Abstract:
目的 分析淄博铁路交通事故伤员心理行为反应特点以寻找救治交通事故后患者的心理的有效方法。方法 采用心理危机结构式访谈问卷,对2008年4月28日发生的山东淄博胶济铁路重大交通事故中的226伤员进行心理状态评估,并对22名ASD患者进行眼动脱敏再加工(eye movement desensitization and reprocessing, EMDR)治疗,比较EMDR治疗前后的心理行为反应的差异。结果 有22名达到ASD(急性应激障碍)诊断标准,本次铁路交通事故中ASD的发生率为9.73%,伤员中主要以闯人、警觉性增高表现为主,并伴随着其他的负性情绪体验。女性组ASD的发生率(14.85%)高于男性组(5.60%),P<0.05。女性组心理行为反应结果明显重于男性组(P<0.05),EMDR能够显著改善ASD患者的闯入、警觉性增高症状(P<0.01),但愤怒情绪没有显著改善((P=0.227))。结论 铁路交通事故后,女性比男性更容易发生ASD。EMDR可有效地解决ASD患者除愤怒以外的心理危机。

Objective: To investigate the psychological characteristics in the casualties of 4.28 train crash on the Jiao-Ji railway track and to find a effective way to relieve the psychological crisis induced by traffic accident. Method: A total of 226 casualties were assessed in respect of psychological crisis with interview questionnaire after 4.28 train track on Jiao-ji railwayine. Twenty-two casualties meeting acute stress disorder (ASD) criteria from DSM-IV were treated with EMDR. The therapeutic effects of eye movement desensitization and reprocessing (EMDR) on ASD were assessed. Results: The incidence of ASD was 9.73% (22 casualties). The major psychological consequences in casualties were intrusive symptoms, symptoms of hyperarousal, and negative emotional symptoms. Significant differences on gender had been found in incidence rate of ASD female 14.85% and male 5.60%, P < 0.05. The women manifested more severe psychological consequences than men in this train crash accident. Significant treatment effect was found in EMDR on ASD. EMDR can significantly improve the intrusive symptoms and symptoms of hyperarousal (P < 0.05), but can not significantly improve negative emotional symptoms (P > 0.05). Conclusions: The women showed more severe psychological consequences than men after train crash accident. EMDR was effective treatment on ASD but negative emotional symptoms.

Abstract:
We previously reported on the outcomes of a controlled study of eye movement desensitization and reprocessing (EMDR) effectiveness in the treatment of traumatic memory (Wilson, Tinker, & Becker, 1994; Wilson, Becker, & tinker, in press). In that study we found that three, 90-minute sessions of EMDR (Shapiro, 1995) "normalized the psychological functioning of the previously traumatized participants (g = 80) on all dependent measures. The present study is a 15-month follow up of those participants. I Method: The research design is shown in Table 1. Participants were randomly assigned to EMDR or to Delayed EMDR conditions. Pretreatment measurement occurred at measurement time TI. Participants in the EMDR condition received EMDR between T1 and T2; those in the Delayed EMDR condition received EMDR between T2 and T3. All participants were tested immediately following treatment and at 3 months following treatment (at T4). The 15 month, long-term follow up occurred at measurement time T5. An independent assessor collected all of the following dependent measures: Subjective Units of Disturbance Scale (SUDS; Wolpe, 1990), Impact of Events Scale (IES; Hmowitz, Wilner, & Alvarez, 1979), State/Trait Anxiety Inventory (STAI; Spielberger, Gorsuch, Lushene, Vagg, & Jacobs, 1983), and the Symptom Checklist (SCL-90-R, Derogatis, 1992). [Table 1. Results: Two analyses were performed to assess the impact of EMDR treatment at the 15-month follow up. First, in order to assess the overall, long-term impact of EMDR, the 15-month follow-up scores were compared with the pretreatment scores. There was significant improvement on all nine measures at the 15-month follow up: The multivariate effect was significant (Wilk's Lambda =.11, p<.0005) as were all nine of the univariate effects (all p <.0005). Second, in order to assess whether the improvement shown immediately following EMDR treatment had been maintained over the following year the immediate posttreatment scores were compared with, the 15-month follow-up scores. The multivariate test was nonsignificant (Wilk's lambda=.74, p=.079), indicating the improvement shown immediately following EMDR was maintained 15 months later. The univariate analyses indicated additional improvement for the PTSD symptoms of intrusions (IES Intrusion: F(1,56)=7.71, p=307) and avoidance (IES avoidance: F_(1,56) -4.44, p=.040). None of the nine measures showed deterioration at the 15-month follow up. Prior to EMDR treatment 45% (g= 9) of the responders had been diagnosed as PTSD, at the 15-month follow up only 7% (g = 4) were diagnosed as PTSD (chi-squareo, N=61)= .72, p < .05). III. Responders Versus Nonresponders at the 15-Month Follow up.: At the time of writing this abstract, 75% of the participants (g=61) have responded to the 15-month follow up. In general, measures taken prior to treatment did not differentiate responders fiom nonresponders. Responding at the 15-month follow up was unrelated to age, gender, marital status or years of education, although the annual income of the responders (Mdn=21,500) was higher than that of the nonresponders (Mdn = 14,750, Mann-Whitney U=372.5, p=.017). Responding or not at 15 months was unrelated to the type of trauma experienced, whether or not the participants had been in therapy prior to EMDR treatment, or how long ago the trauma had occurred. It was also unrelated to the severity of the trauma as measured by the pretreatment scores on the nine dependent variables and to whether or not the participant met the PTSD diagnosis criteria prior to treatment. A multiple regression analysis used the immediate posttreatment and 90-day posttreatment scores to predict whether or not the participant responded at the 15-month follow up. Nonrespondents were more likely to be depressed at 90-days following treatment than were respondents (R square=.O8, B=-.16, Beta = -.28, F_L1,71)=5.99, p=.017). No other variables entered into the regression model. IV Discussiona and Conclusion, Tretement effects found immediately following EMDR treatment wer maintained or improved 15 months later and thee was a significant decrease in the number of participants diagnosed as PTSD at the 15 month follow up. The comparison of responders to nonresponders at the 15 month follow up showed that the nonresponders were more depressed than the responders, raising the possiblity that the present results may be favorably biased to some extent. The discussion will include the additional, subjective impressions of participants who did not respond to the follow up. Limitations of EMDR with this population will be discussed, including the influence of comorbidity, multiple traumas, retraumatization after treatment, and spontaneous recurrence of symptoms. V. Clients with complex PTSD and dissociation present many challenges. The neurosciences have helped us to begin to understand and deal with them through a recent clarification of mirror neurons and associated neural structures in both the clinician and client. Concepts from the Boston Change Process Study Group and ego state therapy provide methods of analyzing and intervening in the “microprocesses” that occur in treatment. Hoppenwasser’s concept of “dissociative attunement” challenges thinking about the “multiple self states” both clinician and client operate from. Her ideas push us to rethink current conceptualizations of relatedness. Participants will learn how to deal with ruptures in positive empathy that may result in the history taking, assessment, and desensitization phases. In the preparation phase, participants learn to use the therapeutic relationship as an additional resource for containment. Concepts of dyadic regulation of affect, now moments and moments of meeting will be taught to deal with ruptures to the therapeutic relationship throughout treatment. Dealing productively with countertransference ruptures poses additional challenges. Participants will learn a strategy called the “relational interweave”. Its function is to restore EMDR processing when an interpersonal “event” has temporarily derailed the work. A practicum using Dworkin’s Clinician Self Awareness Questionnaire will be held in the afternoon part of the workshop to enhance learning this strategy.

Abstract:
In treating clients with Anorexia Nervosa, what does appear to be consistent is the general view (e.g. Waller; Treasure; Brewer) that recovery will take on average between 1 to 2 years with many clients suffering from this disorder never making a full or complete recovery. 16 clients with a previous diagnosis of anorexia nervosa and under medical supervision with a BMI of between 16.5 � 18 and still meeting criteria of Anorexia Nervosa (DSM IV) were offered either EMDR (N.10) or CBT (N. 6). No therapeutic emphasis was placed directly on food in either the CBT or the EMDR group (e.g. Fairburn). Those receiving EMDR recovered substantially quicker once the target memory precipitating the Anorexic Onset was identified. Interestingly, food intake and weight increased without needing to be targeted as a separate matter, once the �feeder-memory� had been adapted in 8 out of the 10 EMDR clients. Whilst the CBT group did improve, they continued to fluctuate in terms of active anorexic patterns of behaviour and their weight or food intake did not appear to improve significantly. Although this is only a preliminary study, the EMDR group reached �recovery� at around the 3 � 4 month mark, compared to the CBT group, who still reported phobic reactions to both body-image and food at the 4 month mark. At the initial 1- month follow-up post-study, the CBT group were still exhibiting symptoms of mental pre-occupation with diet / weight compared to the EMDR group, who had reached normal BMI ranges (N. 9) and all reported little to no pre-occupation with diet / weight. It would appear, on the basis of this preliminary study that the precipitators to the development of Anorexia respond well to EMDR treatment targeting the precipitant memory. Furthermore, 6 of the EMDR group reported using the safe-space imagery to reduce their anxieties surrounding food and this may have been a contributory factor in their recovery.

