Nella presentazione verrà esposta l’evoluzione di un protocollo di ricerca in psicoterapia che considera l’andamento dell’alleanza terapeutica e del clima emotivo tra paziente e terapeuta, confrontati con la situazione clinica del paziente. Verranno inoltre presentati i primi dati di una ricerca condotta con gli stessi criteri e che prevede l’osservazione di psicoterapie con approccio terapeutico EMDR e di psicoterapie con approccio cognitivo-costruttivista. Le indicazioni ottenute saranno discusse attraverso riflessioni e stimoli per la ricaduta pratica.
In the presentation will be outlined the evolution of a research protocol that considers the trend in psychotherapy and emotional climate of the therapeutic alliance between patient and therapist, compared with the clinical situation of the patient. Will also be presented the first data of a survey conducted by the same criteria and requiring compliance with therapeutic approach to psychotherapy and EMDR psychotherapy with cognitive-constructivist approach. The indications obtained will be discussed through reflections and stimuli for relapse practice.

"Eye Movement Desensitization and Reprocessing (EMDR) is the name of the psychotherapeutic method that was developed by American psychologist Francine Shapiro (Shapiro 1998). Symposium on Anxiety Disorders: Diabetes, Mediators and Therapeutic Implications; (NSC 92-2825-C-002-072-H) Research background & aims: This study examined possible therapeutic
mechanisms of Eye Movement Desensitization and Reprocessing (EMDR) from
REM-sleep dependent memory reprocessing model (Shapiro, 1995; Stickgold, 2002)
that proposed that sleep induced change in associative memory via activation of weak
association during REM state and EMs functioned as REM sleep to integrate the
episodic memory of trauma into general semantic memory. The effect of EM on
change of semantic associations after saccadic bilateral eye movements was examined
in the present study.
Methods: Twenty two college students were recruited as participants. Via semantic
priming task, a 2 (eye movement condition: horizontal saccadic eye movements vs. no
eye movements) × 2 (strength of semantic association: strong vs. weak) between
subject factorial design was performed to examine the effect of EM on change of
strength of semantic association for negative words. Change of primeability of weak
associations for negative stimuli as opposed to that of strong ones under different EM
conditions was compared.
Results: The 2 × 2 ANOVA showed a significant main effect for EM and a
significant interaction effect of EM × strength of semantic association, indicating that
weak priming significantly exceeded strong priming after EM, while the opposite was
true after non-EM.
Conclusions: The results echoed REM-sleep dependent memory reprocessing model,
suggesting that EM in EMDR might reflect a shift in associative memory systems by
activating different strength of associations of negative semantic nodes for different
semantically related words. The implications of the results for psychotherapy and
future research possibilities are discussed.

(Shapiro, 1989a),但其可能涉及的療效機制之研究尚
少。本研究擬從工作記憶模式的角度來探討動眼作業在
EMDR 的療效機制。根據工作記憶模式(working memory
model),動眼作業可視為一種爭奪工作記憶中視覺和空
間訊息處理能力(VSSP)系統資源的干擾作業。因此,當
回憶創傷事件時,施以動眼作業可降低創傷受害者對於
創傷事件記憶的清晰程度,並降低患者對於創傷事件的
情緒反應,以達到某種程度的治療目的。實驗一(N = 120)
採動眼作業(有、無)× 圖片的性質(正向、負向)× 時
間(前測、後測)的三因子混合設計,依變項 為圖片
影像的清晰度與情緒感受度。實驗二(N = 120)採動
眼作業(有、無)× 刺激材料的性質(圖片、影片)× 時
間(前測、後測)的三因子混合設計,依變項同實驗一。
實驗一與實驗二的結果皆顯示,如同工作記憶模式的預
期,動眼作業造成VSSP 所同時處理的影像訊息的清晰
度降低,此結果與工作記憶模式的預期一致。另外,本
研究亦顯示動眼作業亦造成情緒感受度降低。上述結果
顯示動眼作業可能具有臨床治療上的功效,且可藉由工
作記憶模式瞭解其療效機制。本研究亦對結果所顯示的
意義與未來研究方向提出討論
Eye‐movement desensitization‐reprocessing therapy (EMDR) has recently been the new effective technique to treat post‐traumatic stress disorder and other disorders. Contrasted with other psychotherapies, eye‐movements are the novel component.
According to the working memory model, eye‐movements could reduce the vividness
of distressing images and the intensity of the emotion associated with the images. In
this study, we designed two experiments to test the effects of eye‐ movements on
vividness and emotionality of imagery. In Experiment 1, with positive and negative
photos as stimuli, the results showed that eye movements significantly reduced the
vividness and emotionality of traumatic images. In Experiment 2, with negative photos
and negative films as stimuli, we also found eye‐movements significantly reduced the
vividness and emotionality of traumatic images. The implications and future research
possibilities are discussed.

Symposium on Evidence-Based Research, in Taiwan; Section of Mental Disorders. (NSC 91-2413-H-009-SSS-); NSC 92-2413-H-002-024-); NSC 93-2413-H-002-002-) Eye Movement Desensitization and Reprocessing (EMDR; Shapiro, 1989, 1995,
2001) has recently been claimed to be effective in ameliorating the symptoms of
PTSD and phobic disorders. Eye movement (EM) was one of the crucial treatment
elements, of which the therapeutic mechanisms await elucidation. The aims of this
talk were to report series of studies from our lab regarding the role of EM in the
therapeutic mechanism of EMDR. Specifically, after EMDR procedure being briefly
described, the efficacy of EM and its process compared to exposure were addressed.
Further, the effects of EM on changes of emotionality and vividness of images or
autobiographic memories, and changes of amount and strength of semantic
associations were reported. Also, the issues regarding how EMDR might work,
whether EM did add anything beyond the mechanisms of pure exposure, and the
interplay of EM, valence of stimuli, and their presentation order were discussed.
Finally, the role of EM in terms of stimulus valence presentation order from
theoretical point of view and its clinical implications are proposed, and future
research possibilities are discussed.

The Absorption that allows an athlete, a surgeon, an astronaut or a musician to get into the Zone, i.e., to block out all distractions
unrelated to performance has been assessed by Tellegen, Csikszentmihalyi and others in self-report measures. It occurs relatively rarely at the very highest levels and is very elusive to achieve. Qualitatively speaking, it is the phenomena of being totally immersed in the activity with time moving slowly, senses being sharpened, but pain not recognized. Thoughts and images are clear and controllable while physical performance seems effortless and automatic. To measure this phenomenon accurately and completely is not possible in a dynamic state, but to shape it's appearance and to extend duration is essential in finite psychomotor skills like golf.
Physical preparation for performance is mentally grueling and fatiguing. If often results in trance-like, dis-associative and sometimes
dissociative states where depersonalization is a valuable technique to block out the intense suffering and pain associated with running, swimming or bicycling. The difficulty in sport is being able to switch in and out of full alertness for some strategic tasks while remaining in the dissociative state for endurance. The experience of flow, absorption and being in the zone is to harness power and ultimately unleash explosive yet finely titrated effort.
Concentration is the umbrella concept that also encompasses EMDR. The process of actively shifting eye focus from left to right while striving to hold an image or statement of emotionality is exceedingly difficult and ultimately beyond control. The combination of EMDR with neurofeedback is an innovative intervention that holds potential for greater impact in removing negative images of failed effort or in solidifying the recall of a successful effort. The neurofeedback serves to reinforce the development of greater mental stamina toward intensively focus comparable to zooming in a camera lens thus blocking out distractions and irrelevant stimuli. Enhanced quality of visualization is the desired outcome for the performance enhancement sport psychology consultant and his/her client.

entwickelt und seit 1989 als manualisiertes Therapieverfahren zur Behandlung von
Patienten mit Posttraumatischen Belastungsstörungen (PTSD) und anderen
traumabezogenen Symptomen eingesetzt. Die Grundvorgehensweise besteht darin,
dass der Patient in der Sicherheit einer haltgebenden therapeutischen Beziehung
eine Konfrontation mit seinen traumatischen Erinnerungen erlebt. Ziel der
Traumabearbeitung ist die Integration von kognitiven, emotionalen und körperlichen
Reaktionen auf das Trauma indem die Erinnerungen wiederbelebt, wahrgenommen
und verarbeitet werden. Anders formuliert, wird die durch das Trauma induzierte
Dissoziation wieder aufgehoben. Die in der traumatischen Situation unterbrochene
Verbindung zwischen Wahrnehmungen, Gedanken, Emotionen und

EMDR combines stimuli that evoke divided attention – e.g. eye movements – with exposure to traumatic memories. Our objective was to investigate psycho-physiological correlates of EMDR during treatment sessions. A total of 55 treatment sessions from 10 patients with PTSD was monitored applying impedance cardiography. Onset of every stimulation/exposure period (n = 811) was marked and effects within and across stimulation sets on heart rate (HR), heart rate variability (HRV), pre-ejection period (PEP) and respiration rate were examined. At stimulation onsets a sharp increase of HRV and a significant decrease of HR was noticed indicating de-arousal. During ongoing stimulation, PEP and HRV decreased significantly while respiration rate significantly increased, indicating stress-related arousal. However, across entire sessions a significant decrease of psycho-physiological activity was noticed, evidenced by progressively decreasing HR and increasing HRV. These findings suggest that EMDR is associated with patterns of autonomic activity associated with substantial psycho-physiological de-arousal over time. [Author Abstract]

Respiratory Sinus Arrhythmia: Insights into Autonomic Function in PTSD: Studies of cardiac responses to trauma-related cues have defined the mainstream of laboratory research in posttraumatic stress disorder. Examinations
of respiratory sinus arrhythmia now challenge the view that exaggerated sympathetic tone and reactivity provide a sufficient account of the autonomic abnormalities seen in this diagnosis.
Alterations in autonomic tone during trauma therapy with EMDR: It has been hypothesized that EDMR, by pairing stimuli that evoke divided
attention with exposure to trauma memories, elicits repetitive orienting
responses followed by enhanced parasympathetic tone, resulting in significant
within-session psychophysiological de-arousal. We monitored 10 standard
EMDR treatments for PTSD (55 sessions) with impedance cardiography.
Heart rate (HR), parasympathetic tone (RMSSD), sympathetic tone (PEP), and
respiration rate (RESP) were assessed. Markers were set at the onset of every
stimulation/exposure period (N = 811). Effects within and across stimulation
sets were examined. An orienting response, with associated sharp increase
of parasympathetic tone and significant decrease of HR, was found at stimulation
onsets. During ongoing stimulation, sympathetic arousal increased
while parasympathetic tone decreased, responses consistent with stressrelated
arousal during trauma exposure. However, across entire sessions
there was a significant pattern of psychophysiological de-arousal, evidenced
by progressively decreasing HR and increasing RMSSD.

According to the latest statistical evidence Post-Partum
Depression develops in approximately 13% of women
during the second -third month after childbirth with symptoms
lasting between few weeks and a year and risks of relapse.
Unlike the Baby Blues (affecting 70% of mothers, with onset in
the 3'd - 6" day after delivery and spontaneous recovery within
approximately two weeks), likely to be caused basically by hormone
modifications in the immediate aftermath of childbirth.
PPD development would seem to be solely determined by psychological
factors: the experience of childbirth, the surfacing of
unresolved problems in the relationships with attachment figures,
the change in the woman's role both in the social sphere and
within the couple relationship, the fear of being unable to adequately
attend to the new responsibilities (both in terms of skills
and of the ability to cope with the additional workioad), etc.
Consequently, women experiencing childbirth as a traumatic
experience are more destabilized by the event, and therefore.
at a higher risk of developing PPD.
Childbirth requires the deployment of many personal resources.
A woman in labor must be able to bear the pain, while having
to "push", 1.e. contrast the automatic antalgic reaction (which
would close the delivery channel) and "meeting the pain", during
the "expulsion" phase. Considering that "Peak Performances"
require moving out of a person's comfort zone and
stretching a person's boundaries, childbirth experience can be
rightfully considered a "Peak Performance".
This work describes RDI application times and modes during Delivery
Preparation in order to strengthen the different personal
resources needed by pregnant women to experience her childbirth
as an ego syntonic experience. In this sense, RDI associated
with EMDR can be considered an actual Primary Prevention intervention,
capable of teaching women something positive about
themselves, thus effectively offsetting the onset of PPD. Furthermore
the results of the application of this technique collected
during the Post-Partum phase on 48 women will be discussed.

This workshop focuses on the clinical application of EMDR with specific phobias. There are a number of advantages in using EMDR for the treatment of specific phobias compared to other approaches, such as exposure in vivo. One of the most important advantages seems to be the possibility to utilize EMDR under circumstances where the critical elicitors cannot be reproduced or simulated in real life (e.g., certain sexual, illness or death situations) or, more generally, for which phobic stimuli are hard to obtain. However, unlike patients suffering with PTSD, after deconditioning of an anxiety associated with a traumatic incident, phobic individuals till have to anticipated future anxiety evoking situations. This has important implications for treatment. For example, contrary to its application with PTSD, the treatment of specific phobias with EMDR should not be concluded until clients are prepared for future interactions with (former) anxiety-eliciting stimuli or situations. This may involve acquisition of adaptive coping skills, such as mental strategies to relax or to distract oneself, and the use of exposure (in vivo) techniques. Participants of this workshop will learn when and how to apply EMDR with specific phobias and integrate this into general (cognitive-behavioral) treatment approach. This approach is illustrated by several videotaped treatments.

Introduction: The aim of this study was to clarify the effect of bilateral eye movement as one of the important therapeutic
components through EEG analysis. We examined differences between pre-bilateral eye movement and post-bilateral eye
movement of normal healthy volunteers in scalp recorded EEGs in two different ways. First, we used qEEG to examine
differences in specific band frequencies after bilateral eye movement. Secondly, we utilized low-resolution electromagnetic
tomography (LORETA) source imaging (Pascual-Marqui et al., 1994) to explore the underlying neural generators of changed
EEG activity. Methods:32 college students participated in the study. EEG was recorded continuously during the experiment.
All participants experienced 3 blocks: 1) pre-experiment, eye-closed EEG in resting state, 2) stimuli condition (either eye
movement or fixation) and 3) post-experiment, eye-closed EEG in resting state. 32-channel, eyes-closed EEG (30 artifactfree
s/subject) was analyzed (source localization using FFT approximation and LORETA). Results: We demonstrated different
functional connectivity patterns of the precuneus/posterior cingulate cortex (Brodmann area 23, 31) between bilateral eye
movement and eye fixation phase. Conclusion: These results provided evidence to support that the bilateral eye movements
in EMDR procedure gives rise to the changes of ‘brain default network’, accompanied by the alteration of regional brain
electrical activity.

Explanations for the effects of the rapid eye movements induced during Eye Movement Desensitization Reprocessing (EMDR; Shapiro, 2001) have drawn upon an analogy with the eye movements of REM sleep (Kuiken, Bears, Miall, and Smith, 2002). An extension of that analogy posits two orienting systems, one involving threat-fear related mnemonic contextualization and another involving loss-pain related monitoring of conflicting response alternatives. In a study involving individuals who had recently experienced significant loss or trauma, we found that experimentally induced saccadic eye movements decreased reaction times to unexpected stimuli among those reporting traumatic distress (characterized by hyperarousal and intrusive thoughts) and increased reaction times among those reporting separation distress (characterized by vivid reminiscences and the sense of a foreshortened future). Also, we found that saccadic eye movements increased the perceived strikingness of metaphoric sentence endings among those reporting amnesia for events related to either loss or trauma. The eye movements of both EMDR and REM sleep may differently affect the attentional and cognitive reorienting activity of those living with the consequences of loss or trauma. These differences may be evident in their waking reflections and in their dreams.

