Abstract:
EMDR as a psychotherapeutic approach has been shown to be effective. However, there is a paucity of studies examining the efficacy of EMDR for clients diagnosed with mild mental retardation. The DSM-IV defines mild mental retardation as below average intelligence (IQ between 70 and 55) along with some deficiencies in adaptive functioning skills demonstrated before age 18. The purpose of our clinical project was to explore the applicability of EMDR for our adult clients (n = 12; mean age 22.5) diagnosed with mild mental retardation by adapting the standard protocol. Studies showed that individuals with mental retardation learn and retain information more effectively when materials are presented to them in a visual, concrete, and interactive manner while utilizing the principles of positive reinforcements.
The prevalence of mental health problems and the wide range of clinical symptoms among these individuals have been reported in several studies. Additionally, these adults are misunderstood as being overly limited in their ability to reap any therapeutic benefit from counseling interventions. Prout and Strohmer (1998), for instance, argued that adults with mental retardation do benefit from counseling interventions. However, they stressed the need for more sophisticated or modified use of psychotherapeutic interventions. Psychotherapeutic techniques and models should be modified, if feasible, in regards to language and cognitive levels commensurate with the clients’ background.
Based on our clinical observations, the following are some examples of proposed guidelines to assist the EMDR clinicians in thinking more creatively when adapting the standard protocol.
1) Considerable amount of preparation at the onset of EMDR is necessary, as it plays a pivotal role in ensuring a successful outcome.
2) Visual depiction of the SUDs and VOCs on a scale of 0 through 5, or 0 through 10, depending on the client’s cognitive abilities is beneficial. Use of “faces” to depict concretely various levels of distress should be made.
3) The concepts of PC and NC may be too abstract for some in this population. We assist clients by operationally defining those concepts with the list of commonly used PCs and NCs in simpler language.
4) Coping resources are sometimes limited for these adults. Clients will benefit from having multiple reinforcements of self-soothing skills thorough the installation of the “safe place” and “resources”.
5) Positive reinforcements (e.g., frequent verbal reminders) should be used regularly in sessions throughout treatment. However, be mindful of clients’ desire to please the clinician.
6) Role-playing should be used when feasible throughout treatment, e.g., during the installation of future templates, as it heightens more sensory, affective, and behavioral modes of learning rather than verbal modality alone.
Based on the treatment outcomes reported by our clients, EMDR is an effective treatment option, as evidenced by sustained reduction in their level of distress to traumatic memories. Findings from this clinical project have practice and research implications. First, the standard protocol should be adapted for use with adults with mild mental retardation to achieve optimal gain. Second, empirical research is needed to provide further evidence for the efficacy of EMDR for adults with mild mental retardation.

Abstract:
Eye movement desensitization and reprocessing (EMDR) is a relatively new form of psychotherapy to emerge in the West. Using both a case analysis and literature review we situate EMDR within the use of altered states of consciousness (ASCs) in psychological healing practices across times and cultures. We discuss EMDR's unique predicament as a therapy that draws upon techniques common to most therapeutic ASCs, while at the same time distancing itself from this tradition through its pseudoscientific language and technologic aesthetic. Our conclusion attempts to shed light on this paradox and raise questions for further study.

The MRI approach to brief therapy originated out of the serendipitous coming together of several incredibly creative minds that resulted in a form of psychotherapy in which the major goal was to make psychotherapy more efficient and more effective. It evolved out of research project on communication begun by anthropologist Gregory Bateson that soon involved the work of hypnotherapist Milton H. Erickson and psychiatrist Don Jackson. John Weakland, Jay Haley, Paul Watzlawick and Richard Fisch began to publish the ideas that resulted fiom the early research findings and in doing so developed a particular set of assumptions about the formation and resolution of human problems that differed significantly from traditional treatment models of the time. Further refinements through the clinical application of these methods resulted in a model of treatment that was a pioneer of the brief psychotherapy movement. It is based on a non-normative and non-pathological way of viewing people with problems; it looks at people in the context of their living situations; it resists the idea of client resistance, it places great emphasis on the use of language; and it seeks to amplify client assets and resources and minimize client liabilities and shortcomings. Brief therapists assume a willingness to be an active change agent for the benefit of their clients. They accept responsibility for creating an atmosphere of respect, patience, and creativity in which clients can find alternative ways to think and behave. They believe they have a set of tasks to perform that will hopefully result in the resolution or, as a minimum, the diminishment of the problem situation for which the client originally sought help. These tasks consist of a combination of ways of thinking and acting that are designed to increase the likelihood that the client will experience relief from a painful problem. One of the main tasks for a brief therapist is to find ways to construe the problems presented by the client so that a solution can be found. Brief therapists inquire into the interactional systemic aspects of a problem, the context or environment in which the problem occurs, the people involved in the problematic situation, and the ways the client has attempted to resolve the problem thus far. Another very important task is to identify and gain access to the persons who are the most interested in and willing to work toward changing the problem situation. The idea here is to spend the bulk of the therapeutic time and effort working with the person who is most invested in the change process. Brief therapists find ways to appeal to this person's values and belief systems so that (s)he will engage in activities and/or alter her/his behavior in ways that are likely to change the problem situation. A third task on which brief therapists concentrate is the establishment of clear, concrete, and doable goals of treatment. They collaborate with the client to determine what the client hopes to gain from treatment and when the client will know she is ready to handle life on his/her own, this assumes an emphasis on the client's present and the possibilities for the client's future rather than his/her past. The fourth task brief therapists focus on is the development of ways of intervening in the way the presenting problem is being handled in the present time. This is based on the central assumption that one of the main goals of psychotherapy is to induce clients to change the way a problem is handled. Such intervening is the result of thoughtful and careful consideration of many factors surrounding the problem situation and involves the use of a variety of skills. A final task for the brief therapist is to find ways to remove him/herself from the client's life in such a way that the client has faith in her/his own ability to function effectively without the therapist. This treatment model offers clinicians an opportunity to work in positive, goal-directed ways that clients find helpful and therapists find challenging and satisfying. It calls upon clinicians to develop keen observation skills, the ability to see things fiom a variety of perspectives, and an appreciation for the vast resources clients bring with them to therapy. While it is a simple model of treatment, it is by no means an easy one to master. It requires clinicians to step outside their usual frames of reference in the pursuit of creative solutions to difficult human problems. It rewards them with a greater sense of accomplishment and increased client satisfaction. In the ever-changing world of mental health, this is no small achievement.

This paper discusses the use of EMDR with refugee and asylum seeking children and adolescents using clinical case examples and shares the clinician’s own personal reflections on the lessons learned. The challenges of working with this group are explored drawing on research findings as well as the clinician’s experiences. Language and cultural differences can act as barriers to assessing children’s mental health or their suitability for EMDR even though in some cases it might be thought of as the treatment of choice. Working with Interpreters is often a key feature of this therapeutic work. The challenges are discussed with direct reference to The British Psychology Society’s recently published guidelines. There are ongoing stresses for these children and young people associated with displacement and their current situation in the UK. These stresses contribute to their psychological distress. Fear of being sent home or mistrust may also prevent them from providing full and accurate information hindering the clinician from obtaining an accurate assessment or trauma history. The importance of good therapeutic skills as the basis of an EMDR Approach is highlighted drawing upon a host of interventions (behavioural, physical, cognitive and motivational) to support clients in building up their own resources so that they can manage their distress between sessions. The therapist’s role in managing expressed emotions within the sessions is examined, not forgetting the impact of this on the therapist.

Individuals with intellectual disabilities (ID) are to some extent discriminated against when it comes to the psychotherapies. The long held belief that individuals with ID do not benefit from such interventions has been attributed in part to communication difficulties, lack of insight and lack of language sophistication. Yet, it is well known that these individuals are about four-fold at risk of developing mental health problems. Cognitive behavior therapies (CBT) and other related therapies are becoming more recognized in the field of intellectual disabilities. Loss and bereavement is common among people with ID but their exposure to interventions such as Eye Movement Desensitization and Reprocessing (EMDR) has been limited. EMDR is a psychological intervention that encompasses many aspects of traditional therapies such as CBT and psychodynamic therapies. There is little research in this area for people with ID. This paper reports two cases in individuals with intellectual disabilities who were struggling with bereavement. They appear to have benefited from EMDR sessions and there may be scope for research to establish its efficacy in this field.

