Abstract:
سابقه و هدف: با توجه به ماهيت علامت محوربودن درمان دارويی، از روش های درمانی حساسيت زدايی توام با حرکات چشم و پردازش مجدد (EMDR) و شناختی- رفتاری (CBT) در درمان اختلال استرس پس از ضربه (PTSD) استفاده می شود. هدف اين مطالعه بررسی تفاوت تاثير دو روش درمانی EMDR و CBT در اختلال استرس است.
روش بررسی: در اين مطالعه مورد شاهدی 51 رزمنده مبتلا به PTSD بستری در بيمارستان ايثار اردبيل يا ساکن در شهر اردبيل به روش نمونه گيری تصادفی ساده انتخاب شدند و به صورت تصادفی به سه گروه تقسيم شدند.روش مطالعه، آزمايشی گسترش يافته و طرح تحقيق از نوع پيش آزمون ـ پس آزمون چندگروهی بود. ابزارهای مورد استفاده شامل آزمون خاطره های آزاردهنده، مقياس براشفتگی ذهنی، مقياس شناخت واره های مثبت و مقياس اضطراب و افسردگی بيمارستانی بود.
يافته ها: روش های درمانی EMDR و CBT باعث کاهش معنی داری در متغيرهای خاطره های آزاردهنده، اضطراب و افسردگی و برآشفتگی ذهنی شد و ميزان اعتماد به شناخت واره مثبت به طور معنی داری افزايش يافت. روش درمانی EMDR در مقايسه با CBT در کاهش علايم PTSD رزمندگان ايرانی موثرتر بود، با اين وجود هر دو روش در کاهش علايم اين اختلال موثر بودند.
نتيجه گيری: با توجه به اثر درمانی EMDR و CBT در درمان PTSD، پيشنهاد می شود به منظور پيشگيری و کاهش علايم اختلال استرس پس از سانحه جنگ در رزمندگان ايرانی از روش های درمانی فوق در مراکز درمانی استفاده شود.
Background: According to symptom oriented of drug therapy in the treatment of post-traumatic stress disorder (PTSD), eye movement, desensitization and reprocessing (EMDR) and cognitive behavioral therapy (CBT) were increasingly used. The aim of this study was to compare effects of EMDR and CBT in acute stress.
Materials and Methods : In this case-control study of 51 combat PTSD patients admitted to hospital or residing in the sacrifice of Ardabil Ardabil simple random sampling were selected randomly divided into three groups. Methods, expanded testing and research design type were tested before Chndgrvhy test. Test tools used included disturbing memories, anger scale, mental, cognitive scale Varh positive and the hospital anxiety and depression scale.
Results : EMDR and CBT treatments significantly reduced the variables disturbing memories, anxiety and depression and mental frustration and level of confidence in recognizing the positive Varh significantly increased. EMDR therapy compared with CBT in reducing PTSD symptoms was more effective Iranian combatants, however, both methods were effective in reducing symptoms of this disorder.
Conclusion : According to the therapeutic effect of EMDR and CBT in treating PTSD, is recommended to prevent and reduce symptoms of post traumatic stress disorder in war veterans of the Persian mentioned therapies used in treatment centers.

The purpose of this study is to explore the treatment outcomes from the intervention of Eye Movement Desensitization and Reprocessing (EMDR) with personal loss. Through in-depth understanding of the client’s subjective experiences and changing courses throughout the entire therapeutic process, the researcher intends to prove that EMDR therapy has efficaciously helped the client overcome the trauma from personal loss and regain confidence and vitality. The researcher has interviewed a client who is willing to share his life and EMDR therapy experiences through a semi-structured questionnaire. With the client’s permission, the researcher has obtained the client’s clinical records. After methodically synthesizing the various data, the researcher has developed a case description presenting both the life experience and the intervention of EMDR therapy of the client. The researcher has analyzed the data, generalized the client’s adaptive coping processes throughout EMDR therapy and discussed the effects from the psychiatrist’s and other counselors’ interventions. El estrés subsiguiente después de un traumatismo, representa un trastorno disfuncional – tanto interno como externo – que se manifiesta en alteraciones en el reconocimiento cognitivo y en los comportamientos, llevando además asociados síntomas somáticos, afloramiento de problemas inconscientes y ansiedades. Pues una de las características del estrés post-traumático, es precisamente la pérdida de equilibrio entre el mundo interno y externo de quien lo sufre. Por eso, ante la complejidad de las respuestas post-traumáticas, éstas se pueden catalogar en gran medida dentro de las perturbaciones psicopatológicas.
El estrés psicológico surge por una situación estresante “real”, externa, tangible y la reacción ante esta difícil experiencia, evoca un conjunto bastante universal y coherente de síntomas y respuestas que provocan reacciones primitivas relacionadas con temores inconscientes ante las amenazas a la vida, que hacen aflorar incipientemente fantasías e impulsos abrumadores. Los resultados son los pensamientos disfuncionales que conducen a las respuestas y a los comportamientos desadaptativos.
Objetivos: el taller está diseñado para proporcionar a estudiantes y profesionales de la Psicología – que trabajan o desean trabajar en esta área del trauma y el cambio de comportamientos -, la habilidad para utilizar eficaz y rápidamente intervenciones breves, que puedan poner en practicar incluso en casa. En el taller se estudiará la forma inicial de evaluar, tanto al trauma como a la clientela. Se trabajará la historia del trauma y se profundizará en sus consecuencias y en cómo diseñar las intervenciones breves para hacerles frente. También se centrará en averiguar qué cambios quieren los y las clientes a través de relatos y visualizaciones, utilizando una adaptación de las terapias EMDR y la Solución Enfocada tanto a crear el cambio deseado, como a mantenerlo.
En consecuencia el taller es de particular interés para quienes trabajan con personas que han sufrido cualquier tipo de trauma, o quienes perciben acontecimientos de la vida, experiencias, etc. que les afectan de forma negativa en su día a día. También es útil para las personas que sufren de TOC, sobre todo trastornos del pensamiento.
Los aspectos más útiles de las dos técnicas que se han adaptado junto con otras innovaciones de la autora, guardan relación con la creación de un ambiente seguro, no-traumático, que actúa rápidamente y que además, se puede realizar en casa sin peligro de consecuencias negativas.
Objetivos específicos:
1. Ser capaz de describir e identificar las manifestaciones del trauma.
2. Aprender y describir dos técnicas terapéuticas breves en el tratamiento del trauma
3. Definir una técnica breve terapéutica que puede ser utilizada para fomentar el cambio
4. Destacar el rol del o de la terapeuta durante el tratamiento de los traumatismos
5. Ser capaz de describir, diseñar y establecer metas de la terapia y promover cambios mediante el uso de técnicas de terapia breve.
Métodos: la Desensibilización y Reprocesamiento por Movimientos Oculares (EMDR), es un método complejo e integrador de la psicoterapia individual, mediante el que se guía al cliente utilizando un procedimiento para acceder a sus experiencias y resolver sus problemas conductuales y emocionales. El EMDR utiliza elementos de múltiples orientaciones psicoterapéuticas tanto psicodinámicas, como cognitivo- conductuales, enfoques centrados en el cliente, gestalt y bioenergéticos.
La premisa subyacente de EMDR es que las experiencias de pánico y ansiedad se procesan de forma diferente por el cerebro que las experiencias habituales. La teoría subyacente es que durante el estrés, la memoria grava en una parte del cerebro responsable de las emociones de modulación (la amígdala) y se cierra temporalmente otra parte del cerebro (el hipocampo), responsable de procesamiento de la memoria normal. La experiencia traumática queda atrapada en el exterior y potencialmente no forma parte del procesamiento normal del cerebro, y el EMDR permite a la persona acceder a la experiencia y transformarla en memoria declarativa en el hipocampo. Con el método EMDR, el hipocampo se puede abrir a las emociones evocadas por la experiencia para que el/la cliente pueda soportarlas mientras se realiza el tratamiento. La distracción y la atención a la estimulación bilateral, desempeñan un importante papel que ayuda al cliente a experimentar las emociones como tolerables. Aunque cómo la distracción bilateral en concreto, facilita el procesamiento de las experiencias dolorosas, sea algo que todavía no se termina de entender.
Por otra parte, las Intervenciones Breves de Terapias enfocadas al Cliente se centran en las excepciones del problema, pensando que a continuación se desarrollará un cambio natural en el comportamiento. Es una especie de visión orientada no en las formas tradicionales, sino hacia el futuro, sin profundizar demasiado en la “patología” sino más bien centrándose en lo que el sistema puede hacer para adaptarse a ella, puesto que ambos pueden decidir si esa “patología” es un problema o no lo es.
Las Intervenciones de Terapia Breve enfocadas al Cliente se utilizan para resolver una variedad de problemas de comportamientos y actitudes, mediante el uso de los propios recursos de los y las clientes y las observaciones de las estrategias que utilizan para alcanzar los resultados deseados, en sus situaciones vitales habituales. Se trata de una buena técnica para establecer y mantener un contexto de cambio en el que los pequeños, pero útiles cambios, se anticipan y se buscan.
En definitiva, la combinación de ambas técnicas con algunas variaciones desarrolladas por la Dra. Barreda-Hanson, han demostrado ser una herramienta poderosa para mejorar la respuesta al tratamiento en un período de tiempo más corto, teniendo también la ventaja de permitir practicar los ejercicios en casa.
Aplicaciones: la aplicación habitual del EMDR ha sido el tratamiento de trastornos emocionales relacionados con eventos muy perturbadores o traumáticos. Pero también se usa para trabajar síntomas preocupantes como la ansiedad, la depresión, la culpa y la ira. E igualmente, se puede utilizar para mejorar recursos emocionales tales como la confianza y la autoestima.
Procedimientos:
- El taller se impartirá en español y el alumnado recibirá amplios folletos complementarios.
- Se realizará en una única jornada, en sesión de mañana para teoría y de tarde para prácticas, trabajando cada modelo por separado.
- Se espera que quienes asistan lleven una cuestión-problema sobre la que trabajar utilizando las diversas técnicas, pues aunque se utilizarán múltiples ej. de casos reales, se alentará a quienes participen a traer sus propias experiencias e ideas para debatir y trabajar sobre ellas.

Methods: Desensitization and Reprocessing Eye Movement (EMDR), is a complex and inclusive method of individual psychotherapy, which is guided by the client using a procedure to access their experiences and address their behavioral and emotional problems. The EMDR uses multiple elements of both psychodynamic psychotherapeutic approaches as cognitive-behavioral, client-centered approaches, gestalt and bioenergy.
The underlying premise of EMDR is that experiences panic and anxiety are processed differently by the brain than normal experiences. The underlying theory is that during stress, gravel memory part of the brain responsible for emotions modulation (amygdala) and temporarily closes another part of the brain (hippocampus), responsible for normal memory processing. The trapped traumatic experience abroad and potentially not part of the normal brain processing, and EMDR allows people access to the experience and transform it into declarative memory in the hippocampus. With EMDR, the hippocampus can be opened to the emotions evoked by the experience that he / the client is able to bear while performing the treatment. Although bilateral distraction how specifically facilitates the processing of painful experiences, is something that is not yet fully understood.
Moreover, brief interventions focused Customer Therapies focus on the exceptions of the problem, thinking that then will develop a natural change in behavior. It is a kind of non-oriented view on traditional forms, but to the future, without going too deeply into the "pathology" but rather focus on what the system can do to adapt to it, since both can decide whether this "pathology "is a problem or not.

I had only three weeks to work. Only 15 days to restore self-confidence, serenity, and determination for a woman rider. One week during the competition.
The woman rider had a bad experience during the Olympic Games of Athens. She had the best and the worst during the Olympic Games, The best was a bronze model and 0 mistakes during the jump.
The worst, she made several mistakes and fell down with her horse.
I felt during the first meeting a big atmosphere of fragility (no self confidence, no serenity, big stress) because there were difficulties with her ex-husband and family (her boys).
We had a short time and I decided to use an arrangement with mental imagery and specific EMDR exercise, like butterfly exercise, to install peak performance.
I used Sam Foster’s protocol for the sport and butterfly technique. We began with a SUD=8 and VOC=6 and the SUD finished at 2. This combination gave an amazing result because she rode well and she had a good result in these world equestrian games of AAchen (semi final: 23 place).
The specific exercise in EMDR (butterfly exercise) permits a peal performance in a few times. I gave you an example about the power of EMDR in sport.

