Abstract:
Clients with complex PTSD and dissociation present many challenges. The neurosciences have helped us to begin to understand and deal with them through a recent clarification of mirror neurons and associated neural structures in both the clinician and client. Concepts from the Boston Change Process Study Group and ego state therapy provide methods of analyzing and intervening in the “microprocesses” that occur in treatment. Hoppenwasser’s concept of “dissociative attunement” challenges thinking about the “multiple self states” both clinician and client operate from. Her ideas push us to rethink current conceptualizations of relatedness. Participants will learn how to deal with ruptures in positive empathy that may result in the history taking, assessment, and desensitization phases. In the preparation phase, participants learn to use the therapeutic relationship as an additional resource for containment. Concepts of dyadic regulation of affect, now moments and moments of meeting will be taught to deal with ruptures to the therapeutic relationship throughout treatment. Dealing productively with countertransference ruptures poses additional challenges. Participants will learn a strategy called the “relational interweave”. Its function is to restore EMDR processing when an interpersonal “event” has temporarily derailed the work. A practicum using Dworkin’s Clinician Self Awareness Questionnaire will be held in the afternoon part of the workshop to enhance learning this strategy.

Abstract:
The data presented in this article was collected after reviewing clinical findings gathered from using various anxiety control methods on apprehensive patients. We examined clinical applications of the eye movement (EM) component of Eye Movement Desensitization (EMD) on fearful dental patients who have histories of traumatic dental experiences. We also used Bi-Digital O-Ring Test (BDORT) to select the proper dosage of sedative to minimize the adverse side effects. For patients who did not respond well to EM, we used BDORT to select the proper sedative medication and its dosage. In certain difficult cases, we supplemented these techniques with acupuncture to augment the sedative effects. Findings were based on the clinical impressions and assessments of both the patients and the operating team. Results showed that EM, although effective in enabling patients to undergo non-invasive dental procedures such as clinical examination and simple prophylaxis, had only limited beneficial effect with invasive procedures such as extraction, drilling, and injections, etc. We also found that BDORT greatly reduced adverse side effects of sedatives such as hypertension, hypotension, hypoxia, tachycardia, bradycardia, nausea, and vomiting. For most apprehensive patients, we found that EMD and acupuncture combined with BDORT predetermined dosage for the submucosal sedation enabled these patients to undergo the complete dental treatment. The authors try to explain the mechanism of BDORT and EM in terms of visual awareness (or consciousness) and preferred patterns, where neurons in the brain respond to the actions and/or direction of movement. The authors believe that BDORT and EM could have better results if the persons performing BDORT have visual awareness and are focused on the task; whereas in EM, the patient's eye on the therapist's hand movements. A more focused approach via visual pathway will result in more favorable results in EM. Likewise, performing BDORT absentmindedly could lead to false results if visual awareness (or consciousness) is absent. "Preferred pattern" will arouse neurons in the brain to cause conscientiousness, and performing BDORT with 'open eyes' arouse the necessary visual awareness that is necessary for the successful performance of BDORT tasks

Progress in neuroscience reveals that attachment learning, affect dysregulation, and traumatic experience are predominantly held in the right hemisphere (Schore, 2009). Therapists can access trauma and neglect held in the right hemisphere using EMDR, somatic awareness, imaginal excursions, and the therapists mirror neuronal experience. This workshop summarizes the Early Trauma approach of EMDR (O'Shea, 2009; and its variations for the dissociative client (Paulsen, in press). Of note is a new stabilization procedure that appears to work directly on the subcortical affective circuits (Panksepp, 1998). The Early Trauma approach to EMDR processes preverbal implicit memory in sequential time periods, while attending to the nuances of somatic and affective experience as they emerge both in the clients subjective report and in the relationship field. As the infants story is told with the deciphering of these nuances, the therapy repairs developmental milestones imaginally. This enables integration from the bottom up, by time frame, called Temporal Integration (Paulsen, 2009), which contrasts with Strategic and Tactical Integration. Preliminary clinical findings are that the procedure helps repair injuries of attachment, structural dissociation, affect dysregulation and personality. The workshop will interest both EMDR and non-EMDR practitioners because of its significant implications for theory and practice Learning Objectives: Participants will be able to list a modification of the ET procedure for dissociative clients for each of the four steps. Participants will be able to list the seven hardwired subcortical affective circuits described by Panksepp. Participants will be able to name four steps of the Early Trauma procedure for non-dissociative clients.

Abstract:
Alternating left-right eye movements (EMs) have been shown to enhance episodic memory recall (Christman & Garvey, 2000). In turn, enhanced recall of episodic, or autobiographical, memories has been linked to increased interhemispheric interaction (Christman & Propper, 2001; Propper & Christman, 1997). Since leftward versus rightward EMs induce right versus hemisphere activation, respectively (Bakan & Svorad, 1969), bilateral EMs may facilitate interhemispheric interaction by balancing hemispheric activation levels. In other words, asymmetries in cortical activation may impair interhemispheric communication. Christman and Garvey (2000), however, demonstrated only that bilateral EMs enhance episodic memory performance; they did not directly measure hemispheric activation. To test this hypothesis directly, participants were administered two behavioral tests that are sensitive to variations in hemispheric activation. The Chimeric Faces Test (Levy, Heller, Banich, & Burton, 1983) involves mirror-image pairs of faces, with one smiling hemiface and one neutral hemiface, presented under conditions of free viewing. Participants are asked to judge which member of a pair looks happier. Right versus left hemiface biases are associated with relative increases in left versus right hemispheric activation. Line bisection tasks also tap into hemispheric activation, with leftward versus rightward bisection errors reflecting increased right versus left hemisphere activation. Bisection tasks, however, are also influenced by premotor asymmetries (e.g., Luh, 1995), and thus represent a less pure measure of hemispheric activation.
METHODS Participants performed blocks of chimeric face and line bisection trials. They then engaged in 30 seconds of alternating left-right saccades (2 eye movements per second). Participants then performed a second block of trials for both tasks.
RESULTS Results showed that behavioral asymmetries in the chimeric face task were significantly reduced after the EMs (p<.029). This supports the hypothesis that bilateral EMs reduce hemispheric activation asymmetries. Performance on line bisection task, however, was unaffected by EMs (F<1), perhaps reflecting the fact that this task is a less direct measure of hemispheric activation.
DISCUSSION These results provide further insight into mechanisms underlying EMDR therapy used for PTSD patients. Reduction in hemispheric activation asymmetry arising from bilateral EMs employed in the current study (and in EMDR) may enhance interhemispheric communication. This interpretation is consistent with Ramachandran (1995), who showed that vestibular stimulation (inducing optokinetic nystagmus and resulting in involuntary left-right EMs) reduces the denial of deficit found in anosagnosia patients. Ramachandran (1995) suggested that the procedure benefits the hypoaroused right hemisphere, bringing its activation level up to par with the left hemisphere. Similarly, EMs used in EMDR may balance hemispheric activation levels, in turn enhancing interhemispheric communication and retrieval of episodic memories.