The information processing model has been described in many different ways and is potentially the underlying process that describes the psychotherapy change process, regardless of orientation. In this poster I will briefly compare Eye Movement Desensitisation and Reprocessing and Meta-Cognitive Therapy in light of this information processing model and conclude that although they may use different language to describe their theories, a unifying information processing model describes both paradigms. Human beings are meaning making machines (Robbins, 2009). We give meaning to the world, the deeds we do and the interactions we have, as well as the more fundamental senses we experience (Frankl, 2004). Information arises either from external stimulus or internal process’s and this information goes through our filters where we delete, distort and generalise this information based on the rules created in our information processing unit (Fig 1). This information processing mechanism gives meaning to this filtered information and generates a response, this response interacts with this information and the cycle then repeats in a self regulatory manner. This process is repeated for everything we do; we are and continues until we die. Mental health issues arise when this self regulatory system fails to make a useful functional meaning of the information. Eye Movement Desensitisation and Reprocessing (EMDR) was developed by Francine Shapiro (1989) for the treatment of trauma memories and associated emotions, changing the meaning the person gives to their experiences. Metacognitive Therapy (MCT) was developed by Wells and Matthews (1984) as a way of defining the processes that underpin a range of psychological disorders. One of the features of psychological disorders such as anxiety or depression is that thinking becomes difficult to control and biased in particular ways that lead to a worsening and maintenance of emotional suffering. Many clients report that they feel that they have ‘lost control’ over their thoughts and behaviours. Another important feature is that the persons thinking and attention becomes fixed in patterns of brooding and dwelling on the self and threatening information. Both EMDR and MCT describe an information management/processing system that organises and carries out various tasks such as how to open a door, how to get dressed, how to be anxious, how to respond to a trauma memory Both therapies appear to change this system / process to something that is much more adaptive for the client, as if they change the master information management control programme that runs this system, in the same way a conductor influences the orchestra and the music they produce. Both models describe the same process, although they utilise different language to describe this change process. Perhaps a change of focus for the development of pychotherapy towards more process change models rather than content change interventions.

This presentation will present and demonstrate my clinical observations, that clients while talking or working on their issues will sometime lean their heads toward the right or the left shoulder. People with their heads to the left will tend to report thoughts, while those with their heads to the right will tend to report images and emotions. In therapy, utilizing EMDR when clients have leaned their heads to one side, I have tended to reinforce the side they put their head to, utilizing eye movements in a diagonal direction, corresponding to the direction the head was learning. That is. If the person learned their head to the left, I would move my hand from their upper left side to their lower right side, and opposite if their heads leaned to the right. The outcome of these diagonal eye movements was that it tended to evoke cognitions when the head leaned to the left or images when the head learned to the right. Client who come to therapy requesting help concerning fears, tend to lean their heads to the rights, corresponding with imagery and imagination of the right hemisphere and may lack the cognitive, logical thinking skills (left hemisphere) to counter their fears. With these clients, when the SUDS do not drop significantly, I will tend to do body-cognitive interweave. I request that the client lean their head on the left and work over and over with cirrsponding sets of diangonal eye movmenets. The left hemisphere of the brain is thus stimulated and logical thinking (PC) is enhanced. This helps to counter and balance out the negative images, and the fear decreased.
Through care histories and examples, I would like to accomplish the following objectives: 1) to make the participants more aware of body language and it its significance in therapy; 2) to examine the differences between the right and left hemispheres of the brain and their relationship to therapy with EMDR; and 3) to introduce and demonstrate a body-cognitive interweave in EMDR therapy with client s who request help with fears.

In our therapeutic EMDR work with deaf and hard of hearing clients, we are forced to strongly focus on the visual world. But focus on visual aspects is not only necessary with this client group! When working with pre-­‐verbal trauma, with children or adults who are (not yet) fluent in their native speech or when we use EMDR with clients that use a second language, we will have to use alternatives to spoken languages. When used correctly, visual input can enrich and enable the EMDR process greatly. In this presentation we invite visitors to explore the visual aspects and possibilities of the EMDR process with us. Visitors will feel more adequate in using mimic, visual and imaging techniques with their clients. Video material will enrich our presentation. We will present in English and Dutch sign language. Our interpreter will translate the Dutch sign language in spoken English.

Los infantes (0-­‐4 años) pueden ser expuestos a diversos tipos de eventos traumáticos, por ejemplo procedimientos médicos intrusivos, abuso sexual y otras formas de violencia física o emocional, y pueden desarrollar síntomas de estrés post-­‐traumático. Por otro lado, no es fácil determinar si están traumatizados desde esa edad, la mayoría de los niños no son capaces de comunicar verbalmente sus experiencias. En la mayoría de los ejemplo, son los padres los que notan un cambio en el comportamiento de su hijo y van en busca de ayuda. Los terapeutas junto con los padres y usando la información disponible del propio niño, desarrollan una hipótesis sobre el origen de los síntomas. Si existen indicios de que los síntomas actuales están relacionados con un evento traumático sin procesar, EMDR es idóneo. El método de Cuenta-­‐cuentos de Lovett puede ser usado para este propósito. Se necesita prestar una especial atención a la interacción entre los padres y el niño. Los padres pueden encontrar difícil apoyar a su hijo o reaccionar adecuadamente al comportamiento del niño cuando se sienten ansiosos o culpables de este evento traumático. Este tipo de cuestiones deben ser tomadas en cuenta ya que pueden agravar los síntomas ya existentes en el niño Después de una pequeña introducción teórica, numerosos casos serán mostrados en video. Allereerst worden enkele mislukkingen geanalyseerd. Het blijkt lastig te zijn om patiënt in de EMDR-procedure te krijgen en te houden. De rol van cultuur, van motivatie en committent en van taal wordt bekeken. Vervolgens worden aanpassingen getoond in: de voorbereiding, de uitleg en het te bereiken resultaat. Tevens wordt het belang aangegeven van het werken ‘als team’ met een vaste tolk. Alle inspanningen zijn gericht op het ‘gewoon’ uit kunnen voeren van EMDR.

While EMDR is treatment of choice for traumatized adults with PTSS, its efficacy has not been validated with traumatized refugees. As long as no evidence is available for EMDR with this population, EMDR with refugees should be informed by theoretical approaches to treatment of PTSS in adults. Four approaches are discussed: the phased approach that advocates stabilization before EMDR; the multimodal approach that advocates combining EMDR with other interventions; the trauma-focused approach that advocates offering EMDR to all adults with PTSS, including refugees and, the transcultural approach that advocates a culturally sensitive administration of EMDR. Several pilot studies on EMDR with traumatized refugees, randomized controlled trials of EMDR with multiply traumatized populations, and a trial of EMDR in a non-western setting may further inform EMDR-therapists working with traumatized refugees. Suggestions are made for combining the four approaches and scientific evidence in treatment planning and execution of EMDR with this population. The EMDR protocol itself prescribes several interventions for treating multiply traumatized patients, such as resource development and installation, clustering of traumatic experiences, and cognitive interweaves. Additionally, culturally sensitive interventions may enable the refugee to actively partake in the treatment process by increasing motivation for trauma processing, by diminishing language barriers, and by facilitating the formulation of culturally congruent meanings to trauma. The question remains whether EMDR, which offers an individualistic approach to trauma, is sufficiently suited to address the collective traumatization of victims of war and organized violence.

Die Behandlung dieser Menschen werden manchmal große Schwierigkeiten. Nicht nur wegen der Sprache, sondern auch wegen der relativen Bedeutung bestimmter Symptome in einem kulturellen Kontext, ist es wichtig, Kultur Hintergrund arbeiten Milieu und Unordnung bestimmten psychiatrischen / psychotherapeutischen betrachten. Aber was bedeutet Kultur-und Milieu sensiblen Psychiatrie oder Psychotherapie bedeuten? Basierend auf den vorhandenen Studien über Menschen mit Migrationshintergrund und die Untersuchungen des Sozio-Vision-Institute, und unsere eigenen Erfahrungen, die wir klären, wie eine interkulturelle Begegnung und ein Milieu sensiblen Psychotherapie erfolgreich angewandt werden.

