Currently, the statistics associated with PTSD are staggering. Countless numbers of men, women, and children around the world are impacted every moment of every day by this extremely disruptive disorder. PTSD is very difficult to live with and can be even more challenging to resolve. A primary reason that the resolution of traumatic memories is such a challenge to treat is the fact that whenever any ounce of negative experience connected to the initial sensitizing event is sensed, the victim immediately reacts in a self-protective fashion by avoiding the experience any way he or she can. Cognitive behavioral therapy (CBT) appears to be the treatment of choice for many mental health clinicians who attempt to help patients recover from their traumatic memories. This author believes that CBT offers some benefit with regard to an understanding of the mechanism behind post-traumatic stress, as well as offer numerous ways to manage stress related symptoms. However, it does poorly in terms of completely resolving multiple traumas or working with chronic complex cases. In addition, a CBT approach has the proclivity to make the disorder more challenging by further increasing insult on the already malfunctioning autonomic nervous system of the victim. Instead, this manual suggests the combined use of EMDR and hypnosis as a more healthy and effective therapeutic modality model that can assist most individuals who suffer from even the most severe post-traumatic stress. The combination of EMDR and hypnosis takes a holistic approach towards healing by working with the defensive systems and the complete neuroanatomical system of the human being, as opposed to against.

EMDR combines stimuli that evoke divided attention – e.g. eye movements – with exposure to traumatic memories. Our objective was to investigate psycho-physiological correlates of EMDR during treatment sessions. A total of 55 treatment sessions from 10 patients with PTSD was monitored applying impedance cardiography. Onset of every stimulation/exposure period (n = 811) was marked and effects within and across stimulation sets on heart rate (HR), heart rate variability (HRV), pre-ejection period (PEP) and respiration rate were examined. At stimulation onsets a sharp increase of HRV and a significant decrease of HR was noticed indicating de-arousal. During ongoing stimulation, PEP and HRV decreased significantly while respiration rate significantly increased, indicating stress-related arousal. However, across entire sessions a significant decrease of psycho-physiological activity was noticed, evidenced by progressively decreasing HR and increasing HRV. These findings suggest that EMDR is associated with patterns of autonomic activity associated with substantial psycho-physiological de-arousal over time. [Author Abstract]

of respiratory sinus arrhythmia now challenge the view that exaggerated sympathetic tone and reactivity provide a sufficient account of the autonomic abnormalities seen in this diagnosis.
Alterations in autonomic tone during trauma therapy with EMDR: It has been hypothesized that EDMR, by pairing stimuli that evoke divided
attention with exposure to trauma memories, elicits repetitive orienting
responses followed by enhanced parasympathetic tone, resulting in significant
within-session psychophysiological de-arousal. We monitored 10 standard
EMDR treatments for PTSD (55 sessions) with impedance cardiography.
Heart rate (HR), parasympathetic tone (RMSSD), sympathetic tone (PEP), and
respiration rate (RESP) were assessed. Markers were set at the onset of every
stimulation/exposure period (N = 811). Effects within and across stimulation
sets were examined. An orienting response, with associated sharp increase
of parasympathetic tone and significant decrease of HR, was found at stimulation
onsets. During ongoing stimulation, sympathetic arousal increased
while parasympathetic tone decreased, responses consistent with stressrelated
arousal during trauma exposure. However, across entire sessions
there was a significant pattern of psychophysiological de-arousal, evidenced
by progressively decreasing HR and increasing RMSSD.

Neuro-imaging investigations in PTSD have reported findings for either functional or structural modifications.
Only two studies have investigated the changes in cerebral blood flow (CBF) following EMDR therapy and one
single-patient study recently reported EMDR-related structural changes. The aim of this study was to analyse the
structural and functional changes occurring in brain regions implicated in the patho-physiology of PTSD along
with the impact on CBF and volumetric data after EMDR treatment. Thirty six train drivers having being exposed
to traumatic experiences at work were assigned to different two groups: symptomatic (S, n=17) or nonsymptomatic
(NS, n=19), according to PTSD diagnosis. SPECT, following administration of an individualised

trauma script, and MRI were performed and EMDR therapy was administered to S. According to EMDR outcome S
were further subdivided in responders (R, n=11) and non-responders (NS, n=5). One patient was not treated. CBF
and volumetric analyses were performed on temporal lobes, hippocampi and nuclei caudati. ANOVA showed a
significantly higher CBF distribution in the left temporal lobe of S as compared to NS and the right nc.caudatus
was significantly larger in S. The R group had larger hippocampi and nc.caudati as compared to NR and the size of
hippocampi correctly identified 81% of R and NR. The increased left temporal lobe CBF distribution among
symptomatic PTSD subjects may reflect the affective significance of higher effort in emotional episodic memory
stimulation among symptomatic individuals. Hippocampi volume as measured before EMDR was shown to
predict with high accuracy the therapy outcome.

There is increasing evidence to support the biological basis of mental disorders. Subsequently, understanding the neurobiological context from which mental distress arises can help counselors appropriately apply cognitive behavioral therapy and other well-researched cognitive interventions. The purpose of this article is to describe the neurobiological context underlying the formation and treatment of posttraumatic stress disorders, a mental disorder frequently encountered by counselors, from a cognitive therapy framework.
Recent changes to the Council for Accreditation of Counseling and Related Educational Programs (2009) accreditation standards include the need for counselors-in-training to understand the neurobiological basis of behavior, which marks a new direction for the training of professional counselors who have historically reacted ambivalently toward medical models for understanding client concerns and treatments. Yet recent findings in neuroscience actually support the verbally based interventions that counselors typically use in treatment; therefore, there is much to be gained by counselors and counselor educators in understanding the basics of human neurobiology and how commonly used counseling interventions intervene on these biological systems. The National Institute of Mental Health (2010) stated in a recent strategic plan that “Important discoveries in areas such as genetics, neuroscience, and behavioral science largely account for the substantial gains in knowledge that have helped us to understand the complexities of mental illnesses and behavioral disorders over the past 15 years” (“Introduction,” para. 4).
Given the increasingly biological focus of mental health research, the practicing counselor is faced with the task of understanding and using the emerging mental health treatments and explaining to clients, to reimbursing agencies, and to the broader public how counseling fits within the medically dominated mental health culture. Some counselors have long reacted ambivalently toward the pathologically oriented diagnostic categories of the Diagnostic and Statistical Manual of Mental Disorders (4th ed.; DSM-IV;American Psychiatric Association, 1994) system and the medication-dominated world of psychiatry. For example, the contrasting viewpoints on this issue were published in the Journal of Counseling & Development between Allen and Mary Ivey (1998, 1999) and Scott Hinkle (1999). Ivey and Ivey (1998) argued for a developmental interpretation to the DSM-IV, opposing what they called the “pathological view” (p. 334) of the manual. According to Ivey and Ivey, disorders could be viewed through a positive development tradition to lie not within the individual but within the contextual systems in which a person lives. Subsequently, disorders are viewed as a “logical response to a developmental history” (Ivey & Ivey, 1999, p. 484). By contrast, Hinkle (1999) argued that because anxiety and depressive disorders “are the most common clinical symptoms associated with presentation to counseling” (p. 475), the counseling profession is weakened if counselors shy away from direct participation in the DSM nomenclature and treatment parlance. As Hinkle indicated, “mental disorders according to the medical model describe disease processes, not people” (p. 475). Regardless of the reader's philosophical perspective, practicing counselors know participation in medical and psychiatric systems is necessary at times. Also, recent discoveries in the field of neuroscience are providing evidence that interventions often used by counselors have direct physiological impact on client neurobiology (Kennedy et al., 2007; Linden, 2006). For example, Felmingham et al. (2007) demonstrated significant differences in brain activity before and after 8 weeks of exposure therapy, which correlated with a reduction in posttraumatic stress disorder (PTSD) symptom severity. Similarly, Paquette et al. (2003) found that cognitive behavioral therapy (CBT) alters the activation and metabolism of specific brain regions following successful treatment of spider phobia. These findings, along with others (for a detailed review, see Beauregard, 2007; Frewen, Dozois, & Lanius, 2008), are significant because they support the techniques, interventions, and approaches used by counselors and provide a mechanism by which counseling positively affects brain physiology. Within the emerging physiologically based treatment milieu, counselors should be prepared to articulate how cognitive counseling interventions make measurable changes to the client. Although cognitive-behavioral-based approaches are effective in the treatment of a number of psychiatric illnesses, adult PTSD is arguably one of the best understood mental disorders from a neurological perspective. It thus presents a valuable model for exploring not only the basic tenets of neurobiology but also the mechanisms behind its successful treatment. Furthermore, PTSD is a disorder that counselors will likely encounter in practice.
PTSD is a mental disorder characterized by a sudden onset of symptoms due to environmental exposure to a psychologically stressful event such as war, natural disaster, or sexual victimization. Thus, it provides a clear example of how, even in adulthood, neurological adaptation (in this case maladaptive changes) can functionally “rewire” the brain in a short period of time, resulting in a sustained array of clinical symptoms. The diagnostic criteria for PTSD are a history of exposure to a traumatic event meeting two criteria and symptoms from each of three symptom clusters: intrusive recollections, avoidant/numbing symptoms, and hyperarousal symptoms. A fifth criterion concerns duration of symptoms and a sixth assesses functioning (American Psychiatric Association, 2000).
The National Comorbidity Survey Replication, conducted between February 2001 and April 2003 (Kessler et al., 2005), determined that the estimated lifetime prevalence of PTSD among American adults is 6.8%, with women (9.7%) twice as likely as men (3.6%) to have the disorder at some point in their lives. These findings are very similar to those of the first National Comorbidity Survey conducted in the early 1990s (Kessler, Sonnega, Bromet, Hughes, & Nelson, 1995), which was composed of interviews of a representative national sample of 8,098 Americans ages 15 to 54 years. In this earlier sample, the estimated prevalence of lifetime PTSD was 7.8% in the general population. As in the more recent survey, women (10.4%) were more than twice as likely as men (5%) to have PTSD at some point in their lives (Kessler et al., 2005; Kessler et al., 1995).

