This workshop addresses the application of standard and modified EMDR treatment protocols to addictive and compulsive behaviors including substance abuse/dependence, overeating, smoking, love addiction. Individuals with addictive and compulsive behaviors frequently have suffered from childhood trauma and neglect resulting in developmental arrests, as well as a variety of maladaptive behaviors which are trauma-related and serve to minimize pain. The successful implementation of EMDR to addictive behaviors requires that EMDR be used as part of an overall treatment program carefully addressing the needs of individuals who have been traumatized and are exhibiting addictive behaviors. A thorough diagnostic work up is needed aimed at assessing comorbidity, dissociation, and a detailed trauma history covering childhood traumas and traumas suffered as adults including traumas that occur as a consequence of addictive behaviors. Careful client preparation is essential to assist individuals in coping adequately with the high levels of emotion experienced during EMDR Clients' readiness to stop compulsive/addictive behaviors needs to be carefully evaluated. A decision tree aimed at determining the appropriateness of EMDR to individuals diagnosed with addictive behaviors is presented which assists clinicians in minimizing the premature use of EMDR. EMDR is a client centered method, and thus, careful pacing is needed with this population to reprocess underlying traumatic issues. This frequently implies utilizing a modified EMDR treatment protocol with only partial resolutions of underlying traumatic material. Guidelines will be discussed to assist clinicians in selecting EMDR targets for optional results which relate to the stages of recovery. EMDR can be used at all stages of recovery to neutralize the negative impact of memories contributing to problematic behaviors, such as urges to use, ambivalence about treatment, fear of facing painfull feelings from the past. EMDR also has the power to install templates for future actions which assist individuals with skill deficits in more rapidly acquiring necessary skills for a successful recovery. Examples of cognitive interweaves are presented which take into consideration clients' readiness, as well as the need to accelerate the recovery process. EMDR has a unique role in the recovery of traumatized individuals with addictive and compulsive behaviors since the accelerated processing of negative experiences and the installation of positive adaptive cognitions assist clients in more rapidly overcoming barriers throughout the recovery process. The symposium addresses the application of this protocol to a range of addictive and compulsive behaviors. The Wades' integrative psychotherapy combines ego-state therapy and EMDR in a psychosocial developmental context. Their substance use disorders treatment program incorporates specialized applications of their integrative psychotherapy, which includes both individual and group therapy and employs hypnosis as well as EMDR Their presentation focuses on applications of the standard EMDR protocol in individual therapy, which is limited primarily to desensitization of dysphoric affect and reprocessing negative cognitions associated with grief and trauma. Their conceptual framework of substance use disorders proceeds from a goal of reducing the harm caused by substance use and a primary distinction between functional and autonomous use (rather than the DSM conceptualizations of "dependence" or "abuse") because this guides interventions. Initial treatment planning depends upon external constraints (e.g., lack of support for positive change, hostile environment), internal limitations (e.g., severity of substance use and its effects, neurocognitive deficits, inadequate "ego strength," lack of skills, disrupted psychosocial development, psychological trauma) and the nature of the substance use disorder (i.e., functional, autonomous, or both). Methods include education about substance use disorders and processes of change, group therapy to develop skills and obtain feedback and support, individual therapy to correct disrupted development and resolve traumatic stress reactions, and exercises to apply what is learned in real-life situations. The standard EMDR protocol is applied to disrupted development involving grief and to resolve psychological trauma that lead to substance use. Case vignettes in which such applications of the standard EMDR protocol were employed are presented in detail.

This article describes a comprehensive model that identifies key target of memories for the treatment EMDR. This has led to a greater emphasis on the use of groups and on the development of more accessible information.

Conduct disorder and other disruptive behaviours represent the most common form of child and adolescent psychopathology referred for treatment by parents and teachers. A number of studies conducted in university research centers have demonstrated meaningful treatment gains especially with preadolescents with conduct disorder. However, studies conducted with older adolescents, especially those with comorbid disorders, as well as studies conducted in community-based clinic settings have been mostly unsuccessful. With a few exceptions, the extant treatments have not made any special effort to address effects of traumatic experiences that research has demonstrated to be very prevalent among conduct disordered children and adolescents. Motivation-Adaptive Skills-Trauma Resolution (MASTR) therapy developed by Ricky Greenwald (2002a) has shown promising results in his open trial study. MASTR is a complex treatment approach which addresses several key areas crucial in treating adolescents with conduct disorder: development/enhancement of motivation for treatment, development/enhancement of anger management and problem solving skills, and treatment of past trauma effects. The present study evaluated the effectiveness of MASTR therapy with 10 adolescent boys with conduct disorder placed in a residential treatment facility. This study combined single subject and qualitative research methodologies to offer a detailed look into the implementation and evaluation of MASTR therapy in a residential treatment centre. Foremost, this study demonstrates the formidable problems of conducting treatment research with this population in this kind of setting. Results suggested that, when it can be sufficiently implemented, MASTR therapy may be an effective treatment method producing some meaningful changes in behaviour for some severely disturbed adolescents with conduct problem. The study also demonstrated that EMDR may be an effective treatment for reducing emotional distress associated with past trauma in adolescents with conduct disorder. Clinicians who have grown to accept and support Eye Movement Desensitization Reprocessing (EMDR) have begun to diversify the types of trauma populations in which EMDR is applied. Psychology of injury researchers have suggested, on the basis of their work, that an exciting new direction in sport psychology is the implementation and testing of new interventions aimed at modifying risk factors for athletes. It has also been suggested that extant models of athletic injury may reasonably be re-interpreted to account for other traumatic stressors, additional to injury, in sport. EMDR may reduce stress and trauma reactions in sport participants. EMDR, however, has been developed as a clinical tool and there are limitations on entrance to training in the approach. There are myriad ways, however, in which valuable partnerships may be formed, among psychologists, sport psychologists, and educational sport psychology consultants to use EMDR on behalf of clients. This symposium, within its five sections, will report on many such collaborations. The following are the objectives of the session: (a) provide a brief overview of the research and theory base for EMDR and its use in performance work; (b) describe case reports of successful partnerships among EMDR-trained sport psychologists and variably trained professionals from sport performance.

EMDR has demonstrated effectiveness in treating chronic PTSD and old trauma memories, yet Early EMDR Intervention (EEI) protocols have not received sufficient attention from EMDR researchers or clinicians.
As part of a comprehensive approach to EEI, this workshop presents the Recent Traumatic Episode Protocol (R-TEP), which is an integrative protocol that incorporates and extends existing EMDR protocols within a new conceptual framework, together with additional measures for containment and safety. The application of the R-TEP will be presented with video case illustrations as well as a report of its utilisation in a mass disaster situation.
Intervening with EMDR in mass disasters has proven to give a significant contribution to this field. During the workshop the structure of an intervention in the acute phase will be described. Recent developments have been seen in the earthquake that hit northern Italy earlier this year, where EMDR was the most widely used approach and utilised with more than 2000 survivors. Epidemiological data and measured changes in post-traumatic stress before and after EMDR will be presented and practical guidelines for implementation of EMDR in the acute and chronic phase of trauma after a mass disaster outlined.

Abstract:
Objective.- This dissertation explored the effectiveness of a treatment package, Motivation-Adaptive Skills-Trauma Resolution (MASTR) in combination with Eye Movement Desensitization and Reprocessing (EMDR). This intervention was assessed in a sample of traumatized adolescents manifesting conduct problems (CPs) admitted to youth protective services. CP adolescents have been found to be particularly treatment-resistant and the treatments used with them often neglect to target the trauma that many of these youths have faced. Therefore, it seemed promising to implement a trauma-focused treatment with these youths that accounts for their resistance to treatment. MASTR-EMDR was studied with this population due to the favorable findings in the few studies assessing its use with high-risk populations. In addition to examining the effects of this treatment with CP youth exposed to various types of trauma, a particular focus was given to victims of sexual abuse (SA). This type of trauma seemed particularly suited for EMDR due to its circumscribed nature, which may be more easily worked through in this treatment that targets one trauma at a time.
Method.- Participants in the first study were 40 adolescents (ages 13-17) exhibiting CPs and exposed to trauma in youth protective services. A subsample (n = 30), consisting of victims of SA, was included in the second study. Participants in both studies were randomly assigned to MASTR-EMDR treatment or to a wait list condition where they were offered routine care. Self-report questionnaires and semi- structured interviews were administered to participants and one of their parents or caregivers by independent evaluators at three points in time: pre-treatment, post-treatment (12 weeks later) and follow-up (12 weeks after post-treatment). These measures evaluated trauma history, trauma-related sequelae, CPs, social competence and internalizing problems. The MASTR-EMDR sessions were administered once a week over a 12 week period, with each session lasting a maximum of 1.5 hours.
Results.- ANCOVAs and repeated measures ANCOVAs were used to assess treatment effects and the maintenance of gains at a 3-month follow-up. As predicted, MASTR-EMDR led to significant gains in outcome measures compared to routine treatment with both samples. In addition, gains were maintained at follow-up.
Conclusions.- This dissertation supports the use of MASTR-EMDR in populations exposed to general trauma and SA who exhibit CPs. This research was innovative in its implementation of a novel treatment-approach in youth protective services, where empirically-supported treatments are necessary and sometimes lacking. Therefore, the results have both clinical and scientific value and can help pave the way toward more trauma-focused treatments for CP youth, more evidence-based practices in youth protective services as well as enrich current understanding of the effects of this treatment approach.[Author Abstract]