Currently migration has reached extensive proportions. Globally, we assume a hundred to two hundred million migrants (WHO). Due to civil wars, natural disasters, political and economic circumstances the true figure is likely even higher. So it increasingly occurs that psychiatrists and psychotherapists encounter in their treatment individuals from other cultures, presenting with various psychiatric symptoms. The treatment of these people will sometimes face major difficulties. Not only because of language but also because of the relative importance of certain symptoms in a cultural context, it is essential to consider culture background, working milieu and disorder specific psychiatric/psychotherapy. But what does cultural and milieu sensitive psychiatry or psychotherapy mean? Based on the existing studies about people with immigrant backgrounds and the investigations of the Socio-Vision-Institute, and our own experiences we will elucidate how an intercultural meeting and a milieu sensitive psychotherapy can be successfully applied.

This presentation will outline the utilisation of EMDR with deaf clients exploring the challenges it presents for clinicians whilst exploring the potential for EMDR. Through the use of case examples this session will outline the ways in which EMDR needs to be adapted for this population Many clinicians struggle with Cognitive Behavioural Therapy (CBT) with d/Deaf clients due to some of the abstract ideas used and the heavy reliance on spoken language. Many people, both hearing and deaf find it difficult to think about thinking, and develop levels of insight into their emotions and behaviours simply by talking about them. Deaf people experience trauma and abuse at a much higher rate than hearing people, but there are few established therapies to help them. As Deaf people do not pick up all of the auditory stimuli it is likely that the trauma or difficult experiences that bring them to therapy may have been stored visually or kinaesthetically rather than linguistically. Trauma may also be caused by smaller events due to lack of information available to them. EMDR is an evidence based therapy that attempts to resolve emotional reactions to traumatic memories and their triggers. It does this through a combination of physical stimulation and the recollection and discussion of memories. As such, it seems ideally suited for use with people who are Deaf and have experienced trauma in their past. I describe how EMDR lends itself for successful use with Deaf clients and their families, giving two brief case examples where I used EMDR with Deaf children in British Sign Language.

As a complement to the strategies already used in EMDR, drawing gives form to the inner representations of the trauma, objectivising it. Thus the person no longer relates only to something inside him/her, but rather to something he/she can see and can concretely represent and modify. In the act of drawing, the patient makes an initial reorganisation of the form of the trauma, and begins to differentiate the adaptive ego, which has the tools and the ability to restructure the experiences, and the traumatic emotional part that suffers those experiences in a condition of impotence and passivity. The person may rapidly access preverbal and motor-sensory language, activating inborn creative skills. The use of this tool enables us to access the traumatic material gently, limiting dissociative reactions, bypassing avoidance and flight behaviour and setting a distance from pain by objectivising it. A protective space is created between the self and the part that holds the suffering. The patient is offered the possibility of drawing what is occurring in the self’s here and now, and given a choice of different graphic materials. At the end of the drawing and assessment phase the person is asked to note what has emerged, and a brief space of time may be allowed for description without interpretation. The represented image is treated as the inner image in the classic protocol. To start, the patient is asked to focus on the drawing, on the negative cognition and on the bodily location of the emotional disorder, while bilateral stimulation is applied. At the end of each set, the patient is asked what he/she has noticed and the therapist verifies where the person now is in the re-elaboration. If there is a change the person may either work on the drawing, modifying it, or, if the change of image is radical, may produce another drawing. The use of EMDR with children aged 4 years and over who have been exposed to physical abuse, disasters and other traumatic incidents in their infancy and early childhood is beginning to reveal some interesting material. The characteristics of the play and behaviours of traumatized young children were described by Terr (1991). It is suggested that the use of EMDR as a treatment approach is indicative that long-term retention for pre-verbal traumatic events does occur for children traumatised in infancy and perhaps even in the womb. This presentation presents two cases treated by EMDR in which it appears that the children are accessing and processing experiences that occurred too early in development to be encoded with language. EMDR helps these children access and process very early traumatisation and in some cases they are then able to report on them verbally. It is hypothesized that the EMDR technique helps create a verbal narrative for the early memories, and that this allows the child to process and integrate their sensations, emotions and cognitions within a more coherent personal biography. The ability of EMDR to help a child to process and bring to resolution hitherto almost inaccessible infantile memories particularly those relating to abuse, has enormous implications for the child's ability to move on and form healthier attachments and more construction behaviour patterns. This paper examines the empirical evidence in support and against all'EMDR understood as effective therapeutic approach for treating post-traumatic stress disorder. This case concerns a 4,5 year old girl with Selective Mutism. In this particular case. I considered Selective Mutism a symptom of an attachment trauma. Since the trauma wasn't accessible seeing her age and the complexity of the trauma, I used the symptom as a target. I applied EMDR within a phase model: the preparation phase, confrontation phase and integration phase. So through storytelling I offered her different themes to which she could respond by making drawings, figures in clay, etc. During the preparation phase, I focused on safety, ego strengthening and affect management to reduce the fear to speak. 1. Working with safety : the eye movements were first accomplished using a safe Image which brought up her own sense of security. Then, after imagining this safe place, the child was willing to play tapping games to strengthen feelings of safety. 2. Ego strengthening : to feel as strong as possible by installing resources and positive cognitions, and guiding the child towards acceptance and development of its unique being. Bilateral stimulation was used to strengthen the positive experiences. 3. Affect management: in the process of strengthening affect management, the child was given access to her anxiety by storytelling linked to visualization, the use of images and bodywork. Again, bilateral stimulation was used to strengthen the positive experiences/skills. After a few sessions. I introduced the use of language and stimulated her to make sounds, followed by pronouncing places of words and finally the pronunciation of complete words and sentences. Through this whole process, 1 combined the specific exercises to learn how to speak with bilateral stimulations. During the twelfth session, the child started talking spontaneously Given the fact that there wasn't any direct confrontation work during the sessions, we are left to wonder whether there has or hasn't occurred any trauma processing. The symptom came to a halt, together with the disappearance of other symptoms that were Inked to the trauma. The question is whether it is necessary to confront young children with their trauma in order to heal. Nevertheless, it seems like the combination of bilateral stimulation with storytelling, art therapy, play therapy and visualization speeded up the elimination of the child's trauma symptoms considerably.

Background: The study arose from an EMDR Humanitarian Assistance Programme training set up in response to the 2005 earthquake in Northern Pakistan. Part of this project set out to evaluate an Urdu version of IES-R. We wanted to evaluate the work done by newly trained EMDR Practitioners and there was paucity of measurement tools in Urdu language. We decided to translate IES-R and evaluate it. It will give us reliable scale to measure progress against treatment. The present study aims to present this Urdu version of the IES-R as a psychometrically sound instrument for both research and clinical use in its Urdu translation. Methods:Translation was completed in four stages. Two stages involved translation and two involved back translation. Recruitment for the evaluation took place at a Medical School in Abbotabad, and involved (N=118) participants. The inclusion criteria were that subjects must be fluent in both Urdu and English. Results: The results of Urdu and English versions of IES_R and GHQ were compared for Linguistic Equivalence, conceptual equivalence, and scale equivalence. All these measures showed no statistically significant differences. In conclusion the Urdu version of the IES-Revised can be used for clinical populations in Pakistan with evidence of good reliability and satisfactory validity. In developing EMDR research in Pakistan the utilisation of the U-IES-R will be an extremely useful addition.

