The fictional world imaginatively constituted during literary reading is sometimes compared with
the imaginal world created during dreaming. At the core of both reading and dreaming may be the
type of attentional adjustment that occurs when departures from expected events emerge in
experience. During dreaming, markers of this attentional adjustment – and of the related
transformations of dream content – are the eye movements characteristic of REM sleep. Recent
research suggests that eye movements induced during wakefulness similarly prompt dreamlike
transformations of imaginal activity. Therefore, we hypothesized that, during reading, induced eye
movements would facilitate defamiliarization in response to the deviations from literal meanings
found in metaphoric expressions. To test this hypothesis, twenty-five undergraduates completed 20
seconds of eye movements or 20 seconds of visual fixation before each of two tasks: (a) a covert visual
attention task (Posner & Cohen, 1984), in which a cue indicated the likely position of a subsequent
target, and (b) a sentence rating task, in which sentences with either metaphoric or non-metaphoric
endings were rated for strikingness. Repeated measures ANOVAs indicated that the eye movement
manipulation facilitated attentional adjustments to targets presented in invalidly cued locations and
increased the extent to which metaphoric sentence endings were found striking. These results suggest
that induced eye movements facilitate attentional reorientation toward the novel meanings found in
metaphoric expressions, providing evidence that dreaming and reading involve a similarly
“defamiliarizing” attentional adjustment.

I fell in love with EMDR after my Level I training in October of 1995 and have used it as much as possible since. Using it with people who have experienced trauma is like having a front row seat to the healing processes of the universe. I no longer struggler with the perennial question: “What do I do next?” When processing becomes stuck, cognitive interweaves usually come to me during an eye movement set.

Abstract:
This study used a naturalistic design to investigate the effectiveness of eye movement desensitization and reprocessing (EMDR) with children and adolescents who were exposed to single-incident trauma. Participants were 36 children and adolescents ranging in age from 1 year 9 months to 18 years 1 month who were referred consecutively to the author's private practice. Assessments were conducted at intake, post-waitlist/pretreatment, and at follow up. EMDR treatment resulted in significant improvement (Cohen's d = 1.87). Follow-ups after 6 months revealed stable, further slight improvement. It was shown that children younger than 4 years of age can be treated using EMDR and that the group of preschool children had the same benefit from the treatment as the school-age children.

Eye Movement Desensitization Reprocessing (ENDR) therapy has burst upon the

by its inventor, Franche Shapiro, it has distinct historicd precedents. The explanatory models of

objectivity rnake EMDR therapy famiIiar and salieut. Notions of suggestion and hypnosis, aud
the eye-movement component of therapy are presented as discontinuous with clinical and
theoreticai practice, in order to fiee them h m the tainting associations of pseudo-science and
quackery. By co~ect ingth e curent EMDR movement with the conceptual and practical history
of traumatic memory, dissociation, and suggestion, 1 argue that EMDR is not revolutionary. It is
a powerful technotogy of the self, nomaking and valourking certain ways of behaving and

early traumatic events are indelibly encoded, stored and dissociated in the brain are

Eye movement desensitization and reprocessing (EMDR), an emerging
therapy for psychological trauma, has been in use for nearly a decade.
Although it has stimulated strong interest and enthusiasm, EMDR has
also received intense critical scrutiny. This chapter provides an
overview of the history and theory of EMDR. Next, the ENDR procedure
is summarized, followed by a review of the outcome literature.
Dismantling studies of the contribution of eye movements to the
efficacy of the EMDR procedure are then reviewed, followed by an
overall rating reflecting the current knowledge of EMDR's efficacy,
along with recommendations for its use. The chapter concludes with

Every day, children are exposed to traumatic events. As a result, a significant subgroup of these children
develops post-traumatic stress symptoms and co-morbid problems. Although Post-Traumatic Stress
Disorder (PTSD) is a major problem in children and adolescents, European treatment outcome studies
are still scarce in this population.
The current study is a randomized controlled trial of the Trauma Focused Cognitive Behavioral Therapy
(TF-CBT) protocol as designed by Cohen, Mannarino, and Deblinger; and the Dutch eye movement
desensitization and reprocessing (EMDR) protocol for children (de Roos, Beer, de Jongh en ten Broeke).
Up to now a direct comparison of these treatment protocols is lacking.
Children between the age of 8 and 18 years with (partial) PTSD were randomly assigned to either 8
sessions TF-CBT or 8 sessions EMDR. In this presentation we will present first results from our trial.
Treatment results will be presented on outcome measures including PTSD-symptoms, co-morbid
symptoms and cognitions.

