Prazosin is recommended for treatment of Posttraumatic Stress Disorder (PTSD)-associated nightmares. Level A. Image Rehearsal Therapy (IRT) is recommended for treatment of nightmare disorder. Level A. Systematic Desensitization and Progressive Deep Muscle Relaxation training are suggested for treatment of idiopathic nightmares. Level B. Venlafaxine is not suggested for treatment of PTSD-associated nightmares. Level B. Clonidine may be considered for treatment of PTSD-associated nightmares. Level C. The following medications may be considered for treatment of PTSD-associated nightmares, but the data are low grade and sparse: trazodone, atypical antipsychotic medications, topiramate, low dose cortisol, fluvoxamine, triazolam and nitrazepam, phenelzine, gabapentin, cyproheptadine, and tricyclic antidepressants. Nefazodone is not recommended as first line therapy for nightmare disorder because of the increased risk of hepatotoxicity. Level C. The following behavioral therapies may be considered for treatment of PTSD-associated nightmares based on low-grade evidence: Exposure, Relaxation, and Rescripting Therapy (ERRT); Sleep Dynamic Therapy; Hypnosis; Eye-Movement Desensitization and Reprocessing (EMDR); and the Testimony Method. Level C. The following behavioral therapies may be considered for treatment of nightmare disorder based on low-grade evidence: Lucid Dreaming Therapy and Self-Exposure Therapy. Effects of treatment of PTSD on psychobiological measures: It is well known that PTSD is associated with changes in several biological
systems. However little research has been done on whether it is possible to
“reset” these biological systems with effective psychotherapy of pharmacological
therapy. This symposium will present data on neuroendocrine and
neuroimaging outcome measures.
Biological changes in arousal and cortisol following PTSD treatment: This presentation will discuss the results of a controlled treatment outcome
study comparing EMDR, fluoxetine and pill placebo and demonstrate how
effective treatment resulted in changes in memory processes, utilizing the
Traumatic Memory Inventrory. In the EMDR condition, but not fluoxetine,
the change in traumatic memory towards an integrated narrative was linearly
correlated with physiological arousal in response to script driven
imagery. This presentation will also present the relationship between clinical
improvement in the three conditions and change in the cortisol
response to a dexamethasone challenge.

regard to biological markers. The questions here are whether pretreatment
psychobiology or physiological responding can be used to
predict treatment outcome, or whether they themselves change as a
result of effective treatment.
Cortisol pre and posttreatment with EMDR or
prolonged imaginal exposure in PTSD assault
survivors: Many studies have noted increased cortisol production in trauma
survivors with PTSD, but it is not clear whether effective treatment
alters these responses. As part of a larger study, 60 female sexual
assault survivors with PTSD began one of two types of cognitivebehavioral
treatment (Prolonged Exposure (PE) or EMDR). Each
treatment consisted of nine sessions. Sessions 1 and 2 included
information gathering, trauma education, and therapy preparation.
Sessions 3 through 9 consisted of processing traumatic memories
and emotions via either imaginal exposure or EMDR.To examine
potential cortisol changes over the course of treatment, salivary cortisol
samples were collected at three time points during treatment. A
baseline sample was taken at session 1, a second sample was taken at
the start of the treatment portion of therapy (session 3), and a third
sample was taken at the end of treatment (session 9). Of the original
sample of 60 participants, 50 women completed treatment, and ten
dropped out. Cortisol responses will be examined in treatment
responders and non-responders as well as in treatment completers
vs. treatment dropouts.

This study examined changes in salivary cortisol levels pre-to-post-treatment in adult female rape victims diagnosed with post traumatic stress disorder (PTSD) randomly assigned to be treated with either Prolonged Exposure Therapy or Eye Movement Desensitization and Reprocessing. Salivary cortisol was collected at baseline, session 3, and session 9. A significant decrease in salivary cortisol levels was observed in individuals classified as treatment responders in both treatment conditions. Findings suggest that successful exposure-based treatments for PTSD which result in trauma-related and depressive symptom reduction may impact the action of the hypothalamic-pituitary-adrenal axis as measured by changes in level of salivary cortisol from pre-to-post-treatment.