The term “post-traumatic stress disorder” (PTSD) is a relatively new diagnostic label, being formally recognized in 1980 in the Diagnostic Statistical Manual for Psychiatric Illness – Third Edition (DSM-III) of the American Psychiatric Association (APA, 1980). Complex Post-Traumatic Stress Disorder (CP) is a more recently discussed, and newly-classified, phenomenon, initially discussed in the early 1990s (Herman, 1992a). Thus, as research into effective treatments for CP is sparse, the treatment of CP is the topic of this study, in which a guideline-based treatment program developed by the researcher for the treatment of CP is implemented and evaluated. Ten individuals participated in this study, undertaking individualized, guideline-based treatment programs spanning a period of six months. In providing background information relevant to this study, an explanation is provided regarding the nature of CP, and the reasons for its consideration as a separate phenomenon to PTSD. The adequacy of the PTSD formulation in enabling effective assessment and treatment of CP is also explored, with endorsement of previous researchers’ conclusions that the CP construct is more useful than the PTSD construct for assessing and treating survivors of long-term and multiple forms of abuse. The PTSD classification is restrictive, and not necessarily appropriate for certain forms of trauma (such as prolonged trauma, or multiple forms of trauma), as such trauma experiences may lead to specific effects that lay outside those formerly associated with PTSD. Such effects include alterations in affect regulation, consciousness, self-perception, interpersonal relationships, and in systems of meaning. Following discussion regarding the PTSD/CP classification, an examination of treatment methods currently used in the treatment of PTSD, and a review of treatment outcome studies, takes place. The adequacy of primary treatment methods in treating CP symptoms is then examined, with the conclusion that a range of treatment methods could potentially be useful in the treatment of CP symptoms. Individuals with a diagnosis of CP may benefit from the adoption of an eclectic approach, drawing on different treatment options for different symptoms, and constantly evaluating client progress and re-evaluating interventions. This review of treatment approaches is followed by details of an initial study undertaken to obtain feedback from individuals who had suffered long-term/multiple trauma and who had received treatment. Participants in this initial study were asked open-ended questions regarding the treatment approach they had experienced, the most useful aspect of the treatment, the least useful aspect, and other strategies/treatment approaches that may have been useful – but which were not used. The feedback obtained from these individuals was used to inform the development of treatment guidelines for use in the main study, as were recommendations made by Chu (1998). The predominant focus of the treatment guidelines was “ego strengthening”, a term coined by Chu (1998) to describe the “initial (sometimes lengthy) period of developing fundamental skills in maintaining supportive relationships, developing self-care strategies, coping with symptomatology, improving functioning, and establishing a positive self identity” (p.75). Using a case study approach, data are then presented relating to each of the ten individuals involved in the treatment program: details of his/her trauma experience(s)and the impact of the trauma (as perceived by each individual); details of each individual’s treatment program (as planned, and as implemented); post-treatment evaluation of the positive and negative aspects of the treatment program (from the therapist’s perspective); and details of the symptoms reported by the individual post-treatment, via psychometric assessment and also during interview. Analysis and discussion of the data relating to the ten participants in the study are the focal point of this study. The evaluation of the effectiveness of each individual’s treatment has been based predominantly on qualitative data, obtained from an analysis of language (discourse analysis) used by participants to describe their symptoms pre- and post-treatment. Both blatant and subtle changes in the language used by participants to describe themselves, their behaviour, and their relationships pre- and post-treatment have provided an insight into the possible changes that occurred as a result of the treatment program. The language used by participants has been a rich source of data, one that has enabled the researcher to obtain information that could not be obtained using psychometric assessment methods. Most of the participants in this study portrayed notable changes in many of the CP symptoms, including being more stable and having improved capacity to explore their early abuse. Although no direct cause-effect relationship between the participants’ treatment program and the improvements described can be established from this study, the participants’ perception that the program assisted them with their symptoms, and reported many aspects of “ego strengthening”, is of major importance. Such self-perception of strength and empowerment is important if an individual is going to be able to deal with past trauma experiences. In fact, abreactive work may have a greater chance of succeeding if those who have experienced long-term or multiple trauma are feeling more empowered, and more stable, as were the participants in this study (post-intervention). In concluding this study, recommendations have been made in regard to the use of guideline-based treatment programs in the responsible treatment of CP. Strengths and limitations of this study have also been highlighted, and recommendations have been made regarding possibilities for future research related to CP treatment. On the whole, this study has supported strongly other research that highlights the importance of focusing on “ego strengthening” in assisting those who have suffered long-term/multiple trauma experiences. Thus, a guideline-based program focusing on assisting sufferers of long-term trauma with some, or all, of the symptoms of CP, is recommended as an important first stage of any treatment of individuals who have experienced long-term/multiple trauma, allowing them to develop the emotional and psychological strength required to deal with past traumatic events. Clinicians who are treating patients whose history depicts long-term or multiple trauma experiences (either from their childhood, or at some stage in their adult life) need, therefore, to be mindful of assessing individuals for symptoms of CP – so that they can treat these symptoms prior to engaging in any work associated directly with the past traumatic experiences. Imagine tapping into that healing process in the brain and helping a child who witnessed her brother accidentally killed by a school bus, who then developed a school phobia, be able to return to school and eliminate her depression. What if you could help a foster child with a history of severe and chronic abuse, reduce his disruptive symptoms within a 9-month period so that he could stabilize and be adopted? Eye movement desensitization and reprocessing (EMDR) can be used in psychotherapy to help children heal from stressful experiences of both traumatic and developmental origins. And, while EMDR is not a magic wand, it is remarkable in its efficiency in reducing or eliminating significant mental health symptoms and healing the origins of trauma. This chapter is written for clinicians who have had little or no exposure to the EMDR treatment methodology or for those who may have wondered what it is and how it works. The goal of this chapter is to summarize the use of EMDR with children with case presentations woven through the steps of the EMDR protocol. As a potential paradigm shift for child and adolescent therapists who have been trained in child development and play therapy, this chapter will not only explain why EMDR with children and adolescents makes sense, but why EMDR is the treatment of choice for many children presenting with symptoms of trauma. The experienced child therapist will also learn how child development, play therapy, and other child-focused therapies can be integrated to overall case conceptualization with the eight phases of the EMDR protocol. This protocol is designed for small groups of children from the age of 5 upward. The language can, of course, be adjusted to suit the developmental level of the group. The protocol is to be used only by EMDR-trained therapists. The therapist must have the ability to react on the spot, evaluate, and provide further treatment for clients who are overwhelmed by the traumatic material. We recommend that work with this protocol include at least two group facilitators, in addition to the leader, in order to monitor the group and help the children carry out the instructions. Positive Psychological Change (PPC) following trauma is a developing field for which there is no standard terminology. The plethora of labels, of which Post Traumatic Growth (PTG) is probably the most common descriptor, arguably masks a significant gap in clinical and theoretical understanding of the phenomenon. One specific gap addressed by this study is PPC following psychological trauma stemming from a Road Traffic Accident (RTA) in which the person involved has subsequently received Eye Movement Desensitisation & Reprocessing (EMDR). To investigate this gap in knowledge, an Interpretative Phenomenological Analysis (IPA) approach was used and twelve participants recruited via a snowball sampling method. The participants were then interviewed using a Semi-structured Interview Questionnaire (SSIQ) and the interviews were then transcribed for IPA analysis. Key themes that emerged included Navigational Struggle (NS) to describe Negative Psychological Change (NPC), and Network Growth (NG), to describe PPC. At any one post-RTA/EMDR point there was a preponderance of one over the other, however, NS and NG were inseparable and found to co-exist along an NS-NG continuum. In addition, Figurative Language Use (FLU) had a significant role in both NS and NG yet was independent of both and apparently driving change towards the development of NG. Whilst NS and NG were both post-trauma phenomena, FLU seemed to hallmark expansion of memory networks as part of a general maturation process post-RTA. Furthermore, there was evidence that participants were incorporating their traumatic experiences via FLU into the rebuilding of their assumptive worlds. To account for these findings, an extension to Adaptive Information Processing (AIP) – the theory widely accepted to underpin EMDR - is proposed based upon a hypothesised Plasticity of Meaning (PoM), which is observable through FLU. PoM predicts which, why and how memory networks connect resulting in the adaptive processing predicted by AIP. The study’s findings are re-examined in terms of consequential modifications to the clinical use of EMDR. Extensive suggestions for further research are provided.

Through our work with deaf clients, we are forced to focus on the visual world. This world is by no means reserved for the hearing impaired. Even when working with preverbal trauma, adults and children who speak the language (yet) and control eg EMDR in a second language, we will need to express visually. We would like to take the viewer / listener along in exploring the visual possibilities offered by the EMDR process and offers them hope thereby to mimic, plastic and visual possibilities to enrich. The presentation is supported with imagery and of course there will be a Dutch sign language interpreter present.

