Abstract:
Using therapeutic experiences from the USA (Herman, Putnam, Ross) and the Netherlands (Olthuis, van der Hart) Luise Reddemann (Bielefeld) and Ulrich Sachsse(Goettingen) developed an inpatients’ program for female and some male patients with symptoms, that result from type II traumata, fulfill the phenomenological criteria of BPD and are understood as chron. PTSD/DES. The program utilizes the coping strategies of the patients for stabilisation: splitting (building up an only good world of safety, support and shelter against the only bad, demonized world of trauma); derealisation, dissociation(imagery); depersonalisation (Qi Gong, Feldenkrais). We tell and teach our patients: Be Borderlines- but inside, not in your outer social life or your therapeutic relationship! Trauma-synthesis is done after stabilisation by trauma-exposition every two weeks (EMDR, screen-technique). The patients stay for 3-5 month, sometimes twice, with very good results.

Abstract:
This case is presented with the client’s full knowledge and consent. Personal details have been changed to protect the client’s identity.
This case involved an adult male client with an over-riding sense of low self esteem and self-sabotaging behaviours, including binge eating disorder, gambling and drinking.
The client had no clear target memories relating to any of his behaviours or beliefs about himself – but reported a deep-seated sense of self-disgust – with no clear idea of where this feeling originated from or the thought behind it.
With nothing else to work with, we focused on the target body sensation of self-disgust, orientating in the heart area. Upon this point, the client reported having a circumcision operation as a child of around 7 or 8 years of age. Although this did not initially appear relevant, the client was asked to stay with the body sensation in the groin area, upon which the client reported “seeing” the surgeon’s face with a look of disgust on it. The surgeon removed the client’s foreskin and threw it in a plastic basin, looking at the client with a look of utter disgust, which the client interpreted and internalised as “You disgust me.”
Following on this unusual revelation, with the client’s involvement, collaborative evidence was sought on the memory. The surgical procedure was confirmed to be true by the client’s mother, although it was also confirmed by medical staff and the client’s mother that the client has remained under anaesthetic throughout the entire procedure. This may explain the lack of initial memory and why the memory was only accessible through body sensation.
Upon further inquiry, the client stated: “I didn’t see the surgeon with my eyes – I saw his disgust in my heart.”
The client’s mother further confirmed that the surgeon had indeed been disgusted and possibly expressed his disgust – however, not at the client, but rather at the previous inferior surgical attempt at a circumcision which had been botched during the client’s infancy, hence the client’s need for the second operation. The client somehow had “felt” the surgeon’s disgust – but being of such a young age, interpreted it as being disgust at his boy, rather than the previous operation.
What is interesting to note is that the client made a full recovery with a normal attribution of self-esteem and a complete absence of self-sabotaging behaviours with two treatment sessions, following his initial body memory. At 6-month follow up there continues to be no return of any previous self-sabotaging behaviours (e.g., drinking, gambling, binge eating) and the client expresses a healthy self-esteem.

