Abstract:
Shapiro's (2001) adaptive information processing (AIP) model portrays an innate healing system hypothesized to be composed of neurophysiological mechanisms of action causally related to the resolution of disturbing life experiences. The author expands the model to include psychosocial mechanisms and suggests that a model of a biopsychosocial system can best depict causal properties related to positive outcomes of eye movement desensitization and reprocessing (EMDR). Teleofunctionalist and evolutionary perspectives are applied: the first, to explain the inclusion of the psychological and social features highlighted in the updated model; the second, to support the hypothesis that AIP is a goal of the human attachment system. It is posited that bonding, following a disturbing life experience, facilitates the access of information related to previous states, thus allowing an update of self/world models. These interactions are analogous to psychotherapeutic encounters, with multiple levels of information processing at subpersonal, personal, and interpersonal levels. Analysis of the causal properties of personal and interpersonal levels supports a broader understanding of AIP's scope in conceptualizing psychopathology and informing treatment applications and research.

Argues that eye movement desensitization and reprocessing (EMDR) and hypnosis are qualitatively different states of neurophysiological functioning. An experiment is reported in which the effects of EMDR on brain functioning of 3 Ss are examined. EEGs taken during EMDR for these Ss were within normal range, and did not differ from waking state EEGs, indicating that EMDR does not appear to produce the altered consciousness state associated with hypnosis or confabulation. Research psychologists often complain that practitioners disregard research evidence whilst practitioners sometimes accuse researchers of failing to produce evidence with sufficient ecological validity. We discuss the tension that thus arises using the specific illustrative examples of two treatment methods for post-traumatic stress disorder: eye movement desensitisation and reprocessing and exposure-based interventions. We discuss the contextual reasons for the success or failure of particular treatment models that are often only tangentially related to the theoretical underpinnings of the models. We discuss what might be learnt from these debates and develop recommendations for future research.

Two experiments are reported that investigate the effects of saccadic bilateral eye movements on the retrieval of item, associative, and contextual information. Experiment 1 compared the effects of bilateral versus vertical versus no eye movements on tests of item recognition, followed by remember-know responses and associative recognition. Supporting previous research, bilateral eye movements enhanced item recognition by increasing the hit rate and decreasing the false alarm rate. Analysis of remember-know responses indicated that eye movement effects were accompanied by increases in remember responses. The test of associative recognition found that bilateral eye movements increased correct responses to intact pairs and decreased false alarms to rearranged pairs. Experiment 2 assessed the effects of eye movements on the recall of intrinsic (color) and extrinsic (spatial location) context. Bilateral eye movements increased correct recall for both types of context. The results are discussed within the framework of dual-process models of memory and the possible neural underpinnings of these effects are considered. [PsycINFO]

This article argues the impact and consequences of psychological trauma on the body and its impact on neurophysiological level. Background
Efficiency of the EMDR procedure is based on a presumption of neuropsychological changes in therapeutic process.The aim of the investigation is to scann and give evidence of electroactivity changes, during the process of EMDR procedure and after finishing it.
Materials and methods
We have recorded a continual polygraph EEG, before, during and after EMDR therapy, in patient with combat-related PTSD.
Results
Before the treatment, EEG recorded basic activity of low voltage (attenuation) of 20 μV, frequency of beta range (17-26 Hz), bioccipital, with no pathologic activity. Patient had prominent vegetative symptoms (anxiety, heart rate 100/min). Background activity immediately after the treatment records the amplitude values of around 50 μV, frequency of around 11-12 Hz. After the end of the treatment background activity possesses the amplitude value of about 37 μV, holding the persistence in frequency.
Conclusions
If the EMDR treatment is successful, sudden increase of amplityde activity is noted imensly. This sharp border line, which signifies normal activity, appears in 2-3 seconds affter the desensitize phase. The investigation suggest that from neurophysiological point of view, cortex (in EMDR procedure), works according to the principle "all or nothing". If there is processing of traumatic memory, the activity gets completly normal. If the therapy is not successful, there are numerous artefacts, because of increased muscle activity. This kind of activity, in our investigation is marked as "Artefact therapy".
The results, indicate maintaining low level of amplitude values of electrocortical activities during the treatment, as well as increase after successful treatment. The increase of amlitude is corelated to decrease of anxiety after the successful treatment.

