lack of skills, disrupted psychosocial development, psychological trauma) and the nature of the substance use
disorder (i.e., functional, autonomous, or both).
Methods include education about substance use disorders and processes of change, group therapy to develop skills and obtain
feedback and support, individual therapy to correct disrupted development and resolve traumatic stress reactions, and exercises to
apply what is learned in real-life situations. The standard EMDR protocol is applied to disrupted development involving grief and
to resolve psychological trauma that lead to substance use. Case vignettes in which such applications of the standard EMDR
protocol were employed are presented in detail.

Infants (0-4 jarigen) kunnen allerlei traumatische gebeurtenissen meemaken zoals intrusieve medische handelingen, een ongeluk, seksueel misbruik en andere vormen van mishandeling. Daarnaast zijn er negatieve ervaringen die niet duidelijk identificeerbaar zijn zoals chronische oorpijn- of buikpijn. Ook deze ervaringen beinvloeden de ouder-kind interactie/ hechtingsrelatie en hebben daarmee hun weerslag op het vermogen tot emotieregulatie en het gedrag.
Herinneringen aan deze gebeurtenissen zijn preverbaal en dus niet op bewust nivo toegankelijk. Daardoor worden zij gemakkelijk over het hoofd gezien als mede-oorzaak of instandhoudende factor van emotionele of gedragsproblematiek in de basisschoolleeftijd. Na aanmelding bij de Geestelijke Gezondheidszorg wordt behandeling daarom vaak gericht op de aanpak van de huidige gedragsproblematiek (symptoombestrijding). Onze ervaring is dat EMDR hier een goede aanvulling biedt. Deze methode richt zich immers op de ‘onderlaag’ van de problematiek door negatieve ervaringen die hieraan gerelateerd zijn te verwerken. Daarna kan een inhaalslag gemaakt worden met betrekking tot de emotieregulatie waardoor zowel gedrag als de ouder-kind interactie verbeteren.
Aan de hand van casuïstiek van infants en schoolkinderen wordt de indicatiestelling en toepassing getoond van het EMDR protocol bij de behandeling van kinderen die in de eerste 4 levensjaren getraumatiseerd zijn. In de presentatie wordt geillustreerd hoe de ‘verhalenmethode’ geintegreerd kan worden in een breder behandelaanbod.

Infants (0-4 years), all kinds of traumatic experience as intrusive medical procedures, accidents, sexual abuse and other forms ofof abuse . There are also negative experiences that are not clearly identifiable as chronic ear pain or abdominal pain. These experiences affect the parent-infant interaction / attachment relationship and thus have their impact on the capacity for emotion regulation and behavior.
Memories of these events are preverbal and not accessible on a conscious level. Thus they are easily overlooked as a cause or co-maintaining factor of emotional or behavioral problems in primary school. After reporting to the Mental Health Treatment is therefore often aimed at addressing the current behavioral problems (symptoms). Our experience here is that EMDR provides a good addition. This method is focused on the 'layer' of the problem by negative experiences related to this process. Then caught up with regard to both behavior and emotion regulation allowing the parent-child interactions improve.
Through case studies of infants and schoolchildren being shown the indication and application of the EMDR protocol in the treatment of children who are traumatized life on April 1. The presentation illustrated how the 'stories'method can be incorporated into a broader range of treatment.

Progress in neuroscience reveals that attachment learning, affect dysregulation, and traumatic experience are predominantly held in the right hemisphere (Schore, 2009). Therapists can access trauma and neglect held in the right hemisphere using EMDR, somatic awareness, imaginal excursions, and the therapists mirror neuronal experience. This workshop summarizes the Early Trauma approach of EMDR (O'Shea, 2009; and its variations for the dissociative client (Paulsen, in press). Of note is a new stabilization procedure that appears to work directly on the subcortical affective circuits (Panksepp, 1998).
The Early Trauma approach to EMDR processes preverbal implicit memory in sequential time periods, while attending to the nuances of somatic and affective experience as they emerge both in the clients subjective report and in the relationship field. As the infants story is told with the deciphering of these nuances, the therapy repairs developmental milestones imaginally. This enables integration from the bottom up, by time frame, called Temporal Integration (Paulsen, 2009), which contrasts with Strategic and Tactical Integration. Preliminary clinical findings are that the procedure helps repair injuries of attachment, structural dissociation, affect dysregulation and personality. The workshop will interest both EMDR and non-EMDR practitioners because of its significant implications for theory and practice