In certain cases, we can be confronted to a need for retreatment of targets that the person in front of us cannot have experienced by herself: second war events, being in a prison or concentration camp…
But the person in therapy show real disorders, including dissociation which seems related to those targets.
How could be treat such transgenerational trauma with EMDR? With which results? Which hypotheses could we formulate out of this work? Which are the implications on the persons’ family environment?
Out of three clinical cases, I propose to share my experience and think together on transgenerational trauma transmission and its treatment, by using the concept of mirror neurons.

Ann Norris first went to see Laguna Beach psychologist Doug Sawin in 1988. She had recently graduated from college with a degree in music and suffered from insomnia and drank alcohol to sleep. But it was her relationship with her mother, Judy, that troubled her most. After Ann's triumphant college graduation vocal recital, Judy hadn't even congratulated her. Two days later, Judy had called and angrily attacked Ann over the phone until Ann cried.
It was the kind of issue that a good family or individual therapist might have addressed by building on Ann's obvious strengths, teaching her to contain and manage her feelings, and coaching her to develop a better relationship with her mother. But Sawin instead focussed intensely on the past. Ann soon had memories of her father sexually abusing her, and later of elaborate cultic abuse, which her three siblings didn't come close to corroborating. She was hospitalized after attempting suicide, and Sawin bluntly told her father, Al, over the phone, of Ann's charges Al collapsed in tears.
Over the years, Ann drew closer to Sawin while her relations with her family and her own mental state grew more troubled. She was diagnosed with Multiple Personality Disorder and, with Sawin's support, sued her parents and grandparents for $20 million. She spent six years in therapy with Sawin She now describes psychiatric hospitals where she still stays periodically because she cuts and burns herself as her "institutional mothers." She has not spoken to her true mother in six years. And she no longer sings.
It doesn't take a PhD in psychology or a seat on a state licensing board to see that Ann is worse off than when she entered therapy. Millions of nontherapists undoubtedly made just such an assessment when Ann, her therapist and her family told their stories before millions of prime-time viewers on "Divided Memories," a four-hour PBS Frontline documentary screened in early May.
In her wide-ranging investigation of therapy, sexual abuse and memory, producer Ofra Bikel used as her primary subject families divided by recovered memories of abuse. She also managed to persuade nearly half a dozen therapists to do therapy while her camera was running. It was a remarkable event, in which all of America was invited behind the one-way mirror to see therapy in action in the midst of its most divisive controversy and to judge it for themselves.

The focus of this 3-hour workshop will be on using EMDR to alleviate emotional distress attendant to coping with acute physical pain and living with chronic pain. Participants in this workshop will learn: (a) how to conduct a targeted assessment of the patient whose chief complaint is physical pain; (b) a guided pain healing meditation; (c) an EMDR protocol for installing pain relief imagery and self-care techniques; (d) an EMDR protocol for reprocessing covert pain talk, pain behaviors and pain-related memories; (e) how to apply A.J. Popky's EMDR protocol for reducing substance abuse (i.e., Overeating, drinking) and dependence on pain medication in this population. The presentation will first address how to conduct a structured assessment that identifies targets for EMDR treatment. The dysfunctional information package associated with chronic pain (termed the "biogram")and "seven keys" to understanding chronic pain will be discussed. Then, essential principles for designing an effective individual psychotherapy program for the pain sufferer will be presented. Next, use of the "seven keys for constructing a guided healing meditation (i.e., Self-care technique) for the pain patient that also incorporates Francine Shapiro's "light stream" and "spiral" guided meditations will be demonstrated. A script for this, termed the "C.O.M.P.I.S.S. Pain Healing Meditation," will be distributed. The workshop will then address how to introduce EMDR to the patient whose chief complaint is physical pain. What EMDR can do and probably cannot do for pain patients will then be discussed. Next, principles for choosing an initial tarqet for reprocessing will be discussed. The presentation will then cover (with clinical case examples): (1) Red flags and cautions to consider before proceeding with EMDR- (2) What to do and what not to do if the patient is dissociative; (3) How to "mirror" and install with EMDR empathic responses to underlying beliefs associated with "secondary gains" minus primary losses; (4) Teaching the distinction between pain sensations and suffering; (5) How to teach and install with EMDR self-care and pain coping techniques such as pain relief imagery, mental distraction techniques, safe place imagery, and positive motivation for healthy self-care behaviors; (6) How to directly address with the patient the application of "cognitive psychology" and imagery for pain reduction; (7) EMDR reprocessing of memories around the pain's origins; (8) EMDR reprocessing of pain-related conflicts, negative beliefs, negative past experiences, internalized self-identifications, self-punitive tendencies and self-defeating behaviors; (9) Eliciting core negative pain coping cognitions and suggesting preferable positive cognitions to the pain patient; (10) EMDR reprocessing of negative cognitions associated with depression and anxiety. (11) Use of EMDR to facilitate mental rehearsal of coping responses to pain triggers; (12) Material that often comes up in using EMDR with pain patients; (13) Strategically restructuring patient "resistance" with coanitive interweave; (14) Managing narcotic and pain medication seeking behavior and substance abuse; (15) Use of Popky's EMDR protocol for reducing medication dependence and substance abuse in this population; (16) Treating pain patients who also have PTSD. De werking van „Eye Movement Desensitization and Reprocessing‟ (EMDR) is lange tijd onbekend gebleven, maar inmiddels zijn meerdere theorieën beschikbaar om de effecten te verklaren. De werkgeheugentheorie lijkt momenteel de theorie met de meeste wetenschappelijke ondersteuning. Het doel van het huidige onderzoek was het kritisch toetsen van de werkgeheugentheorie door het werkgeheugen te belasten met oogbewegingen tijdens het ophalen van een herinnering of maximale belasting door imaginatie en hyperconcentratie op de herinnering. Verwacht werd dat 1) oogbewegingen tijdens ophalen de herinneringen minder emotioneel, helder, compleet en waarheidsgetrouw maakt, terwijl 2) imaginatie van de herinnering resulteert in spiegelbeeldige effecten. In totaal participeerden 52 studenten verdeeld over twee experimenten, waarbij zij oogbewegingen en imaginatie aangeboden kregen tijdens het ophalen van een herinnering. Zowel oogbewegingen als imaginatie belastten het werkgeheugen in een reactietijdentaak. Imaginatie leidde tot meer complete en waarheidsgetrouwe herinneringen, terwijl voor oogbewegingen geen significante resultaten werden gevonden. Deze opmerkelijke resultaten zouden verklaard kunnen worden door de aard van de gebruikte herinneringen en de lage power van het onderzoek. De werkgeheugentheorie blijft de theorie met de meeste wetenschappelijke ondersteuning.