Objectives: Post traumatic stress disorder (PTSD) is a severe,
distressing, and chronic condition. Limited availability of
appropriately trained professionals is a significant barrier to
accessing appropriate treatment. This presentation reports the
results of two pilot studies: (i) a pilot RCT of Internet-based
cognitive behavioral therapy (CBT); (ii) an open trial of Internetbased
eye movement desensitzation and reprocessing (EMDR) for
PTSD. RCT: Internet-delivered CBT
Methods: 43 people with PTSD were randomly allocated to
receive clinician-assisted Internet-based treatment for PTSD, or to
a waitlist control condition. Participants in the clinician-assisted
version received access to the 7-lesson PTSD program plus regular
emails from a clinician, automatic reminder emails, and access to
an online discussion forum.
Results: Participants reported significant reductions in PTSD symptoms, depression and anxiety (Cohen’s d respectively: 1.3,
1.2 and 0.7). Furthermore, participant satisfaction with the
treatment program was high.
Conclusions: PTSD is a disabling disorder, but access to
treatment is limited for many people. Developing addditional
effective techniques for treating patients with PTSD is an
important priority for mental health clinicians. OPEN TRIAL:
Internet-delivered EMDR
Methods: This trial is due to commence in March, 2011 and will
run for 8 weeks. It involves 15 people with PTSD allocated to
receive internet-based EMDR in addition to the existing internetbased
CBT protocol used in the above RCT. To our knowledge, this
is the first time that EMDR has been delivered via the internet or
tested despite EMDR being recommended as a first-line
intervention for PTSD by several treatment guidelines for PTSD [1,
2]. We expect that clinically significant improvements will be
observed and that participants will rate the EMDR module as
acceptable.

The purpose of this project was to propose a long-term, theoretically sound and research
supported person-centered grief counseling group for adult women who were sexually abused as
children. A review of the literature indicated that child abuse survivors can benefit from
supportive group counseling; sharing a context of common experience seems to aid in their
healing process. The proposed program recognizes the need to provide women who were abused
with a trusting, social environment that helps to remove the secrecy and isolation, decrease the
feelings of shame and self-blame, and increase self-esteem and self-worth. The integration of a
nondirective approach with grief counseling creates a more comprehensive approach in which to
support the development of social skills and healthy and trusting relationships. The group is
structured for survivors to share their experiences, heal from their traumas, and find the tools to
move forward into happier, healthier, and better functioning lives.

Background and Objectives: Eye Movement Desensitisation and Reprocessing (EMDR) is now considered evidence based practice in the treatment of trauma symptoms. Yet in a previous meta-analysis, no significant effect was found for the eye movement component. However methodological issues with this study may have resulted in a type II error. The aim of this meta-analysis was to examine current published studies to test whether eye movements significantly affect the processing of distressing memories.
Method: A systematic review of the literature revealed two groups of studies. The first group comprised 15 clinical trials and compared the effects of EMDR therapy with eye movements to those of EMDR without the eye movements. The second group comprised 11 laboratory trials that investigated the effects of eye movements while thinking of a distressing memory versus the same procedure without the eye movements in a non-therapy context. The total number of participants was 849.
Results: The effect size for the additive effect of eye movements in EMDR treatment studies was moderate and significant (Cohen's d = 0.41). For the second group of laboratory studies the effect size was large and significant (d = 0.74). The strongest effect size difference was for vividness measures in the non-therapy studies (d = 0.91). The data indicated that treatment fidelity acted as a moderator variable on the effect of eye movements in the therapy studies.
Conclusions: Results were discussed in terms of current theories that suggest the processes involved in EMDR are different from other exposure based therapies.