Les techniques d’hypnose éricksonienne sont des outils très utiles au cours d’une psychothérapie, spécialement avec les personnes ayant connu des traumatismes répétés dans l’enfance. En effect, par le biais d’une dissociation thérapeutique, patient et thérapeute on accès à la mémoire traumatique neurobiologique et psychologique et le patient pourra (ré)experimenter la confiance dans ses propres forces naturelles.
Cependant, manié avec peu de précaution l’hypnose risqué d’aggraver la dissociation pathologique. L’association des techniques éricksoniennes et du protocole de la thérapie EMDR augmente les capacitiés de l’espirt et du corps et offre au paitent un meilleur contrôle émotionnel et un amélioration del la (ré)orientation à la réalité.

Ericksonian hypnosis techniques are useful tools in the course of psychotherapy, especially with people who have experienced repeated trauma in childhood. Standardized effect sizes and confidence intervals thereof are extremely useful
devices for comparing results across different studies using scales with incommensurable
units. However, exact confidence intervals for standardized effect
sizes can usually be obtained only via iterative estimation procedures. The present
article summarizes several closed-form approximations to the exact confidence
interval bounds in the two-independent and two-dependent samples
design. Monte Carlo simulations were conducted to determine the accuracy
of the various approximations under a wide variety of conditions. All methods
except one provided accurate results for moderately large sample sizes and converged
to the exact confidence interval bounds as sample size increased.

Behandeling van ‘Complex trauma’ is lastig, omdat er vaak op veel verschillende levensgebieden problemen zijn. Daarbij is er sprake van een opeenstapeling van traumatische ervaringen. Dit kan leiden tot een soort schrik of terughoudendheid bij behandelaren, om complex trauma adequaat aan te pakken. In deze workshop willen wij duidelijk maken dat complex trauma goed te behandelen is, mits men de ruimte heeft om een langere behandeling aan te gaan, een therapeutische relatie (met tegenoverdrachtelijke valkuilen) aan kan gaan met cliënten met een geschokt wantrouwen in hun medemens en men niet te snel terugschrikt en mits men goed overzicht houdt over het verloop van de behandeling. Wij presenteren een model dat richting geeft aan de behandeling van complex trauma. We gaan uit van het drie-fasen model (Herman, 1992) met stabilisatie, verwerking en integratie en vullen dit aan met handvatten voor praktisch gebruik. We zullen uit elke fase een of meerdere technieken demonstreren en op een rijtje zetten hoe EMDR toegepast wordt bij de behandeling van j complexe traumaproblematiek.
Treatment of 'Complex trauma is difficult, because there are often many different areas of life problems. In addition, there is an accumulation of traumatic experiences. This can lead to a kind of fear or reluctance of clinicians to adequately handle complex trauma. In this workshop we want to make clear that complex trauma can be treated well, provided they have the space for a longer treatment to enter a therapeutic relationship (with counter-transference traps) to can deal with clients with a shaken confidence in their fellow man and one not afraid to quickly and if one does good overview over the course of treatment. We present a model that gives direction to the treatment of complex trauma. We assume the three-phase model (Herman, 1992) with stabilization, processing and integration and supplement this with handles for practical use. The model we use to systematically change the options to determine in complex trauma problems. We will phase out any one or more techniques and demonstrate how this straight EMDR is used in the treatment of complex trauma problems j.

This case study shows that after 10 sessions, the pain intensity had diminished from 10 to 1 (on a scale of 10). Further sessions, consisting mainly of discussions, focused on consolidation of the result, namely on finding a new physical and mental balance and on strengthening self-confidence in the new situation.

EMDR (Eye Movement Desensitization and Reprocessing) is a trauma treatment with origins date back to 1987 by its founder Francine Shapiro. EMDR is based on an information-processing model and is being used to treat traumatic symptoms. Clinicians in many types of clinical settings worldwide employ EMDR for a board range of treatment issues including grief, depression, anxiety, physical and sexual abuse. Although there is evidence that EMDR is effective in treatment of Posttraumatic Stress Disorder, more research needs to be done in clinical practice settings to contribute to the much needed research base about effective of EMDR with other issues.
Therapists working with trauma are often dealing with high caseloads and administrative requirements of the agencies that they work with. The clinicians involved in this research have observed that there have been some encouraging developments in their community which have served to build bridges for clinicians to increase their confidence in the world of research. In 2007, an intiative called Research Skills Development Program offering mentoring in research skills development was offered in association with Lakehead University and Northern Ontario School of Medicine, Ontario, Canada.
Completion of this 1-year program proved to be the first step in make the transition from clinician to researcher. From there, partnerships were formed within the agency and community. With the support of our agency, researchers were able to gain access to testing and computer software that made data collection and data anaylsis possible with least disruption to our clinical services.
The clinicians were encouraged by previous results from our first study in 2007 (n=6) to continue systematic data collection with more clients (n=6). Approval for this recent study was received by the Ethics Committee of St. Joseph's Care Group.

Elevated psychophysiological parameters and heightened physiological reactivity to trauma-related cues
are acquired changes following trauma exposure. Measuring improvement in these variables is an appropriate
evaluation of outcome in treatment studies. Heart Rate Variability (HRV) is a computerized measure
of physiological responsivity derived from Holter ECG recording. Four female outpatients with persistent
post-traumatic symptoms and personal impairment following “small t” trauma exposure underwent a
course of EMDR treatment and were assessed at baseline, end of treatment, day 30 and day 90 of followup,
using self-report symptom scales and 90-min Holter ECG recordings. Symptom scores decreased
between baseline and end of treatment, with improvement maintained at follow-up. Several HRV measures
changed favorably in different recording intervals. HRV is a feasible and sensitive method to measure
physiological changes in the treatment of individuals distressed by “small t” trauma. Further investigation
is advisable to expand these preliminary data.

This study investigated factors contributing to clinician attrition from eye movement desensitization and reprocessing therapy. The primary areas under investigation were factors contributing to attrition and retention, as well as the quest to obtain information pertaining to training experiences. A sample of 239 clinicians was purposefully obtained. Several patterns emerged within the data, highlighting the issues of attrition: Loyalty to other treatment modalities and discomfort with using EMDR, either through lack of confidence, inadequate training, or discomfort with client distress, were both correlated with clinician attrition. Treatment efficacy and ongoing consultation were remarkable for retention. Practice setting surfaced as statistically significant, and operating in private practice was positively correlated with greater participation in EMDR support activities. Additionally, the adequacy of the training format was presented, as well as how prepared the participants felt after completion of EMDR training. A number of findings were consistent with earlier studies and the converging results of loyalty to previous modalities and discomfort using EMDR gave rise to recommendations for future training and support of newly trained clinicians. The limitations of the current study were presented, in addition to directions for future research.

This paper reports on a qualitative, exploratory
study that creatively combined two therapeutic approaches: eye movement desensitisation and reprocessing (EMDR) and solution focused brief therapy. It was expected that the use of techniques drawn from solution focused brief therapy could help children to facilitate and enhance the use of techniques within EMDR. In particular, techniques focusing on current and future positive resource installation were utilised. Five primary school children age 9 to 11 years were identified by class teachers and parents as presenting as shy, anxious and lacking self-confidence. The group intervention took place in school over six sessions. Within the group setting, the children targeted their own individual area that they wished to improve. Data were collected during and after each session and post-intervention. Increases in children’s self confidence and an improvement in targeted areas were reported by children, parents and teachers. Children found the intervention helpful and were able to identify particular aspects of solution
focused brief therapy and EMDR that were useful.

Although hypnosis is now less popular as an interviewing technique in forensic investigations than it used to be, recent evidence suggests that some of the components of hypnotic interviewing might still be useful in the development of brief memory facilitation procedures. Two experiments are described which continue this componential approach to hypnotic interviewing. In the first experiment, the effects on episodic memory of a brief context reinstatement (revivication) procedure were examined together with a focused breathing meditation technique which shares similarities with traditional hypnotic induction. A second experiment investigated the effects of horizontal eye movements which some have also associated with hypnotic responding. Results indicated that a combined context reinstatement and focused meditation procedure was more effective than context reinstatement alone in facilitating memory for an emotional event without the increase in false positive errors familiar to more traditional hypnosis techniques. In contrast, an instruction to perform horizontal eye movements was not effective in facilitating memory and, when combined with a suggestion for improved recall, produced higher confidence in incorrect responses. Abstract: While the EMDR aims at disturbing information processing towards an adaptive state, the fact remains that a thorough and after preparation of the person, in order to ensure its internal resources, greatly facilitates the work, cooperating greatly to the success. I consider, therefore, a matter of responsibility to nurture therapeutic advance for patients with a wide and effective range of exercises to strengthen their sense of security and confidence. In this sense, the contribution of my experience may show that: encourage the development of "Psicocorpórea Consciousness" (1) becomes not only a great ally of the humans involved in the therapeutic process, but also of EMDR, the therapeutic method itself. Deze jongeren komen slechts zelden met een eigen hulpvraag bij de GGZ. Meestal trekken de ouders aan de bel, soms school, soms een andere hulpverlener.
Om met adolescenten te kunnen werken is het essentieel dat we aansluiting zoeken en vinden bij de jongere zelf en hoe die zijn probleem ervaart, ook al kan deze probleemdefinitie lijnrecht tegenover de aanmeldingsreden staan. Als er een hulpvraag is gevonden die erkent kan worden door de jonger zelf en door zijn ouders/opvoeders heeft de behandeling meer bodem om te kunnen starten.
Een indicatie voor EMDR-behandeling vraagt altijd om goede voorlichting en uitleg. Bij jongeren speelt vervolgens mee dat de identiteitsontwikkeling en de daarmee gepaard gaande onzekerheidsgevoelens er vaak aan bijdraagt dat EMDR als een “ vreemde gang van zaken” wordt beschouwd. De therapeut moet genoeg vertrouwen in zichzelf en de therapie hebben om de jongere ook het vertrouwen te geven om in proces te durven gaan. Soms gaat dit in golfbewegingen.
Het effect van de behandeling kan om dezelfde reden ontkend worden door de jongere zelf ( er zijn dingen veranderd maar dat kan natuurlijk nooit komen door dat rare gedoe, dat komt vast door die nieuwe verkering die nu zo gelukkig maakt) en belangrijk is dan om ook de context van de jongere erbij te betrekken. Ouders, brusjes of/en vrienden en school kunnen belangrijke informatiebronnen zijn om een goede inschatting te maken over het eventuele effect van de behandeling.
Timing wanneer een EMDR-behandeling kan starten zal rekening moeten houden met schoolvakanties, proefwerken, PTA’s, (examen)feesten…omdat het belangrijk is om niet teveel risico te nemen op verbreking van het verwerkingsproces.
Teenagers and adolescents, aged between 12 and 18 years have intense, rapidly varying stages of development where it is much matching the therapist asks how he or she can enter into contact with juveniles and their parents.
These young people are rare with an own demand for care in mental health. Usually the parents pull the bell, school sometimes, sometimes another helper.
To be able to work with adolescents is essential that we seek and find connections with the young people themselves and what they are experiencing problems, even though this problem definition are diametrically opposed to notification reason. If there is a demand for care is found to be acknowledge by the younger self and his parents / guardians, the more soil treatment to start.
One indication of EMDR therapy always requires good information and explanation. Among young people is then that the identity development and the associated uncertainty feelings that often contributes EMDR as a "foreign affairs" is seen. The therapist must have enough confidence in himself and the therapy to the young people the confidence to dare to go to trial. Sometimes this in waves.
The effect of the treatment can be denied for the same reason the young people themselves (there are some things changed but that can obviously never get through the crazy stuff that is established by the new traffic that is so happy) and also important is to the context of the younger involvement. Parents, siblings and / or friends and school information important to a good estimate on the possible effect of the treatment.
Timing when an EMDR treatment will have to start with school, papers, PTA's, (exam) parties ... because it's important not to take too much risk of breaking the process.