Ever since people's responses to overwhelming experiences have been systematically explored, researchers have noted that a trauma is stored in somatic memory and expressed as changes in the biological stress response. Intense emotions at the time of the trauma initiate the long-term conditional responses to reminders of the event, which are associated both with chronic alterations in the physiological stress response and with the amnesias and hypermnesias characteristic of posttraumatic stress disorder (PTSD). Continued physiological hyperarousal and altered stress hormone secretion affect the ongoing evaluation of sensory stimuli as well. Although memory is ordinarily an active and constructive process, in PTSD failure of declarative memory may lead to organization of the trauma on a somatosensory level (as visual images or physical sensations) that is relatively impervious to change. The inability of people with PTSD to integrate traumatic experiences and their tendency, instead, to continuously relieve the past are mirrored physiologically and hormonally in the misinterpretation of innocuous stimuli as potential threats. Animal research suggests that intense emotional memories are processed outside of the hippocampally mediated memory system and are difficult to extinguish. Cortical activity can inhibit the expression of these subcortically based emotional memories. The effectiveness of this inhibition depends, in part, on physiological arousal and neurohormonal activity. These formulations have implications for both the psychotherapy and the pharmacotherapy of PTSD.[MLM MEDLINE]

EMDR’s basic working theory assumes that traumatic memories remain unprocessed because the innate information processing system is stuck by the psychophysiological effects of trauma. Traumatic events are stored in their original form, and the recall of traumatic memories causes a high level of disturbance.
Left-right rhythmic stimulations of EMDR seem to remove the block in the traumatic memories processing and help memories storing mechanism function at an adaptive level. In recent years, many scientific researches have focused on the opportunity to assess the functional connection between different brain areas through quantitative analysis of EEG.
According to the aim of this presentation, our attention was drawn to the correlation between slow bands (4- 7 Hz) and the activity in the sub-cortical areas involved in working memory and, also, high frequency ranges (> 36 Hz) and cortical activities during sensorial stimuli processing. There are a few quantitative EEG studies on patients with PTSD. The EEG analysis of subject with childhood abuse histories revealed less synchronization in the two hemispheres functioning compared to normal control subjects.
EEG quantitative analysis in abused children showed a higher intra-hemispheric left coherence and a lower intra-hemispheric right coherence in comparison with normal control subjects. According to these results, the aim of this presentation is to test if there are recordable changes in the intra and inter hemispheric synchronization between brain areas where information processing occurs (limbic system, prefrontal cortical area, and posterior cortical areas) in patients with PTSD after EMDR treatment.

Objective: Although eye movement desensitization and reprocessing (EMDR) is said not only to reduce trauma-related
symptoms but also to enhance ability to function in life, its effectiveness in other than reducing trauma-related symptoms
has yet to be verified. The objective of this study was to explore the broad range of effectiveness of EMDR, especially changes
in the personality functioning during the early phase of treatment. Methods: Using the non-randomized design, we assessed
and compared the treatment changes in subjects treated with EMDR and subjects who participated in the psycho-education
course of trauma. Eligible subjects were adult women who experienced human-caused trauma such as domestic violence,
rape, or childhood abuse. The Rorschach Comprehensive System (CS) was used as the first outcome measure to evaluate
personality functioning, and self-report questionnaires and a clinical interview for trauma-related symptoms were used as
the secondary outcome measures. Subjects of both groups were assessed at the time of enrollment in the study and 4 months
after the enrollment. We completed the evaluation of 5 and 6 subjects in the EMDR and the control group, respectively.
Results: The CS index for self-esteem and self-concern was improved in the EMDR sample compared with the controls.
Women treated with EMDR showed increased openness to internal and external stimuli (assessed by CS F%), whereas some
women in the control group even developed a tendency to avoid internal and external stimuli after 4 months. Conclusion:
The differences of early changes in personality functioning between the EMDR group and the controls will be discussed in
detail.

Eye Movement Desensitization and Reprocessing (EMDR) has gained notoriety as a brief treatment for PTSD. In EMDR, clients repeatedly bring up traumatic events in memory and visually track stimuli moving back and forth in front of them. Previous group studies of EMDR have had methodological problems, however. The present study attempted to (1) assess EMDR's effectiveness compared to an Eye Focus treatment controlling for eye movement; (2) compare both treatments to a wait-list control; and (3) rate therapist characteristics to check experimenter bias.20 participants suffering from PTSD were randomly assigned to either EMDR or Eye Focus (EF) treatments, and immediate treatment or wait-list conditions. After two 90-minute treatment sessions, outcomes were assessed on subjective measures of anxiety (SUDs) and cognitive beliefs (VOCs). The mean SUDs pre-post differences on matched pairs t-tests indicated significant reduction of disturbance for both the EMDR group, p < .001, and the Eye Focus group, p < .001. VOC ratings showed similar treatment gains. Two-factor analyses assessed changes across groups (2 administrations x 2 treatment groups) on two standardized measures, the Impact of Event Scale (IES) and the Modified PTSD Symptom Scale (MPSS). The IES showed a main effect for treatment F (2, 18) = 16.45, p < .001, while wait-list remained unchanged F (2, 18) = 1.09, p > .05. A significant interaction F (2, 18) = 6.69, p < .05 supported treatment efficacy. EMDR and EF participants improved equally on the IES, i.e., no differences for type of treatment, F (2, 18) = 0.54, p > .05. The MPSS also showed a main effect for treatment, F (2, 18) = 6.46, p < .05, with wait-list unchanged, F (2, 18) = 0.27, p > .05. This interaction was not significant, p =.10. Further, both EMDR and EF groups demonstrated improvement on the MPSS. Results suggest that (1) EMDR and Eye Focus are both effective in reducing some self-reported PTSD symptoms; (2) EMDR and Eye Focus scores improved significantly compared to wait-list; and (3) ratings of experimenter bias suggested that the EMDR and Eye Focus participants were treated equally by the therapist. Until now, the effects of Eye Movement Desensitization and Reprocessing (EMDR), has been best explained by Working Memory (WM) theory. Research on the effects of EMDR has always concerned emotional memory, even though the WM account does not consider the emotional element in EMDR. Insights from neuroscience about emotion and its role in the consolidating of stimuli have led to questions that WM theory cannot address. Therefore, the present study introduces the WM/emotion theory on EMDR, which complements WM theory by incorporating knowledge about emotion and its expected role in EMDR treatment. WM/emotion theory expects emotion to play an important role in EMDR treatment, because the re-experiencing of emotion during the recall of an (traumatic) emotional memory seems to enhance (re)consolidation of the ‘blurred’ (as a result of taxing the WM) memory. If this is true, it means EMDR treatment would only affect memories with high emotional charge, and EMDR would be less effective with neutral memories. To test this hypothesis, an experiment was conducted in which negative emotional- and neutral memories of participants were treated with a lab version of EMDR (recall + eye movements (Recall+EM’s)) or a control intervention (Recall-only). It was expected that after Recall + EM’s the reduction of the vividness (and emotionality) would be larger for emotional memories than for neutral memories.It was found that after Recall+EM’s only the vividness (and emotionality) of emotional memories was reduced. The results of the present study suggest that emotion is a crucial element in EMDR treatment and knowledge about the workings of EMDR should be revised according to WM/emotion theory.

The present study was designed to isolate the effects of the eye-movement component of the Eye Movement Desensitization and Reprocessing (EMDR) procedure in the treatment of fear of public speaking. Seventy-one undergraduate psychology students who responded in a fearful manner on the Fear Survey Schedule II and on a standardized, self-report measure of public speaking anxiety (Personal Report of Confidence as a Speaker; PRCS) were randomly assigned to one of four groups in a 2 × 2 factorial design. The two independent variables assessed were treatment condition (imagery plus eye movements vs. imagery alone) and type of imagery (fear-relevant vs. relaxing). Dependent variables assessed were self-reported and physiological anxiety during exposure and behavioral indices of anxiety while giving a speech. Although process measures indicated exposure to fear-relevant imagery increased anxiety during the procedure, no significant differences among groups were found on any of the outcome measures, except that participants who received eye movements were less likely to give a speech posttreatment than participants who did not receive eye movements. Addition of the eye movements to the experimental procedure did not result in enhancement of fear reduction. It was concluded, consistent with the results of past research, that previously reported positive effects of the EMDR procedure may be largely due to exposure to conditioned stimuli (ScienceDirect).

Sindsdien hebben diverse EMDR–;therapeuten geopperd dat deze interventie ook toepasbaar is bij andere trauma–gerelateerde klachten. In een studie van Hekmat, Groth en Rogers (1994) is geclaimd dat EMDR effectief is bij de behandeling van pijn. In het onderhavige onderzoek is deze claim verder onderzocht. Daartoe werden 45 gezonde vrijwilligers onderworpen aan 8 elektrische prikkels. De proefpersonen werden verdeeld over 3 groepen: een groep die behandeld werd met EMDR, een groep die ter afleiding van de prikkels naar een cartoon keek, en een controlegroep waarbij geen verdere interventie werd uitgevoerd. Zowel fysiologische als subjectieve reacties op de pijnprikkels werden geregistreerd. Op geen enkele effectmaat werden verschillen tussen de drie groepen gevonden. In de discussie wordt dit resultaat in een breder perspectief geplaatst.
Eye Movement Desensitization and Reprocessing (EMDR) was introduced by Shapiro in 1989 as a therapeutic technique for posttraumatic stress disorder (PTSD). Since then several EMDR, therapists suggested that this intervention is also applicable to other trauma-related symptoms. In a study by Hekmat, Groth and Rogers (1994) has claimed that EMDR is effective in treating pain. In the present study further investigated this claim. For this purpose, 45 healthy volunteers subjected to 8 electrical stimuli. The subjects were divided into 3 groups: one group treated with EMDR, a group that as a distraction from the stimuli to a cartoon look, and a control group with no further intervention was performed. Both physiological and subjective responses to pain stimuli were recorded. In no effect size differences were found between the three groups. In the discussion, this results in a wider perspective.

Exp 1 laat zien dat de positieve bevindingen voor oogbewegingen (ten opzichte van een controle conditie) ook gevonden worden bij negatieve herinneringen aan een verlieservaring. Dit geeft een empirische basis voor de suggestie dat EMDR ook zinvol toegepast kan worden bij gecompliceerde rouw.
Exp 2 gaat over het werkgeheugen en een eventuele dosis respons relatie. Met andere woorden: geven taken die een grotere belasting voor het werkgeheugen vormen ook grotere SUD-dalingen?

There are several theories about the mechanisms of action of EMDR. The main theories will be briefly explained and summarized the scientific evidence before.
Then we will present a series of own experiments: studies in three students and a study in PTSD patients. In these experiments will be compared with each eye movement or other minor assignments stimuli. Implications for theory and for clinical practice will be discussed during the workshop by means of propositions.
Exp 1 shows that the positive findings for eye movements (compared to a control condition) also found associated with negative memories of a loss experience. This provides an empirical basis for the suggestion that EMDR is also useful can be used for complicated grief.
Exp 2 is about memory and a possible dose response relationship. In other words, tasks that give a greater burden on working memory are also larger SUD decreases?
Exp 3 Shows that the memory load of eye movements is much greater than the known Headphones clicks. Then we compare the efficacy of eye movements, clicks, and a control condition in which students negative memories.
Exp 4 A clinical study in PTSD patients, we eye movements, clicks and control (memory image without bilateral stimulation) compared. It is expected that the preliminary data available at the conference.

being exposed to fearful stimuli. Methods: Fifty two healthy
adult women were randomly allocated to eye movement or eye
fixed groups after watching a five minute fear-inducing film clip.
ECG was recorded during the resting state, after watching the
clip, and the treatment. A spectral power analysis of the heart
rate variability was performed. As the variables violated the rule
of normal distribution and the number in each group is small
the non-parametric test was used. Results: Overall, we did not find the differences between the groups in both time and
frequency domains. Some minor differences found were not
consistent with results from previous studies. Conclusions:
Effect of eye movement on autonomic nervous system during fear
desensitization was not supported in this experiment. Further study
with other psychophysiological measures is needed to understand
the role of eye movements in treatment of traumatic memory.

Abstract:
Eye Movement Desensitization and Reprocessing (EMDR) was introduced (Shapiro, 1989) as a treatment for traumatic memories. The necessity of eye movements or another activating stimuli in the treatment of test anxiety and the effectiveness of EMDR as a treatment for test anxiety is the focus of the present study. This study screened subjects for adequate study skills and the presence of test anxiety, and randomly assigned 45 subjects to one of three conditions: EMDR, EMDR with no activating stimuli (EMDR/NS; eyes closed), and a no treatment control group. The results of this study suggest that EMDR and EMDR/NS were equally effective in treating test anxiety and more effective than the control group as measured by the Achievement Anxiety Test, the Emotionality scale of the Test Anxiety Inventory (TAI), the Subjective Units of Disturbance scale, and the Validity of Cognition scale. Test performance and the Worry Scale of the TAI were not significantly impacted by the treatment groups. The inability of EMDR to impact the worry scale or the test performance of subjects in this study draws into question the usefulness of EMDR as a treatment of test anxiety. Abstract:
Eye-Movement Desensitization and Reprocessing (EMDR), a therapy that treats many trauma-related disorders by requiring patients to perform rapid eye movements, has raised controversy because it lacks the support of a proven theoretical rationale. A recent theoretical explanation proposes that the eye movements reduce the vividness of the distressing images by disrupting the function of the visuospatial sketchpad (VSSP) of working memory, but support for this model has been weakened by methodological flaws that the current study attempted to overcome. The present study compared the effects of tracking rhythmic and arrhythmic stimuli on the recall of arousing television shock-ads. Eye-movement conditions did not significantly differ in terms of vividness, emotionality, or accuracy of memory as compared to the control condition. Arrhythmic targets increased the negative emotional response and decreased the vividness of the memories, but neither rhythmic nor arrhythmic target patterns produced responses that differed from the control condition. Heart rate recordings taken throughout the study did not suggest that arousal mediates the relationship between eye-movement patterns and vividness. The present study does not support the VSSP theory but provides valuable insights on the direction of future research.

Assim, para assegurar a recuperação dos militares brasileiros que estavam no Haiti, escolhemos a técnica EMDR criada pela Dra.Francine Shapiro, que encontra respaldo teórico em descobertas recentes no campo neuropsicológico para realizar as intervenções. Justificativa: O EMDR é uma técnica eficaz, que permite que o trauma armazenado na memória de curto prazo passe para a memória de longo prazo, uma vez que com os estímulos criam-se novas conexões neurais que trazem alívio, paz e aceitação. Atualmente existem mais de 20 estudos randomizados que apoiam sua eficácia. Objetivo: Avaliar a eficácia da técnica EMDR na recuperação de vítimas de terremoto no Haiti, que apresentavam Transtorno Estresse Pós-Traumático e/ou Transtorno Estresse Agudo. Método: O estudo foi realizado com 14 militares sobreviventes do terremoto ocorrido no Haiti em janeiro de 2010, divididos em dois grupos: A (soterrados) e B (não soterrados). Foram utilizados para avaliação: entrevista inicial entrevista inicial e final, os testes IES- Escala de Impacto de Eventos e ISSL - Inventário de Sintomas de estresse de Lipp (antes e após a intervenção), EMDR, e reencontro (follow-up) seis meses após o último atendimento. Resultados: De acordo com os resultados obtidos no IES, os sujeitos que tinham o nível de estresse entre grave, moderado e leve, passaram após a intervenção para o nível leve e recomendado. Em relação aos sintomas físicos e emocionais (flashbacks, insônia, pesadelos, agressividade, instabilidade de humor e aumento no consumo de álcool), os sujeitos não apresentavam mais a queixa ao término das intervenções. Conclusões: Com base nos resultados apresentados podemos afirmar que o EMDR é uma técnica eficaz para resolução do transtorno estresse pós-traumático, assim como na sua prevenção, em vítimas de terremoto.
Introduction: We know that when a person experiences a critical incident, such exposure has considerable impact on their overall functioning. Thus, to ensure the recovery of the Brazilian military who were in Haiti, we choose the EMDR technique created by Dra.Francine Shapiro, who finds theoretical support in recent discoveries in the field to perform neuropsychological interventions. Rationale: The EMDR is an effective technique that allows the trauma stored in short-term memory to pass the long-term memory, since with the stimuli it creates new neural connections that bring relief, peace and acceptance. Currently there are more than 20 randomized trials that support its effectiveness. Objective: To evaluate the efficacy of EMDR technique in the recovery of victims of the earthquake in Haiti, which had Post Traumatic Stress Disorder and / or Acute Stress Disorder. Method: The study was conducted with 14 military survivors of the earthquake in Haiti in January 2010, divided into two groups: A (buried) and B (not buried). All patients were evaluated: initial initial interview and final interview, tests IES-Impact of Events Scale and ISSL - Symptom Inventory stress Lipp (before and after the intervention), EMDR, and reunion (follow-up) six months after the last treatment. Results: According to the results of the IES, the subjects who had the stress level between severe, moderate and mild, passed after the intervention to the level recommended lightweight. En concreto, los pacientes con depresión mayor fueron obligados a participar en un proceso terapéutico que involucró la metodología EMDR (Eye Movement desensibilización y reprocesamiento) y los experimentos de facilitación afectiva, lo que demuestra el impacto terapéutico sobre el procesamiento emocional cognitiva acerca de la información pertinente negativo de eventos traumáticos. Los resultados mostraron un cambio significativo y también que hubo participantes que implementaron un filtro cognitivo para eliminar las palabras negativas autobiográficas. Además, se implementó un sesgo para facilitar el reconocimiento de los positivos, así como los estímulos negativos. En la sección de debate, las coincidencias entre las evaluaciones conscientes e inconscientes se analizan sobre la superación de la depresión en esta terapia.