It has been long unknown what the mechanisms are behind Eye Movement Desensitization and Reprocessing‟ (EMDR), meanwhile there are several theories available to explain its effects. The workingmemory (WM) account seems to be a theory with most scientific support. The effort of the present study was to critically test the WM account by taxing WM with eye movements during recall or by maximum taxation with imagination of and concentration on the memory. Hypotheses were 1) eye movements during recall reduces the emotionality, vividness, completeness and veracity of the memory, while 2) imagination increases emotionality, vividness, completeness and veracity of the memory. In total participated 52 students divided in two experiments, in a within-subjects design. Both eye movements and imagination taxed WM in a reaction time task. Imagination increased the completeness and veracity of the memories, while there were no significant results in the eye movements condition. These remarkable results could be explained by the nature of the memories that were used in the experiments and low statistical power of the experiments. The WM account remains a theory with most scientific support.

The wellbeing of rescuers: Relational, organizational and technical aspects that can affect rescuers' wellbeing during rescue activities: Stress inoculation, role playing and the role of mirror neurons in training, also through the use of video recordings. Relaxation techniques, psychological debriefing and EMDR in trainings.

Focus of our intervention is the wellbeing of the rescuer. The study and research on this matter came and were carried out thanks to the activity done both during trainings and simulations of the Civil Protection than real emergencies. Our team work received contribution by some psychologists of OPP (Parma’s Psychologists’ Observatory: A.Sozzi, E.Pedrelli, F.Frati, A. Bocelli, T. Serra). Wellbeing, defined as a subjective and positive emotional state together with a global life satisfaction (Diener, 1984), is strongly at risk during rescuer’s emergency activities and can affect the rescuer both physically and psychologically. The rescuer's capabilities, that we think are technical “know how” and thorough knowledge, are essential to give the best performance according to the complexity and urgency of the intervention. These skills can really contribute to the rescuer's wellbeing, because they can improve the self-efficiency perception. To effectively manage and train rescuers, it is furthermore important to consider and acknowledge the influence of interpersonal relationships on technical performances. It is, in fact, particularly important to recognize and support the typical relationships that can be created in a team with the same task and specialization, as well as in multidisciplinary teams, or teams belonging to different Institutions but operating in the same scenario.

In recent years increasing attention has been given to training activities, even through the use of the role play for interventions in artificial emergency scenarios. To recreate scenarios of massive emergencies, different Civil Protection Associations, as well as First Aid volunteer associations and the local Institutions have been involved. In these simulations, most cases focus on improving technical performances. Lately psychologists have been asked to join the rescuers team. During these simulations, the role-play of emotional and psychological problems occurs thanks to the cooperation between emergency psychologists and the medical team. The introduction of the role and expertise of psychologists allowed to extend and strengthen the attention to cross support and care aspects for the psychological wellbeing of both victims and rescuers. The psychologist must therefore consider the “wellbeing” in all the emergency scenarios and contexts, as a sum of all the components that we talked about here and the ones we will describe during our intervention. He must first of all be aware of the complexity of each intervention in the field, and adopt a kind of approach aimed at creating and recovering wellbeing strategies, that can be used by himself as well. Strategies on how to build, recover and maintain the wellbeing identify stress as the first danger source the rescuer has to face in his training and emergency activity. When external events or stimuli are perceived as difficult to face compared with resources available at that moment, the individual gets stressed. When the person's efforts are not adaptive to the external requests and/or coherent with his performance expectations, he becomes vulnerable to emotional, behavioural, cognitive and physical reactions, which can be even very difficult to manage both in the short and/or in the medium-long term. This can happen when the sources of stress depend on the rescuer’s performance, and it can also happen in case of post traumatic stress, visible in different stages after the event. From the psychologist's specialist background and from the integration of this with the result of field experiences, the demand for a range of different tools to manage the different kinds of stress emerges, and these tools must be applicable both to the individual and to the group. This range is still improving, and the results of our observational activity from past and present experiences lead us to see the opportunity to carry on our research of tools of efficacy. During this speech we would like to underline that approaches like Stress Inoculation Training (SIT, Michenbaum, 1983) and the use of role playing allow the technical appraisal and let the rescuers improve their stress management skills, and all that can lead to a decrease in the risk of PTSD. In past simulations of emergencies, we found out that the use of videotapes for the role plays is a tool that should be taken more into account. We think it is important to evaluate its potential for the rescuers' benefit, because it seems to be not only “a record of technical performances”, but also an observation and learning tool about the rescuer's own defence and adaptive strategies. In fact, during these simulations we found out that the rescuers' psychological and emotional vulnerability emerged in several situations. The fact that even in these artificial situations there were acute stress episodes and O codes urged us to focus more on the matter of mutual influence between technical performance and internal experience of stress. We understand that such acute stress episodes may occur during real life critical events but we can see how role playing and video recordings show that such acute stress episodes affected the simulators themselves even during the simulation. The videos show that even apparently “high immunity” simulators, who are considered 'immune' thanks to their comprehensive and strong experience, experienced acute stress, perhaps because of an incorrect selfevaluation of their own stress management skills. The interest in the use of videos as a training and reprocessing tool for rescuers led some of us to specialize in role playing recording, so as to carry out a more accurate and comprehensive study on those same videos and use them as a mirror of reality and better educational tool through a vicar experience or through “seeing oneself from within the experience” and in the interpersonal dynamics that took place in the scenario. Videotapes are a very known and widely used tool in other kinds of trainings, disciplines and therapies (i.e. Family Therapy and CBT). The discovery of mirror neurons by Rizzolati, Gallese et Al., provides the evidence that when someone observes the same action performed by another person, the neurons "mirrors" the behaviour of that person, as though the observer were itself acting. Thanks to these researchers it is now proven that this can happen thanks to the motor neurons in the pre-motor cortex. Therefore, we would like to underline the role of videos as very useful and versatile training tools, since they expose a situation in an unexpected realistic manner “as if” it were true and “as if” we were really experiencing that situation, with the consequent learning movements at the emotional, cognitive and behavioural level, at the stress management level, as well as at the level of team work dynamics. Visual imagination activates the same brain regions that are active during visual perception and motor imagination activates the same brain regions activated the movement is really happening. More importantly, it was possible for us to verify that the videos recorded by other operators were not focused on showing the important psychological aspects we mentioned for the goal of the trainings, thing that happened instead with the videos recorded by psychologists. We think therefore that the use of videotapes recorded by psychologists should be given more consideration in the trainings of rescuers. During this intervention we will devote part of the time to broadcasting two short videos; the first one shows the role playing of an intervention in an emergency context, and the second one shows a part of an EMDR session (Eye Movement Desensitization Reprocessing). We think it is important to recreate and protect rescuers wellbeing in the post-role playing and post emergency stages too. For years EMDR has been proven effective in improving the individual's coping skills and in reprocessing, wherever necessary, the post traumatic aspects resulting from critical events to whom not only the victims, but also the rescuers too, are exposed during emergencies.