treatment and no treatment (NT) were compared in reducing test anxiety. Thirty college students with high
test anxiety were randomly assigned to each condition. Pre-post assessments were conducted using the
Test Anxiety Inventory (TAI), State-Trait Anxiety Inventory (STAI), Rational Behavior Inventory (RBI), and
Autonomic Perception Questionnaire (APQ). Treatment therapists were licensed professionals with at least
2 years experience in their respective modality. Statistical analysis using a two-way analysis of variance with
repeated measures found significant interactions between time (pre-post) and treatment conditions for all
measures except the RBI. Post hoc Newman–Keuls analyses were conducted on the change scores, indicating
that both EMDR and B/SIT significantly reduced test anxiety. EMDR generally outperformed B/SIT.

Eye Movement Desensitization and Reprocessing (EMDR) treatment, Biofeedback/Stress Inoculation Training (B/SIT) treatment, and no treatment (NT) were compared in treating test anxiety. Subjects (30) reporting test anxiety were randomly assigned to EMDR, B/SIT or NT. Treatment consisted of three 90 minute sessions using B/SIT and one 45 minute session/three 75 minute sessions using EMDR. Treatment therapists were licensed psychologists, counselors or social workers with two to twenty years experience using EMDR or B/SIT. Subjects in the NT group were offered treatment following the study. Six dependent variables were measured pre/post: trait and state anxiety (State Trait Anxiety Inventory, STAI-T.S.), test anxiety (Test Anxiety Inventory, TAI-T.W.E.), rationality of personal beliefs (Rational Behavior Inventory, RBI), negative self statements (TAI-W) and self report of physiological response (Autonomic Perception Inventory, APQ) using.05 level. ANOVAs were performed followed by a Newman-Keuls Multiple Comparison. There was a significant interaction between pre-post tests and type of group on all dependent measures except the RBI. EMDR showed a tendency toward greater improvement from pre to post test than B/SIT and NT. There were no significant differences between groups on the pre test. Post group means indicated: (a) STAI-S, there was no difference between the groups; (b) STAI-T and APQ, EMDR and B/SIT had a greater reduction than NT; (c) TAI-E, EMDR showed more reduction than B/SIT and NT; (d) TAI-T and W, there were differences between all three groups with EMDR showing the most reduction. A meta-analysis was performed to determine clinical significance. Prior to treatment all three groups were above the normative effect size of 1 SD. The NT group showed no change on post tests. The EMDR group showed a larger reduction in effect size than B/SIT. Based on the data both EMDR and B/SIT reduced test anxiety with EMDR generally outperforming B/SIT. Eye Movement Desenzibilizace a přepracování (EMDR) představuje slibný přístup k léčbě posttraumatické stresové poruchy. Empirické poznatky poskytují důkazy o vlivu EMDR na snížení subjektivně vnímané živostí a emocionální zátěž z autobiographic vzpomínek a na snížení vzrušení autonomního nervového systému přesouváním rovnováhy směrem aktivace parasympatiku. Poslední hypotézy naznačují, že účinky EMDR spoléhají na neurobiologických mechanismů pracujících v duálním zaměřit pozornost, orientující reflex a REM spánek. Toto přepracování se zdá vyvolat specifickou neurobiological odpověď, že dělá pro zpracování informací v centrálním nervovém systému efektivnější, zejména u jedinců s PTSD.
Eye Movement Desensitization and Reprocessing (EMDR) represents a promising approach to treatment of posttraumatic stress disorder. The empirical findings provide evidence for the effect of EMDR on decreasing of subjectively perceived vividness and emotional burden of autobiographic memories and on reducing arousal of the autonomic nervous system by shifting the balance towards parasympathetic activation. Recent hypotheses suggest that the effects of EMDR rely on neurobiological mechanisms employed in dual focus attention, orienting reflex and REM sleep. The process of understanding PTSD has been a long and difficult one. It is safe to say our understanding of this disorder is incomplete, and our exploration into its pathophysiology is fairly recent. As with any disorder of the brain, the complexities of PTSD are extensive and require integrating cognitive, functional, and chemical components. Given this complexity, it is no wonder that treating PTSD has also been a challenge. Treating a disorder whose components are not fully understood is similar to shooting in the dark. Some shots have hit their mark and some have missed. More than ten years after its conception, the question of whether Eye Movement Desensitization and Reprocessing (EMDR) is a hit or a miss is still debated. If understanding the pathophysiology of PTSD is still recent, understanding the possible physiology behind EMDR is just beginning. Objectives: There has been a continued
debate regarding the role of eye movements in Eye Movement
Desensitization and Reprocessing (EMDR). This study examined
the possible autonomic effect of horizontal eye movements after
being exposed to fearful stimuli. Methods: Fifty two healthy
adult women were randomly allocated to eye movement or eye
fixed groups after watching a five minute fear-inducing film clip.
ECG was recorded during the resting state, after watching the
clip, and the treatment. A spectral power analysis of the heart
rate variability was performed. As the variables violated the rule
of normal distribution and the number in each group is small
the non-parametric test was used. Results: Overall, we did not find the differences between the groups in both time and
frequency domains. Some minor differences found were not
consistent with results from previous studies. Conclusions:
Effect of eye movement on autonomic nervous system during fear
desensitization was not supported in this experiment. Further study
with other psychophysiological measures is needed to understand
the role of eye movements in treatment of traumatic memory.