All too often it is assumed that infants or young children are too young to remember frightening, painful, and confusing experiences, and therefore will not suffer harm because they are too young to be traumatized. Clinical experience; however, as well as research on neurobiology of trauma, child development, and attachment, tell a different story. While very often not verbalized, overwhelming early experiences are nonetheless remembered implicitly. As such they tend to find expression in a myriad of debilitating emotional and somatic symptoms across a persons life span; even into adulthood. Understanding how early trauma can affect the child developmentally, and how it might be expressed, is crucial not only to clinicians treating children but also to those working with adults. It is through understanding early traumatic markers and the subtle, often coded signs, sometimes rooted as far back as infancy, that one can begin the process of untangling these symptoms and freeing the individual from a lifetime of pain, suffering, and failed relationships. This workshop will examine the types of early traumamedical, severe neglect, all forms of abuse, disrupted attachmentand how these traumatic experiences impact the young child from birth to five years. The signs and symptoms of traumatized and dissociative children, as well as available research on the topic will be described, along with case examples. The need for careful interpretation of often convoluted and subtle symptoms will be explained. Accurate interpretation of a childs presentation relies on understanding the neurobiology of trauma and dissociation. That, as well as understanding the impact on child development and attachment is crucial for effective intervention. Strategies such as Eye Movement Desensitization Reprocessing (EMDR) and involvement of caregivers in the therapy will be described. Despite growing evidence that all treatments intended to be therapeutic (i.e., bona fide
treatments) are equally efficacious, the question of relative efficacy persists. In fact, cognitivebehavioral
treatments (CBT) have gained a more favorable status over non-CBT treatments for
adult anxiety disorders. However, the assertion that CBT treatments are superior is premature
due to conceptual and methodological issues affecting the extant CBT research. This metaanalysis
addressed these limitations by consensually identifying CBT treatments and determining
the true relative efficacy of bona fide CBT and bona fide non-CBT treatments for adult anxiety
disorders. The study employed strict inclusion criteria to identify randomized clinical trials that
contained at least one direct comparison of a bona fide CBT treatment and a bona fide non-CBT
treatment. Additionally, 91 CBT experts from the Association of Behavioral and Cognitive
Therapists (ABCT) were surveyed to identify the bona fide treatments as CBT or non-CBT.
Thirteen clinical trials met inclusion criteria. CBT treatments and non-CBT treatments were
found to be equally efficacious across targeted and non-targeted outcome measures. Additional
analyses revealed that researcher allegiance did not account for the significant heterogeneity. The
results are consistent with the increasing evidence for uniform efficacy among treatments
intended to be therapeutic, and stand in contrast to assertions for the superiority of CBT
treatments for adult anxiety. This meta-analysis contributes to the growing body of research
revealing that a particular therapeutic approach is not more effective than another treatment
when intended to be therapeutic.

Prior meta-analyses have suggested that eye-movement desensitization and reprocessing (EMDR) may be effective in alleviating the symptoms of post-traumatic stress disorder (PTSD). EMDR is now being recommended as a treatment for military combat veterans who suffer from PTSD. We provide a review of published outcome studies that appeared in print from 1987 - April, 2008 which examined the specific effects of EMDR on PTSD among military combat veterans. Studies were identified through electronic bibliographic databases, web sites, and manual searches of article reference lists. A total of six randomized controlled trials (RCTs) and three quasi-experimental studies met our inclusionary criteria and are reviewed. The evidence supporting the use of EMDR to treat combat veterans suffering from PTSD is sparse and equivocal, and does not rise to the threshold of labeling the therapy as an empirically supported treatment. It is premature to incorporate EMDR into routine care for veterans to alleviate combat-related PTSD. EMDR needs a considerably stronger evidentiary foundation which includes large-scale RCTs involving credible placebo controlled treatment conditions. Copyright © 2009 John Wiley & Sons, Ltd.