Abstract: INTRODUCTION: Idiopathic Parkinson’s disease (PD) is proposed to be the result of a multifactorial process that causes the loss of dopaminergic neurons in the substantia nigra. The first symptoms appear when dopamine is reduced by 80% in the striatum nucleus or when 60% of the negro-striatal neurons have disappeared. This indicates that the cause of PD happens or starts to be active years if not decades before the first symptoms appear. There is evidence from animal and human models that PD has an early pre-symptomatic or masked onset as a result of pre- or perinatal disruption of the neuronal development of the brain and/or by a postnatal traumatic impact very early in life affecting the neurogenesis and/or making the nigro-striatal system vulnerable to subsequent experiences (Diseth, 2005; Le, Chen and Jankovic, 2009). Medical research identifies such (subsequent) traumatic impacts in genetic or environmental factors or following a neurodevelopmental insult by aging alone (Le et al., 2009).

In this presentation I would emphasize the relationship between attachment, trauma and the development of the AMN (adaptive memory network). From a psychobiological point of view, we understand that early relational experiences shape brain growth and organization and that the major environmental influence on the development of the brain is the attachment relationship. Reductions in brain volume and dysfunctional memory networks following traumatic experiences in early childhood are documented. When there is a distressing incident, it may become stored in state-­‐specific form, unable to connect with other memory networks that hold adaptive information. The research of the neurobiology of the social brain and the mirror neuron system let us assume that the AMN is developing in the presence of an attuned caretaker. Healing traumatic memories is relational and procedural. I use EMDR within the Phase-­‐ model of trauma-­‐informed treatment. During the preparation phase (phase 1 and 2 EMDR protocol) I would like to stress the importance of: • evaluating the attachment pattern of the child. It affects how the child relates to the therapist. Establishing a healing therapeutic relationship is a goal of phase 2. • the activation of networks containing adaptive information and positive memories • increasing coping abilities, self-­‐efficacy and sense of mastery. That may result in reduction of the fear responses and enabling changes in the meaning of the experiences, and a new memory can be formed.

En esta presentación, queremos enfatizar la relación que existe entre apego, trauma y desarrollo de la red adaptativa de memoria (AMN). Desde un punto de vista psicológico, entendemos que una temprana experiencia relacional forma el cerebro y hace crecer la organización y consideramos que la principal influencia ambiental del desarrollo del cerebro es la relación de apego. Las reducciones en el tamaño del volumen del cerebro y las redes de memoria disfuncionales seguidas de experiencias traumáticas en la infancia están documentadas. Cuando existe un evento vital estresante, puede ser almacenado en una forma específica de estado, impidiendo conectar con otras redes de memoria que retienen la información adaptativa. La investigación de la neurobiología del cerebro social y el sistema de neuronas espejo, nos permite asumir que la AMN se desarrolla en presencia de un cuidador acostumbrado. Sanar recuerdos traumáticos es relacional y referente al procesamiento. Yo uso EMDR dentro del modelo-­‐fase del tratamiento para el trauma informado por el paciente. Tratamiento del modelo de fase para el trauma informado: Durante la preparación fase (fase 1 y 2 del protocolo EMDR) me gustaría recalcar la importancia de: -­‐ Evaluar el patrón de apego del niño. Que afecta en como el niño se relaciona con el terapeuta. -­‐ La activación de redes que contienen información adaptativa y recuerdos positivos. -­‐ Incremento de las habilidades de afrontamiento, autoeficacia y autocontrol. Esto puede conllevar una reducción de las respuestas de miedo e inhibir cambios en significado de las experiencias y puede llevar a la formación de un nuevo recuerdo.

Cet article traite de la transmission de traumatismes aussi bien transgénérationnels que vicariants en intégrant la notion de neurones miroirs. A partir du travail thérapeutique mené avec deux patientes présentant des séquelles post-traumatiques en lien avec des événements du passé qu’elles n’ont pas vécus, nous questionnons le concept de traumatismes transmis à la lumière de la notion de neurones miroirs. Nous terminons par une réflexion épistémologique sur le débat entre processus intrapsychiques et interactionnels en vue d’obtenir une lecture et une intervention thérapeutique intégrative.

This paper aims to reflect about transmitted trauma, as well transgenerational trauma as also vicarious trauma with the support of the concept of mirror neurons. Out of empirical therapeutic work with two persons who presented post-traumatic stress disorder related to historical dated events that they cannot have experienced, we reflect on transmitted trauma and its possible links with the concept of mirror neurons. We conclude with an epistemological reflection on the intrapsychic – interactional debate in order to join more integrated views and therapeutic interventions.