This essay is about EMDR method. A psychotherapeutic approach designed to process the memories of traumatic events and mitigate the psychological consequences. Francine Shapiro developed this method in 1989 for the treatment of trauma. A critical incident is any situation that causes unusually strong emotional reactions that have the potential to interfere with a person's
ability to function immediately after the incident or later. These are situations that overwhelm a person's sense of vulnerability
and/or control.
A critical incident stress debriefing (CISD) is a psychoeducational group meeting or discussion about a traumatic incident which
ideally takes place within 72 hours of the event. The goals of a CISD are to mitigate the psychological impact of a traumatic event,
prevent subsequent development of a post-traumatic syndrome, accelerate recovery, and serve as an early identification mechanism
for people who need further follow-up, including EMDR.
The steps of a CISD include:
1) introduction - to introduce the intervention team, explain the process, and set expectations.
2) fact - to describe the event from each participant's perspective on a cognitive level.
3) thought - to allow participants to describe cognitive reactions and to transition to emotional reactions.
4) reaction - to identify the most traumatic aspect of the event for participants.
5) symptom - to identify personal symptoms of distress and transition back to the cognitive level.
6) teaching - to educate as to normal reactions and adaptive coping strategies
7) reentry - to clarefy ambiguities and prepare for termination; access for follow-up.
In the opinion of the authors, the CISD facilitates the processing of the traumatic information before it becomes crystallized in
dysfunctional form.
EMDR can be very effective shortly following a CISD, and is particularly usehl for participants who are experiencing distress or
intrusive symptoms after the CISD. The CISD structure helps the participant understand the traumatic impact of the incident and
provides support and guidance toward adaptive resolution. The EMDR process begins where the CISD leaves off. The CISD helps
to delineate the traumatic image, negative cognition, and emotions associated with the event, making the subsequent EMDR process
more efficient. EMDR appears to have a very powerful and rapid effect after the CISD, perhaps, because of the initial processing.
In other words, the CISD initiates an adaptive processing of the traumatic information; EMDR completes it.
EMDR can be implemented individually immediately following the CISD, or the next day. While the CISD is a group process,
EMDR is an individual method. EMDR can be explained during the teaching phase of the CISD or after the CISD to the whole
group, but EMDR treatment is done individually and privately. EMDR can go beyond a CISD in targeting previous traumas that
may underlie the current incident, delve deeper into the meaning of the incident for the person, and target specific stimuli that are
relevant to the individual (e.g. Smells, tastes, etc.).
The workshop will discuss the application of EMDR to critical incidents. The protocol for recent events will be reviewed.
Guidelines for negative and positive cognitions will be discussed. For example, a critical incident usually involves issues of
responsibility ("Is it my fault?"), Safety ("Am I safe?"), And/or control ("Do I have choices in life?). It is important that such
dynamics are understood when formulating the negative cognition.
The dynamics of fear, a framework for understanding a critical incident and resolving issues of vulnerability and powerlessness, will
be presented. The model discusses the importance of going beyond defining the moment of peak stress to elucidating subsequent
thoughts, actions, and decisions. The implications for cognitive interweaves will be discussed.

Samenvatting
Patiënten met Eetstoornissen (Anorexia Nervosa, Bulimia Nervosa, Binge eating disorder) krijgen doorgaans een multidisciplinaire behandeling, waarin verschillende modules een complementaire rol spelen. Resultaten zijn echter vaak teleurstellend en daarom zijn nieuwe ideeën en technieken welkom. Mogelijkheden en beperkingen van EMDR in combinatie met Cognitieve Gedragstherapie (C.G.T.) worden besproken en geïllustreerd met videobeelden.
Renee Beer zal toelichten en demonstreren

Hellen Hornsveld zal vervolgens haar ervaringen presenteren met een eigen protocol ter behandeling van eetbuien en eetdrang. Dit protocol is voortgekomen uit de observatie dat patiënten vaak heftige, negatieve emoties vertonen tijdens cue exposure. Cue exposure is een gedragstherapeutische techniek, waarbij de patiënt blootgesteld wordt aan de prikkels die doorgaans een eetbui uitlokken (geur, smaak,aanwezigheid van “verboden” voedsel) zonder dat de eetbui daarbij plaats mag vinden (responspreventie). De procedure is gericht op het laten uitdoven van de fysiologische reacties op de eetbui prikkels (Jansen, 1997). Op de inhoud van de reacties en de emotionele betekenis die geassocieerd is met het eten, wordt verder niet ingegaan. De EMDR variant van dit protocol is juist gericht op de betekenisverlening en het creëren van een zo hoog mogelijke emotionele lading (“balletje op de berg”) door zoveel mogelijk het EMDR protocol te volgen. Positieve ervaringen met dit protocol zullen aan de hand van videomateriaal worden gedemonstreerd.

In view of potential, but largely undocumented benefits of eye movement desensitization and reprocessing (EMDR) as an intervention for PTSD in combat veterans, in our laboratory a study of EMDR treatment included (1) randomized patient assignment, (2) clinically appropriate comparison (treatment and control) groups, (3) a 12-session EMDR protocol administered by experienced, EMDR trained clinicians, and (4) extensive clinical assessment, including physiological evaluation at pre-treatment, post-treatment, and 3-month follow-up. Thirty-five veterans who met DSM-IV criteria for PTSD completed an extensive multimodal assessment protocol. Assessment instruments included: The Mississippi Scale for Combat-Related PTSD, the Impact of Events Scale (IES), the Clinician Administered PTSD Scale (CAPS), a self-rating of overall severity of "PTSD symptoms," the Beck Depression Inventory, and the Spielberger State and Trait Anxiety Inventories (STAI). In addition, each subject completed a Stressful Scene Construction Questionnaire (SSCQ) in which scripts of specific traumatic combat incidents were prepared for presentation during psychophysiological assessment. Following pre-assessment, a subset of the subjects constituted a waiting list control (CON, N = 12). Routine clinical care for these subjects was available at the VA Medical Center. Seven of these subjects also participated in group sessions for discussion of PTSD designed as an attentional control. There were no differences between the two control subgroups and their data was combined for all subsequent analyses. For the treatment groups, subjects assigned to the EMDR (EMD, N = 10) and relaxation (RXT, N = 13) groups were seated in a semi-reclined chair and continuous measures were taken of muscle tension levels (four sites), hand temperature, skin conductance levels, heart rate, and blood pressure. For all subjects, there were 20 minutes in each of the baseline sessions with no additional stimuli presented. At the end of session 2 of baseline, the patients remained in the experimental room and were assessed for an additional 20 minutes (pre-treatment) during which the SSCQ scripts also were presented. There were two sessions per week with a minimum of one day between sessions. Each subsequent treatment session for the EMD and RXT subjects was approximately 60 minutes in duration, allowing for set-up time and briefing. In the EMD group, a standard protocol for the EMDR interventions was administered, including periodic SUDS ratings and VoC scaling of combat and related images and cognitions (cf Shapiro, 1995). In the RXT group, home relaxation tapes and biofeedback on four sites (face, neck, arm, and back) to assist lowered muscle tension were provided. Following 12 treatment sessions (post-treatment), and again after three months (follow-up) the psychometric instruments and psychophysiological assessment were readministered using the format outlined above. Relative to the other conditions, the EMDR treatment produced substantially more positive clinical effects at post-treatment and follow-up. Comparing the EMD group to the CON group, significant effects (p<.05 or better) were obtained on measures of PTSD including the Mississippi and PTSD symptoms self-rating, and on the Beck and STAI-Trait. Comparing the EMD group to the RXT subjects, significant differences were found on the Mississippi, the IES-Intrusion scale, the CAPS, PTSD symptoms ratings, and the STAI-Trait scale. No differences were obtained on any of the physiological measures. Therefore, the present results support the effectiveness of EMDR with combat veterans with chronic PTSD. The data strongly suggest that some previous negative results obtained when EMDR was applied to chronic and severe combat PTSD may have resulted from methodological artifacts, such as inadequate amount of treatment and therapist inexperience. While the failure to find physiological effects is consistent with results of other controlled treatment exposure trials in PTSD, this finding raises clinical and conceptual questions with respect to the arousal component of the disorder.

that the same traumatic event will happen again. For
example, if a client has been raped and succesfully been
treated with EMDR, generally not many clinicians will feel the
urge to prepare the client for a next rape. Conversely, in a number
of cases (for instance phobic conditions) the client does have to
anticipate future situations in which the former phobic stimuli are
present; and where he will have to interact with these. As a result
of the application of the EMDR basic protocol, the likelihood or
severity of the initial threat may have been reappraised, and the
incident that initially felt traumatic may have been reattributed to
an innocuous event. However, if a dental phobic has been
successfully treated for his phobia, it is likely that he will still
have to undergo invasive dental work, such as injections, root canal
treatments or extractions. This has implications for treatment.
Therefore, with specific phobias, after any successful reprocessing
of anxiety related material occurs, it is of paramount importance
that the client be properly prepared for future confrontations with

He had shown little remorse towards the boy and described the incident as funny. The family had experienced a series of traumatic events. They were forced to evacuate their home when criminal gangs attempted to burn them out; his was to use the house for drug-related activity. Gang members had assaulted my patient and his mother in the street after going to the police. After meeting with the parents and brother and older sister, I had identified that on top of this recent trauma L had extreme hostility towards his mother who he described as “that woman.” My initial EMDR sessions were with L and his father. During processing of the trauma, my video will show bizarre movement including rolling his head back, hypotonic posture and behavior similar to an infant. This can be understood in relation to L’s early development. He was born at 33 weeks gestation and spends his first 6 weeks in a special care baby unit (CBU) in an incubator. My presentation will discuss the neurological consequences of early trauma and how a narrative approach using EMDR can help in recovery. This approach is based on the work of Dr. Joan Lovett. Psychologische Behandlung von traumatischen Belastungsstörungen mit EMDR. Die EMDR-Behandlung ist nach unserer Erfahrung eine sehr effektive Technik zur Behandlung von traumatischen Erinnerungen bei Patienten mit dissoziativen Störungen. Das Ziel der Traumabehandlung bei dissoziativen Patienten zusätzlich zu der Verringerung der Belastung von posttraumatischen Symptomen, Aussetzen der zuvor abgespaltenen Erinnerungen im Sinne einer Integration von dissoziierten Erfahrung liegt. Die EMDR-Behandlung dissoziativer Patienten brauchen, wenn nötig. durch Ändern der Standard-Protokoll auf den einzelnen Elastizität eingestellt werden. Da dissoziative Speicher Barrieren durch eine EMDRBehandlung sehr schnell untergraben werden kann, kombiniert mit dem Risiko der Übererflutung durch aversive Reize, sollte dies nur von entsprechend erfahrenen Therapeuten und mit Sorgfalt im Rahmen einer umfassenden Traumatherapie eingesetzt werden.
Psychological treatment of traumatic stress disorders with EMDR. The EMDR treatment is, in our experience a very effective technique for the treatment of traumatic memories in patients with dissociative disorders. The aim of trauma treatment in dissociative patients in addition to reducing the burden of post-traumatic symptoms, exposing the previously split-off memories in the sense of an integration of dissociated experience content. The EMDR treatment of dissociative patients need, if necessary. be adjusted by modifying the standard protocol on the individual resilience. Since dissociative memory barriers through a EMDRBehandlung may be undermined very quickly, combined with the risk of Übererflutung by aversive stimuli, this should only be used by suitably experienced therapists and with care in the context of a comprehensive trauma therapy.
I recently traveled to the South American Rainforest whilst on vacation as I had heard reports that some native peoples use eye movement in healing rituals (e.g., Australian Aboriginals and some South American Indians). As the author of EMDR self-use tapes, I am interested in other applications of REM type stimuli. I was curious to find out first hand whether native people utilize eye movements and how. I was also interested in investigating native approaches to pain and healing because of my work with chronic pain sufferers.
The most reasonabIe conclusion to be drawn from the extant literature is that EMDR is no more effective than standard treatments that rely on exposure to anxiety-provoking stimuli and is almost certainly effective because it happens to incorporate such exposure. In the words of Harvard psychologist Richard McNally, "What is effective in EMDR is not new, and what is new is not effective." Importantly, controlled data do not support the use of EMDR for anxiety disorders other than PTSD (e.g., phobias, obsessive-compulsive disorder, generalized anxiety disorder), mood disorders, sexual disorders, eating disorders, or psychotic disorders, although it is commonly used to treat the symptoms of these and other conditions. This presentation will outline the utilisation of EMDR with deaf clients exploring the
challenges it presents for clinicians whilst exploring the potential for EMDR. Through the use of case
examples this session will outline the ways in which EMDR needs to be adapted for this population
Many clinicians struggle with Cognitive Behavioural Therapy (CBT) with d/Deaf clients due to
some of the abstract ideas used and the heavy reliance on spoken language. Many people, both
hearing and deaf find it difficult to think about thinking, and develop levels of insight into their
emotions and behaviours simply by talking about them. Deaf people experience trauma and abuse
at a much higher rate than hearing people, but there are few established therapies to help them.
As Deaf people do not pick up all of the auditory stimuli it is likely that the trauma or difficult
experiences that bring them to therapy may have been stored visually or kinaesthetically rather than
linguistically. Trauma may also be caused by smaller events due to lack of information available to
them. EMDR is an evidence based therapy that attempts to resolve emotional reactions to traumatic
memories and their triggers. It does this through a combination of physical stimulation and the
recollection and discussion of memories. As such, it seems ideally suited for use with people who are
Deaf and have experienced trauma in their past. I describe how EMDR lends itself for successful use
with Deaf clients and their families, giving two brief case examples where I used EMDR with Deaf
children in British Sign Language.

An appointment at the dentist’s is not what most people look forward to. For some people, however, even the mere thought of having to undergo dental treatment causes them to feel extreme fear and panic. Between 5 and 15% of the world population suffer from such a pathological form of dental fear, called dental phobia. Dentally anxious individuals commonly avoid necessary dental procedures for many years despite deteriorating oral health, agonizing pain and severe psychosocial problems. The majority of individuals with dental phobia recall a traumatic event as cause for the onset of their fear. Interestingly, nearly 50% of them even suffer from posttraumatic stress symptoms, such as intrusive memories, nightmares, hyperarousal and avoidance behavior. EMDR can be a very useful instrument to help dentally anxious people to cope with past traumatic experiences and to prepare them for future confrontations with phobic stimuli.
The 90-minute workshop will give background information to the problem of pathological dental fear and present evidence from research showing that EMDR is an effective treatment method for dentophobia. The main focus will be on the practical procedure, which will be illustrated using video sequences of patient sessions.

very gradually I reduced the number of stimuli
being presented until John was able to track a

24 repetitions, the tracking object glides into the
centre of the screen and stays at the centre of the