According to the latest statistical evidence Post-Partum Depression develops in approximately 13% of women during the second -third month after childbirth with symptoms lasting between few weeks and a year and risks of relapse. Unlike the Baby Blues (affecting 70% of mothers, with onset in the 3'd - 6" day after delivery and spontaneous recovery within approximately two weeks), likely to be caused basically by hormone modifications in the immediate aftermath of childbirth. PPD development would seem to be solely determined by psychological factors: the experience of childbirth, the surfacing of unresolved problems in the relationships with attachment figures, the change in the woman's role both in the social sphere and within the couple relationship, the fear of being unable to adequately attend to the new responsibilities (both in terms of skills and of the ability to cope with the additional workioad), etc. Consequently, women experiencing childbirth as a traumatic experience are more destabilized by the event, and therefore. at a higher risk of developing PPD. Childbirth requires the deployment of many personal resources. A woman in labor must be able to bear the pain, while having to "push", 1.e. contrast the automatic antalgic reaction (which would close the delivery channel) and "meeting the pain", during the "expulsion" phase. Considering that "Peak Performances" require moving out of a person's comfort zone and stretching a person's boundaries, childbirth experience can be rightfully considered a "Peak Performance". This work describes RDI application times and modes during Delivery Preparation in order to strengthen the different personal resources needed by pregnant women to experience her childbirth as an ego syntonic experience. In this sense, RDI associated with EMDR can be considered an actual Primary Prevention intervention, capable of teaching women something positive about themselves, thus effectively offsetting the onset of PPD. Furthermore the results of the application of this technique collected during the Post-Partum phase on 48 women will be discussed. Learning objectives: 1 identification of the specific issues predisposing the development of PTSD due to Childbirth and of Post-Partum Depression. 2. I had only three weeks to work. Only 15 days to restore self-confidence, serenity, and determination for a woman rider. One week during the competition.
The woman rider had a bad experience during the Olympic Games of Athens. She had the best and the worst during the Olympic Games, The best was a bronze model and 0 mistakes during the jump.
The worst, she made several mistakes and fell down with her horse.
I felt during the first meeting a big atmosphere of fragility (no self confidence, no serenity, big stress) because there were difficulties with her ex-husband and family (her boys).
We had a short time and I decided to use an arrangement with mental imagery and specific EMDR exercise, like butterfly exercise, to install peak performance.
I used Sam Foster’s protocol for the sport and butterfly technique. We began with a SUD=8 and VOC=6 and the SUD finished at 2. This combination gave an amazing result because she rode well and she had a good result in these world equestrian games of AAchen (semi final: 23 place).
The specific exercise in EMDR (butterfly exercise) permits a peal performance in a few times. I gave you an example about the power of EMDR in sport.

BPD patients present difficulties with self-regulation and relating to others. The management of these difficulties is central to the treatment of BPD. Working with cases of BPD and complex trauma is intrinsically relational, often involving the need to manage moments of intense affect and affect phobias in the transference and countertransference. It has proven that EMDR is successful in helping people who have experienced psychological difficulties that originate from some kind of traumatic experience. I used standard EMDR protocol successfully in single session for two persons who experience short negative moments. One client was a young man who had intrusive recalls of the male acquaintance telling him about oral sex intercourse between men that happened 38 hours before he asked for psychological help. The other client was a young nurse, one of my colleagues, who was scolded and threatened by the husband of a patient less than 2 hours ago before I did EMDR for her. They both had moment of trauma was very precise and very short: the moment the words were spoken. The recent events don't have several hot spots (difficult affect laden moments in the experience), but just one clearly shocking moment. So I used standard protocol and it helped stabilizing them very quickly, they both retouched their resources quickly. Two months later, I followed up my colleague. She was still stable and had the same positive cognition about the negative moment.

Zodra de trein reed, ging het wel, maar bij elk station, als ze de mogelijkheid had om uit te stappen, kwam de angst weer terug. In juli kreeg ze opnieuw een aanval, ditmaal tijdens haar vakantie, anticiperend op de vliegreis terug naar huis. Sindsdien was ze constant bang voor nieuwe aanvallen. Vlak voor het eerste gesprek heeft ze voor het eerst ook's nacht in bed een aanval gekregen. Treinreizen en lange autoritten werden sindsdien zo veel mogelijk vermeden. Doriene in August 2007 was signed by the physician associated with panic attacks. Doriene said in the interview that she feels that her anxiety about falling. During a panic attack while they are cold and hot, she gets palpitations, she feels a knot in the stomach and tingling in the arms. At such times she is afraid of losing control and going crazy. In May 2007 she had an attack on the train. Once the train was, it went well, but at each station, when she had the opportunity to step out, the fear came back. In July she was again attacked, this time during her vacation, anticipating the flight back home. Since then, she was constantly afraid of new attacks. Just before the first meeting for the first time she's in bed a night attack received. Train travel and long car trips have since been avoided wherever possible. Doriene said she feels overworked lately. She could still have difficulty concentrating and poor sleep at night. She said many are concerned about her mother from chronic bronchitis and emphysema (COPD) suffering and probably not have long to live. Doriene show in 1998 before a brief period of panic attacks had. Previously in 1994 and 1995, a period she is severely depressed. Both depression and panic disorder at the time by means of weekly meetings and taking an antidepressant treated successfully.

One major limitation of EMDR is that it is an individual treatment modality. However, individual treatment of a child may be insuflicient, and broader interventions are often required. This is especially likely when environmental forces are unsupportive or in opposition to the healing process. This presentation will focus on the use of EMDR on several levels in child treatment, including individual treatment of traumatic memories, enhancing family support for healing, and addressing family obstacles to healing. Format will include lecture, vignettes, and a video case presentation of the EMDR treatment of a young girl who had been raped by a babysitter, along with the EMDR treatment of her older brother who bullied her. Assessment of child problems includes consideration of many factors. The focus here will be on the child's trauma history, and on the current family situation it pertains to treatment of the child's traumatic memories. Methods of assessment addressed here include interview of the child and the parent, observation of family interactions, and observation of the child's progress, both during and after EMDR treatment. A number of interventions are available to enhance or augment individual EMDR treatment of the child. Vignettes will be presented to illustrate the following interventions: referral to family therapy when successful EMDR highlighted the symptom's functional role; EMDR with a parent to reduce reactivity to the child; and prompting the parent(s) to produce statements and behaviors to be used later as content for installations. A challenging case will be presented in which a family, though motivated, demonstrated a number of behaviors which threatened to undermine the child's treatment. The family consisted of a single father in his late twenties, an eight year-old boy, and a seven-yearold girl. The presenting problem was the girl's ongoing post-traumatic symptoms, particularly nightmares and social withdrawal, some two years after having been raped by a babysitter. (the boy also had social and behavioral problems in school.) Unfortunately, the "lessons" of the girl's traumatic experience were frequently reinforced in the family context, through the brother's bullying of his younger sister, the father's complicity in the bullying, and the father's own tendency to be overly controlling and threatening. Treatment began with two family sessions and one with the father alone. The next three sessions were split to provide some individual time for each child as well as for the father. The seventh, final session included a family meeting and then some time for each individual. Work with the father was difficult and slow, as he was very defensive regarding his own possible contributions to his children's problems. Early interventions included delicate attempts to help the father understand the effect of his yelling and threatening - even though he was no longer in the habit of physically striking his children. Meanwhile, in part to enhance the therapeutic alliance, the primary focus was on direct treatment of the children. Some of this is shown on video. The girl was asked to draw a picture of her bad dream, and then to draw it "all better." She first drew a dark picture of a large man with fangs dripping blood. The next picture was of a nicer man on a sunny day. This activity was used as part of her introduction to the upcoming EMDR work. In the next session she agreed to do EMDR and completed processing in 25 minutes. The following session she indicated that the memory was no longer disturbing, and many of the symptoms had disappeared. She began to raise her next concern, by playing with a doll and a baby bottle, and complaining that she did not get to see her mother enough. Over the same three sessions the boy was also treated with EMDR for a number of relatively minor traumatic memories, including a car accident, the loss of two pets, and a vision of the devil. Despite apparently successfull processing, he was unable to conclude that he was a "good boy," due to evidence to the contrary: memories of his father's anger at him. Cognitive interweave was used to access a sense of inner goodness. The bullying behavior reportedly disappeared both at home and at school, and he also moved on, to express concerns about missing his mother. Treatment was interrupted due to a change in insurance coverage, so continued treatment and follow-up was not accomplished. This case illustrates some ways that EMDR can be enlisted to address aspects of the family context which may constitute obstacles to healing. The girl's brother was treated with EMDR to reduce his mistreatment of her; and the boy's sense of badness, largely gained by interaction with his father, was overcome by accessing internal resources in the absence of parental support. In conclusion, EMDR can play multiple roles in both the diagnosis and treatment of family obstacles to healing.