Introduzione: molti studi concordano sulla riduzione del volume ippocampale nei pazienti affetti da PTSD 1 e che tale alterazione anatomica è correlata con deficit cognitivi e con la gravità dei sintomi. Comunque ad oggi non è stato ancora chiarito se l’atrofia ippocampale rappresenta l’esito di un effetto neurotossico del trauma o, piuttosto, una condizione preesistente che predispone allo sviluppo di alcune patologie psichiatriche. Già da tempo studi su animali dimostrano come lo stress causi atrofia ippocampale e inibizione della neurogenesi, con meccanismi verosimilmente legati ai glucocorticoidi, all’increzione del fattore corticotropo, all’aumento degli aminoacidi eccitatori, all’inibizione fattore neurotrofico cerebrale con perdita della plasticità neuronale. Nell’uomo, tuttavia, i risultati non sono così lineari. Da un lato alcuni studi hanno individuato come fattore principale l’aumento dei glucocorticoidi 2, dall’altro tale teoria è stata fortemente criticata 3. Secondo Yehuda il meccanismo di atrofia ippocampale è dovuto ad un’alterazione dell’asse Ipotalamo-Ipofisi-Surrene (HPA), ma in termini di una bassa increzione di glucocorticoidi che determina un aumento del feedback negativo dell’asse stesso ed un’ipersensibilità recettoriale. Indipendentemente dal meccanismo d’azione, la perdita di neuroni a livello ippocampale nei soggetti che hanno subito eventi traumatici sembra sufficientemente dimostrata e, fino a poco tempo fa, era considerata irreversibile. In realtà l’ippocampo sembra presentare una inusuale e spontanea capacità rigenerativa. Questo dato è stato individuato in molte specie animali e, in un unico studio, anche nell’uomo 4. Inoltre recenti evidenze pre-cliniche e cliniche hanno indicato che gli SSRI (Selective Serotonin Reuptake Inhibitors) promuovono la neurogenesi e riducono l’atrofia ippocampale indotta dallo stress nell’animale 5 e nell’uomo sono in grado di ridurre i sintomi del PTSD, incrementare le dimensioni dell’ippocampo e ridurre i deficit mnesici tipici della patologia 6 7. Un altro fattore che sembra essere in grado di stimolare la neurogenesi negli animali sembra essere “l’ambiente arricchito” verosimilmente tramite i meccanismi molecolari dell’apprendimento che sembrano in grado di attivare la trascrizione dell’mRNA per il Brain Derived Neurotrophic Factor. Questo dato della letteratura supporta il razionale dell’efficacia della psicoterapia anche se l’unico studio che valuta le modificazioni morfostrutturali dopo psicoterapia non ha riportato risultati positivi 8. Gli scopi di questo studio sono: – valutare la presenza di atrofia ippocampale nei pazienti affetti da PTSD (T0-drug-free); – valutare l’effetto della terapia: farmacologica con SSRI e psicoterapica con EMDR (Eye Movement Desensitization and Reprocessing) sia sul piano clinico e neuropsicologico, che sul volume ippocampale, sia sulla memoria (T1). Metodologia: abbiamo analizzato un campione di 20 pazienti, di età compresa tra i 15 ed i 65 anni, reclutati nell’ambulatorio psichiatrico del Policlinico universitario di Siena affetti da PTSD e un gruppo di controllo di soggetti sani appaiati per sesso, età, peso e altezza. I soggetti di entrambi i gruppi sono stati sottoposti ad uno studio morfovolumetrico computerizzato dell’Ippocampo tramite RM (Risonanza Magnetica). Inoltre, i diciassette pazienti con PTSD sono stati valutati tramite la somministrazione di test neuropsicologici e scale psicometriche per approfondire il quadro psicopatologico e valutare l’eventuale presenza di deficit cognitivi. Nei soggetti affetti da PTSD dopo un periodo di sei mesi di terapia psicofarmacologica sono stati ripetuti i test neuropsicologici, le scale psicometriche e l’analisi morfovolumetrica dell’ippocampo tramite RM. Tre pazienti, dopo le valutazioni al T0, hanno effettuato un protocollo terapeutico con solo EMDR e sono stati rivalutati dopo 8 sedute (due mesi). Risultati: i risultati della prima parte sperimentale (T0-drug-free) evidenziano che le dimensioni dell’ippocampo di sinistra nei soggetti affetti da PTSD sono significativamente minori rispetto ai controlli sani. Dai risultati osservati al follow-up (T1-post-terapia) è possibile evincere che la terapia nei soggetti considerati è associata ad un miglioramento della sintomatologia e ad un aumento dei volumi ippocampali, pari al 9,87% per l’ippocampo di destra e dell’8,37% per l’ippocampo di sinistra. Questi dati sono concordi con i dati presenti in letteratura, anche se la percentuale di recupero su base neuroplastica nel nostro studio risulta sensibilmente superiore rispetto ai due studi presenti in letteratura incremento pari al 4,6% 6; pari al 5% 7. I tre pazienti che hanno effettuato terapia con EMDR hanno anch’essi mostrato al T1 un miglioramento sintomatologico (CAPS non più positiva per i criteri diagnostici) ed un aumento medio dei volumi ippocampali pari a 338,25 mm3 per l’ippocampo DX e 357,93 mm3 per l’ippocampo SN. Conclusioni: la terapia nei soggetti considerati si è associata ad un aumento dei volumi ippocampali (9,87%-8,37%). L’aumento dei volumi ippocampali appare rilevante, consistente con i dati in letteratura, sebbene quantitativamente superiore, sottolineando l’efficacia degli SSRI verosimilmente tramite il meccanismo di attivazione della neurogenesi; è ipotizzabile che l’aumento di volume non sia da imputare ad un aumento delle cellule gliali ma ad un aumento di neuroni ippocampali visto il contemporaneo miglioramento clinico. Particolarmente interessante ci sembra il dato relativo all’efficacia clinica e sulla plasticità neurale della EMDR. Questa osservazione su solo tre casi, necessita chiaramente di essere confermata su un campione più ampio ma rappresenta la prima evidenza in letteratura di un’azione della psicoterapia diretta alla struttura cerebrale.