Reprocessing and Desensitization (EMDR) as an intervention
to assist an athlete in recovering from the distressing effects of
a critical incident and the adverse impact that such an event
can have on the athlete's performance. Two cases will be
presented; that of a major league baseball player and a male
pair skater and national competitor. Each experienced a life threatening
critical incident which triggered symptoms of
posttraumatic stress disorder. The EMDR methodology was
first used as a treatment for trauma, its original utilization.
However, the extension of the EMDR protocol for
performance enhancement was also employed, to help restore
the athlete's desire for engaging in his sport and to provide
mental rehearsal for future participation. The presenter will

Beginning in the 1990s, Francine Shapiro had discovered and began researching a new neurophysiological technique for treating traumatic material. This technique, alternating bilateral stimulation, was quickly subsumed by Shapiro’s psychotherapeutic orientation known as Eye Movement Desensitization and Reprocessing (EMDR). After many years of rigorous empirical studies demonstrating its uses and effectiveness in trauma treatment, EMDR has become one of only three approved treatments recommended by the veteran’s administration for the treatment of PTSD.
Through didactic illustration and explanation; videotaped demonstrations; and experiential exercises, this seminar provides clinicians the basic principles of EMDR and EMDR- related techniques and how each would be incorporated into their personal therapeutic orientation and practice.

Eye Movement Desensitization and Reprocessing (EMDR) is a controversial new treatment for anxiety disorders that is proported to evince rapid and lasting treatment gains. EMDR theorists argue that humans have an innate biological drive to process events and their memories into adaptive and useful forms. Traumatic memories become "stuck" and unprocessed, resulting in a host of psychopathological symptoms. EMDR's curative power rests primarily in the eye movements. Shapiro argues that the eye movements directly access and alter traumatic memories at a neurophysiological level. Memories are then processed down into adaptive forms, with a concomitant abatement of symptoms. Research supports the clinical efficacy of EMDR. However, there is much debate as to the actual mutative element in the EMDR protocol. Support for eye movements playing a curative role is meager and equivocal. This study examines the proposition that the primary mutative component of EMDR is response to suggestion. Specifically, this study proposes that the EMDR treatment protocol is laced with suggestion for improvement. Highly hypnotizable individuals will respond to these suggestions even though EMDR is an explicitly nonhypnotic treatment. That is, subjects will improve to the extent they respond to suggestion. Further, this study tests the viability of Shapiro's eye movement theory. An eye movement condition is compared to an eye fixation condition in terms of treatment outcome. Results indicate a positive correlation between subjects' hypnotizability and treatment outcome. In addition, eye movement conditions were as efficacious as eye fixation conditions where treatment outcome is concerned. I want to begin by thanking the Division for honoring
me with the 2009 Award for Outstanding Contributions
to Practice in Trauma Psychology. I was very touched
to have received the award in the 20th year since the publication
of my first article in the Journal of Traumatic
Stress in 1989. I also appreciate this invitation to
provide an update on some of the recent advances
in eye movement desensitization and reprocessing
(EMDR) practice that are relevant to the Division,
along with a clarification of its current procedures
and theoretical underpinnings. For instance, due in
part to my first publication, which described “EMD”
solely in terms of desensitization with repeated
return to the target memory, many in the field
are unaware that, as EMDR, it is no longer simply
an exposure treatment. In fact, with the accent
on “reprocessing,” EMDR pays only occasional attention to the initial target and, importantly,
includes the facilitation of an association process
that actually contradicts most of the tenets of current exposure
therapies. Therefore, I will also take this opportunity to explain
some of the differences between these treatment orientations,
since this distinction points the way to additional clinical

This paper addresses how creativity has been interwoven into the discovery and development of EMDR, how EMDR is an effective tool in unblocking and enhancing creativity and how our understanding of the brain enhances our knowledge in this area. Dr. Shapiro’s inspiration leading to her discovery and development EMDR are the underpinnings of EMDR and its interweave with creativity. EMDR processing is also an essentially creative process at the core of healing trauma-based blocks. The therapeutic relationship in EMDR can be conceptualized as a co-creative process.
This paper also addresses using EMDR in addressing issues of creativity. Creative blocks are regularly reported by both artists and non-artists and often cripple and traumatize the artist, and interfere with the creativity of daily living of non-artists. The EMDR protocol can be used to target creative blocks as trauma is an integral part of these blocks. The EMDR future template is a tool for enhancing creativity with artists free of significant blocks. This includes actors, singers, dancers, writers and graphic artists.