We publish in this issue a preliminary and tentative account of the reduction of deviant sexual arousal, as measured by phallometry, by eye movement desensitisation and reprocessing (EMDR). The purpose of this editorial is to show that the conclusions of Davidson and Parker (2001), and the comment by Salkovskis, can now be set aside, and to present our readers with some theoretical thoughts on some of the mechanisms by which EMDR could induce its effects, including trauma reduction. A major bar to the further acceptance of EMDR as a treatment and as an inviting research topic stems from the fact that workers still cannot see how eye movements can cause the reported clinical changes and the increasing number of temporally related psycho-physiological phenomena. This editorial suggests that the organs of computation of the mind have evolved by natural selection to solve problems of survival and, signally, include corollary discharge and feed forward (CD-FF) mechanisms by which they intrinsically function and also interact with one another. The effect of early interpersonal trauma on adult clients has been extensively studied and reported. Less attention has been
given to the effects of complex trauma on children. Because childhood is the time of most rapid brain development as well as the time when
children are dependent on adult caregivers, early physical, sexual, and emotional abuse, including neglect and witnessing domestic violence,
has severe effects on infants and children – disorganized attachment, affect and behavioral dysregulation, and dissociation. In this preconference
Sandra Wieland will describe how early trauma and neglect affects the infant/child’s brain, autonomic nervous system, and body
system. She will explain how dissociation is often used by children to protect themselves from the impact – both emotional and physical – of
complex trauma. Given this high level of fragmentation and fright, children are often not able to participate successfully in the standardized
EMDR protocol. Dr. Wieland will teach ways in which the EMDR protocol can be adapted in order to access more completely the child’s
fragmented internal system. She will use clinical vignettes to describe ways EMDR can assist in each of the three stages of trauma treatment
– safety/stabilization, trauma processing, and integration. Maria Elena Aduriz will describe a case of a girl whose traumatization is the direct

Abstract: In the year (2007) the manual of the neuropsychotherapeutic intervention program EMDR and biofeedback in the therapy of posttraumatic stress disorder by Jacobs and de Jong was published. The therapeutic concept based on new neuroscienctific findings, which declare a dissociation of implicit and explicit traumamemory as one reason for emergence a PTSD. On this background the treatment program integrates selective modules. The allocation of information carried out by an educational movie for patients, specific cognitive-behavioural intervention techniques, which are supplemented by biofeedback-supported Eye Movement Desensitisation and Reprocessing (EMDR). During the treatment the skin conductance (electrodermal activity), which is a stress parameter, is mesured. This program was already evaluated in the pilot-study. The patient pool could be enlarged, so that 28 patients were available for the evaluation. Additionally the educational movie was examined on efficiency. The total feedback was positive. After therapy-end the PTSD-symptomatology decreased consistently (demp=2.48), as well as the psychological stress in another problem areas (demp=1.30). Moreover the EMDR-method achieved objective a significant decrease of the autonomic arousal (demp=.79) and subjective an explicit reduction of the felt stress (demp=2.40), while growth of the coherence of a worked out positive cognition (demp=2.52). Three- and 12-month follow-up analysis could demonstrate the stability and sustainability of the changes. Dazu gehören spezielle kognitiv-behaviorale Interventionstechniken, Biofeedback-gestütztes Eye Movement Desensitization and Reprocessing (EMDR) sowie die gezielte Bereitstellung von Informationen über die Störungszusammenhänge für den Patienten. Mittels Biofeedback wird die elektrodermale Aktivität der Patienten während der EMDR-Sitzungen als Indikator für die autonome Erregung aufgezeichnet. Die bei der Traumaexposition implizit ablaufenden Prozesse werden somit an Therapeut und Patient zurückgemeldet. The manual presents a newly developed multi-modal neuropsychotherapeutisches therapy program for treatment of post traumatic stress disorder (PTSD before). Based on recent findings from the neurosciences that suggest that a dissociation between implicit and explicit memory of trauma is the main basis of the PTB, different modules were integrated into the treatment program. These include specific cognitive-behavioral intervention techniques, biofeedback-assisted Eye Movement Desensitization and Reprocessing (EMDR) and the targeted provision of information about the disorder correlations for the patient. Biofeedback is recorded, the electrodermal activity of patients during the EMDR sessions as an indicator of autonomic arousal. The case of trauma exposure implicit processes involved are therefore reported to the therapist and patient. This makes it possible to check whether the degree of autonomic arousal could be lowered so far that a successful processing and storage of traumatic memories is possible in the explicit memory system. Studies show the effectiveness of the program. Results nevertheless show a significant reduction of symptoms and PTB a strong decrease of the autonomous arousal and subjective burden.

Ausgehend von neueren Erkenntnissen in der Forschung neuroscienctific, so dass eine Dissoziation zwischen impliziter und expliziter Trauma-Speicher die wichtigste Grundlage der PTBS ist, verschiedene Module wurden im Rahmen der Behandlung integriert anzuzeigen. Die vereinigten Komponenten sind: spezifische Hintergrundinformationen über die Unordnung und typische PTSD-Symptome, eine pädagogische Film für Patienten, spezifische kognitive Verhaltenstherapie und Biofeedback-Techniken unterstützte Eye Movement Desensitization and Reprocessing (EMDR). Das Ziel der Biofeedback-Sitzungen während EMDR ist es, den Patienten eine direkte Rückmeldung über die implizite Prozesse während der Trauma-Exposition. Darüber hinaus Erfassung der physiologischen Daten über Biofeedback ermöglicht das Testen, ob es eine Korrelation zwischen dem Grad der subjektiven Belastung durch traumatische Erinnerungen ausgelöst (quantifiziert mit der SUD-Skala), und messbare physiologische Erregung. Elektrodermale Aktivität (EDA; Hautleitfähigkeit) wurde als eine physiologische Parameter gemessen. Die Ergebnisse einer durchgeführten Pilot-Studie (16 Patienten auf der Grundlage, mit einem wartenden Gruppe als Kontrollgruppe) zeigen verschiedene EDA-Muster während EMDR-desensitivation (fad und assoziative Wiederaufbereitung). Ein offensichtlich Reduktion der PTBS-Symptome gefunden (d = 2,27) sein, die stärker ist als in anderen Behandlungen. Die traumatischen Erinnerungen mit EMDR behandelt wurde weniger Anstrengung, die ebenfalls reflektiert in der Physiologie (verminderte autonome Erregung) und in der subjektiven Belastung fühlte sich durch die Patienten. Die Kürzungen der Erregung (d = 1,01) und subjektive Belastung (d = 2,55) zeigen, dass eine effektive Hemmung der Aktivierung der Amygdala-und damit der Angstreaktion selbst-aufgrund der Intervention geschaffen. Mit EMDR reduziert die Amygdala physiologische Erregung. Wir vermuten, dass aus diesem Grund den medialen präfrontalen Kortex und im Hippocampus kann eine kortikale Inhibition, die erfolgreich reduziert die Angst-Reaktion (Grawe, 2004) zu etablieren. Die berichteten Ergebnisse wurden durch einen dreimonatigen Follow-up-Bewertung bestätigt. The combined components are: specific background information regarding the disorder and typical PTSD-symptoms, an educational movie for patients, specific cognitive behavioral intervention techniques and biofeedback-supported Eye Movement Desensitization and Reprocessing (EMDR). The aim of using biofeedback during EMDR sessions is to give patients a direct feedback about the implicit processes during trauma-exposition. In addition, recording the physiological data via biofeedback allows testing if there is a correlation between the level of subjective strain, triggered by traumatic memories (quantified with the SUD-scale), and measurable physiological arousal. Electrodermal activity (EDA; skin conductance) was measured as a physiological parameter. The results of a conducted pilot-study (based on 16 patients, with a waiting group as a control group) show different EDA-patterns during EMDR-desensitivation (bland and associative reprocessing). An evident reduction of the PTSD-symptoms could be found (d = 2.27), which is stronger than in other treatments. The traumatic memories treated with EMDR became less straining, which reflects likewise in physiology (decreased autonomous arousal) and in the subjective strain felt by the patients. The reductions of arousal (d = 1.01) and subjective strain (d = 2.55) indicate that an effective inhibition of the amygdala activation—and thereby of the anxiety reaction itself—is created due to the intervention. Using EMDR reduces the amygdala induced physiological arousal. We suppose that for this reason the medial prefrontal cortex and the hippocampus can establish a cortical inhibition, which successfully reduces the anxiety reaction (Grawe, 2004). The reported results were confirmed by a three month follow-up evaluation. This report reviews a research proposal, the major objective of which is to assess the relation between PTSD (Post Traumatic Stress Disorder) and limbic, paralimbic, and prefrontal brain function as assessed with functional MRI (Magnetic Resonance Imaging), and to determine whether an Eye Movement Desensitization and Reprocessing (EMDR) intervention causes those structures to return to a normal mode of functioning. The proposed study secondarily explores the effectiveness of EMDR in reducing PTSD symptomatology. While the first and second objectives of the study may have scientific merit, the mandates of other funding agencies would appear to be more appropriate for investigations of basic neuroscience processes associated with brain functioning in PTSD (e.g., NSERC or CIHR). Importantly, a critical review of the EMDR research reveals that a great deal of controversy surrounds the effectiveness of this therapy. EMDR has not been shown to be more effective than presently validated PTSD exposure-based therapies, and the eye movement component of EMDR appears to provide no therapeutic benefit. Finally, there is a lack of clarity around specific items listed in the proposed budget. Given these concerns it is not recommended that this proposal be funded at this time.