There is increasing evidence to support the biological basis of mental disorders. Subsequently, understanding the neurobiological context from which mental distress arises can help counselors appropriately apply cognitive behavioral therapy and other well-researched cognitive interventions. The purpose of this article is to describe the neurobiological context underlying the formation and treatment of posttraumatic stress disorders, a mental disorder frequently encountered by counselors, from a cognitive therapy framework.
Recent changes to the Council for Accreditation of Counseling and Related Educational Programs (2009) accreditation standards include the need for counselors-in-training to understand the neurobiological basis of behavior, which marks a new direction for the training of professional counselors who have historically reacted ambivalently toward medical models for understanding client concerns and treatments. Yet recent findings in neuroscience actually support the verbally based interventions that counselors typically use in treatment; therefore, there is much to be gained by counselors and counselor educators in understanding the basics of human neurobiology and how commonly used counseling interventions intervene on these biological systems. The National Institute of Mental Health (2010) stated in a recent strategic plan that “Important discoveries in areas such as genetics, neuroscience, and behavioral science largely account for the substantial gains in knowledge that have helped us to understand the complexities of mental illnesses and behavioral disorders over the past 15 years” (“Introduction,” para. 4).
Given the increasingly biological focus of mental health research, the practicing counselor is faced with the task of understanding and using the emerging mental health treatments and explaining to clients, to reimbursing agencies, and to the broader public how counseling fits within the medically dominated mental health culture. Some counselors have long reacted ambivalently toward the pathologically oriented diagnostic categories of the Diagnostic and Statistical Manual of Mental Disorders (4th ed.; DSM-IV;American Psychiatric Association, 1994) system and the medication-dominated world of psychiatry. For example, the contrasting viewpoints on this issue were published in the Journal of Counseling & Development between Allen and Mary Ivey (1998, 1999) and Scott Hinkle (1999). Ivey and Ivey (1998) argued for a developmental interpretation to the DSM-IV, opposing what they called the “pathological view” (p. 334) of the manual. According to Ivey and Ivey, disorders could be viewed through a positive development tradition to lie not within the individual but within the contextual systems in which a person lives. Subsequently, disorders are viewed as a “logical response to a developmental history” (Ivey & Ivey, 1999, p. 484). By contrast, Hinkle (1999) argued that because anxiety and depressive disorders “are the most common clinical symptoms associated with presentation to counseling” (p. 475), the counseling profession is weakened if counselors shy away from direct participation in the DSM nomenclature and treatment parlance. As Hinkle indicated, “mental disorders according to the medical model describe disease processes, not people” (p. 475). Regardless of the reader's philosophical perspective, practicing counselors know participation in medical and psychiatric systems is necessary at times. Also, recent discoveries in the field of neuroscience are providing evidence that interventions often used by counselors have direct physiological impact on client neurobiology (Kennedy et al., 2007; Linden, 2006). For example, Felmingham et al. (2007) demonstrated significant differences in brain activity before and after 8 weeks of exposure therapy, which correlated with a reduction in posttraumatic stress disorder (PTSD) symptom severity. Similarly, Paquette et al. (2003) found that cognitive behavioral therapy (CBT) alters the activation and metabolism of specific brain regions following successful treatment of spider phobia. These findings, along with others (for a detailed review, see Beauregard, 2007; Frewen, Dozois, & Lanius, 2008), are significant because they support the techniques, interventions, and approaches used by counselors and provide a mechanism by which counseling positively affects brain physiology. Within the emerging physiologically based treatment milieu, counselors should be prepared to articulate how cognitive counseling interventions make measurable changes to the client. Although cognitive-behavioral-based approaches are effective in the treatment of a number of psychiatric illnesses, adult PTSD is arguably one of the best understood mental disorders from a neurological perspective. It thus presents a valuable model for exploring not only the basic tenets of neurobiology but also the mechanisms behind its successful treatment. Furthermore, PTSD is a disorder that counselors will likely encounter in practice.
PTSD is a mental disorder characterized by a sudden onset of symptoms due to environmental exposure to a psychologically stressful event such as war, natural disaster, or sexual victimization. Thus, it provides a clear example of how, even in adulthood, neurological adaptation (in this case maladaptive changes) can functionally “rewire” the brain in a short period of time, resulting in a sustained array of clinical symptoms. The diagnostic criteria for PTSD are a history of exposure to a traumatic event meeting two criteria and symptoms from each of three symptom clusters: intrusive recollections, avoidant/numbing symptoms, and hyperarousal symptoms. A fifth criterion concerns duration of symptoms and a sixth assesses functioning (American Psychiatric Association, 2000).
The National Comorbidity Survey Replication, conducted between February 2001 and April 2003 (Kessler et al., 2005), determined that the estimated lifetime prevalence of PTSD among American adults is 6.8%, with women (9.7%) twice as likely as men (3.6%) to have the disorder at some point in their lives. These findings are very similar to those of the first National Comorbidity Survey conducted in the early 1990s (Kessler, Sonnega, Bromet, Hughes, & Nelson, 1995), which was composed of interviews of a representative national sample of 8,098 Americans ages 15 to 54 years. In this earlier sample, the estimated prevalence of lifetime PTSD was 7.8% in the general population. As in the more recent survey, women (10.4%) were more than twice as likely as men (5%) to have PTSD at some point in their lives (Kessler et al., 2005; Kessler et al., 1995).

The relationship between patient and therapist is unique in important ways when compared to relationships between clients and other professionals such as physicians, dentists, attorneys, and accountants. The key difference is present from first contact: it is not clearly understood exactly what will transpire. There is no other professional relationship in which consumers are more in the dark than when they first go to see a therapist.
In other fields, the public is fairly well informed about what the professional does. Tradition, the media, and general experience have provided consumers with a baseline by which to judge what transpires. If you break your arm, the orthopedist explains she will take an X ray and set the bone; she tells you something about how long the healing will take if all goes well and gives you an estimate of the cost. When you go to a dentist, you expect him to look at your teeth, take a history, explain what was noted, and recommend a course of treatment with an estimate of time and cost. Your accountant will focus on bookkeeping, tax reports, and finances, and help you deal with regulatory agencies. Consumers enter these relationships expecting that the training, expertise, and ethical obligations of the professional will keep the client's best interests foremost. Both the consumer and the professional are aware of each person's role, and it is generally expected that the professional will stick to doing what he or she is trained to do. The consumer does not expect his accountant to lure him into accepting a new cosmology of how the world works or to "channel" financial information from "entities" who lived thousands of years ago; or for his dentist to induce him to believe that the status of his teeth was affected by an extraterrestrial experimenting on him. Nor does the patient expect the orthopedist to lead him to think the reason he fell and broke his arm was because he was under the influence of a secret satanic cult. But seeing a therapist is a far different situation for the consumer. In the field of psychotherapy there is no relatively agreed upon body of knowledge, no standard procedures that a client can expect. There are no national regulatory bodies, and not every state has governing boards or licensing agencies. There are many types and levels of practitioners. Often the client knows little or nothing at all about what type of therapy a particular therapist "believes in" or what the therapist is really going to be doing in the relationship with the client. In meeting a therapist for the first time, most consumers are almost as blind as a bat about what will transpire between the two of them. At most, they might think they will probably talk to the therapist and perhaps get some feedback or suggestions for treatment. What clients might not be aware of is the gamut of training, the idiosyncratic notions, and the odd practices that they may be exposed to by certain practitioners. Consumers are a vulnerable and trusting lot. And because of the special, unpredictable nature of the therapeutic relationship, it is easy for them to be taken advantage of. This makes it all the more incumbent on therapists to be especially ethical and aware of the power their role carries in our society. The misuse and abuse of power is one of the central factors in what goes wrong. Questions to Ask Your Prospective Therapist Ultimately, a therapist is a service provider who sells a service. A prospective client should feel free to ask enough questions to be able to make an informed decision about whether to hire a particular therapist. We have provided a general list of questions to ask a prospective therapist, but feel free to ask whatever you need to know in order to make a proper evaluation. Consider interviewing several therapists before settling on one, just as you might in purchasing any product. Draw up your list of questions before phoning or going in for your first appointment. We recommend that you ask these questions in a phone interview first, so that you can weed out unlikely candidates and save yourself the time and expense of initial visits that don't go anywhere. If during the process a therapist continues to ask you, "Why do you ask?" or acts as though your questioning reflects some defect in you, think carefully before signing up. Those types of responses will tell you a lot about the entire attitude this person will express toward you - that is, that you are one down and he is one up, and that furthermore you are quaint to even ask the "great one" to explain himself. If you are treated with disdain for asking about what you are buying, think ahead: how could this person lead you to feel better, plan better, or have more self-esteem if he begins by putting you down for being an alert consumer? Remember, you may be feeling bad and even desperate, but there are thousands of mental health professionals, so if this one is not right, keep on phoning and searching.

Following EMDR training, clinicians experience a steep learning curve when they attempt to implement the protocol with clients. Anecdotal evidence points to a significant reduction in numbers of EMDR trained clinicians actually utilizing it correctly with clients on a regular basis. Anecdotal evidence points to a significant reduction in numbers of EMDR trained clinicians actually utilizing it correctly with clients on a regular basis. EMDR is much more complex than it first appears; therefore those who use the protocol only occasionally or loosely will miss significant opportunities to grasp the subtleties and effective application. There is a need, even for experienced EMDR clinicians, to learn and apply a structured way of identifying the decisions we make both before and as we proceed through the protocol. A growing body of researcs exists to back up our decisions; however, some choices are made based on intuition and clinical judgment. Topics will include a self-evaluation (passions and expertise); why we sharpen our phone skills up-front; rationales for tightening up intake and case conceptualization; decisions during preparation, safe place, resource development, assessment, desensitization, and later phases of the protocol; decisions regarding cognitive interweaves; and decisions regarding time management and flexing a treatment plan. Participants will be encouraged to sharpen their thinking about how they do EMDR with their clients, by means of identifying the most important "forks in the road" before and during treatment; be able to name at least one decision point pertaining to each of the Phases covered; and finally, to provide a rationale for the choices they do make at these critical moments. 8

Foram realizadas 20 sessões de EMDR e as experiências traumáticas da relação mãe x filha, bem como os medos de repetir a história passada, foram processados a uma resolução adaptativa, que permitiram a assimilação de suas lembranças passadas e a incorporação de padrões para experiências positivas, possibilitando a oportunidade de ampliar conexões com redes positivas. As memórias armazenadas que serviam de base para percepção, atitudes e comportamentos foram reprocessadas e, com isso, os sintomas físicos foram extintos, de maneira a permitir que a paciente assumisse uma nova postura frente à maternidade. A percepção sobre sua mãe foi alterada e conseguiu trazer à tona bons momentos de sua infância com a mãe. Também foi reprocessada uma situação relacionada ao perdão mútuo. A paciente hoje vive o processo de maternidade forma saudável e feliz, e 04 meses após o encerramento das sessões, adotou um menino de 03 meses de vida, sem que os sintomas reaparecessem.

This paper aims to present a clinical case of a patient of 46 years in the adjustment period from the adoption of a child of five. Difficulties were initially related: the linking maternal affection, exercise and physical care of the child with affection, exercise and perception of the maternal role, coupled with strong somatic symptoms. With the existence of a great desire to be a mother 15 years ago was receiving treatment for getting pregnant. The adoption did not represent a taboo for the same. However, childhood came to the fore and often represented a large part of his suffering: his mother was 46 years old when his birth and has since been stricken with illness throughout his life, preventing the achievement of physical and emotional care to her. Were performed 20 sessions of EMDR and traumatic experiences of the mother x daughter, as well as fears of repeating past history, were processed to an adaptive resolution, which allowed the assimilation of their past memories and incorporation of standards for positive experiences, allowing the opportunity to expand connections with positive networks. The stored memories that served as the basis for perception, attitudes and behaviors were reprocessed and, therefore, the physical symptoms were abolished, so as to allow the patient to assume a new stance facing the motherhood. The perception has changed about his mother and managed to bring out good moments of his childhood with his mother. It was also reprocessed a situation related to mutual forgiveness. The patient now lives the process of motherhood healthy and happy, and 04 months after the close of the session, adopted a boy of 03 months, without symptoms reappeared.

This article describes the use of the Developmental Needs Meeting Strategy (DNMS) for the treatment of dissociative identity disorder (DID). The DNMS is an ego state therapy which guides a client’s own internal resources to meet developmental needs that were not met in childhood. After 17 months of DNMS treatment a client with DID reported a near total elimination in frequency and severity of symptoms of depression, anxiety and suicidal thoughts, her Trauma Symptom Inventory scores indicated no trauma symptoms, and her Multidimensional Inventory of Dissociation scores indicated she no longer meets the diagnostic criteria for DID. She was functioning well without any medication. Further research is warranted.