This paper reviews the present state of knowledge about the efficacy of eye movement desensitisation and reprocessing (EMDR) as a treatment for traumatic memories, and draws on information-processing theory to identify basic problems with much of the research on this procedure. The general failure of this research to take into account the complexity and hypothesised theoretical underpinnings of EMDR is discussed, and suggestions are made for future research. Although EMDR has shown some promise as an effective intervention for PTSD, well controlled comparative outcome studies are required to establish its efficacy before investigation of its active therapeutic components should be undertaken. Eye Movement Desensitisation and Reprocessing (EMDR) is one of the most recent additions to the armoury of treatments for posttraumatic stress disorder (PTSD). This chapter briefly outlines the EMDR procedure, and reviews the growing number of outcome evaluation studies, before considering some of the recent theoretical explanations that have been offered. EMDR has been used with a variety of populations, and its use with children and adolescents is considered here. At the heart of EMDR is the notion that accelerated processing of disturbing material can be directly facilitated at a neurophysiological level using a variety of dual attention tasks. Shapiro nel 1989 dell'azione dei movimenti oculari sullo stress traumatico e sulle situazioni ansiogene. Negli anni successivi l'EMD è diventato EMDR, infatti, la desensibilizzazione è il primo effetto che avviene durante una seduta dove viene applicata la stimolazione bilaterale, ma successivamente l'osservazione massiccia ed approfondita degli effetti clinici di questo metodo terapeutico ha rivelato che il processo che avviene è molto più complesso. È stato notato che simultaneamente alla desensibilizzazione avviene una ristrutturazione cognitiva e che entrambe sono il risultato dell'elaborazione dell'informazione legata ai ricordi traumatici. Nel lavoro realizzato con la paziente Shirley, Wolpe descrive tutta la storia di terapie a cui era stata sottoposta fino a quel momento che però non erano riuscite ad affrontare e risolvere l'esperienza traumatica della violenza sessuale. L'EMDR è stato efficace perché ha lavorato sul piano neurofisiologico, dove l'informazione è immagazzinata in modo disfunzionale ed è accessibile soltanto a questo livello, infatti, le terapie prettamente verbali non sempre riescono ad accedere e ad agire su questo piano e di conseguenza non riescono a modificare gli aspetti clinici legati alle esperienze di stress traumatico. Il programma terapeutico di Wolpe per l'introduzione dell'EMDR si è focalizzato inizialmente sull'esperienza traumatica della violenza (2 sedute) e dopo aver elaborato l'esperienza in sé sono stati individuati altri bisogni terapeutici legati all'ansia sociale e agli spunti agorafobici. Le 5 sedute successive sono state utilizzate per la desensibilizzazione di una serie di situazioni a cui era stato attribuito dalla paziente un valore della scala SUD. Si è iniziato dalla situazione più ansiogena (85) nella scala SUD e anziché utilizzare il rilassamento proprio della desensibilizzazione sistematica, sono stati realizzati movimenti oculari. La paziente riportava una riduzione dell'ansia dopo ogni set di stimolazione bilaterale e l'immagine della situazione temuta tendeva a sparire e in alcune situazioni addirittura diventava positiva. La ristrutturazione cognitiva è avvenuta nella paziente in modo spontaneo man mano che la terapia procedeva, fino a riportare che poteva camminare per strada sentendosi una cittadina normale, avendo un progetto per il futuro, inoltre aveva smesso quasi completamente di bere e si sentiva che un peso si era sollevato dalle sue spalle. Riportava di essere più rilassata sul lavoro, si godeva le passeggiate solitarie sulla spiaggia, aveva cominciato a curare il suo aspetto fisico e aveva iniziato ad avere interesse per gli uomini e ad avvicinarsi a qualcuno, ha riportato che non temeva più di essere violentata e non aveva più i pensieri suicidi e gli autori hanno ritenuto la terapia conclusa. I risultati si sono mantenuti ai vari follow up realizzati nel tempo. Quindi possiamo vedere i risultati della terapia con EMDR a livello cognitivo, nelle attribuzioni che fa del mondo e di se stessa, differenziando i pericoli veri da quelli meramente ansiogeni, soggettivo per quello che la paziente riporta emotivamente e a livello comportamentale, dove ha messo in atto dei nuovi comportamenti adattivi in modo spontaneo. Nella parte conclusiva gli autori menzionano una serie di 100 casi che sarebbero stati pubblicati nel Journal of Behaviour Therapy and Experimental Psychiatry e si augurano che altre pubblicazioni potessero rispondere al bisogno di delucidare i meccanismi che permettono questi cambiamenti dopo una seduta di EMDR. Come si evince dagli articoli che vengono pubblicati in questo numero la ricerca, il lavoro empirico e le osservazioni cliniche hanno dato un grande contributo in questo senso negli ultimi anni. The article by J. Wolpe, J. Abrams has a case treated in 1991 with Eye Movement Desensitization (EMD), were the first clinical applications since the discovery of F. Shapiro in 1989 of the eye movements on traumatic stress and anxiety-provoking situations. In subsequent years the EMD became EMDR, in fact, the first effect is the desensitization that occurs during a session where the stimulation is applied bilaterally, but then observing massive and thorough clinical effects of this therapy revealed that the process that takes place is much more complex. It was noted that desensitization occurs simultaneously with a cognitive restructuring, and that both are the result of information related to traumatic memories. In work done with the patient Shirley, Wolpe describes the history of therapies that had been subjected up to that point but had failed to address and resolve the traumatic experience of sexual violence. EMDR was effective because he worked on the neurophysiological level, where information is stored in a dysfunctional and is accessible only at this level, in fact, purely verbal therapies are not always able to access and act on this plan and therefore can not modify the clinical aspects related to the experience of traumatic stress. The treatment plan for the introduction of Wolpe EMDR focused initially traumatic experience of violence (2 sessions) and after processing the experience itself more therapeutic needs were identified related to social anxiety and agoraphobic cues . 