Abstract:
There is no qualitative knowledge of Eye Movement Desensitization and Reprocessing (EMDR) with a sub-clinical stressed population. The vast majority of EMDR research has focused on traumatized populations, leaving a significant gap in what the non-traumatized or sub-clinically stressed clients experience. Sub-clinical stress includes any level of stress that does not meet the DSM-IV-TR criteria for PTSD. The purpose of this study was to evaluate the lived experiences of body sensations, emotions, beliefs, and imagery during EMDR treatment of participants with sub-clinical stress. Participants fit into either a young adult (18-35), adult (36-49), or older adult (50-60) maturity category and did not meet the criteria for PTSD. The sample consisted of 12 participants, from a large metropolitan area in the Northeastern United States. A qualitative phenomenological design was used to gather data following the EMDRIAs treatment protocol including a final interview asking questions about what the participants experienced in their body, thoughts, emotions, and memory images. The data was analyzed using constant comparative techniques using open coding and will be verified with member check techniques. The results identify five thematic holistic experiences across the participants. The themes of responsibility, safety, choices, power, and value emerged from the data. The results imply that is may be necessary to address all 5 themes for effective stress resolution. Also, the scholarly, clinical, and practical understanding of the Adaptive Information Processing Model concepts of responsibility, safety, and choices manifest in participants lived sensory experiences are now expanded and in need of additional research. This book is about what I have learned about EMDR and its clinical use, especially with combat veterans. It is also about what trying to understand how EMDR works has taught me about psychotherapy in general. That second lesson is what I call the Four-Activity Model (FAM) of Psychotherapy, which grows out of a concept that Francine Shapiro refers to as Accelerated Information Processing (AIP). Shapiro's AIP description gives name to the idea that learned psychopathology can be considered dysfunctional held information, including thoughts, emotions, sensations, and behavior, that can be modified more quickly than previously believed by most therapists. The Four-Activity Model is an attempt to conceptualize how psychotherapeutic activity can be used most efficiently to reprocess dysfunctional held material and thereby resolve psychological problems.Finally, this book is about what psychotherapy in general has taught me about EMDR. Even in her early explanations of EMDR, Shapiro taught that it was an integrative method, that it relied on the lessons learned by years of clinical work using dynamic, behavioral, and humanistic methods. In this book I will attempt to elaborate on that relationship and offer specific therapeutic suggestions that will rely on the wisdom of previously established therapeutic methods, as well as the wisdom of past philosophical inquiry and religion. The book starts with EMDR, proceeds to try to describe how EMDR and other methods can be integrated into an overall model of psychotherapy, and then works its way back to the concrete practical integration of psychotherapy in general. The second half of the book has a practical focus on examples that are created mostly from my experience working with combat trauma. There have been suggestions in the literature since 1994 that eye movement desensitization and reprocessing (EMDR) may serve as an effective adjunct to the addiction treatment process; however, follow-up research in this area has been limited. This case study of a cross-addicted female includes a case review illustrating how EMDR was used in the continuing care process and a semistructured phenomenological interview conducted at 6-month follow-up. Prior to this course of treatment, the participant was treated 12 times with traditional approaches but was unable to achieve more than 4 months of sobriety at any given time. Following EMDR, the participant reported 18 months of sobriety and important changes in functional life domains. The phenomenological interview revealed six critical themes about the addiction and recovery process that can offer insight to clinicians treating co-occurring addiction and trauma.

The field of positive psychology is emerging as a specialty area within psychology. As such, it has much to offer us as clinicians and those of us who have been involved in peak performance work. This workshop provides the participants with immediately usable techniques that can enhance their existing resource work with clients. There is a well established empirical basis for the efficacy of the techniques presented, very much in keeping with EMDR’s scientific roots. This workshop is the first effort (originally conducted in 2003 at the EMDRIA Conference) 20 to integrate into EMDR Peak Performance work specific techniques from positive psychology, the emerging science of happiness and excellence and strengths such as courage. The presenter will explain how the EMDR Peak Performance protocol (which she co-created) is based on the standard EMDR protocol with three modifications: the Future is emphasized; resource development is oriented toward empowerment and possibility; and skills are taught to help clients reach optimal functioning. While the ‘Peak’ protocol already had self-actualization as its goal, its efficacy is enhanced by integrating the positive psychology techniques of using: positive emotions in the performance venue of importance to the client; the Appreciative Inquiry method for envisioning the future and for performance improvement; and optimistic explanatory style. Given time constraints, it is not possible to fully prepare participants to use the Peak Performance protocol. However, in an interactive format that includes a mini practicum, participants will be able to: 1-describe the key differences between the Peak and basic protocols, 2-help clients develop ‘Peak’ resources, 3-begin to establish an empowerment stance with clients, 4-summarize the benefits of teaching clients basic positive psychology concepts, 5-pursue further learning from the extensive positive psychology bibliography

According to Tedeschi & Calhoun (1995; 2004 and 2006), Post Traumatic Growth (PTG) consists of five ‘domains’: discovery of new possibilities in life; improvements in relating to others; an increased sense of personal strength; greater appreciation of life and spiritual changes. The first author’s experience of treating Road Traffic Accidents (RTA) victims with Eye Movement Desensitisation & Reprocessing (EMDR) suggests that the experience of PTG is far wider and more complicated than Tedeschi & Calhoun’s domains. This paper reports on the current status of a study whose aim is to identify PTG that may be occurring at any point from the immediate aftermath of an RTA through to completion of EMDR. The study so far consists of 11 interviews utilising an Interpretative Phenomenological Analysis (IPA) methodology. So far well over 50 PTG themes have emerged although only the first hermeneutical analysis has been completed on the interviews analysed so far, it is currently difficult, therefore, to establish the degree of ‘theme overlap’. However, new themes include growth ‘by proxy’ (growth in those who have come into contact with the participants, but who have not themselves been traumatised by the RTA); somewhat paradoxically, ‘pre-trauma growth’ apparently activated by a subsequent RTA; and a ‘cascade’ of growth attributed to events subsequent to the initial trauma (such as the EMDR and the EMDR therapist). Some implications of these themes are also discussed, particularly in relation to EMDR.