Im Sitzungsverlauf zeigte sich jedoch eine signifikante Abnahme der Herzfrequenz und eine Zunahme des Parasympathikotonus. Unsere Befunde weisen darauf hin, dass es während EMDR-Sitzungen zu einem signifikanten psychophysiologischen Dearousal kommt. Die Stimulation durch Augenbewegungen scheint eine Orientierungsreaktion auszulösen, die über eine passagere Erhöhung des Parasympathikotonus eine Entspannungsreaktion auslöst. Es stellt sich die Frage, inwieweit die von uns beobachteten psychophysiologischen Reaktionen mit dem Therapieerfolg korrelieren.[Author abstract]
The EMDR trauma processing method (Eye Movement Desensitization and Reprocessing) combines the imaginary exposure to the traumatic memory with repetitive stimulation of eye movements or tactile or auditory stimuli. While the effectiveness of EMDR is well documented, the specific effects of bilateral stimulation are still largely unknown. We conducted a psycho-physiological monitoring using impedance cardiography of EMDR treatment sessions (N = 55) of 10 patients with PTSD. Recorded were heart rate (HR), parasympathetic tone (RMSSD), sympathetic (PEP) and respiratory rate (RESP). Beginning and end of the stimulation periods (N = 811) were selected. There was a steep increase in parasympathetic tone associated with a significant decrease in heart rate at the beginning of the stimulation periods, according to the psycho-physiological Reaktionsmuter a clearly marked orientation reaction. In the course of the stimulation of the parasympathetic tone took off again and the sympathetic tone increased, which can be interpreted as a stress effect during exposure. During the session, however, showed a significant decrease in heart rate and an increase in parasympathetic tone. Our findings indicate that occurs during EMDR sessions to significant psychophysiological Dearousal. The stimulation of eye movements seems to trigger a policy response that triggers a transient increase in parasympathetic tone a relaxation response. Im Rahmen des zuvor dargestellten Projektes werden die Patienten vor und nach der EMDR-Intervention mit einer Magnetencephalografie (MEG) untersucht. Dabei soll erfasst werden, ob die erwarteten angstassoziierten kortikalen Aktivierungen nach der EMDR-Behandlung zurückgehen. Als Angst auslösende Stimuli werden im MEG Bilder, Geräusche und Gerüche dargeboten werden, die in Zusammenhang mit der Zahnbehandlung stehen.
As part of the [Wirksamkeit des Eye Movement Desensitization and Reprocessing (EMDR) bei Patienten mit Zahnbehandlungsphobie, (Doering, Bisping, Bohnke)] project a Magnetencephalografie (MEG) will be presented before the patients were examined before and after the EMDR intervention. It should be recognized that the expected angst associated cortical activations to go back to the EMDR treatment. As fear-inducing stimuli in the MEG images, sounds and smells will be presented, which are associated with dental treatment.
The purpose of this study is to find out whether Eye Movement Desensitization and Reprocessing (EMDR) is effective in the treatment of patients with dental phobia. La présente thèse s'intéresse à l'évaluation de l'efficacité à court et à long terme d'une nouvelle intervention dyadique, brève et précoce ayant pour but la prévention du trouble de stress post-traumatique (TSPT). L'exposition à un événement traumatique provoque, chez certains individus, une souffrance émotionnelle considérable qui entrave leur fonctionnement quotidien de manière parfois importante. Au cours des deux dernières décennies, des interventions curatives efficaces ont été créées, soit la thérapie cognitive-comportementale
(TCC) et la désensibilisation par mouvement oculaire (EMDR). On remarque cependant un manque criant d'interventions efficaces à court et à long terme qui ont pour but la prévention du TSPT, malgré les efforts du débriefing et de la TCC brève en ce sens. Cette thèse vise donc, dans un premier temps, à faire le point sur l'état des connaissances au niveau de l'efficacité des interventions précoces et, dans un deuxième temps, évaluer de manière empirique l'efficacité à moyen (3 mois) et surtout à long terme (2 ans) d'une nouvelle intervention dyadique, brève et précoce pour prévenir le TSPT. La nouvelle intervention dont il est question dans le présent ouvrage se déroule en deux séances et est offerte à la victime et une personne-soutien de son choix par une travailleuse sociale ou une infirmière. Elle met l'accent sur la psychoéducation, l'apprentissage à la communication exempte de soutien social négatif (minimisation, hostilité, impatience, etc.) et sur l'importance de ne pas éviter les stimuli associés à l'événement traumatique. La thèse comporte quatre chapitres. Le premier propose une recension de la littérature qui met à jour les principales conclusions quant à l'efficacité du débriefing et de la TCC brève pour prévenir le TSPT. Cette recension met en évidence les principales failles méthodologiques et les difficultés d'application de ces interventions. De plus, elle expose le rationnel derrière l'utilisation du soutien social comme ingrédient actif d'une intervention de prévention de ce trouble.
Le second chapitre présente une étude d'efficacité randomisée et contrôlée de l'efficacité de la nouvelle intervention. Soixante-six participants ont été randomisés soit dans la condition intervention ou dans la condition de contrôle. Les résultats ont démontré qu'au post-test de trois mois, les participants de la condition intervention manifestaient des symptômes de TSPT significativement moins intenses que ceux de la condition contrôle. De plus, les participants de la condition intervention rapportent une diminution significative du soutien social perçu, contrairement aux participants de la condition de contrôle qui n'observent pas cette différence. Il est intéressant de souligner que le taux d'abandons en cours de traitement fut équivalent et très bas au sein des deux groupes. Ceci suggère que les participants tolèrent bien cette nouvelle intervention. Aucune différence n'a été observée entre les deux groupes en ce qui a trait à l'occurrence de psychopathologies comorbides. Le troisième chapitre se veut une relance à 2 ans post-trauma de l'étude présentée au chapitre précédent. Quarante-six des 66 participants ont accepté de prendre part à cette relance. Les résultats révèlent que, deux ans après l'événement traumatique, les participants ayant reçu l'intervention étaient encore beaucoup moins symptomatiques que les participants
n'ayant pas reçu cette intervention. Un fait plus qu'intéressant qui ressort de cette étude est qu'aucun participant de la condition intervention ne rapporte un TSPT et cinq participants de la condition contrôle souffrent encore de ce trouble deux ans après l'événement traumatique. Parallèlement, il a été observé que les participants ayant reçu l'intervention perçoivent encore moins de soutien social négatif que ceux de la condition contrôle. This thesis focuses on the evaluation of the effectiveness in the short and long-term reoperation dyadic brief and early aimed at the prevention of posttraumatic stress disorder (PTSD). Exposure to a traumatic event causes in some individuals, suffering considerable emotional hinders their daily operations be significant. Over the past two decades, effective curative interventions were created either cognitive-behavioral therapy
(CBT) and eye movement desensitization (EMDR). We note, however, a dearth of effective interventions in the short and long-term aim of preventing PTSD, despite the efforts of debriefing and brief CBT in this direction. This thesis therefore aims, firstly, to take stock of the state of knowledge at the effectiveness of early interventions, and in a second step, empirically assess the efficacy medium (3 months) especially in the long term (2 years) of a new intervention dyadic brief early to prevent PTSD. The new intervention mentioned in this book takes place in two sessions and is offered to the victim and a support person of their choice by a social worker or nurse. It focuses on psychoeducation, learning to communicate free of negative social support (minimization, hostility, impatience, etc..) And the importance of not avoiding stimuli associated with the traumatic event. The thesis consists of four chapters. The first provides a literature review that updates the main conclusions about the effectiveness of debriefing and brief CBT to prevent PTSD. This review highlights the main methodological flaws and difficulties in implementing these interventions. In addition, it outlines the rationale behind the use of social support as an active ingredient of an intervention to prevent this disorder.
The second chapter presents an efficacy study randomized controlled effectiveness of the new intervention. Sixty-six participants were randomized to either intervention or condition in the control condition. The results showed that post-test three months, participants in the condition action had symptoms of PTSD significantly less intense than those of the control condition. In addition, participants reported response provided a significant reduction in perceived social support, unlike the members of the control condition who do not observe this difference. It is interesting to note that the dropout rate during treatment was low and similar in both groups. This suggests that participants tolerate this new intervention. No difference was observed between the two groups in regard to the occurrence of comorbid psychopathology. The third chapter is a raise to 2 years post-trauma of the study presented in the previous chapter. Forty-six of the 66 participants agreed to take part in this revival. The results show that two years after the traumatic event, participants who received the intervention were still much less symptomatic than participants
who did not receive this intervention. A more than interesting fact that emerges from this study is that no participant intervention condition reported PTSD and five participants from the control condition still suffer from this disorder two years after the traumatic event. Meanwhile, it was observed that participants who received the intervention perceive less social support than the negative control condition. Like what has been reported in post-test three months, the two groups did not show a significant difference in the expression of other psychological ailments. The fourth and final chapter provides a general discussion of the results based on interpretations that can be drawn, different strengths and inherent limitations as well as theoretical and clinical implications that may arise.
10 patients suffering from PTSD following a severe traumatic event, were assessed with event-related brain potentials (ERPs) in a modified oddball paradigm containing auditory standard, target, and novel tones. ERPs were assessed before and after a treatment session using the eye movement desensitization and reprocessing method. Compared to a control group that underwent sham treatment, ERPs of the patients showed a reduction of the P3a component in the post-treatment recording, suggesting a reduced orienting to novel stimuli and reduced arousal level after the treatment. Moreover, psychometric assessment revealed a marked improvement of the PTSD symptoms after treatment. [Author Abstract]
A core feature of post-traumatic stress disorder (PTSD) is hypervigilence to threatening material. This study measured processing of threat material in PTSD with simultaneously acquired initial eye movements and electrodermal activity, following presentation of threatening and neutral words. Ten PTSD subjects and 10 controls were presented with 4 words in parafoveal range. On trials in which a threat word was present, PTSD subjects demonstrated initial eye fixations on the threat word more than controls. PTSD subjects also demonstrated more orienting responses on all trials than controls. These results suggest that processing of threat information in PTSD can be usefully investigated with convergent psychophysiological methodologies.

Utfallet av experimentet mättes i olika fysiologiska mätparametrar,
samt självskattningar av obehagsnivån (SUD). Resultaten visade signifikant
högre hudkonduktansnivå vid ögonrörelsestimulering jämfört
med kontrollgruppen, relaterat till aktivering av det sympatiska nervsystemet.

Mönstret av autonom aktivering kan tyda på att ögonrörelsestimuleringen

In the present study were randomized thirty-six healthy subjects to one of the three stimulation conditions: eye movement stimulation, tactile stimulation or static visual stimuli (control group). The stimulation was combined with imaginary exposure, negative and positive. The outcome of the experiment were measured in various physiological test parameters, and self-estimates the level of discomfort (SUD). The results showed significantly higher hudkonduktansnivå of eye movement stimulation compared with control group, related to the activation of the sympathetic nervous system. No significant group differences were found in the other measurements. The pattern of autonomic activation may indicate that ögonrörelsestimuleringen trigger or reinforce an orientation response, as some theorists suggested is the active mechanism in EMDR. However, further research is needed to understand this effect, and access to therapeutic efficacy.

Post-traumatic stress disorder (PTSD) is a fairly common disorder. Roughly 8-10% of the population will suffer from PTSD at some point in their lives. For victims of violent crimes such as rape, the rate of PTSD may be 60-80%. (1) According to the DSM-IV, diagnostic criteria for PTSD include: 1) perceived or actual threat to life or physical integrity, accompanied by an emotional response of horror, helplessness, or intense fear; 2) re-experience of the trauma (e.g. flashbacks and nightmares); 3) avoidance of trauma-related stimuli and numbing of interest and affect; and 4) increased unwanted arousal, such as concentration difficulties, irritability, and insomnia. Specific criteria exist regarding the numbers of symptoms in each category that must be present.

