Abstract:
Working with severely damaged clients we can hardly expect EMDR-processing to go smoothly by itself. ‘Just staying out of the way’ is not enough to help these clients process their traumatic material. A more active approach is called for to bring trauma resolution. In EMDR this type of intervening is called ‘(cognitive) interweaving’. The therapist may need to intervene on the process level with process interweaves, in order to keep the client within his window of (affect)tolerance.Or the therapist has to intervene on the content level with content interweaves, when the client gets stuck in a specific problem is unable to solve with the information he has available. In this workshop the participant learns to use process interweaves to modulate the arousal level of the client. In addition to this a toolkit of useful content interweaves is presented, cognitive and non-cognitive, e.g. sensory, physical, humorous, symbolic and spiritual interweaves. The presentation includes metaphors for all kinds of blocks, bits and pieces of psycho-education for specific stuck points, unorthodox, creative or hilarious interweaves to get round resistance, and interweaves to bring reconciliation and forgiveness.

This workshop addresses the application of standard and modified EMDR treatment protocols to addictive and compulsive behaviors including substance abuse/dependence, overeating, smoking, love addiction. Individuals with addictive and compulsive behaviors frequently have suffered from childhood trauma and neglect resulting in developmental arrests, as well as a variety of maladaptive behaviors which are trauma-related and serve to minimize pain. The successful implementation of EMDR to addictive behaviors requires that EMDR be used as part of an overall treatment program carefully addressing the needs of individuals who have been traumatized and are exhibiting addictive behaviors. A thorough diagnostic work up is needed aimed at assessing comorbidity, dissociation, and a detailed trauma history covering childhood traumas and traumas suffered as adults including traumas that occur as a consequence of addictive behaviors. Careful client preparation is essential to assist individuals in coping adequately with the high levels of emotion experienced during EMDR Clients' readiness to stop compulsive/addictive behaviors needs to be carefully evaluated. A decision tree aimed at determining the appropriateness of EMDR to individuals diagnosed with addictive behaviors is presented which assists clinicians in minimizing the premature use of EMDR. EMDR is a client centered method, and thus, careful pacing is needed with this population to reprocess underlying traumatic issues. This frequently implies utilizing a modified EMDR treatment protocol with only partial resolutions of underlying traumatic material. Guidelines will be discussed to assist clinicians in selecting EMDR targets for optional results which relate to the stages of recovery. EMDR can be used at all stages of recovery to neutralize the negative impact of memories contributing to problematic behaviors, such as urges to use, ambivalence about treatment, fear of facing painfull feelings from the past. EMDR also has the power to install templates for future actions which assist individuals with skill deficits in more rapidly acquiring necessary skills for a successful recovery. Examples of cognitive interweaves are presented which take into consideration clients' readiness, as well as the need to accelerate the recovery process. EMDR has a unique role in the recovery of traumatized individuals with addictive and compulsive behaviors since the accelerated processing of negative experiences and the installation of positive adaptive cognitions assist clients in more rapidly overcoming barriers throughout the recovery process. It also challenges rigid approaches to recovery which frequently stress that trauma work should not be attempted before abstinence has been accomplished for a specified period of time. EMDR is especially valuable in processing core issues which center around shame and manifest in cognitions, such as "I am defective," "There is something wrong with me," "I am not good enough," "I am not quite right," "I don't belong," "I don't deserve to live." Case examples will be given as to how such core issues can be targeted to accelerate the recovery process. A.J. Popky has developed a specialized EMDR treatment protocol which targets levels of urges of addictive/compulsive behaviors directly and installs a positive internal state of feeling empowered without relying on compulsive and addictive behaviors. Case examples fiom clinical practice indicate that when levels of urges are targeted directly, underlying traumas frequently emerge without increasing clients' usage. The symposium addresses the application of this protocol to a range of addictive and compulsive behaviors. The Wades' integrative psychotherapy combines ego-state therapy and EMDR in a psychosocial developmental context. Their substance use disorders treatment program incorporates specialized applications of their integrative psychotherapy, which includes both individual and group therapy and employs hypnosis as well as EMDR Their presentation focuses on applications of the standard EMDR protocol in individual therapy, which is limited primarily to desensitization of dysphoric affect and reprocessing negative cognitions associated with grief and trauma. Their conceptual framework of substance use disorders proceeds from a goal of reducing the harm caused by substance use and a primary distinction between functional and autonomous use (rather than the DSM conceptualizations of "dependence" or "abuse") because this guides interventions. Initial treatment planning depends upon external constraints (e.g., lack of support for positive change, hostile environment), internal limitations (e.g., severity of substance use and its effects, neurocognitive deficits, inadequate "ego strength," lack of skills, disrupted psychosocial development, psychological trauma) and the nature of the substance use disorder (i.e., functional, autonomous, or both). Methods include education about substance use disorders and processes of change, group therapy to develop skills and obtain feedback and support, individual therapy to correct disrupted development and resolve traumatic stress reactions, and exercises to apply what is learned in real-life situations. The standard EMDR protocol is applied to disrupted development involving grief and to resolve psychological trauma that lead to substance use. Case vignettes in which such applications of the standard EMDR protocol were employed are presented in detail.