Dies wird oft wie ein Riß im Selbstverständnis oder wie ein Zusammenbruch des persönlichen Weltbildes beschrieben. Plötzlich wird die Welt als bedrohlich erlebt. Der Betroffene fühlt sich schutzlos und ausgeliefert. Typische Symptome, die in der Folge von belastenden Ereignissen auftreten, sind Schlafstörungen und Alpträume, quälende Wiedererinnerungen, Ängste und situationsbezogenes Vermeidungsverhalten sowie erhöhte Schreckhaftigkeit und Konzentrationsstörungen. Unter Zugrundelegen der Diagnosekriterien der Posttraumatischen Belastungsstörung nach DSM-III-R liegt die Lebenszeitprävalenz in den USA für beide Geschlechter bei 7,8 Prozent bis 12,3 Prozent, wobei etwa doppelt so viele Frauen betroffen sind wie Männer (5). Für die deutsche Bevölkerung gib es noch keine epidemiologisch gesicherten Prävalenzzahlen, aber die Bedeutung von "Traumatisierungen" für die Entstehung oder für die erhebliche Verschlechterung psychischer Störungen wird immer deutlicher. Ein Beispiel hierfür ist die lebhafte Diskussion in den Medien über die psychischen Folgen von Traumatisierungen für Unfallopfer und Rettungskräfte in der letzten Zeit. Die empirischen und klinischen Befunde zu Traumatisierungen in der Kindheit haben Egle, Hoffmann & Joraschky jüngst in einer Monographie zusammengestellt (1). Danach ist die Rolle von Vernachlässigung, Mißbrauch und Mißhandlung für eine Reihe von psychischen Störungen wie Selbstverletzendem Verhalten, Borderline-Störungen und Dissoziativen Störungen mittlerweile unstrittig und scheint auch für Subgruppen von Patienten mit Eßstörungen, Angststörungen, Persönlichkeitsstörungen und Somatisierungsstörungen von erheblicher Relevanz zu sein. Weit unterschätzt ist zudem die Häufigkeit von Posttraumatischen Belastungsstörungen infolge von Unfällen oder Einsätzen in Krisengebieten. 20 Jahre nach dem Vietnamkrieg leiden noch immer ca. 15 Prozent aller Vietnamkriegsveteranen an einer PTSD. Opfer von Gewaltverbrechen und Überfällen sowie auch Zeugen von Gewalttaten, wie z.B. Rettungspersonal und Feuerwehrangehörige, stellen eine weitere Risikogruppe für die Entwicklung einer PTSD dar.

Not since the train wreck of Eschede and the related media reports, is increasingly known to the public that psychological trauma can lead to a deep and persistent breach of the feeling of personal safety and invulnerability. This is often described as a crack in the self or as a breakdown of the personal worldview. Suddenly the world is experienced as threatening. The person concerned feels defenseless and delivered. Typical symptoms that occur as a result of stressful events are insomnia and nightmares, distressing recollections, fears and situational avoidance behavior and increased nervousness and difficulty concentrating. Usually these symptoms are back within a few days to a few weeks. But they remain psychological and psychosomatic disturbances exist, so with a presence of more than 3 months is a post-traumatic stress disorder to diagnose. In recent years, research on consequences of trauma was significantly intensified. It gained new insights that have contributed to the psychotherapeutic method for the treatment of traumatized people could be greatly improved. For four years, the Department of Psychosomatic Medicine and Psychotherapy, MHH treats people with post-traumatic stress disorder in a trauma clinic and explores the psychological and biological effects of trauma. Epidemiology According to results of a large number of epidemiological studies, post-traumatic stress disorder (Post Traumatic Stress Disorder, PTSD short) as a result of trauma in childhood or in later life, a decrease in frequency and socio-economic importance of long underestimated disease. Cognitive interweaves are strategic interventions, whose utilisation relies upon therapist recognition of their necessity at given times during the EMDR procedure, and their effective use requires skilled therapeutic judgement and the introduction of the appropriate clinicianled cognitive material in order to counteract the blocked processing. The use of cognitive interweaves therefore diverges from the established EMDR protocol by introducing therapist generated material rather than just following the clients spontaneously processed material. It will be argued in this presentation that, due to the divergence from the established EMDR protocol, the effective implementation of cognitive interweaves, requires the knowledge of additional therapeutic strategies, predominantly those used in Cognitive Behaviour Therapeutic (CBT) approaches, which may not be readily accessible to EMDR practitioners not trained in this approach. In deze lezing willen wij vooral aan de orde stellen hoe de behandeling van het daderschap zich verhoudt tot de behandeling van het slachtofferschap van de patiënt. We willen hiervan zowel de inhoudelijke als de ethische kant belichten. Van belang hierbij is ook de discussie rondom de indicatiestelling: richt men zich eerst op het daderschap of juist op het slachtofferschap? Verder willen we de gelaagdheid van de behandeling benoemen: de behandelaar heeft niet alleen te maken met de problematiek van de patiënt maar ook te maken met de gesloten kliniek en verschillende functionarissen daarin. Die gelaagdheid heeft direct gevolgen voor de vorm en inhoud van de behandeling.
Als hypothese willen we naar voren brengen dat de behandeling van trauma’s essentieel kan zijn voor de behandeling van persoonlijkheidsproblematiek. Schemagerichte Cognitieve Therapie (J. Young), sluit in onze ogen naadloos aan op EMDR. Beide stromingen richten zich op dezelfde type existentiële “Ik ben ….“ opvattingen.

This lecture is about the implementation of EMDR In a very complex (TBS) of institution, and security are inextricably intertwined treatment, with a heavily loaded diagnostic target: • Multiple personality disorder • Multiple severely traumatized • Addiction Problems • Specific psychiatric problems • Forensic issues • Combinations
The TBS system of treatment (with security as an important point) does sometimes difficult to EMDR treatment of individual patients. Fear of the manageability of a patient during treatment as may be considered a contraindication for EMDR treatment. The question is whether EMDR is contraindicated in a TBS clinic for security reasons. The embedding of treatment in the TBS clinic and the use of self-control techniques are important factors. Complicating factor is that many TBS patients, both self-traumatized as well have committed serious crimes with which they have traumatized others.
In this talk we mainly discussed how the treatment of the perpetrator is related to the treatment of the victimization of the patient. We wish them both the substantive and ethical angles. Of importance here is the discussion surrounding the needs assessment: the focus is first on the perpetrator or at the very victimization? We also want to appoint the stratification of treatment: the therapist has not only faced with the problem of the patient but also to do with the clinic closed and several officers therein. Such layering has direct implications for the form and content of treatment.
As a hypothesis we want to highlight that the treatment of trauma may be essential for the treatment of personality problems. Schema-Focused Cognitive Therapy (J. Young), close our eyes seamlessly with EMDR. Both schools focus on the same existential type "I am ...." opinions.