In particular, the trauma-focused cognitive behavioral therapy and the method of Eye Movement Desensitization and Reprocessing find more in addition to other intervention methods in the literature attention. parameters during activation of internal resources k g .
positive memories) and to compare these to activated traumatic
internal networks.
Antecedent studies show that traumatic stimulation on patients
with posttraumatic stress disorder (PTSD) finalizes in various
psychophysiological correlates. During therapy of these patients
a strong demand for activation of internal resources, i.e. activation
of fortitude and positives thoughts, exists. Especially EMDR
therapy uses resource stimulating elements such as position of
power and absorption in preparation for exposure. In this study
standardized EMDR protocols establish a solid basis to explore
individual internal resources.
Researches on trauma stimuli in EMDR- patients show effects
on parasympathetic tonus (Sack 2006) as well as increased cerebral
blood flow in defined brain regions (Levin 1999. Lamprecht
2000). Especially the heart rate variability (HRV) may describe
the sympatheticovagal balance (Cohen, 2002, Porges 1991). This
study focuses on psychophysiological effects and neurobiological
regulative mechanisms of stabilizing methods and activation of
internal resources in PTSD patients and healthy control group.
Methods: Healthy subjects and patients with diagnosed PTSD
(DIAX) listened to a commonly neutral script, an individual
trauma script and an individual absorption script. Following
each script measurements of heart rate variability (HRV), respiratory
flow, skin conductance responses (SCR) and skin blood
flow (LCF, TU50%) took place.
Results: Preliminary results revealed a significant reduced heart
rate variability in patients compared to the healthy controls in
reaction to the stress script as well as to the positive and the
neutral scripts.
Conclusion: To our knowledge this is the first time to be proven
that altered autonomous functions are found in PTSD not only
in reaction to traumatic reminders, but even to a positive, resource
activation situation. This provides our basement for further
research. Detailed analysis of different effects to each script
on both groups are currently underway.

Rorschach protocols from patients whose treatment includes the use of
EMDR are analyzed. Rorschach protocols are obtained prior to a first EMDR
session and following treatment. The structural summary for the pretreatment
protocols are used to formulate ideas (hypotheses) concerning
the desired and/or expected effects of treatment. Post-treatment protocols
are compared with pre-treatment protocols. Effects reported by patients in
treatment is related to changes in Rorschach protocols. The study is

Föreningen EMDR Sverige vill lämna följande kommentarer till
utkastet till riktlinjer avseende behandling av akut stressyndrom
och posttraumatiskt stressyndrom.
International Society for Traumatic Stress studies gör regelbundet sammanställningar
av evidensläget [1]. Det är viktigt att beakta att psykologiska
behandlingsformer i form av traumafokuserad KBT och EMDR är de viktigaste
och mest effektiva behandlingar. EMDR har varit kontroversiell men
är det inte längre, utan är en internationellt accepterad behandlingsmetod för
PTSD. Det är fortfarande inte allmänt accepterad att ögonrörelser har betydelse,
men nyligen har svensk och australisk forskning visat att ögonrörelser
under EMDR har tydliga fysiologiska effekter som är meningsfulla [2, 9],
dessutom har alla studier av fysiologi vid EMDR hittills samstämmande
visat dessa effekter, enligt en litteratursammanställning [3].
Under senaste åren har forskning om minnesfunktion och sakkadiska
ögonrörelser visat att minnessystem som till exempel episodminne (som ofta
är störd vid PTSD) påverkas på ett gynnsamt sätt av ögonrörelser [4-8]. Således
finns det i dag mycket som stödjer att ögonrörelser är meningsfulla
även om det kan vara svårt att förstå vid första anblicken. EMDR och exponeringsbehandling
är lika effektiva enligt metastudier, bland annat Cochrane
och i ISTSS aktuella genomgång av effektiva behandlingsmetoder för
PTSD.

EMDR Association of Sweden makes the following comments to
Draft Guidelines for the treatment of acute stress disorder
and post-traumatic stress disorder.
International Society for Traumatic Stress Studies makes regular summaries
of evidence-mode [1]. It is important to note that psychological
treatments in the form of trauma-focused CBT and EMDR is the most important
and most effective treatments. EMDR has been controversial but
it is no longer, but is an internationally accepted method of treatment for
PTSD. There is still no generally accepted that eye movements are important,
but lately, Swedish and Australian research has shown that eye movements
during EMDR has clear physiological effects that are meaningful [2, 9],
Moreover, all studies of physiology at the convergence of EMDR to date
shown these effects, according to a literature review [3].
In recent years, research on memory function and Sakka wash
eye movements showed that memory systems, such as episodic memory (which is often
is disturbed in PTSD) is affected in a favorable way of eye movements [4-8]. Thus
today there are a lot of support that eye movements are meaningful
although it may be difficult to understand at first glance. EMDR and exposure therapy
are as effective as meta-studies, including Cochrane
and in ISTSS current review of effective treatments for
PTSD.