The present study was designed to isolate the effects of the eye-movement component of the Eye Movement Desensitization and Reprocessing (EMDR) procedure in the treatment of fear of public speaking. Seventy-one undergraduate psychology students who responded in a fearful manner on the Fear Survey Schedule II and on a standardized, self-report measure of public speaking anxiety (Personal Report of Confidence as a Speaker; PRCS) were randomly assigned to one of four groups in a 2 × 2 factorial design. The two independent variables assessed were treatment condition (imagery plus eye movements vs. imagery alone) and type of imagery (fear-relevant vs. relaxing). Dependent variables assessed were self-reported and physiological anxiety during exposure and behavioral indices of anxiety while giving a speech. Although process measures indicated exposure to fear-relevant imagery increased anxiety during the procedure, no significant differences among groups were found on any of the outcome measures, except that participants who received eye movements were less likely to give a speech posttreatment than participants who did not receive eye movements. Addition of the eye movements to the experimental procedure did not result in enhancement of fear reduction. It was concluded, consistent with the results of past research, that previously reported positive effects of the EMDR procedure may be largely due to exposure to conditioned stimuli (ScienceDirect).

After the status of PTSD was established, subjects were randomly assigned to one of three conditions; EMDR, exposure control (EC), and group (GT). Subjects in EMDR condition received at least five but no more than eight sessions of EMDR. EC condition subjects therapy procedure as the EMDR subjects but without the eye movements. Subjects in the GT condition received five to eight session of group therapy only.
Outcome measures were at intervals; prior to therapy, immediately following therapy and at two follow-up periods. These include: (1) self report or interview-related psychological measures (Combat Exposure scale, MMPI-2 PTSD, Veterans Adjustment Scale (VETS), Mississippi Scale, Hamilton (Depression and Anxiety); (2) behavioral outcome measures (employment, treatment seeking behavior medication therapy, and re hospitalization rate); and (3) psychophysiological response measures (skin conductance, frontalis EMG, heart rate and blood pressure). The last measures involved a change measure in psychological arousal during exposure to tape recorded scripts depicting the patients' most traumatic combat memory. In addition to these pre-, post-,
follow-up measures, measures (SUD, profile of mood scale (POMS), and impact of events scale (IOE) were taken at each therapy session.
Early results on selected outcomes show differences in positive outcome between conditions POMS EMDR>GT (p<.01); IOE Avoidance, EMDR>GT (p<.04); IOE Intrusion, EMDR>GT(p<.03); Heart Rate, EMDR>GT (p<.04). Presently, there were no other significant differences between EMDR or EC. Trends, however suggest that EMDR may be superior to EC on several of the measures.
These results indicate that EMDR may be producing greater reduction in the conditioned emotional response to traumatic memories in these patients, when compared to group therapy approach commonly used to treat these types of patients in a special VA treatment program.

A few years ago, my friend Ally, who had a somewhat unwarranted confidence in my knowledge of Things Psychological, asked if I knew anything about EMDR; she had suffered some severe childhood trauma, and was thinking of trying it. "EMDR?" I asked, "Is that a new street drug?" I guess that showed her what I knew. She explained that her therapist had suggested this relatively new technique, Eye Movement Desensitization and Reprocessing, that had been shown to ease symptoms of Post Traumatic Stress Disorder (PTSD) in soldiers and rape victims. From what she understood, the therapist would help her to focus on her devastating memories of childhood abuse while directing her eyes to twitch rapidly from side to side. This, according to her shrink, would help her to better "process" her memories. "Sounds like hypnotism," I expertly analyzed. "Who knows, maybe it will work." More recently, when I wrote here about the neurobiological advantages of emotionally "finding a safe place," several readers also wrote in to ask me about EMDR. Now I understand why people want an expert opinion (and I'm no expert, by the way): a heap of controversy surrounds this popular technique.

Working with traumatized clients can be overwhelming
for the therapist, especially when the trauma is complex, involves
dissociative symptoms or different personality states, fragmented
memories or client affect is intense and poorly regulated. My
experience as a supervisor of EMDR practitioners has shown that
it is not uncommon for therapists, in an attempt to be helpful to
the traumatized clients, to unintentionally use strategies, which
are experienced as re-traumatizing or which lead to an increase
in their clients’' survival based coping strategies, including the
further strengthening of the ANP (Apparently Normal Personal-
~ t y- van der Hart, Nijenhuis and Steele. 2006). This workshop
highlights different therapeutic challenges, which often arise
for therapists when working with trauma and introduce strategies
that EMDR practitioners can use in their work with such
clients. This workshop embeds the principles of Positive Growth
Therapy (PGT - Herbert, 20071, which encompasses combined
knowledge from a variety of disciplines, including positive psychology,
information-processing theory, neurobiology, somatic
psychology, developmental psychology and attachment theory,
mindfulness and others. These strategies, designed to nurture
growth rather than dysfunction, are linked to specific therapeutic
factors relevant to the work with trauma, such as different types
of trauma, the nature of dissociation, the therapeutic pathway
toward integration. the concept of safety, the importance of resource
installation, individual pacing of therapy and the window
of tolerance, different types of processing, and the integration of
rational and experiential processing systems and others, which
will be explored in the course of this workshop
This workshop offers opportunities for both, EMDR therapists,
who are fairly new to the trauma field and would like to enhance
and deepen their knowledge base, and those already experienced in the trauma field who would like to use this workshop
as an opportunity to re-view, further refine or validate

El EMDR, es un método psicoterapéutico, efectivo para reducir la ansiedad extrema a los exámenes académicos, a niveles normales, en un 88.8%, de los casos, en 9.25 sesiones de tratamiento, como promedio, con un nivel de confianza del 95 y 99%, en las alumnas de los sextos cursos, del Instituto Tecnológico “Eloy Alfaro, de la ciudad de Esmeraldas, durante el año 2.006. In this study, which is kind of explanation, then use the hypothetical-deductive method with a quasi-experimental design with experimental and control group, formed at random, with pre and post treatment, with statistical processing of data, with the Student t test, and ANOVA before and after treatment, and once tested the hypothesis, it was established that:
EMDR is a psychotherapeutic method, effective in reducing extreme anxiety to academic tests to normal levels in 88.8% of the cases, treatment sessions 9.25, on average, with a confidence level of 95 and 99 % of pupils in the sixth course, the "Eloy Alfaro" Technological Institute in Esmeraldas City Ecuador during the year 2006.

Abstract:
Two new brief treatments, Alpha Theta Brainwave Training (ATBT) and Eye Movement Desensitization and Reprocessing (EMDR) were compared to Systematic Muscle Relaxation (SMR) training during the treatment of 45 hospitalized veterans in the Chemical Dependency Treatment Program of the Fresno Department of Veteran Affairs Medical Center. Fifteen veterans were randomly assigned to three treatment groups, ATBT, EMDR, and SMR, to investigate whether these treatments affected self-efficacy, or confidence in the ability to resist the urge to drink. In behavioral change, self-efficacy levels are consistent predictors of short and long-term success. It was hypothesized that the greater the amount of time spent in theta brainwave frequency (4-8 Hz) during treatment, the greater the increase in self-efficacy. Brainwave activity, temperature, skin conductance and electromyographic levels were recorded during the treatment sessions. Measures of self-efficacy, self-efficacy expectancy, outcome expectancy, and level of overall physical and emotional symptoms were taken before and after treatment. Results indicated that all treatments increased self-efficacy and decreased overall physical and emotional symptoms in alcoholic subjects. Overall, there was no significant difference in the amount of time spent in theta brainwave frequency between groups, but results indicated that the treatments did produce a significant difference in the amount of time spent in theta brainwave frequency between the first and last treatment sessions in the groups. This case study chronicles the application and effects of a customized combination of therapies, including neurofeedback, Interactive Metronome® (IM), and Eye Movement Desensitization and Reprocessing (EMDR) created to help a professional athlete improve his brain function and performance. Brian, a 28-year-old professional baseball player, sought help for difficulty maintaining focus and concentration while playing baseball. He felt his challenges impeded his athletic performance, and he wanted to perform at the highest possible level during the upcoming spring training season. Brian's history combined with the results of a QEEG brain map led to a diagnosis of Attention Deficit Disorder–Inattentive Type. The individualized treatment program for Brian included neurofeedback to lower theta activity and increase beta, IM to improve coordination and timing, and EMDR to address his feelings of anger and lack of self-confidence. At the conclusion of his training, Brian described feeling “clear-headed” and was able to focus his mind when needed. His timing and coordination improved, and he showed a positive attitude about playing baseball and felt confident that he would perform well at spring training.

A recent meta-analysis of PTSD treatments (van Etten & Taylor,
1998) found EMDR to be as efficacious as behavioral and drug
treatments. There is considerable controversy, but little research, on
the underlying mechanisms of EMDR. A conditioning model by
Dyck (1993) suggests that eye movements (EM) effect a distraction
from trauma related thoughts, causing an extinction trial.
According to this model, the distraction of EM should cause
thoughts to be directed outward. A psychodynamic model by Allen
and Lewis (1996) suggests that EM facilitate the formation of new
associations to traumatic memories and allow the client to “remain
in the present while thinking of the past.” This model postulates
that EM cause thoughts to be directed inward. We report two
experiments in which thought processes were studied using a
stream of consciousness (SOC) technique (Singer, 1993). In both
studies, undergraduate participants wrote down a sad (or happy)
target event from their life. They then thought about the target
event and let their thoughts go where they may for 10 minutes. At
approximately 1-minute intervals they were asked to report their
thoughts. The baseline study (n = 42) looked at SOC with eyes
closed; the second study (n = 27) compared SOC with eyes open,
eyes closed, and with EM. Relative the to the eyes-open condition,
EM tended to keep the SOC internally focused. During the last 4
minutes of the SOC, eyes open participants were externally
focused (thoughts about the surroundings) about 50% of time;
EM participants were externally focused 25% of the time; and eyes
closed participants were externally focused 3% of the time, F(1,
11) = 6.08, p = .017. Eye movements produced a blend of external
(eyes open) and internal (eyes closed) thoughts, offering support
to the psychodynamic model.

I recently developed a simple new statistic that can increase the
power to detect the actual effects of EMDR (or any treatment) in a
treatment study. Most treatment studies now report effect sizes,
which, on the specific outcome measure being evaluated, provide a
standardized way of conveying the impact of the treatment, the amount
of the change in the outcome measure. In treatment comparison studies
in which the design specifies that the treatment ends when symptom
reduction criteria are met, the number of sessions to termination is
normally reported. Although these are appropriate outcomes to examine,
when treatment efficiency is divided into two separate findings, the
effect is watered down and may be insufficient for statistical significance.
Thus, mere trends favoring EMDR (or any treatment) cannot be
interpreted with confidence.

The study shows use of EMDR therapeutic method in treatment of eating disorders with binge eating episodes with and without purging with clients treated with cognitive-behavioral psychotherapy.
Dissatisfaction with the client’s own body and inadequacy stemming from the influence of cultural elements seem to be the highest maintenance factors of eating disorders associated with binge eating episodes.
The study analyzes n.10 cases of clients with various dysfunctional ideas caused by distorted constructions with respect to their self-esteem, self-acceptance and personal value, as shown both in their EDI 2 (Eating Disorder Inventory – 2 by David M. Garner) scores and the analysis of their life histories.
The SQ (Symptom Questionnaire by R. Keller) was also administered since it is particularly indicated for administration at short intervals.
The EDI 2 and SQ tests have been administered after n.6 EMDR sessions.
Results: Use of EMDR on the negative cognitions formulated, learnt and maintained by clients in their experience, as well as on alternatives to binge eating, has produced a considerable reduction of their dysfunctional ideas, and binge eating behaviors showing additionally an increase of the drive to change, leading to an increased compliance and autonomy in therapy progress.