Eye Movement Desensitization and Reprocessing, EMDR, har i psykoterapiforskning
visats vara en effektiv behandlingsmetod av posttraumatiskt
stressyndrom, PTSD. Däremot är ögonrörelsestimuleringen,
som är ett centralt moment i metoden, kontroversiell och dess
funktion inte klarlagd.
I föreliggande undersökning randomiserades trettiosex friska försökspersoner
till en av tre stimuleringsbetingelser: ögonrörelsestimulering,
taktil stimulering eller orörligt visuellt stimuli (kontrollgrupp). Stimuleringen
kombinerades med imaginär exponering, negativ och positiv.
Utfallet av experimentet mättes i olika fysiologiska mätparametrar,
samt självskattningar av obehagsnivån (SUD). Resultaten visade signifikant
högre hudkonduktansnivå vid ögonrörelsestimulering jämfört
med kontrollgruppen, relaterat till aktivering av det sympatiska nervsystemet.
Inga signifikanta gruppskillnader fanns i de övriga mätningarna.
Mönstret av autonom aktivering kan tyda på att ögonrörelsestimuleringen
utlöser eller förstärker en orienteringsrespons, vilket vissa
teoretiker föreslagit är den verksamma mekanismen i EMDR. Dock
behövs fortsatt forskning för att förstå denna verkan, samt koppling
till terapeutisk effekt.
Eye Movement desensitization and Reprocessing, EMDR, psychotherapy research has shown to be an effective treatment of post-traumatic stress disorder, PTSD. However, ögonrörelsestimuleringen, which is the lynchpin of the method, controversial and its function is not clear. In the present study were randomized thirty-six healthy subjects to one of the three stimulation conditions: eye movement stimulation, tactile stimulation or static visual stimuli (control group). The stimulation was combined with imaginary exposure, negative and positive. The outcome of the experiment were measured in various physiological test parameters, and self-estimates the level of discomfort (SUD). The results showed significantly higher hudkonduktansnivå of eye movement stimulation compared with control group, related to the activation of the sympathetic nervous system. No significant group differences were found in the other measurements. The pattern of autonomic activation may indicate that ögonrörelsestimuleringen trigger or reinforce an orientation response, as some theorists suggested is the active mechanism in EMDR. However, further research is needed to understand this effect, and access to therapeutic efficacy.

Eye Movement Desensitization and Reprocessing (EMDR) was developed in 1987 by Francine Shapiro, as a modality for relieving anxiety, traumatic memories, intrusive thoughts, and reprocessing negative self-beliefs to positive self-beliefs. One of the most common uses of EMDR in recent years has been the treatment of PTSD.This current study investigated the effects of EMDR across a sample of 3 sexually abused women diagnosed with PTSD using a multiple baseline design across subjects. The study specifically focused on whether or not intervention with EMDR effects traumatic memory and negative/irrational cognitions, decreases stress or changes levels of anxiety, depression and heart rate. The study intended to assess the efficacy of EMDR while simultaneously reduce human suffering and answer some of the more serious criticisms which have blurred confidence in EMDR outcome research. Specifically, the study controlled for a number of the criticisms in the literature predominantly through a confirmation of an accurate PTSD diagnosis and through the use of a multiple baseline design. The multiple baseline design was applied sequentially to the same problem across different but matched subjects sharing the same environmental conditions. Heart rate level and well-known psychometrics were used to obtain baseline, intervention and post-intervention measures. Psychometric scores reflecting levels of depression, anxiety, and subjective levels of the impact of distress regarding the trauma were assessed along with the levels of anxiety currently experienced about the trauma and subjective ratings regarding the acceptance of the preferred, self-generated positive cognition. The measures used in this study were an initial clinical interview, an Anxiety Disorders Interview Schedule for the DSM-IV, Beck Depression Inventory, Beck Anxiety Inventory, Wolpe's Subjective Unit of Disturbance Scale, Validity of Cognition, Impact of Event Scale and heart rate. The study reported descriptive statistics to analyze the multiple baseline study and to determine EMDR's clinical significance in treating PTSD. The effects of EMDR on the three PTSD subjects of this study demonstrated that meaningful changes occurred in several areas. Subjective disturbance and stress surrounding the traumatic memory decreased, positive self-cognitions increased, and both depression and anxiety levels decreased following EMDR treatment. No change in heart rate physiology occurred. All of the study's treatment measures were maintained at follow-up. The results of this study suggest that EMDR may be a powerful and effective intervention to reduce patient suffering in a relatively painless fashion. [Author Abstract]

18 subjects distressed by memories of a specific traumatic event were randomly assigned to a single session of 1 of 3 conditions: Eye Movement Desensitization and Reprocessing (EMDR), a Time Interval Condition (TIC), or Tapping Alternate Phalanges (TAP). Compared to TIC and TAP, autonomic measures showed distinct changes during EMDR: (1) respiration synchronized with the rhythm of the eye movements in a shallow, regular pattern; (2) heart rate slowed significantly overall; (3) systolic blood pressure increased during early sets, invariable declined during abreactions, and decreased overall; (4) finger tip skin temperature consistently increased; and (5) the galvanic skin response consistently decreased in a clear "relaxation response." This relaxing effect of the eye movements suggests that at least one of the mechanisms operating during EMDR is desensitization by reciprocal inhibition, by pairing emotional distress with an unlearned or "compelled" relaxation response. It is hypothesized that eye movements used in EMDR practice influence both memory and emotional state, some laboratory studies provide evidence for such influence (Christman et al, 2003, Neurobiology; Barrowcliff et al., J. Forensic Psychiatry and Psychology, 2004). From a neurophysiological point of view, it is also interesting to examine to what extent some eye movement parameters may be modulated by memory and emotions. For instance, it is well established that memory driven saccades in simplified laboratory tests, present different characteristics (longer preparation time, lower accuracy, lower velocity) than visually guided movement. During EMDR the eye movements (mostly pursuit) are elicited externally by the therapist, yet at the same time the patient is in touch mentally with the traumatic image and related negative emotions. The physiology of such movements might be different and this could be explored with specific studies. Another important issue is to what extent movements of the eyes are helpful in coding and retrieving visual images. Accordingly to scanpath theory, eye movements are cognitively driven by sensory and motor representation; repetitive eye movements to the same area of an image are attributed to checking if the image corresponds to what the observer imagines (Noton & Stack, 1971; Brandt & Stark, 1997). Future laboratory research could bring together such theories and EMDR psychotherapy, e.g., by examining spontaneous eye movements related to negative image.
In this conference, we will present preliminary data from a laboratory study on eye movement patterns during the Roschach test. Eight healthy students conducted the experiment. Seated in front of a computer screen, subjects were equipped with a binocular video eye tracker (Chronos) sampling eye position images from each eye every 4 msec. Instantaneous eye position data were stored on a computer and analyzed afterwards with laboratory software. Each are of the Roschach test was presented for 30 sec., after which, the experimenter (a psychologist) recorded the response given by the subject.
Analysis of eye movement patterns were examined in relation to subjective verbal reports.
Results: The first question asked was to what extent the eye movement pattern reflected the imaged projected by the subject or by the visual properties of the Rorschach image itself. To gain some insight, we concentrated on the cards for which our subjected gave no standard responses, thus very different from one subject to the other (e.g., card IX). Eye movement exploration (the surface of the image explored, its patiaol composition e.g. the resprctive proportion of horizontal versus vertical movements) was correlated with the verbal report and the specific mental imagery (larger surface, multiple focusing points when several personages or objects were imaged). This provides evident for top-down influence; the eyes are exploring the projective interpretative image; repetitive movements back and forth to same points which help in turn consolidate this mental imagery.
Psychological analysis of eye movements (saccades and fixation) was also made. Saccades are found to be as fast as when looking reflexively to single visual targets; during fixations, however, between saccades, the eyes were more instable, particularly the visual uses were crossing at different depths from one fixation to another. Further ongoing analysis will correlate fixation duration and depth instability to spatial and emotional content of the subjective report.