This study was set in a mainstream primary school in Scotland where a group of children had been identified by their class teachers and parents as presenting as quiet, shy, withdrawn and/or anxious. The researcher was an educational psychologist in the local authority where the study took place. 5 primary aged children at stages P5, P6 and P7 (aged 9 to 11 years) of the Scottish Primary education system were involved in a group process which took place in their school, consisting of 6 sessions over the course of 2 months. The intervention combined elements from Eye Movement Desensitization and Reprocessing (EMDR) and Solution Focused Brief Therapy (SFBT). This was a strength based approach which aimed to focus on positive strengths and increase resilience. The individual nature of the therapy required a flexible model. A qualitative research methodology was used. The general aim of this research was to investigate this combined therapeutic process within an applied psychology perspective. The research set out to study the experiences of a group of children during and after they participated in the process. A further aim of the research was to explore the process from my own perspective as the therapist and facilitator. Data was collected during and after each session and post intervention data was collected from children, parents and teachers. Interpretative Phenomenological Analysis (IPA) was used to analyse the data. Findings indicated that the children found the intervention helpful and they were able to identify particular aspects of SFBT and EMDR which they had found useful.

The purpose of this study is to explore: (a) the lived experiences of clients participating in Eye Movement Desensitization and Reprocessing (EMDR) treatment as part of their addiction continuing care, and (b) the impact of the EMDR experience on their lives as individuals recovering from addiction. A review of the critical literature was conducted to reveal a wealth of information concerning EMDR's efficacy with posttraumatic stress disorder (PTSD), suggestions for implementing EMDR into addiction treatment, and various ethical-clinical issues that continue to be addressed within the scope of EMDR treatment. Research on implementing EMDR as part of the overall addiction recovery process is minimal at present. In this study, ten women who received EMDR at a treatment program in the urban Midwest participated in a semi-standardized phenomenological interview to share their experiences with active addiction, treatment, EMDR, and recovery. Using Giorgi's Descriptive Phenomenological Psychological Method to analyze the data, four major thematic areas emerged: safety as an essential crucible of the EMDR experience, accessing the emotional core as vital to the recovery experience, lifestyle change, and using a combination of factors for successful treatment. All ten of the women who came forward through the established recruitment process expressed positive sentiments about their EMDR experiences, and in various degrees, they credited their EMDR treatment with being a crucial competent of their addiction continuing care processes. As a collective sample, the participants shared experiences about how EMDR altered their perspectives of self, others, and situations. Traditional models of addiction treatment and relapse prevention fail to consider the role that unresolved trauma plays in an addicted woman's recovery experience. Implementing Eye Movement Desensitization and Reprocessing (EMDR) into the treatment process offers a potential solution to this problem. Ten women (alumnae of an extended-care treatment facility) participated in a semistandardized interview to share their experiences with active addiction, treatment, EMDR therapy, and recovery. With the use of A. P. Giorgi's descriptive phenomenological psychological method for analysis, four major thematic areas emerged from the interview data: the existence of safety as an essential crucible of the EMDR experience, the importance of accessing the emotional core as vital to the recovery experience, the role of perspective shift in lifestyle change, and the use of a combination of factors for successful treatment. All 10 women, to some degree, credited EMDR treatment as a crucial component of their addiction continuing-care processes, especially in helping with emotional core access and perspective shift. Since Francine Shapiro's first published paper on EMD therapy in 1989, there has been a tendency toward polarization in EMDR research. Those who tend to believe in the effectiveness of EMDR tend to find results which confirm their point of view. Those who have been very skeptical about the effectiveness of EMDR have tended to produce findings which validated their perspective also. The result of this has been years of back and forth research, without a great deal of moving forward by asking new questions based on previous findings. This literature review involved evaluating all available research on EMDR published in English as of March 1, 2002. Studies were categorized as contributing to knowledge about EMDR in general, or emphasizing specific aspects. Specific aspects were breadth of application, subjectivity of effects, EMDR's effect on intrusive PTSD symptoms, the necessity for eye movements, how EMDR works, if it does, and whether it produces lasting change.Findings included a probable effect from EMDR in treating traumatic memories. It has not been found equally effective in treating other kinds of anxiety or other psychological maladies. Subjectivity is an ongoing issue in EMDR research, yet there are several forms of data indicating an effect in a context in which subjectivity could not have been a significant factor. If EMDR works better for intrusive PTSD symptoms compared to others, the difference is minor. The necessity of eye movements has not been clarified, largely because of the use of alternate forms of bilateral brain stimulation as a control condition when these in fact may promote a similar process. EMDR appears to produce change that is as lasting as any other form of psychotherapy. The main conclusion is that there is a paucity of research including a variety of independent variables. The ongoing battle as to whether EMDR works or not has delayed thorough inquiry into for whom it works, compared to for whom it does not work. It is argued that the field, as well as the clinical population, would be well served if research could move in the direction of rectifying this situation. Additionally, a gap in the literature exists in giving a voice to the participants’ experience of EMDR treatment. The purpose of this study was to evaluate the lived experiences of body sensations, emotions, beliefs, and imagery during EMDR treatment of participants with sub-clinical stress. Participants fit into either a young adult (18-35), adult (36-49), or older adult (50-60) maturity category and did not meet the criteria for PTSD or ASD. The sample consisted of 12 participants, from a large metropolitan area in the Northeastern United States. The participants chief complaints included economic stress, relationship stressor, and critical self-talk. The researcher employed a qualitative phenomenological design to gather data in order to answer the research question: what are the lived experiences of sub-clinically stressed participants’ body sensations, beliefs, emotions, and memory imagery during EMDR treatment? The data was collected using the EMDRIA approved research treatment protocol. The researcher included the floatback technique in every reprocessing session to complying with the tenet of the Adaptive Information Processing Model. After installing a safe-place and five reprocessing sessions, the researcher administered a final interview asking questions about what the participants’ experienced in their body, thoughts, emotions, and memory images. All reprocessing session were completed when the participant indicated a SUDs of ‘0’ and a VOC of ‘7’. The data collected during every reprocessing session and the final interviews were analyzed using constant comparative techniques and open coding; verified with member check techniques. The results identify five thematic holistic experiences common in all participants. The themes of responsibility, safety, choices, power, and value emerged from the data. The findings indicate a participants’ lived experience may expand the cognitive themes described in the Adaptive Information Processing Model. The themes of responsibility, safety, power, and value were targeted and reprocessed as disturbing memories. The participants experienced these themes as feeling overly responsible, unsafe, valueless, and/or powerless. The holistic manifestation of the themes of choices emerged as the outcome towards a more adaptive perspective of the disturbing targeted memories. The results of this study further indicate that it may be beneficial to address all four maladaptive themes in mind and body for effective sub-clinical stress resolution. The findings inform scholarly and clinical understanding of the Adaptive Information Processing Model concepts of responsibility, safety, and choices. The findings of this study preliminarily expand the previously unknown holistic manifestation of these themes in sub-clinical participants’ lived sensory experiences. These themes are now in need of additional research to verify and validate the findings of this study.