Epidemiologisch onderzoek wijst op een prevalentie van enuresis nocturna bij
adolescenten van 1.2% (Nappo, 2002; Chiozza, 1998). De behandeling van
deze groep wordt belemmerd door een fors gedaalde motivatie voor
behandeling zowel als gevolg van de vele faalervaringen met eerdere
behandelpogingen, als door een toegenomen negatief zelfbeeld en gebrek aan
zelfvertrouwen.
Dit artikel laat zien hoe EMDR (Eye Movement Desensitization and
Reprocessing) op experimentele basis werd gebruikt bij enkele adolescenten
met zeer hardnekkige enuresis nocturna. Het traumatiserende effect van
langdurende enuresis nocturna wordt omschreven. Met een casus wordt
geïllustreerd hoe EMDR wordt ingevlochten in de gedragstherapeutische
behandeling. Ten slotte worden de bevindingen beschreven bij de adolescenten
waar inmiddels ervaring mee is opgedaan.
There are several treatment choices for children with enuresis
nocturnal (Azrin, 1974, London, 1984, Hira Sing, 2004, Directive Enuresis
the PAZ, NHG standard nocturnal enuresis). Despite these
treatment options, there is a group of children into adolescence
faced with a very stubborn form of primary nocturnal enuresis.
Epidemiological research indicates a prevalence of nocturnal enuresis in
1.2% of adolescents (Nappo, 2002; Chiozza, 1998). The treatment of
This group is hindered by a sharp fall in motivation
treatment both because of the many failures of previous experiences
treatment attempts, and by an increased self-esteem and lack of
confidence.
This article shows how EMDR (Eye Movement Desensitization and
Reprocessing) was used on an experimental basis in some adolescents
very persistent nocturnal enuresis. The traumatic effects of
long-term nocturnal enuresis is defined. With a case is
illustrate how EMDR is interwoven in the behavioral
treatment. Finally, the findings described in adolescents

This research project investigated how EMDR Therapists integrated EMDR into their clinical
practice post-training and what, if any, difficulties they experienced. A total of 74 respondents
completed a questionnaire and 9 respondents were interviewed using a semi-structured interview,
giving a total of 83 respondents. 40% of both samples (which came from around the world)
reported experiencing difficulties with integrating EMDR into their clinical practice. The types of
difficulties which they reported included differences between EMDR protocol and the therapists’
original training and orientation, patient characteristics, therapists’ own anxieties and confidence
as well as organizational and management hostility to EMDR up to and including bullying of the

The case study presents EMDR-psychotherapy on a female patient,
physically and emotionolly abused by her partners during the course of
three marriages. EMDR-therapy was the second part of the therapeutic
process; the first part successfully treated PTSD (the patient was violently
raped by a stranger) using imaginative stabilisation techniques a half a year
ago. Complex PTSD symptoms in the patient (constant tension, sleep
disorder, anxiety, anhedonia] surfaced during a new relationship. Therapy
took 6 sessions in 8 weeks, in 3 of which the EMDR-technique was used. At
the end of therapy all symptoms decreased and feelings of inferiority were
transformed into increased self-worth, self-confidence, inner satisfaction and
new autonomy.

Resource development and installation (RDI) is an eye movement desensitization and reprocessing (EMDR)-related procedure developed to strengthen positive associations in positive and resourceful memories (Korn & Leeds, 2002). This study tested the assumption that bilateral stimulation (horizontal eye movements [EM]) in RDI “appears to lead to spontaneous, rapid increases in affective intensity . . . and to rich, emotionally vivid associations” (Korn & Leeds, p. 1469). This study also tested whether eye movement effects could be better accounted for by working memory or by interhemispheric interaction theory. Fifty-three undergraduate students each recalled three memories of pride, perseverance, and self-confidence. They provided pretest and posttest ratings of each memory for vividness, pleasantness, and experienced strength of the positive quality, before and after performing three simultaneous tasks during recall: horizontal EM, vertical EM, and recall only. Results were fully in line with working memory predictions, with significant decreases for all variables following both eye movement tasks. There was no support for the interhemispheric hypothesis. It is concluded that the effectiveness of bilateral stimulation in RDI is questionable. Clinical implications are discussed.

Eye Movement Desensitization and Reprocessing (EMDR) is a one-on-one form of psychotherapy that is designed to reduce trauma-related stress, anxiety, and depression symptoms associated with posttraumatic stress disorder (PTSD) and to improve overall mental health functioning. Treatment is provided by an EMDR therapist, who first reviews the client's history and assesses the client's readiness for EMDR. During the preparation phase, the therapist works with the client to identify a positive memory associated with feelings of safety or calm that can be used if psychological distress associated with the traumatic memory is triggered. The target traumatic memory for the treatment session is accessed with attention to image, negative belief, and body sensations. Repetitive 30-second dual-attention exercises are conducted in which the client attends to a motor task while focusing on the target traumatic memory and then on any related negative thoughts, associations, and body sensations. The most common motor task used in EMDR is side-to-side eye movements that follow the therapist's finger; however, alternating hand tapping or auditory tones delivered through headphones can be used. The exercises are repeated until the client reports no emotional distress. The EMDR therapist then asks the client to think of a preferred positive belief regarding the incident and to focus on this positive belief while continuing with the exercises. The exercises end when the client reports with confidence comfortable feelings and a positive sense of self when recalling the target trauma. The therapist and client review the client's progress and discuss scenarios or contexts that might trigger psychological distress. These triggers and positive images for appropriate future action are also targeted and processed. In addition, the therapist asks the client to keep a journal, noting any material related to the traumatic memory, and to focus on the previously identified positive safe or calm memory whenever psychological distress associated with the traumatic memory is triggered.
The underlying mechanism for how this process works to reduce trauma-related stress, anxiety, and depression is unknown. Researchers have theorized that the positive effect is due to adaptive information processing, the theoretical model behind EMDR. Through adaptive information processing, the dual-attention exercises disrupt the client's stored memory of the trauma to allow for an elimination of negative beliefs, emotions, and somatic symptoms associated with the memory as it connects with more adaptive information stored in the memory networks. Once recall of the trauma no longer elicits negative beliefs, emotions, or somatic symptoms and the memory simultaneously shifts to a more adaptive set of beliefs, emotions, and somatic responses, it is stored again, overwriting the original memory of the trauma.
EMDR is typically delivered in 60- to 90-minute sessions, although shorter sessions have been used successfully. The number of sessions varies with the complexity of the trauma being treated. For an isolated, single traumatic event, one to three sessions may be sufficient for treatment. However, when the trauma involves repeated traumatic events, such as combat trauma and physical, sexual, or emotional abuse, many more sessions may be needed for comprehensive treatment.

In the context of managed care, effective short-term treatment has become a priority for psychologists. This is particularly true for the recalcitrant symptoms of PTSD which are often associated with protracted treatment and disappointing outcome.This study investigated the efficacy of Eye Movement Desensitization and Reprocessing (EMDR) as a short-term treatment for PTSD. The independent variable, EMDR, was introduced sequentially in a multiple baseline design across subjects. 5 survivors of rape who met DSM-III-R criteria for PTSD received 4 to 6 weekly sessions of EMDR provided by five licensed psychotherapists with Level 2 EMDR training. Treatment outcomes included (a) large reductions in symptomatology evident in self-monitored data and objective measures, (b) replication of treatment effect in 5 out of 5 subjects, (c) qualitative and behavior change data which corroborate treatment effect, and (d) analyses which demonstrate the clinical significance of the changes, as well as the statistical significance of the differences between pretreatment and follow-up scores (with a confidence level of .05 or beyond). The study suggests that EMDR is very effective for treating long-term, recalcitrant symptoms of PTSD which have not resolved with time or previous therapy. The study provides a methodological model for calibrating treatment and developing accountability for treatment efficacy which can be applied across treatments and settings. [Author Abstract]

Eye Movement Desensitization and Reprocessing (EMDR) was developed in 1987 by Francine Shapiro, as a modality for relieving anxiety, traumatic memories, intrusive thoughts, and reprocessing negative self-beliefs to positive self-beliefs. One of the most common uses of EMDR in recent years has been the treatment of PTSD.This current study investigated the effects of EMDR across a sample of 3 sexually abused women diagnosed with PTSD using a multiple baseline design across subjects. The study specifically focused on whether or not intervention with EMDR effects traumatic memory and negative/irrational cognitions, decreases stress or changes levels of anxiety, depression and heart rate. The study intended to assess the efficacy of EMDR while simultaneously reduce human suffering and answer some of the more serious criticisms which have blurred confidence in EMDR outcome research. Specifically, the study controlled for a number of the criticisms in the literature predominantly through a confirmation of an accurate PTSD diagnosis and through the use of a multiple baseline design. The multiple baseline design was applied sequentially to the same problem across different but matched subjects sharing the same environmental conditions. Heart rate level and well-known psychometrics were used to obtain baseline, intervention and post-intervention measures. Psychometric scores reflecting levels of depression, anxiety, and subjective levels of the impact of distress regarding the trauma were assessed along with the levels of anxiety currently experienced about the trauma and subjective ratings regarding the acceptance of the preferred, self-generated positive cognition. The measures used in this study were an initial clinical interview, an Anxiety Disorders Interview Schedule for the DSM-IV, Beck Depression Inventory, Beck Anxiety Inventory, Wolpe's Subjective Unit of Disturbance Scale, Validity of Cognition, Impact of Event Scale and heart rate. The study reported descriptive statistics to analyze the multiple baseline study and to determine EMDR's clinical significance in treating PTSD. The effects of EMDR on the three PTSD subjects of this study demonstrated that meaningful changes occurred in several areas. Subjective disturbance and stress surrounding the traumatic memory decreased, positive self-cognitions increased, and both depression and anxiety levels decreased following EMDR treatment. No change in heart rate physiology occurred. All of the study's treatment measures were maintained at follow-up. This study explored the effects of eye movements, as compared with eye stabilization and an attention control in reducing anxiety related to public speaking in a non-clinical sample. Nineteen subjects with public speaking anxiety identified their worst memory of speaking in public and either moved their eyes in cadence with the experimenter's fingers, fixated their eyes on the experimenter's fingers, or simply talked about negative experiences related to public speaking. Reduction in anxiety was measured by the Subjective Units of Disturbance scale (Wolpe, 1982) or the Personal Report of Confidence as a Speaker (Paul, 1966). Cognitive change was measured by the Validity of Cognition scale (Shapiro, 1989). Global symptoms were measured by the Symptom Checklist-90-Revised (SCL-90-R, Derogatis, 1983). Analysis of the data indicated that there was no significant difference between groups on any of the measures. This study did not explore the efficacy of Eye Movement Desensitization and Reprocessing (EMDR), but rather the effects of eye movements alone. Abstract:
Forty-two female gymnasts ages 10-16 participated in a study to determine the effectiveness of Eye Movement Desensitization and Reprocessing (EMDR) on state anxiety. Each subject identified excessive anxiety on a gymnastics element resulting from a fall, injury, observed fall, or debilitating repetitive thought process. Subjects were divided into random treatment and control groups. The treatment groups received up to 3 sessions of EMDR. Both process and outcome measures were analyzed using the Subjective Units of Disturbance scale, Validity of Cognitions scale, Sport Competition Anxiety Test, and Competitive Sport Anxiety Inventory-2. EMDR was found effective in reducing cognitive anxiety, somatic anxiety, and increasing self-confidence on the targeted element. Comparison over time from pretest to follow-up revealed no significant differences in trait anxiety for either the control or experimental group. Treatment effects of reduced cognitive anxiety, reduced somatic anxiety, and increased validity of positive cognitions were maintained for 90-days following EMDR treatment. Results indicated four processesing patterns facilitating trauma resolution: methodological, transitional object, unfinished business, and insight oriented processing. Recommendations for future research include the use of EMDR with physiological measures of state anxiety, and the replication of the four distinct processing styles found in this research. We assessed the effects of psychotherapeutic treatment on psychophysiological
hyperarousal and self-regulation during confrontation with a traumatic
reminder in 15 subjects with PTSD before, shortly after EMDR-treatment
and at 6-month follow-up.All subjects underwent a psychphysiological evaluation
with a modified traumascript paradigm including assessment of scriptprovoked
heart rate changes and heart rate variability. Respiratory sinus
arrhythmia (RSA) as the component of heart rate variability closely related
to vagal tone was derived from the time-series of inter-beat intervals via
polynomial band-pass filtering (Porges 1992). Psychometric instruments (IES,
PDS, SUD-Scale) were used to assess treatment outcome.We found a significant
overall reduction in psychometric measures of PTSD-symptomatology
as well as significantly decreased levels of script provoked HR-acceleration
after EMDR-treatment (11.3 ± 10.8 bpm vs. 5.1 ± 5.7 bpm, p < .03). As
expected, traumascript presentation suppressed RSA-levels reflecting the
effect of stress on vagal regulation. In comparison pre- vs. follow-up RSA
increased significantly during baseline (5.06 ± .98 vs. 5.86 ± 1.2, p < .004) as
well as during traumascript (4.55 ± 1.26 vs. 5.55 ± 1.23, p < .02). Our findings
of higher RSA-levels in combination with reduced HR reactions on a traumatic
reminder after successful psychotherapy can be interpreted as a therapy
mediated enhancement of biologically determined self regulation capacities.