First, the influence of the migration context is considered with regard to the possible sickening role of the asylum procedure, the burden for immigrant women who have undergone female genital mutilation and now live in a society that seriously condemns it, and the dilemma of Islamic migrants who experience homosexual feelings while living in a secularized society. Following are some contributions on cultural diversity in the expression of trauma symptoms, like somatization among refugees and the articulation of rage and embitterment among labor migrants. The impact of trauma in combination with migration on families and how children can be affected by the traumatization of their parents is the topic of two contributions in which the role of the (extended) family is explored and a contextual approach to working with families with trauma-related symptoms (a mentalization based multifamily therapy) is described. The factors that impede therapyseeking behaviour and drop-out are discussed in a study on Moroccan or Turkish girls who were raped. Finally, the applicability and efficacy of evidence-based interventions for treating ethnic minority patients with posttraumatic stress disorder is illustrated by a study on Eye Movement Desensitization and Reprocessing with refugees and an article on specific elements of intercultural trauma therapy that appear to be culturally sensitive.

Structural dissociation alienates from body reactions. Nevertheless it is the body that "holds" the discomfort and painful memory of neglect and violence. Trauma Survivors tend to perceive their body as hostile. They suffer from Alexithymia deficiency of interpreting the meaning of body reactions and muscle activation. Trauma Survivors are easily irritated and tend to react with rage on very slight provocations and freeze when they are frustrated. Even minor problems cause fear and helplessness. The Polyvagal Theory (S. Porges 2010) proves the neurological aspect of behavioral patterns. Neurozeption describes how we perceive others in a neurological way. Certain behavioral patterns are established through life experiences. This research underlines Francine Shapiros AIP model and confirms the importance of a body orientated approach. We know that experiencing the effect of eye movement -­‐ and other bilateral stimulation, is a gentle and powerful way to bring the voice of the body into the therapeutic space. EMDR helps to integrate cognitive, emotional and body sensations. De EMDR therapeut dient daarvoor actief te zijn in het bepalen van het optimale tempo van het verwerkingsproces gedurende de EMDR zitting. Dit is van belang om toegang te kunnen krijgen tot de eerder gedissocieerde kennis, gedragsmatige impulsen, gevoelens, en/of sensaties, en deze te kunnen blijven verdragen. De therapeut moet in staat zijn de signalen van disregulatie (bv hyper/hypo-arousal, bevriezen, dissociëren) te herkennen en door middel van specifieke interweaves erop in te kunnen spelen, om de cliënt in het proces te houden en te helpen met het verwerken van diverse aspecten met betrekking tot verantwoordelijkheid, veiligheid en keuze. Deze keynote zal een kader neerzetten voor het werken met complexe PTSS en disregulatie. When clients with early, chronic trauma treated by EMDR, the therapist often as a kind of 'psychobiological regulator "function, to satisfy itself that its customers remain within their Window of Tolerance. The EMDR therapist is therefore to be active in determining the optimal pace of the process during the EMDR session. This is important in order to be granted access to the previously dissociated knowledge, behavioral impulses, feelings, and/or sensations, and to continue to bear. The therapist should be able to dysregulation of signals (e.g. hyper / hypo-arousal, freezing, dissociate) to recognize specific interweaves through it in order to respond to the client in the process to keep and help in processing various aspects of responsibility, security and choice. This keynote will provide a framework drop for working with complex PTSD and dysregulation. Some specific techniques designed to circumvent these therapeutic pitfalls in chronically traumatized clients, will be briefly discussed. This will be clarified by means of recordings of sessions in which EMDR is from moment to moment interactions between therapist and client will be analyzed. The one-day workshop on the Monday after the congress will be much more detailed presentation on the 'how, what, when and why questions "in the treatment of Complex PTSD.

Cognitive interweaves are strategic interventions, whose utilisation relies upon therapist recognition of their necessity at given times during the EMDR procedure, and their effective use requires skilled therapeutic judgement and the introduction of the appropriate clinicianled cognitive material in order to counteract the blocked processing. The use of cognitive interweaves therefore diverges from the established EMDR protocol by introducing therapist generated material rather than just following the clients spontaneously processed material. It will be argued in this presentation that, due to the divergence from the established EMDR protocol, the effective implementation of cognitive interweaves, requires the knowledge of additional therapeutic strategies, predominantly those used in Cognitive Behaviour Therapeutic (CBT) approaches, which may not be readily accessible to EMDR practitioners not trained in this approach. This presentation aims to bridge the gap between the traditionally 'too-heavily protected' territories of CBT and EMDR, by introducing some CBT strategies, such as 'Socratic Questioning (Padesky, 1993)' or the Cognitive 'Separation of the Past from the Present (Herbert & Wetrnore, 1999'), which might be usefully drawn on, in order to generate effective cognitive interweaves during the use of EMDR. The Instituto de Psicotrauma de Alicante and the Social Services of San Juan use, since 2008, a brief intervention protocol for minors exposed to gender violence. A critical incident is any situation that causes unusually strong emotional reactions that have the potential to interfere with a person's ability to function immediately after the incident or later. These are situations that overwhelm a person's sense of vulnerability and/or control. A critical incident stress debriefing (CISD) is a psychoeducational group meeting or discussion about a traumatic incident which ideally takes place within 72 hours of the event. The goals of a CISD are to mitigate the psychological impact of a traumatic event, prevent subsequent development of a post-traumatic syndrome, accelerate recovery, and serve as an early identification mechanism for people who need further follow-up, including EMDR. The steps of a CISD include: 1) introduction - to introduce the intervention team, explain the process, and set expectations. 2) fact - to describe the event from each participant's perspective on a cognitive level. 3) thought - to allow participants to describe cognitive reactions and to transition to emotional reactions. 4) reaction - to identify the most traumatic aspect of the event for participants. 5) symptom - to identify personal symptoms of distress and transition back to the cognitive level. 