Animal models conceptualize anxiety as a response to potential danger while fear is a response to present danger. The way humans experience anxiety involves our capacity for higher thinking while the human experience of fear appears to be much the same as the animal model. This article examines these differences at both a phenomenological and neurological level and highlights implications for the treatment of conditioned fear in PTSD. The stimuli for human fear are sensory-perceptual, while the stimuli for most forms of anxiety are conceptual-linguistic. Individuals in a state of fear/terror undergo a radical shift from top-down to bottom-up processing in which access to conceptual-linguistic thought processes is severely restricted and the frontal regions of the cortex are no longer able to override impulses from brain stem and midbrain regions. Conditioned fear involves actual neurological changes in the limbic system. To overcome a traumatic memory, the individual must (1) gain some level of access to the bottom-up state in order to habituate or extinguish the conditioned fear response, and (2) also achieve access to the top-down state in order to process the fear. ]Sage Journals]

Abstract:
Positive Psychological Change (PPC) following trauma is a developing field for which there is no standard terminology. The plethora of labels, of which Post Traumatic Growth (PTG) is probably the most common descriptor, arguably masks a significant gap in clinical and theoretical understanding of the phenomenon. One specific gap addressed by this study is PPC following psychological trauma stemming from a Road Traffic Accident (RTA) in which the person involved has subsequently received Eye Movement Desensitisation & Reprocessing (EMDR). To investigate this gap in knowledge, an Interpretative Phenomenological Analysis (IPA) approach was used and twelve participants recruited via a snowball sampling method. The participants were then interviewed using a Semi-structured Interview Questionnaire (SSIQ) and the interviews were then transcribed for IPA analysis. Key themes that emerged included Navigational Struggle (NS) to describe Negative Psychological Change (NPC), and Network Growth (NG), to describe PPC. At any one post-RTA/EMDR point there was a preponderance of one over the other, however, NS and NG were inseparable and found to co-exist along an NS-NG continuum. In addition, Figurative Language Use (FLU) had a significant role in both NS and NG yet was independent of both and apparently driving change towards the development of NG. Whilst NS and NG were both post-trauma phenomena, FLU seemed to hallmark expansion of memory networks as part of a general maturation process post-RTA. Furthermore, there was evidence that participants were incorporating their traumatic experiences via FLU into the rebuilding of their assumptive worlds. To account for these findings, an extension to Adaptive Information Processing (AIP) – the theory widely accepted to underpin EMDR - is proposed based upon a hypothesised Plasticity of Meaning (PoM), which is observable through FLU. PoM predicts which, why and how memory networks connect resulting in the adaptive processing predicted by AIP. The study’s findings are re-examined in terms of consequential modifications to the clinical use of EMDR. Extensive suggestions for further research are provided.