Eye Movement Desensitization and Reprocessing, EMDR, har i psykoterapiforskning visats vara en effektiv behandlingsmetod av posttraumatiskt stressyndrom, PTSD. Däremot är ögonrörelsestimuleringen, som är ett centralt moment i metoden, kontroversiell och dess funktion inte klarlagd.
I föreliggande undersökning randomiserades trettiosex friska försökspersoner till en av tre stimuleringsbetingelser: ögonrörelsestimulering, taktil stimulering eller orörligt visuellt stimuli (kontrollgrupp). Stimuleringen kombinerades med imaginär exponering, negativ och positiv. Utfallet av experimentet mättes i olika fysiologiska mätparametrar, samt självskattningar av obehagsnivån (SUD). Resultaten visade signifikant högre hudkonduktansnivå vid ögonrörelsestimulering jämfört med kontrollgruppen, relaterat till aktivering av det sympatiska nervsystemet. Inga signifikanta gruppskillnader fanns i de övriga mätningarna. Mönstret av autonom aktivering kan tyda på att ögonrörelsestimuleringen utlöser eller förstärker en orienteringsrespons, vilket vissa teoretiker föreslagit är den verksamma mekanismen i EMDR. Dock behövs fortsatt forskning för att förstå denna verkan, samt koppling till terapeutisk effekt.
Eye Movement desensitization and Reprocessing, EMDR, psychotherapy research has shown to be an effective treatment of post-traumatic stress disorder, PTSD. By contrast, ögonrörelsestimuleringen, which is the lynchpin of the method, the controversy and its function is not clear.
In the present study were randomized thirty-six healthy subjects to one of three stimulation conditions: eye movement stimulation, tactile stimulation or static visual stimuli (control group). The stimulation was combined with imaginary exposure, negative and positive. The outcome of the experiment were measured in various physiological measurement parameters, and self-estimates of the level of discomfort (SUD). The results showed significantly higher hudkonduktansnivå the eye movement stimulation compared with control group, related to the activation of the sympathetic nervous system. No significant group differences were found in the other measurements. The pattern of autonomic activation may indicate that ögonrörelsestimuleringen trigger or reinforce an orientation response, as some theorists suggested is the active mechanism in EMDR. However, further research is needed to understand this effect, as well as access to therapeutic efficacy.

allo stress e può essere misurata nei domini di tempo o di frequenza. Lo studio della HRV è stato applicato in psichiatria ai disturbi
d’ansia (panico, DOC, PTSD) e dell’umore (depressione unipolare) in ricerche trasversali (confronto con controlli sani) e longitudinali di trattamento farmacologico (triciclici,
SSRIs) ma in pochi casi a trattamenti psicoterapici. Gli AA. valutano l’impiego della HRV come indicatore biologico nel trattamento psicoterapeutico di pazienti con Disturbo dell’adattamento erdurante da oltre un mese dopo l’esposizione ad eventi vitali stressanti (EVS). Discussione: a livello basale gli indicatori HRV di distress vegetativo correlano positivamente con collera-ostilità e sintomi intrusivi, negativamente con ansia e depressione. Anche dopo tre mesi dalla fine del trattamento gli interventi psicoterapeutici tendono a ridurre i punteggi sintomatici, e migliorano il bilancio simpato-vagale durante il riascolto dell’evento traumatico.
Introduction: trauma patients have symptomatic changes such as intrusiveness, avoidance and increased arousal, which hampers the development / integration
of memory traces, as well as speak at the peripheral level. Specific techniques of treatment seem to promote
the formulation and integration of traumatic memories, including eye movement desensitization and reprocessing
(EMDR). In evaluation research is therefore necessary to combine psychological scales appropriate indicator
organic autonomic activation. The heart rate variability (Heart Rate Variability - HRV) expresses
functional integrity of the autonomic nervous system in response
stress and can be measured in time or frequency domains. The study of HRV has been applied in psychiatric disorders
anxiety (panic, DOC, PTSD) and mood (unipolar depression) in cross-disciplinary (compared with healthy controls) and longitudinal drug treatment (tricyclic
SSRIs), but in a few cases in psychotherapeutic treatment. The AA. evaluate the use of HRV as a biological indicator in the psychotherapeutic treatment of patients with adjustment disorder erdurante more than a month after exposure to stressful life events (EVS). Methods: 6 subjects were recruited (M / F = 1 / 5, age 40.5 ± 11.0) exposed to EVS and initiated treatment cycle psychotherapeutic

We tried to “dissect” the EMDR method in the component desensitization through rapid eye movement (EMD), that is also the original part, to explain the neurophysiological functioning, Charney et al, say that a lot of persistent symptoms of PTSD are caused by an elevate arousal of NVS, particularly due to the increase of norepinephrine. Others hypothesize, again, that EMDR method is rapid because it is based on Classic Conditioning. Pavlov found that every stimulus of environment causes to the organism (animal or human) an immediate orienting response due to the arousal of the Autonomic Nervous System. In this situation the animal begins to look around moving its eyes laterality (as it happens during ocular movements of EMD); if it perceives a danger in the environment it stimulates a visceral negative response (hyperactivation that means fear) that leads an an avoidance behaviour (stiffening) and/or fighting. In other cases it causes a visceral positive response (de-arousal process that means reassurance) that leads to an approach behaviour through interaction or exploration.
The EMD should produce an orienting reflex in the patient in a protective sitaution like a psychotherapeutic setting; that means an important reduction of neuro-vegetative nervous system activity (dearousal) and some visceral positive response. We propose an experimental design with a single subject (ABACADA) by interruption treatment and by monitoring Neurophysiological functions through biofeedback instruments to test this neurophyiological hypothesis.

arousal, stay present rather than dissociating, tolerate positive or negative affect, or
differentiate past and present. Beset with an array of baffling, intense symptoms that
“tell the story” without words, they become uncertain both of what happened and
how they endured it. To make sense of the sensations and overwhelming emotions,
clients rely upon trauma-related cognitive schemas to interpret their experience: “I am
still not safe,” “I am a marked woman,” “I am worthless and unlovable.” These
cognitive schemas often increase the bodily dysregulation, resulting in looping or
inability to fully process and integrate the traumatic events. With such clients, the use
of body-centred techniques in preparation for or during EMDR processing can help to
increase affect and autonomic tolerance, strengthen both somatic and psychological
resources, and increase EMDR effectiveness by facilitating optimal levels of autonomic
arousal, which is neither too high nor too low, however is necessary for successful
desensitization and integration.
This workshop will introduce a number of interventions for working with traumatically
encoded somatic experience derived from Sensorimotor Psychotherapy, a bodycentred
talking therapy for trauma developed by Pat Ogden, Ph.D. that addresses the
non-verbal, autonomic components of PTSD by using the body both as a source of
information and a reservoir of resources. Sensorimotor Psychotherapy offers simple
body-oriented interventions for tracking, naming, and safely exploring trauma-related
experience, modulating a dysregulated autonomic nervous system, creating new
resources and competencies, and restoring a somatic sense of self. Sensorimotor
Psychotherapy can be easily integrated into EMDR treatments, used during
stabilization to prepare clients for more effective EMDR processing, during processing
to ensure effective and complete desensitization, or to enhance installation of positive cognitions and facilitate integration.