The Indonesian Province of Nanggroe Aceh Darusalam (NAD) is a region which is facing a unique set of problems, among which is the protracted internal conflict, exacerbated by the tsunami on December 26, 2004. These events have generated a widespread impact on the lives of the communities. One of the most crucial issues to be addressed aside from legal, security, social and economic problems is the matter of health, including mental health. In regards to mental health issues, comprehensive steps have been formulated into various mental health care programs. One of the most needed programs is establishment of an educational system rooted in Indonesia for the treatment of the posttraumatic stress syndrome (PTSD) of victims of crises and catastrophes through the implementation of specific methods of treatment with a focus on the introduction of EMDR. With great support from BMZ- TDH-Germany, HAP-Germany and Trauma Aid, capacity building on EMDR training has been developed. Even though EMDR is highly effective as trauma healing therapy it is also a complex treatment to be addressed in this specific population like in the province of NAD. Further than time constrain, limited numbers of trauma therapist available and high numbers of severe cases that urgently need to be treated, complexities also arises from cultural and religious aspects. The society in NAD is marked by decade long isolation, violent conflicts for political self-determination and the strict interpretation of the Islam. The Sharia (doctrine of the Islam including moral and judicial duties) was introduced as part of the laws. Due to this condition, for the time being stabilization technique in EMDR is the most common technique that can be of widely used. In this presentation, varieties of stabilization technique that have been used in this population will be addressed. More specifically, as culturally adjustable method in therapy, this presentation will also introduce several culturally acceptable stabilization techniques such as combining religious rituals (chanting, reciting) as personal resource with stabilization technique. These techniques might be useful for other population with similar culture and religion.

This presentation will encompass the findings from a current and ongoing research study on EMDR with young children, with implications for clinical practice arising from this study. The clinical experiences of the presenter, which include treating traumatized children and training EMDR therapists, led to the first EMDR fidelity study on children. From that study, our preliminary findings led us to formulate suggestions about training therapists; these ideas will be explained in the workshop.
Examples will be discussed of how issues related to the therapist, client, and patient, home environment, clinical environment, and therapist training all impact the EMDR treatment protocol with children 20 to 10 years of age. Participants will also learn to identify developmentally appropriate and child-specific languaging in order to conceptualize the treatment of children using the EMDR protocol. Using Dr. Shapiro’s Adaptive Information Processing model, participants will learn to attune to the child verbally and non-verbally to understand how the child has learned to store the trauma in their memory network, versus how the parent or therapist believes the trauma to be stored. Specific tools like mapping and graphing that are used to tease out all the pieces of the EMDR protocol and develop case conceptualization will be demonstrated with associated videos. Through the use of Powerpoint presentation, case presentation, and handouts, additional practical and interesting tools will be presented to assist therapists in using Resource Development, Mastery, and Safe Place exercises in the efficacious treatment of young children. Creative tools used to identify targets, emotions, body sensation, and negative and positive cognitions, will be demonstrated, as well as measurements to aid the child in eliciting the VOC and SUDs. Also, the important of the three-pronged approach (the process of addressing targets from the past, present, and future), and how to develop targets from a child’s often concrete perspective, will be discussed. Finally, participants will be able to use a specific format for reevaluation from both the child’s and parents’ point of view. With these advanced skills in translating EMDR into developmentally appropriate terms and imaginative tools for implementation, participants will return to their practices encouraged to use the entire EMDR protocol with even the youngest of clients. The workshop, which is based on clinical experience and research, will teach creative skills in applying the eight-phase protocol to young children.

EMDR is an innovative and rapidly expanding new, treatment technique. Therapists are provided with two weekends of intensive training and encouraged to obtain supervision and be active in peer consultation groups and networks, and work to refine their skills. It is uncertain what percentage follow through in this regard. Estimates of the numbers of people who are victims of sexual trauma in our society are staggering (one in 3 girls, one in seven boys). Research finds that approximately half the people who present for psychotherapy have some sexual trauma in their history. EMDR has been hailed as an important new technique in facilitating the treatment of those who have been sexually traumatized. With correct usage, EMDR can greatly facilitate the treatment. At the same time, sexual trauma is a complex and volatile issue and awkward, poorly timed, or overly simplistic usage of EMDR could potentially lead to adverse consequences for the patient and treatment. As with any therapeutic technique or paradigm it can only be as helpful as that of the overall treatment. In addition the mere usage of EMDR will have an impact on the therapy, as well as the therapeutic container, and therapists need to be cognizant of what that impact will be as well as to be sure that they know how to incorporate the patient reactions into the treatment in a positive way and not allow them to undermine, or otherwise detrimentally effect both patient and therapist primary therapy goals. Particularly in these times of increased litigation, malpractice suits, and professional grievances against therapists those professionals utilizing EMDR would be wise to be aware of the particular risks inherent in the patient population in which they work, as well as the inherent vulnerability of utilizing a newly developed technique. In treating sexual trauma many experts agree that the crucible of the therapeutic container- is whether the healing will occur if the therapist sucessfully deals with the challenges the patient will offer lip. Again how those utilizing EMDR negotiates those challenges may be he difference between a successful course of therapy and a disaster. Finally, working with sexual trauma is an emotionally, intellectually, and sometimes physically demanding undertaking. Many therapists do not fully realize or acknowledge the toll that this type of work exacts and may be blind to the countertransferentia1 responses which arise and how they are communicated to the patient. In this workshop we will first reveal, some of the current thinking on the primary treatment issues (and obstacles) in therapy with victims of sexual trauma. We will then examine how and when is the most propitious time to use EMDR with this population and what reactions patients are likely to have to this type of intervention. Specific ways that EMDR and its implementation may activate certain issues in sexual trauma victims be elucidated as well as strategies for addressing those issues. Finally participants will engage in a series of experiential exercises designed to heighten their awareness of their own personal reactions and feelings (i.e., countertransference) to working with the intensity of sexual trauma. Once again these potential reactions will be linked to more or less effective usage of EMDR.

Autism is characterized by impairment in information processing, with direct consequences for the AIP in EMDR as a result. This core impairment in autism strongly influences attachment development, relation to and communication with others, sense of self, relation to the own body and thus to bodily senses, functioning of the sensory system (hypo or hyper), view on the world, development of cognitive schemata and development of the stress system. People with autism are easily traumatized, even by 'small-­‐small-­‐t events'. Often a single trauma leads to complex traumatization. Because PTSD symptoms mimic symptoms of autism, 'diagnostic overshadowing' is a frequently seen phenomenon. This is even more the case in clients with some form of intellectual disability. In this workshop several cases of people with autism treated with EMDR will be presented and discussed, with the use of abundant video material: cases in which treatment is influenced by the autism, as well as cases in which the diagnosis 'autism' no longer fits after treatment of PTSD symptoms, or the other way round when a client referred for PTSD shows up in the course of treatment as having an autism spectrum disorder. Autism has consequences for all phases of the 8 phase 3 pronged therapy model of EMDR. This workshop emphasizes on the creative implementation of the core principles of EMDR in the treatment of people with various autism spectrum disorders.

In the recent past several neuroimaging studies aimed at evaluating the neural correlates of PTSD-related psychotherapies revealing their neurobiological effects on brain function. Functional studies by single photon emission computed tomography (SPECT) and electroencephalography (EEG) detected changes in cerebral blood flow and neuronal activation patterns, identifying the brain areas implicated in the various components of emotional processing and/or affected by the disorder. Investigations by magnetic resonance imaging (MRI) have also revealed PTSD-related structural changes. The first part of the workshop will review the neuroimaging methodologies and findings in PTSD treatment-related research with an extensive review of previous literature on the neurobiological effects of the various psychotherapies. The second part will deal with the description and implementation in research and clinic of neuropsychological testing with brief comments and discussion about their use in recent studies published by our group. In the third part the EEG monitoring of a complete set of Eye Movement Desensitization and Reprocessing therapies in 30 patients suffering of major trauma as compared to 20 healthy controls will be presented. These findings will also be compared to the neurobiological effects of trauma-focussed Cognitive Behavioural Therapy in a second group of psychologically traumatized clients. The results are the first report ever on the neurobiological changes occurring before, during and after PTSD-related psychotherapies shedding light on the neuronal processes underlying their clinical efficacy. The description and the discussion about the contents of the workshop will provide the audience (1) the necessary information to understand the methodological principles behind neuroimaging techniques (SPECT, EEG and MRI) and their possible applications in research and clinic; (2) the up-dated critical knowledge of the published papers in the field of PTSD-related psychotherapies functional and anatomical studies; (3) the basic research principles and examples to be motivated to start, take part and/or collaborate to functional studies in order to better understand the neural basis of psychotherapeutic techniques. Abstract:
Eye movement desensitization and reprocessing (EMDR(R)), developed in 1987 and proved highly effective in treating psychological disturbances rooted in traumatic memories. It was hypothesised that EMDR(R) would enhance coping in patients traumatised by a cancer diagnosis and / or treatment, as indicated by their subjective responses and levels of depression, anxiety, satisfaction with life, positive-negative affect balance and sense of coherence. A descriptive multiple case-study method was implemented. Three cases were treated by EMDR(R) and three by a supportive method. Data collection was triangulised in terms of semi-structured interviews, quantitative measuring instruments and interviews by an external validator. Findings were consistently in favour of EMDR(R). The results confirmed EMDR(R)'s efficacy in terms of beneficial clinical outcomes on both objective and subjective measures of change.[Journal abstract]