La rilevazione di disturbi uro-ginecologici è in costante crescita, è ciò probabilmente a causa della maggiore attenzione che i clinici dimostrano nei confronti di disturbi variegatati, a volte quasi impalpabili, ma che possono anche dimostrarsi invalidanti. Oltre al vaginismo e alla dispareunia, ben noti nella letteratura scientifica da decenni, crescente interesse stanno dimostrando disturbi come la cistite interstiziale, il dolore pelvico e la vulvodinia. Complessivamente considerati, la componente psicosomatica di tali disturbi viene abbondantemente confermata dalla letteratura. A prescindere dagli approcci più chiaramente monolaterali e riduttivi, si nota nella letteratura un interesse consolidato per una terapia che sia per definizione integrata: ginecologica/riabilitativa, psicoeducazionale, sessuologica e psicoterapeutica. L’EMDR si propone in questo ambito clinico come uno strumento di straordinaria versatilità, potendo infatti intervenire sia a livello delle cause remote (eventi stressanti/traumatici, educazione sessuale distorta, etc.), che delle contingenze attuali che mantengono o peggiorano la sintomatologia, offrendo inoltre la possibilità di intervenire direttamente sia sul dolore inteso come sintomo, che sugli scenari futuri connotati negativamente dalla paziente, che frequentemente sono connessi alla percezione del dolore stesso. Infine, si rivela di particolare utilità nel caso in cui il dolore sia correlato a difficoltà relazionali caratterizzate da scarsa assertività. The detection of uro-gynecological disorders is growing, this is probably due to the increased attention that clinicians demonstrate against variegatati disorders, sometimes almost intangible, but can also prove crippling. In addition to vaginismus and dyspareunia, well known in the scientific literature for decades, are showing increasing interest in disorders such as interstitial cystitis, pelvic pain and vulvodynia. Overall, the psychosomatic component of these disorders is abundantly confirmed by the literature. Apart from unilateral and reductionist approaches more clearly, there is a vested interest in literature for a treatment that is by definition integrated: gynecological / rehabilitation, psycho-educational, sexology and psychotherapy. EMDR is proposed in this clinical setting as an instrument of extraordinary versatility, allowing it to intervene at the level of remote causes (stressful events / trauma, distorted sex education, etc..) That the current quotas that maintain or worsen the symptoms, offering the possibility to intervene directly understood as a symptom is pain, which adversely on future scenarios of patient characteristics, which are frequently associated with pain perception itself. Finally, it proves particularly useful in cases where the pain is related to interpersonal difficulties characterized by lack of assertiveness. As part of the workshop will examine the following issues: • Differential diagnosis between different types of pain, uro-gynecology. • Evaluation multifactorial etiology: biological, psychosexual, relationship and presence of traumatic events. • Role of sexual abuse. • psychophysiological mechanisms in the onset of the disorder: the role of muscle tension. • Psychological characteristics of women with uro-gynecological pain. • psycho-diagnostic screening tools. • Work on the main strategies for assessment and intervention uro-gynecological and pharmacological • Planning and stages of treatment with EMDR, and their integration with different psychotherapeutic approaches: areas of inquiry, psychoeducational aspects, technical sexological specific target features, using EMDR in different stages of treatment.