Patients atteints de SSPT démontrer anormale des réponses psychophysiologiques aux événements stressants. Ce dérèglement automatique est donc l'une des mesures objectives de stress post-traumatique qui pourrait également être utilisé pour évaluer les effets traitement du SSPT. Étant donné que la thérapie EMDR semble être un traitement de choix pour les victimes de traumatismes, l'objectif de l'étude était de vérifier que les réponses psychophysiologiques au stress a diminué après une séance EMDR unique. Six patients atteints du SSPT ont été traités par un thérapeute EMDR. Tout d'abord, au niveau clinique, à la fin de la session, les six patients ont eu une réduction très nette de leur niveau de perturbation subjective (SUD), leurs scores SSPT diminué (évaluée par le PLC-S), et leur auto-évaluation ( COV) est devenue positive, confirmant des études antérieures sur l'efficacité de l'EMDR. Deuxièmement, leurs réponses psychophysiologiques (rythme cardiaque, la conductance de la peau, le rythme respiratoire et la température de la peau) pendant un état de détente et tout en visualisant leur propre événement traumatique ont été enregistrées avant et après la session de l'EMDR. Malgré un nombre restreint de patients, après une seule séance EMDR, les réponses physiologiques à l'évocation de l'événement traumatique a diminué de manière significative. Cela comprenait la conductance de la peau, la fréquence cardiaque et la température de la peau. Ces résultats de l'étude préliminaire de confirmer l'efficacité du traitement EMDR sur le SSPT de la première session. Ils mettent également en évidence les effets thérapie EMDR sur le système nerveux autonome. Les changements psychophysiologiques enregistrés peuvent faire partie de mécanismes sous-jacents de traitement EMDR. D'autres études, y compris ces mesures seront donc nécessaires pour tester l'hypothèse.
PTSD patients demonstrate abnormal psychophysiological responses to stressful events. This automatic dysregulation is thus one of the objective measures of PTSD which could also be used to assess therapy effects on PTSD. Given that the EMDR therapy appears to be a treatment of choice for trauma victims, the aim of the study was to verify that the psychophysiological responses to stress decreased after a single EMDR session. Six PTSD patients have been treated by an EMDR therapist. First, at the clinical level, at the end of the session, all six patients had a very clear reduction of their subjective disturbance level (SUD), their PTSD scores diminished (as assessed by PLC-S), and their self-assessments (VOC) became positive, confirming previous studies on EMDR’s efficacy. Second, their psychophysiological responses (heart rate, skin conductance, respiration rate, and skin temperature) during a relaxing state and while visualizing their own traumatic event were recorded before and after the EMDR session. Despite small number of patients, after only one EMDR session, physiological responses to the evocation of the traumatic event decreased significantly. This included skin conductance, heart rate and skin temperature. These preliminary study results confirm the EMDR treatment efficiency on PTSD from the first session. They also highlight the EMDR therapy effects on the autonomic nervous system. The psychophysiological changes recorded may be part of the mechanisms underlying EMDR treatment. Further studies including these measures will therefore be necessary to test the hypothesis.

In a trauma survivor, physiological responses are exacerbated when the affect-laden memories stored in associative networks are triggered by environmental sensory inputs or cues and activate the autonomic nervous system. Recovery from trauma involves not only amelioration of physiological and dissociative symptoms, but also the cognitive rebuilding of a viable assumptive world view which integrates the realms of vulnerability, meaning and self-esteem. This world view is contextualized in cultural idioms and values.
From an Ericksonian perspective, persons are usually attempting to problem-solve, even in a dissociative date. A symptom such as an intrusive recollection or recurrent dream of a traumatic event is therefore construed as a request for help in problem-solving. Hypnosis is a structured dissociation which facilitates cognitive flexibility; that is, the broadening of choices of the client' s belief system, rather than direct work on changing affect or behaviors. The goal of Ericksonian hypnosis is to recontextualize the traumatic memory, the affect of fear, and the physiological hyperarousal cued by the traumatic memory. This occurs within a broader context of pride, mastery and courage, and within a context inclusive of other memories and affects, which are positive for the client.
Eye Movement Desensitization and Reprocessing (EMDR), similarly has as its goal the facilitation of a transfer of traumatic data from the cortical right hemisphere to the left hemisphere. EMDR also utilizes the attainment of a state of heightened awareness, or collaborative structured dissociation, in order to facilitate the orientation of the traumatized client's conscious mind toward "revisiting" traumatic memories.
Both interventions can facilitate the self-narrative reconstruction process of trauma survivors by simultaneously modulating the person's hyperarousal while attending to the culturally significant metaphors which form the building blocks of a person' s world of meanings.

In the recent years, the number of studies using neuro-imaging to evaluate neural correlates of psychotherapy
has steadily increased revealing its clear neurobiological effects on brain function across a wide range of
psychiatric disorders. Functional studies by single photon emission computed tomography (SPECT) and positron
emission tomography (PET) can now reliably detect changes in cerebral blood flow (CBF) and metabolism
patterns, suggesting a specific role for each of the brain areas in various components of emotional processing.
Investigations by magnetic resonance imaging (MRI) have also revealed psychiatry disease-related structural
changes. Some regions have been reported to be associated with emotional response to trauma, and with
symptom formation in posttraumatic stress disorder (PTSD). Several studies have provided evidence for the
efficacy of Eye Movement Desensitization and Reprocessing therapy (EMDR) in the treatment of PTSD. However
a very limited number of studies have investigated the neurobiological substrate of such therapy in clinical
practice. SPECT and MRI studies, performed to examine the effects of EMDR on brain patho-physiology have
provided some preliminary evidence that changes in brain CBF and structure patterns may follow effective
treatment. In general in PTSD and in anxiety disorders functional deactivations parallel symptoms relief and
decreased hyperreactivity to emotional and memory disturbances. Functional neuro-imaging is a promising tool
for the investigation of the physiological impact of psychotherapy in anxiety related disorders and may thus pave
the road for a better detection of its effects in psychiatric treatment. The scientific literature reporting
PTSD/EMRD related neuro-imaging studies will be extensively reviewed.

This study explored physiological correlates of eye movements during EMDR in relation to current hypotheses; distraction, conditioning, orienting response activation, and REM-like mechanisms. During EMDR therapy, fingertip temperature, heart rate, skin conductance, expiratory carbon dioxide level, and blood pulse oximeter oxygen saturation, were measured in male subjects with PTSD. The ratio between the low and high frequency components of the heart rate power spectrum (LF/HF) were computed as measures of autonomic balance. Respiratory rate was calculated from the carbon dioxide trace. Stimulation shifted the autonomic balance as indicated by decreases in heart rate, skin conductance and LF/HF-ratio, and an increased finger temperature. The breathing frequency and end-tidal carbon dioxide increased; oxygen saturation decreased during eye movements. Abstract: The aim of the study was to explore the working mechanism of the bilateral stimulation component in EMDR in relation to current hypotheses. Physiological hypotheses (of orienting response and relaxation response) were tested in an analogous experimental setting. Moreover, the clinical advice to use different stimulation speeds for different phases of EMDR was examined at the autonomic level, exploring two different speeds of eye movements. In a within-subject design 42 female students performed 7 short sets each of rapid eye movements (1 Hz), slow eye movements (0.3 Hz) and eye fixation while engaging in negative autobiographical memories. Order of the tasks was counterbalanced. Electrooculogramm (EOG), electrocardiogramm, electrodermal activity and distal pulswave were recorded continuously. Skin conductance level (SCL), heart rate (HR), heart rate variability (RMSSD) and pulse wave transit time (PWTT) were calculated. Performance of the eye movement tasks were controlled through EOG. All three conditions showed a clear-cut de-arousal at stimulation onset, which was 30 indicated by a decrease in HR and SCL and an increase in RMSSD and PWTT. That means, fast and slow eye movements as well as eye fixation initiated a moderate, but clear relaxation response with increased parasympathetic and reduced sympathetic activity. These results suggest that the speed of eye movements do not influence autonomic functions. Furthermore, the results of this study do not support most of the current hypotheses regarding the working mechanism of the bilateral stimulation in EMDR. Alternative explanations are discussed