In questi anni l’EMDR si è affermata come trattamento di evidenza nel più classico e più grave dei disturbi direttamente legati allo stress, cioè il PTSD (Bisson & Andrew, 2007) e sono stati evidenziati i suoi effetti sul SNC. Nonostante questo ed il suo progressivo impiego in una ampia varietà di situazioni sono ancora poche le riflessioni sull’EMDR dal punto di vista dell’integrazione mente-corpo, in particolare utilizzando i dati offerti dal campo di ricerca della PNEI. Nel presente lavoro ci soffermeremo su alcuni di questi aspetti per evidenziare come l’EMDR rappresenta una delle terapie più rispondenti alle nuove evidenze scientifiche ed alle esigenze di una scienza integrata della salute. Tre gli elementi salienti offerti dalla ricerca: 1. La vita ha plasmato nel percorso evolutivo strutture altamente integrate, descrivibili come sistemi complessi a rete in retroazione continua con il contesto. In particolare i sistemi nervoso, endocrino ed immunitario costituiscono nel network psicocorporeo un “super-sistema” di relazione e regolazione in grado di assicurare gli equilibri adattativi incorporando l’esperienza e gestendo la dinamica mantenimento-cambiamento (“regolazione allostatica”). Quindi l’organismo non è una macchina fatta di parti autonome tra loro assemblate, ma una realtà che parte dall’unità (zigote) e mantiene tale carattere unitario in tutte le sue articolazioni. 2. Lo stress non è qualcosa di per sé eccezionale e patologico, deve invece essere visto – all’opposto – come l’espressione di un insieme di processi (un sistema) che modulano la regolazione individuo-contesto a fini adattativi. In sostanza il sistema dello stress è sempre attivo e solo in situazioni di eccesso o di carenza (acute o croniche) produce effetti progressivamente dannosi per l’individuo. Le attività legate allo stress producono un “carico allostatico” (peso biologico, energetico) che – in relazione a fattori soggettivi ed esterni – può divenire “sovraccarico” innescando una catena di successive alterazioni nel funzionamento dell’organismo. Parallelamente vi è un “carico” ed un “sovraccarico psicologico” legati al primo da processi circolari. 3. Nel network corporeo e nel sistema dello stress, ovvero nei processi di regolazione generale interni e individuo-contesto, la mente svolge un ruolo cruciale di modulazione come dimensione nella quale i segnali (interni ed esterni) vengono trasformati in informazioni (assegnazione di significati) in base ai percorsi ed alle esigenze individuali di adattamento. La mente rappresenta così la più alta espressione dell’evoluzione della vita e delle sue strategie adattative e ciò spiega il suo ruolo rispetto al funzionamento complessivo dell’organismo. Le recenti acquisizioni delle neuroscienze hanno mostrato l’interdipendenza tra attività psichica, espressione genica e plasticità cerebrale che rende possibile il ruolo della mente. Il sovraccarico allostatico e psicologico legati allo stress dipendono in gran parte dall’attività mentale e costituiscono il principale fattore di rischio per la salute nelle società occidentali, antecedenti accertati delle patologie più diffuse. Se guardiamo a quanto detto sinora nell’ottica dello sviluppo individuale (fase intrauterina, processi di attaccamento, eventi significativi successivi) possiamo avere una idea di come mente, corpo e contesto interagiscono nella strutturazione dell’individuo ed il ruolo condizionante e de-strutturante delle situazioni e degli eventi che provocano una alterazione da stress. Le ricerche sul rapporto tra stress e sviluppo psicobiologico, tra modalità di attaccamento, strutturazione dei circuiti cerebrali, o tra queste e salute, confermano tali assunti (Lazzari, 2007). Venendo più da vicino all’EMDR, gli studi sulla psicobiologica del PTSD stanno mostrando che siamo di fronte sostanzialmente ad una manifestazione (particolarmente complessa ed accentuata) degli effetti dello stress sul network psicocorporeo (Iribarren et al., 2005). La ricerca sul PTSD (e gli altri disturbi da stress del DSM) deve incrociare necessariamente i filoni di studio sopra richiamati ed utilizzare la cornice concettuale che ne scaturisce. Gli effetti psicobiologici dello stress estremo e del trauma vanno inquadrati nell’ambito delle interazioni mente-corpo e della regolazione allostatica (carico vs sovraccarico allostatico). A nostro avviso, al di là degli specifici effetti sul SNC, l’EMDR si è mostrata efficace nel PTSD in relazione alla sua caratteristica generale di ricercare ed utilizzare come “target” non singoli aspetti di una esperienza, bensì l’esperienza nei suoi diversi aspetti: cognitivi, emotivi, fisiologici e relazionali. Il lavoro dell’EMR va a recuperare una integrazione perduta e lavora su questa. Si deve tenere presente infatti che, in via generale e preminente, lo stress compromette i livelli di integrazione, coerenza e flessibilità del sistema: lo stress cronico per processi progressivi di disregolazione, quello acuto per “rotture”, blocchi e sconnessioni. Pertanto l’elaborazione dell’informazione (e delle conseguenze) legata ad eventi e situazioni stressanti passa attraverso una re-integrazione di tutti gli aspetti psicobiologici correlati per giungere ad una ristrutturazione. Il livello di “penetrazione” (efficacia) di un approccio terapeutico è molto legato alla sua omogeneità con la realtà su cui interviene,cioè al fatto di parlare lo stesso linguaggio. E questo è un punto di forza notevole per l’EMDR. I dati disponibili sono coerenti con quanto enunciato: l’EMDR modifica i parametri fisiologici riducendo ed annullando l’attivazione da stress, spostando la bilancia dall’inibizione simpatica all’attivazione parasimpatica (Sack et al. 2007) e risulta associata l’abbassamento dell’arousal fisiologico, cioè con pattern di attività regolari e trofiche (Sack et al. 2008), mostrando similarità con quanto accade durante il sonno REM (Elofsson et al., 2008). L’EMDR incide altresì su disturbi fisici legati al sistema dello stress, come ad esempio hanno mostrato ricerche su patologie dermatologiche (Gupta, 2002).