Si les troubles des comportements alimentaires (TCA) sont considérés par de nombreux auteurs comme une pathologie addictive du fait de schémas comportementaux et d’un support neurobiologique comparables aux autres dépendances (substances ou comportements), l’accord paraît unanime pour y voir une réponse spécifique à un modèle biopsychosocial. Un tel modèle implique : des facteurs inducteurs et déclencheurs, et des facteurs facilitants et de pérennisation. Par exemple, des travaux récents mettent l’accent sur la prépondérance de facteurs socio-culturels indissociables des forces médiatiques actuelles. L’importance de ces derniers facteurs se fait particulièrement ressentir depuis la seconde moitié du 20ème siècle et pourrait aller « crescendo ». En outre, la problématique des TCA est rendue plus complexe par l’existence d’une lourde comorbidité dont les éléments pathologiques sont autant causes que conséquences. Notons que 40% des patients souffrant de TCA ont eu, à un moment de leur vie, un PTSD. Nous, praticiens, ne sommes pas étonnés de constater que bon nombre de ces patients souffrent d’une altération notable de leur identité. Le caractère dit « synclétique » de la thérapie EMDR permet une approche intégrative dans le traitement des TCA : un aspect cognitif indéniable, le processus associatif induit par les stimulations alternées met souvent en lumière des matériaux reflétant des conflits intrapsychiques plus ou moins archaïques. Le travail portant sur l’imagerie mentale ou les états dissociés du moi peut aussi être associé dans les cas difficiles de patients souffrant de TCA. Le travail d’anamnèse et l’approche phénoménologique jouent un rôle primordial dans l’approche EMDR des TCA. L’un des aspects forts de la thérapie EMDR est l’identification de « clusters possibles » représentatifs des thématiques inductrices des souffrances et des symptômes inhérents à l’expérience de vie tragique de ces patients. Le plan de la thérapie est bien sûr personnalisé pour chaque patient.

This research study seeks to illuminate the impact of post-traumatic anxiety on elite level competitive cycling performance. Although there is much outcome research supporting the effectiveness of EMDR in treating post- traumatic anxiety in general, relatively little is known about the nature of anxiety among athletes who have experienced accident, injury or other trauma in sporting contexts. Interpretative Phenomenological analysis (IPA) will be used to gather data from an opportunity sample of competitive cyclists who have experienced trauma during sporting participation. Analysis of the data will attempt to capture these participants’ experience of anxiety and its impact upon performance. This information will be used to inform therapeutic practice when using EMDR with this client group. It is hypothesised that due to the specific psychomotor actions necessary for competitive cycling, adaptations to the future template within EMDR will be required to maximise therapist efficacy when working with this particular client group.

There is some debate (Greenwald & Shapiro 2010) as to the usefulness of AIP, the theory, (which is more correctly a hypothesis), widely accepted to underpin EMDR. On the one hand AIP is seen as redundant because of the extensive evidence base for EMDR. In essence: EMDR works therefore what role does AIP have? On the other hand theorising results in research hypotheses, the expansion of knowledge and thus further understanding. The current author supports the latter position, but believes AIP does not fully account for psychological change in EMDR. I shall argue that AIP, as it stands, actually constricts research into EMDR as it is essentially a theory of the reduction of negative psychological change, although it does ‘leave the door open’ to the possibility of positive psychological change (PPC). However, negative psychological change and its reduction is by no means the only effect of EMDR. This suggests that AIP is a ‘partial theory’ of psychological change. It also means more fundamentally that there is a ‘blind spot’ in research and that the full potential of EMDR is being consistently – and considerably – underestimated. In the current form of AIP, the use of words such as ‘digested’ or ‘metabolised’ whilst extending the physiological corollary central to AIP, does little to explain the How, Why, and When, of PPC. I shall propose a theory extension I have coined: ‘Plasticity of Meaning’ (POM), which is grounded in phenomenological findings, superficially parallels the Consolidation of Memory Theory and goes at least some way to converting AIP into a unified theory of psychological change and thus provide research ‘pointers’ to expand the appreciation of the potential of EMDR.

Existe algo de debate (Greenwald & Shapiro 2010) con respecto a la utilidad del SPIA, la teoría (mas correctamente nombrada como una hipótesis), ampliamente aceptada para corroborar el EMDR. Por un lado el procesamiento adaptativo de la información se ha visto redundante debido a la extensiva evidencia que existe para el EMDR. En esencia: EMDR funciona, por lo tanto ¿Qué papel juega el procesamiento de la información? Por el otro lado teorizando los resultados en las hipótesis de las investigaciones, la expansión del conocimiento y mas profundizado entendimiento. El presente autor apoya la última posición descrita, pero piensa que SPIA no explica completamente los cambios psicológicos que acontecen en el EMDR. Voy a argumentar que SPIA, tal y como esta, actualmente reduce la investigación dentro del EMDR como es esencialmente una teoría sobre la reducción del cambio psicológico negativo, aunque deja una puerta abierta a la posibilidad de cambio psicológico positivo. Sin embargo el cambio psicológico negativo y su reducción es sin ningún significado el único efecto del EMDR. Esto sugiere que la SPIA es una teoría parcial del cambio psicológico. También significa más, fundamentalmente que existe un “punto ciego” dentro de la investigación que tiene un gran potencial en el EMDR que está siendo consistentemente y considerablemente ignorado. En la actual forma del SPIA, el uso de palabras como “digerido” o “metabolizado”, mientras se extiende el corolario fisiológico central para la SPIA, hace poco para explicar el cómo el porqué y el cuándo del cambio psicológico positivo. The proposal for an extension to Adaptive Information Processing (AIP) (Shapiro 1995, 2001) is derived from the author’s doctoral thesis (Blore 2012a) – a phenomenological study of positive psychological changes experienced by survivors of road traffi c accidents in the post Eye Movement Desensitisation & Reprocessing (EMDR) treatment context. It is argued that AIP does not fully account for the totality of psychological change following EMDR, partly due to lack of knowledge and partly because of the emphasis on explaining the reduction of negative psychological change (rNPC). The main presentation expands on this reasoning, whilst this poster focuses on the proposed theory extension a: ‘Plasticity of Meaning’ (PoM). To illustrate this theory extension, three examples of fi gurative language use (FLU) obtained during interviews with participants are subjected to microtextual analyses (see Smith 2004, p51). It is argued that FLU is a phenomenological (i.e. observable) event that suggests a ‘trading of words’ in turn suggesting neurological networks connecting – a central tenet of AIP. The phrase ‘PoM’ has been coined because of hypothesised similarities to Frey & Morris’ (1997) synaptic plasticity and Cahill & McGaugh’s (1998) reconsolidation of memory theory.