Introduction: Many studies agree on the reduction of hippocampal volume in patients with PTSD and that an anatomical alteration is correlated with cognitive deficits and the severity of symptoms. However to date has not yet been clarified whether hippocampal atrophy is the result of a neurotoxic effect of trauma or, rather, an underlying condition that predisposes to the development of some psychiatric disorders. For some time animal studies show that stress causes hippocampal atrophy and inhibition of neurogenesis, by mechanisms probably related to glucocorticoids, all'increzione corticotropo factor, increased excitatory amino acid, inhibition of brain neurotrophic factor with loss of neuronal plasticity. In humans, however, the results are not so linear. On the one hand, some studies have identified as the main factor increasing glucocorticoid two other such theory was strongly criticized 3. According to Yehuda mechanism of hippocampal atrophy is due to an alteration of hypothalamic-pituitary-adrenal (HPA), but in terms of a low secretion of glucocorticoids leading to an increase of negative feedback axis and the same receptor hypersensitivity . Regardless of the mechanism of action, loss of neurons in hippocampus in people who have suffered traumatic events seems sufficiently established and, until recently, was considered irreversible. In fact, the hippocampus appears to be an unusual and spontaneous regenerative capacity. This figure has been identified in many animal species and in one study in humans 4. Moreover, recent evidence pre-clinical and clinical studies have shown that SSRIs (Selective Serotonin Reuptake Inhibitors) promote neurogenesis and reduce stress-induced hippocampal atrophy in animals 5 and humans are able to reduce symptoms of PTSD, increase the size of the hippocampus and reduce the deficit mnesic typical of the disease 6 7. Another factor that seems to be able to stimulate neurogenesis in animals seems to be "enriched environment" probably through molecular mechanisms of learning that seem able to activate the transcription of mRNA for Brain Derived Neurotrophic Factor. This finding supports the rationale of the literature of the effectiveness of psychotherapy, even if the only study that evaluates changes morphostructural after psychotherapy has shown positive results 8. The aims of this study are: - To evaluate the presence of hippocampal atrophy in patients with PTSD (T0-drug-free) - to assess the effect of therapy: pharmacological SSRI and psychotherapy with EMDR (eye movement desensitization and reprocessing) is a clinical and neuropsychological, and on hippocampal volume, and memory (T1). Methods: We analyzed a sample of 20 patients, aged between 15 and 65, recruited nell'ambulatorio Psychiatric University Hospital of Siena with PTSD and a control group of healthy subjects matched by sex, age, weight and height. Subjects in both groups were subjected to a computerized study morfovolumetrico dell'Ippocampo using MRI (Magnetic Resonance). In addition, seventeen patients with PTSD were assessed through administration of psychometric scales and neuropsychological tests to study the psychopathological picture and evaluate the possible presence of cognitive deficits. In subjects with PTSD after a period of six months of pharmacological therapy were repeated neuropsychological tests, scales psychometric analysis morfovolumetrica hippocampus by MRI. Three patients at T0 after assessments, carried out a treatment protocol with only EMDR and were reassessed after eight sessions (two months). Results: The results of the first experiment (T0-drug-free) show that the size of the left hippocampus in patients with PTSD are significantly lower compared to healthy controls. The results observed during the follow-up (T1-post-therapy) can be inferred that therapy in patients considered to be associated with improvement in symptoms and an increase in hippocampal volume, equal to 9.87% for the right hippocampus and 8, 37% for the left hippocampus. These data are consistent with the data in the literature, although the recovery rate based on neuroplastic in our study is significantly higher than in the two studies in the literature increase of 4.6% 6; 5% 7. The three patients who have treatment with EMDR have also shown an improvement in symptoms at T1 (CAPS no longer positive for the diagnostic criteria) and an average increase in hippocampal volume amounted to 338.25 mm3 for the hippocampus and DX 357, 93 mm3 for the hippocampus SN. Conclusions: Therapy in patients considered was associated with an increase in hippocampal volume (9.87% -8.37%). The increase in hippocampal volume appear to be relevant, consistent with the literature data, although quantitatively greater, stressing the effectiveness of SSRIs probably through the mechanism of activation of neurogenesis, it is conceivable that the increase in volume is attributable to an increase glial cells but an increase of hippocampal neurons seen the simultaneous clinical improvement. Seems particularly interesting given the relative clinical effectiveness of EMDR and neural plasticity. This observation on only three cases, clearly needs to be confirmed on a larger sample but represents the first evidence in the literature of action of psychotherapy directed at brain structure.

I limiti mentali autoimposti acquisiti da copioni familiari reiterati in età evolutiva e da modelli operativi interiori acquisiti dalle autorità societarie attraverso mezzi di comunicazione di massa e regole educativo-lavorative, inducono percentuali sempre più rilevanti della popolazione europea alla vulnerabilità psicopatologica. L’esordio delle sintomatologie psichiche e somatiche di varia entità, avviene già in fasi precoci, nella primissima infanzia e spesso già nelle fasi prenatali, e il limite di età tende percentualmente a essere sempre più sensibile fin dal primo mese di vita. In età scolare si manifestano situazioni comportamentali quali il cosiddetto “bullismo” e fenomeni con campionature rilevanti di sindromi ipercinetiche con deficit attentivo e disturbi del-l’apprendimento. In adolescenza il contesto si complica e gli attacchi di panico, le sindromi depressive e i disturbi alimentari psicogeni dilagano, fino all’esordio di disturbi post-traumatici da stress che si incrementano in seguito alle difficili scelte di orientamento universitario o lavorativo. La vulnerabilità dell’età adulta si manifesta con disturbi somatoformi di diversa natura, disturbi psicocardiologici, sindromi ansioso-depressive, attacchi di panico, disturbi di coppia e relazionali, per citare i più frequenti e limitandomi solo ad accennare l’esistenza della molteplicità di disturbi iatrogeni. In tali soggetti, la consapevolezza di essere indotti in stati di shock che incrementano molteplici disturbi somatoformi e psicopatologici è praticamente assente. Essi, come pazienti, si rivolgono agli specialisti in ambito sanitario con la convinzione, spesso indotta da propagande dei mass-media, che tutto sia solo genetico, e vada "curato" con farmaci per lunghi periodi se non per tutta la vita. Anche da parte degli operatori sanitari vi sono ampie aree di inadeguatezza metodologica: ad esempio, la gravidanza viene gestita come fosse una malattia, riducendo la donna partoriente a una paziente alla quale troppo facilmente si “consiglia” il parto cesareo (statisticamente tra i più frequenti in Italia!) come metodo “veloce e sicuro” di parto, togliendo la competenza materna dell’imprinting alla nascita del bambino con le conseguenze psicologiche che ne derivano per la relazione madre-bambino e per la crescita serena di quest’ultimo, e, sempre a titolo esemplificativo, ignorano quasi del tutto gli aspetti di psicocardiologia, e il loro intervento si riduce a esami medici invasivi e a somministrazione di farmaci. Eppure, la psicoterapia cognitiva costruttivista, e specificamente modelli teorici e tecniche strategiche specialistiche note come EMDR (Eye Movement Desensitization and Reprocessing), l’utilizzo di biofeedback, l’innovativo training emotivo-cognitivo-comportamentale da me ideato nelle due versioni per la psicoterapia e per i gruppi in formazione che incrementa il riconoscimento emotivo e l’implementazione di immagini mentali idonee a modificare cognizioni e comportamenti irrazionali, quando eticamente e competentemente applicati, fanno molto per questi pazienti, sia in quanto si incrementa sensibilmente il livello della loro consapevolezza e della loro capacità di farsi protagonisti nel-l'evoluzione positiva della loro “guarigione”, sia in quanto si può intervenire in modo mirato con sperimentati protocolli per la risoluzione dello stato di trauma psichico in tempi ragionevolmente rapidi e con risultati attendibili e verificabili. Passando dalla dimensione individuale a quella collettiva, ossia alla psicopatologia collettiva cagionata dall’esposizione (anche solo mediatica) ad eventi catastrofici o angoscianti (magari associati a senso di impotenza, insicurezza, precarietà) o a diversi tipi di stress e vulnerabilità, possiamo aggiungere che, analogamente, mediante un lavoro su sistemi di neuroni specchio e sull'attivazione di nuove connessioni di reti neurali con un modello operativo non invasivo, si potrebbe migliorare la condizione di intere popolazioni rispetto a disturbi che, oggettivamente, sono in continua diffusione. Sarebbe opportuno iniziare una sensibilizzazione collettiva partendo dalla formazione per livelli differenziati degli operatori educativi e sanitari, per poi estenderla alla popolazione suddividendola per fasce di età e per territori di appartenenza; purtroppo la consapevolezza non è tra le aspettative primarie di committenti rivolti solo al profitto economico. In un contesto storico-culturale dove l’etica, le relazioni umane, la cooperazione sembrano utopiche fiabe, questa è la sfida di essere una perturbatrice emotivamente orientata ad amplificare la consapevolezza attraverso un nuovo modello psicoterapeutico e formativo integrato, al quale sto lavorando da alcuni anni con risultati incoraggianti e che sarà mia premura esporre dettagliatamente durante il Congresso EMDR 2008.