Las ideas que las personas tienen de una cirugía están impregnadas de fantasías con una excesiva carga de ansiedad que impiden un adecuado control emocional y proceso de recuperación. Una intervención quirúrgica implica un desequilibrio biológico y psicológico que requiere que la persona vuelva a lograr su estabilidad.
El presente trabajo tiene como objetivo verificar que el método del EMDR (Desensibilización y reprocesamiento a través de movimientos oculares) es eficiente en la psicoprofilaxis quirúrgica logrando reducir estas ansiedades y temores en los niños de 5 a12 años del Servicio de Cirugía del Hospital de Niños Baca Ortiz que tendrán que enfrentar un acto quirúrgico. A través de la identificación de los recursos necesarios para hacer frente a cada una de estas situaciones, con el EMDR se procede a instalarlos usando estimulación bilateral y acoplándolos con la correspondiente creencia positiva permitiendo que el infante consiga una exitosa evolución pre y post-quirúrgica.
Se trata de una investigación correlacional cuasi-experimental en la que se seleccionó una muestra infantil con los criterios de inclusión y exclusión, que son infantes de 5 a 12 años que requieren ser operados, y que cumplan los criterios para el tratamiento con EMDR; donde se demuestra que el 100% de los niños estudiados presentan un alto nivel de ansiedad antes de la cirugía y que luego de aplicarse este método psicoterapéutico esta sintomatología disminuye llegando a niveles bajos, observándose una evolución favorable en su recuperación.
Se considera por lo tanto que el EMDR es un método eficiente en la psicoprofilaxis quirúrgica infantil.
ABSTRACT.
The ideas that people have about a surgery are impregnated with fantasies and an excessive burden of anxiety that impede an adequate emotional control and recovery process. Surgery involves biological and psychological imbalance that requires a person to be able to achieve stability again.
The present work aims to verify that the method of EMDR (Eye Movement Desensitization and Reprocessing) is efficient in reducing these pre surgical anxieties and fears in children patients from 5 to12 years old at Children's Hospital Baca Ortiz who will face a surgical procedure. Through the identification of resources needed to address each of these situations, EMDR is appropriate to install them using bilateral stimulation and coupling them with the corresponding positive belief allowing the child to get a successful development of pre-and post-surgical procedure.
This is a quasi-experimental correlational research in which a sample was selected according to the criteria of inclusion and exclusion, infants that are 5 to 12 years who require surgery, and who meet the criteria for treatment with EMDR; which shows that 100% of the children studied, had a high level of anxiety before surgery and then applying this psychotherapeutic method these symptoms decreased to low
levels, showing a favorable trend in his recovery. It is considered therefore that the EMDR is an effective method in child surgery preparation.

Cet article est le fruit d’une étude personnelle basée sur
une technique psychothérapique nouvelle, l’Eye-
Movement Desensitization and Reprocessing (EMDR),
appliquée à la prise en charge de l’éjaculation précoce.
Le protocole thérapeutique a comporté trois consultations
d’une heure, à trois semaines d’intervalle environ,
précédées d’une première consultation visant à expliquer
la méthode et recevoir l’accord des patients. Parmi
11 cas traités, 8 ont vu s’améliorer leur sexualité (le critère
d’amélioration était le passage à une durée de rapport
intravaginal « acceptable » pour les deux partenaires,
avec disparition de l’anticipation négative).
Deux patients n’ont remarqué aucun changement. Le
dernier a dû interrompre son traitement pour une raison
non liée à celui-ci. Cette expérience pilote est encourageante,
et encourage à poursuivre cette recherche sur un
échantillon plus large de patients.
This is the report of a pilot study of Eye-Movement
Desensitization and Reprocessing (EMDR), a new psychotherapeutic
method initially proposed as treatment
of Post-Traumatic Stress Disorder, in 11 men with
Premature Ejaculation. After an initial visit aiming to
explain the principles and modalities of this therapy,
and to collect the patients’ consent, each man attended
3 therapeutic sessions of one hour duration at 3 weeks
interval. Eight of the 11 patients reported an increase in
the duration of vaginal penetration till a length acceptable
for both partners, and a disappearance of their
negative anticipation. These results are encouraging,
and justify to continue this research on a larger sample
of patients.

