Abstract:
We previously reported on the outcomes of a controlled study of eye movement desensitization and reprocessing (EMDR) effectiveness in the treatment of traumatic memory (Wilson, Tinker, & Becker, 1994; Wilson, Becker, & tinker, in press). In that study we found that three, 90-minute sessions of EMDR (Shapiro, 1995) "normalized the psychological functioning of the previously traumatized participants (g = 80) on all dependent measures. The present study is a 15-month follow up of those participants. I Method: The research design is shown in Table 1. Participants were randomly assigned to EMDR or to Delayed EMDR conditions. Pretreatment measurement occurred at measurement time TI. Participants in the EMDR condition received EMDR between T1 and T2; those in the Delayed EMDR condition received EMDR between T2 and T3. All participants were tested immediately following treatment and at 3 months following treatment (at T4). The 15 month, long-term follow up occurred at measurement time T5. An independent assessor collected all of the following dependent measures: Subjective Units of Disturbance Scale (SUDS; Wolpe, 1990), Impact of Events Scale (IES; Hmowitz, Wilner, & Alvarez, 1979), State/Trait Anxiety Inventory (STAI; Spielberger, Gorsuch, Lushene, Vagg, & Jacobs, 1983), and the Symptom Checklist (SCL-90-R, Derogatis, 1992). Results: Two analyses were performed to assess the impact of EMDR treatment at the 15-month follow up. First, in order to assess the overall, long-term impact of EMDR, the 15-month follow-up scores were compared with the pretreatment scores. There was significant improvement on all nine measures at the 15-month follow up: The multivariate effect was significant (Wilk's Lambda =.11, p<.0005) as were all nine of the univariate effects (all p <.0005). Second, in order to assess whether the improvement shown immediately following EMDR treatment had been maintained over the following year the immediate posttreatment scores were compared with, the 15-month follow-up scores. The multivariate test was nonsignificant (Wilk's lambda=.74, p=.079), indicating the improvement shown immediately following EMDR was maintained 15 months later. The univariate analyses indicated additional improvement for the PTSD symptoms of intrusions (IES Intrusion: F(1,56)=7.71, p=307) and avoidance (IES avoidance: F_(1,56) -4.44, p=.040). None of the nine measures showed deterioration at the 15-month follow up. Prior to EMDR treatment 45% (g= 9) of the responders had been diagnosed as PTSD, at the 15-month follow up only 7% (g = 4) were diagnosed as PTSD (chi-squareo, N=61)= .72, p < .05). III. Responders Versus Nonresponders at the 15-Month Follow up.: At the time of writing this abstract, 75% of the participants (g=61) have responded to the 15-month follow up. In general, measures taken prior to treatment did not differentiate responders fiom nonresponders. Responding at the 15-month follow up was unrelated to age, gender, marital status or years of education, although the annual income of the responders (Mdn=21,500) was higher than that of the nonresponders (Mdn = 14,750, Mann-Whitney U=372.5, p=.017). Responding or not at 15 months was unrelated to the type of trauma experienced, whether or not the participants had been in therapy prior to EMDR treatment, or how long ago the trauma had occurred. It was also unrelated to the severity of the trauma as measured by the pretreatment scores on the nine dependent variables and to whether or not the participant met the PTSD diagnosis criteria prior to treatment. A multiple regression analysis used the immediate posttreatment and 90-day posttreatment scores to predict whether or not the participant responded at the 15-month follow up. Nonrespondents were more likely to be depressed at 90-days following treatment than were respondents (R square=.O8, B=-.16, Beta = -.28, F_L1,71)=5.99, p=.017). No other variables entered into the regression model. IV Discussiona and Conclusion, Tretement effects found immediately following EMDR treatment wer maintained or improved 15 months later and thee was a significant decrease in the number of participants diagnosed as PTSD at the 15 month follow up. The comparison of responders to nonresponders at the 15 month follow up showed that the nonresponders were more depressed than the responders, raising the possiblity that the present results may be favorably biased to some extent. The discussion will include the additional, subjective impressions of participants who did not respond to the follow up. Limitations of EMDR with this population will be discussed, including the influence of comorbidity, multiple traumas, retraumatization after treatment, and spontaneous recurrence of symptoms. V. While most research on PTSD has studied subjects exposed to single trauma, in clinical practice the vast majority of treatment seeking patients have histories of multiple traumas, usually interpersonal, abuse. This gives rise to complex clinical pictures, of which PTSD is just one dimension. The Trauma Center in Boston is a large, multidisciplinary, developmentally focused Clinic which specializes in the treatment of traumatized children and adults. Our clinic uses a developmentally based assessment tool which helps in the staging of appropriate treatment interventions. Special emphasis is placed on providing patients with skills to deal with complex trauma-based symptoms, such as dissociation, by teaching stablization with DBT techniques, psychoeducational groups, resource installation, SIT, and body-oriented methods, in which patients are taught skills to increase their internal locus of control. We will review the rationale for various psychopharmacological interventions and the role of groups to enhance the capacity for mutual relationships. All treatment occurs on the foundation of continuity of care with one individual therapist who follows the patient’s progress,explores life issues, helps deal with re-enactment behaviors, and does trauma-specific treatment, such as EMDR or CBT for alleviation of trauma-specific symptoms. This conference will explore these issues in depth and discuss in detail the staging and applications of various treatment techniques in clinical practice.

All too often it is assumed that infants or young children are too young to remember frightening, painful, and confusing experiences, and therefore will not suffer harm because they are too young to be traumatized. Clinical experience; however, as well as research on neurobiology of trauma, child development, and attachment, tell a different story. While very often not verbalized, overwhelming early experiences are nonetheless remembered implicitly. As such they tend to find expression in a myriad of debilitating emotional and somatic symptoms across a persons life span; even into adulthood. Understanding how early trauma can affect the child developmentally, and how it might be expressed, is crucial not only to clinicians treating children but also to those working with adults. It is through understanding early traumatic markers and the subtle, often coded signs, sometimes rooted as far back as infancy, that one can begin the process of untangling these symptoms and freeing the individual from a lifetime of pain, suffering, and failed relationships. This workshop will examine the types of early traumamedical, severe neglect, all forms of abuse, disrupted attachmentand how these traumatic experiences impact the young child from birth to five years. The signs and symptoms of traumatized and dissociative children, as well as available research on the topic will be described, along with case examples. The need for careful interpretation of often convoluted and subtle symptoms will be explained. Accurate interpretation of a childs presentation relies on understanding the neurobiology of trauma and dissociation. That, as well as understanding the impact on child development and attachment is crucial for effective intervention. Strategies such as Eye Movement Desensitization Reprocessing (EMDR) and involvement of caregivers in the therapy will be described. Videoed sessions and artwork describing the young traumatized child's inner world and recovery process will augment the presentation.