In recent years, EMDR has established itself as the treatment of evidence in the most classical and most serious of disorders directly related to stress, that is, PTSD (Bisson & Andrew, 2007) and were highlighted its effects on the CNS. Despite this and its progressive use in a wide variety of situations are still few reflections from the perspective sull'EMDR mind-body integration, in particular using data provided by the search of PNEI. In this work we will focus on some of these issues to highlight how EMDR is one of the therapies are more responsive to new scientific evidence and the needs of an integrated science of health. Three main elements of research are: 1. Life has shaped the evolutionary highly integrated structures, we describe how systems complex feedback network continues with the context. In particular, the nervous, endocrine immune network and provide psycho in a "super-system" of relationship and adjustment able to ensure the balance incorporating adaptive expertise and managing the dynamic maintenance-change ( "regulation allostatica"). Then the body is a machine made of autonomous parts, assembled together, but a reality that leaves the unit (zygote) and maintains that unitary in all its joints. 2. Stress is not something in itself exceptional and pathological, but must be seen - in contrast -- as the expression of a set of processes (a system) that modulate the individual-regulation framework for adaptive. In essence, the system of stress is always on and only in situations of excess or deficiency (acute or chronic) effects progressively damaging to the individual. The Stress-related activity produces a "load allostatico" (weight biological energy) that -- relation to subjective factors and external - can become "overloaded" by triggering a chain of subsequent alterations in the functioning of the organism. In parallel there is a "load" and a "Overload psychological" processes related to the first round. 3. In the network system and body of stress, or in the process of setting general internal and individual-context, the mind plays a crucial role as a dimension of modulation in which signals (internal and external) are transformed into information (assigning meanings) depending on the paths and individual needs of adaptation. The mind is so the highest expression of the evolution of life and its adaptive strategies, which explains its role in relation to the overall functioning of the organism. The recent acquisitions of neuroscience have shown the interdependence of psychic activity, gene expression and plasticity brain that makes the role of the mind. Overload allostatico and psychological stress-related depend in large part by activism mental and constitute the main risk factor for health in Western societies, antecedents established disease spreading. If we look to the foregoing view of personal development (stage intrauterine attachment process, significant events later) we can have an idea of how mind, body and environment interact in the structuring the individual and the role conditioning and de-structuring of situations and events that cause an alteration by stress. Research on the relationship between stress and psychobiological development, including mode of attachment, structure of brain circuits, or between them and health, confirmation of these given (Lazzari, 2007). Coming closer all'EMDR, psychobiological studies of PTSD are showing that we are dealing essentially an event (especially complex and pronounced) the effects of stress on the network psycho (Iribarren et al., 2005). Research on PTSD (and other stress disorders DSM) must necessarily cross the strands study mentioned above and use the conceptual framework that arises. Psychobiological effects of extreme stress and trauma should be classified within mind-body interactions and regulation allostatica (load vs. overload allostatico). In our view, beyond the specific effects on the CNS, EMDR has been shown effective in PTSD in connection with his characteristic broad research and use as a "target" rather than individual aspects of an experience, but experience in its different aspects: cognitive, emotional, physiological and relational. When working with pre-­‐verbal trauma, with children or adults who are (not yet) fluent in their native speech or when we use EMDR with clients that use a second language, we will have to use alternatives to spoken languages. When used correctly, visual input can enrich and enable the EMDR process greatly. In this presentation we invite visitors to explore the visual aspects and possibilities of the EMDR process with us. En nuestro trabajo terapéutico con EMDR con clientes sordos o con hipoacusia, nos vemos obligados a centrarnos mucho en el mundo visual. Sin embargo, ¡el énfasis en los aspectos visuales no solo es necesario con este grupo de pacientes! A technique is presented that measures the severity of PTSD as a result of a traumatic event. The technique is exclusively based on the symptoms of PTSD as defined by DSM IV. The level of distress of each symptom is measured on an 8 point scale by Ordered Metric (OM) Scaling which is quick to use, efficient, easily understood and unbiased. It uses words in the language and does not require people to form an analogy between a quality and their level of distress. The scale is within a person’s natural span of measurement. (7 +/- 2). We present a therapeutic protocol that combined EMDR and narrative family therapy externalisation techniques. We expose in a format of clinical case report the therapeutic work with a family that have adopted a child who suffer of a simple phobia. Using the protocol we describe above we carried out an analysis of the therapeutic issues and benefices of EMDR and narrative therapy in these family processes. This protocol has been useful to manage the phobia symptoms and to improve the adoptive family identity and cohesion. The combined approach we describe could help adoptive families to understand and to interpret the meanings from the construction that family systems make of their own world and relieve psychological symptoms that children could present.

This presentation will highlight the effects of domestic violence and other traumatic experiences on children from pre birth to 12 years of age and how EMDR can reduce the symptoms and give the subject a more appropriate perception of their experiences. The neurobiological aspects will be discussed at pre and post treatment of EMDR. EMDR therapy has proven to be a highly successful technique in the relief of psychological distress after trauma. It will be shown that babies traumatised before birth can be treated as effectively as children traumatised after birth. The impact on the developing baby will be discussed in relation to the stage of gestation that the mother experienced trauma. Knowledge of sensory development in pregnancy can inform the treatment plan for mother and baby subsequently. The impact of domestic violence and traumatic birth will be explored. If untreated in the mother there can be long lasting effects in the mother, child and the parent child relationship. Clinical examples will explain how EMDR can be modified to treat unresolved traumatic events. In infancy and early childhood memories are stored in sensory form often with little language. We will illustrate on video a narrative approach combined with parent facilitated EMDR in a traumatised 30 month old infant whose parents have a history of drug use. The impact of traumatic stress on the developing brain will be discussed and illustrated by video of two EMDR sessions with 10 and 12 year old children. This will show how the normal EMDR protocol must be modified to take childhood factors into account.

The proposal for an extension to Adaptive Information Processing (AIP) (Shapiro 1995, 2001) is derived from the author’s doctoral thesis (Blore 2012a) – a phenomenological study of positive psychological changes experienced by survivors of road traffi c accidents in the post Eye Movement Desensitisation & Reprocessing (EMDR) treatment context. It is argued that AIP does not fully account for the totality of psychological change following EMDR, partly due to lack of knowledge and partly because of the emphasis on explaining the reduction of negative psychological change (rNPC). The main presentation expands on this reasoning, whilst this poster focuses on the proposed theory extension a: ‘Plasticity of Meaning’ (PoM). To illustrate this theory extension, three examples of fi gurative language use (FLU) obtained during interviews with participants are subjected to microtextual analyses (see Smith 2004, p51). It is argued that FLU is a phenomenological (i.e. observable) event that suggests a ‘trading of words’ in turn suggesting neurological networks connecting – a central tenet of AIP. The phrase ‘PoM’ has been coined because of hypothesised similarities to Frey & Morris’ (1997) synaptic plasticity and Cahill & McGaugh’s (1998) reconsolidation of memory theory.

Furthermore, the article discusses the ethical necessity for clinicians and supervisors to practice within the limits of their competency. EMDR is a particularly excellent example because of the unknown mechanisms at work to create change in clients. Therefore, there is no common language or knowledge base that supports the use and supervision of EMDR in the same way that practicing or supervising a variety of "traditional talk" counseling techniques might be appropriate without specialized training. The majority of supervisors of students and practicing clinicians were trained in variety of theoretical orientations. Background: Post-traumatic stress disorder (PTSD) is highly prevalent in children and adolescents who have experienced trauma and has high personal and health costs. Although a wide range of psychological therapies have been used in the treatment of PTSD there are no systematic reviews of these therapies in children and adolescents. Objectives: To examine the effectiveness of psychological therapies in treating children and adolescents who have been diagnosed with PTSD. Search methods: We searched the Cochrane Depression, Anxiety and Neurosis Review Group’s Specialised Register (CCDANCTR) to December 2011. The CCDANCTR includes relevant randomised controlled trials fromthe following bibliographic databases: CENTRAL (the Cochrane Central Register of Controlled Trials) (all years), EMBASE (1974 -), MEDLINE (1950 -) and PsycINFO (1967 -). We also checked reference lists of relevant studies and reviews. We applied no date or language restrictions. Selection criteria: All randomised controlled trials of psychological therapies compared to a control, pharmacological therapy or other treatments in children or adolescents exposed to a traumatic event or diagnosed with PTSD. Data collection and analysis: Two members of the review group independently extracted data. If differences were identified, they were resolved by consensus, or referral to the review team. We calculated the odds ratio (OR) for binary outcomes, the standardised mean difference (SMD) for continuous outcomes, and 95% confidence intervals (CI) for both, using a fixed-effect model. If heterogeneity was found we used a random-effects model. Main results Fourteen studies including 758 participants were included in this review. The types of trauma participants had been exposed to included sexual abuse, civil violence, natural disaster, domestic violence and motor vehicle accidents. Most participants were clients of a traumarelated support service. The psychological therapies used in these studies were cognitive behavioural therapy (CBT), exposure-based, psychodynamic, narrative, supportive counselling, and eye movement desensitisation and reprocessing (EMDR). Most compared a psychological therapy to a control group. No study compared psychological therapies to pharmacological therapies alone or as an adjunct to a psychological therapy. Across all psychological therapies, improvement was significantly better (three studies, n = 80, OR 4.21, 95% CI 1.12 to 15.85) and symptoms of PTSD (seven studies, n = 271, SMD -0.90, 95% CI -1.24 to -0.42), anxiety (three studies, n = 91, SMD -0.57, 95% CI -1.00 to -0.13) and depression (five studies, n = 156, SMD -0.74, 95% CI -1.11 to -0.36) were significantly lower within a month of completing psychological therapy compared to a control group. The psychological therapy for which there was the best evidence of effectiveness was CBT. Improvement was significantly better for up to a year following treatment (up to one month: two studies, n = 49, OR 8.64, 95% CI 2.01 to 37.14; up to one year: one study, n = 25, OR 8.00, 95% CI 1.21 to 52.69). PTSD symptom scores were also significantly lower for up to one year (up to one month: three studies, n = 98, SMD -1.34, 95% CI -1.79 to -0.89; up to one year: one study, n = 36, SMD -0.73, 95% CI -1.44 to -0.01), and depression scores were lower for up to a month (three studies, n = 98, SMD -0.80, 95% CI -1.47 to -0.13) in the CBT group compared to a control. No adverse effects were identified. No study was rated as a high risk for selection or detection bias but a minority were rated as a high risk for attrition, reporting and other bias. Most included studies were rated as an unclear risk for selection, detection and attrition bias. Authors’ conclusions: There is evidence for the effectiveness of psychological therapies, particularly CBT, for treating PTSD in children and adolescents for up to a month following treatment. At this stage, there is no clear evidence for the effectiveness of one psychological therapy compared to others. There is also not enough evidence to conclude that children and adolescents with particular types of trauma are more or less likely to respond to psychological therapies than others. The findings of this review are limited by the potential for methodological biases, and the small number and generally small size of identified studies. In addition, there was evidence of substantial heterogeneity in some analyses which could not be explained by subgroup or sensitivity analyses. More evidence is required for the effectiveness of all psychological therapiesmore than one month after treatment.Much more evidence is needed to demonstrate the relative effectiveness of different psychological therapies or the effectiveness of psychological therapies compared to other treatments. More details are required in future trials in regards to the types of trauma that preceded the diagnosis of PTSD and whether the traumas are single event or ongoing. Future studies should also aim to identify the most valid and reliable measures of PTSD symptoms and ensure that all scores, total and sub-scores, are consistently reported.