EMDR has not achieved full acceptance as a practice model within community mental health and agency settings. Some agencies are reluctant to develop an EMDR practice due to administrative concerns, clinical apprehensions or implementation reluctance. This resistance has constrained the development of EMDR in communities with the greatest need and often in settings where post traumatic stress is the focus of treatment. Trauma histories with co-occurring Substance Use Disorder (SUD) are disproportionately prevalent for individuals in the criminal justice system. A study was implemented in the Thurston County Drug Court Program to determine the prevalence of trauma exposure and evaluate the feasibility of implementing an Integrated Trauma Treatment Program (ITTP) combining two empirically supported treatments: Eye Movement Desensitization and Reprocessing (EMDR) and Seeking Safety (SS). It was hypothesized that individual trauma treatment would lead to improved program outcomes, including increased graduation rates and lower recidivism. Two hundred nineteen males and females, ages 18-65 were screened. One hundred sixty one participants (73.5%) were eligible for the ITTP based on a self-report of at least one “criterion A” event in their lifetime. Fifty-eight participants (26.5%) did not report criterion A trauma and were assigned to program as usual (PAU). Participants who completed only the SS groups (N=50) graduated at a rate of 62% compared to 91.3% of those who completed both SS and EMDR (N=69). After implementation of the ITTP, recidivism for graduates was 7.4% and 18% for terminators, compared to 25% and 30.6% respectively prior to the ITTP. These outcomes provide preliminary evidence that individual trauma treatment can improve graduation rates and decrease recidivism in a Drug Court Program.

This paper explores the application of interoceptive exposure (IE) therapy to treat the arousal and avoidant symptoms in veterans with posttraumatic stress disorder (PTSD). The historical background of PTSD and the functional impact of the disorder in veterans from Vietnam and Operation Enduring Freedom/Operation Iraqi Freedom are discussed in the first chapter. Literature on romantic and family relationship impairment, employment challenges, decreased physical health and overall quality of life, and increased mental health issues in veterans of combat are presented. Following the introductory chapter is a brief description of the history of treatment for combat trauma and a detailed review of the most common treatments for PTSD in their application to the veteran population. Research on psychophysiological approaches to treatment, pharmacotherapy, and EMDR is discussed. The extensive literature on cognitive behavioral treatment approaches for combat trauma is reviewed. As noted, exposure therapy appears to be the treatment approach with the most scientific support. A relatively new form of exposure therapy known as IE, as well as the small but promising body of research on the potential to augment conventional long-term exposure therapy with IE, are also addressed here. A new treatment protocol proposed here is built upon the foundation of empirical support for cognitive behavioral therapy for PTSD. It is intended to incorporate trauma-informed best practices and exposure therapy tenets through the implementation of group based IE for individuals with combat-related PTSD. Outlines of the following two sections will provide detailed descriptions of the group design and the specific treatment modules, the first of which addresses therapeutic rationale and group composition, and the second lists the specific twelve treatment modules.

A structured six-session group therapy has been developed and approved for overcoming craving. The six session protocol can easily be integrated to well-applied EMDR protocols (DeTur from Popky, CravEx from Haase) supported by systematic implementation of cue exposure techniques and guided imagery. The EMDR protocol supported by cue exposure catalyses the recovery process as follows: At the beginning the patients are taught a set of three ideodynamic resources for coping with the urge to drink; “Rewards of sobriety”, “Support of relatives and friends” and “Irrepressible commitment to sobriety”. The patients learn rapid activation of these resources by the use of the “Seven Cue Word Induction–technique” and kinaesthetic bilateral stimulation (SingleblAiR). Then they are exposed to alcohol until the urge to drink reaches it´s peak. At this moment they are taught to initialize resourcefulness - with continued exposure to alcohol. Subsequently, the power of one ideodynamic strategy diminishes the intensity of craving significantly. Craving symptoms finally disappear and are replaced by self-reinforcing thoughts and feelings due to state dependent learning. In addition to this new experience the patients acquire a high level of self-efficacy as well as greater and deeper knowledge about their personal drinking triggers. They also find out which strategy is the most effective one for each specific trigger. After regaining self-control over triggers, the patients are more receptive to working with their core addiction issues, which have now become easier to treat by especially using Desensitization and Reprocessing of the Standard- EMDR protocol. Workshop participants will become acquainted with the six-session protocol by the use of DVD-demonstrations and by practising. In addition they will receive a manual with standardised instructions. Finally they will also be given the opportunity to participate in a cross-cultural research project proving the effectiveness of the six-step program which starts in 2011. This approach is designed to improve the treatment of various types of addiction and can easily be integrated into existing EMDR treatment strategies.

Nondimeno, è esperienza abituale del clinico come tale modello incappi regolarmente all’interno di stalli rielaborativi che impediscono una adeguata desensibilizzazione e rielaborazione degli eventi stressanti o francamente traumatici. Per tale motivo, l’EMDR può massimizzare la sua efficacia psicoterapeutica attraverso l’impiego di specifiche strategie di “intervento cognitivo integrativo”, finalizzate all’implementazione adattiva ed ecologica delle risorse inattingibili da parte del paziente. In questa comunicazione, che prende in parte ispirazione dalle domande e dalle difficoltà emergenti nei colleghi durante l’attività di supervisione, attraverso numerose esemplificazioni cliniche si intenderà mostrare l’impiego flessibile ed euristico di strategie immaginative e corporee come interventi integrativi di particolare efficacia. In particolare, verrà posta una specifica attenzione nei confronti degli stalli integrativi dovuti principalmente a meccanismi dissociativi che possono causare difficoltà anche notevoli all’attività dello psicoterapeuta. Fenomeni dissociativi massicci, infatti, possono mostrarsi sin dall’inizio (in particolare qualora l’evento abbia suscitato una dissociazione peritraumatica), rendendo apparentemente non affrontabile il ricordo del paziente, in quanto non evocatore di alcuna risonanza emotiva; diversamente, la gestione dissociativa dei ricordi comparirà tra una seduta e l’altra, oppure all’interno del processo elaborativo condotto con l’EMDR. Verrà mostrato come la dissociazione, che può rendere impossibile l’elaborazione, possa essere gradualmente ridotta iperassociando il paziente nei confronti dei propri ricordi, di volta in volta privilegiando attività rivolte nei confronti delle immagini mentali oppure attraverso l’adozione di posture corporee finalizzate alla rottura delle barriere dissociative.

O objetivo do trabalho é apresentar uma revisão sobre o efeito dos movimentos oculares durante a exposição a memórias traumáticas a partir do paradigma da Teoria da Memória de Trabalho. Shapiro (2001) afirmou que a compreensão do mecanismo de ação do EMDR por meio da análise do efeito de seus componentes pode permitir cada vez mais seu aprimoramento. A meta-análise de Lee e Cuijpers (2012) mostra a importância dos movimentos oculares para a diminuição da vivacidade e das emoções negativas associadas às memórias traumáticas. Segundo a Teoria da Memória de Trabalho, a realização de movimentos oculares durante a manutenção de memórias traumáticas causaria a taxação da memória de trabalho, tornando as imagens menos vívidas e emocionais (ex. Gunter e Bodner 2008; van Den Hout et al. 2011). Quanto maior a taxação da memória de trabalho, desde que não cause prejuízo da manutenção da memória na mente, maior é a redução da vivacidade e intensidade (Gunter e Bodner 2008; van Den Hout et al. 2011, Engelhard et al. 2010, 2011). Portanto, a intensidade dos movimentos oculares parece ser importante para o efeito terapêutico. Smeets et al. (2012), ao investigarem o decurso temporal desses efeitos, verificaram que os movimentos oculares interferem primeiro com a vivacidade da memória. A redução da emocionalidade ocorre posteriormente. Por sua vez, Engelhard et al. (2010, 2011) têm mostrado que os movimentos oculares também são benéficos em relação a imagens negativas de eventos futuros, indicando um efeito terapêutico do EMDR sobre psicopatologias em que essas imagens estão presentes.Assim, em consonância com a afirmação de Shapiro (2001), estudos mostram a importância de se correlacionar a prática clínica com as investigações científicas sobre componentes do EMDR (ver Engelhard 2012). Além de favorecer a compreensão, a aplicação e a revisão dos protocolos, a correlação possui implicações clínicas importantes.