Following exposure to trauma, a large number of survivors will develop acute symptoms of posttraumatic stress disorder (PTSD), which mostly dissipate within a short time. In a minority, however, these symptoms will evolve into chronic and persistent PTSD. A number of factors increase the likelihood of this occurring, including characteristic autonomic and hypothalamic-pituitary-adrenal axis responses. PTSD often presents with comorbid depression, or in the form of somatization, both of which significantly reduce the possibilities of a correct diagnosis and appropriate treatment. Mainstay treatments include exposure-based psychosocial therapy and selective serotonin reuptake inhibitors, such as paroxetine and sertraline, both of which have been found to be effective in PTSD. Clients with chronic and complex Posttraumatic Stress
Disorder (PTSD) caused by severe traumata in childhood,
are often still treated inadequately, especially when they
developed additional dissociative symptoms or a Dissociative
Disorder.
There is a current consensus about the need tomodify the
EMDR standard protocol in the treatment of complex
traumatized and dissociative clients. From the theoretical
background of structural dissociation theory and Janet’s
system of action systems, the therapist has not only to recognize
clients’ deficits but also to address action tendencies
in the client’s inner system that could ameliorate
coping strategies in solving problems in daily life.
With the help of bilateral stimulation, blending of parts
of the personality containing different information can
be promoted often resulting in remarkable changes in the
behavior and resource activation. The main principles
that need to be regarded treating complex and dissociative
clients with bilateral stimulation will be explained
and basic rules for treatment plans including the use of
EMDR will be developed.
English subtitled video examples demonstrate the proposed modification
of the EMDR standard protocol and will
be discussed in detail. I will present shortly recent research
findings on autonomic nervous system alterations
during EMDR. These results help to define special
working mechanisms of our modified EMDR protocol in

This study was designed to investigate the question of whether psychophysiological changes during
EMDR sessions are related to subjective and objective reduction of PTSD symptoms. During-session
changes in autonomic tone in relation to session-to-session changes of subjective stress, trauma-related
symptoms, and psychophysiological reactions during a traumatic reminder were investigated in 10 patients
suffering from single-trauma PTSD. Treatment duration followed each patient’s individual needs
and ranged between 1 and 4 sessions, resulting in a total of 24 EMDR treatment sessions from which
psychophysiological data were completely recorded. Treatment with EMDR was followed by a significant
reduction of trauma-related symptoms, elimination of the PTSD diagnosis in 8 of the 10 participants,
as well as by significantly reduced psychophysiological reactivity to an individualized trauma script. Psychophysiological
dearousal in sessions correlated significantly with decrease in script-related reactions
in heart rate and parasympathetic tone, and with changes in subjective disturbance. Our results indicate
that information processing during EMDR is followed by during-session decrease in psychophysiological
activity, reduced subjective disturbance and reduced stress reactivity to traumatic memory.

We report about an empirical investigation of EMDR working mechanisms. 55
EMDR sessions were psycho physiologically monitored including start and end
of each stimulation set (N = 811). The results demonstrate a sharp increase of
parasympathetic tone (RMSSD) and a prolonged decrease of heart rate as
soon as stimulation begins. Observations during treatment sessions show a
significant psychophysiological dearousal in terms of a decrease of HR and
a significant increase of parasympathetic tone.

new
possible
intervention
models
in
psychotherapy
directly
acting
on

or

process,
without
disregarding
its
psychobiological
quality.
Clinical
Application
Since
I
believe
that
the
inter-‐brain
perspective
is
the
most
efficient
not
only
for
the
etiological
explanation
but
especially
for
the
restoration
of
relational
impairments
occurred
during
brain-‐brain
interactions,
I
chose
eye
contact
(EC),
because,
according
to
several
scholars,
it
is
a
privileged
communication
channel,
in
particular
between
mother
and
child.
Several
scholars
agree
that
all
forms
of
psychopathology
share
a
failure
in
emotional
regulation,
which
can
be
mostly
traced
back
to
the
failure
in
the
child-‐
caregiver
adaptive
tuning
and
therefore
to
the
impairment
of
their
inter-‐brain
communication.
An
intervention
on
the
EC
shifts
the
therapy
focus
on
this
dysregulation
to
restore
its
functions.
The
EMDR
AIP
approach
relies
on
the
brain
adaptive
processing
ability.
EMDR
has
proved,
in
appropriate
conditions
(good
therapeutic
alliance,
client
stabilization,
compliance
with
the
EMDR
protocol),
our
brain
can
repair
traumatic
injuries,
i.e.,
reacquire
and
use
information
dysfunctionally
stored
after
a
trauma.
Successful
use
of
EMDR
on
target
not
directly
traceable
to
a
traumatic
event
(e.g.,
defenses,
chronic
pain,
etc.)
allows
for
the
possibility
to
use
this
processing
tool
in
increasingly
broad
fields
and
refines
its
resources.
Thanks
to
its
three-‐pronged
approach
to
dysfunctionally
stored
information
in
the
brain
(EMDR
works
on
the
cognitive,
emotional
and
somatic
level),
the
inter-‐brain
quality
of
its
scope
(the
therapeutic
alliance
is
part
of
the
healing
process)
and
for
its
focus
on
the
present
(EMDR
works
on
the
present,
i.e.,
on
the
current
and
active
components
of
the
pathogenetic
memory,
bypassing
all
mediations
and
interpretation),
EMDR
seemed
the
most
appropriate
therapeutic
tool
to
intervene
on
the
EC
dysregulation
found
in
several
psychiatric
pathologies.
Conclusion
An
EMDR
protocol
for
the
exploration
and
modulation
of
the
EC
is
proposed.
This
protocol
proved
particularly
useful
with
depressed
or
severely
dissociative
clients.
After
making
clients
aware
of
their
difficulty
in
maintaining
the
EC,
they
are
retrained
to
use
this
contact
first
on
objects,
then
on
animals
(excellent
mediators
of
a
primitive
form
of
social
contact)
until
they
are
able
to
achieve
eye
contact
with
the
therapist.
During
this
training,
clients
are
encouraged
to
become
aware
of
their
body
sensations,
emotions
and
beliefs,
and
the
positive
ones
are
installed
with
BLS.
Memories
of
relational
situations
where
clients
identify
an
impaired
EC
are
identified
and
these
are
targeted
with
the
standard
protocol.
The
focus
then
shifts
to
present
and
future
situations.
The
regulation
purpose
of
this
protocol
affects
the
application
mode:
interventions
must
never
be
dysregulating,
therapists
must
proceed
slowly.
Clients
must
be
rigorously
kept
within
their
window
of
tolerance,
must
be
trained
to
recognize
it
and
able
of
staying
within
its
boundaries
with
respect

Porges’ polivagal theory of the hierarchical interpretation of the autonomic nervous system (ANS), in addition to extending the range of human beings’ possible responses to environmental demands, links the first line ventral-vagal response with the regulation of important viscera as well as that of facial and head muscles, mediating social interactions, and associates its activation with the feeling of safety, identifying this latter condition as essential for a person’s well-being.
Without safety no social relations, physiological regulation or healing are possible. Hence the importance for EMDR therapists to lead their clients to this condition: lacking the activation of the ventral-vagal circuit there can be no processing. EMDR therapists will be provided with tools to keep their clients in safe conditions within the setting.
Clients exposed to trauma and/or insecure attachment do not have a good ANS regulation and maintain inadequate defensive attitudes – as demonstrated by Porges’ results, easily comparable with Schore’s on affective development and with those of several multi-disciplinary scholars.
Exploring this dysfunction provides EMDR therapists with useful elements to guide their clients in the difficult task of confronting what they did not/could not process at that time. We propose a three-pronged cross-sectional assessment, regardless of the pathology, aimed at identifying the defensive arousal state of the ANS needed to face the dysregulating impact at that time, focusing on the prevalent activation style of the client, when meeting environmental challenges, and that emerging in the session. Starting from this assessment, EMDR therapists will be provided with tools to help clients recognize and master their defenses to increase their flexibility.
Using the regulation as a healing instrument and goal, and given the two-directional psychophysiological approach, where psychological and physiological processes meet, a new intervention model, stemming from the AIP-EMDR approach, is proposed, acting directly on the missing or impaired developmental stages of the self-regulation ability, consistently with what Porges hoped for.
The intervention focuses on Eye Contact (EC), because, as confirmed by several scholars, this is a privileged communication pathway, in particular in the mother-child dyad, to learn self-regulating skills and is easily impaired in psychiatric clients.

parts still as vivid as the day they were formed and other parts forgotten. At the same time, the memory becomes
integrated into wide-ranging memory networks that create a context for the original memory and, in the process,
construct an implicit interpretation of the memory.
While the processing of small, everyday, distressful events is normally handled efficiently by these automatic
mechanisms, grief and painful traumas can overwhelm them. When this happens, the memory is left frozen in its
original form—raw, intrusive, distressing, and unexplained––a condition that can lead to PTSD. The goal of any
treatment for PTSD, then, should be to “restart” these processes of memory evolution, and to thereby support
resolution of the trauma. How each of the various scientific models of EMDR might explain this facilitation of
memory processing will be the focus of the workshop.