Behavior Modification, 27, 300–312] recommended that empirically supported principles be listed instead of empirically supported treatments because the latter approach enables the creation of putatively new therapies by adding functionally inert components to already listed effective treatments. This article attempts to facilitate inquiry into empirically supported principles by reviewing possible mechanisms responsible for the effectiveness of systematic desensitization and exposure therapy. These interventions were selected because they were among the first empirically supported treatments for which some attempt was made at explanation. Reciprocal inhibition, counterconditioning, habituation, extinction, two-factor model, cognitive changes including expectation, self-efficacy, cognitive restructuring, and informal network-based emotional processing explanations are considered. Logical problems and/or available empirical evidence attenuate or undercut these explanations. A connectionist learning-memory mechanism supported by findings from behavioral and neuroscience research is provided. It demonstrates the utility of preferring empirically supported principles over treatments. Problems and limitations of connectionist explanations are presented. This explanation warrants further consideration and should stimulate discussion concerning empirically supported principles.

SEE FAR CBT is a suggested new protocol for the treatment of anxiety disorders and post-traumatic stress disorder (PTSD) using creative form treatment based on empowerment through fantastic reality. The model emphasizes the role of fantastic reality and the use of imaginal re-narration of the traumatic event with the use of cards as a means of externalization or distancing. The treatment protocol incorporates methods of somatic memory reduction as well as CBT elements. The main objective of this study was to introduce the model and test the therapeutic efficacy of this new integrative therapeutic approach by comparing it to a well-established treatment approach; eye movement desensitization and reprocessing (EMDR). Adult PTSD patients, divided into EMDR (n = 12) and SEE FAR CBT (n = 9) groups, were assessed for traumatic symptoms at three time intervals (pre-treatment, post-treatment and 1-year follow-up). Both EMDR and SEE FAR CBT were associated with effective alleviation of traumatic symptoms, showing statistically significant decreases in their trauma symptoms over time but not differing in treatment efficacy during any of the assessment times. With some methodological limitations, results suggest further inquiry of the proposed model in clinical and experimental settings.

This paper is a further progress report (see Blore, Farrell & Clifford 2008; Blore 2007) on data analysis from N=12 interviews from an Interpretative Phenomenological Analysis (IPA) study into Post Traumatic Growth (PTG) following successful EMDR treatment. The second round of hermeneutic analysis identified eight categories of PTG outcomes. Five of the categories: New Directions in Life; Enhancement of Social Development; Enhancement of Personal Development; Appreciation of Life and Spiritual & Philosophical Development, are consistent with the five factor analysis categories of the Post Traumatic Growth Inventory (PTGI)(Tedeschi & Calhoun 1996). The remaining three categories are: Development of ‘Success Heuristic’; Expression of Positive Emotions; and Growth Aspirations which can be seen as yet-to-be-attained PTG. One implication of these categories is that positive outcomes of EMDR are consistently being under-reported in the scientific literature simply because evidence-based practice is focussed solely on reducing negative symptomatology.