Existing research into the after effects of
traumatic experiences with regard to children and adolescents
is scanty. Early intervention is intended to prevent or at least
reduce chronic manifestation of acute traumatic strain (Zehnder,
Hornung & Lanolt, 2006) since such strain has a negative
impact on the child's day-to-day quality of life and overall development, including the development and functioning of the
brain (Cohen, Perel, DeBellis, Friedman & Putnam, 2002).
Studies of multiple trauma among adults and adolescents have
shown that the severity of any impairment upon their psychological
health must be seen in relation to the number of traumatic experiences
that took place during childhood (Turner RJ, Lloyd DA
1995, Finkelhor D, Omrod RK, Turner HA 2007-1, Finkelhor D,
Omrod RK, Turner HA 2007-11, Holt MK. Finkelhor D, Kantor CK
2007). In this process, interpersonal traumatic experiences such
as accidents or severe illnesses can adversely affect development
as much as traumatic exposure connected to elements of crime.
Objectives: Interventions following acute traumatic strain will
be examined with regard to the symptoms and the mental
health of children and adolescents with multiple trauma experience
in the long term. The study will examine whether early
intervention has a positive effect on symptoms and whether
such effects are of a short or long-term nature.
The study should show whether gender specific and/or age
specific correlation can be identified in the development of
symptoms according to specific types of trauma, and whether
risk groups can be identified as a result.
The study will examine whether there exists an independent sub-group of children with multiple trauma under the age of 6,
whose symptoms correspond to a developmental trauma disorder
(van der Kolk 2005).
Methods: The study will be divided into a retrospective and
prospective part. The retrospective part will contain an examination of the treatment results of 150 children and adolescents
with multiple trauma experiences in the Vestische Children's
Clinic in Datteln between 2002 and 2009. This will be followed
by an evaluation of the treatment results by way of a newly
developed telephone catamnesis, based on validated questionnaires
(CRIES-13, ILK, Telekat) for measurement points TI-T3
Results: First results of the retrospective examination of children
and adolescents with multiple trauma experience will be presented
in comparison to the results of the evaluation of specific
therapeutic early interventions following acute strain among
children and adolescents with mono trauma experience.

Excerpt: Practice guidelines for the assessment and treatment of children and adolescents
with posttraumatic stress disorders (PTSD) were first developed by an expert
panel convened more than a decade ago by Cohen and the American Academy
of Child and Adolescent Psychiatry Work Group on Quality Issues (1998). Since
the release of that seminal set of practice guidelines, substantial additional validation
has been provided in scientific studies of the most robustly evidence-based
treatment model, trauma-focused cognitive behavior therapy (TF-CBT; Cohen
et al., 2006, 2008). Other approaches to the treatment of children and adolescents
with PTSD have been sufficiently clinically or scientifically tested to be
included as actually or potentially evidence-based (Saxe et al., 2007b; Vickerman
and Margolin, 2007) in the recent second edition of the International Society
for Traumatic Stress Studies (ISTSS) Practice Guidelines, Effective Treatments
for PTSD (Foa et al., 2008). These include eye movement desensitization and
reprocessing (EMDR; Spates et al., 2008), school-based cognitive behavior therapies

This article describes the integration of e-mail correspondence with EMDR treatment for a woman with a fear of driving and a diagnosis of dissociative disorder, not otherwise specified (DDNOS). When the client first presented for treatment, her diagnosis was unrecognized, and treatment showed limited success. With recognition of the DDNOS diagnosis, the treatment contract was renegotiated, with the focus of therapy shifting to addressing her dissociative experiences. Therapeutic progress was being made when the course of the treatment was interrupted by war in the north of Israel, and regular meetings became impossible. Communication was maintained by e-mail correspondence. When face-to-face sessions recommenced, the e-mail therapy continued because writing had become a powerful therapeutic tool. Therapy concluded with the successful treatment of both the dissociative disorder and the fear of driving. Cautions regarding the use of e-mail therapy are provided.