La ricerca ha lo scopo di indicare se l’intervento con EMDR produce significativi cambiamenti dei parametri medico-clinico, oltre che psicologici, in pazienti oncologici che abbiamo ricevuto una diagnosi di PTSD o di un disturbo dello spettro post-traumatico da stress durante ciascuna delle fasi dell’evoluzione della malattia.
In particolare si vuole verificare se dopo trattamento con EMDR:
- i valori baseline di cortisolo nel sangue cambiano e riproducono anche in pazienti oncologici l’andamento dimostrato in letteratura: in presenza di un PTSD acuto generalmente i livelli sono alti, quando invece il PTSD č cronico i livelli di cortisolo sono bassi ma in ambedue i casi un trattamento con EMDR produce una normalizzazione di suddetti valori.
- si osserva una riduzione della quantitŕ di citochine che rappresentano gli indicatori immunologici correlati a stati di depressione ed ansia sia sottosoglia che non, quasi sempre presenti tra i disturbi dello spettro PTSD e quindi anche nella malattia oncologica (Cantelmi, 2008 in preparazione).

The research is intended to indicate whether the intervention with EMDR produces significant changes in clinical parameters and medical as well as psychological, in cancer patients who have received a diagnosis of PTSD or a spectrum disorder post-traumatic stress disorder during each of the stages of the disease.
In particular, we want to check if after treatment with EMDR:
- The values of baseline cortisol in the blood change and reproduce even in cancer patients the trend shown in the literature: in the presence of an acute PTSD generally the levels are high, when instead the PTSD is chronic cortisol levels are low but in both cases treated with EMDR produces a normalization of these values.
- There is a reduction in the amount of cytokines representing the immunological indicators related to states of depression and subthreshold anxiety is that, almost always present between the spectrum disorders PTSD and therefore also in the oncological disease (Cantelmi, 2008 in preparation).

Neki ljudi doţive traumatska stanja koja ne mogu u potpunosti opisati rijeĉima. Mnogi od njih nikada ni ne progovore o tome što su doţivjeli. Ali patnja se u njima nastavlja i organizam, a ĉesto i okolina plaćaju ogromnu cijenu. U takvim sluĉajevima psihoterapija koja koristi rijeĉi je nemoćna. Istraţivanja upućuju da jedna, relativno nova psihoterapijska metoda desenzitizacija pokretima oĉiju i reprocesiranje (EMDR), moţe pomoći i takvim osobama.
Kada neko iskusi ozbiljnu psihološku traumu, ĉini se da dolazi do narušavanja ravnoteţe u nervnom sistemu. Ta ravnoteţa je, moguće, narušena i posredstvom razliĉitih medijatora poput adrenalina, serotonina, dopamina, kortizola itd. Zbog takve neravnoteţe onemogućeno je optimalno funkcioniranje sistema za procesiranje informacija, a informacije vezane uz traumatiĉan dogaĊaj poput slika, zvukova, afekata i fiziĉkih senzacija se zadrţavaju u disfunkcionalnom, uznemirujućem obliku. Danas izranja shvatanje da postoji неуролошки balans u razliĉitim fiziološkim sistemima što omogućuje da informacije budu procesirane na adaptivan naĉin. EMDR kod nekih traumatiziranih djeluje ĉak i kada osoba ne moţe govoriti o tome što je preţivjela. Ono što je bitno jeste da se fokusira na traumatski dogaĊaj i da suraĊuje sa terapeutom u procesu stimulisanja dualne paţnje što omogućava adaptivno procesiranje informacija. Adaptivno procesiranje znaĉi uspostavljanje adekvatnih asocijacija i pojavu da iskustvo biva konstruktivno ugraĊeno u pozitivne kognitivne i emocionalne sheme pojedinca. Odnosno, oslobaĊa od simptoma i omogućava da se nova iskustva doţive bez blokirajućeg uticaja traume.