De eye movement desensitization and reprocessing (EMDR) interventie wordt veelvoudig gebruikt in de gezondheidszorg en is een van de meest onderzochte interventies voor posttraumatische stress-stoornis. De populariteit van deze interventie kan mede worden toegeschreven aan de aangetoonde werkzaamheid van EMDR voor het verminderen van traumasymptomen. EMDR is een behandeling die bedoeld is om de helderheid en aversiviteit van traumaherinneringen te verminderen. Tijdens de interventie staat het ophalen van traumatische herinneringen en tegelijkertijd het maken van oogbewegingen centraal. De laatste jaren zijn labstudies toegepast om te achterhalen hoe EMDR precies werkt. Uit deze studies blijkt dat de belasting van het werkgeheugen door een duale taak een belangrijk component van EMDR is. Het werkgeheugen heeft namelijk een beperkte capaciteit. Dit zorgt ervoor dat er minder capaciteit beschikbaar is voor de traumatische herinnering wanneer deze tijdens de behandeling wordt belast met een taak (bv. oogbewegingen). De werkgeheugentheorie is bruikbaar, maar er is wellicht een simpelere verklaring voor de werkzaamheid van EMDR: afleiding. The Eye Movement Desensitization and Reprocessing (EMDR) intervention is multiple used in health care and is one of the most studied treatments for posttraumatic stress disorder. The popularity of this intervention may also be attributed to the demonstrated efficacy of EMDR for reducing trauma symptoms. EMDR is a treatment designed to reduce the brightness and aversiviteit memories of trauma reduction. During the intervention is retrieving traumatic memories while making eye movements centrally. In recent years labstudies used to determine how EMDR works. These studies show that the load on working memory by a dual task is an important component of EMDR. The memory has a limited capacity ie. This ensures that there is less capacity available for the traumatic memory during treatment when it is subjected to a task (eg eye movements). The working memory theory is useful, but there may be a simpler explanation for the efficacy of EMDR: distraction. It is practical to know how EMDR works so that more clarity about exactly how EMDR should be applied. In addition, the concept of the operation of EMDR important for determining the potential usefulness in multiple disorders. The purpose of the present experiment is to critically examine whether there is a difference between getting a negative memory with a visual distraction task and retrieval of a negative memory making eye movements on the degree of vividness and emotionality. Research must also as a replication of Kavanagh et al (2001) that the same was intended to investigate, but took no account of the cognitive load of the tasks. The results of the present study show that the oogbewegingstaak and visual distraction task differences in the extent of delayed response task. The eye movements lead to a longer delay. Nevertheless, the results of the experiment show that while applying the intervention visual noise to greater reduction in both vividness and emotionality results in comparison with the eye movements and do nothing. Eye movements show intermediate results. It seems that the results are contrary to the usual working memory theory and the "distraction theory 'support as an explanation for the efficacy of EMDR. It might distractions that may be sufficient to establish the efficacy of EMDR to achieve. More research is needed to support these results.

A randomized experimental evaluation found support for the effectiveness of eye movement desensitization and reprocessing (EMDR) in reducing trauma symptoms among adult female survivors of childhood sexual abuse. 59 women were assigned randomly to one of three groups: (1) individual EMDR treatment (six sessions); (2) routine individual treatment (six sessions); or (3) delayed treatment control group. A MANOVA was statistically significant at both posttest and follow-up. In univariate ANOVAs for each of four standardized outcome measures EMDR group members scored significantly better than controls at posttest. In a three-month follow-up, EMDR participants scored significantly better than routine individual treatment participants on two of the four measures, with large effect sizes suggestive of clinical significance. Abstract:
Despite the emergence of many theories on biological EMDR mechanisms, research is still needed to understand the healing processes of EMDR. We conducted four experiments to explore the effects of EMDR on PTSD with 17 to 22 patients suffering from one unique trauma. The first experiment evidenced attentional bias in PTSD towards negative words that disappeared after successful EMDR Therapy. The second experiment has shown a less efficient control of emotion in PTSD as compared to healthy controls. This altered emotional suppressing measured through psychophysiological responses was restored after symptoms disappearance following EMDR. The third experiment also using psychophysiological measures confirmed the increased fear sensitization and delayed fear extinction in PTSD and again the restoration of a normal fear conditioning and extinction processes after EMDR. The last experiment explored the negative emotional cerebral mechanisms using functional magnetic resonance imagery in PTSD. Activities in prefrontal structures were modified in PTSD as compared to healthy controls. After the EMDR treatment accompanied by symptoms removal, the prefrontal responses were not different between PTSD patients and their controls. Theoretical issues of these results will be discussed in order to integrate cognitive, psychophysiological and cerebral mechanisms observations.

A pesar del emerger de muchas teorías sobre los mecanismos biológicos del EMDR, la investigación aún necesita entender el proceso de curación que se produce en EMDR. Hemos realizado 4 experimentos para explorar los efectos del EMDR en TEPT de 17 a 22 pacientes que sufrieron un único trauma. El primer experimento evidencia un sesgo atencional del TEPT ante las palabras negativas que desaparecen después de una terapia exitosa de EMDR. El Segundo experimento mostró una baja eficiencia del control de las emociones en los TEPT comparados con el control de individuos sanos. Esta alterada supresión emocional medida a través de respuestas psicofisiológicas fue restaurada después de una desaparición de los síntomas realizando EMDR. El tercer experimento también confirma mediante medidas psicofisiológicas el aumento de la sensación de miedo y un retraso en la extinción del mismo en el TEPT. De nuevo tras administrar una terapia EMDR se produjo una restauración a una condición normal de miedo y un proceso de extinción. El último experimento explica los mecanismos negativos emocionales cerebrales usando resonancia funcional magnética en TEPT. La actividad en las estructuras prefrontales fue modificada en el TEPT comparado con el control. Después del tratamiento de EMDR acompañado de una remisión de los síntomas, las respuestas prefrontales no fueron diferentes entre los pacientes con TEPT y los controles. Cuestiones teoréticas sobre estos resultados serán discutidas con el fin de integrar cognitivamente, psicofisiológicamente y observar los mecanismos cerebrales del EMDR.