Hoe kan je preverbaal trauma verwerken, zodat er ruimte ontstaat voor herstel, inhalen en voortgang van de ontwikkeling?
Ook infants (0-4 jarigen) kunnen allerlei traumatische gebeurtenis(sen) in de eerste levensjaren meemaken zoals intrusieve medische handelingen, seksueel misbruik en andere vormen van mishandeling. Herinneringen aan deze gebeurtenissen zijn preverbaal en niet op bewust nivo toegankelijk. Bij deze jonge kinderen is het dan ook moeilijk te zien in hoeverre er sprake is van traumatisering. Door hun beperkte cognitieve ontwikkeling is verbale communicatie over ingrijpende gebeurtenissen niet of beperkt mogelijk. Soms zie je na enige tijd gedragsveranderingen bij dagelijkse handelingen die eerder geen probleem vormden (verzet bij verschonen, bij tandenpoetsen e.d.). Dit gedrag kan echter ook gekoppeld zijn aan de ontwikkelingsfase. Het diagnostisch beeld wordt duidelijker wanneer gedragsveranderingen (verzet, verdriet maar ook submissie!) zich voordoen bij soortgelijke traumatische ervaringen zoals nieuwe medische behandelingen. De link naar de onverwerkte traumatische gebeurtenis(sen) ligt dan voor de hand.
De ouder kan soms uit angst of schuldgevoel over de ingrijpende gebeurtenis(sen) niet goed meer als steunfiguur en opvoeder optreden. Door een klachtbestendigend interactiepatroon kunnen gedragsproblemen van het kind zelfs verergeren. De omgeving gaat denken aan ADHD of ASS...
Aan de hand van casuïstiek van infants bij wie sprake is van traumatisering worden de aanpassingen aan het EMDR protocol getoond bij de behandeling van 0 tot 4 jarigen. Pas na de verwerking van de traumatische herinneringen wordt echt duidelijk wat de invloed is geweest op het verloop van de ontwikkeling van het kind, zoals een sociaal emotionele achterstand, vertraagde spelontwikkeling etc. Er komt ruimte voor herstel, inhalen en voortzetten van de ontwikkeling. Zo nodig wordt een EMDR traject voor ouders ingezet om de behandeling af te maken.

How do you handle preverbal trauma, so there room for rehabilitation, catch up and progress of the development?
Even infants (0-4 year olds) can all traumatic event(s) in the first years of life experience as intrusive medical procedures, sexual abuse, and other forms of abuse. Preverbal memories of these events, not consciously accessible level. In these young children it is therefore difficult to see how there is trauma. Because of their limited cognitive development, verbal communication on major events is not possible or limited. Sometimes you see after a while behavioral changes in daily operations which were previously not a problem (resistance to changing, with teeth, etc.). This behavior can also be linked to the development. The diagnostic picture becomes clearer when behavioral change (resistance, but also sadness Submission!) arise from similar traumatic experiences such as new medical treatments. The link to the unprocessed traumatic event (s) is then obvious.
The parent can sometimes out of fear or guilt about the traumatic event(s) not functioning properly to support action figure and educator. By klachtbestendigend interaction pattern may even worsen the child's behavior. The environment is reminiscent of ADHD or ASD ...
Based on case reports of infants with trauma,0 to 4 years old treated with the EMDR protocol show changes only after the processing of traumatic memories is really clear that the impact has been on the course of the development of the child as a social-emotional retardation, slow game development etc. There is room for recovery, overtaking and continued development. If necessary, an EMDR process for parents is used to finish the treatment.

The effect of early interpersonal trauma on adult clients has been extensively studied and reported. Less attention has been
given to the effects of complex trauma on children. Because childhood is the time of most rapid brain development as well as the time when
children are dependent on adult caregivers, early physical, sexual, and emotional abuse, including neglect and witnessing domestic violence,
has severe effects on infants and children – disorganized attachment, affect and behavioral dysregulation, and dissociation. In this preconference
Sandra Wieland will describe how early trauma and neglect affects the infant/child’s brain, autonomic nervous system, and body
system. She will explain how dissociation is often used by children to protect themselves from the impact – both emotional and physical – of
complex trauma. Given this high level of fragmentation and fright, children are often not able to participate successfully in the standardized
EMDR protocol. Dr. Wieland will teach ways in which the EMDR protocol can be adapted in order to access more completely the child’s
fragmented internal system. She will use clinical vignettes to describe ways EMDR can assist in each of the three stages of trauma treatment
– safety/stabilization, trauma processing, and integration. Maria Elena Aduriz will describe a case of a girl whose traumatization is the direct
consequence of a disorganized attachment to a schizophrenic, unstable and suicidal mother. Through this case she will focus on the importance
of learning how to articulate EMDR interventions with the child and her adoptive parents.