outcome studies using four different treatment modalities (CBT,
EMDR, psychopharmacology and Cognitive Processing) and presents
data on comparative efficacy, treatment responsiveness and
resistance, effects on comorbidity, quality of life, and biological
changes that accompany symptom improvement.
Differential Biological Outcomes of EMDR and Fluoxetine for PTSD: Two major developments in PTSD research have been the development
of effective treatments and the delineation of pathophysiology.
While major gains have been made in both of these areas, they
have occurred in parallel. There is a need for investigations of possible
differential effects of different treatment modalities on biological
aspects of PTSD. In this randomized controlled trial (N = 105), we
compared the exposure treatment Eye Movement Desensitization
and Reprocessing, the serotonergic reuptake inhibitor fluoxetine,
and pill placebo for their effects on both PTSD symptoms and biological
parameters. The outcome measures were severity of reexperiencing,
avoidant/numbing, and hyperarousal symptoms; psychophysiological
reactivity to script-driven imagery; basal salivary
cortisol and dexamethasone suppression test (DST). At pre-treatment,
post-treatment and 3-month follow-up, saliva samples were
acquired at 8 a.m., 11 p.m. (immediately followed by dexamethasone)
and 8 a.m.. Participants also underwent a script-driven
imagery protocol utilizing four 30s scripts, each followed by a 60s
script imaging period and 2 minute recovery periods (fixed order:
neutral, trauma, neutral, trauma). Preliminary analyses partially support
hypothesizes concerning differential efficacies of pharmacological
and psychological treatments on different symptom clusters
and biological markers of the disorder, at post-treatment and 3-
month follow-up. Potential implications for treatment and future
research will be discussed.

Since its introduction, Eye Movement Desensitization and Reprocessing (EMDR) (Shapiro, 1989) has received the attention of many mental health professionals. There has been much critical debate on the subject of EMDR. Most of the clinical discussion has centered on the role of EMDR in the treatment of Posttraumatic Stress Disorder (PTSD).
While the EMDR procedure has been compared to Mesmerism (McNally, 1999), declared as pseudoscience (Herbert, Lilienfeld, Lohr, Montgomery, O’Donohue, Rosen, and Tolin, 2000), or regarded as a highly marketed placebo (Lilienfield, 1996), most studies support the efficacy of EMDR in treating PTSD (Ironson, Freund, Strauss, and Williams, 2002; Lee, Gavriel, Drummond, Richards, and Greenwald, 2002; Marcus, Marquis, and Sakai, 1997; Rothbaum, 1997; Van Etten and Taylor, 1998; Wilson, Becker, and Tinker, 1997). There has been some evidence for accompanying physiological changes in PTSD subjects treated with EMDR with patterns of cortex functioning, (Levin, Lazrove, and van der Kolk, 1999; Nicosia, 1994) event-related potential changes (Lamprecht, Kohnke, Sack, Matzke and Munte, 2004), as well as positive effects on the level of the stress hormone cortisol (Haber, Kellner and Yehuda, 2002).

Trauma survivors with PTSD have been shown to have lower basal cortisol levels in the urine, plasma, and saliva than in trauma survivors without PTSD, nontraumatized mentally ill, or healthy subjects. We report on a case study in which we measured pre- and post-Eye Movement Desensitization and Reprocessing (EMDR) treatment salivary cortisol levels and salivary cortisol response to 0.50 mg of dexamethasone in a 41-year-old female with chronic PTSD symptoms. Our goal was to determine whether symptom improvement following trauma-focused treatment (EMDR) is associated with changes in basal salivary cortisol or in the cortisol response to dexamethasone administration. Our findings show moderate symptom improvement, an increase in basal cortisol levels, and a more attenuated cortisol hypersuppression in response to the dexamethasone suppression test following EMDR treatment. These results suggest the potential utility of including neuroendocrine measures in the assessment of treatment outcome in PTSD. [Author Abstract]