5 The subsequent sessions were used for the desensitization of a variety of situations to which the patient was given a value of SUD scale. It was started by the most anxiety-inducing situation (85) in the scale instead of using SUD and their relaxation in systematic desensitization, eye movements were made. The patient reported a reduction in anxiety after each set of bilateral stimulation and the image of the feared situation, and tended to disappear in some situations even became positive. The cognitive restructuring has taken place in the patient spontaneously as the therapy progressed, he could bring up to walk down the street feeling like a normal citizen, having a vision for the future, also had almost completely stopped drinking and felt that a weight had lifted from his shoulders. Reported to be more relaxed at work, enjoyed solitary walks on the beach, he began to treat her physical appearance and had started to have interest to men and to approach someone, reported that no longer feared being raped and not had more suicidal thoughts and the authors have considered the therapy ended. The results were maintained at various follow-up made in time. So we can see the results of EMDR therapy at the cognitive level, which is within the functions of the world and of itself, differentiating the real dangers than mere anxiety, subjective to what the patient experiences emotional and behavioral level, where he implemented of new adaptive behaviors in a spontaneous way. In the concluding section the authors refer to a series of 100 cases were published in the Journal of Behavior Therapy and Experimental Psychiatry and hope that other publications might respond to the need to elucidate the mechanisms that allow these changes after a session of EMDR. It is clear from articles published in this issue the research, the empirical work and clinical observations have given a great contribution in this direction in recent years. EMDR has been published over 150 articles which demonstrate its effectiveness. Among the most accredited scientific journals which are published on EMDR research work are: the Journal of Consulting and Clinical Psychology, British Journal of Clinical Psychology, Journal of Anxiety Disorders, Journal of Behavior and Experimental Psychiatry, Behavioural and Cognitive Psychotherapy, Behaviour Therapy , Journal of Traumatic Stress, The Clinical Psychologist, Journal of Psychotherapy Integration, Harvard Mental Health Letter, American Journal of Clinical Psychology, Psychotherapy, British Journal of Psychiatry, International Journal of Psychiatry. It is hypothesized that eye movements used in EMDR practice influence both memory and emotional state, some laboratory studies provide evidence for such influence (Christman et al, 2003, Neurobiology; Barrowcliff et al., J. Forensic Psychiatry and Psychology, 2004). From a neurophysiological point of view, it is also interesting to examine to what extent some eye movement parameters may be modulated by memory and emotions. For instance, it is well established that memory driven saccades in simplified laboratory tests, present different characteristics (longer preparation time, lower accuracy, lower velocity) than visually guided movement. During EMDR the eye movements (mostly pursuit) are elicited externally by the therapist, yet at the same time the patient is in touch mentally with the traumatic image and related negative emotions. The physiology of such movements might be different and this could be explored with specific studies. Another important issue is to what extent movements of the eyes are helpful in coding and retrieving visual images. Accordingly to scanpath theory, eye movements are cognitively driven by sensory and motor representation; repetitive eye movements to the same area of an image are attributed to checking if the image corresponds to what the observer imagines (Noton & Stack, 1971; Brandt & Stark, 1997). Future laboratory research could bring together such theories and EMDR psychotherapy, e.g., by examining spontaneous eye movements related to negative image.
In this conference, we will present preliminary data from a laboratory study on eye movement patterns during the Roschach test. Eight healthy students conducted the experiment. Seated in front of a computer screen, subjects were equipped with a binocular video eye tracker (Chronos) sampling eye position images from each eye every 4 msec. Instantaneous eye position data were stored on a computer and analyzed afterwards with laboratory software. Each are of the Roschach test was presented for 30 sec., after which, the experimenter (a psychologist) recorded the response given by the subject.
Analysis of eye movement patterns were examined in relation to subjective verbal reports.
Results: The first question asked was to what extent the eye movement pattern reflected the imaged projected by the subject or by the visual properties of the Rorschach image itself. To gain some insight, we concentrated on the cards for which our subjected gave no standard responses, thus very different from one subject to the other (e.g., card IX). Eye movement exploration (the surface of the image explored, its patiaol composition e.g. the resprctive proportion of horizontal versus vertical movements) was correlated with the verbal report and the specific mental imagery (larger surface, multiple focusing points when several personages or objects were imaged). This provides evident for top-down influence; the eyes are exploring the projective interpretative image; repetitive movements back and forth to same points which help in turn consolidate this mental imagery.
Psychological analysis of eye movements (saccades and fixation) was also made. Saccades are found to be as fast as when looking reflexively to single visual targets; during fixations, however, between saccades, the eyes were more instable, particularly the visual uses were crossing at different depths from one fixation to another. Further ongoing analysis will correlate fixation duration and depth instability to spatial and emotional content of the subjective report.