This article explores the process of memory consolidation leading to persistent modifications in synaptic plasticity as a mechanism by which psychotherapy facilitates changes in the permanent storage of information acquired throughout the individual's life. The psychobiological interrelationships of affect, attachment, and memory offer a perspective regarding the etiology and treatment of clinical disturbances of affect. Föreningen EMDR Sverige vill lämna följande kommentarer till
utkastet till riktlinjer avseende behandling av akut stressyndrom
och posttraumatiskt stressyndrom.
International Society for Traumatic Stress studies gör regelbundet sammanställningar
av evidensläget [1]. Det är viktigt att beakta att psykologiska
behandlingsformer i form av traumafokuserad KBT och EMDR är de viktigaste
och mest effektiva behandlingar. EMDR har varit kontroversiell men
är det inte längre, utan är en internationellt accepterad behandlingsmetod för
PTSD. Det är fortfarande inte allmänt accepterad att ögonrörelser har betydelse,
men nyligen har svensk och australisk forskning visat att ögonrörelser
under EMDR har tydliga fysiologiska effekter som är meningsfulla [2, 9],
dessutom har alla studier av fysiologi vid EMDR hittills samstämmande
visat dessa effekter, enligt en litteratursammanställning [3].
Under senaste åren har forskning om minnesfunktion och sakkadiska
ögonrörelser visat att minnessystem som till exempel episodminne (som ofta
är störd vid PTSD) påverkas på ett gynnsamt sätt av ögonrörelser [4-8]. Således
finns det i dag mycket som stödjer att ögonrörelser är meningsfulla
även om det kan vara svårt att förstå vid första anblicken. EMDR och exponeringsbehandling
är lika effektiva enligt metastudier, bland annat Cochrane
och i ISTSS aktuella genomgång av effektiva behandlingsmetoder för
PTSD.

EMDR Association of Sweden makes the following comments to
Draft Guidelines for the treatment of acute stress disorder
and post-traumatic stress disorder.
International Society for Traumatic Stress Studies makes regular summaries
of evidence-mode [1]. It is important to note that psychological
treatments in the form of trauma-focused CBT and EMDR is the most important
and most effective treatments. EMDR has been controversial but
it is no longer, but is an internationally accepted method of treatment for
PTSD. There is still no generally accepted that eye movements are important,
but lately, Swedish and Australian research has shown that eye movements
during EMDR has clear physiological effects that are meaningful [2, 9],
Moreover, all studies of physiology at the convergence of EMDR to date
shown these effects, according to a literature review [3].
In recent years, research on memory function and Sakka wash
eye movements showed that memory systems, such as episodic memory (which is often
is disturbed in PTSD) is affected in a favorable way of eye movements [4-8]. Thus
today there are a lot of support that eye movements are meaningful
although it may be difficult to understand at first glance. EMDR and exposure therapy
are as effective as meta-studies, including Cochrane
and in ISTSS current review of effective treatments for
PTSD.

Clients with complex and severe trauma histories require stabilization of
symptoms, and containment of affect before ever beginning EMDR. A wealth
of stabilization tools helps mitigate the impact of dysregulated affect and
physiology. The tools reduce risk of retraumatization, client loss of hope,
and abandonment of treatment. They also protect practitioners from

With
an
understanding
of
post-‐traumatic
neurobiology

Structural
Dissociation
model,

disorder
patients

EMDR
become
logical
rather
than
surprising.
If
we
understand

meaning,
we
can
better
address
the
responses
that
interfere
with
successful
EMDR
processing.

simple

Travma sonrası stres bozukluğu (TSSB), kişinin aşırı bir travmatik stresörle karşılaşmasından, yaşamasından
veya duymasından sonra, olayların kişiye sıkıntı veren bir biçimde yeniden yaşanması, kaçınma örüntüsü,
duygulanımda küntlük ile birlikte otonomik, disforik ve bilişsel bulguların değişik derecelerde bulunması ile
belirli bir ruhsal bozukluktur.
EMDR’nin TSSB’da etkililiği kanıtlanmış psikodinamik, bilişsel, davranışsal ve danışan merkezli yaklaşımlar gibi çok
iyi bilinen farklı yaklaşımların öğelerini bir araya getiren psikolojik bir tedavi yöntemidir.
Bu yazıda geçirdiği trafik kazası sonrasında Travma sonrası stres belirtileri gösteren vakamızın Göz Hareketleri

stres bozukluğu vakalarında bu yöntemin daha sık ve yaygın bir şekilde kullanılmasının gerekliliği tartışılacaktır.
Post-traumatic stress disorder (PTSD) is a psychiatric disorder that is characterised with autonomic, dysphoric and cognitive signs together with affective numbing, distressed reexperiencing and avoidance from previous traumatic events at a person who has encountered, lived or heard an excessive traumatic event. EMDR is a psychological method which has proven to be effective and it brings together elements of well established approaches such as psychodynamic, cognitive, behavioral and client-centered approaches. In this paper treatment process with Eye Movement Desensitization and Reprocessing (EMDR) of a case who shows signs of post-traumatic stress disorder after a car accident and the need for using this method by clinicians more frequent and widespread at post-traumatic stress disorder patients will be discussed.

This workshop will explain how early chaos and/or neglect affects a child’s brain, autonomic nervous system,
and body system and how this can lead to the development of complex trauma. To protect the self from the
overwhelming affect or complete shut-down that occurs with complex trauma, a child develops dissociation.
Dr. Wieland will explain dissociation from both a neurological and a conceptual perspective. The three-stage
trauma treatment model will be described with particular attention to the role of EMDR within each stage of this
treatment. Because of the dynamics of dissociation – the separation off of emotions, cognitions/memories, and
body sensations – an adapted form of the EMDR protocol is required in the work with these children and adults.

Objective: The purpose of the study is to show the impact of the use of EMDR in survivors of suicide bomb blasts
in North of Pakistan. Design and Settings: The study involves an ongoing compilation of clinical data and the
study of therapeutic responses to various interventions including EMDR, at a tertiary mental health facility and
Centre for Trauma Research and Psychosocial Interventions (CTRPI), Rawalpindi /Islamabad, Pakistan. This mental
health facility is the catchment area of patients from Northern areas of Pakistan, currently the part of the
country, worst affected by series of suicide bombings targeting military and civil population. Method: Families of
the victims and those who survive suicide bombings without physical injuries are referred to CTRPI from
peripheral areas / hospitals for assessment for psychosocial consequences of facing a man made disaster.
Patients are interviewed at the point in time of referral and scoring is done on Impact of Event Scale (IES). Those
who fulfill the criteria of Post traumatic Stress Disorder according to ICD-10 are registered for further studies and
appropriate interventions. The individuals who fulfil the criteria for PTSD or any other psychiatric morbidity are
then enrolled for regular psychiatric follow up. The patients are first offered the use of EMDR and all who give an
informed consent are then assigned to a psychiatrist trained in EMDR (Level 2). Sessions of EMDR as per the
protocol of 8 stages are carried out. Scoring on IES is recorded serially. According to the degree of improvement
and severity of illness, sessions of EMDR are carried out using the bilateral stimulation during the hospital stay.
Results: The three individuals who have completed EMDR treatment had survived the suicidal bombing attacks
and fulfilled the entry criteria were administered 8 stage protocol EMDR. They all improved in their symptoms of
intrusive images, hyper-arousal, autonomic instability and avoidance. Their sleep improved and nightmares
diminished. Their social and interpersonal functioning improved. There was marked reduction of basal anxiety
levels in all three. Scores on IES done after intervention (EMDR) improved from initial pre EMDR score of 41, 38
and 40 respectively to post EMDR scores of 18, 15 and 14 for the three subjects who completed EMDR protocol
of 8 stages. On reporting to their respective units their occupational effectiveness has returned to previous levels
of functioning. Conclusions: EMDR proves to be an effective non pharmacological intervention in terms of post
traumatic stress disorder in special circumstances of acts of terrorism involving suicide bombing. The data
presented is only preliminary and is based on a small number out of a larger sample.