Abstract:
The effects of three, 1.5-hour Eye Movement Desensitization and Reprocessing (EMDR) treatment sessions on traumatic memories and psychological symptoms of 80 subjects were studied. The treatment sessions were administered within a two week period. 40 and 40 men ranging in age from 21-67 were interviewed and selected from a pool of 117. Subjects reported continuous difficulty and suffering (mean 13 years) in some area of their life since the occurrence of the traumatic event. Approximately 1/3 of subjects had no prior therapy experience. Subjects were randomly assigned to either EMDR treatment or delayed EMDR treatment condition, and to one of five EMDR trained therapists. Treatment therapists (licensed psychologists and counselors) consisted of 2 women and 2 men, each working with 5 men and 5 women in each group (gender study issues). The therapists had been trained in EMDR by Francine Shapiro. Each had various levels of EMDR experience and training, ranging from facilitator training with two to three years EMDR clinical experience, to Level I and minimal EMDR clinical experience. Treatment fidelity was consistent throughout the study. Subjects receiving EMDR showed decreases in anxiety and presenting complaints, and increases in positive self-evaluations. The six standardized tests and subjective reports were administered by an objective independent assessor (licensed psychologist) pre and post treatment, and at a 90-day follow-up. Subjects in the delayed EMDR treatment group showed no improvement on any of these measures during the 30 days before treatment. After treatment, the delayed EMDR treatment group showed decreases in anxiety and presenting complaints and increases in positive self-evaluations. All ANOVA interactions for both groups were significant at p < .001. These effects were maintained or improved at the 90-day follow-up. The effects of three, 1.5 hour eye movement desensitization and reprocessing (EMDR) sessions on traumatic memories of 80 subjects were studied. Subjects were randomly assigned to treatment or delayed treatment conditions and to one of give licenses therapists trained in EMDR. Subjects receiving EMDR showed decreases in presenting complains and in anxiety, and increases in positive cognitions (all ps < .006). Subjects in the delayed treatment groups showed no improvement on any of these measures prior to treatment (all ps > .05). After treatment, the delayed treatment group showed similar improvement on all measures. These effects were maintained at the 90-day follow-up.

EMDR Humanitarian Assistance Programs (HAP) began in 1995 as a response to the Oklahoma City bombing. An FBI agent who had previously received EMDR therapy called requesting help, stating that the local mental health professionals were overwhelmed by the task. After a needs assessment, approximately 100 volunteer clinicians trained in EMDR therapy were rotated in to provide pro bono treatment for the bombing victims and front-line responders. A program evaluation indicated that over 80 percent achieved beneficial treatment effects within three sessions, and, in the same year, a study using a delayed treatment control group also showed positive results (Wilson, Becker & Tinker, 1995). Subsequently, free trainings in EMDR therapy were offered and provided to 290 clinicians in collaboration with local agencies. This randomized, controlled group field study was conducted subsequent to a 7.2 earthquake in North Baja California, Mexico. Treatment was provided according to continuum of care principles. Crisis management debriefing was provided to 53 individuals. After this, the 18 individuals who had high scores on the Impact of Event Scale (IES) were then provided with the eye movement desensitization and reprocessing (EMDR) Protocol for Recent Critical Incidents (EMDR-PRECI), a single-session modified EMDR protocol for the treatment of recent trauma. Participants were randomly assigned to two groups: immediate treatment group and waitlist/delayed treatment group. There was no improvement in the waitlist/ delayed treatment group, and scores of the immediate treatment group participants were significantly improved, compared with waitlist/delayed treatment group paticipants. One session of EMDR-PRECI produced significant improvement on symptoms of posttraumatic stress for both the immediate-treatment and waitlist/delayed treatment groups, with results maintained at 12-week follow-up, even though frightening aftershocks continued to occur frequently. This study provides preliminary evidence in support of the protocol's efficacy in a disaster mental health continuum of care context. More controlled research is recommended to evaluate further the efficacy of this intervention.

This ongoing field study was conducted subsequent to the discovery of clandestine graves with 218 bodies recovered in the Mexican state of Durango in April 2011. A preliminary psychometric assessment was conducted with the 60 State Attorney General employees who were working with the corpses to establish a triage criterion and provide baseline measures. The Impact of Event Scale (IES) and the short posttraumatic stress disorder (PTSD) rating interview were administered, and the 32 individuals whose scores indicated moderate-to-severe posttraumatic stress and PTSD symptoms were treated with the eye movement desensitization and reprocessing (EMDR) Protocol for Recent Critical Incidents (EMDR-PRECI). Participants were assigned to two groups: immediate treatment (severe scores) and waitlist/delayed treatment (moderate scores). Each individual client session lasted between 90 and 120 minutes. Results showed that one session of EMDR-PRECI produced significant improvement on self-report measures of posttraumatic stress and PTSD symptoms for both the immediate treatment and waitlist/delayed treatment groups. This study provides preliminary evidence in support of the protocol's efficacy in a natural setting of a human massacre situation to a group of traumatized adults working under extreme stressors. More controlled research is recommended to evaluate further the protocol's efficacy.

This article reports the follow-up results of our field study (Jarero & Uribe, 2011) that investigated the application of the eye movement desensitization and reprocessing (EMDR) Protocol for Recent Critical Incidents (EMDR-PRECI) in a human massacre situation. A single individual session was provided to 32 forensic personnel of the State Attorney General in the Mexican state of Durango who were working with 258 bodies recovered from clandestine graves. Pre-post results showed significant improvement for both immediate treatment and waitlist/delayed treatment groups on the Impact of Event Scale (IES) and Short PTSD Rating Interview (SPRINT). In this study, we report the follow-up assessment, which was conducted, at 3 and 5 months posttreatment. Follow-up scores showed that the original treatment results were maintained, with a further significant reduction of self-reported symptoms of posttraumatic stress and PTSD between posttreatment and follow-up. During the follow-up period, the employees continued to work with the recovered corpses and were continually exposed to horrific emotional stressors, with ongoing threats to their own safety. This suggests that EMDR-PRECI was an effective early intervention, reducing traumatic stress for a group of traumatized adults continuing to work under extreme stressors in a human massacre situation. It appears that the treatment may have helped to prevent the development of chronic PTSD and to increase psychological and emotional resilience.

This presentation will encompass the findings from a current and ongoing research study on EMDR with young children, with implications for clinical practice arising from this study. The clinical experiences of the presenter, which include treating traumatized children and training EMDR therapists, led to the first EMDR fidelity study on children. From that study, our preliminary findings led us to formulate suggestions about training therapists; these ideas will be explained in the workshop.
Examples will be discussed of how issues related to the therapist, client, and patient, home environment, clinical environment, and therapist training all impact the EMDR treatment protocol with children 20 to 10 years of age. Participants will also learn to identify developmentally appropriate and child-specific languaging in order to conceptualize the treatment of children using the EMDR protocol. Using Dr. Shapiro’s Adaptive Information Processing model, participants will learn to attune to the child verbally and non-verbally to understand how the child has learned to store the trauma in their memory network, versus how the parent or therapist believes the trauma to be stored. Specific tools like mapping and graphing that are used to tease out all the pieces of the EMDR protocol and develop case conceptualization will be demonstrated with associated videos. Through the use of Powerpoint presentation, case presentation, and handouts, additional practical and interesting tools will be presented to assist therapists in using Resource Development, Mastery, and Safe Place exercises in the efficacious treatment of young children. Creative tools used to identify targets, emotions, body sensation, and negative and positive cognitions, will be demonstrated, as well as measurements to aid the child in eliciting the VOC and SUDs. Also, the important of the three-pronged approach (the process of addressing targets from the past, present, and future), and how to develop targets from a child’s often concrete perspective, will be discussed. Finally, participants will be able to use a specific format for reevaluation from both the child’s and parents’ point of view. With these advanced skills in translating EMDR into developmentally appropriate terms and imaginative tools for implementation, participants will return to their practices encouraged to use the entire EMDR protocol with even the youngest of clients. The workshop, which is based on clinical experience and research, will teach creative skills in applying the eight-phase protocol to young children.