Whether you are first learning about Eye Movement Desensitization and Reprocessing (EMDR) or you have participated in EMDR training, the goal of this article is to provide the reader with a brief overview of strategies for using the full protocol with young children. To understand the process by which the phases of the protocol are applied with child clients, it is important to understand the theoretical underpinnings that Adaptive Information Processing (AIP) theory creates as a foundation for healing and health with children. After discussing the application of AIP to children, the article will continue with an overview of skills therapists can use to create a toolbox that can be integrated into the EMDR eight phase treatment protocol with references for additional study and training on using EMDR with children. Finally, therapists will begin to learn how to adapt the full EMDR protocol for effective psychotherapy through developmentally suited language and interventions with even the youngest of clients.

Eye movement desensitization and reprocessing (EMDR) therapy is a novel therapy that has been effective in treating post-traumatic stress disorder (PTSD). Few studies have explored the neurological underpinnings of EMDR effectiveness. Utilizing a symptom provocation study design, this study assessed non-PTSD car accident patients. These pilot participants were scanned for comparison to PTSD patients and to explore the task design effectiveness for the future study of PTSD patients. One pilot participant exhibited activation in the left precuneus, and left medial temporal gyrus, and also in the left medial frontal gyrus. In PTSD patients the medial prefrontal cortex is often hypoactive, and inversely correlated with a hyperactive amygdala. The robust activation of medial frontal gyrus in the pilot subject with a corresponding inactivation of the amygdala indicates the participant's normal processing of the car accident trauma tic memories, and that task design and study parameters are being effectively implemented.

Eye Movement Desensitisation and Reprocessing (EMDR) is a novel approach to treating
Post Traumatic Stress Disorder (PTSD). It relies upon having clients access images of their
traumas, negative self-schemas, emotions, and somatic memories and reprocessing these to
resolution of the traumatic memory. The simultaneous linking of these components is
accompanied by alternating stimulations of the brains hemispheres using either auditory
tones, tactile sensation, or rapid eye movements across the visual field. Successful
completion of the treatment results in trauma images fading, positive cognitive shift,
reduction of negative affect, and disappearance of somatic sensations.
Shapiro (1995) proposed an 'accelerated processing model' for EMDR that essentially pulls
together the different strands of the treatment in a coherent way. It suggests that the brain
heals itself, as with tissue damage, and changes in symptomatology are always from negative
to positive. What Shapiro's model does not do is operationally define her concepts and
explain the way changes in dysfunctional information occur. For example, the EMDR model,
as with Beck's (1987) Clinical Cognitive Model, accepts that clients place new meaning on
dysfunctionally stored information, but lacks explanation of how this occurs: i.e. the shift
from irrational to rational beliefs, and from 'cold' to 'hot' cognitions.
This paper rectifies the difficulties the 'accelerated processing model' has in acting as a
theoretical basis for EMDR. It describes firstly the received wisdom on the
neurophysiological, and psychological correlates of PTSD. It then goes on to examine the
treatment components considered necessary for the effective resolution of the disorder. In its final phase, the paper considers how well models of information processing explain the
acquisition and maintenance of PTSD. It adopts a modification of the Ingerchanging
Cognitive Subsystems (ICS) approach (Teasdale & Barnard, 1993), a theory based in
cognitive science, to operationally define EMDR's component parts and its process in the
treatment of PTSD. The ICS approach is recommended as a useful way to conceptualise the
maintenance of PTSD and a strong theoretical basis for EMDR.