Based on previous research by Lee and Drummond (2007), this research has examined the influence
of the type of therapist instructions (reliving and distancing) on the processing of distressing
memories. Furthermore, this research examined whether the type of the trauma also influences the
way the memory is being processed. The research has been conducted by and among university
students. A detailed protocol – based on the original Eye Movement Desensitization and
Reprocessing (EMDR; Shapiro, 1989) protocol –was used. Participants (13 males and 23 females,
mean age 22.4 year) were asked to recall a distressing memory, after which the content of the
memory was desensitized by either reliving or distancing instructions. Results show no difference in
effectiveness between reliving and distancing conditions. Furthermore, no differences were found
between the conditions powerlessness and guilt/shame. Partly based on the used measures it can be
concluded that the manipulation of the conditions failed. For future research it is recommended that
the protocol be expanded and the conditions of the type of trauma be adjusted. Furthermore it is
recommended to add a control condition to the research, in order to determine whether bilateral stimuli
are effective and whether they influence the effectiveness of therapist instructions.
Eye movement desensitization and reprocessing (EMDR), a controversial treatment suggested for PTSD and other conditions, was evaluated in a meta-analysis of 34 studies that examined EMDR with a variety of populations and measures. Process and outcome measures were examined separately, and EMDR showed an effect on both when compared with no treatment and with therapies not using exposure to anxiety-provoking stimuli and in pre-post EMDR comparisons. However, no significant effect was found when EMDR was compared with other exposure techniques. No incremental effect of eye movements was noted when EMDR was compared with the same procedure without them. R. J. DeRubeis and P. Crits-Christoph noted that EMDR is a potentially effective treatment for noncombat PTSD, but studies that examined such patient groups did not give clear support to this. In sum, EMDR appears to be no more effective than other exposure techniques, and evidence suggests that the eye movements integral to the treatment, and to its name, are unnecessary. This article describes a comprehensive treatment of a case of panic disorder with agoraphobia. A thorough history taking revealed that experiential contributors had a pivotal role in the development of the condition. Therefore, eye movement desensitization and reprocessing (EMDR) was used to address early traumatic events as well as the present stimuli that caused disturbance and had maintained symptomatology for the past 12 years. Although the client's symptoms were resolved after 15 sessions, EMDR was also effective in addressing future behaviors and resolving anticipatory anxiety. During EMDR processing, the client demonstrated emotional and cognitive changes consistent with trauma resolution, insight, and personal growth. The client gradually enacted functional new behaviors spontaneously as treatment unfolded. The therapeutic process and the targets are described in detail. [Author Abstract]
Eye Movement Desensitization Reprocessing (EMDR) is a controversial treatment for PTSD that requires clients to make rapid eye movements while revisualizing a traumatic event. Although seemingly effective, the process by which EMDR exerts its effects is poorly understod. We propose that EMDR's eye movements facilitate the orienting response, i.e., the attentional adjustment to unexpected stimuli. Since the orienting response has been implicated in spontaneous transformations of dream content during REM sleep, we reasoned that, similarly, activation of the orienting response during EMDR may facilitate content transformations in traumatic memories. To examine this hypothesis, 25 undergraduates completed 20 seconds of eye movements or 20 seconds of visual fixation before each of two tasks: (1) a covert visual attention task, in which a cue indicated the likely position of a subsequent target, and (2) a sentence rating task, in which sentences with either metaphoric or non-metaphoric endings were rated for strikingness. Repeated measures ANOVAs indicated that the eye movement manipulation facilitated attentional adjustments to targets presented in invalidly cued locations and increased the extent to which metaphoric sentence endings were found striking. Together these results suggest that the eye movements in EMDR induce attentional and semantic flexibility, thereby facilitating transformations in the client's narrative representation of the traumatic event. The implications of these findings for theories of dream formation and metaphor comprehension are also considered. [Author Abstract]
It has been claimed that the symptoms of post-traumatic stress disorder (PTSD) can be ameliorated by eye-movement desensitization-reprocessing therapy (EMD-R), a procedure that involves the individual making saccadic eye-movements while imagining the traumatic event. We hypothesized that these eye-movements reduce the vividness of distressing images by disrupting the function of the visuospatial sketchpad (VSSP) of working memory, and that by doing so they reduce the intensity of the emotion associated with the image. This hypothesis was tested by asking non-PTSD participants to form images of neutral and negative pictures under dual task conditions. Their images were less vivid with concurrent eye-movements and with a concurrent spatial tapping task that did not involve eye-movements. In the first three experiments, these secondary tasks did not consistently affect participants' emotional responses to the images. However, Expt 4 used personal recollections as stimuli for the imagery task, and demonstrated a significant reduction in emotional response under the same dual task conditions. These results suggest that, if EMD-R works, it does so by reducing the vividness and emotiveness of traumatic images via the VSSP of working memory. Objectives: Theoretical models implicating the orienting reflex as an explanatory mechanism in the eye-movement desensitization and reprocessing (EMDR) treatment protocol are contrasted and tested empirically. We also test whether EMDR effects are due to a distraction effect. Design: A repeated measure design is used in two experiments. The first experiment employed two independent variables, eye condition (moving vs. stationary) and tone (a pseudo-randomized series of low and high intensity tones). In Expt 2, eye condition was replaced by attentional demand conditions (low or high). In both cases, electrodermal responses served as the dependent variable. Method: Participants were recruited from the Psychology Department at Cardiff University. In Expt 1, participants were required to either pursue a moving stimulus following auditory challenge or engage in an eyes-stationary task. In Expt 2, the task following auditory challenge required participants to identify specific items from letter strings in low and high attentional demand conditions. Results: Lower levels of electrodermal arousal were identified in tasks eliciting eye movements, compared to no eye movements. This effect was not due to the attentional requirements of the task. Conclusions: Eye movements following auditory challenge result in an effect of psychophysiological de-arousal. This supports the reassurance reflex model of EMDR proposed by MacCulloch and Feldman (1996).
Animal models conceptualize anxiety as a response to potential danger while fear is a response to present danger. The way humans experience anxiety involves our capacity for higher thinking while the human experience of fear appears to be much the same as the animal model. This article examines these differences at both a phenomenological and neurological level and highlights implications for the treatment of conditioned fear in PTSD. The stimuli for human fear are sensory-perceptual, while the stimuli for most forms of anxiety are conceptual-linguistic. Individuals in a state of fear/terror undergo a radical shift from top-down to bottom-up processing in which access to conceptual-linguistic thought processes is severely restricted and the frontal regions of the cortex are no longer able to override impulses from brain stem and midbrain regions. Conditioned fear involves actual neurological changes in the limbic system. To overcome a traumatic memory, the individual must (1) gain some level of access to the bottom-up state in order to habituate or extinguish the conditioned fear response, and (2) also achieve access to the top-down state in order to process the fear. ]Sage Journals]
The wellbeing of rescuers: Relational, organizational and technical aspects that can affect rescuers' wellbeing during
rescue activities: Stress inoculation, role playing and the role of mirror neurons in training, also through the use of
video recordings. Relaxation techniques, psychological debriefing and EMDR in trainings.
Focus of our intervention is the wellbeing of the rescuer. The study and research
on this matter came and were carried out thanks to the activity done both during
trainings and simulations of the Civil Protection than real emergencies.
Our team work received contribution by some psychologists of OPP (Parma’s
Psychologists’ Observatory: A.Sozzi, E.Pedrelli, F.Frati, A. Bocelli, T. Serra). Wellbeing, defined as a subjective and positive emotional state together with a
global life satisfaction (Diener, 1984), is strongly at risk during rescuer’s
emergency activities and can affect the rescuer both physically and
psychologically. The rescuer's capabilities, that we think are technical “know how” and thorough
knowledge, are essential to give the best performance according to the complexity
and urgency of the intervention. These skills can really contribute to the rescuer's
wellbeing, because they can improve the self-efficiency perception.
To effectively manage and train rescuers, it is furthermore important to consider
and acknowledge the influence of interpersonal relationships on technical
performances. It is, in fact, particularly important to recognize and support the
typical relationships that can be created in a team with the same task and
specialization, as well as in multidisciplinary teams, or teams belonging to
different Institutions but operating in the same scenario.
In recent years increasing attention has been given to training activities, even through the use of the role play for interventions in artificial emergency
scenarios. To recreate scenarios of massive emergencies, different Civil Protection
Associations, as well as First Aid volunteer associations and the local
Institutions have been involved. In these simulations, most cases focus on improving technical performances.
Lately psychologists have been asked to join the rescuers team.
During these simulations, the role-play of emotional and psychological problems
occurs thanks to the cooperation between emergency psychologists and the
medical team. The introduction of the role and expertise of psychologists allowed to extend and
strengthen the attention to cross support and care aspects for the psychological
wellbeing of both victims and rescuers.
The psychologist must therefore consider the “wellbeing” in all the emergency
scenarios and contexts, as a sum of all the components that we talked about here
and the ones we will describe during our intervention.
He must first of all be aware of the complexity of each intervention in the field,
and adopt a kind of approach aimed at creating and recovering wellbeing
strategies, that can be used by himself as well.
Strategies on how to build, recover and maintain the wellbeing identify stress as
the first danger source the rescuer has to face in his training and emergency
activity.
When external events or stimuli are perceived as difficult to face compared with
resources available at that moment, the individual gets stressed.
When the person's efforts are not adaptive to the external requests and/or
coherent with his performance expectations, he becomes vulnerable to emotional,
behavioural, cognitive and physical reactions, which can be even very difficult to
manage both in the short and/or in the medium-long term.
This can happen when the sources of stress depend on the rescuer’s
performance, and it can also happen in case of post traumatic stress, visible in
different stages after the event.
From the psychologist's specialist background and from the integration of this
with the result of field experiences, the demand for a range of different tools to
manage the different kinds of stress emerges, and these tools must be applicable
both to the individual and to the group.
This range is still improving, and the results of our observational activity from
past and present experiences lead us to see the opportunity to carry on our
research of tools of efficacy.
During this speech we would like to underline that approaches like Stress
Inoculation Training (SIT, Michenbaum, 1983) and the use of role playing allow
the technical appraisal and let the rescuers improve their stress management
skills, and all that can lead to a decrease in the risk of PTSD.
In past simulations of emergencies, we found out that the use of videotapes for
the role plays is a tool that should be taken more into account. We think it is
important to evaluate its potential for the rescuers' benefit, because it seems to
be not only “a record of technical performances”, but also an observation and
learning tool about the rescuer's own defence and adaptive strategies.
In fact, during these simulations we found out that the rescuers' psychological
and emotional vulnerability emerged in several situations. The fact that even in these artificial situations there were acute stress episodes and O codes urged us
to focus more on the matter of mutual influence between technical performance
and internal experience of stress.
We understand that such acute stress episodes may occur during real life critical
events but we can see how role playing and video recordings show that such
acute stress episodes affected the simulators themselves even during the
simulation. The videos show that even apparently “high immunity” simulators,
who are considered 'immune' thanks to their comprehensive and strong
experience, experienced acute stress, perhaps because of an incorrect selfevaluation
of their own stress management skills.
The interest in the use of videos as a training and reprocessing tool for rescuers
led some of us to specialize in role playing recording, so as to carry out a more
accurate and comprehensive study on those same videos and use them as a
mirror of reality and better educational tool through a vicar experience or through
“seeing oneself from within the experience” and in the interpersonal dynamics
that took place in the scenario.
Videotapes are a very known and widely used tool in other kinds of trainings,
disciplines and therapies (i.e. Family Therapy and CBT).
The discovery of mirror neurons by Rizzolati, Gallese et Al., provides the evidence
that when someone observes the same action performed by another person, the
neurons "mirrors" the behaviour of that person, as though the observer were itself
acting. Thanks to these researchers it is now proven that this can happen
thanks to the motor neurons in the pre-motor cortex.
Therefore, we would like to underline the role of videos as very useful and
versatile training tools, since they expose a situation in an unexpected realistic
manner “as if” it were true and “as if” we were really experiencing that situation,
with the consequent learning movements at the emotional, cognitive and
behavioural level, at the stress management level, as well as at the level of team
work dynamics.
Visual imagination activates the same brain regions that are active during visual
perception and motor imagination activates the same brain regions activated the
movement is really happening.
More importantly, it was possible for us to verify that the videos recorded by other
operators were not focused on showing the important psychological aspects we
mentioned for the goal of the trainings, thing that happened instead with the
videos recorded by psychologists. We think therefore that the use of videotapes
recorded by psychologists should be given more consideration in the trainings of
rescuers. During this intervention we will devote part of the time to broadcasting
two short videos; the first one shows the role playing of an intervention in an
emergency context, and the second one shows a part of an EMDR session (Eye
Movement Desensitization Reprocessing). We think it is important to recreate and
protect rescuers wellbeing in the post-role playing and post emergency stages
too. For years EMDR has been proven effective in improving the individual's
coping skills and in reprocessing, wherever necessary, the post traumatic
aspects resulting from critical events to whom not only the victims, but also the rescuers too, are exposed during emergencies.
Introduzione:negli ultimi anni il numero di studi condotti nell’ambito dei disturbi causati da eventi traumatici ha subito una forte accelerazione rilevando l’esistenza di alterazioni fisiologiche e morfologiche in specifiche aree cerebrali associate sia alla risposta emotiva al trauma che alla insorgenza dei sintomi della sindrome da stress post-traumatico (PTSD).
L’impiego delle tecniche di neuroimmagine ha consentito di fare luce sui correlati neurali della psicoterapia, rivelando i suoi effetti neurobiologici sulla funzione cerebrale. Nell’ambito dei diversi approcci psicoterapeutici, l’EMDR (Eye Movement Desensitization and Reprocessing therapy) è emerso come promettente risorsa per il trattamento del trauma e di altri disturbi d’ansia, sebbene ancora non sia stata completamente chiarita la sua modalità di azione sui circuiti neurali. Molte ricerche sono state effettuate per valutare l’efficacia dell’EMDR, ma solo un numero esiguo di esse ha indagato il substrato neurobiologico di questa psicoterapia.
Le metodiche di neuroimmagini utilizzate finora per studi sull’EMDR sono la Single Photon Emission Computed Tomography (SPECT) e la Magnetic Resonance Imaging (MRI). La prima raccoglie dati sull’attivita’ cellulare cerebrale mentre la seconda riporta dati anatomici strutturali.
Gli studi funzionali condotti con SPECT consentono di identificare le variazioni del metabolismo e del flusso ematico cerebrale, suggerendo un ruolo specifico per ciascuna area cerebrale coinvolta nel complesso meccanismo che sottende il processamento delle emozioni; la MRI consente invece di rilevare la presenza di variazioni morfologiche e volumetriche di specifiche strutture cerebrali. Negli ultimi anni, studi SPECT e MRI, volti ad esaminare gli effetti dell’EMDR sulla fisiopatologia cerebrale in pazienti affetti da disturbi d’ansia, hanno riscontrato sostanziali variazioni del flusso ematico ed alterazioni strutturali di alcune regioni cerebrali in seguito a terapia.
L’efficacia dell’EMDR nel trattamento del PTSD è stata confermata da uno studio SPECT su sei soggetti ( Lansing et al.) (1) che ha evidenziato dopo terapia una diminuzione del flusso ematico nel lobo occipitale bilateralmente e nel lobo parietale sinistro e un aumento di flusso nel giro frontale inferiore sinistro. Sempre utilizzando la SPECT Oh e Choi (2) hanno riportato una significativa modificazione del flusso cerebrale in seguito ad EMDR, prevalentemente riscontrabile nell’area limbica e nella corteccia prefrontale. Un recente studio con utilizzo di MRI (3) ha inoltre messo in evidenza un aumento di volume dell’ippocampo in seguito a trattamento con EMDR, aumento invece non riportato in un lavoro di Lindauer et al. su 9 soggetti con PTSD, nonostante il successo clinico della psicoterapia eclettica eseguita (4).
Obiettivi: il nostro gruppo ha studiato gli effetti a breve e lungo termine dell’EMDR (5, 6) dimostrando la sua efficacia nel trattamento di soggetti traumatizzati sul lavoro, e una stabilità del miglioramento clinico ottenuto in risposta alla terapia, ancora presente a distanza di tre anni. Nell’ambito dello stesso progetto sono state anche studiate le variazioni del flusso ematico cerebrale e della volumetria di alcune strutture cerebrali in relazione alla terapia con EMDR. Un primo studio SPECT ha analizzato la variazione della distribuzione di flusso in 11 soggetti guariti clinicamente da PTSD in seguito a EMDR messi a confronto con 5 soggetti nei quali la terapia non ha avuto effetto (7). Risultati: è stato evidenziato nei soggetti con remissione sintomatologica significative differenze di flusso in 4 aree corticali, alla disfunzione delle quali sono ascrivibili alcuni sintomi presenti in corso di PTSD. Diminuzioni di flusso post-terapia sono state registrate nell’ippocampo, nella corteccia parieto-occipitale e nella corteccia visiva primaria. L’ippocampo e’ sede della memoria a breve termine; la corteccia parieto-occipitale processa il riconoscimento di volti, dei corpi e delle parole; la corteccia visiva primaria custodisce la memoria visiva degli eventi. La mancata inibizione e/o l’iperattivita’ di queste regioni nel PTSD sono responsabili della rivisitazione patologica dell’evento traumatico e della presenza di flashback ed immagini allucinatorie. Inoltre la corteccia frontale dorsolaterale ha dimostrato un aumento di flusso nei soggetti che hanno risposto positivamente alla terapia. Questa regione chiave oltre ad essere deputata ad inibire la risposta patologica a stimoli che ricordano l’evento traumatico e’ essenziale per i processi di attenzione e di autostima, diminuiti in corso di PTSD e recuperati in seguito alla remissione della malattia. In ulteriori studi che combinano indagini funzionali e strutturali utilizzando SPECT e MRI abbiamo messo in evidenza il valore predittivo delle dimensioni dell’ippocampo riguardo all’efficacia della terapia con EMDR (Pagani et al. sottomesso).
Conclusioni: gli effetti della terapia con EMDR sono risultati, unitamente ad un consistente miglioramento della sintomatologia e ad una riduzione della iperreattività a stimoli di carattere emotivo, in una normalizzazione funzionale di alcune aree specifiche e in un aumento del volume ippocampale probabilmente ascrivibile a neo-neurogenesi. Questi risultati sono in linea con la pregressa letteratura e con le attuali conoscenze sul PTSD e suggeriscono basi neurobiologiche dell’effetto terapeutico dell’EMDR confermandone l’impatto funzionale su strutture cerebrali coinvolte in patologie ansiogene.

Introduction In recent years the number of studies in disorders caused by traumatic events has been greatly accelerated by detecting the existence of morphological and physiological changes in specific brain areas associated with both the emotional response to trauma to the onset of symptoms Post-traumatic stress syndrome (PTSD).
The use of neuroimaging techniques has allowed to shed light on the neural correlates of psychotherapy, revealing the neurobiological effects on brain function. Under the different psychotherapeutic approaches, EMDR (eye movement desensitization and reprocessing therapy) has emerged as a promising resource for the treatment of trauma and other anxiety disorders, although still not been fully elucidated its mode of action on neural circuits. Many studies have been carried out to assess the effectiveness EMDR, but only a small number of them have investigated the neurobiological substrate of this psychotherapy.
The methods used so far for neuroimaging studies on EMDR are Single Photon Emission Computed Tomography (SPECT) and Magnetic Resonance Imaging (MRI). The first collects data on 'brain cells while the second contains structural anatomical data.
The functional studies performed with SPECT to identify changes in metabolism and cerebral blood flow, suggesting a specific role for each brain area involved in the complex mechanism underlying the processing of emotions, instead of MRI allows detection of morphological changes and volume of specific brain structures. In recent years, MRI and SPECT studies, aimed to examine the effects of EMDR on brain pathophysiology in patients with anxiety disorders, have found substantial variations in blood flow and structural alterations of some brain regions after treatment.
The effectiveness of EMDR in the treatment of PTSD was confirmed by a SPECT study in six subjects (Lansing et al.) (1) showed that after treatment reduced blood flow in the occipital lobe bilaterally and in left parietal lobe and a increase of flow in left inferior frontal gyrus. Always using SPECT Oh and Choi (2) reported a significant change of cerebral blood flow after EMDR, mainly found in limbic and prefrontal cortex. A recent study using MRI (3) has also highlighted an increase in volume of the hippocampus after treatment with EMDR, but no increase in reported work of Lindauer et al. on 9 subjects with PTSD, despite the clinical success of eclectic psychotherapy performed (4).
Objectives: Our group has studied the effects of short and long term EMDR (5, 6) demonstrating its effectiveness in treating traumatized individuals at work, and stability of clinical improvement obtained in response to therapy, yet this distance three years. Within the same project were also studied changes in cerebral blood flow and volume of certain brain structures in relation to treatment with EMDR. A first SPECT study analyzed the change in flow distribution in 11 subjects clinically recovered from PTSD after EMDR compared with 5 subjects in whom treatment had no effect (7). Results: It was shown in patients with symptomatic remission, significant differences in flow in 4 cortical areas, which are attributable to dysfunction of some symptoms of PTSD being present. Flow decreases post-treatment were recorded in the hippocampus, cortex parietal-occipital and primary visual cortex. The hippocampus and 'seat of short-term memory, the parietal-occipital cortex processes the recognition of faces, bodies and words, the primary visual cortex preserves the visual memory of events. The lack of inhibition and / or 'hyperactivity' of PTSD in these regions are responsible for the pathological review of the traumatic event and the presence of flashbacks and hallucinatory images. Moreover, the dorsolateral frontal cortex showed an increase in flow in subjects who responded positively to therapy. This key region in addition to being appointed to inhibit the pathological response to stimuli that recall the traumatic event and 'essential for the processes of attention and self-esteem, decreased in the course of PTSD and recovered following the remission of the disease. In further studies that combine functional and structural investigations using SPECT and MRI have shown the predictive value of the size of the hippocampus on the effectiveness of EMDR therapy (Pagani et al. Submitted).
Conclusion: the effects of EMDR therapy were coupled with a significant improvement in symptoms and a reduction of hyperreactivity to stimuli, emotional, functional in a normalization of some specific areas and an increase in hippocampal volume probably due to neo- neurogenesis. These results are consistent with previous literature and with current knowledge about PTSD and suggest a neurobiological basis of therapeutic EMDR confirming the functional impact on brain structures involved in anxiety-disorders.