Deze studies lieten zien dat de kenmerkende oogbewegingen (EM) van EMDR als secundaire taak, tijdens het ophalen van de negatieve herinnering, zorgen voor een afname van emotionaliteit en levendigheid. Dit doordat beide taken competeren voor het werkgeheugen welke slechts een beperkte capaciteit heeft. Deze studie onderzoekt een nieuwe theorie, de afleidingstheorie, welk gebruik maakt van visuele ruis (VN) als alternatieve secundaire taak. Huidig onderzoek richt zich op de vraag of EMDR verklaard kan worden volgens de actieve werkgeheugen belasting van de EM-taak of volgens de passieve belasting van de VN-taak. Om beide condities te vergelijken werd middels een pilot de mate van cognitieve belasting gelijk gemaakt. Aan deze pilot namen 10 participanten deel. Eye Movement Desensitisation and Reprocessing (EMDR) has been a commonly used method of treatment for Post-traumatic Stress Disorder (PTSD). In previous studies, an explanation mechanism for the effect of EMDR demonstrated the working memory theory. These studies showed that the characteristic eye movements (EM) of EMDR as a secondary task during retrieval of negative memories, ensuring a reduction of emotionality and vividness. This is because both tasks compete for the main memory to which only has a limited capacity. This study examines a new theory, the theory derivation, which uses visual noise (UN) as alternative secondary task. Current research focuses on the question whether EMDR can be explained by the active working memory load of the EM task or by the passive load of the UN mission. In order to compare both conditions was a pilot held the degree of cognitive load equal. 10 participants in this pilot took part. The results showed that a delayed cycle of 8 seconds for an equal load caused all conditions. In total there are 30 participants in this study. The first part of the study consisted of a reaction time task (RT) consisting of RT alone, RT + RT + EM and UN. The second part consisted of retrieving the memory only, recall + EM and recall + UN. During the second part was also in each measurement the emotionality and vividness of the memory were measured. The working memory theory predicts that (a) after the intervention the degree of vividness and emotionality in recall + EM significantly lower for the intervention and (b) the average decrease of the score of recall + EM compared to the other two conditions greater will be. The derivation theory predicts that (a) after the intervention the degree of vividness and emotionality in recall + UN significantly lower for the intervention and (b) the observed effect is greater than and equal to only remember remind + EM. The results show that both hypotheses can not be confirmed. It can be seen that visual noise in comparison with the pre-and post-test, contrary to expectation, both emotionality and vividness to a larger decrease in the scores causes compared with eye movements and the control condition. When it was examined between the measurements was to see that the scores of visual noise (eye movements + vividness) in the middle layer between recall and eye movements only in. Previous research has indicated that individuals with chronic pain who exhibit adaptive coping mechanisms are more likely to manage their pain effectively. If they experienced little success with pain management in the past, they are not likely to exhibit adaptive coping strategies in the present. However, there remains an important gap in the literature regarding the use of eye movement desensitization and reprocessing (EMDR) as a strategy for chronic pain. One purpose of this study was to establish the effectiveness of EMDR in the reduction of chronic pain; another was to examine the relationship between chronic pain intensity and emotion, as suggested by the biopsychosocial model of pain and measured by the Multiple Affect Adjective Checklist-Revised. In this matched 2-group design, participants were matched on pretest chronic pain scores and randomized to EMDR and a delayed treatment group. Pre- and posttreatment chronic pain levels were determined using the Short-Form McGill Pain Questionnaire. Data were analyzed using paired samples t-tests. There was a significant reduction in chronic pain following six EMDR sessions for the participants in the experimental group and the delayed-treatment group. The results also indicated a reduction in dysphoria for both groups following EMDR. The findings may help to clarify whether EMDR is effective in alleviating chronic pain, which may lead to more satisfying lives for patients and their families. This study is an important contribution to the literature and enhances social change initiatives by showing that collaboration between EMDR and other types of treatment may offer more expedient and long-term relief to patients, thus affecting the costs (e.g., health insurance) associated with chronic pain.[Author abstract]