Introduzione: i pazienti traumatizzati presentano alterazioni sintomatiche quali intrusività, evitamento ed aumentato arousal, che ostacolerebbe la possibilità di elaborazione/integrazione
delle tracce mnesiche, oltre ad esprimersi a livello periferico. Tecniche specifiche di trattamento sembrano promuovere
l’elaborazione/integrazione delle memorie traumatiche, tra cui la Eye Movement Desensitization and Reprocessing
(EMDR). Nella ricerca valutativa è quindi necessario affiancare alle scale psicologiche un appropriato indicatore
biologico di attivazione neurovegetativa. La variabilità della frequenza cardiaca (Heart Rate Variability – HRV) esprime
l’integrità funzionale del sistema neurovegetativo in risposta
allo stress e può essere misurata nei domini di tempo o di frequenza. Lo studio della HRV è stato applicato in psichiatria ai disturbi
d’ansia (panico, DOC, PTSD) e dell’umore (depressione unipolare) in ricerche trasversali (confronto con controlli sani) e longitudinali di trattamento farmacologico (triciclici,
SSRIs) ma in pochi casi a trattamenti psicoterapici. Gli AA. valutano l’impiego della HRV come indicatore biologico nel trattamento psicoterapeutico di pazienti con Disturbo dell’adattamento erdurante da oltre un mese dopo l’esposizione ad eventi vitali stressanti (EVS). Metodi: sono stati reclutati 6 soggetti (M/F = 1/5, età 40,5 ± 11,0) esposti ad EVS ed avviati a ciclo di trattamento psicoterapico
breve (4-6 sedute a cadenza settimanale) di tipo
specifico (EMDR) o generico (colloqui supportivi). Costituivano criteri di esclusione: età < 18 o > 65 anni; comorbilità psichiatrica, neurologica e cardiologica; uso di farmaci interferenti. Le valutazioni psicopatologiche sono state eseguite al reclutamento
(TBASE: colloquio anamnestico, MINI, Brief
COPE), a inizio e fine ciclo di trattamento (TINI, TFINE: IES, SCL-90-R), a uno e tre mesi di follow-up (T30, T90: IES, SCL-90-R, Brief COPE). Le registrazioni Holter sono state effettuate ad ogni intervallo valutativo coprendo: 60 min. attività libera, 10 min. tilt-test, 3-5 min. ascolto dell’evento traumatico, 30-45 min. seduta psicoterapica. Sono stati impiegati test statistici non parametrici per l’analisi
delle correlazioni (Spearman) e delle differenze
(Wilcoxon). Risultati preliminari: vengono valutate le correlazioni a TINI e le variazioni T90 vs. TINI. Sono significative le seguenti correlazioni: ansia fobica SCL e SDNN (dev. standard intervalli R-R) [r = + 0,9; p = ,037]; collera-ostilità
SCL e SDNN [r = -0,95; p =.014]; depressione SCL e
r-MSSD (radice media somma quadrati diff. R-R) [r = + 0,9; p = ,037]; sint. intrusivi IES e LF (basse frequenze) [r
= -0,9; p = ,037]. Sono risultate statisticamente significative le seguenti variazioni:
IES totale [Z = -1,99; p = ,046], sintomi intrusivi IES [Z = -2,21; p = ,027], sintomi di evitamento IES [Z = -1,99; p = ,046], ideazione paranoide SCL [Z = -2,21; p = ,027]; R-R, LF e LF/HF (rapporto basse/alte frequenze) durante ascolto evento [Z = -2,02; p = ,043].
Discussione: a livello basale gli indicatori HRV di distress vegetativo correlano positivamente con collera-ostilità e sintomi intrusivi, negativamente con ansia e depressione. Anche dopo tre mesi dalla fine del trattamento gli interventi psicoterapeutici tendono a ridurre i punteggi sintomatici, e migliorano il bilancio simpato-vagale durante il riascolto dell’evento traumatico.
Introduction: trauma patients have symptomatic changes such as intrusiveness, avoidance and increased arousal, which hampers the development / integration
of memory traces, as well as speak at the peripheral level. Specific techniques of treatment seem to promote
the formulation and integration of traumatic memories, including eye movement desensitization and reprocessing
(EMDR). In evaluation research is therefore necessary to combine psychological scales appropriate indicator
organic autonomic activation. The heart rate variability (Heart Rate Variability - HRV) expresses
functional integrity of the autonomic nervous system in response
stress and can be measured in time or frequency domains. The study of HRV has been applied in psychiatric disorders
anxiety (panic, DOC, PTSD) and mood (unipolar depression) in cross-disciplinary (compared with healthy controls) and longitudinal drug treatment (tricyclic
SSRIs), but in a few cases in psychotherapeutic treatment. The AA. evaluate the use of HRV as a biological indicator in the psychotherapeutic treatment of patients with adjustment disorder erdurante more than a month after exposure to stressful life events (EVS). Methods: 6 subjects were recruited (M / F = 1 / 5, age 40.5 ± 11.0) exposed to EVS and initiated treatment cycle psychotherapeutic
short (4-6 sessions weekly) type

Discussion: At baseline HRV indicators of distress correlated positively with growing anger-hostility, and intrusive symptoms, negatively with anxiety and depression. Even after three months of the end of psychotherapeutic treatment interventions aimed at reducing symptom scores and enhance sympathetic vagal balance during the playback of the traumatic event.

cognitive behavioral approaches. Strategic utilization of EMDR to move clients through the various stages of recovery will be
discussed.
1) In the first stage of treatment, safety, stabilization, coping, and development of a strong therapeutic relationship are emphasized.
Treatment focuses on decreasing (1) suicidal and parasuicidal behavior, (2) treatment - interfering behavior, and (3) quality-of-life-interfering behavior (Linehan, 1993). Efforts are made to assist the client in developing a repertoire of cognitive-behavioral coping
skills; relevant skills address grounding, trigger awareness, basic self - care, mindfullness, distress tolerance, affect regulation,
assertiveness, relaxation, self - monitoring, stress inoculation, and cognitive restructuring. At this stage, EMDR can be used to shift
negative cognitions which interfere with commitment to treatment, skill development, and the restoration of hope. The following are
examples of negative cognitions whlch interfere with first stage stabilization goals:
- I will only get acknowledgment of my pain if I act out. - I don't deserve to feel better.
- If I take care of myself, no one will know I hurt. - I'm pathetic, a failure.
- I will die/go crazy fiom these feelings. - I can never do anything right.
- I can't stand this feeling. I must cut myself. - Don't trust anyone or anything.
Newly learned information about coping can be reinforced and further integrated in the course of an EMDR session. Clients can be
encouraged to notice their ability to tolerate affect and to practice their assertiveness skills, grounding skills, mindfulness skills, etc.
2) In the second stage of treatment, the focus is on processing traumatic memories and decreasing behaviors related to post-traumatic
stress. EMDR interventions can be designed to assist clients with specific recovery tasks or issues:
- fear/terror and associated avoidance
- sense of powerlessnesshelplessness
- responsibility/accountability
- safety - self, others, environment
- self-esteem/self as bad, defective, unlovable
- lack of individuation
- dependency
- anger
- grief/mouming
- trust/mistrust
- fear of abandonment
- guilt/self-blame
- shame/self-loathing
With regard to each of these issues, maladaptive schemas can be addressed via effective cognitive interweave strategies. Ideas for
supplementing EMDR work with written assignments, imagery exercises, recovery rituals, and planned in vivo exposure will be
discussed. Strategies for handling possible problems, obstacles, or resistance at this stage will also be noted. Finally, the role of ongoing
assessment and data collection in making decisions about EMDR targets will be addressed.
3) In the third stage of treatment, the emphasis is on personal development and increased connection with others. Recovery tasks and
issues addressed via EMDR include:
- Increasing intimacy and healthy connections - Increasing self-esteem
- Increasing self-efficacy and sense of mastery - Reclaiming sexuality
- Increasing self-efficacy and sense of mastery - Identity exploration and development
- Establishing goals, initiating new projects, and taking reasonable risks
At this stage, EMDR can be useful in detecting remnants of shame, fear, etc. In addition, EMDR can be used to reduce anxiety and
increase confidence as a client sets his/her sights on the future and prepares to face new and challenging situations. EMDR can aid
in the generalization of skills and adaptive schemas across time and place. It can facilitate the integration of a new, more positive
and vital self-image.
The presentation will conclude with a videotape case presentation highlighting relevant recovery tasks and issues in applying
EMDR at a specific stage of treatment.

These were subject to statistical analysis using Analysis of Variance (ANOVA)
and Chi-Square tests to examine the relationships between Questionnaire items for
significance. A total of 9 subjects agreed to be interviewed regarding their EMDR practice
and integration issues. This qualititative data was content analyzed. No differences were
found between both groups on Biodata factors, years of experience as a Therapist or years
using EMDR which gives further confidence in the results when comparing both cohorts.
Results: Respondents endorsed two types of explanation as to why they believed EMDR
works (ie) EMDR facilitates communication between the Limbic system and Frontal Lobes
(77%) and Adaptive Information Processing (73%). This suggests that Therapists are not
just following Shapiro’s AIP model slavishly but are rather making up their own minds.
There were no significant differences between Analytic, CBT, Integrative and Humanistic
Therapists in terms of explanatory mechanisms endorsed about EMDR’s “active
ingredient”. CBT Therapists found it easier to incorporate EMDR into their clinical
practice than Analytic or Humanist Therapists. This finding was supported by the results of
the qualitative interview data. Indeed, up to 40% of Therapists sampled experienced
difficulties in integrating EMDR into their clinical practice, post training. The types of
difficulties identified included: Differences in theoretical beliefs, more active style of
EMDR, structure of EMDR Protocol, Therapist confidence issues, Organizational
issues and hostility from clinical colleagues, bullying, lack of supervision and support
post training.
Conclusions: The results of this study confirm findings from earlier studies regarding
Therapists’ beliefs about EMDR but also extend those finding internationally so that
previous findings can now be accepted as universal given that this present study
contained respondents from three other continents other than North America. This study
also found that up to 40% of Therapists trained in EMDR experienced significant
difficulties in integrating EMDR into their clinical practice post-training. Analytic and
Humanist Therapists reported the biggest difficulties which included conflict with the
Therapists’ original theoretical model, the EMDR Protocol structure itself, Therapist
confidence and lack of supervision and support, Organizational and Management issues of
opposition to EMDR and Therapist Bullying. The implications for EMDR training are
discussed and communicated to relevant EMDR Training Organizations.