6) teaching - to educate as to normal reactions and adaptive coping strategies 7) reentry - to clarefy ambiguities and prepare for termination; access for follow-up. In the opinion of the authors, the CISD facilitates the processing of the traumatic information before it becomes crystallized in dysfunctional form. EMDR can be very effective shortly following a CISD, and is particularly usehl for participants who are experiencing distress or intrusive symptoms after the CISD. The CISD structure helps the participant understand the traumatic impact of the incident and provides support and guidance toward adaptive resolution. The EMDR process begins where the CISD leaves off. The CISD helps to delineate the traumatic image, negative cognition, and emotions associated with the event, making the subsequent EMDR process more efficient. EMDR appears to have a very powerful and rapid effect after the CISD, perhaps, because of the initial processing. In other words, the CISD initiates an adaptive processing of the traumatic information; EMDR completes it. EMDR can be implemented individually immediately following the CISD, or the next day. While the CISD is a group process, EMDR is an individual method. EMDR can be explained during the teaching phase of the CISD or after the CISD to the whole group, but EMDR treatment is done individually and privately. EMDR can go beyond a CISD in targeting previous traumas that may underlie the current incident, delve deeper into the meaning of the incident for the person, and target specific stimuli that are relevant to the individual (e.g. Smells, tastes, etc.). The workshop will discuss the application of EMDR to critical incidents. The protocol for recent events will be reviewed. Guidelines for negative and positive cognitions will be discussed. For example, a critical incident usually involves issues of responsibility ("Is it my fault?"), Safety ("Am I safe?"), And/or control ("Do I have choices in life?). It is important that such dynamics are understood when formulating the negative cognition. The dynamics of fear, a framework for understanding a critical incident and resolving issues of vulnerability and powerlessness, will be presented. The model discusses the importance of going beyond defining the moment of peak stress to elucidating subsequent thoughts, actions, and decisions. The implications for cognitive interweaves will be discussed.

While EMDR is treatment of choice for traumatized adults with PTSS, its efficacy has not been validated with traumatized refugees. As long as no evidence is available for EMDR with this population, EMDR with refugees should be informed by theoretical approaches to treatment of PTSS in adults. Four approaches are discussed: the phased approach that advocates stabilization before EMDR; the multimodal approach that advocates combining EMDR with other interventions; the trauma-focused approach that advocates offering EMDR to all adults with PTSS, including refugees and, the transcultural approach that advocates a culturally sensitive administration of EMDR. Several pilot studies on EMDR with traumatized refugees, randomized controlled trials of EMDR with multiply traumatized populations, and a trial of EMDR in a non-western setting may further inform EMDR-therapists working with traumatized refugees. Suggestions are made for combining the four approaches and scientific evidence in treatment planning and execution of EMDR with this population. The EMDR protocol itself prescribes several interventions for treating multiply traumatized patients, such as resource development and installation, clustering of traumatic experiences, and cognitive interweaves. Additionally, culturally sensitive interventions may enable the refugee to actively partake in the treatment process by increasing motivation for trauma processing, by diminishing language barriers, and by facilitating the formulation of culturally congruent meanings to trauma. The question remains whether EMDR, which offers an individualistic approach to trauma, is sufficiently suited to address the collective traumatization of victims of war and organized violence.

This workshop introduces 'dyadic resourcing,' a form of resourcing designed to facilitate the processing of very early trauma with severely deprived clients, including those with attachment disorders. The goal of this process is to help a client connect affectively to the experience of being in a nurturing relationship Through this process clients experience both roles, the role of the adult who loves them and the role of the child who is lovable and loved. These roles become increasingly real to them and clients come away with access to a loving non-judgmental view of themselves as a child. Clients whose original trauma was a result of or exacerbated by a lack of a strong connection to a nurturing caregiver will benefit from a variety of resources, but the resource that is essential is access to a secure internal nurturing relationship, which this process provides. This procedure is particularly useful for clients who think they were bad or worthless as children, who think the abuse or neglect they suffered chronically was deserved, who are overwhelmed by the intensity of their pain from early childhood experiences, or who cannot view their child selves in an accepting nurturing way. In other words, this type of resourcing is ideal for some of the most difficult EMDR clients, and helps to prepare them for trauma processing. Once developed, these resources allow the EMDR clinician to utilize cognitive interweaves in which the adult client is able to support the child self. Dyadic resourcing is typically a five step process: identifying a nurturing adult resource, make the resource real for the client, formulating a parent-child relationship involving the resource, intensify the client's experience of that relationship, and helping the client to have the experience of both the child and adult in the resource dyad. This workshop will address each of these steps, covering the basic principles and processes central to this form of resourcing. The process will be illustrated using clinical videos, transcripts, and a live demonstration. Currently migration has reached extensive proportions. Globally, we assume a hundred to two hundred million migrants (WHO). Due to civil wars, natural disasters, political and economic circumstances the true figure is likely even higher. So it increasingly occurs that psychiatrists and psychotherapists encounter in their treatment individuals from other cultures, presenting with various psychiatric symptoms. The treatment of these people will sometimes face major difficulties. Not only because of language but also because of the relative importance of certain symptoms in a cultural context, it is essential to consider culture background, working milieu and disorder specific psychiatric/psychotherapy. But what does cultural and milieu sensitive psychiatry or psychotherapy mean? Based on the existing studies about people with immigrant backgrounds and the investigations of the Socio-Vision-Institute, and our own experiences we will elucidate how an intercultural meeting and a milieu sensitive psychotherapy can be successfully applied.

