Abstract:
This protocol (D’Antonio, 1997) integrates Bowen’s (1976) concept of differentiation with a trauma model of relationships. Differentiation refers to a quality of both individuals and relationships: non-defensiveness, responsiveness rather than reactivity, connection without fusion, centeredness without self-absorption, openness with clear boundaries. It might best be though of as a continuum from extremely undifferentiated to well differentiated. Individuals pair off with others as roughly the same level of differentiation.

Da diverso tempo il Dipartimento di Scienze Chirurgiche in Età Pediatrica e la S.O.S. di Psicologia dell’Ospedale Infantile dell’Azienda Ospedaliera Nazionale di Alessandria “Santi Antonio e Biagio e Cesare Arrigo” collaborano nel trattamento dell’enuresi in età evolutiva, mediante associazione di intervento medico/farmacologico e di psicoterapia ad orientamento cognitivo-comportamentale.
La decisione di costruire e progettare uno spazio comune in questo ambito è stata presa dopo alcune considerazioni nate dall’esperienza nel trattamento di questo disturbo: da anni l’ambulatorio di Urologia si occupa della diagnostica e della terapia dell’incontinenza, come da molto tempo si osservano innumerevoli casi di bambini con enuresi, che richiedono un sostegno presso il Servizio di Psicologia.
In questo tipo di intervento è stato inserito anche l’EMDR al fine di favorire la riprocessazione delle esperienze dolorose legate al problema.

Abstract:
When developing a treatment plan, clinicians need to be able to recognize not only the specific effects of trauma but to consider symptoms reflecting limited capacities for emotional self regulation. Such problems are often found when client histories included significant childhood neglect or other disruptions of each childhood attachment (Damasio, 1999; Schore, 2000; Sigel, 1999). Clients with a history of secure attachment appear to be more vulnerable to PTSD (Alexander, et al., 1998; Muller, Sicoli, & Kemieux, 2000) and initially need to be addressed with procedures different from those for trauma specific symptoms. Therefore in the consensus model of posttraumatic treatment (Browm Scheflin, & Hammond, 1998; Chu, 1998; Courtois, 1999) clinicians are urged to focus on clients’ personal safety, stabilization, and the development of client capacities for tolerating and modulating strong affect in the early phases of treatment.