People suffering from PTSD and the consequences of experiencing the body and its symptoms often an enemy. The gentle, conducted training of body awareness is a way to make friends with the body. Learning to perceive pleasant body sensations increased confidence and facilitate the processing of painful experiences. Learning to deal with unpleasant or painful body awareness, reduces anxiety.
The focus to addressing the body sensations presupposes being able to cope with the developing body memories, pain and / or hyperarousal.
The combination of body and body movement in the conscious perception and impulse EMDR process provides the necessary security and allows the body through deep and conscious experience a gentle processing and integration of traumatic experiences.

Objective: The authors present a multidimensional meta-analysis of studies published between 1980 and 2003 on psychotherapy for PTSD. Method: Data on variables not previously meta-analyzed such as inclusion and exclusion criteria and rates, recovery and improvement rates, and follow-up data were examined. Results: Results suggest that psychotherapy for PTSD leads to a large initial improvement from baseline. More than half of patients who complete treatment with various forms of cognitive behavior therapy or eye movement desensitization and reprocessing improve. Reporting of metrics other than effect size provides a somewhat more nuanced account of outcome and generalizability. Conclusions: The majority of patients treated with psychotherapy for PTSD in randomized trials recover or improve, rendering these approaches some of the most effective psychosocial treatments devised to date. Several caveats, however, are important in applying these findings to patients treated in the community. Exclusion criteria and failure to address polysymptomatic presentations render generalizability to the population of PTSD patients indeterminate. The majority of patients posttreatment continue to have substantial residual symptoms, and follow-up data beyond very brief intervals have been largely absent. Future research intended to generalize to patients in practice should avoid exclusion criteria other than those a sensible clinician would impose in practice (e.g., schizophrenia), should avoid wait-list and other relatively inert control conditions, and should follow patients through at least 2 years. Background: Only few studies have reported functional or structural modifications in Post Traumatic Stress Disorder (PTSD) patients following pharmacological treatment or psychotherapy. Eye movement desensitization and reprocessing (EMDR) is a novel eclectic psychotherapy utilising, among other techniques, relaxation and safe place exercises, cognitive restructuring, future projections, and imaginal exposure of the trauma combined with sensory stimulation. The aim of the study was to analyse the differences in regional cerebral blood flow (rCBF) distribution and in brain volumetry before and after EMDR therapy.
Subjects and Methods: Fifteen subjects with chronic PTSD following occupational health hazards were treated with five sessions of EMDR. They were assessed with psychometric scales and diagnostic interviews before and directly after treatment. SPECT, during administration of an individualised trauma script, was performed using 99mTc-HMPAO. After EMDR, the subjects were subdivided into responders (R, n=10) and non-responders (NS, n=5), based on the absence or presence, respectively, of full PTSD diagnosis. SPECT and volumetric data (MRI) analyses were carried out by Statistical Parametric Mapping (SPM2). SPECT and MRI data were covaried by age and by time elapsed from trauma to SPECT. SPECT data were further covaried by the amount of grey matter normalised by the total intracranial volume.
Results: Immediate significant post-treatment changes towards normality in all scales measuring psychological status were found in responders. As compared to NR, R showed a significantly decreased tracer uptake in parieto-occipital (Brodmann Area, BA, 37, fusiform gyrus) and in primary visual cortex (BA17) and in the hippocampus (p<0.001). The opposite comparison highlighted an increased tracer uptake in left frontal cortex (BA 44; p<0.05). Structural grey matter modifications were found in visual, posterior cingulate and parieto-temporal cortex, paralleling the functional changes.
Conclusion: The positive EMDR outcome corresponded to increased 99mTc-HMPAO uptake in the left dorsolateral frontal cortex, processing attention and self confidence and exerting an inhibitory effect on the amygdala whose firing is supposed to be responsible for PTSD. After successful treatment significant decreases were found in primary visual cortex, processing images of traumatic memories and flashbacks; in fusiform gyrus, processing the memories of faces, bodies and words and in the hippocampi, involved in episodic and autobiographical memories. Volumetric changes paralleled the ones in tracer uptake in all regions Taken as a whole these findings suggest that the positive clinical outcome following EMDR therapy causes functional and structural neurobiological changes towards normality.

EMDR is an effective and principled intervention to help people with the impact of
trauma. It also offers powerful resource development approaches. These are useful as preparation
work in processing trauma, but also valuable interventions in themselves. This can be very helpful when circumstances preclude doing trauma work, or when only brief interventions are possible.
Positive installations and resource development are also apt for use with children and young
people, addressing real world functioning in what is often an enjoyable and rewarding way.
Many resource development approaches are well known, and have been used in both standard
and novel ways by practitioners. Others have been developed by individual practitioners, and
also their clients. Often, therefore, there are good methods that could be more widely known.
This workshop presents an account of both standard and non-standard resource development
techniques, with an opportunity for participants to present their own good practice and share
knowledge. There will also be a discussion of resource development within the EMDR protocol,
as part of a principled model for this kind of work., but with a practical view as to how resource
development can make trauma processing both easier and more effective. This paper gives an
overview of using RID with children. We will look at a whole range of RID’s- common ones and
some non-standard, innovative ones. With the participants we will look at purposes, pitfalls, the
extent to which any can be safely ‘given away’, and any experiences of using these in groups. To
our knowledge, the field has not been drawn together in this way, and we hope through a mixture
of presentation and discussion to begin to share our experiences, and develop peoples’ confidence
in extending their repertoire.

Background: No standard intervention with proved effectiveness is available for women with posttraumatic stress following childbirth because of insufficient research. The objective of this paper was to evaluate the possibility of using eye-movement desensitization and reprocessing treatment for women with symptoms of posttraumatic stress disorder following childbirth. The treatment is internationally recognized as one of the interventions of choice for the condition, but little is known about its effects in women who experienced the delivery as traumatic. Methods: Three women suffering from posttraumatic stress symptoms following the birth of their first child were treated with eye-movement desensitization and reprocessing during their next pregnancy. Patient A developed posttraumatic stress symptoms following the lengthy labor of her first child that ended in an emergency cesarean section after unsuccessful vacuum extraction. Patient B suffered a second degree vaginal rupture, resulting in pain and inability to engage in sexual intercourse for years. Patient C developed severe preeclampsia postpartum requiring intravenous treatment. Results: Patients received eye-movement desensitization and reprocessing treatment during their second pregnancy, using the standard protocol. The treatment resulted in fewer posttraumatic stress symptoms and more confidence about their pregnancy and upcoming delivery compared with before the treatment. Despite delivery complications in Patient A (secondary cesarean section due to insufficient engaging of the fetal head); Patient B (second degree vaginal rupture, this time without subsequent dyspareunia); and Patient C (postpartum hemorrhage, postpartum hypertension requiring intravenous treatment), all three women looked back positively at the second delivery experience. Conclusions: Treatment with eye-movement desensitization and reprocessing reduced posttraumatic stress symptoms in these three women. They were all sufficiently confident to attempt vaginal birth rather than demanding an elective cesarean section. We advocate a large-scale, randomized controlled trial involving women with postpartum posttraumatic stress disorder to evaluate the effect of eye-movement desensitization and reprocessing in this patient group.

Abstract: Attachment theory has identified adults with a secure attachment style as more resilient to stress and trauma than insecurely attached adults. The secure adult tends to have supportive relationships which buffer him from stress (Bowlby, 1988), and he is able to reflect upon his inner state and process emotions without becoming overwhelmed (Fonagy, 2000; van der Kolk 1996). In infancy secure attachment is related to the capacity to be soothed and comforted by the caregiver. The caregivers of secure infants are observed to be emotionally attuned, responsive and nurturing (Solomon & George, 1999). Schore (1996) explains that mothers of secure babies synchronize with the infant. The mother's organized brain synchronizes or harmonizes with the baby's disorganized brain, attuning to its feelings and needs, helping it calm when distressed and stay regulated when it is happy and excited. As a result, the infant's brain develops optimal circuitry for emotion regulation, and also learns through experience to trust and be comforted and to comfort himself. His basic sense of safety and trust gives him confidence as he begins leaving his parent's side and exploring the world.

SEE FAR CBT is a suggested new protocol for the treatment of anxiety disorders and post-traumatic stress disorder (PTSD) using creative form treatment based on empowerment through fantastic reality. The model emphasizes the role of fantastic reality and the use of imaginal re-narration of the traumatic event with the use of cards as a means of externalization or distancing. The treatment protocol incorporates methods of somatic memory reduction as well as CBT elements. The main objective of this study was to introduce the model and test the therapeutic efficacy of this new integrative therapeutic approach by comparing it to a well-established treatment approach; eye movement desensitization and reprocessing (EMDR). Adult PTSD patients, divided into EMDR (n = 12) and SEE FAR CBT (n = 9) groups, were assessed for traumatic symptoms at three time intervals (pre-treatment, post-treatment and 1-year follow-up). Both EMDR and SEE FAR CBT were associated with effective alleviation of traumatic symptoms, showing statistically significant decreases in their trauma symptoms over time but not differing in treatment efficacy during any of the assessment times. With some methodological limitations, results suggest further inquiry of the proposed model in clinical and experimental settings.

Background:
Post-traumatic stress disorder (PTSD) is highly prevalent in children and adolescents who have experienced trauma and has high
personal and health costs. Although a wide range of psychological therapies have been used in the treatment of PTSD there are no
systematic reviews of these therapies in children and adolescents.
Objectives:
To examine the effectiveness of psychological therapies in treating children and adolescents who have been diagnosed with PTSD.
Search methods:
We searched the Cochrane Depression, Anxiety and Neurosis Review Group’s Specialised Register (CCDANCTR) to December 2011.
The CCDANCTR includes relevant randomised controlled trials fromthe following bibliographic databases: CENTRAL (the Cochrane
Central Register of Controlled Trials) (all years), EMBASE (1974 -), MEDLINE (1950 -) and PsycINFO (1967 -). We also checked
reference lists of relevant studies and reviews. We applied no date or language restrictions.
Selection criteria:
All randomised controlled trials of psychological therapies compared to a control, pharmacological therapy or other treatments in
children or adolescents exposed to a traumatic event or diagnosed with PTSD.
Data collection and analysis:
Two members of the review group independently extracted data. If differences were identified, they were resolved by consensus, or
referral to the review team.
We calculated the odds ratio (OR) for binary outcomes, the standardised mean difference (SMD) for continuous outcomes, and 95%
confidence intervals (CI) for both, using a fixed-effect model. If heterogeneity was found we used a random-effects model. Main results
Fourteen studies including 758 participants were included in this review. The types of trauma participants had been exposed to included
sexual abuse, civil violence, natural disaster, domestic violence and motor vehicle accidents. Most participants were clients of a traumarelated
support service.
The psychological therapies used in these studies were cognitive behavioural therapy (CBT), exposure-based, psychodynamic, narrative,
supportive counselling, and eye movement desensitisation and reprocessing (EMDR). Most compared a psychological therapy to a
control group. No study compared psychological therapies to pharmacological therapies alone or as an adjunct to a psychological
therapy.
Across all psychological therapies, improvement was significantly better (three studies, n = 80, OR 4.21, 95% CI 1.12 to 15.85) and
symptoms of PTSD (seven studies, n = 271, SMD -0.90, 95% CI -1.24 to -0.42), anxiety (three studies, n = 91, SMD -0.57, 95% CI
-1.00 to -0.13) and depression (five studies, n = 156, SMD -0.74, 95% CI -1.11 to -0.36) were significantly lower within a month of
completing psychological therapy compared to a control group.
The psychological therapy for which there was the best evidence of effectiveness was CBT. Improvement was significantly better for
up to a year following treatment (up to one month: two studies, n = 49, OR 8.64, 95% CI 2.01 to 37.14; up to one year: one study,
n = 25, OR 8.00, 95% CI 1.21 to 52.69). PTSD symptom scores were also significantly lower for up to one year (up to one month:
three studies, n = 98, SMD -1.34, 95% CI -1.79 to -0.89; up to one year: one study, n = 36, SMD -0.73, 95% CI -1.44 to -0.01),
and depression scores were lower for up to a month (three studies, n = 98, SMD -0.80, 95% CI -1.47 to -0.13) in the CBT group
compared to a control. No adverse effects were identified.
No study was rated as a high risk for selection or detection bias but a minority were rated as a high risk for attrition, reporting and
other bias. Most included studies were rated as an unclear risk for selection, detection and attrition bias.
Authors’ conclusions:
There is evidence for the effectiveness of psychological therapies, particularly CBT, for treating PTSD in children and adolescents for
up to a month following treatment. At this stage, there is no clear evidence for the effectiveness of one psychological therapy compared
to others. There is also not enough evidence to conclude that children and adolescents with particular types of trauma are more or less
likely to respond to psychological therapies than others.
The findings of this review are limited by the potential for methodological biases, and the small number and generally small size
of identified studies. In addition, there was evidence of substantial heterogeneity in some analyses which could not be explained by
subgroup or sensitivity analyses.
More evidence is required for the effectiveness of all psychological therapiesmore than one month after treatment.Much more evidence
is needed to demonstrate the relative effectiveness of different psychological therapies or the effectiveness of psychological therapies
compared to other treatments. More details are required in future trials in regards to the types of trauma that preceded the diagnosis
of PTSD and whether the traumas are single event or ongoing. Future studies should also aim to identify the most valid and reliable
measures of PTSD symptoms and ensure that all scores, total and sub-scores, are consistently reported.