The objective is to present a review on the effect of eye movements during exposure to traumatic memories from the paradigm of the Theory of Working Memory. Shapiro (2001) stated that understanding the mechanism of action of EMDR through the analysis of the effect of its components can increasingly allow their improvement. A meta-analysis of Lee and Cuijpers (2012) shows the importance of eye movements to decrease the vividness and negative emotions associated with traumatic memories. According to the Theory of Working Memory, conducting eye movements during maintenance of traumatic memories cause the tax working memory, making the images less vivid and emotional (eg Gunter and Bodner 2008; van den Hout et al. 2011) . The higher taxation of working memory, since it does not cause prejudice to the maintenance of memory in mind, the greater the reduction in vividness and intensity (Gunter and Bodner 2008; van den Hout et al., 2011, Engelhard et al. 2010, 2011 ). Therefore, the intensity of eye movements seem to be important to the therapeutic effect. Smeets et al. (2012) investigated the time course of these effects and found that eye movements interfere first with the vividness of memory. The reduction in emotionality occurs later. In turn, Engelhard et al. (2010, 2011) have shown that eye movements are also beneficial in relation to negative images of future events, indicating a therapeutic effect of EMDR on psychopathology in which these images are presentes.Assim, in line with the statement Shapiro (2001) studies show the importance of correlating clinical practice with scientific research on components of EMDR (see Engelhard 2012). Besides promote understanding, implementation and review of protocols, the correlation has important clinical implications.

The trauma field can be proud of having evidence-based effective psychotherapy protocols for PTSD. Especially CBT and EMDR have been recognized as first choice treatments (NICE Guidelines 2005). However, having these protocols available new questions that need to be answered are emerging. There is no large scale evidence yet on phase 4 implementation showing its effectiveness. Too many patients drop out of treatment. Many patients suffer from comorbid conditions. The question on how research outcomes on the biology of PTSD should be translated into different psychotherapeutic approaches is a rather new one. Especially, is habituation still the correct fundament of exposure in PTSD, or should it be replaced by the concept of extinction? A third question is the mixed feeling in many societies about the concept of PTSD and it´s consequences in the need for treatment. Especially after disasters, but also after domestic violence, treatment can be seen as the avoidance of society to punish the responsible ones or to ask for material compensation. These questions will lead to find new directions for the psychotherapy protocols, for the combination with biological routes of intervention and for the societal acceptance of treatment for PTSD.

In the recent years the number of neuroimaging studies evaluating neural correlates of psychotherapy has steadily increased revealing its clear neurobiological effects on brain function across a wide range of psychiatric disorders. Functional studies by single photon emission computed tomography (SPECT) and positron emission tomography (PET) detect changes in cerebral blood flow and metabolism patterns, identifying the brain areas processing the various components of emotional processing and/or affected by the disorders. Investigations by magnetic resonance imaging (MRI) have also revealed psychiatry disease-related structural changes. The first part of the workshop (20 minutes) will describe the neuroimaging methodologies and findings in PTSD/EMDR research with and extensive review of previous literature on the neurobiological effects of EMDR. The second part of the workshop (20 minutes) will deal with the description and implementation in research and clinic of neuropsychological testing with brief comments and discussion about their use in the recent experiments performed by our group. In the third part the EEG monitoring of a complete set of EMDR therapies in 10 patients suffering of major trauma will be presented. The relative results are the first report ever on the neurobiological changes occurring before, during and after EMDR therapy sheding light on the neuronal processes underlying its clinical efficacy. Learning objectives: The description and the discussion about the contents of the workshop will provide the audience (1) the necessary information to understand the methodological principles behind the neuroimaging techniques (PET, SPECT and MRI) and their possible applications in research and clinic; (2) the critical knowledge of the limited number of published papers in the field of EMDR-related functional and anatomical studies; (3) the basic research principles and examples to be motivated to begin, take part and/or collaborate to EMDR research in order to better understand the neural basis of this fascinating psychotherapeutic technique.

Zijmenen dat het effectonderzoek naar mindfulness weliswaar nog uitgebreider en beter kan, maar dat het onderzoek tot nu toe voldoende evidentie heeft opgeleverd om toepassing te rechtvaardigen. Nyklíčekmerkt hierbij op dat in de psychologie een nieuwe therapie meestal eerst in de klinische praktijk jarenlang wordt toegepast voordat wetenschappelijk deugdelijk wordt onderzocht of de therapie wel werkt. Havermans blijkt verre van overtuigd en fileert de aangedragen evidentie genadeloos. Deze interessante discussie roept de vraag op wanneer we een behandeling evidence based mogen noemen. Het standpunt dat hiervan pas sprake kan zijn als gecontroleerd onderzoek de effectiviteit van de behandeling heeft aangetoond, zal door de meeste vakgenoten worden onderschreven. Maar wat is ‘gecontroleerd onderzoek’? Volstaat een wachtlijstcontrolegroep of moet de (nieuwe) behandeling worden vergeleken met andere actieve interventies, waarvan al eerder de effectiviteit is aangetoond? Ook de relatie tussen praktijk en theorie is interessant. Afgezien van de vraag of de opmerking van Nyklíček nog steeds hout snijdt in deze tijd van evidence based interventies, is het wel verantwoord om op grote schaal een nieuwe psychologische interventie toe te passen als de effectiviteit of specifieke werkzaamheid nog niet is aangetoond? Havermans meent dat men een nieuwe gedragstherapeutische interventie ontwikkelt op basis van veelbelovende klinische observaties en gedragswetenschap, met andere woorden er moet ook een theoretische onderbouwing van de interventie zijn. Voor dit laatste is inderdaad veel te zeggen, maar de geschiedenis leert dat de theorieën die aanvankelijk als verklaring voor de werkzaamheid van de interventie werden geformuleerd, meestal bij nader inzien de toets van de wetenschappelijke kritiek niet konden doorstaan. Onderzoek in de traditie van de experimentele psychopathologie (Jansen, Van den Hout & Merckelbach, 2010) heeft al heel wat reinigend werk verricht op theoretisch gebied. Op de keper beschouwd is van heel wat evidence based interventies aangetoond dat deze effectief zijn, maar hoe deze werken is veelal nog onduidelijk of voor de theoretische onderbouwing ervan is nog onvoldoende steun gevonden. Het laatste Najaarscongres van de Vereniging voor Gedragstherapie en Cognitieve Therapie (VGCT) had als thema ‘Change. Verandermechanismen en cognitieve gedragstherapie’. Tijdens het congres werd duidelijk dat over de verandermechanismen van evidence based interventies nog veel onduidelijkheid bestaat en dat het onderzoek hiernaar soms verrassende resultaten laat zien (Jaspers, 2011). Het is bepaald niet alleen EMDR (eye movement desensitization and reprocessing), waarover de theoretische inzichten zijn veranderd, ook al bestaat over de werkzaamheid van de interventie geen twijfel. In het volgend nummer van Psychologie & Gezondheid leest u hier meer over. In dit nummer vindt u nog een forumbijdrage, waarin de spreekwoordelijke knuppel in het hoenderhok wordt gegooid. De prikkelende titel ‘Huidige behandeling depressie is weggegooid geld’ nodigt op zijn minst uit tot lezing. Hoezo weggegooid geld? Als er een probleem is waarvoor evidence based behandelingen bestaan, is het immers depressie. Kok en collega’s laten echter zien dat ondanks de enorme bedragen die jaarlijks in Nederland worden uitgegeven aan de behandeling van depressie, in de huidige financiering van de gezondheidszorg nog onvoldoende rekening wordt gehouden met het hoge risico op terugval bij depressie. Het door velen, om uiteenlopende redenen verfoeide DBC-systeem (Diagnose Behandel Combinatie) ontmoedigt om langdurig met behandelingen door te gaan. Bestaande effectieve interventies om het risico op terugval te verminderen worden nauwelijks toegepast, terwijl deze bij de behandeling van een vaak chronische aandoening als depressie uitdrukkelijk zijn aangewezen. Hiermee wijzen de auteurs impliciet op een belangrijke tekortkoming van het bestaande effectonderzoek: het gebrek aan evaluatie van de langetermijneffecten van de onderzochte interventie. Ook voor psychologische interventies bij depressie is duidelijk dat deze werkzaam zijn. En al geldt ook voor depressie dat we nog lang niet weten wat de specifieke werkingsmechanismen zijn (hoe deze werken), de noodzaak van implementatie van evidence based interventies om terugval te vermijden of uit te stellen kan niet genoeg worden benadrukt. Het recidiverend karakter maakt depressie immers tot een aandoening met zowel hoge maatschappelijke kosten als een zeer hoge ziektelast, lijdensdruk en risico op suïcide.