Survivors of early emotional neglect experience pervasive difficulties including vulnerability to adult psychiatric disorders and inability to regulate emotional states (Schore, 1996, 1997, 2000, 2001a, 2001b; Teicher, 2000, 2002; Teicher et al, 1993; Teicher et al., 1997). Their inability to regulate emotional states is not solely linked to effects of adverse events, but is significantly linked to insufficient exposure to normal, developmental attachment sequences that foster capacities for self-regulation.
A subset of adult survivors of early, pervasive, emotional neglect who meet full or partial criteria for posttraumatic stress disorder also present with comorbid Cluster C Axis II symptoms (Avoidant, Dependent, Obsessive-Compulsive) and meet criteria for dismissing (or fearful) insecure attachment (Cassidy & Shaver, 1999; Main, 1996).
Clinical assessment reveals these patients have low tolerance for positive interpersonal emotions and engage in defensive strategies to dismiss, minimize, deny or subtly avoid experiencing and assimilating this positive emotional states into their internal model of self. These strategies include overt and covert behavioral avoidance as well as dissociate defenses. Paradoxically, these patients may show superficial characteristics or competence, interpersonal skills, or emotional stability which on closer examination prove to be fragile or which collapse in the face of social stressors.
The general theoretical base for the Positive Affect Tolerance and Integration (PAT) protocol is related to McCullough’s (1996, 2003) model of affect phobia and recognizes McCullough’s emphasis on an anxiety regulating, titrated approach to developing tolerance for adaptive affect and associated coping behaviors. Putnam’s (1997) discrete behavioral states model provides an important conceptual framework for understanding these patients’ needs to gradually develop new discrete behavioral (psychophysiological and affective) states and new pathways (schemes and scripts) fostering access to these shared positive states often as a crucial early phase of treatment to help resolve their impairments in emotional self-regulation.
This presentation describes the use of standard EMDR procedural steps in a treatment plan that postpones the standard three pronged (past, present, future) PTSD protocol, but which is consistent with the consensus model for Complex PTSD (Brown, Scheflin & Hammond, 1998; Chu, 1998; Courtois, 1999; Hart, Nijenhuis, Steele, 2006) and other EMDR approaches focused initially on improving response to current stimuli (Hoffman, 2004; Leeds & Korn, 1998; Leeds & Shapiro, 2000) before attempting to target childhood traumatic memories. Targets for PAT are recent experiences in which the patient was exposed to positive, shared, interpersonal emotional states. The purposes for applying PAT to these targets are: to decrease defensive avoidance; dissociation and anxiety about shared positive emotional states; to increase capacity to tolerate and enjoy these shared positive emotional states; and to integrate these shared positive emotional states into positive schemas and self-concepts. Observed clinical gains following PAT included: improved mood and resilience, and decreased depersonalization during subsequent use of EMDR to reprocess traumatic memories.
The goal in presenting this “Positive Affect Tolerance and Integration Protocol” case series is to encourage research to evaluate the clinical effectiveness of this application of the standard EMDR procedures for a clinical subpopulation generally considered challenging to treat.
In this poster I am going to raise the question of integrating EMDR in family and couple therapy. This is a
philosophical question. I actually am asking if it's possible to integrate an individual approach with a systemic
one. Those scholars who dealt with this question found some common roots in the attachment period. For
example, the Hendrix' IMAGO approach presume that the marital crisis has its roots in the childhood wounds. We
can find other approaches that enforce this assumption. The literature that deals with integrating EMDR with
couple therapy does not find differences between traumas caused by marital crisis and traumas caused by other
stimuli. This brings us to the idea it is possible that EMDR will be useful in a marital crisis also. The novelty was
that those therapists who mastered both skills, family therapists and EMDR therapists, started with protocols
that integrate EMDR in couple therapy in the presence of both spouses. From the first beginning, Francine
Shapiro mentions in her basic book on EMDR that are cases where EMDR is contra-indicated in couple therapy in
the presences of both mates. Later on we found that we can classify the cases when to use EMDR in the presence
of both spouses and when to use it individually. Of course, this categorization is based on clinical observation. We
like to refer this question to the members of this poster presentation if they have some more insights about this
The absolute lack of skepticism in this article was a disappointing lapse from Salon's
usual standards. The writer failed to point out that 1) The evidence that EMDR is
beneficial is somewhat weak; 2) the treatment is not accepted by the American
Psychological Association; and 3) assuming the treatment works, there is no sound
theoretical explanation of how it's supposed to work, the invocations of "left brain,
right brain" notwithstanding. You did note the broad variation of physical stimuli
used by different practitioners (eye movement, hand tapping and alternating sounds),
but this variation throws into question the basic practice. If they all work, what's
really going on?
With Meredith, what had seemed an insurmountable impasse using a standard PTSD protocol had become amenable to significant resolution when addressed with a non-standard protocol. The key to this approach was to install multiple positive resources without deliberately activating the distressing emotions and associations of a specific, disturbing memory or current stimuli. I have coined the phrase "EMDR resource installation" to describe this protocol. I have since used this approach with other challenging clients who have childhood histories of significant failures of attachment with their primary caregivers. In these cases, their histories and current functioning led me to conclude that their capacity for self-soothing and affect modulation was not yet developed to the point where they could tolerate directly targeting distressing memories using the standard EMDR protocol. Research background & objectives: Based on REM-sleep dependent memory reprocessing model, this study examined possible therapeutic mechanisms of eye
movements in Eye Movement Desensitization and Reprocessing (EMDR; Shapiro, 1989,
1995, 2001). Stickgold (2002) proposed that sleep induced change in associative memory
via activation of weak association during REM state and EMs functioned as REM sleep to
integrate the episodic memory of trauma into general semantic memory. Specifically, the
effect of EM in relation to order of relatedness of associations on change of strength of
semantic associations for negative words after saccadic bilateral eye movements was
examined in the present study.
Methods: Sixty-four college students were recruited as participants. Via semantic
priming task, a 2 (eye movement condition: horizontal saccadic eye movements vs. no
eye movements) × 3 (strength of semantic association: strong priming vs. weak priming
vs. unrelated priming) × 2 (block order of presentation: related prime first vs. unrelated
prime first) mixed factorial design was performed, with strength of semantic association
serving as within Ss factor and the other two variables serving as between Ss factors. The
primes consisted of negative word stimuli. Each trials consisted of an 8 seconds saccadic
horizontal eye movement manipulation (200 ms per movement in EM condition),
followed by 500-msec fixation point and then prime displayed for 32msec, which was
followed immediately by the target. The participant was instructed to read the target as
soon as possible and the reaction time was recorded by the compuer. While absolute
primeability index for strong (or weak) association was calculated as (RT
related – RT unrelated) for strong primes or weak primes, respectively, relative primeability index was calculated as (RT related – RT unrelated)/RT
unrelated. The dependent variables were correct (%), error (%), and primeability index for each of strong prime and weak prime. Change of primeability
of weak associations for negative stimuli as opposed to that of strong ones under different
EM conditions and different block order was compared.
Results: For absolute primeability index, the 2 (EM condition) × 2 (order) × 2 (strength
of priming) ANOVA showed that only the interaction effect of EM × strength of semantic
association was approaching significance, indicating that weak priming significantly
exceeded strong priming after EM (F (1, 56)
= 18.01, p < .001; partial ω 2= .210), while the
opposite was true after non-EM (F (1, 56)
= 25.86, p < .0001; partial ω 2 = .280). Further, for weak prime, the priming effect was stronger after EM than after non-EM (F
(1, 56) = 74.62,
p < .0001; partial ω 2 = .535). For relative primeability index, it also revealed that only the interaction effect of EM × strength of semantic association was significant (F (1, 56) = 6.09, p < .02; partial ω 2 = .074), and tests of simple main effects showed similar patterns as those of absolute primeability index. Conclusions: 8s EMs was associated with change of strength of semantic associations.
While EM didn’t weaken the primeability for the strong association, EM did enhance the
primeability for weak associations. Further, weak priming significantly exceeded strong
priming after EM, while the opposite was true after non-EM. The results echoed
REM-sleep dependent memory reprocessing model, suggesting that EM in EMDR might
reflect a shift in associative memory systems by activating different strength of
associations of negative semantic nodes for different semantically related words. Given
that order of relatedness didn’t play a role and previous research showed that EM resulted
in decreased vividness and emotionality and generated greater amount of associations for
negative stimuli as well, the implications of the present results from theoretical and
psychotherapy point of views and future research possibilities are discussed.
Mindfulness mediation (based on Buddhist meditative techniques) has previously been advocated for depression relapse prevention and chronic pain reduction in former research trials (Teasdale; Kabat-zinn, etc.). Because the practice of mindfulness relates to body scans and focusing attention upon the body, it has been advised that it may cause problems in using such a method for sexual abuse and rape survivors, although no research appears to have been carried out in this area. In a single case study of child-sexual-abuse adult survivor who frequently dissociated and experienced “flashbacks” during clinical sessions, the methods of Mindfulness Mediation were introduced to her, in an attempt to keep an awareness of her surroundings whilst doing imagined exposure in for PDSD (post duress stress disorder). From dissociative phases, flashbacks and cognitive avoidance of stimuli, the client became able to focus on the traumatic information being presented and to emotionally relate and process trauma memories, which previously she had been unable to do. Her ability to attenuate upon traumatic information was increased, habituation was easier from a clinical perspective, her anxiety levels decreased (BAI) and her depression levels eased (BDI). (N.B. The client’s improved anxiety and depression scores may have been due to Mindfulness Mediation practices or to the processing of highly emotive information).
Conclusion: Mindfulness Meditation can be a useful adjunct to trauma processing when imagined exposure is being implemented for PTSD/PDSD for processing of un-integrated traumatic information, where dissociation, cognitive avoidance or flashbacks occur within the context of treatment, creating a barrier to habituation from traumatic information. A cognitive memory experiment with between group design was conducted to investigate the
auditory bilateral stimulation effect on the episode of animal memory. The background to
study was that many clinical observations made in the psychotherapeutic treatment
Eye Movement Desensitization and Reprocessing (EMDR) regarding
bilateral stimulation effect on memory processes. The issue was: Did
auditory bilateral stimulation a positive effect on the episodic memory? A randomized
sample of 110 university student volunteers (55 women
and 55 males), evenly distributed over five groups, aged 19-46 years, participated. Audi TIVE
stimulus was given in four groups. The dependent variable was episode-Semitic memory performance
and measured with the help of meaningful test and ordtest. A general effect of
bilateral stimulation of episode animal memory could not be demonstrated. Bilateral stimuli at
learning, however, showed the significant results relating to memory performance in relation
to the two groups who received bilateral stimulation at reproducing. Significance was obtained
including performance in relation to the trial subject's experience of stimulus.
The results are discussed from the trial subject's experience of stimulus and the influence
of arousal. Lack of emotional components, which ultimately contributed
to differences in memory processing, was adopted as the primary explanation for non -
positive effect of bilateral stimulation.
Francine Shapiro, the founder of eye movement desensitization therapy (EMD) claims to have serendipitously discovered this technique by experiencing spontaneous saccadic eye movements in response to disturbing thoughts during a walk in the woods. This explanation is difficult to accept because normal saccadic eye movements appear to be physiologically undetectable and are typically triggered by external stimuli. In the present limited study, all subjects failed to elicit any experience of eye movements, thus calling into question how best to interpret Dr. Shapiro's experience and the origin of EMD. One-Session Treatment (OST) is a form of massed exposure
therapy for the treatment of specific phobias. OSTcombines
exposure, participant modeling, cognitive challenges, and
reinforcement in a single session, maximized to three hours.
Clients are gradually exposed to steps of their fear hierarchy
using therapist-directed behavioral experiments. Although
there are several studies in the literature examining the
efficacy of OST, little has been done to summarize this
research. In the following review, research on and empirical
support for OST are reviewed with an emphasis on the types
of stimuli, samples, and methodologies utilized. Research
generally supports OST's efficacy, although replication by
independent examiners using adult and child samples is
needed using more rigorous comparisons (e.g., psychological
placebo or other treatments). Overall, OST continues to
be a promising treatment for specific phobias; however, a
great deal more investigation is needed to identify mechanisms
of change, mediators, and moderators.
Participants will learn about: 1) the Orienting Response (OR) interpretation of EMDR phenomena, the relationship of the OR to sleep and dream research, affect theory, and information processing; 2) research into the effects of variations of stimuli in EMDR applications with respect to modality (audio, visual, tactile), speed, complexiity, and content on measures of autonomic functioning, relaxation, information processing, and memory; 3) the implications of this research for clinical applications of EMDR; and 4) new instrumentation for implementing new treatment approaches.
Introduction: Eye movement desensitization and reprocessing (EMDR) is a relevant technique to improve post-traumatic stress disorder (PTSD) symptoms. Objectives: To compare the electrophysiological profile of patients suffering post traumatic stress disorder before and after EMDR treatment. Aims: The authors are in search of a specific event-related brain potentials profile for post-traumatic stress disorder (PTSD). Methods: Eight patients suffering from post-traumatic stress disorder (PTSD) following a severe traumatic event, were assessed with event-related brain potentials (ERPs) in a modified oddball paradigm containing auditory standard, target, and novel tones. ERPs were assessed before and after a treatment session using the eye movement desensitization and reprocessing method. Results: Psychometric assessment revealed a marked improvement of the PTSD symptoms after treatment. Compared to a control group that underwent sham treatment, ERPs of the patients showed morphological changes in the post-treatment recording, suggesting a reduced orienting to novel stimuli and reduced arousal level after the treatment. Conclusions: EMDR therapy provides clinical improvement and event-related potentials changes that could be used in clinical practice as an interesting marker to assess diagnosis and successful treatment of PTSD.
Eye Movement Desensitization and Reprocessing (EMDR) is an innovative therapy that is currently used to treat anxiety disorders. Discovered by Francine Shapiro in 1987, the treatment was originally utilized on individuals suffering from Post Traumatic Syndrome Disorder; however, the original treatment protocol has evolved to include other forms of trauma such as performance anxiety. The main hypothesis of EMDR states that traumatic memories cause the nervous system to become dysfunctional and unbalanced. As a result, stimuli such as sound and images from these events are looped continuously within the nervous system until the body can functionally process the events toward an adaptive solution. Until a solution is established, the original thoughts and feelings encountered during previous disturbing events resurface each time a resembling experience occurs. EMDR serves as a performance enhancement for brass musicians by desensitizing and reprocessing maladaptive memories through bilateral stimulation of both hemispheres of the brain. The role of psychological trauma (e.g., rape, physical assaults, torture, motor vehicle accidents) as an etiological factor in mental disorders, anticipated as early as the 19th century by Janet, Freud, and Breuer, and more specifically during World War I and II by Kardiner, was "rediscovered" some 20 years ago in the wake of the psychlogical traumas inflicted by the Vietnam war and the discussion "in the open" of sexual abuse and rape by the women's liberation movement. 1980 marked a major turning point, with the incorporation of the diagnostic construct of PTSD into DSM-III and the definition of its main diagnostic criteria (reexperiencing of the traumatic event, avoidance of stimuli associated with the trauma, and symptoms of increased arousal). Initially described as resulting from a onetime severe traumatic incident, PTSD has now been shown to be triggered by chronic multiple traumas as well. This "state-of-the-art" article discusses past and current understanding of the disorder, with particular emphasis on the recent explosive developments in neuroimaging and other fields of the neurosciences that have highlighted the complex interrelationships between psychological, psychiatric, biological, and neuroanatomical components of the disorder, and opened up entirely new therapeutic perspectives on how to help the victims of trauma overcome their past. EMDR has enjoyed a comfortable alliance with Ego-State Therapy. ‘Inner Critics’ or ‘People Pleasers’ may be the repository for such Negative Cognitions (NCs) as, “It’s my fault,” or, “I cannot upset anyone.” But who is the author of Positive Cognitions (PCs)? Are they constructed in the assessment phase of treatment, or do PCs emerge spontaneously with Dual Attention Stimuli from the Self: our unfettered awareness and contemplation? This workshop reveals EMDR as a catalyst for dialogues between the Self and NCs. Extraordinary imagery tools that enhance work with children and adults are offered to illuminate the surfacing of the Self.
Power Therapies claim to achieve rapid results in reducing fear elicited by a large number of situations. This paper presents a theory of how competition among stimuli may be the basis for how the Power Therapies work. The compelling features of these therapies are that they all interrupt old habits and conditioned reflexes and provide new habits and conditioning. Therefore, many of the protocols involve overcoming prior-stimulus dominance. In addition to proposing a mechanism for these therapies, this article also reveals that, despite superficial differences, power therapies fundamentally accomplish the same thing. These therapies reduce the intensify of emotional responses elicited by stimuli associated with trauma. It is proposed that they accomplish this end through working at the subcortical level of brain activity to interrupt the negative emotional responses elicited by the trauma stimuli. [Author Abstract]
Negative autobiographical memories are effectively treated using EMDR
with patients making eye movements during the recall of negative
autobiographical memories. While several studies have measured the effects of
different types of stimulations, we are not aware of any research assessing the
combined effects of two different stimulations applied simultaneously during the
recall of the event.
For this preliminary study involving 15 adult patients from both sexes, 4 EMDR
counselor therapists measure 17 symptoms of the list proposed by Weimann
(1968)). Each patient is evaluated before, during and at the end of each session.
We control the effect of the sequence of one versus two types of stimulations by
assigning each patient to both conditions but in a random way: (a) first target first
session with one type of stimulation following with a second target first session,
including two types of stimulation; (b) the inversed sequence. We compare the
mean level intensity of each symptom (within factor) between one type vs. two
types of stimulations (between factor) using the Anova and the t- Student tests.
We hypothesize that an attenuation of intensity of the stress symptoms occurs
when the patient is treated simultaneously with two types of bilateral stimulation
(eye movements and tactile stimuli), compared to one (eye movements).

light of the theory of the working memory (Andrade et al.1997) and the

Prolonged exposure counterconditioning (PEC) was tested as a treatment for
chronic post-traumatic stress disorder (PTSD) in an adult survivor of repeated child sexual
and physical abuse. PEC utilizes imaginal reliving of very pleasurable life moments in
order to weaken traumatic conditioned emotional responses (CERs). A higher-order conditioned
stimuli (CS) is used as a traumatic CER elicitor. Prolonged imaginal reliving of
pleasurable CSs is used as a counterconditioner to the traumatic CERs. A statistical technique
for analyzing single-case subject designs based on classical test theory was used to
evaluate the client’s progress in treatment. Results showed that PEC effectively decreased
the client’s PTSD symptoms, depression, and anxiety. In addition, the client’s negative
cognitions became considerably more positive. Also, the client lost his comorbid conditions
of chronic major depressive disorder and social phobia. Finally, other clinically
observed symptoms, which are described in the article, improved markedly. All results
were maintained at a 3-month follow-up.
“Protocol for Excessive Grief“ is excerpted from Eye Movement Desensitization and Reprocessing (EMDR) Scripted Protocols: Basics and Special Situations illustrating a scripted protocol from one of Francine Shapiro's 6 basic protocols. “Scripting“ informs and reminds EMDR practitioners of the component parts, sequence, and language used to create effective outcomes, and also generates a template for practitioners and researchers to use for reliability and/or a common denominator so that the form of working with EMDR is consistent. This protocol includes 5 steps: process actual events, including the loved one's suffering or death; process any intrusive images that are occurring; process the nightmare images; process any stimuli/triggers associated with the grief experience; and address issues of personal responsibility, mortality, or previous unresolved losses. The future template is included This protocol addresses the many aspects of grief and mourning to assure the full processing of clients' concerns.
Purpose: It is well known that hyperarousal in PTSD patients leads to an increase in heart rate to trauma related stimuli. The purpose of this study was to see if this peripheral physiological activation in PTSD patients by a trauma script can be reversed by successful trauma treatment including EMDR (Eye Movement Desensitization and Reprocessing).
Methods:
12 PTSD patients fulfilling DSM IV criteria with 52.6 mean level of the impact of event scale (IES) and 8.1 of the subjective unit of distress (SUD) were compared to 12 matched healthy controls (IES level 23, SUD level 4.8). Glued electrodes were placed according to published guidelines for electrophysiological research on thorax (ECG) and palmar skin of the left (non dominant) hand (SCL). Psychophysiological data (heart rate and skin conductance) were recorded continually and stored on a PC card during three conditions: neutral, relaxation and trauma script. In the patient group the procedure was repeated after finishing treatment.
Results: The patient group and control group did not differ in the baseline heart rate, however, the stimulation by the trauma script in the PTSD patients was significant in the mean 15.6 (T-2.88) (p < 0.01) increase in heart rate and in the control group 1.6 not significant (the script here was derived from the worst life event). There was a wide variation in the patient group with three patients without any reaction. In those with a strong reaction after trauma script, successful treatment was accompanied by a decline in heart rate response after trauma script, which remained stable during 6 months follow-up. SCL data did not show any consistent relationship. Since this is an ongoing study with increasing numbers and further analysis, additional data will be given during presentation. A decline of the SUD level to 2.3 and within the IES-score to 21 at the three months follow-up measurement was also significant (p < 0.01).