da Stress. In questa tesi viene riportato uno studio su un caso singolo nel quale
sono stati misurati i livelli di cortisolo urinario pre e post-trattamento EMDR (Eye
Movement Desensitization and Reprocessing) in una ragazzina di quindici anni affetta da
Disturbo Post-traumatico da Stress. L’obiettivo č determinare se il trattamento produce un
decremento dei sintomi, misurati attraverso una valutazione testistica, e se a questo
decremento č associato un aumento dei livelli di cortisolo urinario. I risultati indicano una
diminuzione dei sintomi che ha comportato una remissione del PTSD. Questo
miglioramento č confermato anche da un aumento dei livelli di cortisolo urinario. Questi
risultati confermano l’efficacia dell’EMDR (trattamento evidence-based) per la cura del
Disturbo Post-traumatico da Stress e suggeriscono l’utilitŕ di includere misurazioni

The thesis “Trauma da abuso e EMDR: presentazione di una ricerca in corso” aims at
exploring the trauma from a psychological and neurobiological point of view. According to
the latest studies, trauma survivors with posttraumatic stress disorder (PTSD) have been
shown to have not only posttraumatic symptoms - as Avoidance, Intrusion and Arousal -
but also lower basal cortisol levels than in trauma survivors without PTSD. In this thesis a
single case study, in which pre and post–Eye Movement Desensitization and Reprocessing
(EMDR) treatment urinary cortisol levels have been measured in a 15-year old female with
PTSD, has been reported. The aim of this study is the assessment of the EMDR treatment
effectiveness in symptom reduction measured with psychological instruments; besides, the
assessment of the changes in the urinary cortisol levels following the symptom reduction.
Results show a symptom reduction with a PTSD remission associated with higher urinary
cortisol levels. These results confirm the EMDR treatment effectiveness with PTSD and the
utility of including neuroendocrine measures in the assessment of treatment outcome in
PTSD.

Twenty-four participants from a local domestic violence center were selfselected
into an individual therapy-plus-standard advocacy group (P/SA) or a
standard advocacy (SA) group. The center’s advocacy staffed administered
treatment for the SA group and the P/SA group. In addition to the SA treatment
received by individuals in the P/SA group, there were also six licensed
therapists who provided a manualized treatment incorporating CBT and
EMDR. Symptoms of PTSD, depression, CSE, self-esteem, and coping skills
were assessed.Additionally, saliva sample measurements were taken to examine
the effect of treatment on the regulation of salivary cortisol. Results indicate
that participants in the P/SA treatment condition experienced a
significantly greater reduction in intrusive symptomatology and overall level
of PTSD symptom severity, as well as a greater increase in CSE over time
than those individuals in the SA condition. Moreover, changes in CSE were
related to changes in psychological symptoms in individuals in the P/SA group
from pretreatment to posttreatment and at follow-up.Almost no linear relationship
between changes in CSE and other psychological variables in the SA
group emerged.A minimal impact of treatment on coping skills was observed.
There was a move towards a greater regulation of salivary cortisol at posttreatment
in the P/SA group.

Treatment outcome of fluoxetine vs. EMDR in PTSD: This NIMH funded study compared the efficacy of two widely different
treatment approaches for treating patients with PTSD: fluoxetine,
which acts directly on biological systems (N=30), and Eye
Movement Desensitization and Reprocessing (EMDR) (N=30). There
also was a pill placebo control group (N=30). We assessed subjects
with a multi-modal biological and psychological assessment, in
order to determine whether treatment efficacy is associated with
changes: 1) social adjustment, 2) psychophysiological reactivity to
personalized trauma scripts (heart rate and skin conductance), and
3) basal salivary cortisol. We also tracked the stability of symptom
change for nine months following the cessation of active treatment.
Preliminary results suggest that at the end of 8 weeks of treatment,
there is a 30% improvement in the pill placebo condition, while both
active treatments demonstrate additional symptom improvement,
with EMDR being most effective for the treatment of acute PTSD,
and Prozac for subjects with prolonged childhood histories of trauma.
Clinically significant improvement in CAPS scores is accompanied
by an increase in basal cortisol and improvement in social and
occupational functioning. We will also present data on the differential
rates of symptom change in the different PTSD symptom clusters