In the previous issue of Psychology & Health Havermans Jim wrote a critical forum posting about mindfulness. His thesis, that the efficacy of mindfulness meditation is insufficient evidence to its application in health care to justify, this issue argued contradicted by Schroevers Maya and her colleagues and by Ivan Nyklicek. Zijmenen mindful that the impact study, while still more extensive and better, but that the investigation so far has yielded enough evidence to justify the application. Nyklíčekmerkt in psychology here that a new therapy in clinical practice usually first applied for years before being properly scientifically investigated whether the therapy works. Havermans appears far from convinced the fillets and put forward evidence mercilessly. This interesting discussion raises the question if we may call evidence-based treatment. The view that this only if there can be controlled study the efficacy of treatment has shown, most colleagues will be endorsed. But what is 'controlled study'? Is a waiting list control group or to the (new) treatment are compared with other active interventions whose effectiveness has already been demonstrated? The relationship between practice and theory is interesting. Apart from the question whether the remark Nyklicek still holds water in this era of evidence-based interventions, it is widely recognized for a new psychological intervention should be as specific activity or effectiveness is not proven? Havermans believes that a new behavioral intervention developed on the basis of promising clinical observations and behavioral science, in other words, there is also a theoretical justification for the intervention. For the latter is indeed much to say, but history shows that the theories initially as an explanation for the efficacy of the intervention were formulated, mostly on closer inspection the test of scientific criticism could not stand. Research in the tradition of experimental psychopathology (Jansen, Van den Hout & Merckelbach, 2010) has a lot of work cleaning the theoretical field. On closer examination of many evidence-based interventions shown to be effective, but how they work is often unclear whether the theoretical substantiation is found insufficient support. The last Autumn Congress of the Association for Behavioral and Cognitive Therapy (VGCt)'s theme was "Change. Change mechanisms and cognitive behavioral therapy. During the conference it became clear that the change mechanisms of evidence-based interventions much uncertainty and that the research on this surprising results show (Jaspers, 2011). It provides not only EMDR (Eye Movement Desensitization and Reprocessing), which the theoretical views have changed, even as to the efficacy of the intervention no doubt. In the next issue of Psychology & Health You can read more about. In this issue you will find a forum posting where the proverbial cat among the pigeons thrown. The provocative title "Current treatment depression is a waste of money 'invites at least into reading. Why wasted? If there is a problem for which evidence-based treatments exist, it is indeed depression. Cook and colleagues reveal that despite the enormous sums spent each year in the Netherlands for the treatment of depression in the current financing of health care is still insufficiently taken into account the high risk of relapse in depression. By many, for various reasons detested system DBC (Diagnosis Treatment Combination) discourages long-term treatments to continue. Existing effective interventions to reduce the risk of relapse are rarely used, while in the treatment of a chronic condition such as depression often explicitly designated. This, the authors implied a major weakness in the current outcome research: the lack of evaluation of the long-term effects of the tested intervention. For psychological interventions for depression is clear that this work. And already includes a long depression that we do not know the specific mechanisms of action (how they work), the necessity of implementation of evidence-based interventions to prevent relapse or delay can not be overstated. The recurrent nature makes depression after a disease with both high social cost as a very high disease burden, distress and risk of suicide.

Objective: This study aims to investigate the effectiveness of the "pain provocation technique" (PPT)--a focused treatment strategy incorporating interoceptive exposure (i.e., imagining increases in pain intensity), bilateral stimulation (tactile stimulation), and implementation of pain-related coping to decrease pain intensity--for adolescents suffering from chronic pain. Design: Prospective observational comparative study. Methods: Adolescents utilizing PPT (19 boys and 21 girls) within multimodal inpatient treatment were compared with adolescents in standard multimodal inpatient treatment matched for age, gender, and diagnosis. Core outcome variables (pain intensity, disability, emotional distress) were assessed at admission and 3 months posttreatment. Results: Adolescents in the PPT group demonstrated a sharper decrease in pain intensity and school aversion. Both groups demonstrated significant reductions in disability and emotional distress. Conclusions: Results are discussed in terms of the importance of focused treatment strategies such as interoceptive exposure for adolescents suffering from disabling chronic pain. Future studies are warranted to carefully investigate the effectiveness and possible process of change during the PPT such as sensory, cognitive, emotional, and memory aspects.

I limiti mentali autoimposti acquisiti da copioni familiari reiterati in età evolutiva e da modelli operativi interiori acquisiti dalle autorità societarie attraverso mezzi di comunicazione di massa e regole educativo-lavorative, inducono percentuali sempre più rilevanti della popolazione europea alla vulnerabilità psicopatologica. L’esordio delle sintomatologie psichiche e somatiche di varia entità, avviene già in fasi precoci, nella primissima infanzia e spesso già nelle fasi prenatali, e il limite di età tende percentualmente a essere sempre più sensibile fin dal primo mese di vita. In età scolare si manifestano situazioni comportamentali quali il cosiddetto “bullismo” e fenomeni con campionature rilevanti di sindromi ipercinetiche con deficit attentivo e disturbi del-l’apprendimento. In adolescenza il contesto si complica e gli attacchi di panico, le sindromi depressive e i disturbi alimentari psicogeni dilagano, fino all’esordio di disturbi post-traumatici da stress che si incrementano in seguito alle difficili scelte di orientamento universitario o lavorativo. La vulnerabilità dell’età adulta si manifesta con disturbi somatoformi di diversa natura, disturbi psicocardiologici, sindromi ansioso-depressive, attacchi di panico, disturbi di coppia e relazionali, per citare i più frequenti e limitandomi solo ad accennare l’esistenza della molteplicità di disturbi iatrogeni. In tali soggetti, la consapevolezza di essere indotti in stati di shock che incrementano molteplici disturbi somatoformi e psicopatologici è praticamente assente. Essi, come pazienti, si rivolgono agli specialisti in ambito sanitario con la convinzione, spesso indotta da propagande dei mass-media, che tutto sia solo genetico, e vada "curato" con farmaci per lunghi periodi se non per tutta la vita. Anche da parte degli operatori sanitari vi sono ampie aree di inadeguatezza metodologica: ad esempio, la gravidanza viene gestita come fosse una malattia, riducendo la donna partoriente a una paziente alla quale troppo facilmente si “consiglia” il parto cesareo (statisticamente tra i più frequenti in Italia!) come metodo “veloce e sicuro” di parto, togliendo la competenza materna dell’imprinting alla nascita del bambino con le conseguenze psicologiche che ne derivano per la relazione madre-bambino e per la crescita serena di quest’ultimo, e, sempre a titolo esemplificativo, ignorano quasi del tutto gli aspetti di psicocardiologia, e il loro intervento si riduce a esami medici invasivi e a somministrazione di farmaci. Eppure, la psicoterapia cognitiva costruttivista, e specificamente modelli teorici e tecniche strategiche specialistiche note come EMDR (Eye Movement Desensitization and Reprocessing), l’utilizzo di biofeedback, l’innovativo training emotivo-cognitivo-comportamentale da me ideato nelle due versioni per la psicoterapia e per i gruppi in formazione che incrementa il riconoscimento emotivo e l’implementazione di immagini mentali idonee a modificare cognizioni e comportamenti irrazionali, quando eticamente e competentemente applicati, fanno molto per questi pazienti, sia in quanto si incrementa sensibilmente il livello della loro consapevolezza e della loro capacità di farsi protagonisti nel-l'evoluzione positiva della loro “guarigione”, sia in quanto si può intervenire in modo mirato con sperimentati protocolli per la risoluzione dello stato di trauma psichico in tempi ragionevolmente rapidi e con risultati attendibili e verificabili. Passando dalla dimensione individuale a quella collettiva, ossia alla psicopatologia collettiva cagionata dall’esposizione (anche solo mediatica) ad eventi catastrofici o angoscianti (magari associati a senso di impotenza, insicurezza, precarietà) o a diversi tipi di stress e vulnerabilità, possiamo aggiungere che, analogamente, mediante un lavoro su sistemi di neuroni specchio e sull'attivazione di nuove connessioni di reti neurali con un modello operativo non invasivo, si potrebbe migliorare la condizione di intere popolazioni rispetto a disturbi che, oggettivamente, sono in continua diffusione. Sarebbe opportuno iniziare una sensibilizzazione collettiva partendo dalla formazione per livelli differenziati degli operatori educativi e sanitari, per poi estenderla alla popolazione suddividendola per fasce di età e per territori di appartenenza; purtroppo la consapevolezza non è tra le aspettative primarie di committenti rivolti solo al profitto economico. In un contesto storico-culturale dove l’etica, le relazioni umane, la cooperazione sembrano utopiche fiabe, questa è la sfida di essere una perturbatrice emotivamente orientata ad amplificare la consapevolezza attraverso un nuovo modello psicoterapeutico e formativo integrato, al quale sto lavorando da alcuni anni con risultati incoraggianti e che sarà mia premura esporre dettagliatamente durante il Congresso EMDR 2008.