Objective/Purpose: Eye Movement Desensitization and Reprocessing (EMDR) is a relatively new form of psychotherapy designed to alleviate symptoms of anxiety, depression, avoidance of trauma related thoughts and intrusive memories associated with Post Traumatic Stress Disorder (PTSD). Under therapist instruction, the patient makes rhythmic eye movements while imagining the traumatic event, designed to facilitate cognitive reprocessing to alleviate stress. Methods: Electronic searching of Medline, PsyClit, Health Star, CCTR, The National Research Register and a range of other databases was conducted. Once trials had been identified, data were extracted and methodological quality assessed. Cost-utility was estimated indirectly through consultation with expert clinicians. Costs per Quality Adjusted Life Year (QALY) gained were also calculated. Results: 12 randomised controlled trials (RCTs) were identified, comparing EMDR to alternative psychotherapy treatments; variants of EMDR; and to delayed treatment groups in various combinations. Background: Eye Movement Desensitization and Reprocessing (EMDR) is a relatively new form of psychotherapy for PTSD. We critically reviewed randomized controlled trials of EMDR. Methods: A wide range of electronic databases and reference lists of articles obtained were searched and relevant experts were consulted. Studies were critically appraised according to established criteria. Results: We found 16 published randomized controlled trials (RCTs) comparing EMDR with alternative psychotherapy treatments, variants of EMDR, and with delayed treatment groups. Studies were generally small (mean number of patients = 35) and of variable methodological quality, with only 5 reporting blinding of outcome assessors to treatment allocation, and in some cases with high loss to follow-up. In most cases EMDR was shown to be effective at reducing symptoms up to 3 months after treatment. In one case benefit was maintained up to 9 months and in another (uncontrolled) followup treatment effect was present at 15 months. 2 studies suggest that EMDR is as effective as exposure therapies, 3 claim greater effectiveness in comparison to relaxation training, and 3 claim superiority over delayed treatment groups. Of the studies examining specific treatment components, 2 found that treatment with eyes moving was more effective than eyes fixed, while 3 studies found the two procedures to be of equal effectiveness. Eye movement desensitization and reprocessing (EMDR) is a recently developed psychotherapy treatment procedure which combines imaginal exposure with eye movements and is reported to dramatically reduce negative symptoms associated with trauma related psychological disturbances and memories. The author reviewed and analyzed the current literature regarding EMDR, and conducted a within-subject design investigating the importance of the eye movement component in the EMDR treatment protocol by comparing the efficacy of an eye movement treatment condition, with two non-eye movement treatment conditions. The use of two different control conditions allowed comparisons of the eye movement condition (EMDR), which involved bilateral stimulation of the brain, with exposure to memory of the trauma without eye movements (Eye-Focus Desensitization), which served as a placebo, and exposure to memory of the trauma with a competing motor activity (Single Hand Tapping), which represented unilateral stimulation of the brain. This study also employed a delayed treatment condition to investigate the overall effectiveness of EMDR in treating PTSD. The subject was a 53-year-old Caucasian female who met DSM-IV criteria for PTSD. Dependent variables included a diagnostic instrument, which was the Structured Interview for Posttraumatic Stress Disorder (SI-PTSD); global instruments, which included the Beck Anxiety Inventory (BAI), Impact of Events Scale (IES), and Subjective Units of Distress scale (SUDs); process measures, which included the Subjective Units of Distress scale (SUDs) and Validity of Cognition (VOC) scale; and a self-report measure of overall improvement, which was the Image Desensitization Rating Scale (IDRS).Results demonstrated support for the superiority of an eye movement condition over that of both a no-eye movement condition (EFD), and a competing motor activity of single hand tapping (SHT) on process variables (SUDs and VOC), but not on weekly global measures (IES, BAI, and SUDs) in the single subject studied. Also, this study found support for the effectiveness of EMDR (delayed treatment phase) in reducing symptoms of anxiety, intrusiveness and avoidance, and subjective distress related to memory of trauma as measured by BAI, IES, and SUDs, and also in alleviating DSM-IV symptoms of Posttraumatic Stress Disorder for this subject. This research project evaluated the effectiveness of using Eye Movement Desensitization and Reprocessing (EMDR) techniques on children and adolescents (N = 20) between the ages of 7 to 18, who were experiencing anxiety due to traumatic memories. EMDR therapeutic techniques were administered in one 90-minute therapy session to subjects in an experimental group (N = 10) and after a 30-day waiting period, EMDR was administered to subjects in a delayed treatment group (N = 10) to help substantiate the effectiveness of using EMDR with this population. The results of the study indicated a highly significant reduction in anxiety related to traumatic memories in both an experimental group and a delayed treatment group, as measured by the Subjective Units of Distress Scale (SUDS), the Impact of Events (IES) scale, and in the delayed treatment group using the Children's Manifest Anxiety Scale (CMAS) after treatment. There was a lack of significance in the reduction of anxiety in the experimental group, as measured by the Children's Manifest Anxiety Scale (CMAS). The results showed a significant difference in cognition, changing from negative to positive thinking, as measured by the Validity of Cognition (VoC) scale. The findings demonstrate that EMDR may help to measure, treat, and enhance the lives of children and adolescents experiencing anxiety from traumatic memories. It is recommended that future research focus on using standardized psychological tests to support the efficacy of using EMDR with children and adolescents. Since Francine Shapiro's first published paper on EMD therapy in 1989, there has been a tendency toward polarization in EMDR research. Those who tend to believe in the effectiveness of EMDR tend to find results which confirm their point of view. Those who have been very skeptical about the effectiveness of EMDR have tended to produce findings which validated their perspective also. The result of this has been years of back and forth research, without a great deal of moving forward by asking new questions based on previous findings. This literature review involved evaluating all available research on EMDR published in English as of March 1, 2002. Studies were categorized as contributing to knowledge about EMDR in general, or emphasizing specific aspects. Specific aspects were breadth of application, subjectivity of effects, EMDR's effect on intrusive PTSD symptoms, the necessity for eye movements, how EMDR works, if it does, and whether it produces lasting change.Findings included a probable effect from EMDR in treating traumatic memories. It has not been found equally effective in treating other kinds of anxiety or other psychological maladies. Subjectivity is an ongoing issue in EMDR research, yet there are several forms of data indicating an effect in a context in which subjectivity could not have been a significant factor. If EMDR works better for intrusive PTSD symptoms compared to others, the difference is minor. The necessity of eye movements has not been clarified, largely because of the use of alternate forms of bilateral brain stimulation as a control condition when these in fact may promote a similar process. EMDR appears to produce change that is as lasting as any other form of psychotherapy. The main conclusion is that there is a paucity of research including a variety of independent variables. The ongoing battle as to whether EMDR works or not has delayed thorough inquiry into for whom it works, compared to for whom it does not work. The purpose of the study was to evaluate, through the use of a randomized experimental design, the effectiveness of EMDR in reducing trauma symptoms in adult female survivors of childhood sexual abuse. No EMDR research to date has been exclusively comprised of adult survivors of childhood sexual abuse, a historically difficult treatment population. Additionally, while numerous clinical accounts of treatment with sexual abuse survivors have been published, controlled treatment research has rarely been done. Of the studies found that examine treatment efficacy exclusively with this population, none involved the use of random assignment.A sample of 60 adult female sexual abuse survivors were selected and randomly assigned to one of three groups: (1) individual EMDR treatment; (2) individual eclectic treatment; or (3) delayed treatment control group. The participating survivors' trauma symptoms were measured in pretests and posttests on standardized as well as subjective instruments that measured anxiety, posttraumatic stress, depression, negative beliefs about the sexual abuse, emotional distress and desired positive self beliefs. The survivors in the study assigned to the experimental or comparison treatment groups received six 90 minute individual sessions of either EMDR or eclectic therapy. The delayed treatment control group subjects were pretested, asked to delay treatment for six weeks, and after being post tested were assigned a therapist with which to work. Data analysis consisted primarily of multivariate and univariate analysis of variance. The posttest results indicated that EMDR was very effective in reducing the targeted trauma symptoms compared to the control group. Eclectic therapy at posttest was also found to be very effective, resulting in a lack of statistically significant differences between the experimental and comparison treatments. However, analysis conducted at the three month follow-up revealed that EMDR was significantly more effective than eclectic therapy at maintaining therapeutic gains. The results of this study suggest that while both EMDR and eclectic therapy, when applied as brief psychotherapy models of treatment for survivors, can produce significant alleviation of trauma symptoms, EMDR may provide more enduring resolution. These findings have important implications for both survivors and the service providers available to them. What if the brain had a similar mechanism for healing psychological injuries as the body does, just like a finger can heal a cut? Imagine tapping into that healing process in the brain and helping a child who witnessed her brother accidentally killed by a school bus, who then developed a school phobia, be able to return to school and eliminate her depression. What if you could help a foster child with a history of severe and chronic abuse, reduce his disruptive symptoms within a 9-month period so that he could stabilize and be adopted? Eye movement desensitization and reprocessing (EMDR) can be used in psychotherapy to help children heal from stressful experiences of both traumatic and developmental origins. And, while EMDR is not a magic wand, it is remarkable in its efficiency in reducing or eliminating significant mental health symptoms and healing the origins of trauma. This chapter is written for clinicians who have had little or no exposure to the EMDR treatment methodology or for those who may have wondered what it is and how it works. The goal of this chapter is to summarize the use of EMDR with children with case presentations woven through the steps of the EMDR protocol. As a potential paradigm shift for child and adolescent therapists who have been trained in child development and play therapy, this chapter will not only explain why EMDR with children and adolescents makes sense, but why EMDR is the treatment of choice for many children presenting with symptoms of trauma. The experienced child therapist will also learn how child development, play therapy, and other child-focused therapies can be integrated to overall case conceptualization with the eight phases of the EMDR protocol. Initially, this chapter provides a brief description of EMDR. While Chapter 5 covered EMDR with adult clients, this chapter will focus on translating the EMDR protocol into child language from a developmentally grounded perspective for use with child clients. The effects of “lethal contact” (i.e., close range firefights) in both the military and law enforcement populations can render long-standing psychological impairment. In this study we evaluated the effectiveness and physiological effects of EMDR in police officers involved with on-duty shootings who had delayed PTSD. Method: Six police officers involved with on-duty shootings and subsequent delayed-onset PTSD were evaluated with standard measures, the Posttraumatic Stress Diagnostic Scale and high-resolution brain SPECT imaging, before and after treatment. Results: All police officers showed clinical improvement and marked reductions in the PDS (mean reduction from scores of 43.2 pre EMDR to 5.2 post EMDR). In addition, there were decreases in the left and right occipital lobe, left parietal lobe and right precentral frontal lobe, as well as significant increased perfusion (>0.001) in the left inferior frontal gyrus. Conclusions: In our study EMDR was an effective treatment for PTSD in this police officer group, showing both clinical and brain imaging changes. This multimedia presentation integrates selected case reviews including the dispatch recordings of the officer’s actual shooting incident/s, follow-up “check-in” messages documenting the officer’s reactions upon return to duty as well as pre- and post-treatment brain images. Brief selections of video also are used to further illustrate key principals. Clinical methodologies that were used with this group of subjects also will be discussed. This randomized, controlled group field study was conducted subsequent to a 7.2 earthquake in North Baja California, Mexico. Treatment was provided according to continuum of care principles. Crisis management debriefing was provided to 53 individuals. After this, the 18 individuals who had high scores on the Impact of Event Scale (IES) were then provided with the eye movement desensitization and reprocessing (EMDR) Protocol for Recent Critical Incidents (EMDR-PRECI), a single-session modified EMDR protocol for the treatment of recent trauma. Participants were randomly assigned to two groups: immediate treatment group and waitlist/delayed treatment group. There was no improvement in the waitlist/ delayed treatment group, and scores of the immediate treatment group participants were significantly improved, compared with waitlist/delayed treatment group paticipants. One session of EMDR-PRECI produced significant improvement on symptoms of posttraumatic stress for both the immediate-treatment and waitlist/delayed treatment groups, with results maintained at 12-week follow-up, even though frightening aftershocks continued to occur frequently. This study provides preliminary evidence in support of the protocol's efficacy in a disaster mental health continuum of care context. More controlled research is recommended to evaluate further the efficacy of this intervention.