In the recent years the number of neuroimaging studies
evaluating neural correlates of psychotherapy has steadily
increased revealing its clear neurobiological effects on brain
function across a wide range of psychiatric disorders. Functional
studies by single photon emission computed tomography
(SPECT) and positron emission tomography (PET) detect
changes in cerebral blood flow and metabolism patterns, identifying
the brain areas processing the various components of
emotional processing and/or affected by the disorders. investigations
by magnetic resonance imaging (MRI) have also revealed
psychiatry disease-related structural changes.
The first part of the workshop (20 minutes) will describe the
neuroimaging methodologies implemented in EMDR research
and their possible clinical implementations will be discussed.
In the second part (10 minutes) neuroimaging studies on the
neurobiological effect of EMDR will be reviewed (1-5).
The third part of the workshop (30 minutes) will deal with the
last findings in EMDR research and will focus on a recent studies
published by our group on the Journal of Psychiatry Research
about the predictive value of MRI on the outcome of
EMDR therapy (6).Moreover a collaborator of our group will
describe and present the preliminary findings of an ongoing experiment
aiming to identify the neurophysiological mechanisms
active during EMDR therapy.
The description and the discussion about the contents of the
workshop will provide the audience
1 the necessary information to understand the methodological
principles behind the neuroimaging techniques (PET and
SPECT) and their possible applications in research and clinic;
2, the critical knowledge of the limited number of published
papers in the field of EMDR-related functional and anatomical
studies (1-6);
3. the basic research principles and examples to be motivated
to begin, take part and/or collaborate to EMDR research in order
to shed light on the neural basis of this fascinating psychotherapeutic
technique.
The presented material will represent the state-of-the-art of the
current neuroscience EMDR-related research and of the neuroimaging
methodologies available at the moment.

We tried to “dissect” the EMDR method in the component desensitization through rapid eye movement (EMD), that is also the original part, to explain the neurophysiological functioning, Charney et al, say that a lot of persistent symptoms of PTSD are caused by an elevate arousal of NVS, particularly due to the increase of norepinephrine. Others hypothesize, again, that EMDR method is rapid because it is based on Classic Conditioning. Pavlov found that every stimulus of environment causes to the organism (animal or human) an immediate orienting response due to the arousal of the Autonomic Nervous System. In this situation the animal begins to look around moving its eyes laterality (as it happens during ocular movements of EMD); if it perceives a danger in the environment it stimulates a visceral negative response (hyperactivation that means fear) that leads an an avoidance behaviour (stiffening) and/or fighting. In other cases it causes a visceral positive response (de-arousal process that means reassurance) that leads to an approach behaviour through interaction or exploration.
The EMD should produce an orienting reflex in the patient in a protective sitaution like a psychotherapeutic setting; that means an important reduction of neuro-vegetative nervous system activity (dearousal) and some visceral positive response. We propose an experimental design with a single subject (ABACADA) by interruption treatment and by monitoring Neurophysiological functions through biofeedback instruments to test this neurophyiological hypothesis.