Posttraumatic stress disorder is a challenging condition, as people become captured by their past experiences and have difficulty engaging with the present. At the core of this condition is the role of traumatic memories, which orientate the individual's awareness and reactivity to reminders of the instigating traumatic event. The role of traumatic events has not been fully understood and grappled with in the full range of psychopathological conditions. This has important implications for the application of EMDR as a treatment for disorders above and beyond posttraumatic stress disorder.
However, the problems with information processing in PTSD go above and beyond the fear circuitry and reactivity to traumatic memories. Individuals with PTSD also have major difficulties with their self-orientation, which is reflected in deficits in default networks, the idling systems of the brain. These changes are indicative of problems in self-registration and free-floating reflection. Dissociative symptoms may relate to these abnormalities of individuals resting states as they reflect a sense of disconnection and integration of internal states into consciousness.
Secondly, posttraumatic stress disorder is associated with major problems in dealing with neutral environmental information. This is reflected in the symptoms of difficulty with concentration and emotional numbing. The underlying neurobiology of the working memory abnormalities in posttraumatic stress disorder will be highlighted. These studies show that, in PTSD, relatively simple attentional tasks recruit neural networks normally reserved for more demanding and higher order tasks. When confronted with more demanding challenges, individuals with PTSD do not have any further capacity to allocate to processing complex environments.
Individuals with PTSD also demonstrate a problem with switching their attentional focus from an idling to active state. The data suggests that they continue to use visio-spatial networks more than language-based systems for dealing with verbal tasks. This observation is in keeping with a broad body of literature, which suggests that there are problems with the processing of verbal memory tasks in PTSD. EMDR, as a treatment, may have an advantage, as it is not so dependent on verbal representations of traumatic experiences as other treatment approaches.
Finally, an important development in the field is a better understanding of the patterns of abnormal cortical arousal that accompany the peripheral arousal abnormalities in PTSD. Quantitative EEG has given insights into the instability of the cortical neural networks. Neurotherapy represents a treatment that can further assist clinicians in the management of these patients. It is important to consider the underlying psychosomatic aspects of posttraumatic stress disorder and ensure that treatment addresses these components as well the traumatic memories. Treatment should be thought of as a staged process where the processing of traumatic memories is only one component of a disorder that impacts on a range of information processing domains.

This study examined the traumatic experiences and psychological symptoms of 60 refugees who were recently resettled in the United States. Subjects were from Cuba, Iraq, Haiti, Sudan, and Bosnia. Data was collected for each subject on traumatic experiences occurring prior to their arrival. In addition, anxiety and depression were assessed using the Hopkins Symptom Checklist (HSC-25). Quality of life and coping skills were assessed using the WHO Quality of Life-BREF and the Coping Styles Questionnaire, respectively. Trained, bi-lingual interviewers assessed for PTSD using the Clinician-Administered PTSD Scale. All instruments were translated into the appropriate language for subjects who did not speak English. Measures were back-translated to assure accuracy of translations. Three months following their arrival, information was collected regarding employment and public assistance. Predictors of PTSD, quality of life, and refugee self-sufficiency were analyzed using multiple regression and logistic regression analyses. Trauma-related variables were predictive of PTSD, which in turn had a negative impact on quality of life and self-sufficiency. Implications for the resettlement programs of government and non-governmental organizations are discussed.

We know from research that since neurobiology is the same for humans, then the reaction to stress is the same for all human beings in the world. That is why EMDR works so well with victims of natural disasters and of adverse childhood experiences, including sexual and physical violence and emotional neglect. Because of cognitive functioning, e.g., creating metaphors and images for self-soothing, establishing social contact and/ or personal interpretation, or assigning meaning to stressful experiences, we have to emphasize the importance of cultural influences. We want to invite people from different cultures to join this open meeting to present, share, and discuss their experiences and perhaps difficulties with cultural adaptation of trauma-therapy techniques, including EMDR. For example, we may need to examine where and how the wording of the EMDR-protocol needs to be adapted to recognize and accommodate cultural differences; perhaps we need to look at the language structure and its accessibility. This presentation compares two different descriptions: Paradigm and State of Consciousness, of the changes, observable in a psychotherapy integrated with EMDR, to establish which of the two descriptions is the most likely and consistent. Applying the method of the double description it will seek to identify contributions that may be 25 useful for the construction of a Unified Theory of Psychotherapy. At first, attention is focused upon the necessity to construct a bridge-language between the different dialects of psychotherapies. Short specialized definitions, such as Paradigm and State of Conscience, could be examples of the complex concepts, here denominated synthetic �t� metaphors, transtheorical or Tran disciplinal, to be researched for constructing a common language between the plurality of psychotherapies. Some implications of the methodological innovation carried on by EMDR in psychology, psychotherapy, and in other disciplines are then analysed and proposed. In particular, the easy integration of traditional psychotherapeutic methodologies with EMDR and the discovery of new target of psychotherapy, have led to looking at all psychological activities in terms of a spatial metaphor. On the basis of this a general map of the psychological territories of �sapiens sapiens� has been drafted. This general map is divided in two main areas: A -the psychological territories of the individual, B -the psychological territories of the species, This species map has some specific characteristics; since �sapiens sapiens� are social animals, their relations have clear functions of social and environmental group interface too. These maps will be illustrated. In its original meaning, the definition of Paradigm will be found within the maps and consequently analysed in its variations. The amplified theory of State of Conscience will be presented and analysis, similar to the previous one, will be carried out. A description of EMDR, that may be useful for implementing its understanding, will be proposed at the end.