The UK National Institute for Clinical Excellence Guidelines on PTSD: Based on rigorous meta-analysis, detailed literature review and two public consultations, the national guidelines for the assessment, prevention and treatment of PTSD in the UK will be presented in this symposium on behalf of the Guideline Development Group.

Drug treatments in the management of PTSD: Drug treatments are increasingly used in the management of PTSD and have been supported by a number of recent reviews although there have been increasing concerns about their safety. Recent guidance has been issued in the UK by the Regulatory Agency concerning the use of SSRI drugs in people of all ages, but especially in children and young adults. In a meta-analysis of the clinical trial literature for PTSD, following a rigorous bibliographic search, 26 RCTs meeting strict criteria were identified. Data on two hitherto unpublished trials of sertraline were available for inclusion. This review highlights the methodological issues involved in comparing drug trials and trials of psychological therapies. Nonetheless, meta-analysis demonstrates that drug treatments for PTSD are probably more disappointing than hitherto believed. Only five drugs emerged with recommendations for use. One of these has a reliable if modest effect (paroxetine). The other four are included on the basis of small single trial reports. Although there are clear indications for drug treatments, in general there should be an increasing emphasis on ensuring that trauma-focused psychological treatments (CBT and EMDR) are widely available, delivered by competent practitioners, and easily accessible in a timely fashion in primary and secondary care settings.

A critical incident is any situation that causes unusually strong emotional reactions that have the potential to interfere with a person's ability to function immediately after the incident or later. These are situations that overwhelm a person's sense of vulnerability and/or control. A critical incident stress debriefing (CISD) is a psychoeducational group meeting or discussion about a traumatic incident which ideally takes place within 72 hours of the event. The goals of a CISD are to mitigate the psychological impact of a traumatic event, prevent subsequent development of a post-traumatic syndrome, accelerate recovery, and serve as an early identification mechanism for people who need further follow-up, including EMDR. The steps of a CISD include: 1) introduction - to introduce the intervention team, explain the process, and set expectations. 2) fact - to describe the event from each participant's perspective on a cognitive level. 3) thought - to allow participants to describe cognitive reactions and to transition to emotional reactions. 4) reaction - to identify the most traumatic aspect of the event for participants. 5) symptom - to identify personal symptoms of distress and transition back to the cognitive level. 6) teaching - to educate as to normal reactions and adaptive coping strategies 7) reentry - to clarefy ambiguities and prepare for termination; access for follow-up. In the opinion of the authors, the CISD facilitates the processing of the traumatic information before it becomes crystallized in dysfunctional form. EMDR can be very effective shortly following a CISD, and is particularly usehl for participants who are experiencing distress or intrusive symptoms after the CISD. The CISD structure helps the participant understand the traumatic impact of the incident and provides support and guidance toward adaptive resolution. The EMDR process begins where the CISD leaves off. The CISD helps to delineate the traumatic image, negative cognition, and emotions associated with the event, making the subsequent EMDR process more efficient. EMDR appears to have a very powerful and rapid effect after the CISD, perhaps, because of the initial processing. In other words, the CISD initiates an adaptive processing of the traumatic information; EMDR completes it. EMDR can be implemented individually immediately following the CISD, or the next day. While the CISD is a group process, EMDR is an individual method. EMDR can be explained during the teaching phase of the CISD or after the CISD to the whole group, but EMDR treatment is done individually and privately. EMDR can go beyond a CISD in targeting previous traumas that may underlie the current incident, delve deeper into the meaning of the incident for the person, and target specific stimuli that are relevant to the individual (e.g. Smells, tastes, etc.). The workshop will discuss the application of EMDR to critical incidents. The protocol for recent events will be reviewed. Guidelines for negative and positive cognitions will be discussed. Currently migration has reached extensive proportions. Globally, we assume a hundred to two hundred million migrants (WHO). Due to civil wars, natural disasters, political and economic circumstances the true figure is likely even higher. So it increasingly occurs that psychiatrists and psychotherapists encounter in their treatment individuals from other cultures, presenting with various psychiatric symptoms. The treatment of these people will sometimes face major difficulties. Not only because of language but also because of the relative importance of certain symptoms in a cultural context, it is essential to consider culture background, working milieu and disorder specific psychiatric/psychotherapy. But what does cultural and milieu sensitive psychiatry or psychotherapy mean? Based on the existing studies about people with immigrant backgrounds and the investigations of the Socio-Vision-Institute, and our own experiences we will elucidate how an intercultural meeting and a milieu sensitive psychotherapy can be successfully applied.

The present study is carried out within a psychological counseling project which is governed by Istanbul Metropolitan Municipality. The study was designed to assess the effects of EMDR Integrated Group Treatment Protocol (EMDR-ICTP: on the child victims of a terrorist bombing in Gun Goren, Istanbul. In this study the EMDR-IGTP was used with 60 children who are in 5th, 3rd and 4th grade and effected by the event (exposed, witnessed or their relatives harmed, etc.) with high scores on the Turkish version of 'Impact of Event Scale' (IES). Before the group session. Child Report of Post-Traumatic Symptoms (CROPS) and a survey asking 5 stress-related symptoms of their lives after the bomb attack were applied. The group sessions consist of: meeting with children. explaining trauma, psychological debriefing, EMDR (Installing the safe place, assessment, drawing the first picture, drawing the second and the third picture, tapping, relaxation), healing story and art therapy. Participants completed another survey about what they have realized during and after their group experience. The study is completed with a re-evaluation of one and three months follow-up. Analyses of the data collected from the participants will demonstrate the effectiveness of EMDR in group setting with children for reducing traumatic symptoms and developing new resources to handle problems. In this study the effectiveness of EMDR is examined according to the difference between the ages of children, and the level of exposure (witnessing, watching on TV etc.) to the traumatic event.

In working with clients with anorexia nervosa for the past 18 months, it has become apparent that while many clinicians continue to utilise a variety of treatment means, CBT or systemic family therapy are often treatments most frequently offered to the sufferer. However, what does appear to be consistent is the general view (e.g., Waller; Treasure; Brewer) that ‘recovery’ will take an average between 1 to 2 years with many clients suffering from this disorder never making a full or complete recovery.
With this in mind, sixteen clients with a previous diagnosis of anorexia nervosa and under medical supervision with a BMI of 16.5 – 18 and still meeting the mental criteria of anorexia nervosa (DSM IV) were offered either EMDR (N.10) or CBT (N.6). Starting with the premise that most sufferers of anorexia nervosa argued that simply ‘eating’ is not the answer – but rather the mechanism by which control is maintained – it was agreed that regular weigh-ins and the use of food journals would form part of the study – however, no therapeutic emphasis would be placed directly on food in either the CBT or the EMDR group.
Those receiving EMDR recovered substantially quicker and with less food-related exposure, once the target memory precipitating the anorexic onset was identified. Interestingly, food intake and weight increased without needing to be targeted as a separate matter, once the ‘feeder’ memory had been adapted in 8 out of the 10 EMDR clients. In the remaining two clients, behavioural experiments targeting the “practice of behaving (and eating) normally’ one day a week in order to reduce the fear of ‘normality’ was used with good effect.
Whilst the CBT group did improve, they continued to fluctuate in terms of active anorexic patterns of behaviour and their weight or food intake did not appear to improve significantly.
Although the trial lasted for 6 months duration, on average, the EMDR group reached ‘recovery’ at around 3-4 month mark, compared to the CBT group of which five noticed ‘improvement,’ however, did not appear to have increased weight and still reported phobic reactions to both body-mage and food at the 4 month mark. One in the CBT group did make a significant improvement based on cognitive restructuring alone and this client did increase weight and intake significantly at the same ratio as the EMDR group.
At the initial 1-month follow-up post-study, the CBT group were still exhibiting symptoms of mental pre-occupation with diet/weight compared to the EMDR group who had reached normal BMI ranges (N.9) and all reported little to no pre-occupation with diet/eight.
Although this study is still pending follow-ups of all clients, there would appear, on the basis of this preliminary study that anorexia nervosa appears to respond better to EMDR in shorter treatment duration than CBT.
Furthermore, six of the EMDR group reposed using safe-space imagery to reduce their anxieties surrounding food and this may have been a contributory factor.