In this workshop, participants will be taught to use their “true” authentic selves as a resource during EMDR processing, and to work to create a secure, responsive, and positive relational environment that supports change and integration. There are special considerations when treating children for critical incidents, anxiety, or other "EMDR amenable" conditions. Especially challenging for EMDR practitioners, young children may not be able to verbalize their thoughts, feelings, or beliefs children old enough to understand treatment options may choose to keep their symptoms rather than experience temporarily increased anxiety during treatment. Even cooperative children may not be able to identify a positive cognition because their life experience and/or cognitive development have not yet permitted resources for self-soothing or making sense of life changing events. Furthermore, children are dependent on an adult or family for their physical safety and emotional wellbeing. Although the child may be the "identified patient," the parents' own post-traumatic beliefs may be triggering the child's symptoms, and a successful outcome for the child may depend on the parents' reprocessing of traumatic material. This presentation reviews research which investigated the idiosyncratic effects of sexual abuse perpetrated by Roman Catholic Priests and makes related treatment recommendations. Eye-movement desensitization and reprocessing (EMDR) was introduced by Shapiro (1989) as a cognitivebehavioural therapy for clients with trauma a little over 20 years ago. After an initial period of intense controversy and scepticism regarding its proclaimed evidence base, EMDR has been studied extensively, its efficacy has been confirmed (Van Etten & Taylor, 1998; Shepherd et al. 2000, Alto, 2001; Davidson & Parker, 2001; Maxfield & Hyer, 2002; Bisson & Andrew, 2005; Bradley, et.al., 2005; Bisson, Ehlers, Matthews, Pilling, Richards, & Turner, 2007) and is one of the effective treatments of PTSD recommended by the National Institute of Clinical Excellence (NICE) of the UK Department of Health in March 2005. It has been estimated (Farrell & Keenan, 2010) that in the United Kingdom and Ireland approximately 6,000 mental health clinicians have been trained in EMDR. Although studies have evaluated EMDR as a distinct therapeutic modality, during the course of their different levels of EMDR training, clinicians are encouraged to integrate the EMDR treatment protocol into their predominant therapeutic orientation. Thus, EMDR can be used across different psychological therapies, including the Cognitive Psychotherapies. While this makes EMDR a highly versatile modality, it can pose a practical challenge to clinicians in terms of when and how to integrate EMDR into their work with clients. Further, for therapists not trained in EMDR, the concepts may seem strange and scepticism may remain to this date. This workshop seeks to close the gap between false perception and reality about EMDR, by drawing on the facilitator’s 14 years of practical experience in the use and integration of EMDR alongside her work as a Cognitive Behavioural Psychotherapist. This practice-oriented workshop will explore different applications of EMDR across the trauma spectrum, as well as, some treatment protocols for other client problems. The important roles of resource installation and interweaves will be introduced. Definition of client population: Disorder of the self. The narcissistic character is often identified by his or her grandiose facade concealing an underlying sense of emptiness and worthlessness. To experience the underlying emptiness is so painful for them that these people cut off their inner experience and rely instead on external admiration and praise to support their grandiose or superior view of themselves; their condition is often referred to as a "disorder of the self." Because of their dependence on others for their sense of themselves, they are exquisitely sensitive to criticism or disapproval, often warding off deflation by becoming increasingly grandiose, superior, disdainful or demeaning. Many conceal their grandiosity, maintaining a secret sense of superiority which may be based upon their perfectionism or their quiet devaluing of others. View of others: there is a range of severity of the narcissistic character from personality disorders to a narcissistic character style. People with personality disorders, are unable to form a trusting bond with another person; they view people primarily as interchangeable, performing a function which could equally be performed by many others. Less impaired narcissistic characters, however, are able to form varying degrees of attachments to other people, although their ability to trust and care about other people is limited. Most commonly they relate to people they can idealize or be admired by. People tend to be seen in extremes as either superior and powerful or inferior and worthless; supportive and admiring or critical and attacking. Difficulties in using EMDR: Clinically these clients represent a difficult and often frustrating population to treat; they are brittle and easily injured if they to not feel perfectly understood by their therapists, and they will distance at the slightest hint that they are being judged or used. They resist focusing inward and defining their problems as arising within themselves, and find it difficult to sustain any genuine affect, other than perhaps rage. If they feel understood and accepted, however, they will eventually talk about their sense of emptiness and worthlessness and their confusion about who they are and what is truly meaningful and valuable to them. Beck, Young and others have described factors that make it difficult to treat any personality disorder using a cognitive behavioral approach. There have in fact been very few reported "successes" in the literature. Most of the difficulties are related to the self and object splitting characteristic of these clients. These clients have limited access to feelings, limited access to spontaneous thoughts, body sensations, memories, etc., and vague unfocussed presenting problems making targeting difficult. They usually have difficulty with emotional and often intellectual continuity from session to session; they will rarely keep a log or follow through with homework; transference issues often come into central focus and must be addressed before other targets; and their selfdefeating beliefs and behavior patterns are extremely deeply held, pervasive and resistant to change. In addition to varying degrees of these difficulties, narcissistic clients present all of the problems in EMDR that they do in more traditional therapies; including their tendencies to act out, deny, and avoid. These and other defenses interfere with completion of segments of therapeutic work and make it difficult for the clinician to keep work focused within one neural network. Self and object splitting leads to continuity problems within or between sessions and a difficulty maintaining clarity about the reason for being in treatment. The client may feel suicidal one week and declare himself or herself to be fully recovered the next. Perhaps the most confirming aspect of the