An efficient strategic model is described, one that systematically facilitates a developmental hypothesis about the symptoms or psychopathology of clients and which then efficiently implements EMDR with that developmental perspective or template. Four major features of the model are described including a strategic history-taking format which yields a “Developmental Baseline” from which a macro treatment plan can be formulated; strategic Ericksonian (hypnotic) language to mobilize client resources and bypass resistance; flexible targeting options; and therapeutic attunement. Clinical observations of EMDR treatment sessions indicate that therapeutic results are often achieved through the progressive emergence of an adult perspective, particularly when the client was previously locked into the emotional responses of a childhoodbased trauma. Clearly, most childhood experiences are infused with a sense of powerlessness, lack of choice, lack of control, and inadequacy. Even the best of childhoods have moments when the parents attempt to leave for the evening and the child feels abandoned, powerless, and uncared for. Most clients who enter therapy do not have a simple problem of a single disturbing memory. More typically, clients come to therapy with a mixed presentation, of not only emotional disturbance, but also a history of conscious or unconscious choices about how best to soothe, contain, or avoid that disturbance. When the client has a problem that includes positive and negative affective components, we could say (in the language of Shapiro's Adaptive Information Processing Model) that the chain of experiential associations -- the dysfunctionally stored memory network -- has positively valued experience at the entry point into the network and disturbing material at other, less accessible places. Clients often experience this situation as one of conflicting ego states. Specifically, one ego state may be positively emotionally invested in an outcome that is an obstacle to the person's larger life goals. When this happens and the usual EMDR method of targeting negative affect is stalled, it may be useful to target the positive side of the issue, that is, an image that has a positive emotional valence. Such clients are asked to hold in mind the enjoyable aspects of a problematic wish or identity while engaging in Dual Attention Stimulation (DAS). In this way, they can process these positive aspects, "disinvest" from the problem, and go on to resolve the conflict. Several session transcripts illustrate how this approach can work in practice. Since the introduction of EMDR training courses in Hong Kong a few years ago, the method has been gaining an enormously growing interest among professionals. One of the indications to the growing interest in EMDR is the high rate of participation in the training courses. While there is a volume of studies on the efficacy of EMDR among different populations, therapists’ viewpoints on its use has not been studied extensively. The purpose of the paper is to examine the views of therapists who have undergone EMDR training in Hong Kong and have used the method in their practice. Data were collected through an open-ended questionnaire and follow-up in depth interviews with a group of therapists. The questionnaire sought information about their initial experience of using EMDR, the types of patients they used it with, the contribution of the method to the therapy and the difficulties they encountered. Follow-up interviews aimed to shed more light on issues raised by the respondents and to provide a platform for their further elaboration. Participants’ responses indicated the positive effects EMDR use on clients and its facilitation of the therapy process. The respondents indicated difficulties in following the exact EDMR protocol especially in relation to the terminology such as: “completely false”, “completely true”, “emotions” and “sensation”. Additional issues raised around the use of the scales and of different bilateral stimulations. On the basis of these finding special consideration should be given to the adaptation of the EMDR protocol to the Chinese language and way of thinking.

In Isaac Bashevis Singer’s prolifi c Holocaust novel, Enemies: A Love Story (1972), the main character, Herman Broder, sets his eyes into an oscillating motion whenever he needs to deal with stress or anxiety. The books and poems of Native American author Sherman Alexie (1992, 2009) beautifully document how centuries of tribes have utilized the dance, an activity of tactile bilateral stimulation, to cope with distress and heighten performance. Kyra Gaunt (2006) documented how generations of African American girls have used clapping games, doubledutch jump rope, and other bilateral rhythmic activities to transition into adulthood. This small collection of examples sets a larger context for the development of eye movement desensitization and reprocessing (EMDR) that I feel, in the excitement over the 20th anniversary of Shapiro’s discovery, we have failed to consider. I write this letter with a great deal of respect for Dr. Francine Shapiro as someone who has tapped into the seemingly innate, healing power of bilateral stimulation and systematized it for use in psychotherapeutic settings. This presentation will focus on key factors believed to contribute to the development and maintenance of OCD: a damaged, diffuse sense of self; a perceived inadequacy to function, to meet the vicissitudes of life; and an elaborate defence system designed to offer the illusion of safety and control (avoidance of affect and action). Given the prominence of cognitive distortions and avoidance of affect in OCD, and the impact of EMDR on such phenomena as exhibited in the literature about EMDR treatment with PTSD, Whisman believes EMDR has the potential to be quite beneficial in the treatment of OCD. EMDR seems to allow the OCD client to hold a dual focus: the illusion of the disorder and the reality of the self; and to tolerate the affect accompanying this processing. Bridging past experience and present behavior, EMDR appears to allow new connections to be made that offer the client a distance form the disorder and a strengthening of the self. A model for educating the client about OCD will be presented a model whcih develops a "language" between therapist and client and bridges self and disorder; changes in the standard EMDR protocol will be presented and demonstrated via videotape; and EMDR components such as targeting, cognitive interweaves, and resource installation will be addressed. Clinical observation and client self-report are pointing to EMDR as an effective treatment component for OCD.

Severely traumatized people don't communicate in a direct and clear way, they have their 'own language' and in order to understand the patient's inner world, we need to understand the silences, the somatic symptoms the subtle (and not so subtle) intrusions; all of these are frequent symptoms that the patient can't detect, understand or disclose to us (not directly). It is widely known that EMDR clinicians must be careful when dealing with dissociative patients; techniques that can be used during the stabilization phase have been developed for the treatment of dissociative disorders (Knipe, Forgash .......). These techniques are complementary to the basic protocols and are very useful but the problem arises when therapists are not able to identify and/or understand what we call the 'dissociative language'. We must keep in mind that most dissociative patients have grown in an early environment where their needs were not taken into account. Many never had the possibility to express their feelings openly. For this, it is important to focus and under^ stand the indirect, complex and ambivalent communication of these people especially during an EMDR session. The approach to these difficulties is not only a question of protocol modifications. but a complex learning from the therapist of the 'dissociative language'. Several examples from videos of therapy sessions and case descriptions will be presented.

Today, the dreaming mind is understood to improve learning, organize novel solutions to problems from waking consciousness and, thus, creatively support survival. This function, of finding new creative solutions, is also observed in the desensitization phase four of the EMDR protocol and is a key component in Ego State Therapy (EST). Dreams appear to select targets from the client’s history, that are causing problems in current time. In addition, dreams may respond to interventions made during a therapeutic session, as if the dreaming mind were in a dialogue with the client and the therapist. This type of dream leads to a more creative dialogue in subsequent therapy. Repetitive nightmares are the dreaming signature of PTSD, and these too respond well to EMDR and rehearsed changes in imagery, including that of ego states and can result in improved sleep quality. Dreams can offer a resource figure in blocked therapy; demonstrate the validity of ego states and of ego state relationships. This presentation focuses on the use of dreams, in combination with EMDR, as well as EST, for clients suffering from post traumatic syndromes. A review of the literature will be given but primarily case studies and video material from actual sessions will shared.

Individuals with Autism have a number of complex differences that make Trauma processing exceptionally difficult. In order to use the eight step protocol with these individuals, preparation and some modifications are necessary. The process presented in this workshop will provide some general information about the characteristics of autistic individuals, step by step skill training to precede the EMDR process, the use of Carol Gray's Social Stories to help clarify those targets and situations being processed, and the EMDR protocol with slight adaptations for individuals with speech and language impairments. Also included are cautions for using EMDR with this population due to their complex differences. This process has been used successfully with individuals across the Autistic Spectrum as well as individuals with Asperger's Syndrome and other developmental disorders including Williams Syndrome. Adaptive Information Processing (AIP), the theory that underpins EMDR may seem somewhat redundant given the burgeoning clinical database and numerous international recommendations all of which effectively point to ‘because EMDR works, it therefore works’. Put succinctly, does AIP serve any further purpose? The author argues that it does, but that its current bias toward explaining the reduction of Negative Psychological Change (NPC) needs to change. The author’s recent research has highlighted the extensive role of Figurative Language Use (FLU) in PPC resulting in participants’ increased ability to express him/herself following EMDR. To explain these observations, a ‘Plasticity of Meaning’ extension to AIP is proposed to account for PPC and thus convert AIP into a unifying theory of change. The result is to propose a ‘total beneficial outcome’ of EMDR that combines both existing evidence-based practice together with the optimisation of the Maslowvian concept of a client’s ‘full psychological height’.

Resource oriented group-EMDR in a Trauma- and EMDR-Training group of native helpers who suffered themselves from war-trauma in Bosnia-Herzigowina. The presentation shows a successful and impressive experiment of EMDR group treatment (22 adult helpers) after a new developed ressource installation protocoll by Lutz-Ulrich Besser, EMDRIA Trainer for EMDR with children, in connection with EMDR-Trauma-Exposition and “4 square paint-technique”. The experiment has already been carried out in the year 2000 as a research- and help project by Robert H. Tinker and Sandra Wilson with war traumatised children from Kosovo living in a refugee camp in Germany. It will not only demonstrate the technique with pictures and “SUC-Werten”(subjective units of comfort) but also show the processional way as things develop from negative to positive in pictures, cognitions, emotions and physical feelings.