Abstract:
Despite the prevalence of childhood trauma, studies regarding psychotherapy for children suffering from PTSD are scarce, especially regarding the treatment for pediatric PTSD following single-incident trauma. Treatment practices for this population rely mainly on the paradigms of therapy for adult PTSD and pediatric PTSD following sexual abuse. This review outlines the studies published in the last 10 years pertaining to the treatment of pediatric PTSD following single-incident trauma. This is done in the context of available literature on the paradigms mentioned above. Of 742 articles dealing with treatment of pediatric trauma, 10 were found relevant to the treatment of pediatric PTSD following single-incident trauma. The modalities of treatment most frequently reported in this context were cognitive-behavioral therapy (CBT), eye movement desensitization and reprocessing (EMDR), and play therapy. As a whole, CBT studies were methodologically more rigorous, used manualized, reproducible treatment, and were group, school-based therapies. EMDR treatments were usually short and individual. Most studies showed statistically significant improvement but were still methodologically lacking. We conclude that research on the subject of treatment for pediatric PTSD following single-incident trauma constitutes a neglected part of the study of pediatric PTSD. This stands in contrast to the obvious prevalence of this type of trauma. We encourage future research that will address issues such as clarifying the role of pharmacotherapy, comparing different modes of treatment, dismantling treatment "packages", researching developmentally sensitive treatments, conducting long-term follow-up, and comparing different PTSD populations. Remember the joke about the doctor who says, "The operation was a success, but the patient died"? That's how some clients feel about EMDR. They succeed in accessing deep and important material, but find themselves extremely depressed and/or anxious in the days afterward. For these clients, who are often survivors of multiple trauma such as long-tenn child abuse or incest, the ability to self-soothe (both during and after an EMDR session) makes the difference between whether they regard EMDR as a useful tool or a necessary evil. To find out what self-control techniques work best for such clients, I interviewed EMDR clients (all women) who had experienced long-term sexual abuse in childhood. The first thing I learned was that for survivors of multiple trauma, the ability to feel safe starts long before EMDR is ever used. Many women cited their relationship with their therapist as the foundation of their feeling safe with EMDR: "I trust my therapist absolutely." One client's therapist told her he had used EMDR himself: "That made a huge difference to me," she said. Other advance work included planning and taking preventative measures. Planning means picking the right time (and pace) for doing EMDR: being sure the therapist and/or other support people will be available in the days after the session; not driving or going back to work afterward (if possible); being able to have plenty of alone time; and going slowly, doing EMDR in small increments. "I didn't expect myself to go out in the world and be social afterward. I was pretty raw for a few days, sometimes for a whole week," B. told me. Planning also means taking preventive measures, such as teaching the client how to find "a safe place." Most clinicians know the importance of this, but one of the women I interviewed was emphatic that creating a safe place was very different from being able to go to it when she was in a session and reliving the experience of being a three-year-old overwhelmed by extreme grief or terror. She said she needed a lot of practice accessing her safe place and some special interventions (see below) to get through the intense times. Being able to self-soothe between sets of eye movements was very difficult for most clients. "I cry all the time we do it," S. told me. "I have to sit near the door and not have my therapist sit too close," said M. Another woman said, "We do the eye movements for a few seconds and we talk in between."One successful intervention, especially for clients overwhelmed by the intensity of their feelings, involved the therapist asking his client to listen to the sound of his breathing and to breathe along with him. Another clinician has his client when she gets extremely upset ask her "inner guide or "higher power" whether it's "okay to continue;" a third asks, "Is there more underneath or is it time to wind down?" Letting the client control the pace and progress of his/her own processing can be an important way to teach self-trust -- especially to people for whom loss of power was endemic to their abuse. Some clients are able to repeat special phrases or afirmations over and over between sets to calm themselves. L., a ritual abuse survivor, said she grounds herself by silently reciting a mindfulness verse from Zen master Thich Naht Hanh in time with her inbreath and out-breath: "In, out. Deep, slow, Calm, ease. Smile, release. In, out. Deep, slow ...... Different kinds of self-soothing techniques work best after the eye-movement sets are completed. Immediately afterwards, while still in session, one client said she falls asleep for a few minutes -- she finds this a big help in countering the dissociated state in which she typically concludes an EMDR session. Another said she and her therapist share a cup of tea and talk over what happened as a way to "come down" and normalize the experience. Some clinicians close a session by doing eye movements to reinforce the client's safe place. One woman said her therapist has her "cement the present in place" by doing eye movements on either a present-day image, an image of her inner child in the safe place, or a positive statement. Francine Shapiro has often said that what happens after the EMDR session can be as important as what happens during it. The women I interviewed felt exactly the same way. They had learned the necessity of talung exquisitely good care of themselves in the hours and days that follow. "I take time-and time out," declared B., who often has a delayed fear reaction following EMDR. Most clients said they go home and either curl up in bed or in a favorite rocking chair with their stuffed animals. They cry, sleep, write in their journals, draw pictures, listen to music, look at favorite photographs, and/or call a support person. M. uses self-talk to ease her feelings: "I say to myself, 'You know that knot of fear. I know it's only fear. I know that nothing is going to hurt me right now'." For others, going home immediately is not the best option: D. takes a walk along the shores of Long Island Sound; C., the mother of three young children, finds solace in a favorite bookstore. Sometimes all the planning in the world doesn't help: the abreaction seems to launch the client back to the age she was when she was abused - and she simply can't remember how to calm herself. To counter this, several clients said they carry a list of things they can do to quiet themselves. S. finds reading mystery stories comforting("At the end you always find out what really happened."), but has to keep two of them on her bedside table at all times: "If they're not in full view, I forget about using them." One interesting example of "assigned" self-soothing was given by a ritual abuse survivor who was new to EMDR. After a session when a lot of memories came up about how her sexuality was used and degraded during the abuse, her therapist gave her very specific instructions on how to care for herself, including buying a romantic nightgown and soaking in bath salts for 45 minutes; listening to romantic music; and not touching or kissing her partner for 48 hours. "It worked out great!" she told me happily. "I felt SO pretty and so safe." The conclusion I reached about how multiple-trauma survivors learn to self-soothe in the face of the intense feelings EMDR can trigger is not revolutionary. The recipe is: Step 1. Plan for the worst. Step 2. Let the client select the self-soothing techniques that specifically fit for her or him. Step 3. Make sure s/he is able to use these techniques no matter how intense his/her emotions are. Sometimes this will call for the therapist to take an active role by either leading the client in specific calming techniques or by assigning very clear-cut homework. If the recipe calls for planning and practicing, then the pot in which the ingredients are cooked is labeled "TRUST"-trust before initiating EMDR, trust during the eye movements, and trust after the sets are completed. Unless the client deeply trusts the clinician, the method itself, and his or her own capacity to go into the feelings and me out safely, the recipe for success with EMDR can turn into a recipe for disaster.