The development of eye movement desensitization and reprocessing (EMDR) has been hotly debated, with rhetoric often being posited as evidence either for or against the technique. This paper aims to provide a brief overview of the procedure, a critical review of the studies completed to date, a meta-analytic review of the available data, and a chronology of the evolution of EMDR over the past 10 years. Treatment-outcome studies were of such disparate quality-even studies meeting similar broad criteria-that combining their results in a meta-analysis was of very questionable value. Overall, an appraisal of the published research supported the following conclusions: (1) There is overwhelming evidence that eye movements are neither a necessary nor a useful addition to the procedure; (2) there is strong and consistent evidence that EMDR is better than no treatment, yet only as good as any other treatment that utilizes some aspect of exposure therapy; and (3) there is strong evidence that a full-exposure-based intervention package is superior to EMDR. There is also some evidence that "reprocessing" is likewise superfluous to EMDR and that the effects of EMDR dissipate over time. It is also concluded that the current debate cannot be entirely settled through scientific investigation due to the rapid and constant reshaping of what constitutes EMDR, the similarity to extant alternative methods, and the lack of a falsifiable theory underpinning the procedure. Episodes of significant depression have been a part of my life for as long as I can recall, but psychosis was unknown to me until I was in my mid-thirties, months after the birth of my second child. At first, all I recognized were the emerging symptoms of postpartum depression in the weeks after the birth: a familiar scenario, since it had also occurred with my first child. My doctor immediately prescribed 50mg of Prozac daily. I took the medication, felt much better, and continued to breastfeed my second daughter with no apparent problems. Algunos pacientes, sin embargo pueden tener dificultades para conectar su problema actual con acontecimientos del pasado. Así también, otros pacientes pueden tener dificultades para crear patrones positivos para el futuro, especialmente si ensayar conductas nuevas los pone ansiosos. Para estos problemas las técnicas de "Flotar hacia atrás" y "Flotar hacia delante" desarrolladas por William Zangwill Ph. D., entrenador del Instituto EMDR, son métodos efectivos para ligar el pasado, presente y futuro en un ámbito terapéutico y proveen al terapeuta de instrumentos para abordar eficientemente ambos temas. Durante el procesamiento, el SUD se redujo de 8 a 3 con bastante rapidez pero de ahí no bajaba. El paciente comentó que no bajaba porque esta "peor escena" podría sucederle realmente y le había sucedido en el pasado. Volviendo al target después de esto, le fue posible reducir el SUD a 1 con unos pocos sets de movimientos oculares. Al utilizar la Técnica de Flotar hacia delante para reprocesar la peor escena, el paciente tiene una oportunidad para resolver la ansiedad anticipatoria. A aplicar las Técnicas de Flotar hacia Atrás y hacia Delante y ocuparse así del pasado, presente y futuro, el terapeuta de EMDR puede sanar mejor a su paciente. Es más, las Técnicas de Flotar hacia Atrás y hacia Delante están basadas en EMDR. Its use is most appropriate when the clinician suspects that a disturbance that the patient is experiencing at present, is rooted in past experiences, especially when questions like "What is your earliest memory in relation to what you feel now? "When you see a picture of him / herself by ......, it's the worst that can happen?" Then follow with the rest of the standard questions, ie, CN, CP, VoC, emotions, SUD, and location of bodily sensation. Stimulate the processing of patients with eye movements or other bilateral stimulation. If the development of the patient's worst scene provokes a rational fear, you may have to take practical steps to address these concerns. For example, using the technique of floating forward with a boy of 13 who was in a temporary foster home, the worst scene evoked for him was: "I will return home if this adoption is not." During processing, the LDS was reduced from 8 to 3 fairly quickly but it does not down. The patient said he did not go down because the "worst scene" could really happen and had happened in the past. Eye movements stopped, we chatted a while and developed a plan for: a) a meeting with her adoptive parents to discuss the permanence of the adoption and b) a conference call to his legal adviser to clarify your rights and options. Returning to the target after that, it was possible to reduce the LDS-1 with a few sets of eye movements. Using Floating Technique forward to reprocess the worst scenario, the patient has an opportunity to resolve the anticipatory anxiety. During the installation of the positive cognition, the patient is creating positive patterns for future action. A woman whose brother was verbally abusive to her children and now intimidated, put together a "worst stage" with: "It will be equally unfair when I see him next time." The patient had done much EMDR reprocessing childhood incidents related to verbal abuse of his brother. However, without a positive reference experiential, still anxious every time I interacted with him. Asking him to "float forward" and using EMDR on one of the worst scenes, relieved her anxiety about a family party that was pending. Installing a CP of "I'm stronger now allowed him to create an image of herself driving her brother with humor and feeling safe. To apply the techniques to float back and forth and deal well past, present and future, the EMDR therapist can heal your patient better. Moreover, techniques to float back and forth are based on EMDR. Both incorporate the standard protocol questions and give the therapist and the patient the opportunity to be managed more smoothly with this protocol.

The rationale behind the creation of "The Four Elements Exercise for Stress Management" is to address the cumulative effect of external and internal triggers that occur over the course of the day. Since we know that people cope better with stress when they stay within their arousal "window of tolerance," ways to lower stress—especially when under stress—are essential. The heart of the exercise consists of four, brief, self-calming and self-control activities. The sequence of the four elements—Earth-Air-Water-Fire—is designed to follow the body up from the feet to the stomach and chest, to the throat and mouth, and up through the head. It begins with the ground to signify safety in the present reality and moves up to the imagination of recalled safety. By checking in with stress levels at random times throughout the day and also when stressful events are occurring, the exercise can aid in preventing the accumulation of stress and enables clients to stay within their window of tolerance. The modest goal is to reduce the stress level by 1 or 2 units each time the exercise is performed. The original conceptualization of the Four Elements Exercise was that the first three elements could be a preparation for the Safe Place (or other resource exercise such as the Resource Connection), especially when there is an ongoing emergency situation or when it is difficult to find a Safe Place. Often, the fourth element is introduced at the following meeting, as the first three elements are enough to remember and practice in the beginning for clients. Working on the Safe Place separately during the following session gives it more space and impact. It is advisable to follow up on how the client practiced the four elements at the beginning of the next session and to ask them to show you how they do it. If necessary, demonstrate it again at the beginning of the first few sessions. The EMDR, with its technical "Butterfly" is a very Valuable to be used as basis by both patients and between-sessions less firm as after completing the sessions themselves.
Tension artiele too high (over 140 outraged 75/80) because of the risks (heart attack, heart tightness, myocardial infarction, stroke ...). There are a number of people who tend to see a high blood pressure because of Nevrotic and / or ill-tress spared. Here the "butterfly" makes a very good coping: artiele high voltage may be reduced without medication when they act "simply" stress (névrotiqies reactions in general or because Traumatism).
The presentation was in my travel of staff (illustrated with several examples) and I férvier have already presented in 2006 at the annual meeting al del Company EMDR Austria.

Since 2001, the wars in Iraq and Afghanistan have caused considerable strain on military medicine to effectively manage the large and growing mental health demand from deployed personnel. Current trends, initiatives and on-going barriers in meeting war-related mental health needs for this and future war generations as reported by military officials, including the 2007 Department of Defense’s (DoD) Task Force on Mental Health, is reviewed including training of providers, access to high quality mental health assessment and treatments and research innovations. Lastly, a model for a 21st century modern military mental health care system is proposed within the context of historical and present-day analysis of the cyclical impact of dualistic approaches toward mental and physical health and consequent effects of mental health stigma and disparity. This article is a Modification of a talk at the Swedenborg Scientific Association Annual Meeting in Bryn Athyn on 5 May 2001.
In June 1998 I learned of a technique called Eye Movement Desensitization and Reprocessing (EMDR), for treating painful memories. It basically seems to involve stimulating the patient’s brain bilaterally, while the patient holds the memory or memories in their mind. Originally, Francine Shapiro,1 the discoverer and developer of the technique, used saccadic (sideways movement, to and fro) of the eyes. Subsequently it has been found that bilateral alternating ear sound, or tapping the hands alternately, or even the feet in desperate situations, helps to transform the nature of the memory in various positive ways. Occasionally nothing happens, but usually the memory fades, becomes laughable, of no importance, and with or without a host of other reactions. There are temporary unpleasant reactions on the way to resolution, but if dealt with skillfully, the result is usually positive. Francine Shapiro stresses the importance of getting proper training before practicing EMDR, as these reactions can be quite frightening to someone not used to dealing with them. But the good results are often astonishing. A fresh way of looking at the problem, impossible before, becomes attainable. Though memories be as scarlet they become whiter than snow to use the familiar words from Isaiah.

Mensen met een borderline-stoornis en een geschiedenis complex trauma hebben veel problemen met zelfregulering en met betrekking tot anderen. Het beheer van deze zelfregulering en relationele problemen zijn centrale aspecten in de behandeling van BPS. De stabilisatiefase is opgemerkt als essentieel oor trauma werk. Bij de behandeling van de borderline-stoornis en complexe trauma betekent dit vele bijzonderheden die we moeten in gedachten houden, waaronder: de rol van gehechtheid-gerelateerde gemoedstoestanden en fobieën voor de bevestiging, beïnvloeden en traumatische herinneringen. Werken met gevallen van BPS en complex trauma is intrinsiek relationeel en vaak gepaard gaat met de noodzaak om momenten van intense beïnvloeden en invloed hebben op fobieën beheren in de overdracht en tegenoverdracht. Inzicht in deze aspecten en met strategieën voor het aanpakken van hen is van essentieel belang zowel voor als tijdens EMDR opwerking van traumatische herinneringen om ervoor te zorgen dat de verwerking van traumatische herinneringen veilig en effectief kan worden gedaan met deze patiënten. Deze workshop integreert theoretische uiteenzetting met de presentatie van video's gevallen. De algemene structuur van EMDR therapie bij de behandeling van BPD, interventies in de voorbereidings-en overwegingen voor trauma-gerichte EMDR werk zal worden gedemonstreerd en uitgelegd.

Insecure and disorganized attachments and early relational neglect and trauma profoundly affect the developmental trajectory of the future adult and increase the risk of developing Borderline Personality Disorder (BPD). People with BPD and a history complex trauma have many difficulties with self-regulation and relating to others. The management of these self-regulation and relational difficulties are central aspects in the treatment of BPD. The stabilization phase has been remarked as essential prior to trauma work. In treating BPD and complex trauma this implies many particularities that we should keep in mind including: the role of attachment-related states of mind and phobias for attachment, affect and traumatic memories. Working with cases of BPD and complex trauma is intrinsically relational and often involves the need to manage moments of intense affect and affect phobias in the transference and countertransference. Understanding these aspects and having strategies for addressing them is essential both before and during EMDR reprocessing of traumatic memories to ensure that reprocessing of traumatic memories can be done safely and effectively with these patients. This workshop integrates theoretical exposition with the presentation of videos cases. The general structure of EMDR therapy in treating BPD, interventions for the preparation phase and considerations for trauma-focused EMDR work will be demonstrated and explained.

Both EMDR and Trauma-Focused CBT are the two main recommended treatments for symptoms of trauma meeting diagnostic criteria for Post Traumatic Stress Disorder.
In accordance with current literature, contrasting the use of Trauma-Focused CBT with EMDR, it has been found in practise by the author that overall, clients using EMDR experience significantly less long-term distress and appears to process much more quickly than clients engaging in trauma-focused CBT. However, the effect of bodily sensations does not tend to differ between the two groups nor does flashbacks or dissociative tendencies.
Specifically, dissociation is not uncommon in traumatised clients and in clients with a diagnosis of post traumatic stress disorder. The existence of dissociative tendencies can pose a realistic problem in effectively processing traumatic memories, regardless of whether the method being used is trauma-focused cognitive-behavioural exposure-based methods or EMDR.
Mindfulness has been utilised by the Author as a stabilisation method for reducing dissociation in clients, prior to trauma processing (CEP conference – Darker-Smith, 2005) and has since been found to reduce dissociation on the DES scale, when measured pre and post teaching clients the basics of the MBSR programme (taught on a 1-to-1 basis).
Equally, the level of flashbacks and bodily reactions subside dramatically when Mindfulness is taught prior to trauma processing, compared with clients who engage in trauma processing without any form of stabilisation.
The author has not found any evidence that the use of Mindfulness body-scan increases traumatic body memory in trauma survivors.
The purpose of this workshop is to explore the application of mindfulness, through experiencing aspects of the Mindfulness programme. Case studies will be presented to demonstrate the application of mindfulness as a stabilisation took, paying specific attention to clients with dissociative tendencies and personality disorders evolving from trauma.
In addition, role plays between participants will be used to practise the skills of mindfulness in relation to stabilisation prior to trauma processing.