treatment of these clients, however, is their emotionally impoverished pasts; they have very limited experience of nurturing, loving and caring to draw !?om in order to interweave new meanings and perspectives into traumatic or painful past experiences. Length of treatment: I have found that I have been able to achieve good results with higher level narcissistic clients with whom I have had an established relationship at the time I introduced EMDR into the treatment. My results with clients who have come to therapy asking specifically for EMDR and with whom I have begun using EMDR soon after the beginning of treatment have been generally poorer, varying with the severity of the client's disorder, the less severe doing best. The client needs to be able to establish a meaningful trusting relationship with the therapist; the more severe the client's difficulties with attachment, the more time this process requires. Narcissistic clients do not tend to see their difficulty with vulnerability, trust and intimacy as a problem within themselves. If they are able to recognize personal problems, they are usually in the area of self esteem and obstacles to achievement. When they are able to resolve some of these latter problems fairly rapidly through treatment they tend to terminate, no longer seeing a sufficient purpose for treatment. In a sense, they can become better narcissists; their grandiose view of themselves is enhanced and they are reinforced in their use of self-sufficiency as a defense against interpersonal vulnerability. I view the relative efficiency of EMDR as a problem for deeper treatment of narcissistic clients because there is less time for the therapeutic relationship to develop and consequently a limited opportunity to impact the client's object splitting. I believe that this is why I have found EMDR with narcissistic clients to be most effective when it is introduced after a therapeutic relationship has had time to develop. Negative cognitions: The early maladaptive schemas of narcissistic clients are pervasive in their lives and point to a plethora of negative cognitions. Typical early schemas are: I must control myself (or my feelings, my behavior, my body) at all times; no one cares; my needs will never be met; I can't trust anyone; I am deeply flawed and unlovable; I am dikeable, unattractive to others; I will always fail; my flaws are totally unacceptable to others; I must be perfect or I am worthless; I deserve to be treated more specially than others; I must please others to avoid attack; I'm alone; nobody understands me; I am OK if I am better than others; I am OK only if others admire me. It is often helpful to narrow these cognitions down to make them manageable with EMDR Treatment: In addition to the recommended protocol of establishing a safe space to which the client can retreat if necessary, before doing an EMDR session with one of these clients, the therapist should identify as many of the client's emotional resources possible, in particular expriences if any of having felt loved and accepted ad examples of loving people or relationships the client has observed Among other things, these facilitate more effective copitive interweaves. The initial task in doing an EMDR session with this client population is to establish an appropriate and richly defined target. Since it is more difficult for these clients to access meanm&l memories in an emotionally alive way, the therapist must be more active in helping the client stimulate the associated neural netork as I l l y as possible. In addition to the client's reaction to the plight of children he may be related to or observe (Level I1 training), a rich source of emotional responsiveness and resources is the client's own response to situations he has witnessed in news media, TV, movies or theater. A major challenge in addressing a narcissistic character type using EMDR is tracking the course of the session with these clients and identifying when they drip out of the targeted neural network. This process can be subtle because it requires an ability to differentiate true avoidance hm spontaneous associations which may appear at hat to be irrelevant; it requires a familiarity with and sensitivity to the protective or defensive mechanisms they use to insulate themselves fiom painful memories and affect. As the patterns ofmovement in and out of the targeted network are identified it is important to use interventions that are experienced by the client as supportive but nevertheless make hun or her aware of having wandered. Although the narcissistic client may initiate treatment with the stated goal of improving his performance in specified areas, he will agree upon reflection that the real problem is that he feels an overriding need to perform in order to feel worthwhile. Since he has never known any other way of dealing with his self-worth, he will be skeptical about whether it is possible to feel a sense of worth that is not based upon performance, and it is easy for the therapist to lose perspective and join him in that beliet especially while doing EMDR with its potential for reprocessing with extraordinary precision specific obstacles to performance. EMDR is a powerful integrative psychotherapeutic intervention. However, in the case of disrupted attachment and significant dissociative symptoms EMDR can be destabilizing if used early on in treatment. That is, fragmentation of self and dissociative symptoms commonly interfere with information processing, thus barring the integration and resolution of the traumatic experience through EMDR. Dissociation interferes with clients sense of their own body, their ability to experience emotion, their capacity for emotional regulation and their sense of self. Addressing dissociative symptoms prior to proceeding with EMDR treatment is essential for positive treatment outcomes. A neurobiological model is described that guides therapeutic interventions and integrates diverse approaches that include not only EMDR and relevant target selection, but also mindfulness, body therapy approaches, ego-state interventions, sensory integration, as well as neurobiologically based interventions. Such interventions can be used both in the preparation phase but can also form useful interweaves during EMDR information processing. Using a neurobiologically informed approach, the case is made for the use of somatic and ego-state interventions when dissociation is a significant part of the clinical presentation. Specific focus is on different ego-state and body therapy interventions to increase awareness of the self and ones body. Body therapy and somatic interventions are distinguished from other psychotherapeutic interventions in that they are expressed in markedly slowed-down time, in order to give clients ample time to experience the felt sense of their bodies. Shame is often a key component of post-traumatic stress and one that can inhibit processing because the person concerned feels no compassion for the self that was shamed. Shame may lead to despising or hating that self so that allowing the self to grow, to recovered, feels almost impossible. Shame is experienced cognitively, emotionally, and somatically: in “brain, heart, and body.”