Objective: Eye movement desensitization and reprocessing (EMDR) has been claimed effective in the treatment of a wide variety of psychiatric disorders, including eating disorders. An informal survey suggests that EMDR is now widely offered to patients with eating disorders. Before accepting a new therapy such as EMDR, one must determine that its benefits outweigh its adverse effects. This paper reviews the literature in an attempt to assess the benefits and risks of the use of EMDR in the treatment of eating disorders. Method: We reviewed the literature on the use of EMDR to treat eating disorders and other conditions. Results: Looking at the question of its benefits, we were unable to find any methodologically sound studies that have shown efficacy for EMDR in eating disorders, or, indeed, any psychiatric disorder. We were also unable to find a sound theoretical basis for expecting EMDR to be effective. In addition, EMDR may have adverse effects. First, EMDR is sometimes used in conjunction with efforts to "recover" memories of traumatic events. But "recovered memory" therapy may carry a risk of inducing potentially harmful false memories. Second, use of EMDR may prevent or delay other therapies of established efficacy for eating disorders, such as cognitive behavioral therapy and antidepressants. Discussion: In light of the findings of our review, the risk/benefit ratio of EMDR does not as yet encourage its widespread acceptance.

The watch wait and wonder (www) approach to parental and infant mental
health was developed in Toronto over the last 20 years. In the last few years a number of
therapists have set up www clinics in the UK. We have been running a joint EMDR and www
clinic in the North West since Jan 2007. As far as we know this is the only clinic to offer
trauma focussed psychotherapy together with infant mental health in either the UK or
Ireland.
Infant brain development occurs sequentially implying vulnerabilities during the early years
of life. This has a direct relationship on attunement, which is influenced by developmental
neglect and trauma. The effects of this trauma for EMDR therapy are explored. New
information about the probable mechanism of action of the bilateral stimulation used in
EMDR is presented. The mechanism of translating sensory input into new neuronal patterns
is explained. The hypothesis of a ‘window of tolerance’ in trauma is mentioned with the
reasons why EMDR can help alleviate trauma by expanding the window of tolerance.
An additional model of applying EMDR is outlined. This involves multilateral activation of
sympathetic nervous systems to consciously overcome traumas. This process will be shown
to lead to brainstem stimulation activating the cranial nerves, which innervate the ocular
muscles generating rapid eye movement. This approach involves motor and sensory
integration at the level of the 12 cranial nerves. Some case examples using bilateral olfactory
desensitisation are discussed.
Recommendations for the development of parent infant mental health services within the
NHS are outlined

The couple was assaulted by four men, one appeared to be a minor. They forced the couple into their own car and raped the wife, forcing the husband to watch the rape under gunpoint.
With both parents assaulted and raped, the family reported a history of sexual trauma and underwent an EMDR therapy in addition to Bowen theory.
They presented the following symptoms: The wife: episodes of panic, depression, insomnia and nightmares, anorgasmia and vaginismus. The husband: anxiety disorder, insomnia, intrusive negative thoughts, premature ejaculation and erectile difficulties. The children: Larissa - difficultues in sleeping and concentrating in her studies. Yago - nocturnal enuresis and difficulty sleeping alone in his bedroom.
The EMDR standard protocol was used to clear the trauma within the relationship as well as with outside relationships. Experiences from before and after the rape were also targeted, as well as differentiation in the couple, including unsatisfactory sex.
There were nine encounters, during nine weeks, with an average of three hours each.
Follow up data from the couple was obtaained after six months.