The aims are to be achieved by: Encouraging members to contribute to EMDR conferences, other related presentations, writing of papers for publication, whilst also acting as a resource/ support for EMDR researchers as well as fostering links to other appropriate organisations e.g. members of other National and International EMDR Associations as well as other Positive Psychology Associations e.g. the UK Centre for Applied Positive Psychology (CAPP). The presentation will also set out the SIG’s agenda for the next two years up until the EMDR Europe Conference in Edinburgh 2014.

Courses of study do not adequately present material to students regarding trauma and treatment options. Thus, students leave their educational settings lacking knowledge about what EMDR is. This workshop is focused on both undergraduate and graduate educators who work in colleges and universities in the health care and mental health curriculum. Because students lack knowledge about trauma treatment, it could be years before they discover EMDR in their professional life. A panel discussion will provide the following learning objectives: Identify content areas to include when defining trauma; Define EMDR and summarize how it is used as a treatment option for trauma; Identify the challenges and possible solutions within colleges and university settings to include content on trauma and EMDR; and Create a template for content to include in a current course and strategies to market it to colleges in the health and mental health curriculum. The audience will be invited to give their input into the suggested content and challenged to make recommendations regarding how to best publicize the need for this curriculum and ways to achieve including it in more college course curriculum. The end result will be a suggested template that can be posted on the EMDRIA web site to assist educators and clinicians in advocating for its inclusion in their local colleges and university settings.

Background: Little is known about how to remedy the unmet mental health needs associated with major terrorist attacks, or what outcomes are achievable with evidence-based treatment. This article reports the usage, diagnoses and outcomes associated with the 2-year Trauma Response Programme (TRP) for those affected by the 2005 London bombings. Method: Following a systematic and coordinated programme of outreach, the contact details of 910 people were obtained by the TRP. Of these, 596 completed a screening instrument that included the Trauma Screening Questionnaire (TSQ) and items assessing other negative responses. Those scoring 6 on the TSQ, or endorsing other negative responses, received a detailed clinical assessment. Individuals judged to need treatment (n=217) received trauma-focused cognitive-behaviour therapy (TF-CBT) or eye movement desensitization and reprocessing (EMDR). Symptom levels were assessed pre- and post-treatment with validated self-report measures of post-traumatic stress disorder (PTSD) and depression, and 66 were followed up at 1 year. Results: Case finding relied primarily on outreach rather than standard referral pathways such as primary care. The effect sizes achieved for treatment of DSM-IV PTSD exceeded those usually found in randomized controlled trials (RCTs) and gains were well maintained an average of 1 year later. Conclusions: Outreach with screening, linked to the provision of evidence-based treatment, seems to be a viable method of identifying and meeting mental health needs following a terrorist attack. Given the failure of normal care pathways, it is a potentially important approach that merits further evaluation.

Participants were patients of the Outpatient Psychiatry Clinic of the Academic Medical Center / De Meren who had a diagnosis of PTSD following various kinds of type I trauma. Posttraumatic growth was assessed with the Posttraumatic Growth Inventory, whereas posttraumatic cognitions were examined by use of the Posttraumatic Cognitions Inventory. Both measures were administered at pre- and postassessment to determine whether these concepts change after EMDR and/or BEP. Changes in and relationships between these variables are presented and future directions for research arising from the findings are discussed.

The authors review sample composition and enrollment data for 34 studies cited in the International Society for Traumatic Stress Studies (ISTSS) 2000 Practice Guidelines as meeting the Level A U.S. Agency for Health Care Policy and Research (AHCPR) classification for treatment of adult posttraumatic stress disorder (PTSD), and compare data from more recent research. Findings reveal that many published reports omitted vital data including exclusion criteria and rates, demographics, and trauma exposure history. Moreover, severe comorbid psychopathology, a common feature of treatment-seeking individuals with PTSD, emerged as the predominant reason for exclusion across studies. Subsequently published studies exhibited improved reporting of sample characteristics and demonstrated comparable outcomes despite inclusion of more diverse trauma exposure samples. Findings indicate the need for future efficacy research to adopt more comprehensive reporting requirements and to test the applicability of validated treatments to individuals suffering from as yet unstudied combinations of PTSD and prevalent comorbid disorders.

Abstract:
Prolonged exposure counterconditioning (PEC) was tested as a treatment for chronic post-traumatic stress disorder (PTSD) in an adult survivor of repeated child sexual and physical abuse. PEC utilizes imaginal reliving of very pleasurable life moments in order to weaken traumatic conditioned emotional responses (CERs). A higher-order conditioned stimuli (CS) is used as a traumatic CER elicitor. Prolonged imaginal reliving of pleasurable CSs is used as a counterconditioner to the traumatic CERs. A statistical technique for analyzing single-case subject designs based on classical test theory was used to evaluate the client’s progress in treatment. Results showed that PEC effectively decreased the client’s PTSD symptoms, depression, and anxiety. In addition, the client’s negative cognitions became considerably more positive. Also, the client lost his comorbid conditions of chronic major depressive disorder and social phobia. Finally, other clinically observed symptoms, which are described in the article, improved markedly. All results were maintained at a 3-month follow-up.

O presente estudo tem por objetivo apresentar os fundamentos de porque o EMDR funciona no tratamento da dor crônica e apresentar os dados de um estudo piloto que será realizado até a data da apresentação. Diante de uma experiência de dor, o cérebro desenvolve uma rede associativa de memória que pode ficar congelada devido à intensidade e/ou persistência da dor. Aspectos psicológicos associados ao evento disparador ou possíveis ganhos secundários também podem fortalecer o congelamento dessas redes associativas (Grant 2002). Dessa forma, a dor crônica, mesmo que justificada por uma questão física, vem associada à memória da dor que é expressa em sensações corporais. Entendendo que o EMDR trata o trauma psicológico por descongelar e resignificar os eventos passados por meio da reconexão dessas redes associativas com as redes de recursos psíquicos do sujeito, podemos entender que o reprocessamento da rede associativa da dor irá reorganizar essa memória de forma a desconstruir a memória da dor enquanto uma sensação corporal presente (Schneider et al 2008). O Estudo piloto será composto por 4 sujeitos adultos que desenvolveram dor crônica a partir de um trauma físico. Os sujeitos serão submetidos a uma entrevista de linha de base com os seguintes instrumentos traduzidos: MINI (Amorim 2000), Short-Form McGill Melzack Pain Questionnaire (Schneider 2008 e Melzack 1987), Impact of Event Scale (Schneider 2008) e Multiple Affect Adjective Checklist-Revised (Estergard 2008); além do registro das medicações utilizadas. Essa avaliação será realizada em 3 momentos distintos. No início do estudo; após três meses sem intervenção adicional além do tratamento que o sujeito estava previamente submetido e após a intervenção do estudo de um processo de psicoterapia breve de 12 sessões com EMDR fundamentada no Protocolo de Dor (Mark Grant 1998/2009). As diferenças entre a primeira e segunda testagem serão comparadas com a diferença entre a segunda e terceira testagem para verificar a eficácia do tratamento. Espera-se obter uma diferença significativa para fortalecer a tese da eficácia do EMDR nesse tipo de tratamento assim como demonstra a literatura levantada (Bisson et all 2007, Estergard 2008, Friedberg 2004, Grant 2002 e 2009, Schneider et all 2008 e Shapiro 2002).

The present study aims to present the fundamentals of why EMDR works in treating chronic pain and present data from a pilot study that will be held until the date of the presentation. Faced with an experience of pain, the brain develops a network of associative memory that can be frozen due to the intensity and / or persistence of pain. Psychological aspects associated with the event trigger or possible secondary gains can also strengthen the freezing of these associative networks (Grant 2002). Thus, chronic pain, even if justified by a physical issue, comes the pain associated with memory that is expressed in bodily sensations. Understanding the psychological trauma EMDR treats for thawing and reframe past events through the reconnection of these associative networks with the networks of psychological resources of the subject, we can understand that the reprocessing of pain associative network will rearrange this memory in order to deconstruct the memory of pain as a bodily sensation present (Schneider et al 2008). The pilot study will consist of four adult subjects who developed chronic pain from physical trauma. The subjects will undergo a baseline interview with the following instruments translated: MINI (Amorim 2000), Short-Form McGill Melzack Pain Questionnaire (Melzack 2008 and Schneider 1987), Impact of Event Scale (Schneider 2008) and the Multiple Affect Adjective Checklist-Revised (Estergard 2008); beyond the record of the medications used. This evaluation will be conducted in three distinct moments. At baseline and after three months without further intervention beyond treatment that the subject was previously submitted to and after intervention study of a process of brief psychotherapy of 12 sessions with EMDR based on the Pain Protocol (Mark Grant 1998/2009). The differences between the first and second test are compared with the difference between the second and third testing to verify the effectiveness of the treatment. It is expected to obtain a difference significant strengthening the argument of effectiveness of EMDR this type of treatment as well as the literature demonstrates raised (Bisson et all 2007, 2008 Estergard, Friedberg 2004, 2002 and 2009 Grant, Schneider et al 2008 and Shapiro 2002) .