The self-imposed mental limitations acquired from family scripts repeated age and developmental models inner acquired by the company operating through means of mass communication and educational and working rules, induce percentage increasing as the population of Europe vulnerability to psychopathology. The onset of symptoms of various psychological and somatic entity, is already in the early stages, in early childhood and often known during prenatal and age limit percentage tends to be more sensitive since the first month of life. Age school behavioral situations occur where the so-called "bullying" and phenomena samples relevant syndromes of attention-deficit and hyperactive disorder - learning. In adolescence the context is complicated and panic attacks, the syndromes psychogenic depression and eating disorders are rampant, until onset of post-traumatic disorders stress which increases as a result of difficult choices of university or business orientation. The vulnerability of adulthood is manifested by different types of somatoform disorders, disorders psycho, anxious-depressive syndrome, panic attacks, disorders of torque and relational to cite the most frequent is limited only to mention the existence of multiplicity of disorders iatrogenic. In these subjects, conscious of being led into a state of shock that increase multiple somatoform disorders and psychopathology is virtually absent. They, like patients, addressed to specialists in the health field with the belief, often driven by propaganda media, that everything is just genetic, and must be "cured" with drugs for long periods if not for life. Including by health workers there are large areas of inadequacy methodological: for example, pregnancy is managed as if it were a disease, reducing the woman in labor to a patient which too easily "advise" Caesarean (statistically the most frequent in Italy!) as a method of "fast and safe childbirth, removing the competence of imprinting the birth mother of the child with the psychological consequences that entailed for the mother-child and to the peaceful growth of the latter, and, also example, know little about the aspects of psycho, and their intervention reduces to invasive medical examinations and medication. Yet, cognitive psychotherapy constructivist theoretical models and specific strategic and technical specialists known as EMDR (eye movement desensitization and reprocessing), the use of biofeedback, the innovative emotional-cognitive-behavioral training which I designed in two versions for psychotherapy and groups in training that increases the emotional recognition and implementation of mental images likely to change, knowledge and irrational behavior, when ethically and competently applied, do a lot for these patients, both because it increases significantly the level of their awareness and their ability to get players in - the positive development of their "healing", both as it can intervene in a targeted manner with tested protocols for the resolution of the state of psychic trauma in the reasonably rapid and reliable and verifiable results. Moving from individual dimension to that collective, that is caused by exposure to psychopathology group (even the media) to distressing or catastrophic event (perhaps associated with the sense of powerlessness, insecurity, instability) or different types of stress and vulnerability, we can add that, similarly, through a work on systems of mirror neurons and activation of new connections of neural networks with a model operating non-invasive, it could improve the condition of entire populations than disorders that, objectively, are in constant circulation. It would be appropriate to start a collective awareness levels, starting from training differential operators' education and health, then extend it to the population divides by age and territories belonging unfortunately the awareness is not between the expectations primary principals addressed only in profit or loss. In a historical-cultural context where ethics, human relations, cooperation seem utopian fairy tales, this is the challenge of being an emotionally disturbing oriented to amplify the awareness through a new model of psychotherapy and integrated training, which I working for several years with encouraging results and that will spell out my readiness EMDR 2008 during the Congress.

Eating-disordered clients begin treatment with a series of unique and specific behavioral symptoms such as binge eating, bingeing and purging, starving themselves, or overexercising, that impede progress when using purely verbal therapies. The research team has found that the combination of expressive arts techniques and EMDR treatment can be highly effective in overcoming these impediments. This poster will briefly review basic knowledge and practice concerning the major eating disorders and their etiologies. We will then describe our therapeutic process that combines expressive arts with EMDR. This process includes (1) preparation of the client for EMDR treatment through the use of expressive arts techniques done in between sessions at home, (2) the use of client drawing and writing as targets when they appear to be a useful tool, (3) the use of a mirror exercise with BLS as a resourcing tool to address, desensitize and help the client reformulate body image distortion and body hatred, and (4) the reiniorcement of gains from reprocessing through the use of expressive techniques throughout treatment. This poster describes these additions to and modifications of the EMDR protocol with expressive arts techniques, as well as the results of a pilot experiment that compares outcomes for clients treated with expressive arts approaches, with and without EMDR.