Originally a technique that seemed to desensitize disturbing memories, EMDR is now a full-scale protocol that is being used to treat a wide-range of disorders. Even its proponents acknowledge, however, that the mechanism of action in EMDR is still unknown. It is argued that there has been an over-emphasis on neurophysiological explanations of EMDR. After a review of controlled studies and a discussion of proposed mechanisms, two case studies of EMDR therapy (one child and one adult) with pathological grief are presented. The cases are analyzed for their adaptive changes as this term is applied in Horowitz's control process theory (1992). It is argued that Horowitz's theory represents a theoretical foundation by which a psychological understanding of the EMDR psychotherapy process can be achieved. [Background] Eye movement desensitization and reprocessing (EMDR) is a new innovative treatment with a high success rate for psychological disturbances rooted in traumatic memory. However, the neurophysiological mechanisms of EMDR have not yet been elucidated. Why is saccadic eye movement effective for the reprocessing of previously established conditioned reflex? [Aims] The present research analysed (i)the topographical changes of EEG (and ERP) and REM sleep after EMDR treatment, and (ii)the subjective units of emotional distress (SUDs) and VOC. [Methods] Subjects (13 males and 20 females) were assigned to three groups according to varied conditions (control, provocational, and EMDR), and engaged in sets of horizontal saccadic-eye movements lasting approximately 30 seconds per set. Topographical changes of EEG and ERP activities were recorded from 14 placements over frontal, central, parietal, temporal and occipital scalp locations in the international 10-20 system with linked mastoides (A1-A2). Electrodes were also placed on the lateral canthus and above the supercillium of the left eye in order to measure the electrooculographic and electromyographic responses. [Results and Conclusions] The statistical significance of topographical EEG differences and REM densities during the pre-0post EMDR treatment situations, were evaluated using an ANONA and Mann-Whitney U test. The neurophysiological and psychological data indicate that the density of eye movement during REM sleep increased after provocation and EMDR, and the left frontal activities might indicate a treatment efficacy. It supports the hypothesis is that REM sleep is intimately involved with the mechanisms of emotional and memory reprocessing.

The research on Eye Movement Desensitization and Reprocessing (EMDR) has had a significant development in the last 10 years. The EMDR consists on a dual focalization activity (the use o eye movements or other forms of left/right rhythmical stimulation, while focusing on personal disturbing material). Many hypotheses have been made on how EMDR works and why the clinical results are so significant. One of the most possible reasons may regard the fact that there seems to be an innate information processing system that is physiologically configured to facilitate mental health in much the same way the rest of the body is designed to heal itself when injured (Shapiro, 1995). When operating appropriately, this system takes the perceptual and emotional information from a traumatic event to an adaptive resolution - useful information is stored with appropriate affect and is available for future use. The physiological and emotional arousal stemming from a traumatic event may disrupt the information processing mechanism. The blocked processing prevents the traumatic information from progressing through the normal steps of adaptive integration. The physiological stimulation appears to activate the innate information processing systems and may be linked to the mechanisms inherent in memory storage. EMDR apparently intervenes in brain functions, especially in the limbic system and amygdale, which have been already identified as actively involved in traumatic experiences.

EMDR has developed on a theoretical background of information processing. What do we know about the
structure and functioning of the processing parts of the brain, or “the processor”? In this talk I will bring together
information from the research studies of Harvard neurophysiologist Robert Stickgold, the MRI studies of Ruth
Lanius, and my own co-morbidity studies of PTSD, which show a very high co-morbid sleep disorder association.
EMDR practitioners in UK and Europe may be unfamiliar with sleep disorder medicine, and this presentation aims
to introduce them to this realm of interest. I will explain why sleep disorders render people vulnerable to
developing PTSD; and why EMDR will help resolve PTSD; and why sometimes the sleep disorder problem needs
to be recognized and addressed in its own right. Instruction on how to screen for significant sleep disorders and
how to recognize them and distinguish them from sleep disturbance will be given. I will discuss recent and
ongoing studies of limb movement disorder of sleep, which often seems to contribute to vulnerability to
adjustment disorder and PTS symptoms. The ecological neatness of EMDR will be apparent, and difficulties in
achieving results in chronic PTSD will be understandable and seen as remediable. The direction of possible future
research efforts in this area will be discussed.