Vid vårt novembermöte presenterade leg. psykolog Gunvor Ingemansson sin erfarenhet av behandling med EMDR-Eye Movement Desensitization and Reprocessing vid enstaka och komplexa traumatillstånd. Under 90-talet arbetade Gunvor Ingemansson inom psykiatrin i Malmö med Rosengård som upptagningsområde. Där mötte hon många människor, som hade svåra och traumatiserande krigsupplevelser. Hon använde sig av krisbearbetande samtal och symboldramaterapi. Dessa arbetssätt hade hon lång erfarenhet av. Hon märkte emellertid att hon inte riktigt kom åt traumat i botten. Pröva nytt Detta ledde till att hon måste pröva något annat. 1994 hade metoden EMDR kommit till Sverige och introducerats av Roger Salomon. Han i sin tur hade en förstahandserfarenhet av metoden, eftersom han utbildats och samarbetar med Francine Shapiro, som skapat metoden. Hennes grundläggande bok är ”Eye Movement Desensitization and Reprocessing”, utgiven på Guilford förlag. Shapiro har senare benämnt metoden ”Adaptive Information Processing Model”. I vardagligt språk i Sverige benämns metoden ofta som ögonrörelseterapi. Shapiro utvecklade ett sätt att arbeta med traumatiska erfarenheter till följd av egna upplevelser. Hon drabbades nämligen av en svårbotad cancer, som läkaren inte kunde ge besked om hur den skulle utvecklas. Hon lade märke till att när hon under sina promenader rörde ögonen på ett visst sätt hände någonting i hennes kropp. Hon undersökte detta vidare och successivt växte den behandlingsmetod fram som i dag spridit sig till många länder för att behandla bl a traumatillstånd. Trata-se de uma adolescente de 17 anos, atendida no PAV-Programa de Atenção a Vítimas de Violência-SES/DF, que foi abusada sexualmente por um inquilino do lote onde morava com sua mãe, ocorrido durante um período não preciso da infância, por volta dos cinco anos de idade. Flashbacks ocorreram durante todo seu desenvolvimento, porém os sintomas se agravaram durante a adolescência, desenvolvendo transtorno alimentar, transtorno obssessivo-compulsivo, isolamento, despersonalização, depressão e ideação suicida. O tratamento com EMDR teve a duração de 3 meses, tendo como alvo as imagens dos momentos de abuso, a imagem da “substância verde e visguenta presa em sua garganta”, que representava o sexo oral que era induzida a praticar com o abusador, e que a levava a provocar o vômito de forma recorrente. Foram intensificados recursos positivos, visualizações antídotos e utilizados os protocolos clássico e de desenhos. Durante o tratamento alguns sintomas se intensificaram, necessitando de cuidadoso reforço e instalação de recursos positivos, porém as evidências de resolução adaptativa apareceram por meio do retorno à vida social, cessação dos episódios de indução de vômito, relatos de projetos de vida, melhora na auto-imagem, relatos de superação, e desenhos que demonstram a volta da alegria de viver, e a visualização dos eventos de forma diminuída, como algo que cessou e pertence ao passado. A paciente conta com uma sólida rede de apoio, a qual foi fundamental durante o processo.

This is a 17 years attended in PAV-Care Program for Victims of Violência-SES/DF who was sexually abused by a tenant of the lot where he lived with his mother, which occurred during a period not need infancy about five years of age. Flashbacks occurred throughout its development, but the symptoms worsened during adolescence, developing eating disorders, obsessive-compulsive disorder, isolation, depersonalization, depression and suicidal ideation. EMDR treatment lasted three months, targeting the images of abuse of moments, the image of "green substance and visguenta caught in her throat," which represented the oral sex he was induced to practice with the abuser, and which led her to induce vomiting recursively. Resources were enhanced positive views and antidotes used protocols and classic designs. During treatment some symptoms intensified, requiring careful reinforcement and installation of positive resources, but evidence of adaptive resolution appeared through the return to social life, cessation of episodes of induced vomiting, reports of life projects, improved self -image, overcoming reports, and drawings showing the back of the joy of living, and the visualization of the events so diminished, and ceased as something that belongs to the past. The patient has a strong support network, which was instrumental in the process.

Porges’ polivagal theory of the hierarchical interpretation of the autonomic nervous system (ANS), in addition to extending the range of human beings’ possible responses to environmental demands, links the first line ventral-vagal response with the regulation of important viscera as well as that of facial and head muscles, mediating social interactions, and associates its activation with the feeling of safety, identifying this latter condition as essential for a person’s well-being.
Without safety no social relations, physiological regulation or healing are possible. Hence the importance for EMDR therapists to lead their clients to this condition: lacking the activation of the ventral-vagal circuit there can be no processing. EMDR therapists will be provided with tools to keep their clients in safe conditions within the setting.
Clients exposed to trauma and/or insecure attachment do not have a good ANS regulation and maintain inadequate defensive attitudes – as demonstrated by Porges’ results, easily comparable with Schore’s on affective development and with those of several multi-disciplinary scholars.
Exploring this dysfunction provides EMDR therapists with useful elements to guide their clients in the difficult task of confronting what they did not/could not process at that time. We propose a three-pronged cross-sectional assessment, regardless of the pathology, aimed at identifying the defensive arousal state of the ANS needed to face the dysregulating impact at that time, focusing on the prevalent activation style of the client, when meeting environmental challenges, and that emerging in the session. Starting from this assessment, EMDR therapists will be provided with tools to help clients recognize and master their defenses to increase their flexibility.
Using the regulation as a healing instrument and goal, and given the two-directional psychophysiological approach, where psychological and physiological processes meet, a new intervention model, stemming from the AIP-EMDR approach, is proposed, acting directly on the missing or impaired developmental stages of the self-regulation ability, consistently with what Porges hoped for.
The intervention focuses on Eye Contact (EC), because, as confirmed by several scholars, this is a privileged communication pathway, in particular in the mother-child dyad, to learn self-regulating skills and is easily impaired in psychiatric clients.

Via accessibly presented neuroscience, Scarf explains how the body stores memories of intensely stressful experiences. A writer rather than a clinician (she's a senior fellow at Yale's Bush Center in Child Development and Social Policy), Scarf generates her data through meeting women subjects in marital distress and exploring their pasts through gentle discussion. Throughout, Scarf weaves her own autobiographical reflections, centered on painful memories of an autocratic father and a negligent mother. Seeking to advance her own emotional well-being, she enters into a reprocessing therapy session and becomes an advocate of the technique; she persuades one of her subjects to try it out, with apparently successful results. Although the physical ailments presented in Scarf's account seem extremely slight, she makes much of a sense of emotional breakthrough and release. Scarf's investigation into the methodology of reprocessing therapies is scientifically limited, yet she does allow us some insights into how they function. Admirers of her work will enjoy her ability to evoke relationship dynamics (including abusive relationships), her seductively flowing style and her emphasis on perceptive readings of life histories. Readers with a serious interest in psychology will find little cutting-edge scholarship here, and some may question why all Scarf's subjects are women. We know from research that since neurobiology is the same for humans, then the reaction to stress is the same for all human beings in the world. That is why EMDR works so well with victims of natural disasters and of adverse childhood experiences, including sexual and physical violence and emotional neglect. Because of cognitive functioning, e.g., creating metaphors and images for self-soothing, establishing social contact and/ or personal interpretation, or assigning meaning to stressful experiences, we have to emphasize the importance of cultural influences. We want to invite people from different cultures to join this open meeting to present, share, and discuss their experiences and perhaps difficulties with cultural adaptation of trauma-therapy techniques, including EMDR. For example, we may need to examine where and how the wording of the EMDR-protocol needs to be adapted to recognize and accommodate cultural differences; perhaps we need to look at the language structure and its accessibility. This workshop will focus on the types of transference and counter-transference that arise in EMDR with dissociative clients and teach solutions. Procedural modifications have been the focus in dealing with pathological dissociation in EMDR treatment. Separately, transference and counter-transference with dissociative patients have been written about extensively by experts in the dissociation field. Research findings about the effects of mirror neurons and embedded simulation on the inter-subjective field of patient and therapist have also been published. Strategies for dealing with these transference and counter-transference in EMDR treatment have received little attention even though this population has intense transference, and can activate intense counter-transference. These issues may begin during an evaluation of the presenting problems. Strategies for identifying and using transference to enhance dual awareness during history taking will be demonstrated. An elongated preparation phase to develop enough trust and stabilization before exploring traumatic memories can limit induced transference. Different parts of a dissociative patient may have different kinds of transferences. These transferences may cause the patient to withdraw, cling or attack; affecting the therapist’s abilities to stay attuned and focused on the work in different phases of EMDR. Strategies of attunement to the activated part of the client will be demonstrated in order to repair or prevent ruptures of attunement. Interactions are bi-directional, and different (transferential) parts may activate dissociative parts of the therapist. Strategies to somatically identify and use these counter-transferential activations in the therapist will be taught through body based awarenesses. R/D/I strategies can be used to limit countertransference to remain grounded and attuned. Transference and counter-transference during the assessment phase will be identified and solutions presented. During the Desensitization phase under-accessing or over-accessing target memories; abreaction vs. vehement emotions will be discussed as unacknowledged dissociative moments with indications for inducing transference, counter-transference, or both. Decisions need be made collaboratively whether to process or contain these events. Understanding and dealing with dilemmas of dissociative enactments are crucial to keeping the healing process going. These inter-subjective issues may be most intense during the first four phases, but some problems may continue into Installation and the Body Scan. Problems and solutions during Incomplete Closure and the Re-evaluation phases will be given. Activated parts in the patient may cling or be angry with the therapist at the end of an EMDR session. Failure or defectiveness parts of the therapist may become activated as well. Solutions to these issues that occur during different phases will be taught so that participants will leave the workshop with additional strategies to use with their dissociative patients. Attunement to dissociative parts, identifying transference and counter-transference binds; The Clinician Self Awareness Questionnaire ; Compartmentalization; use of self soothing skills; using Relational, Empathic, and Transferential Interweaves; identifying moments of projective identification and enactments, and then to use them to deepen EMDR will be taught, as well as innovative inter-subjective strategies . Dealing with dissociative enactments are crucial to identify ruptures to the therapeutic alliance, restore attunement and resonance, uncover dissociative messages that can be used during EMDR processing.. Solutions to the problems that occur during different EMDR phases will be taught using lecture, discussion, case examples, written and experiential exercises so that participants will leave the workshop with additional strategies. Clinical observations of EMDR treatment sessions indicate that therapeutic results are often achieved through the progressive emergence of an adult perspective, particularly when the client was previously locked into the emotional responses of a childhoodbased trauma. Clearly, most childhood experiences are infused with a sense of powerlessness, lack of choice, lack of control, and inadequacy. Even the best of childhoods have moments when the parents attempt to leave for the evening and the child feels abandoned, powerless, and uncared for. Indeed, an entire generation of children was raised by a book that dictated feeding hours and parents were encouraged to avoid reinforcing the child's crying for food at other times. Consequently, thousands of children were left crying in the dark for food. Regardless of the fact that language was not yet encoded, arguably, this situation set up certain emotional nodes regarding "Self," "Suffering," and "Others." Conjoint use of systemic family therapy and EMDR is examined. A young adult in the "leaving home" phase of the family life cycle, affected by panic attacks and concomitant anxious/depressive disorder has been treated following the systemic approach , with family sessions and individual sessions including the use of EMDR at specific times. The clinical case is taken as an example for theoretical and practical considerations and for the analysis of the possible integration of the two approaches. This analysis underlines the use of EMDR as a "stimulating factor" in different moments of the family therapy treatment. EMDR helped to focus and elaborate a strong but undefined feeling of serious personal danger in the young identified patient and, on the other side, it helped to define clusters of cognitive conflicts which prevented the development of more adaptive behaviors. Elements that suggest a careful and skillful use of EMDR are presented together with the corresponding need for minor modifications, if associated with family therapy. The aspect of timing individual sessions with EMDR is also considered. The problem of how to interweave elements deriving from EMDR sessions and contents deriving from family sessions is discussed and useful hints about the integration are suggested. [Author abstract]