Antecedent studies show that traumatic stimulation on patients
with posttraumatic stress disorder (PTSD) finalizes in various
psychophysiological correlates. During therapy of these patients
a strong demand for activation of internal resources, i.e. activation
of fortitude and positives thoughts, exists. Especially EMDR
therapy uses resource stimulating elements such as position of
power and absorption in preparation for exposure. In this study
standardized EMDR protocols establish a solid basis to explore
individual internal resources.
Researches on trauma stimuli in EMDR- patients show effects
on parasympathetic tonus (Sack 2006) as well as increased cerebral
blood flow in defined brain regions (Levin 1999. Lamprecht
2000). Especially the heart rate variability (HRV) may describe
the sympatheticovagal balance (Cohen, 2002, Porges 1991). This
study focuses on psychophysiological effects and neurobiological
regulative mechanisms of stabilizing methods and activation of
internal resources in PTSD patients and healthy control group.
Methods: Healthy subjects and patients with diagnosed PTSD
(DIAX) listened to a commonly neutral script, an individual
trauma script and an individual absorption script. Following
each script measurements of heart rate variability (HRV), respiratory
flow, skin conductance responses (SCR) and skin blood
flow (LCF, TU50%) took place.
Results: Preliminary results revealed a significant reduced heart
rate variability in patients compared to the healthy controls in
reaction to the stress script as well as to the positive and the
neutral scripts.
Conclusion: To our knowledge this is the first time to be proven
that altered autonomous functions are found in PTSD not only
in reaction to traumatic reminders, but even to a positive, resource
activation situation. This provides our basement for further
research. Detailed analysis of different effects to each script
on both groups are currently underway.
Although psychotherapy is successful in altering emotional distress, the biological mechanism by which it achieves this has not been the subject of intensive neurobiological investigation. Mindful processing of emotion has been proposed to be a key factor in prevention of relapse in depressive illness and here that hypothesis is developed and extended to include other conditions in which emotion processing may be obstructed or dysregulated. Cognitive therapy, interpersonal psychotherapy, psycho-dynamic psychotherapy, and dialectical behaviour therapy, each in a different way and with a distinct emphasis, encourage awareness of emotions and their associated cognitions and biographies, and their varying success may depend on the degree to which they achieve activation of internal healing processes. In eye movement desensitisation and reprocessing (EMDR), the selected target is formatted for endogenous processing which is facilitated and accelerated by eye movements or alternating bilateral auditory or tactile stimulation. The ability to sustain focussed attention on the affect and its visceral, cognitive, and biographical components is postulated to activate a homeostatic process of distress resolution, seen most clearly in treatment of PTSD with EMDR, in which resolution of distress can be intense and rapid while therapist input is non-directive, although supportive, empathic, and non-judgemental. Once the therapist has helped to frame the questions, the patient's brain will find the answers needed for the resolution of the distress and all the components of the traumatic event, whether visceral, cognitive, affective, or interpersonal. The anterior cingulate cortex, especially the dorsal and rostral components, is suggested to be the key neurobiological substrate for the efficacious psychotherapeutic relief of distress, and relevant functional neuroimaging studies are summarised. One limitation of some previous imaging studies of emotion is that they have tended to use mild stimuli to discrete emotions. An alternative approach would be to image the brain during reprocessing of an unpleasant event which has profoundly affected the person so that the associated intense emotions could be clearly labelled and correlated with changes in regional brain functioning. In summary, we submit that the evidence cited by Lipke in support of the proposal that eye movements contribute to treatment outcome does not meet the burden of proof. Specifically, eye movements and other laterally alternating stimuli have not been shown to have an effect on relevant and independently assessed outcome measures of symptoms, such as measures of PTSD severity. Thus, we reaffirm Hembree and Foa's conclusion that assertions about an important role for eye movements or any other laterally alternating stimuli on treatment outcome have not been supported by extant research. Eye Movement Desensitization and Reprocessing (EMDR),
Thought Field Therapy (TFT), Time-Limited Trauma Therapy (TLTT),
Traumatic Incident Reduction (TIR), and Visual/Kinesthetic
Dissociation (V/KD) are relatively new approaches in the treatment
of trauma-related disorders. These approaches have come
under intense scientific and professional scrutiny and criticism,
with proponents and critics offering diverse and intriguing arguments
in support of their respective points of view. This discussion
will focus on the current empirical and anecdotal evidence on
the efficacy of these five approaches, as well as various criticisms
that have been proferred in response to the empirical and clinical
literature on treatment efficacy and theory. EMDR will be highlighted,
given the growing empirical database on EMDR outcome
studies and the contention surrounding the treatment and its’ theory.
The evidence for and/or against the other four approaches will
also be presented, upon which we will raise for discussion issues
relating to methodological rigour, scientific reporting of clinical
data, and the interpretation of efficacy studies in general. The discussion
will occur within the context of philosophical discourse on
the ideal of integration of science and practice, and the feasibility
of attaining this ideal within the current scientific Zeitgeist.

A clinical case study of skills-based treatment of a 48-year-old,
female survivor of child sexual abuse (CSA) reporting severe dissociative
symptomatology is presented. Chief complaints were daily
amnestic episodes, depersonalization, panic attacks, agoraphobia,
and consequent long-term unemployment. In contrast to most
approaches to therapy for dissociative syndromes, it is argued that
treatment does not need to be primarily trauma-focused. Instead
treatment focused on targeting dissociation and anxiety by teaching
skills to be implemented by the client outside of session.
Functional behavioral analysis of particular recent incidents of dissociation
were conducted over several sessions. Imagery-based
relaxation and progressive muscle relaxation were used and monitored
to help the client learn additional methods of reducing distress.
Grounding techniques were implemented to counter the dissociative
tendency to lose experiential awareness of the here and
now. Cognitive interventions were utilized to challenge the erroneous
notions reported about dissociative experiences, such as the
panic she experienced when her dissociation had been revealed to
others. Last, in vivo systematic desensitization was used with a fear
hierarchy, ranging from the least to most fearful stimuli that precipitate
dissociation, where graduated exposure was followed by relaxation.
At termination the client reported elimination of panic
attacks, resumption of driving, and marked reduction in frequency
and duration of amnestic periods, with substantial additional gains
and progress in finding full-time employment reported at 8 month
follow-up. Treatment outcome is assessed based on psychological
test data obtained at intake, discharge, and 8 month follow-up, as
well as client anecdotal report.
The Narrative Constructivist personal psychology model postulates that traumatized children and adults experience disturbances in cognitive schemata within domains of their psychological and interpersonal functioning: safety, trust, power, esteem and intimacy. Their processing of themselves and the world, which is greatly affected by ethno-cultural and beliefs, becomes rigidified around the "trauma story.” Their responses to stimuli are thus limited to repetitive and intrusive manifestations of fear and withdrawal. Utilizing culturally and spiritually salient metaphors as well as appropriate timing, EMDR facilitates the creation of meaningful narratives about the person's present and future and the world, enhancing sense of self and focused, purposeful behaviors. This symposium will introduce the narrative/cultural context model of trauma, with discussion, film clips and handouts; engage participants in a brief group intervention based on this model, to explore the emotional impact of ethno-cultural issues in regard to trauma and treatment interventions; and present clinical cases treated with EMDR based on cultural-sensitive choice-points and useful metaphors in work with diverse populations.

focused, purposeful behaviors. This symposium
will introduce the narrative/cultural context model
of trauma, with discussion, film clips and
handouts; engage participants in a brief group
intervention based on this model, to explore the
emotional impact of ethno-cultural issues in
regard to trauma and treatment interventions; and
present clinical cases treated with EMDR based
on cultural-sensitive choice-points and useful
metaphors in work with diverse populations.

and invited after informed consent for 4 1-week interval treatment sessions and a 1 month
follow-up session. The instruments for outcome measures included Cockroach Phobia
Questionnaire, fear ratings of cockroach slides, FSS, STAI-S, BDI, short form of SCL-90, the
Revised Thought-Action Fusion Questionnaire, White Bear Suppression Inventory, and
cognitive tasks for measuring strength of associations. The SUDs, credibility and therapeutic
relationship rating, and physiological measures such as HR, HRV, EOG, served as process
measures. Due to space limitations, the results of cognitive task and physiological measures
were reported elsewhere. The participants were randomly assigned to one of the four groups:
EM condition (EM vs. Non-EM exposure only) × block order of cockroach theme
presentation (negative cognition first vs. positive cognition first). A 2 (EM condition) × 2
(order of valence presentation) × 9 (time: pre-assessment and post-assessment for each of the
4 sessions plus 1 month follow-up assessment) mixed factorial design was performed, with
time serving as within Ss factor and the other two variables serving as between Ss factors.
There were 20 trials in each therapeutic session. The duration of each trial was 30s for both
the EM and Exposure-Only conditions.
Results: After preliminary analyses for group differences on pretreatment variables, credibility-relationship ratings, and outcome variables were explored, the 2 (EM condition) ×
2 (order of valence presentation) × 9 (time) ANOVA on SUDs showed that the main effects of
time and EM were both significant (p < .001 and p < .034). Subsequently, two 2 (order of
valence presentation) × 9 (time) ANOVAs were performed for EM condition and
Exposure-Only condition, respectively. The results showed that for EM condition, only time
effect was significant (p < .006); while for Exposure-Only condition, there were a significant
time effect (p < .001) and an approaching significant valence presentation order effect (p
< .065), with the SUDs being higher in negative cognition presented first condition compared

EM condition. Using trend analyses and inspection of time effect showed that significant

A standard treatment for anxiety disorders involves exposing clients to anxiety eliciting stimuli. Some theorists have argued that exposure is an essential element in the successful treatment of post traumatic stress disorder (PTSD). Exposure therapy and Eye Movement Desensitization and Reprocessing (EMDR) are two therapeutic approaches that have been identified as efficacious in the treatment of PTSD. It has been proposed that exposure is the active element in all effective treatments for PTSD, including EMDR. This presentation will address various issues that should be considered in evaluating this proposal. Theories underlying exposure therapies will be summarized and differences between process and mechanism will be discussed. Brief overviews of EMDR and exposure therapies will be provided. Video clips of treatment sessions with exposure therapy and with EMDR will be shown, to illustrate the differences in treatment approaches, clinical focus, and client experience. Shared and unique components will be identified. The use of exposure in EMDR and its possible contribution to treatment outcome will be examined. Unlike prolonged exposure therapy, EMDR uses extremely brief repeated exposures (i.e., 20-50 seconds). Instead of continual and uninterrupted exposure, EMDR repeatedly interrupts the client's focus. Exposure therapy typically maintains a focus on the targeted incident, while EMDR uses a free association process to elicit other related material. Research has shown that high levels of anxiety during sessions of exposure therapy typically predict good outcome with that modality; however in successful EMDR treatment, the client's distress is substantially reduced during the session. Suggestions will be made for future research to identify and investigate mechanisms of action in exposure therapy and EMDR.

Since official recognition of post-traumatic stress a number of ways. These include intrusive recollections, indicated by nightmares or “flashbacks” to the event, avoidance of stimuli associated with the trauma, and increased arousal (DSM-III-R, 1987). Behavioural approaches have been somewhat successful in treatment (e.g.., Cooper & Clum, 1989; Keane & Kalouped, 1982) however, such methods have been time consuming, require intense effort by the patient, and symptom improvement can be slow.