The self-imposed mental limitations acquired from family scripts repeated age and developmental models inner acquired by the company operating through means of mass communication and educational and working rules, induce percentage increasing as the population of Europe vulnerability to psychopathology. The onset of symptoms of various psychological and somatic entity, is already in the early stages, in early childhood and often known during prenatal and age limit percentage tends to be more sensitive since the first month of life. Age school behavioral situations occur where the so-called "bullying" and phenomena samples relevant syndromes of attention-deficit and hyperactive disorder - learning. In adolescence the context is complicated and panic attacks, the syndromes psychogenic depression and eating disorders are rampant, until onset of post-traumatic disorders stress which increases as a result of difficult choices of university or business orientation. The vulnerability of adulthood is manifested by different types of somatoform disorders, disorders psycho, anxious-depressive syndrome, panic attacks, disorders of torque and relational to cite the most frequent is limited only to mention the existence of multiplicity of disorders iatrogenic. In these subjects, conscious of being led into a state of shock that increase multiple somatoform disorders and psychopathology is virtually absent. They, like patients, addressed to specialists in the health field with the belief, often driven by propaganda media, that everything is just genetic, and must be "cured" with drugs for long periods if not for life. Including by health workers there are large areas of inadequacy methodological: for example, pregnancy is managed as if it were a disease, reducing the woman in labor to a patient which too easily "advise" Caesarean (statistically the most frequent in Italy!) as a method of "fast and safe childbirth, removing the competence of imprinting the birth mother of the child with the psychological consequences that entailed for the mother-child and to the peaceful growth of the latter, and, also example, know little about the aspects of psycho, and their intervention reduces to invasive medical examinations and medication. Yet, cognitive psychotherapy constructivist theoretical models and specific strategic and technical specialists known as EMDR (eye movement desensitization and reprocessing), the use of biofeedback, the innovative emotional-cognitive-behavioral training which I designed in two versions for psychotherapy and groups in training that increases the emotional recognition and implementation of mental images likely to change, knowledge and irrational behavior, when ethically and competently applied, do a lot for these patients, both because it increases significantly the level of their awareness and their ability to get players in - the positive development of their "healing", both as it can intervene in a targeted manner with tested protocols for the resolution of the state of psychic trauma in the reasonably rapid and reliable and verifiable results. Moving from individual dimension to that collective, that is caused by exposure to psychopathology group (even the media) to distressing or catastrophic event (perhaps associated with the sense of powerlessness, insecurity, instability) or different types of stress and vulnerability, we can add that, similarly, through a work on systems of mirror neurons and activation of new connections of neural networks with a model operating non-invasive, it could improve the condition of entire populations than disorders that, objectively, are in constant circulation. It would be appropriate to start a collective awareness levels, starting from training differential operators' education and health, then extend it to the population divides by age and territories belonging unfortunately the awareness is not between the expectations primary principals addressed only in profit or loss. In a historical-cultural context where ethics, human relations, cooperation seem utopian fairy tales, this is the challenge of being an emotionally disturbing oriented to amplify the awareness through a new model of psychotherapy and integrated training, which I working for several years with encouraging results and that will spell out my readiness EMDR 2008 during the Congress.

Achtergrond: Meer dan 50% van de kinderen maakt een of meerdere ingrijpende gebeurtenissen mee voor het 18e jaar, ongeveer 20% ontwikkelt traumagerelateerde psychopathologie en in 45% van de gevallen is er een verband tussen het meemaken van een ingrijpende gebeurtenis en de start van een kinder- en jeugdpsychiatrische diagnose. Kennis over traumadiagnostiek en behandeling is dan ook niet alleen voorbehouden aan gespecialiseerde centra. Implementatie van deze kennis is belangrijk en gaat zeker niet zonder slag of stoot. Doel: Het geven van een update rond traumadiagnostiek en behandeling. Tevens het bespreken van implementatieperikelen binnen de eigen instelling en hoe hiermee om te gaan. Methoden: Onderzoek van wetenschappelijke literatuur en raadpleging van verschillende richtlijnen, en casuïstiek betreffende implementatie. Resultaten: De laatste jaren is er toenemend onderzoek gedaan naar diagnostiek en behandeling van psychotrauma bij kinderen en adolescenten. Het gaat daarbij om het ontwikkelen van screenings- en diagnostische instrumenten en onderzoek naar effecten van behandeling. Traumagericht cognitieve gedragtherapie en eye movement desensitisation and reprocessing (EMDR) zijn effectieve behandelvormen. Implementatie van deze kennis in de klinische praktijk verloopt moeizaam en vraagt naast een andere manier van werken van de behandelaren ook een organisatiestructuur die hierin faciliterend werkt. Conclusie: Evidence-based traumadiagnostiek en behandeling is voorhanden. Implementatie van deze kennis in de klinische praktijk is belangrijk en vraagt een andere expertise. Weten is nog niet kunnen toepassen.

Background: More than 50% of the children makes one or more major events count for the 18th year, approximately 20% develop trauma related psychopathology and 45% of cases there is a connection between experiencing a traumatic event and the start of a child and adolescent psychiatric diagnosis. Knowledge about trauma assessment and treatment is not only reserved to specialized centers. Implementation this knowledge is important and is certainly not without a struggle. Purpose: To give an update on trauma assessment and treatment. Also discuss implementation turmoil within own institution and how to deal with. Methods: Analysis of scientific literature and consultation with various guidelines, and case studies on implementation. Results: In recent years there is increasing research into diagnostics and psychotrauma treatment in children and adolescents. This involves developing of screening and diagnostic tools and research into effects of treatment. Trauma-focused cognitive behavioral therapy and eye Movement Desensitisation and Reprocessing (EMDR) are effective forms of treatment. Implementation this knowledge in clinical practice runs difficult and requires a different way of working alongside the clinicians also an organizational structure facilitating this work. Conclusion: Evidence-based trauma assessment and treatment is available. Implementation this knowledge in clinical practice is important and requires a different expertise. Knowing is not to apply.