This article evaluates and illustrates the application of the recent-traumatic episode protocol (R-TEP) with three diverse clients: a child with chronic illness, a woman with a significant loss, and an adolescent who self-harmed. The R-TEP is an adaptation of the Eye Movement Desensitization and Reprocessing (EMDR) protocol for early EMDR intervention. Sessions are presented in detail to highlight the shifts in information processing that occur during treatment. Observed markers used to analyze the flow of processing are identified, which include distancing from the trauma; reduction in negative affect or change in reported emotions; accessing more adaptive information; changes in the Subjective Units of Disturbance scale; and the Validity of Cognition scale and Impact of Event Scale--Revised indicating shifts in perception of the traumatic memory. Pre-post R-TEP treatment gains were noted for all clients, with changes in behavior and functioning. Theoretical underpinnings of the R-TEP are discussed with respect to the reported observations. In the past years, Eye Movement Desensitization and Reprocessing (EMDR) has become increasingly popular as a treatment method for Posttraumatic Stress Disorder (PTSD). The current article critically evaluates three recurring assumptions in EMDR literature: (a) the notion that traumatic memories are fixed and stable and that flashbacks are accurate reproductions of the traumatic incident; (b) the idea that eye movements, or other lateralized rhythmic behaviors have an inhibitory effect on emotional memories; and (c) the assumption that EMDR is not only effective in treating PTSD, but can also be successfully applied to other psychopathological conditions. There is little support for any of these three assumptions. Meanwhile, the expansion of the theoretical underpinnings of EMDR in the absence of a sound empirical basis casts doubts on the massive proliferation of this treatment method. (ScienceDirect)

Traditional therapy models often reflect Western values and norms and may be inappropriate for use with many non-Western
cultures. Worldwide, therapists are beginning to examine how they can make these models culturally sensitive to minorities,
immigrants, and non-Western clients; however, it is time to go beyond “being aware,” understanding and appreciating
differences, and adapting individual interventions to actively responding to the needs of their clients by using models that
have theoretical underpinnings, underlying assumptions, and basic tenets that are consistent with those of non-Western
clients (Carlson, 1999) and promote social justice for those clients, their families, and the surrounding communities. This
paper suggests the use of responsive evaluation to explore Eye Movement Desensitization and Reprocessing (EMDR) as
a culturally responsive therapy. Responsive evaluation can be used to consider each unique client and therapist context,
culture, power, needs, and beliefs, and family and individual needs in determining a culturally responsive model of therapy.
In this presentation, we provide therapists and scholars a model for offering culturally responsive EMDR therapy and
research using responsive evaluation as a framework. An in-depth description of responsive evaluation will be provided, and
explanations and examples of how it is consistent with the goals and theories underlying culturally responsive therapy will
be given. Finally, concrete suggestions of how EMDR clinicians and scholars can apply this methodology will be provided. As
the EMDR continues to be taught and used across Asia, it is crucial that we continue to observe and respond to the needs of
clients throughout Asia.

The whiplash syndrome is a complex, poorly understood and controversial cluster of symptoms including spinal pain, cognitive dysfunction, neurologic symptoms and emotional complaints consistent with posttraumatic stress disorder. Perhaps its most perplexing feature is the fact that symptoms frequently are far out of proportion to the severity of the accident itself. The frequency of emotional symptoms has led many physicians to attribute symptoms of whiplash to somatization. The typical syndrome of whiplash includes chronic headaches, spinal and jaw pain, usually classified as myofascial pain. Neurologic symptoms include cognitive dysfunction, positional vertigo, balance disturbance, blurring of vision, photophobia and phonophobia, all of which are attributed to minor traumatic brain injury. Emotional complaints include driving phobias, irritability, hypervigilence, exaggerated startle, flashbacks, depression, nightmares and sleep disturbance. DSM IV compatible or subsyndromal forms of PTSD occur in up to 60% of patients.
I began to question the traumatic basis for whiplash when I discovered that most of my patients with delayed recovery had remarkable past histories of trauma, especially child abuse. I discovered that early and rigorous use of somatically based trauma therapies, especially EMDR and Somatic Experiencing resulted in clearing not only of emotional symptoms, but also neurologic and pain-related complaints in many cases. I have concluded that the neurophysiological basis for traumatization includes not only kindled arousal, explicit and procedural memory circuits, but also automatic patterns of neuromuscular bracing, stored in procedural memory analogous to motor skill memory. Bracing patterns of involved muscles represent protective motor reflexes from the moment of injury. Linked to memory and arousal, this kindled circuit leads to perpetuation of regional myofascial pain. Dissociation plays a major role in perpetuation of this phenomenon, and accounts for many of the unusual neurologic symptoms of whiplash.
This model conforms to current theories of PTSD as a model of kindling, but includes the somatic element that I believe is a universal part of the syndrome of traumatization. The pervasive neurohormonal effects of trauma account for the remarkable amount of somatic complaints in this syndrome, and may be the basis for many poorly understood chronic idiopathic disease processes. Incorporation of the neuromuscular system in the process of traumatization pleads for the study of somatically-based therapies for PTSD.