Recenti studi hanno dimostrato come la sindrome da stress post-traumatico (PTSD) possa causare nel cervello cambiamenti sia strutturali che funzionali. Studi di imaging funzionale tramite tomografia ad emissione di fotone singolo (SPECT) e ad emissione di positrone (PET) hanno evidenziato significative variazioni del flusso cerebrale in pazienti con PTSD durante la rivisitazione del trauma. A questo proposito sono stati riportati aumenti e diminuzioni di flusso nell’ippocampo, nell’amigdala, nella corteccia prefrontale mediale, nel cingolo anteriore e posteriore e nella corteccia temporale. Il modello prevalente collega i sintomi del PTSD ad una mancata inibizione dell’ amigdala, iperattivata dalla sensazione di incombente minaccia, da parte della corteccia prefrontale. E’ stato anche proposto che i cambiamenti strutturali dell’ippocampo e del cingolo anteriore rivelati dalla risonanza magnetica strutturale (RM) siano causati dalla risposta neuronale allo stress. L’obiettivo delle nostre ricerche e’ stato quello di analizzare la risposta funzionale e le variazioni strutturali in due gruppi di soggetti esposti a trauma occupazionale che hanno sviluppato (S=sintomatici, n=20) o no (NS=non sintomatici, n=27) il PTSD. Una parte dei S (n=16) è stata trattata con EMDR. La diagnosi di PTSD prima e dopo la terapia è stata basata sia sui criteri del DSM-IV sia su vari test neuropsicologici mirati. La SPECT (n=47) e la RM (n=33) sono state eseguite da 3 mesi a sei anni dal trauma e la prima è stata ripetuta dopo EMDR. I sintomi sono stati provocati da uno script individualizzato che ha riportato alla memoria il trauma e durante il quale è stato iniettato il tracciante di flusso cerebrale.
Le analisi eseguite hanno mostrato differenze significative tra S e NS nella risposta del flusso cerebrale allo script. Nei 33 soggetti in cui sono state eseguite sia la SPECT che la RM sono state trovate differenze significative sia funzionali che strutturali nella corteccia temporo-parietale sinistra e nell’ippocampo, regioni nelle quali gli score dei test neuropsicologici correlano significativamente con il flusso cerebrale. Nei soggetti con remissione sintomatologica dopo EMDR (R; n=11) sono state trovate rispetto ai soggetti che non hanno risposto alla terapia (NR; n=5) significative differenze di flusso in 4 aree corticali che processano funzioni deteriorate in corso di PTSD. Diminuzioni di flusso dopo la terapia sono state registrate nei R rispetto ai NR nell’ippocampo, nel giro fusiforme (corteccia parieto-occipitale) e nella corteccia visiva primaria. L’ippocampo è sede della memoria episodica ed autobiografica; il giro fusiforme processa il riconoscimento di volti, dei corpi e delle parole; la corteccia visiva primaria custodisce la memoria visiva degli eventi. La mancata inibizione e/o l’iperattività di queste regioni nel PTSD sono responsabili della rivisitazione patologica figurata e somatica dell’evento traumatico e della presenza di flashback ed immagini allucinatorie. Viceversa la corteccia frontale dorsolaterale ha mostrato un aumento di flusso nei R. Questa regione oltre ad essere deputata ad inibire nel sistema limbico la risposta patologica a stimoli che ricordano l’evento traumatico è essenziale per i processi di attenzione e del “senso di sé”, diminuiti in corso di PTSD e recuperati in seguito alla remissione della malattia. In questi soggetti la RM ha inoltre messo in evidenza il valore predittivo delle dimensioni dell’ippocampo rispetto all’efficacia della terapia con EMDR.
I nostri risultati confermano il coinvolgimento della corteccia temporo-parieto-occipitale nel PTSD e sottolineano il valore delle neuroimmagini sia nello svelare gli effetti neurobiologici dell’EMDR che determinare il valore delle indagini strutturali nel predirne l’efficacia.
Recent studies have shown that the post-traumatic stress syndrome (PTSD) may result in both structural and functional brain changes. Imaging studies using functional Single photon emission tomography (SPECT) and positron emission tomography (PET) shown significant changes in cerebral blood flow in patients with PTSD during revisiting the trauma. In this regard have been reported increases and decreases in flow hippocampus, amygdala, medial prefrontal cortex, the anterior cingulate and posterior and temporal cortex. The dominant model linking the symptoms of PTSD to a no inhibition of 'amygdala hyperactivity disorder by the sense of impending threat by the prefrontal cortex. E 'was also proposed that the structural changes of the hippocampus and
anterior cingulate revealed by structural magnetic resonance imaging (MRI) are caused by the response neuronal stress. The aim of our research and 'was to analyze the response
functional and structural variations in two groups of subjects exposed to occupational trauma that have developed (S = symptomatic, n = 20) or not (NS = non-symptomatic, n = 27) PTSD. Part of S (n = 16) was treated with EMDR. The diagnosis of PTSD before and after the therapy was based on both DSM-IV has on several neuropsychological tests targeting. SPECT (n = 47) and MRI (n = 33) are were performed from 3 months to six years from the trauma and the first was repeated after EMDR. Symptoms were caused by a script individual who reported to the memory of the trauma and during which were injected with a tracer of cerebral blood flow. The analysis performed showed significant differences between S and NS in the response of flow brain to the script. In the 33 subjects in which they were performed both SPECT and MRI were found significant differences in both functional and structural temporo-parietal cortex left hippocampus, regions in which the scores of neuropsychological tests correlate significantly with the flow in the brain. In subjects with symptomatic remission after EMDR (R; n = 11) were found compared with subjects who did not respond to therapy (NR, n = 5) significant differences in flow in 4 cortical areas that process functions deteriorated in the course of PTSD. Decreases in flow after treatment were recorded in R than NR hippocampus, within fusiform (parieto-occipital cortex) and in the primary visual cortex. The hippocampus is the seat of episodic memory and autobiographical, and the processes around the fusiform recognition of faces, bodies and words, the primary visual cortex preserves the memory visual events. The non-inhibition and / or 'hyperactivity of these regions in PTSD are
responsible for pathological figured revisiting the traumatic event and physical and presence of flashbacks and hallucinatory images. Contrast, the dorsolateral frontal cortex has showed an increase of flow in R. This region in addition to being deputies to inhibit the system limbic response to pathological stimuli that recall the traumatic event is essential for processes of attention and the "sense of self, decreased in the course of PTSD and recovered following remission. In these subjects, MRI has also highlighted the predictive value the size of the hippocampus compared the efficacy of EMDR therapy. Our results confirm the involvement of the temporo-parietal-occipital cortex in PTSD and emphasize the value of neuroimaging in revealing both the neurobiological effects of EMDR that determine the value of the structural surveys in predicting effectiveness.
Over the past two decades we have seen an increase in the relationship between Trauma and Addiction including eating disorders. Studies have focused on the psychobiological effects on the brain and PTSD symptomology. Eye Movement Desensitization and Reprocessing has gained great respect in the field for its efficacy and long term benefits with PTSD (Trauma Survivors) and Substance Abuse. Research suggests that PTSD clients are more responsive to treatments that specifically "process" traumatic memories such as EMDR. EMDR is an exposure treatment in which clients perform saccidic eye movements to process traumatic memories which in turn accelerates the processing of information involving a shift of cognitive structures ( including the assimilation of positive beliefs)." The application of EMDR apparently stimulates an inherent physiological processing system that allows dysfunctional information to be adaptively resolved, resulting in increased insight, cognitive restructing of potential relaspe triggers and physical cravings";Shapiro F.(1994). What we have learned over the years is that brain function can be altered by external stimuli; with the use of EMDR, eye movement "Naturally" occures in the rem sleep as well as activating it in the wake states has proven to be quite effective in the treatment of maladaptive behaviors: This workshop will look at this treatment modality its effectiveness and use with Trauma and Addictions; Workshop format will include lecture, case examples, and experiental exercises.
Most laboratory studies concerning eye movement desensitization and reprocessing (EMDR) have focused on investigating how EMDR reduces the vividness and emotionality of memories. There has however been less focus on what occurs to the accessibility of the memory following EMDR. To assess accessibility we utilized three experiments. In each participant’s encoded two pictorial stimuli (images), held one of them in-mind, and simultaneously performed or did not perform EMDR-like working memory taxing (WM-taxing) tasks. Participants then freely-recalled or recognized as many details as they could from both images. In experiments 1a and 1b we found that free-recalled accessibility was reduced for the image that was held in-mind whilst WM-taxing tasks were performed, compared to when no-WM-taxing tasks were performed. In experiment 2, accessibility was measured in reaction time (RT). Decreased accessibility (slower RT) was observed when an image was held in-mind whilst WM-taxing tasks were performed. The decrease in accessibility, along with an observed decrease in vividness, possibly helps to give a more complete picture of EMDR’s effectiveness. Additionally it demonstrates that the effects of EMDR can be observed via non self-report measures.

Background:
Initial studies have shown that posttraumatic stress disorder (PTSD) can be effectivelytreated in patients with a psychotic disorder. These studies however used adapted treatment protocols, avoided direct exposure to trauma related stimuli or preceded treatment with stabilizing techniques making treatment considerably longer in duration.
Method:
An open trial in which adult subjects with a psychotic disorder and a comorbid PTSD (n = 27) received a maximum of six Eye Movement Desensitization and Reprocessing (EMDR) therapy sessions. PTSD symptoms, psychotic symptoms and additional symptoms were assessed at baseline and end-of-treatment.
Results:
The dropout rate was 18.5 percent (five subjects). Only five of the twenty-two completers (22.7%) still met criteria for PTSD after treatment. PTSD symptoms, auditory verbal hallucinations, delusions, anxiety, depression, and self-esteem all improved significantly. Paranoid ideation and feelings of hopelessness did not improve significantly. Treatment did not lead to symptom exacerbation in subjects. There were no adverse events, such as suicide attempts, self-mutilation, aggressive behavior or admission to a general or psychiatric hospital.
Conclusions:
This pilot study shows that a short EMDR therapy is effective and safe in the treatment of PTSD in subjects with a psychotic disorder. Treatment of PTSD has a positive effect on auditory verbal hallucinations, delusions, anxiety symptoms, depression symptoms, and self-esteem. EMDR can be applied to this group of patients without adapting the treatment protocol or delaying treatment by preceding it with stabilizing interventions.
A therapy developed in 1987 by psychologist Dr. Francine Shapiro is helping patients diminish and remove the effects of those disturbing memories.The therapy is called eye movement desensitization and reprocessing (EMDR). Trained therapists use it by alternating stimulation between the two hemispheres of the brain while the patient focuses on the trauma."It's a fairly complicated therapy," said licensed counselor Roger Ludwig. "I go through a series of steps with my patients. "Those steps include getting to know the patient and understanding what triggers the anxiety.The patient also must isolate a mental snapshot to represent the event, a feeling about the event and thoughts about the event, such as "He's going to kill me."The doctor then runs the patient through a sequence of bilateral stimuli.
Fears and phobias associated with EMDR situations where a single event for the well-structured, and short-term treatment is not compelling. Exposure in vivo EMDR'ın cognitive behavioral interventions, such as advantages, the fear-inspiring re-creation of the situation, situation (eg, sexuality, illness or death-related moments, situations) or phobic stimuli (eg, aircraft, mouse, snake) real life should be revived. An adult male experiencing symptoms of depression was administered five discrete
treatments based upon Eye Movement Desensitization and Reprocessing (EMDR). The first
treatment was with the ocular hand-tracking described by Shapiro (1995), the next four
were computerized simulations. Each method showed effectiveness in reduction of selfreported
depressive symptoms immediately following treatment. No appreciable difference
was shown between ocular hand-tracking and isomorphic computer simulations; marginal
difference, however, was shown between left-right tracking (either in vivo or computerized
simulation) and frontal presentation of visual stimuli. The results suggest a nontrivial effect
of the treatment(s), and in particular suggest the left-right tracking component of the
treatment merits further study.
In contrast with the learning model which proposes a strategy of gradual exposure to the
44 feared stimuli, the primary goal in EMDR is the processing of traumatic memories which are thought to be impaired. One advantage of EMDR above an exposure approach involves client comfort. Prolonged real-life exposure to anxiety provoking stimuli or thought is not always easy to pursue. Consequently, clients are not always ready or motivated enough to
endure such a procedure; a procedure which also holds a potential risk of drop out before
treatment can often not be successfully concluded. Another possible advantage of EMDR
relates to the costs of treatment. For example, it may be more useful to apply EMDR than
exposure in vivo treat flying phobia for which in many cases, as part of their in vivo
treatment, clients have to take costly flights. The most important advantage seems to be the possibility of utilising EMDR for situations
where the critical triggers cannot be reproduced or simulated in real life (e.g., certain sexual,
illness or death situations) or, more generally, for which phobic stimuli are hard to obtain.
This workshop focuses on the clinical application of EMDR with specific phobias.
Participants will learn when and how to apply EMDR with phobias and integrate this into a
general (cognitive-behavioural) treatment approach. This approach is illustrated by several
video taped treatments.
Objective: to illustrate the EMDR usefulness within the psycho-dynamical therapy. Foreword: a type of pain exists
that’s nameless and inenarrable. During our psycho-dynamical practice as EMDR specialists, we met various
defensive modalities adopted by patients presenting diverse pathologies to elude grief associated to that type of
pain left segregated within the emotional portion of the brain. In particular, we take into account patients keen
to coactively repeat the traumatic experience either directly in-person or through using others. The presumption
is that the traumatic experience, when too early, too invasive or reiterated, may render it impossible to be
expressed verbally (by use of the cortex), leading to express it either through the body or through reiterated use
of the traumatic experience itself (coactions to repeat). This last point, within our clinical experience, seems
being linked to ambivalent feelings vs. the very resolution of the experience itself (healing). Patients living this
type of situation may namely be entangled by two conflicting wishes to either wish a real improvement of their
condition or to seek maintaining that pathology in the fear of loosing the sense of security inspired them by the
type of fake identity they built around the trauma. Benefits: using the technique of bilateral sensorial stimuli
strives breaking off that defensive mood that feeds pathological coactions to reiterate the experience as the
means to tolerate the grief. Namely its purpose is to penetrate through those defensive modalities and to
successfully aid patients to abandon them thanks to a low structured context adequately freed by internal or
external conditioning (ambivalence, judgement, rationalization, etc.).
Often it is said that traumatization in the preverbal
period cannot be treated. Doctors, psychologists, police officers
tell parents to wait until the child is older, so it can talk about
it. In this presentation we will demonstrate the contrary. That
EMDR is a very useful method for processing preverbal traumatic
memories in infants.
We will start by explaining how to structure the EMDR sessions,
illustrated by videotapes of boy (nearly three) and his parents
who was traumatized by medical treatments starting in the first
months of his life. We will explain how to access and activate
the traumatic memories, how to help the child during desensitization
by timing and dosing stimuli, and how to determine
whether the traumatic memory is completely processed.
Then we will illustrate the devastating consequences on the
development of an infant of preverbal traumatization. These
consequences usually are underestimated by parents, pediatricians and even psychologist. As these children get older, their
development is more and more disturbed and they behave like
children with ADHD of Autistic disorders, and sometimes even
get diagnosed.
After EMDR these infants start to recover and grow rapidly in
emotional en social development and the use of play. Their oppositional
behavior changes in daily life activities as changing
a diaper, brushing teeth, going to bed and changing clothes.
They become less resistant.
Parents see that the presumed characteristics of their child
change and looking back they recognize those as symptoms of
traumatization. This is even clearer with children who are
traumatized by medical treatment. They are usually referred to
EMDR therapist because of their resistant behavior in the hospital
which makes medical treatment impossible. The urgent requirement
of medical care is often the main reason for staking
treatment. The realization of the impact of the medical treatment
on their child is very painful for the parents. Nowadays parents are often asked to assist during these treatments and
they feel as perpetrators. If necessary we also offer parents
EMDR.
New and unique: The use of EMDR in infants is quite unique
because it requires knowledge of infant psychiatry and traumatization
and EMDR It is a very complicated but rewarding
treatment.
The aspect of traumatization and the use of EMDR in medical
treatment is very underestimated The material in this workshop
is new and unique for the use of EMDR in these children.
Learning objectives:
- Participant will learn the basic tenants of the assessment of
preverbal traumatization.
- Participant will learn the basic tenants of the assessment of
preverbal medical traumatization, which is very much underestimated.
- Participant will learn how to access and activate the traumatic
memories in infants and how to help the child during desensitization
by timing and dosing stimuli.
-Participant will learn how to determine whether the traumatic
memory is completely processed in these infants.
My second case is J, a 7-year-old boy who witnessed his father murder his mother approx 1 year ago. He was then held hostage by his father for 6 hours with the body of his mother lying in a pool of blood in the kitchen while the police negotiated with J’s father for his release. My first involvement with J was an in depth assessment of his emotional attachment and placement needs for the court. During this process, he developed frequent infantile rages and I advised the system of care around J on their management. At one stage, he had recurrent dreams about joining his mother in heaven and he was discovered attempting to strangle himself. I will discuss how I used the safe place protocol to alleviate his distress. I am now using EMDR directly with J. I use a variety of bilateral alternating stimuli. These include drumming, musical symbols, and a xylophone. I am able to get J to draw sequentially with each new drawing generated by J performing the “Butterfly hug.” I was inspired to use this technique after I attended a wonderful workshop facilitated by Michel Silvestre entitled “integrating family therapy and EMDR.” I hope to discuss in this presentation how EMDR can be combined with other therapeutic approached in a case of extreme trauma experienced by a 6-year-old boy who is now effectively orphaned as his father is in prison probably for the rest of his natural life. I will also allude to some of the techniques discussed by Dr. Atle Dyregrov at the 5th annual UK and Ireland conference in London in March last year. He presented in depth therapeutic work with a girl who suffered the trauma of her mother’s suicide. I will discuss some of the challenges presented by traumatic grief and how the EMDR protocol can be adapted for use in children.
An important feature of PTSD is that
it is not very likely that the same
traumatic event will ever happen
again. Accordingly, if a client has been raped
and succesfully been treated with EMDR,
generally not many clinicians will feel the urge
to prepare this client for a next rape.
Conversely, in a number of cases (particularly
phobic conditions) the client does have to
anticipate future situations in which fear
evoking stimuli are present; and where he will
have to interact with these. For example, if a
person who suffered from a dental phobia has
been successfully treated for his phobia, it is
likely that he will still have to undergo invasive
dental work, such as injections, root canal
treatments, or extractions. This has implications
for treatment. As a result of the successful
application of the EMDR basic protocol, the
likelihood or severity of the initial threat may
have been reappraised, and the incident that
initially felt traumatic may have been
reattributed to an innocuous event. Yet, it is
possible that the client is still not completely
convinced of her ability to cope, and therefore,
avoids certain activities or situations. In that
case, the client should be properly prepared for
future encounters with the anxiety provoking
objects and situations. One way to prepare the
client for such confrontations, is the use of in
vivo exposure.
Behandlung psychotraumatischer belastungsstörungen mit EMDR
Literaturbefunde und eigene klinische Erfahrungen zeigen, dass bei einer großen Anzahl von Schädel- Hirn-Traumatisierten die komorbide Diagnose einer ASD oder einer PTSD vergeben werden muss. Die Entstehung von Intrusionen kann unterschiedlich plausibilisiert werden. Die Autoren schlagen als Erweiterung bestehender Ansätze dazu die Möglichkeit "phantasmatischer Intrusionen" vor, als Ausdruck bildhafter Ausgestaltungen der erlebten Todesgefahr. Kasuistisch wird die erfolgreiche Anwendung von EMDR gezeigt. Es werden bestimmte Modifikationen vorgeschlagen, mit denen ein möglicherweise für diese Gruppe von Patientinnen und Patienten charakteristischer Assoziationsprozess in Gang kommen kann.
Literary findings as well as our own clinical experiences show that a large number of patients with head injuries had to be additionally diagnosed with either ASD or PTSD. The formation of intrusions can be interpreted differently. The authors propose possible "phantasmatic intrusions" as an enhancement of current approaches for the pictorial forms of the experienced life-threatening danger. A case report shows the successful use of EMDR. Certain modifications are proposed in which the characteristic association process for this group of patients can be launched. [Author Summary]