One session of Eye Movement Desensitization and Reprocessing (EMDR) appeared to be an effective treatment for test anxiety, reducing reported physiological distress, worry, and fears of negative evaluation. The research design included two components: a comparison study, comparing Immediate Treatment and Wait List groups, and a replication study comparing the treatment response of Immediate and Delayed (Treated Wait List) groups. 17 test anxious university students were randomly assigned to one session of EMDR or Wait List. At post-test, the Immediate group demonstrated significant improvement, compared to the Wait List group, on the Test Anxiety Inventory (TAI) and Fear of Negative Evaluation Scale. Treatment effects were maintained at follow-up. The Wait List group received treatment after post-measures were taken. Treatment of the Delayed group replicated effects. Improvement was reflected by large treatment effect sizes and a decrease in percentile ranking on the TAI from the 90th to the 50th percentile (Pilots).

Objectives. The psychological study was aimed at evaluation of the Eye Movement Desensitization and Reprocessing (EMDR), hypothesized to be effective in PTSD treatment. Method. Participants in the study were 60 outpatients (16 men and 44 women) aged 18-55, diagnosed with posttraumatic stress disorder (PTSD). They were treated with psychotherapy at the Outpatient Mental Health Clinic in Rzeszów. The patients were randomly divided into two equinumerous groups: "immediate", starting treatment directly after the initial diagnostic assessment, and "delayed", receiving the same treatment after one month delay. In the pre-and post-treatment assessment a PTSD questionnaire by Pasternak was used (in the "delayed" group an additional assessment was made 1 month after the initial one, i.e. directly preceding therapy). Results. In a vast majority of patients a statistically significant reduction in PTSD symptoms severity was noted. Three subgroups differing in treatment outcome (as measured by the Pasternak PTSD Scale scores) were identified, whose therapy was: successful (n = 37), moderately effective (n = 14), or unsuccessful (n = 9). Conclusions. The findings suggest that Eye Movement Desensitization and Reprocessing turned out to be an effective technique in the treatment for PTSD, both in the whole sample under study, and in the subgroups ("immediate" and "delayed"). © 2009 Instytut Psychiatrii i Neurologii

For the past three decades, clinical experience as well as research has supported the validity of counseling and psychotherapy in the treatment of dually diagnosed (MH/MR) clients. At first behavioral therapies and later various forms of process-oriented therapy (Gestalt, creative arts, body-centered, play) have been effectively employed with this population, Particularly through the use of process therapies it has become clear that traditional insight and cognitive therapy must be adapted in favor of experience, action, body-centeredness and “right-brain” functioning to be impactful with this population.
EMDR< given its primarily non-verbal mode of functioning, holds great promise as an effective and efficient therapy for trauma treatment with dually diagnosed clients, a population inherently vulnerable to traumatic impact. Anecdotal case presentations with mental retarded clients, corroborated by both client report and by clinical observation. In all cases, therapeutic gains remained intact after treatment.
This presentation will introduce a “phase model” of trauma treatment for this population and will demonstrate were EMDR would be most effective within this model. Attention will also be given to ways in which the basic EMDR protocol would need to be adapted to meet the needs of these clients.