Twelve days after the 26th of December 2004 a couple came to my private practice, on referral from Prof. Dr. Ad de Jongh. that looked death in the eye when the Tsunami hit Sri Lanka. The couple has four children, age four to eleven, who survived with them. On Sri Lanka they were called 'The fortune family'. They both had severe symptoms of Acute Stress Disorder: reliving the disaster day and night and were, only just, managing to take care of the children and their daily life.
They already read about EMDR and had their hopes up that I could help them stabilize. As soon as they started telling me about their distressing experience I noticed that, especially the woman, started reliving it. Knowing that they had been telling everything already many times to family and friends, I asked them f I could immediately do the first EMDR session with each of them. Quite noticeable was that the experience was still in their minds with every detail and with several peaks of the most distressing moments. In total they had three single sessions each with two-days intervals. Their children who at first were doing relatively well had started to develop serious symptoms and needed treatment; after the three EMDR sessions for each of the parents they were stable and could give their full attention to EMDR-treatment of their children, who went to Carlijn de Roos MA, clinical child-psychologist, who leads a trauma centre for children in the Netherlands. At the end of February the parents were still doing well and at the time of the EMDR Europe Conference I will have seen them for a follow-up.

This report was commissioned by the Department of Veterans Affairs (VA) to assess the scientific evidence on treatment modalities for Posttraumatic Stress Disorder (PTSD). Reviewing the PTSD treatment literature dating back to 1980, the year the disorder was first defined by the Diagnostic and Statistical Manual of the American Psychiatric Association, proved to be a challenging task. Assessing the outcomes of treatment depends entirely upon the self-report of those affected, without “objective” measures such as laboratory tests or imaging. Treatment modalities and research methods used in their evaluation have been in continuous development. The last 30 years have also seen dramatic changes in the way scientific evidence has been assessed in general with emerging international standards for conducting systematic qualitative and quantitative reviews that are quite different from the methods used in the 1980s when research on the treatment of PTSD began.
In applying a rigorous approach to the assessment of evidence that meets today’s standards, the committee identified significant gaps in the evidence that made it impossible to reach conclusions establishing the efficacy of most treatment modalities. This result was unexpected and may surprise VA and others interested in the disorder. Important treatment decisions for most modalities will need to be made without a strong body of evidence meeting current standards (the committee summarizes clinical practice guidelines developed by others in the face of this scientific uncertainty). This overall conclusion of scientific inadequacy is not a clinical practice recommendation or guideline. It is also not a judgment on the quality of the research in this field using methods acceptable at the time. The overall conclusion also adds urgency to the committee’s recommendations for a more strategic research effort that defines the relevant populations and subpopulations; develops and tests treatment modalities alone and in combination, in individual and group formats (for psychotherapy), and of various intensities and durations; uses the latest and most rigorous methods for designing and executing study protocols; and follows all study participants through the end of treatment and for meaningful periods thereafter.
The committee was also struck by the scant evidence exploring some of the possibly unique aspects of PTSD in veterans. For the most part we cannot say whether the treatment of PTSD in veterans should be the same as in civilians, and whether important subpopulations of veterans defined by age, sex, trauma type, socioeconomic status, educational level, comorbidities, and brain injury should be treated the same or differently.
The committee could only conclude that well-designed research is needed to answer the key questions regarding the efficacy of treatment modalities in veterans. Success will depend on the collaboration of VA and other government agencies, researchers, clinicians, and patient and veterans’ groups and will further require the continued support and attention of policymakers and the public. The individuals returning from current conflicts and now re-entering civilian life with this disorder deserve no less.

In 1995 the Division 12 Task Force on Promotion and Dissemination of Psychological Procedures published its report in this journal. A major focus of that report was increasing training in psychological interventions that have been supported in empirical research by making clinical psychologists and students more aware of these treatments and facilitating training opportunities. To provide the basis for a survey on the degree to which clinical programs and internships were currently providing training in empirically supported therapies, the task force constructed a list of examples of treatments meeting criteria for efficacy as established by the task force. Based on feedback that members of the profession found this list of interventions to be very useful in training and clinical work, while also recognizing its very incomplete basis, the Division 12 board charged the succeeding task force (Task Force on Psychological Interventions), appointed in succession by Presidents Martin Seligman and Gerald Koocher, with adding to this preliminary list on an annual basis. This is one purpose of the current report. In addition, we raise several issues about the use and limitations of empirically supported treatments as currently identified. In keeping with the practice established by the first task force, the members of the group who constructed the present report are diverse in theoretical orientation and work in a variety of settings -- psychology departments, medical schools, and private practice.

Abstract: I am presenting the case of a 13-year-old boy who was referred because of uncontrolled rage evident in his relationship with peers in one incident he had a fellow pupil’s head under water until he was gasping for air.. He had shown little remorse towards the boy and described the incident as funny. The family had experienced a series of traumatic events. They were forced to evacuate their home when criminal gangs attempted to burn them out; his was to use the house for drug-related activity. Gang members had assaulted my patient and his mother in the street after going to the police. After meeting with the parents and brother and older sister, I had identified that on top of this recent trauma L had extreme hostility towards his mother who he described as “that woman.” My initial EMDR sessions were with L and his father. During processing of the trauma, my video will show bizarre movement including rolling his head back, hypotonic posture and behavior similar to an infant. This can be understood in relation to L’s early development. He was born at 33 weeks gestation and spends his first 6 weeks in a special care baby unit (CBU) in an incubator. My presentation will discuss the neurological consequences of early trauma and how a narrative approach using EMDR can help in recovery. This approach is based on the work of Dr. Joan Lovett. She is a California-based pediatrician who has developed a protocol for working with children who have experienced significant trauma following premature birth.
My second case is J, a 7-year-old boy who witnessed his father murder his mother approx 1 year ago. He was then held hostage by his father for 6 hours with the body of his mother lying in a pool of blood in the kitchen while the police negotiated with J’s father for his release. My first involvement with J was an in depth assessment of his emotional attachment and placement needs for the court. During this process, he developed frequent infantile rages and I advised the system of care around J on their management. At one stage, he had recurrent dreams about joining his mother in heaven and he was discovered attempting to strangle himself. I will discuss how I used the safe place protocol to alleviate his distress. I am now using EMDR directly with J. I use a variety of bilateral alternating stimuli. These include drumming, musical symbols, and a xylophone. I am able to get J to draw sequentially with each new drawing generated by J performing the “Butterfly hug.” I was inspired to use this technique after I attended a wonderful workshop facilitated by Michel Silvestre entitled “integrating family therapy and EMDR.” I hope to discuss in this presentation how EMDR can be combined with other therapeutic approached in a case of extreme trauma experienced by a 6-year-old boy who is now effectively orphaned as his father is in prison probably for the rest of his natural life. I will also allude to some of the techniques discussed by Dr. Atle Dyregrov at the 5th annual UK and Ireland conference in London in March last year. He presented in depth therapeutic work with a girl who suffered the trauma of her mother’s suicide. I will discuss some of the challenges presented by traumatic grief and how the EMDR protocol can be adapted for use in children.

Now that the therapeutic relationship is firmly part of EMDR, it is time to show its uses with difficult populations. Skilful emphasis on empathic attunement beginning in the history taking phase with emphasis on using the Procedural Steps Outline diagnostically, and Light stream as an affect management tool, starting in the first session will be shown to be of use specifically with this population. This population needs special attention regarding alterations in affect regulation, self perception, consciousness and attention, somatisation, trust, and identity. In the preparation phase participants will learn various relational strategies to accomplish these tasks. They will also learn to use the relationship as an additional resource for containment with appropriate boundaries. Relational concepts such as “Implicit Relational Knowing”, “Moments of Meeting”, and “Dyadic Expansion of Consciousness” will be taught to expand methods of stabilization for preparation, and for active trauma work. Grade 8 Up–Anna is driving a very drunk friend home from a party. Moments into the journey, a head-on collision leaves Ellen with a punctured lung and other serious injuries, Anna with a lacerated eye, and the other driver dead. The dead teen happens to be her brother's girlfriend. Anna clearly remembers Cameron's final screams, and she suffers nightmares. Her father is an emotionally repressed tyrant who at first won't allow his daughter to receive counseling. Frank develops and sustains credible characters whose problems are realistic and interconnected. Brief flashbacks allow readers to become acquainted with Jack as he was before Cameron's death and even as he was when he and Anna were children. Their father's brittle personality is not evil or even cruel, but clearly riddled with flaws bred of deeply held fears. In spite of some plot twists that seem convenient rather than realistic, such as the teens' pre-Thanksgiving trip to Florida with Ellen's parents, this story is compulsively readable both because Anna is likable and imperfect and because Frank's writing is so fluid. A special series on Thought Field Therapy in the Journal of Clinical Psychology provides an opportunity for psychologists to learn about techniques and theories outside the mainstream of our field. Unfortunately, by publishing this series of manuscripts without meeting the standards of peer review, the Journal also provides an avenue for the misuse of its good reputation and the improper promotion of untested methods. “Echo attributions” can be made whereby an author attributes the source of his own words to the professional journal in which the text appears. Historical examples illustrate that such misuse of scientific journals and institutions occurs. A formal statement of guidelines is needed to instruct authors on appropriate versus unethical representations of their publications.