Search strategy:
CCDANCTR-References was searched on 12/5/2008, electronic databases were searched, reference lists of review articles and included
studies were checked, a specialist journal was handsearched, specialist books were checked and we contacted experts and trialists.
Selection criteria:
Randomised and quasi randomised controlled trials were eligible. The types of participants were people employed directly in law
enforcement, including police officers and military police, regardless of gender, age and country of origin, and whether or not they
had experienced some psychological trauma. All types of psychosocial intervention were eligible. The relevant outcome measures were
psychological symptoms, adverse events and acceptability of interventions.
Data collection and analysis:
Datawas entered intoReviewManager 4.2 for analysis, but this reviewwas converted toRevMan 5.0 for publication.Quality assessments
were performed. Two authors independently selected studies, extracted data and assessed the quality of studies. Summary effects were
to be calculated using RevMan but no meta-analyses were possible. For individual studies, dichotomous outcome data are presented
using relative risk, and continuous outcome data are presented using the weighted mean difference. These results are given with their
95% confidence intervals (CI).

Ten studies were included in the review but only five reported data that could be used. Three of the ten studies were related to exercisebased
psychological interventions. Seven were related to psychological interventions. No meta-analyses were possible due to diversity of
participants, interventions and outcomes. Two studies compared a psychosocial intervention versus another intervention. Three studies
compared a psychosocial intervention to a control group. Only one primary prevention trial reported data for the primary outcomes
and, although this study found a significant difference in depression in favour of the intervention at endpoint, this difference was no
longer evident at 18 months. No studies of primary prevention comparing different interventions and reporting primary outcomes of
interest were identified.
The methodological quality of the included studies was summarised. No study met our full quality criteria and one was regarded as
low-quality. The remainder could not be rated because of incomplete data in the published reports and inadequate responses from the
trialists.
Authors’ conclusions:
There is evidence only from individual small and low quality trials with minimal data suggesting that police officers benefit from
psychosocial interventions, in terms of physical symptoms and psychological symptoms such as anxiety, depression, sleep problems,
cynicism, anger, PTSD, marital problems and distress. No data on adverse effects were available. Meta-analyses of the available data
were not possible. Further well-designed trials of psychosocial interventions are required. Research is needed on organization-based
interventions to enhance psychological health among police officers.

The Turkish Psychologists Association has initiated psychological treatment for earthquake victims through the Trauma Treatment Center, established jointly with experts from the American Eye Movement Desensitization Reprocessing Institute (EMDR). The Trauma Treatment Center was introduced on Wednesday by Dr. Emre Konuk, the director of the Turkish Psychologists Association's Istanbul Office, and representatives of EMDR Dr. Philip Manfield and Dr. James Knipe. Konuk said they had contacted EMDR three days after the earthquake for psychological treatment of the victims. Konuk, who noted that they had prepared a one-year program as the result of the cooperation, said they established a Trauma Treatment Center for carrying on work as an organization. The training of volunteer psychologists who will work at the center is being undertaken by EMDR, experts of problems related to trauma. Seventeen experts will come to Turkey twice at four-month intervals to provide training to 115 psychologists on active treatment techniques for trauma.

EMDR is a procedure capable of assisting with MPD patients by defusing acute distress associated with current crises. In order to benefit from EMDR without risking retraumatization, the diagnosis of MPD needs to be established and consent obtained from the system as a whole. The systems needs to agree that a decrease in distress is a desirable treatment outcome.
In treating MPD, crises may arise before the system has been fully mapped. EMDR amy be cautiously used in this situation by 1) asking the entire system to listen, 2) explaining the procedure, 3) asking for any parts, know to the therapist or unknown, who disagress to let their concerns be known or they will have to be construed to have consented. The relief provided by the successful defusing of the crisis tends to increase confidence in the therapist and encourage others to alters to reveal themselves.
Several case examples will be presented illustrating the application of EMDR to crisis intervention with MPD patients. Preliminary data from case examples indicate that (1) clients report lasting relief from distress associated with current crisis; (2) clients report relief even though distress levels did not reach zero; (3) EMDR for crisis intervention is a cost-effective procedure for reducing the frequency of hospitalizations by managing crises in an outpatient basis.

In 1987, Professional School of Psychological Studies graduate student Francine Shapiro was on a walk in the park when she personally observed that randomized eye movements seemed to make disturbing thoughts less upsetting (Colwell, 2000). From this initial experience Shapiro created the therapy known as Eye Movement Desensitization (EMD), integrating rhythmic eye movements with “other treatment elements” (EMDR 2004). Shapiro investigated this phenomenon further in a controlled study on twenty-three subjects with traumatic memories and Post-Traumatic Stress Disorder (PTSD) symptoms. In the study, the subjects were first asked to rate their feelings of discomfort regarding a particular traumatic memory using the Subjective Units of Disturbance (SUDs) scale which uses a 0 – 10 rating system to express emotional distress. Subjects were also asked to think of a Positive Self-Statement (PSS) they would like to attach to the memory, erasing a negative one. The subjects then evaluated their confidence in the truth of the PSS on a scale from 1 - 7, where one was completely false, and seven were completely true. On average, before treatment the subjects reported a SUDs score of seven and expressed the validity of the PSS as four. After a single EMD session, subjects on average evaluated the memory as less than one on the SUDs scale, and the positive self-thought was receiving ratings above six. These results remained consistent three months later (Lipke & Botkin, 1992), leading Shapiro claim in her dissertation that “a single session of the procedure was sufficient to desensitize subjects” (EMDR, 2004). The publication of Shapiro’s dissertation came nine years after the American Psychiatric Association added Post-Traumatic Stress Disorder to the DSM-III in 1980 (Friedman, 2006), and EMD quickly became a popular PTSD treatment.

In the present case study, a patient who had become physically disabled due to a fracture caused by a traffic accident and who subsequently showed signs of gross psychological trauma underwent eye movement desensitization and reprocessing (EMDR) in the Clinic of Physical Treatment and Rehabilitation Center, Ankara Ministry of Health Education and Research Hospital. Before EMDR, the patient was so resistant to medical treatment that he would allow neither doctor nor physiotherapist to approach him. After two sessions of EMDR, he remembered neither the traumatic event of the traffic accident itself nor the resulting medical treatment. He began to cooperate with doctors and physiotherapists during subsequent care. Weekly follow-up interviews over 6 months revealed a dramatic improvement in well-being, which continued to be observed in two follow-up interviews occurring at 3-month intervals after his discharge from the hospital. The ostensible purpose of EMDR technique is the reorganization of traumatic memory and the provision of a kind of psychic respite for the patient. Indeed, the patient in this study was able to lower his resistance to medical treatment and physical therapy as a direct consequence of EMDR sessions. Our observations with this patient provide the basis of a discussion regarding the efficacy of the EMDR technique.

Abstract:
Effective dissemination of treatment methods requires
not only training in high-profile interventions but also in
cases of conceptualization and treatment planning skills
that facilitate use of the interventions. In a series of six
studies, the authors tested one training module with 303
paraprofessionals and mental health professionals in various
training settings and five countries. Participants
completed self-report ratings in response to a challenging
acting-out client, both before and after completing a
trauma-informed case-formulation exercise. The training
intervention led participants to report decreased distress
while considering challenging work-related scenarios,
increased empathy and caring toward challenging
clients, and increased comfort and confidence in their
helping roles. In the final two studies, a trauma-informed
treatment planning module was added, yielding additional
benefit. At follow-up participants reported that the
effects persisted and led to improved behaviors toward
the clients. Such empirical validation of training methodologies
can lead to more reliably effective dissemination.

The time-course of changes in vividness and emotionality of unpleasant autobiographical memories associated with making eye movements (eye movement desensitisation and reprocessing, EMDR) was investigated. Participants retrieved unpleasant autobiographical memories and rated their vividness and emotionality prior to and following 96 seconds of making eye movements (EM) or keeping eyes stationary (ES); at 2, 4, 6, and 10 seconds into the intervention; then followed by regular larger intervals throughout the 96-second intervention. Results revealed a significant drop compared to the ES group in emotionality after 74 seconds compared to a significant drop in vividness at only 2 seconds into the intervention. These results support that emotionality becomes reduced only after vividness has dropped. The results are discussed in light of working memory theory and visual imagery theory, following which the regular refreshment of the visual memory needed to maintain it in working memory is interfered with by eye movements that also tax working memory, which affects vividness first.

Twelve days after the 26th of December 2004 a couple came to my private
practice, on referral from Prof. Dr. Ad de Jongh. that looked death in the
eye when the Tsunami hit Sri Lanka. The couple has four children, age four to
eleven, who survived with them. On Sri Lanka they were called 'The fortune
family'. They both had severe symptoms of Acute Stress Disorder: reliving the
disaster day and night and were, only just, managing to take care of the
children and their daily life.
They already read about EMDR and had their hopes up that I could help
them stabilize. As soon as they started telling me about their distressing
experience I noticed that, especially the woman, started reliving it. Knowing
that they had been telling everything already many times to family and friends, I asked them f I could immediately do the first EMDR session with each of them. Quite noticeable was that the experience was still in their minds with every detail and with several peaks of the most distressing
moments. In total they had three single sessions each with two-days intervals.
Their children who at first were doing relatively well had started to develop
serious symptoms and needed treatment; after the three EMDR sessions for
each of the parents they were stable and could give their full attention to
EMDR-treatment of their children, who went to Carlijn de Roos MA, clinical
child-psychologist, who leads a trauma centre for children in the
Netherlands. At the end of February the parents were still doing well and at
the time of the EMDR Europe Conference I will have seen them for a follow-up.

Disruptive behavior disorders such as Conduct Disorder, Oppositional Defiant Disorder, Tourette's Disorder, Intermittent Explosive
Disorder and, to a lesser extent, some children with Bipolar Disorder, are among the most difficult conditions for clinicians to treat.
The difliculty is two-fold: Clinicians must find the balance between developing a rapport and working with the disruptive child
while also addressing the concerns of the parents who may be beyond their level of tolerance and looking for prompt improvement
and frequently the child's very diagnostic condition - disruptiveness - may prevent the child from willingly participating in the
treatment, despite the good rapport that the child and clinician may have.
EMDR is, a technique which has shown great promise in helping children who are 'stuck' break through their own stubbornness and
disruption to help them achieve a sense of self-control, insight and self-confidence. It also provides parents with reason to hope that
a prompt improvement maybe possible.
The essential aspect of successful utilization of EMDR with disruptive disorders resides in the creativity of the clinician and the
modfication of the standard EMDR protocol. Using EMDR with children, especially with young children, requires a nonstandard
administration only loosely based on the standard protocol. Children often won't - or can't - verbalize cognition's or adequately
employ SUDS scales reliably. Disruptive children may have access to their cognitions, but may decline to cooperate with clinicians.
The key in using EMDR with Children - as with many other techniques employed in treating children - is to make -the technique
challenging or fun or maybe a bit mysterious, depending upon the presentation of the child and his or her ability to take some
responsibility for the treatment process. The key with disruptive children is knowing how to combine EMDR usage with
engagement of the difficult child.
This workshop explores the use of such nonstandard EMDR administrations as auditory cueing and hand tapping and will review
how EMDR has been used to engage very difficult children.
The presentation will include both successful and unsuccessful treatment outcome, focusing on the identification of hidden
handicaps which could prevent engagement of a diflicult child and techniques on how to overcome those handicaps to help the child
to help him or herself.