Trauma treatment after sexual abuse for people with developmental disabilities People with disabilities have an increased vulnerability to experience violence and abuse. Also they have more negative experiences in general and are more often treated in an unpleasant manner. They are therefore at greater risk to develop trauma. Beside that they have fewer capacities to cope with traumatic experiences in a healthy way. For a long time it was thought that people with developmental disabilities couldn’t profit from psychotherapy. Still people with developmental disabilities are being told that they can’t get treatment. In this workshop we will show that psychotherapy is very well possible for people with developmental disabilities and we will show how adjustments can be made to the regular approach to meet the special needs of these people. We will explain that to meet these needs a broader scope is necessary which include therapeutic interventions towards key persons and organizations in their lives. We will introduce two women with mental disabilities who both have been sexually abused. We will tell about the therapy in which we used this broader approach (involving parents, group counselor, church) combined with EMDR, solution focused therapy methods, cognitive behavioral techniques and creative techniques.
Learning Outcomes Participants will know that trauma treatment is possible for people with developmental disabilities. Participants will know adjustments can be made to the regular approach to meet the special needs of people with developmental disabilities. Participants will know how to involve key persons from the network of the developmentally disabled client in order to make trauma treatment work well.

The use of ego-state interweaves and/or extensive ego-state therapy (utilized in the preparation phase) and integrated into EMDR targeting (phases 3-7), in the treatment of personality disorders, has not received a great deal of attention at conference presentations or in the published media. This has led to minimal or nonresponsiveness in the EMDR treatment of personality disorders, since aspects of dissociation in these clients have not been addressed. In the past ten years, renown neuroscientists, such as Eric Kandel, Joseph LeDoux, Michael Gazzaniga and V.S. Ramachandran, in response to empirical findings in the fields of memory, neuromodularity, split-brain research and information processing, have begun to suggest that the “self” may very well be a collection of memories that are structured in a fragmented (neuromodular) multiplicity that is developmentally inherent. Ramachandran, LeDoux and Gazzaniga have, each, stated, explicitly, that the sense of a “cohesive and monolithic” self is an “illusion” created by areas in the left cerebral hemisphere. Accordingly, just as the EMDR standard protocol was adapted for recent traumatic events (in response to acute memory fragmentation), so must it be modified for inherent memory and personality fragmentation, by the use of extensive ego-state work (preparation) and ego-state-specific EMDR targeting (phases 3-7). The implementation of these techniques has shown a remarkable advance in the treatment of personality disorders, which had, previously, been rather impervious to EMDR treatment.

Objectives: Exposure, trauma-focused cognitive therapy and eye-movement desensitisation and re-processing (EMDR) are effective treatments for posttraumatic stress disorder (PTSD) producing equivalent outcomes. How might the field advance? One way is to base new treatments on PTSD maintenance mechanisms. A treatment that does this, metacognitive therapy (MCT), underwent preliminary controlled evaluation in this study. Method: Twenty participants aged 18 to 65 years with chronic PTSD were randomly allocated to either a total of 8 sessions of MCT or a delayed treatment control. Measures of PTSD, emotional symptoms, and underlying metacognitive variables were obtained at pretreatment and posttreatment. Patients were followed-up at 3 and 6 months postintervention. Results Statistically significant reductions in PTSD symptoms, depression, and anxiety at posttreatment were observed in the MCT group but not in the control group. Changes were maintained over follow-up. The average number of sessions delivered was 6.4. Eighty percent of patients (intention to treat) met clinical significance criteria for recovery based on the IES. Treatment was well tolerated with only one (10%) dropout. Changes in thought control strategy hypothesized to be involved in the maintenance of PTSD were found. Conclusions: MCT appeared to be a brief treatment producing high recovery rates. The data add to existing uncontrolled evaluations and provide strong justification for future evaluation of this treatment against existing evidence-based interventions. [Abstract from author].

Bonding and attachment are neurobiologically driven survival mechanisms; but when irreconcilable conflict exists within the innate mechanisms of survival and attachment, individuals can experience extreme distress leading to mental health disorders and physical disease. Although these conditions may begin in infancy, if untreated they can endure for a lifetime. Through the lens of AIP, this workshop will teach a developmentally grounded approach to treating attachment issues in clients of all ages by integrating EMDR with Erikson’s stages and the re-organization of the three-pronged approach in a Reverse Protocol (Adler-Tapia, 2012).

This article describes the treatment of a young man who developed PTSD following the sudden unexpected miscarriage of his unborn child. He was diagnosed with this anxiety disorder while serving a term of imprisonment. He was treated with a controversial psychological treatment for PTSD, eye movement desensitisation reprocessing (EMDR) therapy, during one, 1-hour session. After this session, his progress was monitored by a further three follow-up appointments at 1 week, 1 month, and 3 months, where the self-report questionnaires were repeated and compared with pre-treatment scores. These demonstrated improvement post-treatment and at a 3-month follow-up session. The discussion arising from the treatment will concentrate on EMDR as an effective treatment for PTSD, particularly within a prison setting. Data from recent research highlight the potential risks of PTSD within a community population and the implications for midwives and nurses who treat and care for traumatized patients are discussed. The whiplash syndrome is a complex, poorly understood and controversial cluster of symptoms including spinal pain, cognitive dysfunction, neurologic symptoms and emotional complaints consistent with posttraumatic stress disorder. Perhaps its most perplexing feature is the fact that symptoms frequently are far out of proportion to the severity of the accident itself. The frequency of emotional symptoms has led many physicians to attribute symptoms of whiplash to somatization. The typical syndrome of whiplash includes chronic headaches, spinal and jaw pain, usually classified as myofascial pain. Neurologic symptoms include cognitive dysfunction, positional vertigo, balance disturbance, blurring of vision, photophobia and phonophobia, all of which are attributed to minor traumatic brain injury. Emotional complaints include driving phobias, irritability, hypervigilence, exaggerated startle, flashbacks, depression, nightmares and sleep disturbance. DSM IV compatible or subsyndromal forms of PTSD occur in up to 60% of patients. I began to question the traumatic basis for whiplash when I discovered that most of my patients with delayed recovery had remarkable past histories of trauma, especially child abuse. I discovered that early and rigorous use of somatically based trauma therapies, especially EMDR and Somatic Experiencing resulted in clearing not only of emotional symptoms, but also neurologic and pain-related complaints in many cases. I have concluded that the neurophysiological basis for traumatization includes not only kindled arousal, explicit and procedural memory circuits, but also automatic patterns of neuromuscular bracing, stored in procedural memory analogous to motor skill memory. Bracing patterns of involved muscles represent protective motor reflexes from the moment of injury. Linked to memory and arousal, this kindled circuit leads to perpetuation of regional myofascial pain. Dissociation plays a major role in perpetuation of this phenomenon, and accounts for many of the unusual neurologic symptoms of whiplash. This model conforms to current theories of PTSD as a model of kindling, but includes the somatic element that I believe is a universal part of the syndrome of traumatization. The pervasive neurohormonal effects of trauma account for the remarkable amount of somatic complaints in this syndrome, and may be the basis for many poorly understood chronic idiopathic disease processes. Incorporation of the neuromuscular system in the process of traumatization pleads for the study of somatically-based therapies for PTSD.