Dissociative Identity Disorder (DID), a chronic childhood onset posttraumatic stress
disorder (PTSD), is currently recognized as a treatable condition. It is considered the
paradigmatic dissociative disorder and carries with it extreme posttraumatic
symptomatology which lends itself to be an apt target for treatment combining the use of
EMDR and hypnosis. Therapists skilled in the treatment of DID and Dissociative Disorder
Not otherwise specified (DDNos) have augmented their therapeutic arsenal with the fluent
uses of hypnosis for stabilization, affect management, building a safe place and
grounding. EMDR, which has come to the forefront of clinical awareness in the last fifteen
years, seems well suited for the treatment of trauma, but can be destabilizing. The
presenters recommend a protocol, called Wreathing Protocol, for the imbricated use of
EMDR and hypnosis in the treatment of not only DID, but also DDNOS and complex
chronic PTSD. The Wreathing Protocol is useful to advanced clinicians skilled in both
modalities independently. This workshop will explain and illustrate with a clinical vignette
the sequential steps of the Wreathing Protocol; it will describe the required contextual
treatment frame for its safe use. The presenters will explore the consequences of the
premature uses of EMDR and offer planful structured responses to a disequilibrated
treatment and a destabilized patient. The clinical implications of the use of the Wreathing
Protocol will be discussed.

Abstract: Attachment theory has identified adults with a secure attachment style as more resilient to stress and trauma than insecurely attached adults. The secure adult tends to have supportive relationships which buffer him from stress (Bowlby, 1988), and he is able to reflect upon his inner state and process emotions without becoming overwhelmed (Fonagy, 2000; van der Kolk 1996). In infancy secure attachment is related to the capacity to be soothed and comforted by the caregiver. The caregivers of secure infants are observed to be emotionally attuned, responsive and nurturing (Solomon & George, 1999). Schore (1996) explains that mothers of secure babies synchronize with the infant. The mother's organized brain synchronizes or harmonizes with the baby's disorganized brain, attuning to its feelings and needs, helping it calm when distressed and stay regulated when it is happy and excited. As a result, the infant's brain develops optimal circuitry for emotion regulation, and also learns through experience to trust and be comforted and to comfort himself. His basic sense of safety and trust gives him confidence as he begins leaving his parent's side and exploring the world.

Eye Movement Desensitization and Reprocessing (EMDR) is an effective treatment for posttraumatic stress disorder (PTSD). During EMDR, patients make eye movements (EMs) while recalling traumatic memories, but recently therapists have replaced EMs by alternating beep tones. There are no outcome studies on the effects of tones. In an earlier analogue study, tones were inferior to EMs in the reduction of vividness of aversive memories. In a first EMDR session, 12 PTSD patients recalled trauma memories in three conditions: recall only, recall + tones, and recall + EMs. Three competing hypotheses were tested: 1) EMs are as effective as tones and better than recall only, 2) EMs are better than tones and tones are as effective as recall only, and 3) EMs are better than tones and tones are better than recall only. The order of conditions was balanced, each condition was delivered twice, and decline in memory vividness and emotionality served as outcome measures. The data strongly support hypothesis 2 and 3 over 1: EMs outperformed tones while it remained unclear if tones add to recall only. The findings add to earlier considerations and earlier analogue findings suggesting that EMs are superior to tones and that replacing the former by the latter was premature.

Abstract:
Often it is said that traumatization in the preverbal
period cannot be treated. Doctors, psychologists, police officers
tell parents to wait until the child is older, so it can talk about
it. In this presentation we will demonstrate the contrary. That
EMDR is a very useful method for processing preverbal traumatic
memories in infants.
We will start by explaining how to structure the EMDR sessions,
illustrated by videotapes of boy (nearly three) and his parents
who was traumatized by medical treatments starting in the first
months of his life. We will explain how to access and activate
the traumatic memories, how to help the child during desensitization
by timing and dosing stimuli, and how to determine
whether the traumatic memory is completely processed.
Then we will illustrate the devastating consequences on the
development of an infant of preverbal traumatization. These
consequences usually are underestimated by parents, pediatricians and even psychologist. As these children get older, their
development is more and more disturbed and they behave like
children with ADHD of Autistic disorders, and sometimes even
get diagnosed.
After EMDR these infants start to recover and grow rapidly in
emotional en social development and the use of play. Their oppositional
behavior changes in daily life activities as changing
a diaper, brushing teeth, going to bed and changing clothes.
They become less resistant.
Parents see that the presumed characteristics of their child
change and looking back they recognize those as symptoms of
traumatization. This is even clearer with children who are
traumatized by medical treatment. They are usually referred to
EMDR therapist because of their resistant behavior in the hospital
which makes medical treatment impossible. The urgent requirement
of medical care is often the main reason for staking
treatment. The realization of the impact of the medical treatment
on their child is very painful for the parents. Nowadays parents are often asked to assist during these treatments and
they feel as perpetrators. If necessary we also offer parents
EMDR.
New and unique: The use of EMDR in infants is quite unique
because it requires knowledge of infant psychiatry and traumatization
and EMDR It is a very complicated but rewarding
treatment.
The aspect of traumatization and the use of EMDR in medical
treatment is very underestimated The material in this workshop
is new and unique for the use of EMDR in these children.
Learning objectives:

preverbal traumatization.