This workshop will focus on the types of transference and counter-transference that arise in EMDR with dissociative clients and teach solutions. Procedural modifications have been the focus in dealing with pathological dissociation in EMDR treatment. Separately, transference and counter-transference with dissociative patients have been written about extensively by experts in the dissociation field. Research findings about the effects of mirror neurons and embedded simulation on the inter-subjective field of patient and therapist have also been published. Strategies for dealing with these transference and counter-transference in EMDR treatment have received little attention even though this population has intense transference, and can activate intense counter-transference. These issues may begin during an evaluation of the presenting problems. Strategies for identifying and using transference to enhance dual awareness during history taking will be demonstrated. An elongated preparation phase to develop enough trust and stabilization before exploring traumatic memories can limit induced transference. Different parts of a dissociative patient may have different kinds of transferences. These transferences may cause the patient to withdraw, cling or attack; affecting the therapist’s abilities to stay attuned and focused on the work in different phases of EMDR. Strategies of attunement to the activated part of the client will be demonstrated in order to repair or prevent ruptures of attunement. Interactions are bi-directional, and different (transferential) parts may activate dissociative parts of the therapist. Strategies to somatically identify and use these counter-transferential activations in the therapist will be taught through body based awarenesses. R/D/I strategies can be used to limit countertransference to remain grounded and attuned. Transference and counter-transference during the assessment phase will be identified and solutions presented. During the Desensitization phase under-accessing or over-accessing target memories; abreaction vs. vehement emotions will be discussed as unacknowledged dissociative moments with indications for inducing transference, counter-transference, or both. Decisions need be made collaboratively whether to process or contain these events. Understanding and dealing with dilemmas of dissociative enactments are crucial to keeping the healing process going. These inter-subjective issues may be most intense during the first four phases, but some problems may continue into Installation and the Body Scan. Problems and solutions during Incomplete Closure and the Re-evaluation phases will be given. Activated parts in the patient may cling or be angry with the therapist at the end of an EMDR session. Failure or defectiveness parts of the therapist may become activated as well. Solutions to these issues that occur during different phases will be taught so that participants will leave the workshop with additional strategies to use with their dissociative patients. Attunement to dissociative parts, identifying transference and counter-transference binds; The Clinician Self Awareness Questionnaire ; Compartmentalization; use of self soothing skills; using Relational, Empathic, and Transferential Interweaves; identifying moments of projective identification and enactments, and then to use them to deepen EMDR will be taught, as well as innovative inter-subjective strategies . Case examples and awareness exercises will used throughout the workshop to facilitate intellectual and experiential learning.

R/D/I strategies will be reviewed and applied to the therapist to limit countertransference activations. Dealing with dissociative enactments are crucial to identify ruptures to the therapeutic alliance, restore attunement and resonance, uncover dissociative messages that can be used during EMDR processing.. Solutions to the problems that occur during different EMDR phases will be taught using lecture, discussion, case examples, written and experiential exercises so that participants will leave the workshop with additional strategies. Solutions include how to maintain attunement to dissociative parts during transferential activations while enhancing dual awareness; how to identify transference and countertransference problems during phases 1-4 and 7-8 and use them as additional sources of dissociated communications that can be used in EMDR processing ; how to use the Clinician Self Awareness Questionnaire to identify and process countertransference problems ; how to use compartmentalization strategies using R/D/I to limit countertransference activations; how to develop self soothing skills for the therapist’s dissociated parts; how and when to use Relational, Empathic, and Transferential Interweaves during Desensitization; and how to identify moments of enactments, and using EMDR strategies to deepen the EMDR experience .

L’EMDR viene applicato sui ricordi dei momenti più traumatici, sulla situazioni attuali che causano ansia e disagio e su quelle future che creano ansia anticipatoria e angoscia. Il trauma della diagnosi di cancro quindi deve essere affrontato per primo, per evitare che il paziente si ritrovi costantemente bloccato a pensare all’idea della morte. In questo modo si può facilitare e favorire il processo che porterà ad intraprendere la via della guarigione.

The first objective to establish the patient is as follows: to transform its role from being a victim of uncontrollable events (illness experienced as a conviction that comes from) to an active player of his life (I will do everything possible to cure disease). Another important objective is to resolve symptoms related to PTSD. After treatment, EMDR, the patient is no longer blocked at diagnosis and the fear of not succeeding, but with a more aware and stronger associated with the feeling that I can do. Post-traumatic stress disorder (PTSD) is a condition that can be produced by traumatic experiences. A new study has shown that a brain scan called magneto encephalography (MEG) can identify people who have PTSD with an accuracy of 95%. Sensors measure tiny magnetic fields generated by currents flowing in and around neurons. In addition they have recently stated in the Journal of Neural Engineering that they can now watch the brain as it experiences PTSD. Imaging shows that the brain becomes hyperactive in the right temporal lobe, the location responsible for memory. Besides diagnosing PTSD, the researchers also are able to judge the severity of how much patients are suffering. Eye-movement desensitization and reprocessing (EMDR) was designed in 1987 as a treatment for PTSD. EMDR is a structured eight-phase therapy that allows for adequate reprocessing of dysfunctionally stored memory. In the processing phases the client attends to the disturbing memory in brief intervals of 15-30 seconds while also experiencing bilateral stimulation (eye movements, tones or tapping). The eight phases integrate effective elements of psychodynamic, imaginal exposure, cognitive therapy, interpersonal, experiential, physiological and somatic therapies. Now that we can locate specific biomarkers for PTSD using MEG, my hypothesis is that we will find a statistically significant difference between the control group and the group that has EMDR treatment, and that EMDR will be shown to be effective in resolving PTSD as measured by pre and post therapy MEG scans. Also using the MEG, we may be able to observe those brain actions responsible for the therapeutic efficacy of EMDR and isolate which components of EMDR trigger those brain actions. The significance of finding the answer to these questions could potentially help millions of people overcome years of suffering from psychological pain due to the after effects of severe trauma and restore them to productive lives. It could establish the status of EMDR, assisting in the decision of whether it should be listed among the evidence-based, best-practice therapy modalities and covered by insurance. Also knowing the underlying pathophysiology could contribute to the evolution, revision and refinement of diagnostic constructs for PTSD.