This paper will draw on clinical work with people who have experienced rape, torture and organized violence and explore ways of unlocking the inhibiting factors of shame, enabling the victim of personal violence to have compassion for themselves, and forgiveness. With compassion, a person can allow themself to recover, and processing the memories of the traumatic incident or incidents can move to adaptive resolution.
The paper will present case material using the adaptive information processing model as a helpful way of enabling clinets to normalize their mental, emotional and somatic reactions, to structure what often seems like a chaotic inner world and to address issues including shame.
This paper will propose a protocol for EMDR psychotherapy with people who have been victims of rape, torture, and organized violence and will introduce a “protocol interweave” for working with people for whom shame is a factor that impedes effective processing.

Procedural modifications have been the focus in dealing with pathological dissociation in EMDR treatment. Separately, transference and counter-transference with dissociative patients have been written about extensively by experts in the dissociation field. Research findings about the effects of mirror neurons and embedded simulation on the inter-subjective field of patient and therapist have also been published. Strategies for dealing with these transference and counter-transference in EMDR treatment have received little attention even though this population has intense transference, and can activate intense counter-transference. These issues may begin during an evaluation of the presenting problems. Strategies for identifying and using transference to enhance dual awareness during history taking will be demonstrated. An elongated preparation phase to develop enough trust and stabilization before exploring traumatic memories can limit induced transference. Different parts of a dissociative patient may have different kinds of transferences. These transferences may cause the patient to withdraw, cling or attack; affecting the therapist’s abilities to stay attuned and focused on the work in different phases of EMDR. Strategies of attunement to the activated part of the client will be demonstrated in order to repair or prevent ruptures of attunement. Interactions are bi-directional, and different (transferential) parts may activate dissociative parts of the therapist. Strategies to somatically identify and use these counter-transferential activations in the therapist will be taught through body based awarenesses. R/D/I strategies can be used to limit countertransference to remain grounded and attuned. Transference and counter-transference during the assessment phase will be identified and solutions presented. During the Desensitization phase under-accessing or over-accessing target memories; abreaction vs. vehement emotions will be discussed as unacknowledged dissociative moments with indications for inducing transference, counter-transference, or both. Decisions need be made collaboratively whether to process or contain these events. Understanding and dealing with dilemmas of dissociative enactments are crucial to keeping the healing process going. These inter-subjective issues may be most intense during the first four phases, but some problems may continue into Installation and the Body Scan. Problems and solutions during Incomplete Closure and the Re-evaluation phases will be given. Activated parts in the patient may cling or be angry with the therapist at the end of an EMDR session. Failure or defectiveness parts of the therapist may become activated as well. Solutions to these issues that occur during different phases will be taught so that participants will leave the workshop with additional strategies to use with their dissociative patients. Alternative to violence has developed a group treatment model structured by the theory of structural dissociation and EMDR trauma treatment theory. Woman with ethnic minority background received short terms group treatment at a shelter for victims of domestic violence at an outpatient clinic and at a domestic violence family treatment center. The groups were supplements to individual therapy/counseling. We have had 10 groups; one with only Pakistani women, several mixed ethnic minority cultural groups with translation and groups in “simple Norwegian.” Recruitment was enhanced by the policy of sharing of symptoms and problems today with no obligation to share about personal past. The model has low drop out rate and therapist working with the individuals report more effective treatment sessions. For some women the group becomes the preferred choice of treatment. We discovered that early phase trauma work can be done in a group format with severely and recently traumatized women. Methods used are resource installation and safe place work, increase awareness of negative/positive cognitions, butterfly hug, nightmare protocol, expressive art therapy techniques as grounding, breathing techniques working with personal borders, working with imagination and playfulness. Structural therapy of dissociation concepts as ANP/EP structures and mental capacity, working from here and now, focusing on the ANP above EP's are woven into how the therapists regulate the group process and plan content. The theory organizes how we handle flashbacks, current acute crisis and how we focus on the womens’ personal trauma. We also teach about the effect of violence in relationships, the need to work on personal safety and the needs of children in the aftermath of violence. Theory from the field of intercultural communication gave us guidelines in working with women from high context, indirect and collectivistic cultures. A workbook for the clients on violence, PTSD symptoms and stabilisation treatment has been developed in the aftermath of these groups and is translated into several languages. We will present the material at the conference in the structure of the early fase trauma treatment group format. Showing in vivo how we apply the theory to severely traumatized women. We will share some of our favorite group exercises, metaphors and group rituals. Our goal is: 1. to show how the theory of structural dissociation serves as guideline for organizing and resulting treatment with severely traumatized clients in groups. 2. Give insight into typical adjustments that have been made to tailor treatment to ethnic minority populations. 3. Explain how expressive art work needs to make adjustments to the population of severely traumatized women. 4. Finally show how the group uses elements from EMDR and enhances individual EMDR work. In our experience, the stabilisation groups have integrated the heart, mind and body in the work of healing with a population that is often found difficult to treat effectively. We hare started to retain other therapists in using the model and are in the process of applying for a research grant.