Eye movement desensitization and reprocessing (EMDR) is a psychological treatment method used primarily for people who have experienced trauma. This article provides a systematic research synthesis of EMDR studies targeting trauma symptomatology published between 1997 and 2003. This synthesis builds on the Revised Gold Standard (RGS) as a guide to evaluate empirical EMDR studies. Modifications and additions to the RGS criteria are proposed. The resulting standard is referred to as the Platinum Standard (PS). 16 EMDR studies are reviewed and critiqued using the PS criteria. None of the studies reviewed met full PS criteria. The mean score for the studies on the PS was 8.28, with 9 of the studies exceeding the mean. The review calls for researchers to employ more rigorous research designs for EMDR effectiveness using PS criteria. Implications for practice, policy, and research are discussed. [Author Abstract]

Las estadísticas acerca del maltrato físico de los niños son alarmantes. Se estima que cientos de miles de niños han recibido abuso y maltrato a manos de sus padres o parientes. Los que sobreviven el abuso, viven marcados por el trauma emocional, que perdura mucho después de que las consecuencias físicas hayan desaparecido. A menudo el daño emocional severo a los niños maltratados no se refleja hasta la adolescencia, o aún más tarde, cuando muchos de estos niños maltratados se convierten en padres abusivos y comienzan a maltratar a sus propios hijos. El reconocer y dar tratamiento inmediato es importante para minimizar los efectos a largo plazo causados por el abuso o maltrato físico. En este sentido, en los últimos años venimos trabajando con menores que han sido víctimas de abuso, negligencia, violencia familiar o abandono. El abordaje terapéutico desde el EMDR se muestra como un paradigma muy eficaz a la hora de trabajar con niños y menores. El trabajo con la red de recuerdos y el reprocesamiento de recuerdos traumáticos ayuda a su recuperación. A través de los dibujos que los niños han ido haciendo en las sesiones se observa como el reprocesamiento en el niño es diferente al del adulto. Además la etapa del desarrollo del niño interfiere tanto en el modo de procesar información traumática como en la construcción de los recuerdos y su posterior reprocesamiento. Por lo que los protocolos de EMDR en el niño son diferentes a los del adulto para que sean igualmente eficaces. En esta comunicación se presenta el protocolo de trabajo para tratar a los menores víctimas de abuso, desarrollado desde la perspectiva de EMDR, exhibiendo cómo funciona el protocolo a través de la presentación de los datos que se han ido recogiendo durante la aplicación del programa de intervención.

The statistics on physical child abuse are alarming. It is estimated that hundreds of thousands of children are physically abused by their parents or relatives. Those who survive abuse, living marked by the emotional trauma remains long after the physical consequences are gone. Often the emotional damage severely abused children is not reflected until adolescence or even later, when many of these abused children become abusive parents and begin to abuse their own children. Early recognition and treatment is important for minimize long-term effects caused by abuse or physical abuse. In this regard, in recent years we have been working with children who have been victims of abuse, neglect, family violence or neglect. The therapeutic approach from the EMDR appears as a very effective paradigm for working with children and minors. Working with the memory network and reprocessing of traumatic memories aid their recovery. Through drawings children have been doing in the sessions were observed as the reprocessing in children is different from the adult. also stage of child development interfere much in the way of processing information traumatic as the construction of memories and subsequent reprocessing. by what EMDR protocols in children are different from the adult to be equally effective. In this paper we present the working protocol for dealing with juveniles victims of abuse, developed from the perspective of EMDR, showing how protocol through the presentation of the data have been collected during the implementation of the intervention program.

"EMDR therapy has achieved an extense international recognition as treatment of choice for PTSD. Its use has been extended to a broaden field of pathologies. From the EMDR perspective, symptoms are considered to feed on previous traumatic experiences, and the patient´s problems resolution will be achieved throughout the processing of this dysfunctionally stored information, that becomes reconsolidated as an ordinary memory. The standard EMDR procedure includes not only the processing of past events, but also the processing of present situations and future scope. The use of ego-state interweaves and/or extensive ego-state therapy (utilized in the preparation phase) and integrated into EMDR targeting (phases 3-7), in the treatment of personality disorders, has not received a great deal of attention at conference presentations or in the published media. This has led to minimal or nonresponsiveness in the EMDR treatment of personality disorders, since aspects of dissociation in these clients have not been addressed. In the past ten years, renown neuroscientists, such as Eric Kandel, Joseph LeDoux, Michael Gazzaniga and V.S. Ramachandran, in response to empirical findings in the fields of memory, neuromodularity, split-brain research and information processing, have begun to suggest that the “self” may very well be a collection of memories that are structured in a fragmented (neuromodular) multiplicity that is developmentally inherent. Ramachandran, LeDoux and Gazzaniga have, each, stated, explicitly, that the sense of a “cohesive and monolithic” self is an “illusion” created by areas in the left cerebral hemisphere. Accordingly, just as the EMDR standard protocol was adapted for recent traumatic events (in response to acute memory fragmentation), so must it be modified for inherent memory and personality fragmentation, by the use of extensive ego-state work (preparation) and ego-state-specific EMDR targeting (phases 3-7). The implementation of these techniques has shown a remarkable advance in the treatment of personality disorders, which had, previously, been rather impervious to EMDR treatment.

This article outlines a comprehensive model that helps to identify crucial target memories for EMDR treatment. The “Two Method Approach” can be used for conceptualization and treatment implementation for a broad spectrum of symptoms and problems, other than those related to PTSD per se. The model consists of two types of case conceptualizations. The First Method deals with symptoms whereby memories of the etiological and/or aggravating events can be meaningfully specifi ed on a time line. It is primarily aimed at the conceptualization and treatment of DSM-IV-TR Axis I disorders. The Second Method is used to identify memories that underlie patients’ so-called dysfunctional core beliefs. This method is primarily used to treat more severe forms of pathology, such as severe social phobia, complex PTSD, and/or personality disorders. The two methods of case conceptualization are explained step by step in detail and are illustrated by case examples.

The consistent findings on the adverse psychological effects of war and terror on children have led to a recognized need for suitable early intervention in the immediate, midterm and post term, of mass crisis situation. This presentation will try to overview the clinical and empirically supported interventions operating in the last two years in Israel, with children and families living in a constant threat of bombardment and shelling of populated areas on both sides. This presentation will focus on the war related stress reactions of young Israeli children exposed to these experiences and present some creative interventions done to foster resilience and coping, as well as the treatment of PTSD reactions. It was estimated that during this war, 4,000 Katyusha rockets and missiles hit the northern parts of Israel. More then 1 million people were at the danger zone for a potential direct hit. More then 500,000 Israelis relocated during the war to live in safer areas out of the range of the rockets. The children in the war zones experienced sounds of sirens' explosions and sound of artillery. They spent considerable time in bomb shelters and were exposed to sights (directly or through the media of damaged houses and casualties). This situation continues now in the southern part of the country along the Gaza Strip were children on both sides live under the daily reality of bombardment, casualties and death. Many programs and individual treatments were and are still operating as the professionals are trying to meet the needs of communities and individuals under this chronic ongoing stress situation. A community project is operating in the southern part of the country, where mothers and toddlers as well as the staff members of the kindergartens, are being supported by dyadic trauma focused play therapy and enrolled in stress managements programs. Findings on the interrelation between the degree of the parent's PTSD on the child's aversive symptomology,and the follow up of dyadic treatment data, will be presented. Another program the "Huggy Puppy", exemplifies the implementation of a developmental appropriate intervention with children in the midst of the crisis, and the effects of building a sense of self efficacy and relatedness. This will be illustrated in the study done by Prof. Avi Sadhe from Tel Aviv University on the effects of the "Huggy Puppy". The use of the modified EMDR Group Protocol for children during the war and the efficacy of the EMDR individual treatment in the post war situation as compared to CBT and SE FAR will be examined in the research done by Dr. Moshe Farchi of the Tel Chai College. These diverse interventions and treatments represent the wide range of theoretical and practical thinking which are necessary in order to create a comprehensive holistic model which will serve as a guideline for interventions with children in crisis.

Hyperarousal during trauma inhibits the integration of memory. In DID, memory is further disrupted when alter personalities coalesce around individual memory fragments and either reenact conflicts or disown them. Eye Movement Desensitization and Reprocessing (EMDR) is an innovative psychotherapeutic method which accelerates information processing and facilitates the integration of fragmented traumatic memories. Following a successful EMDR session, patients report that the nature of the traumatic memory has changed and that the event is now less upsetting and “feels over.” A strategy for using EMDR to integrate traumatic memories in patients with dissociative identity disorder (DID) is presented and technical considerations for its implementation are discussed. To the extent that alter personalities often are based on memory fragments, integration of traumatic memory facilitates personality integration. EMDR may be a superior method for working with traumatic memories in that it appears to enhance memory integration and reformulate cognitions concomitantly.

A Performance Enhancement Program using EMDR can be an effective leadership development tool. The issue of management-related stress was explored as a roadblock to effective leadership. APEX, LLC, in partnership with Marathon County (State of Wisconsin), has established an innovative program for leadership development which has demonstrated positive results in improving the participant's self-confidence, optimism, and social skills in the work setting, along with reducing overall job stress. The Performance Enhancement Program incorporates EMDR and a non-clinical personality instrument to assess the individual's coping style for managing stress in the workplace. Specific strategies for identifying negative & positive cognitions unique to this population are noted for the EMDR protocol. The program includes a description of the set-up, protocols, pilot program, implementation, and follow-up ("refresher course option") used with Marathon County managers and administrators.