The use of Eye Movement Desensitization Reprocessing Therapy in the treatment of trauma and stress is well documented (Shapiro, 2001). Since its inception, several studies documenting the effectiveness of EMDR in the treatment of other mental health issues has also been studied (Parnell, 2006). Bae, Kim, and Ahn’s, (2006) clinical case study demonstrates no success with the use of EMDR to treat patients that developed obsessive-compulsive disorder after being diagnosed with posttraumatic stress disorder. Their article suggests little research in support of the use of EMDR in the treatment of OCD.
This presentation illustrates two case studies of men diagnosed with chronic OCD, and their inability to find relief from their symptoms from both pharmacological and psychotherapeutic interventions. The study illustrates the use of Parnell’s modified EMDR protocol with both patients, and the importance of identifying and resolving feeder memories.
Patient A is a male who was diagnosed with OCD twenty years ago. He has received both psychotherapy and psychiatric services from a major university hospital since being diagnosed. Part of the obsessive thoughts include shouting obscenities at his congregation, committing violent acts towards members of the parish, and ultimately jumping over a choir railing with the goal of killing himself.
Patient A began psychotherapy using the Parnell’s modified protocol of EMDR. The patient was able to tap into core (feeder) memories from childhood where he violated trust and confidence with a friend that had not been fully resolved. This also connected with security issues with his mother, which eventually were completely resolved through EMDR. The patient’s obsessive thoughts ceased. The patient has been free of these obsessive thoughts post therapy for one year.
Patient B sought therapy for compulsive behaviors related to exposing himself to unsuspecting victims. Additionally, this patient also engaged in compulsive masturbation behaviors for eight to ten hours a day. His actions not only were psychologically distressing to him, but also causing him problems with the local authorities. Patient B had sought psychotherapy and psychiatric services in the past on several occasions, but with no improvement.
Patient B demonstrated radical improvement using Parnell’s modified EMDR protocol. The patient identified feeder memories that were not initially discussed during the assessment phase. After successful processing, patient B has not engaged in any inappropriate sexual behaviors or compulsive masturbation for three months.
While psychodynamic principles rooted in experiences of life are not novel or innovative in the practice of psychotherapy, often therapists will focus on reducing the problematic symptoms accompanying a diagnosis of OCD, without considering the full implication of prior experiences. Continuing advances in the application of EMDR with an ever expanding array of mental and emotional disorders requires researchers to consider the importance of identifying feeder memories as a possible source of problematic symptoms. These results offer promising techniques for EMDR therapists, and new avenues in research exploring the efficacy of EMDR and OCD. This presentation will illustrate the process involved in identifying feeder memories.

Eye Movement Desensitization and Reprocessing (EMDR) is a relatively new and seemingly effective but controversial therapeutic treatment used primarily for Post-Traumatic Stress Disorder (PTSD) and related anxiety disorders such as phobias. In this study, EMDR was used to treat a public speaking anxiety or public speaking phobia. It was predicted that EMDR would be an effective treatment but that the efficacy would vary as a function of the absorption ability of the participants. 30 healthy male and female participants between the ages of 18 and 55 were categorized into 3 groups based on their being high (n = 10), moderate (n = 10), or low (n = 10) in absorption ability. Only participants who scored low on the Dissociative Experience Scale (DES) were included in the study. Participants received up to 3 sessions of EMDR and were measured on 9 different outcome variables. Significant decreases in Subjective Units of Distress Scale (SUDS) verbal ratings were found from baseline to the end of session 1 for all three groups (ps < .05); no other significant improvements were found after the first session (ps > .10). Confidence in verbal reports of positive cognitions (Validity of Cognition scale; VoC) increased equally over the course of treatment for all absorption groups. Between-group comparisons revealed no significant differences between the absorption groups at the end of treatment on their verbal reports of disturbance (SUDS) and positive cognitions (VoC) regarding past, ongoing, and future projected speaking events. Girls who participate in Miss India Pageants are a mix of different cultures and background. They stay together for 40 days,
and have grueling sessions by expert trainers from different fields. The Pageant Organizers appoint a Psychologist to deal
with their hidden conflicts, fears and pressure of wining. As these girls stay in a very competitive milieu, they tend to lose
their inner confidence and start judging their own beauty negatively. As the day of pageant dawns they report being nervous
and anxious. The psychologist chose to use Resource Development technique from EMDR mainly composed of Calm and
Safe Palce, Butterfly Hug and Future Templates .The participants were told to chose their Calm and safe place. The protocol
was properly followed from the trainers’ manual. Five sessions of Coping Resources treatment were done for the contestants.
Before teaching Calm and Safe Place, each of the contestants was interviewed by the Psychologist to understand their
limitations and flaws, which might hinder them from winning the pageant. After the installation of Future Templates, the
contestants reported that they were better equipped to deal with the pressures of the pageant. This report suggests that the
Resource Development treatment using EMDR may be effective for such important events.

Post-traumatic stress disorder (PTSD) is a condition that can be produced by traumatic experiences. A new study has shown that a brain scan called magneto encephalography (MEG) can identify people who have PTSD with an accuracy of 95%. Sensors measure tiny magnetic fields generated by currents flowing in and around neurons. In addition they have recently stated in the Journal of Neural Engineering that they can now watch the brain as it experiences PTSD. Imaging shows that the brain becomes hyperactive in the right temporal lobe, the location responsible for memory. Besides diagnosing PTSD, the researchers also are able to judge the severity of how much patients are suffering. Eye-movement desensitization and reprocessing (EMDR) was designed in 1987 as a treatment for PTSD. EMDR is a structured eight-phase therapy that allows for adequate reprocessing of dysfunctionally stored memory. In the processing phases the client attends to the disturbing memory in brief intervals of 15-30 seconds while also experiencing bilateral stimulation (eye movements, tones or tapping). The eight phases integrate effective elements of psychodynamic, imaginal exposure, cognitive therapy, interpersonal, experiential, physiological and somatic therapies. Now that we can locate specific biomarkers for PTSD using MEG, my hypothesis is that we will find a statistically significant difference between the control group and the group that has EMDR treatment, and that EMDR will be shown to be effective in resolving PTSD as measured by pre and post therapy MEG scans. Also using the MEG, we may be able to observe those brain actions responsible for the therapeutic efficacy of EMDR and isolate which components of EMDR trigger those brain actions. The significance of finding the answer to these questions could potentially help millions of people overcome years of suffering from psychological pain due to the after effects of severe trauma and restore them to productive lives. It could establish the status of EMDR, assisting in the decision of whether it should be listed among the evidence-based, best-practice therapy modalities and covered by insurance. Also knowing the underlying pathophysiology could contribute to the evolution, revision and refinement of diagnostic constructs for PTSD.

Le principe neuropsychologiques:
Le patient se concentrer sur la "douloureuse" zone associés sensorielle, psychique souvenirs conscients et inconscients; ces souvenirs traités progressivement par le mouvement de la Tac / machine Audioscan qui fonctionne comme un balayage interne pour éliminer les tensions et à intégrer mental, émotionnel, physique et informations par les chaînes successives.
The following is an experimental procedure proposed as preliminary treatment for patients suffering from chronic migraines which require total rest, withdrawal from all social and professional activates and which must be treated by medication. This procedure used the EMDR, hypnosis and relaxation and is intended for patients suffering from chronic pain (symptoms known and treated since five to 10 years).
The three components of the migraines are: MENTAL – expressions of impotence, loneliness, rejection, physical and psychic exhaustion, loss of control; EMOTIONAL – depression, anger, fear; and PHYSICAL – physical feelings in fluctuating sequences evaluated between 6 and 10 on the pain scale.
The average duration for this treatment is 4 months for 6 consultations.
The objective of the treatment is to restore the patient’s confidence by improving physical and psychological well-being.
The clinical principle:
The chronic painful patient alternates between despair, fear of never finding a cure (a mental state which may be provoked by the family and medical entourage), anger against a personal history which may have cause the painful syndrome (parental maltreatment or absence, accident, surgical operation), physical exhaustion and hope that there is still the possibility for a cure. It is therefore important to transmit a message of hope and the vision of a treatment which involves the patients with his physical, emotional, psychic resources. Explaining the suffering as an accumulation of stressful factors and the brain as having the possibility of solution, it is proposed to the patient to concentrate on that part of the body which 'speaks’ in the phenomenon of pain. The patient ‘hears’ or ‘feels’ the alternative movement of the Tac/Audioscan machine (Neurotek) while being assured of the physical and psychic well-being during the treatment.
The neuropsychological principle:
The patient’s concentration on the “painful zone” associates sensory, psychic conscious and unconscious memories; these memories treated progressively by the movement of the Tac/Audioscan machine which operates like an internal sweeping to eliminate tensions and to integrate mental, emotional, and physical information by successive channels.

Veel van de kinderen die in hun vroege kinderjaren herhaaldelijk zijn blootgesteld aan traumatische ervaringen, worden uit huis geplaatst. De gevolgen van de traumatisering zijn meestal ernstig en vergaand doordat het de organisatie van de in ontwikkeling zijnde hersenen negatief beinvloedt. Voor herstel van complex trauma is individuele traumagerichte therapie niet genoeg. Kinderen hebben heel veel nieuwe (herstellende) ervaringen nodig om weer vertrouwen te krijgen in zichzelf, de ander en de wereld om hen heen. De thuissituatie biedt hiertoe de beste mogelijkheden mits de opvoeders goed af kunnen stemmen op wat het kind nodig heeft.
De training ‘Zorgen voor getraumatiseerde kinderen’ biedt opvoeders en begeleiders onmisbare informatie over de effecten van traumatisering op het dagelijks functioneren van hun kind en de relatie die zij als belangrijke volwassene met hun kind ontwikkelen. (Pleeg-)ouders en andere belangrijke volwassenen in het leven van het kind kunnen hierdoor meer doen aan het herstel van hun kind en raken minder snel uitgeput, wat de kans op (nog) een uithuisplaatsing verkleint. De training is gebaseerd op recente wetenschappelijke inzichten die op een overzichtelijke en concrete manier zijn verwerkt in o.a. powerpoint dia’s, casusbeschrijvingen en oefeningen. De training is oorspronkelijk ontwikkeld door de National Child Traumatic Stress Network (NCTSN). Leony Coppens en Carina van Kregten hebben de training bewerkt en vertaald voor de Nederlandse situatie.
In de workshop is aandacht voor de eerste ervaringen met de training in het veld. Daarnaast wordt er ingegaan op achtergronden bij de onderwerpen die in de training aan bod komen en kunnen deelnemers aan de workshop ervaring opdoen met de oefeningen uit de training.

The training 'Caring for traumatized children "provides educators and counselors indispensable information about the effects of trauma on the daily functioning of their child and the relationship which they consider important to their adult child development. (Foster-) parents and other significant adults in the life of the child can therefore do more to restore their child and become less exhausted, the chance of (still) a placement reduces. The training is based on recent scientific insights in a clear and concrete way to digest in PowerPoint slides, case studies and exercises. The training was originally developed by the National Child Traumatic Stress Network (NCTSN). Leony Coppens and Carina of Kregten have the training edited and translated for the Dutch situation.