This study explored the subjective experiences of children from 10 to 16 years who had completed an episode of Eye Movement Desensitization and Reprocessing (EMDR) therapy for the treatment of Post Traumatic Stress Disorder (PTSD) following a single traumatic incident. Interpretative phenomenological analysis (IPA) was used to elucidate the participants’ understanding of the effect of the EMDR therapy on their recovery from PTSD. Three super-ordinate themes are identified describing how the participants engaged in the therapy, were assisted to keep going and what they made of the EMDR process. Abstract:
Eye Movement Desensitization and Reprocessing (EMDR; Shapiro, 1989a, 1989b, 1995) is a relatively new procedure used primarily for treating posttraumatic stress disorder (PTSD). This study is the first to systematically investigate the moment-to moment experiences of PTSD victims during their first treatment session. Using variations of Interpersonal Process Recall (Elliot, 1994), and Colaizzi's (1978) phenomenological research methodology, findings confirmed many of Shapiro's (1995) descriptions of experience, with nothing of a disconfirming nature being discovered. Three distinct patterns of co-researcher experience were identified, with one co-researcher reaching full in-session resolution of her baseline measures. Further, three broad categories of experience were discovered (Participant Experiences and Spectator Experiences [Cochran, 1990]; and Treatment Specific Effects); each of which was further found to consist of four dimensions, or components, of experience. Movement from the Participant to Spectator realm was consonant with co-researchers' working through, contextualizing and making meaning of trauma-related memories.

The proposed research seeks to illuminate the impact of post-traumatic anxiety on elite level competitive cycling performance. Although there is much outcome research supporting the effectiveness of EMDR in treating post- traumatic anxiety in general, relatively little is known about the nature of anxiety among athletes who have experienced accident, injury or other trauma in sporting contexts. Interpretive Phenomenological analysis (IPA) will be used to gather data from an opportunity sample of competitive cyclists who have experienced trauma during sporting participation. Analysis of the data will attempt to capture these participants’ experience of anxiety and its impact upon performance. This information will be used to inform therapeutic practice when using EMDR with this client group. It is hypothesised that due to the specific psychomotor actions necessary for competitive cycling, adaptations to the future template within EMDR will be required to maximise therapist efficacy when working with this particular client group.

This study explored experiences of children treated with Eye Movement Desensitisation and Reprocessing (EMDR) for Post Traumatic Stress Disorder (PTSD). The research aimed to understand more about salient aspects of the therapy such as needing therapy, parent support, the relationship with the therapist and the impact of the EMDR. Semi-structured interviews were carried out with nine children across England and Scotland, aged 10 to 16 years who had completed EMDR therapy within the past two years. Interpretative phenomenological analysis (IPA) was used to elucidate the participants’ understanding of the effect of the EMDR therapy on their recovery from PTSD. Three super-ordinate themes are identified which describe how the participants engaged in the therapy, were assisted to keep going and what they made of EMDR. Results suggest that difficulties in engaging in the therapy were overcome by determination to get better, support of family and friends, the credibility of the therapist and starting to feel better after about two sessions. EMDR was perceived as an effective therapy despite initial scepticism, and an approach that generally did not require a conscious effort by the participant to make it work. Findings are related to neurobiological hypotheses concerning underlying neurological mechanisms for the resolution of traumatic memories. Practical implications fro improving clinical practice with children and families will be presented.

Although there is emerging literature on training clinicians in principles of trauma (Black, 2008; Greenwald, Maguin, Smyth, Greenwald, Johnston, & Weiss, 2008), there is a lack of systematized research on the teaching of EMDR, formation of EMDR therapists, and the role of the therapeutic alliance in EMDR treatment. Despite the numerous changes in approved training models since the creation of EMDR, all the EMDR community has gathered is commentary and hypotheses on what works in terms of training therapists. On Tuesday. November llth 2004 in Gevelsberg, Germany a traffic accident happened with a truck and a school bus. A number of children were slightly injured. 17 pupils of a basic school were involved. A part of them were in need of acute ambulatory medical care in a hospital. In addition were some siblings, attending higher schools likewise casualties of the accident. At request of the "Opferschutzbeauftragten" and the school the Psychiatry for children and youngsters, Herdecke, Germany as hospital in responsibility at Monday, November 29th were asked to treat this incident with this 17 pupils of the school. Three co-worker8 of the Kinder- und Jugendpsychiatrie Herdecke, a policeman, the "Opferschutzbeauftragte" [commissioner for victim protection], a person from the bus company, the principal of the school and the 17 pupils were present. The methods were presented, involving EMDR in the collective. Installation of a safe location. Treatment of the actual accident event in the group as well as strategies for stabilisation of the pupils was represented in detail. Especially the persons present were entered into the legal, organisational and practical topics of the problem. Later a re-inquiry took place in the families, the result will be also presented at congress. On the occasion of this event a concept of proceeding in acute trauma was developed together with the "Opfenchutzbeauftrogten" of our region of providing. This will be presented as well, if finished until then, at congress.