memories in infants and how to help the child during desensitization

Abstract: I am presenting the case of a 13-year-old boy who was referred because of uncontrolled rage evident in his relationship with peers in one incident he had a fellow pupil’s head under water until he was gasping for air.. He had shown little remorse towards the boy and described the incident as funny. The family had experienced a series of traumatic events. They were forced to evacuate their home when criminal gangs attempted to burn them out; his was to use the house for drug-related activity. Gang members had assaulted my patient and his mother in the street after going to the police. After meeting with the parents and brother and older sister, I had identified that on top of this recent trauma L had extreme hostility towards his mother who he described as “that woman.” My initial EMDR sessions were with L and his father. During processing of the trauma, my video will show bizarre movement including rolling his head back, hypotonic posture and behavior similar to an infant. This can be understood in relation to L’s early development. He was born at 33 weeks gestation and spends his first 6 weeks in a special care baby unit (CBU) in an incubator. My presentation will discuss the neurological consequences of early trauma and how a narrative approach using EMDR can help in recovery. This approach is based on the work of Dr. Joan Lovett. She is a California-based pediatrician who has developed a protocol for working with children who have experienced significant trauma following premature birth.
My second case is J, a 7-year-old boy who witnessed his father murder his mother approx 1 year ago. He was then held hostage by his father for 6 hours with the body of his mother lying in a pool of blood in the kitchen while the police negotiated with J’s father for his release. My first involvement with J was an in depth assessment of his emotional attachment and placement needs for the court. During this process, he developed frequent infantile rages and I advised the system of care around J on their management. At one stage, he had recurrent dreams about joining his mother in heaven and he was discovered attempting to strangle himself. I will discuss how I used the safe place protocol to alleviate his distress. I am now using EMDR directly with J. I use a variety of bilateral alternating stimuli. These include drumming, musical symbols, and a xylophone. I am able to get J to draw sequentially with each new drawing generated by J performing the “Butterfly hug.” I was inspired to use this technique after I attended a wonderful workshop facilitated by Michel Silvestre entitled “integrating family therapy and EMDR.” I hope to discuss in this presentation how EMDR can be combined with other therapeutic approached in a case of extreme trauma experienced by a 6-year-old boy who is now effectively orphaned as his father is in prison probably for the rest of his natural life. I will also allude to some of the techniques discussed by Dr. Atle Dyregrov at the 5th annual UK and Ireland conference in London in March last year. He presented in depth therapeutic work with a girl who suffered the trauma of her mother’s suicide. I will discuss some of the challenges presented by traumatic grief and how the EMDR protocol can be adapted for use in children.

This Point/Counterpoint concludes the interchange in Greenwald, R. and Shapiro, F. (2010) What is EMDR?: Commentary by Greenwald and Invited Response by Shapiro Journal of EMDR Practice and Research, 4, 170-179. Greenwald Rejoinder: In this rejoinder, I highlight areas of agreement between Shapiro and me that were obscured by Shapiro's (2010) response to my (Greenwald, 2010) commentary. I also address some of the erroneous statements made by Shapiro (2010) in her arguments against my positions. Finally, I summarize our disagreements, and again assert that until we have an empirical basis for preferring a particular theoretical model of eye movement desensitization and reprocessing (EMDR), it is premature for professional organizations to endorse Shapiro's model. Shapiro Response: In response to Greenwald, I again confine myself to addressing some of the errors and misconceptions in his arguments in relation to important aspects of EMDR therapy, theory, and research. Further, contrary to his assertion, there is already a sufficient empirical basis to support the preferential use of the adaptive information processing (AIP) model from which the EMDR procedures were formulated. His argument against this position is antithetical to the traditional process by which foundational models are challenged, refined, or replaced. Implications are salient to both training and practice.