The use of ego-state interweaves and/or extensive ego-state therapy (utilized in the preparation phase) and integrated into EMDR targeting (phases 3-7), in the treatment of personality disorders, has not received a great deal of attention at conference presentations or in the published media. This has led to minimal or nonresponsiveness in the EMDR treatment of personality disorders, since aspects of dissociation in these clients have not been addressed. In the past ten years, renown neuroscientists, such as Eric Kandel, Joseph LeDoux, Michael Gazzaniga and V.S. Ramachandran, in response to empirical findings in the fields of memory, neuromodularity, split-brain research and information processing, have begun to suggest that the “self” may very well be a collection of memories that are structured in a fragmented (neuromodular) multiplicity that is developmentally inherent. Ramachandran, LeDoux and Gazzaniga have, each, stated, explicitly, that the sense of a “cohesive and monolithic” self is an “illusion” created by areas in the left cerebral hemisphere. Accordingly, just as the EMDR standard protocol was adapted for recent traumatic events (in response to acute memory fragmentation), so must it be modified for inherent memory and personality fragmentation, by the use of extensive ego-state work (preparation) and ego-state-specific EMDR targeting (phases 3-7). The implementation of these techniques has shown a remarkable advance in the treatment of personality disorders, which had, previously, been rather impervious to EMDR treatment.

Given the prominence of cognitive distortions and avoidance of affect in OCD, and the impact of EMDR on such phenomena as exhibited in the literature about EMDR treatment with PTSD, Whisman believes EMDR has the potential to be quite beneficial in the treatment of OCD. Today, the dreaming mind is understood to improve learning, organize novel solutions to problems from waking consciousness and, thus, creatively support survival. This function, of finding new creative solutions, is also observed in the desensitization phase four of the EMDR protocol and is a key component in Ego State Therapy (EST). Dreams appear to select targets from the client’s history, that are causing problems in current time. In addition, dreams may respond to interventions made during a therapeutic session, as if the dreaming mind were in a dialogue with the client and the therapist. This type of dream leads to a more creative dialogue in subsequent therapy. Repetitive nightmares are the dreaming signature of PTSD, and these too respond well to EMDR and rehearsed changes in imagery, including that of ego states and can result in improved sleep quality. This hour and a half presentation addresses the use of cognitive and imaginal interweaves in the treatment of adult survivors of sexual abuse. The overall course of treatment with EMDR is briefly outlined including a variety of interweave interventions for use in the beginning, middle and end of EMDR sessions. In working with sexual abuse survivors with EMDR it is important to understand the issues commonly encountered in their treatment. These include issues of safety, trust, responsibility, choice/control, interpersonal relationships, body awareness and image, sexuality and self esteem. A sexual abuse assessment can be taken which includes information on the perpetrator(s), severity and frequency of abuse, type of abuse, age of onset of abuse, duration of abuse, disclosure and family response. Sexual abuse survivors present themselves in treatment in different ways. Some clients come to treatment remembering abuse and want to clear it with EMDR. Other clients come to treatment with no clear memories of incidents but have a "feeling" something happened to them and have symptoms of abuse. There are clients who have no clear memories but something has triggered flashbacks and nightmares of sexual abuse. Finally, there are clients who have no memory of abuse and come to therapy for another reason but uncover what they believe to be sexual abuse memories with EMDR. There are three phases of treatment in sexual abuse cases. In the beginning phase, a history is taken and there is the establishment of a trusting relationship. The client is prepared for EMDR. In the middle phase, there is the reprocessing and working through of traumatic memories and transference work. In the end phase of treatment there is integration of the information which has been uncovered and preparation for life outside of therapy. Interweaves can be utilized in the beginning, middle and end of EMDR sessions. In the beginning of individual EMDR sessions there is a check-in with clients to see how they have been doing during the week. What has come up for them in their dreams or daily life since the last session? Next there is the selection and development of targets for EMDR (body sensation, memory, flashback, symptom, dream, feeling, vague sense, negative cognition or drawing). A safe place is then established where the client can go at the beginning, middle or end of the session as needed. Along with the safe place an inner advisor or other inner resources can be contacted and developed for use in sessions. A connection with the client's inner child is important which can be done through the use of guided imagery, photographs and/or artwork. Instructions on how EMDR will be used are given with attention paid to issues of safety and control (they are in control, they can stop at any time, they can return to the safe place, they know the signal for stop). Negative and positive cognitions are established along with the EMDR protocol. In the middle of individual EMDR sessions there are commonly problems with looping or being "stuck." This seems to occur frequently with sexual abuse survivors because of the intensity of the trauma and because the child self is often frozen in time lacking access to the adult self's information. Ways to work with this include looking for the blocking beliefs (i.e., The perpetrator can hurt me), look for blocking images, and talking to the child part (what does he/she need?). Imaginal and cognitive interweaves can be used in a variety of different ways in the middle of EMDR sessions. Some of these include: imagining the adult self helping the child self in the traumatic scene, bringing in inner and outer resources for help (i.e., a powdl imaginary being, a strong loving fiend, the therapist, etc.), and reality check interweave where is the perpetrator now?, can helshe hurt you now?) It is also important to educate the child part that his or her feelings are normal, sexual feelings are normal etc. It can be helpful to ask the adult self to talk to the child self explaining things to the child. Another useful interweave is to have the adult self hold the perpetrator and allow the child to beat him or her up or have the adult self beat up the perpetrator allowing anger to be expressed safely. Asking clients if they would like to return to the safe place for a break can also be helpful if they are feeling too overwhelmed. There are a number of ways to end or close incomplete EMDR sessions. Often it will not be possible to completely clear a traumatic memory in a session or the memory worked on is completed but connected to a whole network of other traumatic events. For these cases there are a number of interweaves that can be used. Clients can be requested to have the adult self comfort the child self in the . safe place. The client can imagine putting the scary unfinished disturbance that has been uncovered in a file folder, box, safe, leave it in the therapist's office, etc. The client can return to the safe place where the child and adult selves can play together. The adult can comfort the child or do whatever is needed to create safety and containment. Clients can imagine their child self being held by protector figures repeating cognitions related to safety, responsibility and choice. They can also be asked what they learned from the session, installing their response with eye movements. It is helpful to give homework to clients such as journaling, artwork, walks in nature, meditation, stress reduction, group work, exercise, nutritious diet, and restriction of drugs and alcohol. Loving Kindness or Metta Meditation is another very helpful tool for teaching self soothihg to adult survivors of sexual abuse.

Trauma experienced within the earliest attachment relationships leave children and adults at great risk for the development of psychiatric disorders. Maltreatment by attachment figures and traumatic losses are both closely associated with attachment disorganization, the attachment category identified in 70% of patients in psychiatric hospitals. Research shows insecure and disorganized attachments to be transmitted transgenerationally at a rate of between 70 and 80%.Adults and children with disturbed attachments frequently experience severe emotional dysregulation along with intense feelings of despair, anxiety, shame, and mistrust of others. Affected children and adults frequently lack helpful or adaptive information or insights and exhibit behaviors that elicit negative responses from those around them. This may result in mothers developing “Peri-Natal Post Traumatic Stress Disorder” (P.N. P.T.S.D.) (Slade 2006). Conversion disorder, trauma and major life events are closely linked. While we can not speak of 'cause', there is much in tracking patients experienced an accumulation of stressors and traumas before the emergence of the first symptoms and / or in childhood. The relationship between conversion disorder and trauma makes it worth EMDR as part of treatment to consider. We offer a workshop where we share our experiences, we have gained in EMDR with that audience. We lost their "unexpected" phenomena to, during and between sessions, such as fainting, loss of function and vibrate / shake. Are these complications or we can take as part of the process? When all you have is 90 minutes with the hysterical and/or traumatized client in times of crisis, efficient planning and conceptualization of EMDR casework is essential.
Using EMD(R) as a means of reducing immediate exacerbated stress is effectively enhanced with a genogram format that is brief, and specifically focuses on learned generational reactions to trauma. By tapping into the trans-generational information the processing in crises is facilitated and enhanced.
This suggested case conceptualization and genogram format quickly highlights inherited strengths and resources which is the client’s historical legacy. The client gains perspective recognizes that survival and overcoming the crisis is possible and acquires insight into valuable resources.
Following a crisis, when EMDR therapist and client first meet, the 1st stage is the retelling of the event.
After the story has been told, the EMDR therapist begins to gather historical family information. This has the effect of moving the client one tiny step away from crisis and calming the individual sufficiently for the therapist to begin to assess resources, and potential resources within the family system,
Additionally, the EMDR therapist finds resources for more meaningful and relevant cognitive interweaves while at the same item protecting himself from vicarious traumatization, compassion fatigue, and burn out. The suggested case conceptualization and multi-generational genogram use, uncovers inherited small t and large T trauma material and anxieties as well as leaned dysfunctional behavior patterns which are compounded over time, and which become a source of exacerbated present crisis reactions.

Many children who are adopted due to difficult early circumstances exhibit severe and challenging behaviors due to a history of attachment trauma and loss. This presentation will describe an EMDR Integrative Team Treatment of attachment trauma in children. Abstract: Working with traumatized clients from other cultures can be very demanding, and even frustrating: many times it just does not work out, despite your compassion and efforts. “Just staying out of the way” many times is not enough to do this job.
Intercultural competence is the ability to expand and translate your therapeutic skills to other cultures. It has both an attitude aspect and a technical aspect: a different style of relating and communications with your client, and different things to ask and explore.
Also in other cultures traumatic events happening to people have a different meaning asking for an approach from another angle in doing EMDR: interpreting traumas a stupid bad luck, Allah’s will, the evil eye, karma, or punishment by the ancestors, will have different (therapeutic) consequences.
